Abstract:Objectives: This study aimed to examine the relationships between the types of stress and students' mental health, to distinguish the effects of stressors on mental health problems, and to explore the important role of uncertainty stress on the development of mental disorders in a nationally representative sample of Chinese college students. Methods: A cross-sectional multistage study was conducted. Participants were 11,954 students, who were recruited from 50 Chinese universities located in 43 cities covering 23 provinces, autonomous regions, and municipalities across China. The Student Daily Stress Questionnaire (SDSQ) was applied to measure the different types of stress, and mental health status was measured using the 12-item Chinese Health Questionnaire (CHQ). Both unadjusted and adjusted logistic regression models were utilized in the statistical analyses. Multilevel analyses were performed to examine the variation of mental disorder at both the individual and university levels. Results: The prevalence of mental disorders was 22.8% (95% CI: 22.0-23.5%). The unadjusted models showed that age, gender, grade, major, and university location and type were the correlates of mental disorders among students. The unadjusted models developed in this research found that study stress, life stress, and uncertainty stress were positively associated with mental disorder. The multilevel logistic regression models showed that uncertainty stress was far more likely to result in students' mental disorders than study or life stress after controlling for university level. The greater the perceived uncertainty stress, the higher the prevalence of mental disorders. Conclusion: This study provides robust evidence of the impact of uncertainty stress on mental disorders among college students. Compared with life and study stress, more attention should be given to uncertainty stress. The information from this study should be helpful when considering effective mental health policies and interventions among college students in China.
Geographical Disparities of Uncertainty Stress and Life Stress among University Students in China

Abstract:Abstract Background Ecological models have emphasized that mental stress both is an individual phenomenon and an environmental outcome. However, many mental stress studies focused primarily on individual variables. Few studies have examined the geographical disparities in mental stress, with no comparable studies implemented on this issue with regard to different type of mental stress. This study investigates geographical variations in overall stress, uncertainty stress and life stress among university students in China. Methods Respondents comprised 11,954 students from 50 universities and 31 provinces in Mainland China. Respondents completed the extended version of the Global Health Professional Student Survey (GHPSS) on Tobacco Control in China. The survey included additional health, mental stress, and behavioral items along with regional variables which were retrieved from the National Bureau of Statistics database. Both unadjusted and adjusted methods were used in the logistic regression analysis. Results Uncertainty stress and life stress prevalence were 19.6% (95% CI: 15.9%, 23.3%) and 8.6% (95% CI: 7.2%, 10.7%) respectively, while overall stress prevalence was 37.2%, 95% CI: 24.7%, 49.7%). All stresses varied widely across the 31 provinces. Random parameters of 0.3971 for life stress, 0.2593 for uncertainty stress, and 0.1449 for overall stress were statistically significantly at the 0.01 level, in terms of their geographical distribution, high life stress was concentrated in central provinces, high uncertainty stress mainly in both central and western provinces, while provinces displaying higher overall stress tended to display a north-south distribution. Multilevel logistic regression showed province level per capital disposable income of households partly contributed to uncertainty stress (OR: 0.52) and life stress (OR: 0.59). Conclusions This study maps the geographical distribution of uncertainty stress, life stress, and overall stress in China. These findings underscore the importance of environmental factors affecting stress levels among university students.

Abstract:BACKGROUND:Many studies have examined the association between socioeconomic status (SES) and mental stress. Uncertainty stress is a prominent aspect of mental stress. Yet no research has ever empirically analyzed the impact of SES on uncertainty stress.METHODS:Students were identified through a multistage survey sampling process including 50 universities. Each student participant completed the Global Health Professions Student Survey (GHPSS) on Tobacco Control in China. Regional variables were retrieved from the National Bureau of Statistics database. Both unadjusted and adjusted methods were considered in the analyses.RESULTS:Among the 11,942 participants, severe uncertainty stress prevalence was 19.6%, while severe life stress prevalence was 8.6%. Multilevel logistic regression showed that most SES variables were associated with uncertainty stress. Students with "operation and commercial work" as mother's occupation and "rural or township" as family location exhibited a higher prevalence of severe uncertainty stress. Lower family income and original region gross domestic products (GDP) were also associated with higher severe uncertainty stress prevalence. However, only father's occupation was correlated with life stress.CONCLUSIONS:Based on the literature review, this is the first empirical study examining the impact of SES on uncertainty stress in China and elsewhere in the world. Our research underscores the importance of decreasing socioeconomic inequalities in controlling excessive uncertainty stress.

