Abstract:A video-assisted thoracoscopic surgery (VATS) segmentectomy is a technical challenge. This is due to the sophisticated anatomical variations in the segmental bronchus and vessels as well as an obscure surgical demarcation(s). For some, a segmentectomy procedure can be regarded as a lobectomy in the situations of the absence of lung fissures. Thus, the strategy of single-direction VATS lobectomy was applied in our VATS segmentectomy practice. After the introduction of this strategy, we presented the single-direction VATS left S3 segmentectomy as an example to introduce the technical characteristics of the single-direction segmentectomy.

Abstract:Video-assisted thoracoscopic segmentectomy has become a safe and effective surgical approach for stage IA non-small cell lung cancer.1,2 Therein, thoracoscopic segmentectomy for the lateral basal segment (S9) is the most technically challenging anatomical segmentectomy.3–6 Because the target vessels and bronchus are commonly variable and deeply located in the lung parenchyma, it is difficult to expose and correctly identify them through either an interlobar fissure approach or a posterior approach. Meanwhile, tailoring the intersegmental plane is another challenge that is encountered in a VATS S9 segmentectomy. In this multimedia article, we present a thoracoscopic right S9 segmentectomy following the single-direction strategy through an inferior pulmonary ligament approach, using a novel method named stem-branch to track the target segmental branches along the stem (video).7 The positional relations of the basal segmental vessels and bronchi were preliminarily identified mainly through the high-resolution computed tomography (HRCT). The surgery was initiated through an inferior pulmonary ligament approach. The stems of the basal segmental vein and bronchus were first dissected, followed by dissection of their branches. Then, the target branches were tracked and identified according to the positional relations known via HRCT. Lung parenchyma between S10 and S7 should be divided to facilitate dissection and identification of the basal segmental venous and bronchus branches. After the target vein, bronchus and artery was transected in sequence. The method of inflation-deflation was used to identify the intersegmental plane. Then, stapler-based, three-dimensional tailoring was performed. The operative time was 1.5 h with an estimated blood loss of 30 ml. The chest tube was removed on postoperative Day 3. The patient was discharged on postoperative Day 4 without any complication. The final pathological finding was minimally invasive adenocarcinoma (pTmiN0M0). The chest X-ray on postoperative Day 1 and HRCT scan on postoperative Month 4 revealed that the residual right lung expended well. We identified the stem of the basal segmental bronchus, the number of its branches, and the relative locations of them according the preoperative HRCT. During the surgery, we started with dissection of the inferior pulmonary ligament. From the inferior view, the basal bronchus and its branches are located behind the veins. Division of the lung parenchyma between S10 and S7 would facilitate dissection and identification of the basal segmental venous branches during S9 segmentectomy. Because we already know the positional relations of the stem and its branches, the target segmental bronchus can easily be tracked. For the segmental veins, we should follow the principles of reserving uncertain veins, especially the intersegmental veins. The segmental arteries, which are usually accompanied by the segmental bronchus, could be found after transection of the bronchus. Stapling was started from the peripheral and thin parts of the lung and continued, reaching the segmental hilum and thick parts of the lung step-by-step during the intersegmental plane tailoring. For such a complex curved border, tailoring with the stapler alone was not affecting the expansion of the residual lung and causing atelectasis. Thoracoscopic segmentectomy for S9 can be performed successfully through the inferior pulmonary ligament approach by using the method of stem-branch for tracking anatomy based on HRCT and method of complete stapler-based tailoring for the intersegmental plane management.
[Clinical Application of CT-guided Preoperative Pulmonary Nodule Localization Technique].

