Abstract:Purpose: Pediatric testicular yolk sac tumor is a rare malignant germ cell tumor and there is a lack of large clinical studies. The purpose of this study is to summarize the clinical characteristics of pediatric testicular yolk tumor and evaluate the prognostic factors. Materials and methods: The medical records of children with testicular yolk sac tumor in one pediatric medical centre in China from January 2005 to January 2021 were retrospectively investigated. Data regarding clinical characteristics, treatment and prognosis were collected. Results: A total of 109 patients with a median diagnosed age of 18 months (range 2–69) were included in this study; of them 100 were diagnosed as stage I, 6 as stage II and 3 as stage IV. All patients underwent radical orchiectomy, and 61 of them underwent postoperative chemotherapy. The mean follow-up time was 61.3 months (range 3–259), during that time, 8 patients experienced relapse. The five-year overall survival was 90.6% (95% CI 84.6%–96.7%). Univariate Cox regression analysis showed that disease stage, relapse, maximum tumor diameter, and alpha-fetoprotein returning to normal within 2 months postoperatively were risk factors for survival (HRs of 25.43, 26.43, 1.48 and 0.08, respectively, p < 0.05). Multivariate Cox regression analysis suggested that higher disease stage and relapse were independent adverse factors for survival (HRs of 148.30 and 94.58, respectively, p < 0.05). Conclusions: The prognosis of pediatric testicular yolk sac tumor is generally excellent. A higher disease stage and the occurrence of relapse could predict a poor prognosis. The individualized management of children with testicular yolk sac tumor according to risk classification is feasible.

Stage Ⅰ pediatric testicular germ cell tumors: An analysis of 41 cases

Abstract:Objective To analyze the clinical characteristics and outcomes of children with stage Ⅰ testicular germ cell tumors (TGCT),and to discuss the treatment strategy for this disease after radical inguinal orchiectomy.Methods Clinical data of 41 patients with stage Ⅰ TGCT at Shanghai Children's Medical Center,Shanghai Jiaotong University School of Medicine between June 2003 to December 2013 were retrospectively analyzed.Their clinical characteristics,therapy and outcomes were analyzed.Results (1) The median age at diagnosis was 18 (3 to 43) months old.Among them,38 children(92.7％) were younger than 3 years old.The pathological subtype included 35 cases of yolk sac (85.4％),4 immature teratoma (9.7％) and 2 mixed germ cell tumors (4.9％).(2) Serum AFP levels were elevated in 38 patients (92.7％).AFP reduced to normal in 27 cases(71.1％)4 weeks after surgery and in 35 cases(92.1％) after 2 courses of chemotherapy respectively,but 2 cases of them relapsed with AFP increasing again.(3)Forty out of 41 patients underwent surgery and 3 ～ 4 courses of chemotherapy with cyclophosvnamide,vincristine and dactinomycin D (VAC).One patient received surgery alone.The median follow-up period was 64 months.One of the 32 patients who received adjuvant chemotherapy relapsed and then lost follow-up without treatment.The recurrence rate was 3.1％ and the 5-year overall survival was 100％.No complication related to chemotherapy occurred during the follow-up.The only one patient treated with surgery alone relapsed at 5 months and achieved complete response after salvage treatment.Conclusion (1) The relationship between the decrease of serum AFP and the prognosis is uncertain and needs to be further studied.(2) Short course chemotherapy with mild toxic side effects can reduce the recurrence rate effectively,so it can be used as a necessary treatment for patients with high risk of relapse,and it can also be used as a selective treatment for patients with low risk of recurrence.
Surgical treatment for stage Ⅰ testicular malignant germ cell tumors via scrotal incision