Abstract:Respective associations between uncertainty stress and three dimensions of social capital with suicidal ideation were assessed among 4446 undergraduates from 22 Chinese universities in a cross-sectional study. Multiple logistic regression model and structural equation modeling were used to examine these relationships. Uncertainty stress was more strongly and uniquely associated with suicidal ideation than with life stress. Social capital, especially social networks, shared a dual role as a correlate of suicidal ideation and means to reduce the impact of uncertainty stress. School-based gender- and age-specific suicidal ideation interventions should emphasize social capital enhancement and uncertainty stress coping.
Life Stress and Uncertainty Stress: Which is More Associated with Unintentional Injury?

Abstract:Injury is a serious public health problem with significant social and economic costs. Prior studies reported an association between mental stress and unintentional injury. However, no comparable studies have examined uncertainty stress, along with other types of stress, and their relative impact upon unintentional injury. The purpose of this study was to assess the salience of uncertainty stress as a predictor. Participants were 11,954 students, who were identified through a multistage sampling process that incorporated 50 universities. Stress and unintentional injury status were obtained by self-report. Both unadjusted and adjusted methods were considered in the analyses. Based on a retrospective 12-month reporting window, 12.6% of the respondents had experienced traffic injury, 21.4% home injury, 15.2% work-related injury, and 23.1% sports injury. The multivariable logistic regression model found that uncertainty stress was associated with all categories of unintentional injury, while life stress only was related to traffic injury. These findings underscore the importance of minimizing uncertainty stress and can inform pertinent policies and reinforce the need for uncertainty stress management in China.
The Effect of Urban Green Space on Uncertainty Stress and Life Stress: A Nationwide Study of University Students in China

Abstract:OBJECTIVE:The aim of this study is to investigate the impacts of urban green space on stress among Chinese university students.METHODS:Participants were 11,954 students from 50 universities in 42 cities. Each student completed the Global Health Professional Student Survey (GHPSS) on Tobacco Control in China. Regional variables were retrieved from the National Bureau of Statistics database. Both unadjusted and adjusted methods were used in the logistic regression analysis.RESULTS:Almost one third of the students (31.1%) suffered from high levels of uncertainty and life stress. The multilevel logistic regression model showed that while certain individual and environmental factors, such as higher family income, having parents with a professional occupation and urban home location, reduced uncertainty stress among students, so did the effects of green space. This was particularly important for reducing uncertainty stress but also had some effects on life stress.CONCLUSIONS:The findings support a wider body of literature on the positive effects of green space on stress and mental health. Future studies need to pay greater attention not only to intra-urban differences in student use of green space, but also to how the natural environment features in stress management policies in Chinese universities.
Deliberate Self-Harm among Chinese Medical Students: A Population-Based Study.

