A clinical study of urodynamic data with indefinite P-F findings in BPO

Abstract:Objective To elucidate the possibility of diagnosing BOO with urodynamic data with indefinite P F findings in patients with BPH. Methods Some urodynamic data besides the P F fin dings were studied in BPH patients,and these were compared between the BOO group and the non BOO group. Results There were 252 patients with BOO and 52 patients without.200 patients had no definite P F findings.Several other parameters were compared between the BOO group and the non BOO group.The rate of prostatic plateau was 87.3% and 53.8%, respectively( P <0.05); the rate of detrusor instability at the end of cystometry (detrusor spasm )was 22.2% and 7.7%, respectively ( P < 0.05 );the opening pressure was (90.4±38.5) cm H 2O and (46.9±20.7) cm H 2O,respectively( P <0.05) and the maximum detrusor pressure was (124.9±42.9) cm H 2O and (66.4±21.7)cm H 2O,respectively( P <0.05).No difference between the 2 groups was observed in other parameters studied.In 200 patients with no definite P F findings,the average maximum detrusor pressure of 163 patients was (120±42.8)cm H 2O;the average opening pressure of 100 patients was (98.46±42)cm H 2O;97 patients had detrusor spasm (49%); 28 patients have prostatic plateau(most patients have no prostatic plateau because of being catheterized). Conclusions If there is no definite P F findings,some other urodynamic data could be used as adjuvant parameters in diagnosing BOO, including the maximum detrsor pressure if more than 80 cm H 2O,the opening pressure more than 60 cm H 2O and the presence of detrusor spasm and prostatic plateau.