Abstract:BACKGROUND AND OBJECTIVE:It's difficult to localize the accurate position for some pulmonary nodules in video-assisted thoracoscopic surgery (VATS) wedge resection. The aim of this study is to retrospectively analyze the clinical significance of CT-guided preoperative pulmonary nodule localization technique.METHODS:Between Jan 2010 and Apr 2011, 20 patients of the First Affiliated Hospital of Medical School of Zhejiang University underwent preoperative pulmonary nodule localization technique before performing VATS wedge resection of the pulmonary nodule. Diameter of the lesion ranges from 0.5 cm to 2 cm (average 9.8 cm±5.3 cm). It was evaluated with the success rate in localization technique, rate of localization related complications, and rate of transferring thoracotomy.RESULTS:Eighteen patients underwent successful CT-guided Hookwire localization, with the average time of 14.5 minutes. There was no serious complications.CONCLUSION:CT-guided preoperative pulmonary nodule localization is a promising technique for small solitary pulmonary nodules. It could play an important role in accurate localization of small pulmonary nodules, and it is a safe technique with less postoperative complications.

Abstract:A 63-year-old male was referred to our hospital with two existing lesions in bilateral lungs. Computed tomography (CT) showed a 15-mm ground-glass opacity (GGO) in the superior segment of left lower lung (S6) and a 5-mm GGO in the center of the right upper lobe. The preoperative clinical diagnosis was stage I primary lung cancer for the left lesion while the right lesion needed follow-up. Uniportal video-assisted thoracic surgery (VATS) left superior segmentectomy in the semiprone position was performed in this case and the right upper lobe was kept untouched. Frozen section examination confirmed the diagnosis of lung adenocarcinoma, and systematic lymphadenectomy with non-grasping en bloc dissection technique was then performed. A chest tube was placed at the posterior part of the incision through the dorsal thoracic cavity to the apex. The postoperative pathologic diagnosis was minimally invasive adenocarcinoma, staged T1aN0M0.

Abstract:Ob jective To evaluate the clinical feasibility and application of uniportal video-assisted thoracoscopic surgery ( VATS ) anatomic segmentectomy in the elderly patients with solitary pulmonary nodule.Methods The clinical data of 62 patients with 70 years old underwent uniportal VATS anatomic segmentectomy were retrospectively analyzed , including 7 apical segments , 2 anterior segments , 5 trisegments, 4 lingular segments,7 superior segments and 5 basement segments in the left side, and 9 anterior segments,9 posterior segments,6 apical segments, 3 superior segments and 5 basement segments in the right side.During operation,the fast pathologic examination was carried out to diagnose specimens . Results All of the 62 patients received complete VATS ) anatomic segmentectomy successfully .The operation time was 60-50 min, (75 ±10.5) min, and intraoperative blood loss was 30-200 ml, (62.0 ± 18.2) ml.Patients with lung cancer received 3-7(5.1 ±0.5) stations of lymph node dissection and the number of lymph node dissection was 4-16 (9.1 ±0.8) for each patient.There was no in-hospitai death postoperative complication .Postoperative thoracic drainage time was 1-3 d (1.8 ±0.3) d.The Postoperative hospital stay was 3-5 d (3.3 ±0.8) d.All the patients were discharge successfully .Lung cancer patients were followed up for 6-12 months without recurrence or metastasis .Conclusions Complete uniportal VATS anatomic segmentectomy is a safe and feasible surgical procedure for treatment of elderly patients with solitary pulmonary nodule .
Applications of Video-assisted Thoracic Surgery for the Diagnosis and Treatment of Patients with Small Pulmonary Nodules

Abstract:BACKGROUND AND OBJECTIVE:Chest computed tomography (CT), particularly thin-slice high resolution CT, has low sensitivity and specificity for detecting pulmonary nodules <10 mm in size. This limitation leads to challenges in clinical diagnosis and treatment of small pulmonary nodules. This study introduces the use of video-assisted thoracoscopic surgery (VATS) for the diagnosis and treatment of small pulmonary nodules.METHODS:From November 2009 to May 2012, 64 patients with small pulmonary nodules without prior preoperative pathologic diagnosis were treated by pulmonary wedge resection through VATS. The diagnosis of small pulmonary nodules was established from rapid frozen section. The type of operation depends on the pathology and the condition of the patients. Twenty patients with primary lung cancer were subjected to lobectomy and radical resection of the lymph nodes by complete thoracospic lobectomy or video-assisted thoracoscopic invasive lobectomy. Pulmonary wedge resection was performed in 44 patients, among whom 21 have benign nodule, 18 have precancerous lesion, 3 have metastatic nodule, and 2 have primary lung cancer for which lobectomy was not fit.RESULTS:Confirmative diagnosis is difficult to obtain among patients with small pulmonary nodules. VATS is effective in the diagnosis and treatment of small pulmonary nodules. With VATS, patients with benign small pulmonary nodules can be cured, and patients with primary lung cancer can receive definite diagnosis and effective treatment in time.CONCLUSIONS:CT-guided hook-wire fixation is useful in precise lesion localization for surgical resection.