Abstract:Background: The phenomenon of deliberate self-harm (DSH) among college students has received increased attention in recent decades. Adopting a psychosocial perspective, this study aims to describe self reported DSH among Chinese medical college students, assess respective associations between uncertainty stress and social capital with DSH, and explore the mechanism linking these three phenomena.Methods: A cross-sectional survey employing multi-stage, sampling was conducted. 4446 undergraduate students were recruited from 22 participating Chinese medical universities. Perceived stress from uncertainty and social capital were assessed among the students. The Chi-square test and multiple logistic regression models assessed correlates of DSH. Relationship among social capital, uncertainty stress, and DSH were examined by means of Structural Equation Modeling.Results: The prevalence of DSH in the past 12 months among Chinese students was 9.6%. The most common types of physical DSH reported were scratching, cutting, and pinching. Age (chi(2)=26.63, p < 0.01), gender (chi(2)=30.24, p < 0.01), major field (chi(2)=28.13, p < 0.01), and annual household income (chi(2)=11.10, p < 0.05) were statistically associated with DSH. Uncertainty stress is a unique correlate of DSH, and shows a stronger association than do three certainty stressors. Social capital is also a strong correlate of DSH, especially cognitive social capital. Moreover, social capital may be indirectly associated with DSH through impacting uncertainty stress. Limitations: This study was a cross-sectional and thus could not evaluate causal relationships.Conclusion: We recommend that a DSH intervention study should target uncertainty stress management and social capital accumulation. This study provides scientific evidence and theoretical foundation for future DSH interventions, with a view to enhancing the mental health of medical college students. (C) 2016 Elsevier B.V. Regional Internet Access and Mental Stress among University Students: A Representative Nationwide Study of China (Preprint)

Abstract:The Internet changed the lives of average citizens in the early part of the 21st century, and it has now become an essential part of daily life. Many studies reported that Internet use is positively correlated with psychological and mental problems, included depression, loneliness, and stress. However, previous studies examining Internet use and mental health were confined to local and community subpopulations and limited at the individual level, which increases the potential bias from selection effect at a different level. Regional variables would be a stable estimate of people’s socioeconomic and cultural environments and how these variables affect mental health needed to be studied. The objective of this study was to evaluate the association between regional Internet access, and mental stress among university students. Participants were 11,954 students, who were identified through a multistage survey sampling process conducted in 50 Chinese universities. Regional Internet access was retrieved from a national database, and mental stress was measured using the Perceived Stress Scale (Chinese Version) (CPSS). Both unadjusted and adjusted methods were considered in the analyses. More than one third 36.9% (95% CI: 24.4%–49.5%) of university students in this study suffered from severe mental stress (SMR). The full multilevel logistic regression model found that university students studied in low-level universities had 2.73 (95% C.I. 1.17 to 6.37) times the prevalence of SMR than that reference group. Compared with small cities, students in a large city had a lower prevalence of SMR (OR 0.24; 95%C.I. 0.07 to 0.78). Most importantly, regional Internet access was negatively associated with students’ SMR (OR 0.25; 95%C.I. 0.08 to 0.76). This study provided directive evidences that regional Internet access and other environmental factors including city size contribute to students’ mental health. The findings underscore that efforts to control excessive mental stress among students in China should pay greater attention to environmental determinants of stress and particularly to improve in internet access.
Association Between Bedtime and Self-Reported Illness among College Students: a Representative Nationwide Study of China

Abstract:Objective: This study was to examine the association between bedtime and self-reported illness among Chinese college students. Methods: Participants were 11,942 students, who were identified through a multistage survey sampling process. Sleep and illness status were obtained by self report. Both un-adjusted and adjusted methods were considered in the analyses. Results: The logistic regression model found that late bedtime was positively associated with self reported short and long-term illness (OR: 3.70 and 1, 79) after adjusting for demographic characteristics, short sleep duration, and mental disorders. Conclusions: This study is the first to find a positive relationship between late bedtime and self reported illnesses in China or elsewhere. Stress Status and Related Characteristics among Urban Residents: A Six-Province Capital Cities Study in China

Abstract:OBJECTIVES:To estimate current prevalence levels of stress, and to identify related characteristics among urban residents in China.DESIGN:A cross-sectional, multilevel study. Selected through multi-stage quota-sampling, survey participants were 4,735 urban residents aged 15 years and older who resided in one of six selected Chinese capital cities. Data were collected on stress levels and sociodemographic characteristics. Stress was assessed using the Perceived Stress Scale, Chinese version (CPSS). A multilevel variance component model was employed to analyze associations between sociodemographic variables and stress.RESULTS:The mean stress score for urban residents was 22.34 (SD: 3.22), and 36.8% of those surveyed (95% CI: 33.5-40.2%) were severely stressed (>25). Multilevel regression analysis indicated that residents aged 55 years and older were less stressed than residents under age 25. The most educated and higher income earners had lower stress levels than the least educated and poorest. High levels of stress were apparent among all other occupational groups, relative to managers and clerks, except retirees and operational workers. Residents in the north of China exhibited higher stress levels than counterparts in the south.CONCLUSIONS:This study suggests that higher stress levels are positively associated with social class in China. Our findings could inform health policy, guide prevention strategies, and justify the design and implementation of targeted interventions.