Abstract:Objective:To explore the safety and feasibility of surgery for stage Ⅰ testicular malignant germ cell tumors via scrotal incision.Methods:From January 2014 to December 2019, clinical data were retrospectively reviewed for 41 children with testicular malignant germ cell tumor without inguinal or retroperitoneal lymph node enlargement.The median age was 36(4-96) months.According to operative approach, they were divided into scrotal and inguinal groups.Perioperative related parameters and postoperative complications were compared between two groups.Results:Both groups completed operations smoothly.No significant inter-group differences existed in age, body weight, tumor diameter or pathological type ( P>0.05). Operative duration and postoperative hospitalization time of scrotal group were (39.3±9.2) min and (3.6±1.5) d respectively and that of groin group was (42.0±8.6) min and (4.3±2.2) d and the inter-group difference was not statistically significant ( P>0.05). In scrotal group, preoperative value of alpha fetoprotein (AFP) was (7 203.9±16 096.3) ng/mL, (21.2±14.0) ng/mL at Month 1 post-operation and (12.9±9.4) ng/mL at Month 2 post-operation.In inguinal group, AFP value was (5 048.8±8 106.1) ng/mL pre-operation, (32.2±41.3) ng/mL at Month 1 post-operation and (22.4±37.9) ng/mL at Month 2 post-operation.After operation, AFP in both groups was significantly lower than that pre-operation with statistical significance ( P<0.05). No significant inter-group difference existed in AFP value ( P>0.05). In terms of complications, one case of incision dehiscence occurred in scrotal group while no perioperative complication was observed in inguinal group.The postoperative follow-up period of two groups was (6-74)(8-69) months.The overall survival rate was 100% and the event-free survival rate 13/14 and 24/27 without statistical significance ( P>0.05). Conclusion:Surgery via scrotal incision for stage Ⅰ testicular malignant germ cell tumors can achieve the same efficacy and prognosis as traditional inguinal incision.The operation is simple and the complications are controllable.Wider popularization is worthwhile.
Retrospective review of testical yolk sac tumors during 25 years' period

Abstract:Objective To discuss a proper treatment protocol for testicle yolk sac tumors. Methods The outcomes of 43 children with testicle Yolk sac tumors of the past 25 years were reviewed. All cases were underwent radical high level spermatectomy and orchiectomy, and 27 of them were performed on retroperitoneal lymphadenectomy simultaneously or in stage. The AFP was increased in 16 cases. The follow up results showed that 29 were still alive. There were no difference among patients with retroperitoneal lymphadenectomy or without. Conclusions Patients with testicle yolk sac tumors in stage I needn't retroperitoneal lymphadenectomy. It is indicated in cases with a sustained elevation of AFP, and confirmation of retroperitoneal lymph node metastasis by ultrasound or CT. For chemotherapy, AD protocol is used in stage 1 of yolk sac tumors, and PVB protocol is adopted in stage II or stage III.
Prepubertal Testicular and Paratesticular Tumors in China: a Single-Center Experience over a 10-Year Period

Abstract:OBJECTIVE:Prepubertal testicular tumors are rare and fundamentally distinct from adult testicular tumors. We reviewed our 11-year experience in a single medical center of China.MATERIAL AND METHODS:This study reports the clinical characteristics, histopathologic diagnosis, treatment methods, and outcome in a series of 63 prepubertal boys who were treated between 1997 and 2008.RESULTS:A total of 63 primary prepubertal testicular and paratesticular tumors were identified. The median age at presentation was 11 months. Of these tumors, 27 (42.9%) were mature teratomas, 5 (7.9%) were immature teratomas, 21 (33.3%) were yolk sac tumors, 4 (6.3%) were epidermoid cyst, 2 (3.2%) were Leydig cell tumors, 1 (1.6%) was a mixed malignant germ cell tumor, and 3 (4.8%) were paratesticular tumors. The most common clinical presentation (95.2%) was a painless scrotal mass or swelling. Forty-eight tumors were treated with radical inguinal orchiectomy, and 15, with a testis-sparing procedure. Follow-up was available in 59 cases, range from 4 to 128 months (median, 50 months). One patient with yolk sac tumor had recurrence and progression to metastasis at the end of 4 months after surgery. Other patients were disease free at last follow-up.CONCLUSIONS:Most of the prepubertal testicular lesions were benign, and the most common histologic subtype was teratoma. Our experience with testis-sparing procedures supports the current trends that less invasive treatment should be performed for benign lesions. This study confirms the excellent cure rates obtained in children with prepubertal testicular tumors.
Clinical Outcomes in Patients with Stage I Non-Seminomatous Germ Cell Cancer