Current Challenges in Thoracic Surgery

Abstract:A 62 -year -old female presented with a mixed ground glass opacity (mGGO) at left upper lobe by computed tomography (CT) examination. The lesion had a size of 12 mm and a maximum CT value of -450 HU. But more importantly, there were no obvious changes of the lesion observed after regular using of anti-inflammatory drugs. So, the lesion was diagnosed as an early non -small cell lung cancer (NSCLC). Surgical intervention is the treatment of choice for the patient. Preoperative three-dimensional reconstruction was performed to confirm that the lesion was located in the anterior segment (S3) of the left upper lobe. Meanwhile, the fact that solid components in the lesion were between 25% and 50% is which suggested a noninvasive cancer. Therefore, a S3 segment resection of the left upper lobe was performed by uniportal video -assisted thoracoscopic surgery (VATS). Hookwire was used to guide the exact position of the lesion before operation while the distribution of blood vessels and bronchi was identified in the threedimensional condition. Depending on the model of three-dimensional reconstruction, we successfully removed the S3 segment of left upper lobe without dissection of mediastinal lymph nodes. Minimally invasive adenocarcinoma was confirmed by pathology diagnosis. Here, we present a uniportal VATS of S3 segmentectomy.

Abstract:OBJECTIVES:We summarize our experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for non-small-cell lung cancer and discuss the indications and technical details of the operation.METHODS:From September 2011 to December 2012, 15 patients underwent VATS sleeve lobectomy with bronchoplasty at our institution (right upper lobe 10, right middle and lower lobes 1, left lower lobe 2 and left upper lobe 2), with mediastinal lymphadenectomy. Three incisions were utilized. The utility incision was made at the fourth intercostal space, anterior axillary line. Simple continuous and simple interrupted suturing of the membranous and cartilaginous portions of the bronchus were performed for the anastomosis.RESULTS:All procedures were uneventful, with a median operative time of 165 min, a median bronchial anastomosis time of 44 min and a median blood loss of 150 ml. There were no conversions to thoracotomy. There were 14 squamous carcinomas and 1 adenocarcinoma. All patients recovered well, and 1 experienced a minor complication. The median duration of chest tube drainage was 5.4 days, and the median length of hospital stay was 7 days. All patients were followed postoperatively for a range of 1-16 months without tumour recurrence.CONCLUSIONS:VATS sleeve lobectomy with bronchoplasty is safe and effective. The utility incision placed at the fourth intercostal space, anterior axillary line, is convenient for the anastomosis, and the suturing technique is expeditious and secure. Preserving the azygos vein does not compromise exposure for the anastomosis. This technique is very suitable for centrally located lung cancers <3 cm in diameter, particularly when the cancers are located within the brachial lumen.
Single-port Video-Assisted Thoracoscopic Right Upper Lobectomy Using a Flexible Videoscope

Abstract:Single-port video-assisted thoracoscopic surgery (VATS) has recently been proposed as an innovative minimally invasive alternative to the standard three-port VATS for lobectomies, most of which are performed using a conventional rigid thoracoscope. Here, we report a single-port VATS approach for right upper lobectomy and systematic lymph node dissection using a flexible endoscope. A 61-year-old male smoker presented with a pulmonary nodule. A single-port VATS procedure was performed through a 4-cm intercostal incision using a flexible laparo-thoraco videoscope. Right upper lobectomy and systematic lymph node dissection were performed. The total operating time was 106 in. The procedure was successful and the recovery uneventful. The patient's chest tube was removed on the third day, and he was discharged home on the fourth. The use of a flexible videoscope facilitated the single-port VATS procedure by avoiding interference between the videoscope and other operating instruments and providing ample space for the surgeon.
Combining 3D-CTBA and 3D-VATS Single-Operation-Hole to Anatomical Segmentectomy in the Treatment of Non-small Cell Lung Cancer