Abstract:The Transtheoretical Model of Behavior Change (TTM) has the potential to explain how Chinese collegestudents can initiate and maintain effective stress management (any form of healthy activity, which is practicedto manage stress for at least 20 minutes per day). The TTM regards the process as progression through thefollowing five stages of change: precontemplation (not ready), contemplation (getting ready), preparation (ready),action, and maintenance. Self-efficacy (confidence to manage stress effectively even under tempting situations)is assumed to increase with stage progression. Previous studies have found such relationships, but no study hasexamined these relationships with Chinese college students. The purpose of this study was to examine therelationship between stages of change and self-efficacy for effective stress management. The participantsincluded 366 male and 505 female Chinese college students. The Chinese language version of Pro-Change’sself-efficacy measure was developed based on item response theory. A single scale of 10 items was replicated.Self-efficacy was significantly higher in action and maintenance than in precontemplation and contemplation.Self-efficacy was also significantly higher in men than in women. These results provide initial evidence that theself-efficacy measure can be applied to stress management with Chinese college students.

Abstract:The Chinese College Stress Scale was developed to ascertain stress in university students. Results suggested that the psychometric properties of the Chinese College Stress Scale were satisfactory. Overall, student stress was primarily related to academic, personal, and negative life events. Approximately 8% of Chinese students exhibited high stress associated with attending the university whereas 74% exhibited low stress, and approximately 18% did not report any stress. The general pattern of stressful college events is similar to that found in American studies; however, the rank order of stressors is somewhat different between these findings and the results of American studies. The top academic stressors for Chinese university students were low learning efficiency and competition among students whereas the top academic stressors for American students were related to examinations. Personal hassles for the Chinese students involved concerns that they were not being taught or educated properly and inadequate social skills. In contrast, the major personal stressors of American students were found to be associated with intimate relationships and parental conflicts. (C) 2005 Wiley Periodicals, Inc.
Individual and Regional Factors Affecting Stress and Problem Alcohol Use: A Representative Nationwide Study of China

Abstract:OBJECTIVES:The objective of this study was to examine the association between individual and environmental stressors and problem alcohol use among Chinese university students.METHODS:Participants were 11,942 students, who were identified through a multistage survey sampling process that included 50 universities. Individual information, including feelings of stress and perceptions of problem alcohol use, was obtained by self report. Urban and regional variables were retrieved from the National Bureau of Statistics database. Both unadjusted and adjusted methods were considered in the analyses.RESULTS:Almost one third (32.6%) of the students suffered from some form of severe stress while problem alcohol use prevalence was 7.3%, (95% CI: 4.1-10.4%). The multilevel logistic regression model found that uncertainty stress, gender, father's occupation and monthly expenses were associated with problem alcohol use. Of the contextual factors home region and the university city GDP and unemployment rate were important. When interactions were considered, the relationship between monthly expenses and financial uncertainty and problem drinking was most evident in high level universities. By contrast, the effects of uncertainty stress on problem drinking were most evident in middle and low level universities.CONCLUSIONS:The findings underscore that efforts to control problem alcohol use among students in China should pay greater attention to environmental determinants of stress and particularly to improvements in stress management in university settings.