Abstract:This study assesses the long-term outcomes in Han Chinese patients with clinical stage I non-seminomatous germ cell testicular cancer (CSI NSGCT) treated with surveillance, retroperitoneal lymph node dissection (RPLND) and adjuvant chemotherapy.We retrospectively evaluated 89 patients with a mean age of 26.5 years.After orchiectomy, 37 patients were treated with surveillance, 34 underwent RPLND and 18 were managed with chemotherapy.The overall survival rate, the recurrence-free survival rate and the risk factors were evaluated.The median follow-up length was 92 months (range: 6-149 months).Thirteen of the 89 patients (14.6%) had relapses, and one died by the evaluation date.The overall survival rate was 98.9%.The cumulative 4-year recurrence-free rates were 80.2%, 92.0% and 100% for the surveillance, RPLND and chemotherapy groups, respectively.The disease-free period tended to be briefer in patients with a history of cryptorchidism and those with stage Is.Therefore, surveillance, RPLND and adjuvant chemotherapy might be reliable strategies in compliant patients with CSI NSGCT.Surveillance should be recommended for patients with the lowest recurrence rate, especially those without lymphovascular invasion.This study might aid the establishment of a standard therapy for CSI NSGCT in China.
Ultrasonic misdiagnosis of giant pediatric testicular yolk sac tumor: A case report and literature review

Abstract:Background: Yolk sac tumor is the most common malignant nonseminomatous germ-cell tumor in children characterized by elevated level of α -fetoprotein (AFP), accounting for 70%–80% of all cases. However, giant yolk sac tumors that involve the entire testicle may be misdiagnosed by color Doppler ultrasonography as orchitis. Therefore, we described a case of a 2-year-old pediatric patient with a giant testicular yolk sac tumor that was misdiagnosed by ultrasonography as orchitis, in order to evaluate the role of measuring AFP levels in the initial diagnosis to aid in the accuracy of the definitive diagnosis of testicular yolk sac tumor. Case presentation: A 2-year-old boy received outpatient visits for unintentional swelling of the right scrotum for 7 days. Physical examination showed a rubbery swelling of the right scrotum with rejective touch. Then, the patient underwent perineal color Doppler ultrasonography in outpatient visits. The result showed a right testicle size of 29 mm × 22 mm × 20 mm with heterogeneous echogenicity and abundant blood flow, supporting the initial diagnosis of orchitis. However, the initial surgeon was skeptical of the ultrasonography diagnosis. Thus, the patient was admitted to the Department of Andrology on day 2 for further serological and imaging examination. The serum AFP level on day 3 was 323.77 ng/ml. The results of CT and MRI showed a giant tumor of the right testis (26 mm × 21 mm × 29.6 mm) with multiple lymphoid hyperplasia in the inguinal region bilaterally. The patient received radical orchidectomy without lymph node dissection on day 9. The results of postoperative pathological examination confirmed giant testicular yolk sac tumor (T1N0M0S1, Stage Is) and was positive for AFP and SALL4 in immunohistochemistry staining. The patient received three courses of bleomycin–etoposide–cisplatin chemotherapy in the Department of Pediatrics after multidisciplinary team meeting on postoperative days 14, 37, and 58, respectively. During chemotherapy and follow-up, the patient's AFP and lactate dehydrogenase levels continued to decline, and eventually remained within normal range on postoperative day 84. Conclusion: Measuring the AFP level was necessary for initial diagnosis and follow-up in pediatric cases of testicular enlargement. Radical orchidectomy combined with postoperative bleomycin–etoposide–cisplatin adjuvant chemotherapy was an effective treatment strategy for pediatric giant testicular yolk sac tumors.

Lymphopenia During Routine Follow-Up May Predict Relapse in Patients with Extranodal NK/T Cell Lymphoma

Abstract:Recently, absolute lymphocyte count (ALC) at diagnosis, as a surrogate marker of host immunity, has been reported to be a prognostic factor for clinical outcomes in extranodal NK/T cell lymphoma (ENKTL). In this retrospective study, we set out to investigate whether ALC at the time of confirmed relapse or at last follow-up is a marker for relapse after chemoradiotherapy in 84 patients with stage I/II ENKTL. Receiver operating characteristics (ROC) curve and area under the curve (AUC) analysis showed that ALC at follow-up was a significant marker for relapse (AUC = 0.883, P < 0.001). Using 1.215 × 10 9 /L as the optimal cutoff value of ALC, 44 patients (52.4 %) were in lower ALC group and 40 patients (47.6 %) were in higher ALC group. The sensitivity and specificity for ALC at the time of confirmed relapse or at last follow-up was 94.1 and 76.0 %, respectively. The relative risk of relapse with an ALC < 1.215 × 10 9 /L was 14.5. The positive predictive value with an ALC < 1.215 × 10 9 /L was 72.7 %, and the negative predictive value with an ALC ≥ 1.215 × 10 9 /L was 95.0 %. The 4-year cumulative incidence rate for an ALC < 1.215 × 10 9 /L was 73.2 % compared with 3.2 % for an ALC ≥ 1.215 × 10 9 /L ( P < 0.001). In a multivariate regression analysis, ALC at the time of confirmed relapse or last follow-up remained to be a significant factor for relapse ( P < 0.001). In conclusion, lymphopenia observed during routine follow-up can predict relapse in patients with ENKTL, which needs further validation in prospective trials.
Association of DAZL expression with risk of recurrence in clinical stage 1 testicular germ cell tumors.