Abstract:BACKGROUND:China is a high-incidence area of lung cancer, and its morbidity and mortality were the highest in malignant tumors. At present, the popularization of low-dose computed tomography (CT) examination has significantly improved the detection rate of early lung cancer, anatomical segmentectomy is currently widely used early in non-small cell lung cancer (NSCLC) and those who cannot tolerate lobectomy in patients with lung cancer. However, the anatomical structure of segment and its surgical operation is relatively complex, lead to segmentectomy has a high risk and difficulty. We performed anatomical segmentectomy by use of combining three-dimensional computed tomography bronchography and angiography (3D-CTBA) and three-dimensional video-assisted thoracic surgery (3D-VATS) single-operation-hole minimally invasive surgery in the treatment of NSCLC to investigate its clinical effect, and evaluate its clinical relevant feasibility and theoretical basis.METHODS:We carried out a retrospective review of the 57 cases by use of combining 3D-CTA in preoperative and 3D-VATS single-operation-hole anatomical segmentectomy in intraoperative in the treatment of NSCLC performed in Department of Thoracic Surgery, the First Affiliated Hospital of Soochow University from October 2015 to April 2017.RESULTS:The whole group was successfully performed by use of VATS without anyone conversion to thoracotomy. The mean operation time was (142.2±28.3) min, and the mean blood loss was (93.8±46.5) mL. The mean number of lymph node dissection was (9.1±2.2), and the mean postoperative thoracic drainage was (429.8±181.2) mL. The postoperative retention of chest tube time was (2.8±1.1) d. The mean hospitalization time was (5.2±1.3) d. The postoperative pathology showed 9 cases with benign lesions, accounting for 15.7%, 48 cases with malignant lesions, accounting for 84.2%. Postoperative complications: pulmonary infection in 3 cases (5.2%), atelectasis in 1 case (1.9%), small amount of hemoptysis in 1 case (1.9%), lung leakage >3 d in 2 cases (3.5%). Arrhythmia in 4 cases (7.0%). The patients were followed up for 10 months. No complications occurred such as bronchial pleural fistula, chylothorax, encapsulated pleural effusion and those patients were followed up without recurrence and distant metastasis.CONCLUSIONS:The use of combining 3D-CTBA and 3D-VATS single-operation-hole to anatomical segmentectomy is safe and effective in the treatment of NSCLC and is suitable for early non-small cell lung cancer, especially those can not tolerate lobectomy.

Abstract:Segmentectomy was first done in 1939 by Churchill and Belsey in 1939. Video-assisted thoracoscopic surgery (VATS) segmentectomies are still more technically challenging than VATS lobectomies. With the increasing rate of early stage lung carcinomas, the thoracoscopic segmentectomies may have a major role in a near future. Four anatomical VATS segmentectomy videos clips were shared in this study. CT three-dimensional reformation could exactly show the small pulmonary nodule's precise position, clarifies the surrounding structures' relationship like the pulmonary artery (PA), pulmonary vein (PV), bronchus (B), fissure, and lymph nodes. Besides, the resection margin, skin incision, surgical approach can also be designed preoperatively. This feasible technique could increase the confidence for thoracic surgeons. Unnecessary explore time waste for locating small pulmonary nodules during the operation can be avoid, so the surgical accuracy can be improved, complications can be prevented, the duration of chest tube and length of hospital stay can be shortened.
The Initial Experience of Video-assisted Thoracic Surgery Segmentectomy for Early Stage Lung Cancer

