Abstract:Objective: Cardiovascular disease including coronary artery and peripheral vascular pathologies is the leading cause of mortality in the United States and western countries. There is a pressing need to develop small-diameter vascular grafts for bypass surgery and other vascular reconstructive procedures. Tissue engineering offers the prospect of being able to meet the demand for replacement of diseased vessels. The objective of this study was to make a truly stable, biological vascular graft with excellent healing and remodeling characteristics. In this study, decellularized and heparinized pig carotid artery grafts (D-H grafts) were implanted in dogs and remodeling of the D-H graft was evaluated. Methods: Porcine carotid arteries were decellularized and heparinized by mechanical and chemical means and lyophilized for 48 hours. Prepared vessels (8 cm long) were surgically implanted in end-to-side fashion using 6-0 Proline suture between the femoral artery and femoral vein of dogs. The D-H grafts were harvested at 1 week, 3 weeks and 6 months (2 dogs at each time point) after implantation. All grafts were patent as assessed with duplex ultrasound and by palpating pulses before sacrifice, and underwent histological, and morphometric examination with HE staining. Elastin in the grafts was also determined by Verhoeff's Masson staining. Infiltration and differentiation of smooth muscle cells (SMCs) and endothelial cells were analyzed by immunohistochemistry with antibodies against α-smooth muscle actin (α-SMA) and von willebrand Factor (vWF), respectively. Results: All animals survived and the D-H grafts were patent before harvest. There was no significant change of the elastin content in grafts at 1 and 3 weeks after implantation compared with control grafts (before implantation). However, the amount of elastin in D-H grafts decreased at 6 months, and the graft elastin is approaching to the level of elastin in the host femoral artery of dogs. Both H&E staining and immunohistochemistry analysis revealed the cellular infiltration at the anastomosis and at both luminal and adventitial sides of the graft. By 3 weeks of the implantation, immunohistochemistry with anti-α-SMA antibody showed that some of infiltrating cells were SMCs. In addition to migration of SMCs from host vessels via anastomoses, cell infiltration and differentiation from peripheral blood and adventitial tissues were evident. The graft at 3 weeks showed scattered α-SMA-positive cells in the intima of the middle area of grafts. Moreover, at 6 months after implantation, D-H grafts were repopulated with cells staining positive for α-SMA in the intimal and medial layers of grafts. There were no vWF positive cells in the endothelial layer of D-H grafts at 1 and 3 weeks after implantation. However, by 6 months after implantation, vWF positive cells were detected in the endothelial layer of D-H grafts. Conclusions: Porcine D-H grafts with lyophilization may undergo favorable remodeling towards native vessel characteristics as femoral arteriovenous grafts in dogs. This remodeling process includes migration, infiltration, and differentiation of SMCs and endothelial cells in D-H graft. It is possible that SMCs and endothelial cells of the grafts may come from adjacent host vessels and adventitial tissues and from circulating progenitor cells which are able to differentiate. These findings provide important information for further developing successful tissue-engineered vascular grafts for the human use.