Abstract:To explore the stress related problems and risk factors among urban residents in the social transition period.Samples including 3 666 subjects came from Hangzhou, Guangzhou, Chongqing and Taiyuan under multi-stage sampling procedure. Data were collected through a household interviewing survey. Stress was measured by the Chinese Perceived Stress Scales (CPSS). Distribution of health risk stress (HRS), high level stress were described along the demographic dimension. Multivariate logistic analysis was used to identify the predictors of HRS.CPSS was found to have good validity and reliability among this study sample. The mean score for the total sample was 24.22 +/- 5.81 and the overall prevalence of HRS was 44.54% (95% CI: 42.90 - 46.12). The difference between male (42.95%) and female (46.30%) was statistically significant (P < 0.05). The rate of HRS decreased with age, with the highest rate (59.29%) among those less than 21 years old. The rate of HRS also decreased as educational level increased and differed significantly by occupation. High school students, people who had never been employed and those who had lost their jobs had the highest rates of HRS. The rate of HRS was also related to marriage status, household's economic level, and the level of the social engagement of the subjects.Stress experienced by the urban residents became a serious health problem which requires special attention to be paid to certain subgroups in the population.
Associations Between Uncertainty Stress, Life Stress and Internet Addiction among Medical Students.

Abstract:OBJECTIVES:Internet Addiction (IA) is a growing issue predominate in adolescents and young adults. Although the effects of diverse stressors on IA have been highlighted, there is little consensus about the specific underpinnings of IA. The current study aims to investigate associations between uncertainty stress, life stress and IA among Chinese university medical students.METHODS:A cross-sectional survey employing multi-stage sampling was used. Data were collected from 6,061 students from 27 university medical programs across China. Associations between uncertainty stress, life stress, and IA were examined by means of multivariate logistic regression.RESULTS:The findings indicated that the overall IA prevalence was 12.6% (95% CI 11.7-13.5), life stress prevalence was 8.1% (95% CI 7.4-8.8), and uncertainty stress prevalence was 19.1% (95% CI 18.1-20.1). Multivariate logistic regression showed that uncertainty stress [adjusted OR 2.60 (95% CI 2.14-3.15), P < 0.001] and life stress [adjusted OR 1.71 (95% CI 1.32-2.23), P < 0.001] were positively associated with IA. Population Attributable Risk (PAR) of uncertainty stress associated with IA was 29%, and that of life stress was 15%.CONCLUSIONS:The contribution of uncertainty stress to IA is significantly higher than that of life stress. High uncertainty stress, being male and born in a region of higher economic status were associated with excessive Internet use and IA.
Uncertainty Stress, and Its Impact on Disease Fear and Prevention Behavior During the COVID-19 Epidemic in China: A Panel Study.

Abstract:Objectives: We examined changing trends of uncertainty stress, and its impact on disease fear and prevention behaviors during the Chinese COVID-19 epidemic, using a prospective observational study. Methods: We employed a longitudinal design. We recruited participants for an online panel survey from chat groups on social media platforms. There were 5 waves of interviews. Information on uncertainty stress and related variables were collected via the online survey. Descriptive statistics and the GIM program were used for data analysis. Results: Participants numbered 150 for the linkable baseline survey and 102 (68%) for the final survey. Uncertainty stress (β = -.047, SE = .118, p > .05) did not show a statistically significant temporal change trend over the observation period. Disease fear manifested a statistically significant downwards trend (β = -.342, SE = .157, p < .05), and prevention behaviors indicated an upwards trend (β = .048, SE = .021, p < .05) during the observation period. Uncertainty stress was positively associated with disease fear (β = .45046, SE = .05964, p < .001), and negatively associated with self-efficacy (β = -.6698, SE = .01035, p < .001), and prevention behaviors (β = -.02029, SE = .00876, p =.021). Conclusion: This study yielded new information about uncertainty stress among Chinese people during the COVID-19 epidemic. Policy changes and public education are essential for minimizing the negative effects of uncertainty stress in disease prevention.
Associations of Suicidality with Adverse Life Events, Psychological Distress and Somatic Complaints in a Chinese Medical Student Sample