Abstract:Background and objective Segmentectomy can retains more healthy lung tissue than lobectomy, but it remains controversial in oncology for early stage lung cancer. The aim of this study is to discuss the problems of video-assisted thoracic surgery (VATS) segmentectomy in early stage lung cancer, by analyzing the clinical and patho-logical data of 35 cases and reviewing the literature. Methods There were 35 patients who received segmentectomy by complete video-assisted thoracic surgery, from May 2013 to July 2017, in single operation group in the Third Hospital of Peking University. We analyzed the patient's clinical and pathological data, intraoperative and postoperative com-plications, lymph node number and metastasis its situation, and compared postoperative pathology and preoperative computed tomography (CT) imaging type. In 35 cases of segmentectomy, there were 11 males and 24 females, with an average age of 57.7 years old. The lesions located in the right upper lobe were 8 cases, in the right lower lobe were 8 cases, in the left upper lobe were 13 cases, in the left lower lobe were 6 cases. The mean maximum diameter of CT imaging was 12.7 mm, and the largest diameter of hilar and mediastinal lymph nodes was less than 10 mm. 23 of them were ground glass predominating and 12 were solid components predominating. Results All 35 cases were successfully completed VATS anatomical segmentectomy. The average operation time was 153 minutes, the amount of bleeding was 51 mL. There were 10 cases of air leakage after operation, all of which were not more than 3 days. There was contralat-eral atelectasis in 1 case, chylothorax in 1 case. The average length of hospitalization was 6.1 days. There was no other complications outpatient related to surgery, in 30 days after discharge. The pathological changes were as follow, 2 cases of metastatic tumor, 8 cases of benign lung disease and 25 cases of primary lung cancer. In the 25 cases of primary lung cancer, there were 14 cases of invasive lung adenocarcinoma (7 cases were groundglassopacity (GGO) predominating in CT imaging), 4 cases of micro-invasive adenocarcinoma (3 cases were GGO predominating in CT imaging), 6 cases of adenocarcinoma in situ (all were pure GGO in CT imaging), 1 case of lung squamous cell carcinoma (mainly com-posed of solid in CT imaging). An average of 7.2 lymph nodes were removed in 25 cases of lung cancer, and all lymph nodes had no metastasis. Conclusion VATS anatomical segmentectomy is technically safe and reliable, and the indica-tions for lung cancer need to be strictly controlled. Its advantages still need to be confirmed by prospective randomized controlled trials.

Abstract:Recently, with the proliferation of high-resolution computed tomography (CT), the frequency of small pulmonary nodules appears higher and more precise than previously estimated, especially in CT screening in patients with high risk factors for lung cancer. Video-assisted thoracoscopic surgery (VATS) provides a new minimally invasive treatment for the diagnosis and treatment of small pulmonary nodules. The VATS results in less pain, shorter hospital stay, and reduced surgical complications, making it more widely available. How to accurately locate and mark lesions is important for video-assisted thoracoscopic surgery. This article reviews the various techniques used to locate pulmonary nodules in surgery in recent years and summarizes the advantages and disadvantages of them. .

Abstract:Video-assisted thoracoscopic surgery (VATS) represents a new trend in the development of minimally invasive thoracic surgery. When applied in lung cancer surgeries, VATS can be used for both pulmonary lobectomy and regional lymph node dissection. Currently the main concerns are focused on the completeness of lymph node dissection for lung cancer and the safety of surgery. The lymph node dissection includes two parts: (I) dissection of interlobar and hilar lymph nodes; and (II) dissection of mediastinal lymph nodes. The demonstrated surgical procedures are featured by: (I) the interlobar and hilar lymph nodes are not removed separately; rathr, they are taken out en bloc with the pulmonary lobes during the surgery; and (II) systematic lymph node dissection, instead of systematic sampling, is applied for the removal of mediastinal lymph nodes. Also, during the fully anatomical resection, each blood vessel and bronchus underwent anatomical dissociation, indicating that this surgery is safe.
Uniportal Video-Assisted Thoracic Surgery Basal Segmentectomy: a Single-Center Retrospective Cohort Study.

Abstract:Background:Uniportal video-assisted thoracic surgery (VATS) basal segmentectomy is technically challenging and requires a deep understanding of the segmental anatomy of the lung. This report describes the uniportal VATS segmentectomy of basal segments using a single-direction approach.Methods:A total of 49 patients who underwent uniportal VATS basal segmentectomy between April 2019 and April 2021 were included in this retrospective study. All the surgeries were conducted using a single-direction approach. The resections of segments 7-8 were mainly performed using the interlobar fissure approach, while the resections of segments 9-10 were performed using the inferior pulmonary ligament approach.Results:A total of 33 patients underwent a single basal segmentectomy and 16 patients underwent combined basal segmentectomy/sub-segmentectomy. The median operative time was 120 min (range, 60-180 min), and the median blood loss was 20 mL (range, 10-100 mL). The median chest tube duration was 2 days (range, 1-5 days), and the median hospital stay after surgery was 4 days (range, 2-15 days). The morbidity rate after surgery was 6.1% (3/49). There were no perioperative deaths. The pathological examinations revealed 3 cases of adenocarcinoma in situ (AIS), 33 cases of minimally invasive adenocarcinoma, and 13 cases of lepidic-predominant invasive adenocarcinoma. No recrudescence or mortality was reported during the median follow-up time of 7 months (range, 2-25 months).Conclusions:Uniportal VATS basal segmentectomy is a feasible and reliable technique based on our experience. This single-direction method allows the uniportal VATS basal segmentectomy to be performed in an easy manner with the targeted segmental bronchi and vessels exposed from superficial to deep in order of their appearance while avoiding the repeated turnover of the lung.
"Non-Triangle Plane" Surgical Technique of Video-Assisted Thoracic Surgery Atypical Segmentectomy for Stage IA Non-Small-Cell Lung Cancer: Early Experience

Abstract:Objectives: To evaluate the safety and feasibility of a novel surgical technique ("non-triangle plane" technique) of two-port (mini-utility) video-assisted thoracic surgery (VATS) atypical segmentectomy (S-3+S(1+2)c) with tunneling stapler for small-sized non-small-cell lung cancers (NSCLCs) located in left S-3 close to the intersegmental plane between S-3 and S(1+2)c. Materials and Methods: This retrospective descriptive study included 16 patients who, between April 2016 and December 2019, underwent a single two-port (mini-utility) VATS atypical segmentectomy (S-3+S(1+2)c) with tunneling stapler technique for small-sized NSCLCs with a ground-glass opacity (GGO) rate of more than 50% by a constant surgical team in two hospitals. Perioperative data and survival data were collected and retrospectively analyzed. Postoperative follow-up was performed every 6 months. Results: Six patients were with adenocarcinoma in situ, and ten were with minimally invasive adenocarcinoma. The mean surgical margin was 14.06 +/- 3.02 mm. The mean operation time was 53.88 +/- 9.76 min. The mean duration of chest tube drainage was 4 +/- 1.21 days, and the median length of postoperative hospital stay was 4 days. There was no perioperative morbidity and mortality. The median follow-up was 47.5 months (17-61 months). No recurrences occurred, and all patients were still alive at the last registered follow-up (May 31, 2021). Conclusion: Two-port (mini-utility) VATS atypical segmentectomy (S-3+S(1+2)c) with tunneling stapler technique is a safe and feasible option for the treatment of small-sized NSCLCs located in left S-3 close to the intersegmental plane between S-3 and S(1+2)c.

Abstract:Uniportal video-assisted thoracoscopic surgery (VATS) has now evolved into a sophisticated technique that can be used in some of the most complex thoracic procedures; however, this approach to segmentectomy is not standardized, and the surgical procedure varies between surgeons. Here, we describe the use of our uniportal VATS procedure during right upper posterior segmentectomy in a patient with a nodule in the right upper lobe. Subsequent mediastinal lymphadenectomy was performed. The patient has recovered well after surgery. We believe that uniportal VATS segmentectomy is a technically safe and feasible alternative approach to conventional thoracoscopic techniques for treating lung cancer.
One Hundred and Fifty-Six Cases of Anatomical Pulmonary Segmentectomy by Uniportal Video-Assisted Thoracic Surgery: a 2-Year Learning Experience