Abstract:The present study aimed to examine the associations of suicidality with psychological distress, somatic symptoms, and stressors in medical school students. We conducted a cross-sectional survey of medical students at a large, top-ranked medical school. A total sample of 662 medical students (269 men) completed a battery of self-reported questionnaire. Of the respondents, 136 (20.5%) reported having suicidal ideation, 40 (6.0%) reported having planned suicide, and 10 (1.5%) reported having attempted suicide during the previous year. Students from rural areas and the 3rd year of study reported higher levels of suicidal ideation than their counterparts. General pain/fatigue was associated with suicidal ideation and planning, while interpersonal difficulties were associated only with suicidal ideation. This study provides evidence linking severe psychological distress, general pain/fatigue, interpersonal stress and psychosocial factors with suicide risk among Chinese medical students which provides targets for future psychological interventions.
Changes of College Students' Psychological Stress During the COVID-19 Pandemic in China: A Two-Wave Repeated Survey.

Abstract:COVID-19 has posed unprecedented challenges to the mental health of college students worldwide. We examined the trends in students’ stress levels during and after China’s first wave of COVID-19 outbreaks by analyzing their demographics, behavior, mental health status, career confidence, and Chinese Perceived Stress Scale (CPSS) scores. Our findings indicate an increase in students’ stress levels since the COVID-19 onset, with more students experiencing higher stress levels after the first outbreak than during it (OR = 2.03, 95% CI 1.79, 2.30). Several factors were identified as being associated with higher CPSS scores, including higher class levels, residence in rural or town areas, low family income, and lack of familiarity with COVID-19, among others. Our study highlighted the urgency of developing and implementing effective strategies to cope with students’ stress during and after a global pandemic.
Regional Internet Access and Mental Stress Among University Students: A Representative Nationwide Study of China

Abstract:Background: The Internet changed the lives of average citizens in the early part of the twenty-first century, and it has now become an essential part of daily life. Many studies reported that accessibility of Internet use is associated with mental health. However, previous studies examining this association were confined to local and community subpopulations and limited at the individual level, which increases the potential bias from the selection effect at a different level. Regional variables would be a stable estimate of people's socioeconomic and cultural environments and how these variables affect mental health needed to be studied. The objective of this study was to evaluate the association between regional Internet access, and mental stress among university students. Methods: Participants were 11,954 students, who were identified through a multistage survey sampling process conducted in 50 Chinese universities. Regional Internet access was retrieved from a national database, and mental stress was measured using the Perceived Stress Scale (Chinese Version) (CPSS). Both unadjusted and adjusted methods were considered in the analyses. Results: More than one-third 36.9% (95% CI: 24.4-49.5%) of university students in this study suffered from severe mental stress (SMR). The multilevel logistic regression model found that university students studied in low-level universities had 2.52 (95% C.I. 1.17 to 6.37) times the prevalence of SMR than those in high-level universities. Compared with small cities, students in a large city had a lower prevalence of SMR (OR 0.25; 95%C.I. 0.06 to 0.77). Most importantly, regional Internet access was negatively associated with students' SMR (OR 0.25; 95%C.I. 0.08 to 0.76). Conclusions: This study indicated that regional Internet access and other environmental factors including city size and type of universities contribute to students' mental health. The findings underscore that efforts to control excessive mental stress among students in China should pay greater attention to environmental determinants of stress and particularly to improve internet access.

Abstract:The COVID-19 pandemic has unhinged the lives of people across the globe. In particular, more than 30 million Chinese college students are home-schooling, yet there is little understanding of how academic workload, separation from school, and fears of contagion lead to a decrease in their health. This study examined the relationships between Chinese college students’ three critical stressors and two types of health in the COVID‐19 pandemic context. We used a three-wave lagged design with a one-week interval. All the constructs were assessed by self-report in anonymous surveys during the COVID‐19 pandemic. College students were asked to report their demographic information, academic workload, separation from school, fears of contagion, perceived stress, and health. The results of this study showed that academic workload, separation from school, and fears of contagion had negative effects on college students’ health via perceived stress. In the COVID-19 crisis, multiple prevention and control measures focusing on college students may lead them to have different degrees of stress and health problems. Our results enrich the literature on stress and health and offer novel practical implications for all circles of the society to ensure students’ health under the context of the COVID-19 epidemic.