Abstract:OBJECTIVES: Our goal was to explore the outcome of the study and the feasibility for patients of the technique of anatomical pulmonary segmentectomy by uniportal video-assisted thoracic surgery (VATS). METHODS: A total of 156 consecutive patients with lung lesions who received anatomical pulmonary segmentectomy by VATS between 2015 and 2016 in our hospital were enrolled. All the subjects had high-resolution, thin-section chest computed tomography (CT) examinations with 3-dimensional reconstruction, a pulmonary function test, abdominal ultrasonography, electrocardiogram and cardiac ultrasonography. The lung lesion was localized before the operation using CT reconstruction or a hookwire to plan the operative method. RESULTS: Uniportal VATS pulmonary segmentectomy was successfully completed in 151 (96.8%) patients. Most cases involved the right apical and apical posterior segments and the left trisegment. Only 1 patient had a right middle interior segmentectomy, left upper apical anterior segmentectomy or a right lower posteriolateral segmentectomy, respectively. There were 26 cases of benign lesions (including 17 cases of atypical hyperplasia) and 130 cases of non-small-cell lung cancer. Operation time (146 +/- 56 vs 113 +/- 32 min), blood loss (63 +/- 17 vs 54 +/- 13 ml) and complication rates (13.5% vs 5.8%) were obviously lower in 2016 compared with 2015 (P < 0.01). In contrast, the preoperative hookwire localization rate was markedly higher in 2016 compared with 2015. CONCLUSIONS: Uniportal VATS anatomical pulmonary segmentectomy is safe and feasible in clinical applications. Compared with the 2- or 3-port method, there were some technical difficulties in the early phase of the learning curve for uniportal VATS that were overcome through a period of practice.
[Complete video-assisted thoracoscopic anatomic segmentectomy for pulmonary diseases: the early experiences]

Abstract:Objective: To evaluate the safety and feasibility of video-assisted thoracic surgery (VATS) anatomic segmentectomy for pulmonary diseases. Methods: Between November 2009 and July 2011, 20 patients received consecutive VATS anatomic segmentectomy by single surgical group. There were 3 male and 17 female, aging from 32 to 81 years with a mean of 53 years. The patients included 12 cases of non-small cell lung cancer (NSCLC) (5 cases of bronchioloalveolar carcinoma, 6 cases of adenocarcinoma, and 1 case of squamous cell carcinoma), 7 cases of benign diseases (3 cases of bronchiectasis, 2 cases of inflammatory pseudotumor, 1 case of tuberculosis, and 1 case of sclerosing hemangioma) and 1 case of metastasis tumor. The locations of resected segments included 1 anterior segment, 3 posterior segments, 4 apical segments, and 2 superior segments in the right side; and 5 lingular segments, 3 trisegments, and 2 superior segments in the left side. Simultaneously, 3 patients with bronchiectasis underwent segmentectomy and lobectomy, 1 of 6 patients with adenocarcinoma underwent lingulectomy and thymectomy. The pathological TNM stages of 12 NSCLC patients were 9 cases of T1aN0M0, 1 case of T1bN0M0 and 2 cases of T2aN0M0. Results: Of these 20 patients, the median operative time was 155 minutes (range, 120 to 235 minutes), the median blood loss was 50 ml (range, 10 to 600 ml), the median drainage duration was 3 d (range, 1 to 6 d), and the median hospital stay was 6 d (range, 3 to 9 d). One patient who had undergone lingulectomy had a 600 ml intraoperative bleeding from lingular artery, and the bleeding was controlled by suturing the rupture under VATS. Bloody sputum occurred in 2 patients, prolonged air leak occurred in one patient for 5 days, and one patient developed subcutaneous emphysema that spontaneously resolved. No mortality was observed for 30 days after the surgery. Conclusions: VATS anatomic segmentectomy is a feasible and safe technique with acceptable operative time, less blood loss, fewer complications, and shorter hospital stay.