Abstract:523 Background: One model for testicular germ cell tumor (TGCT) formation posits that these tumors arise from a failure of germ cell determination characterized by persistent pluripotency and apoptotic escape. Risk assessment for relapse in clinical stage 1 (CS1) TGCT is dictated by clinical factors; molecular biomarkers to improve patient stratification for post-orchiectomy care are lacking. In this study, we examined the relationship between protein expression of pluripotency and apoptosis factors and risk of relapse in clinical stage 1 TGCT. Methods: Orchiectomy specimens from 73 patients treated for clinical stage 1 TGCT at one institution (27 seminoma, 46 non-seminoma) were subjected to immunohistochemistry (IHC) to assess protein expression of DAZL, POU5F1, BAK1, DDX4, NANOG, and PMAIP1. Samples were dichotomized as negative or positive for target protein expression. Patients with CS1 TGCT managed with surveillance were annotated for baseline clinical characteristics and whether they experienced relapse within two years of orchiectomy. Exact logistic regression estimated odds ratios (OR) and 95% confidence intervals were calculated for the association between protein expression and relapse. Results: At two years, 17% of patients in our cohort experienced relapse. DAZL expression (score > 0) was detected in 43% of seminoma samples and 13% of embryonal carcinomas. After adjusting for histology, tumors negative for DAZL expression were more likely to recur within two years (adjusted OR for recurrence 0.08, 95% CI 0 – 0.81, p = 0.03). The direction of this relationship was consistent in seminoma when adjusted for tumor size (adjusted OR 0.14, 95% CI 0-1.83, p = 0.18) and non-seminoma after adjustment for embryonal predominance and lymphovascular invasion (adjusted OR 0.53, 95% CI 0-3.10, p = 0.30). No notable findings were observed for POU5F1, BAK1, DDX4, NANOG, and PMAIP1. Validation studies in two independent cohorts from other institutions are ongoing. Conclusions: The absence of DAZL expression by IHC is associated with a greater risk of relapse in CS1 TGCT. As DAZL promotes germ cell commitment by suppressing the pluripotency program, absence of DAZL expression may promote an oncogenic germ cell phenotype and more aggressive tumor behavior. Validation in larger patient cohorts is required to credential DAZL as a biomarker for clinical stage 1 relapse on surveillance.

Relapsed Childhood Acute Lymphoblastic Leukemia: Current Situation in China; a Multicenter Observational Study

Abstract:Background: Relapse is the major reason for death in pediatric acute lymphoblastic leukemia (ALL) patients. It may be worse because of refusing re-treatment in developing countries. So, we investigated the influencing factors for re-treatment decision made by parents and survival data of patients with relapsed pediatric ALL in China. Methods: Clinical characteristics, choice of re-treatment, survival status at the last follow-up time were collected, survival data and prognostic factors were analyzed. Results: Among 390 relapsed cases which were collected from five centers in China, 230 (59.0%) were re-treated. Fifty-eight were alive, 249 had died, and 83 were lost to follow up (median follow up 73.0 months). Receiving retreatment was not associated with gender, initial immunophenotype, or era of relapse, but was associated with age, initial risk group, and recurrence time and site. The 5-year OS rate of patients who received re-treatment or not was 35.5% and 4.5%, respectively. In re-treatment group, the survival analysis showed that initial risk group, site of relapse, time from diagnosis to relapse, and whether the patient achieved CR2 were closely related to 5-year OS, while WBC count (50 × 109/L), and immunophenotype at diagnosis were not. Fortyeight cases received transplantation and 20 (41.7%) survived, while only 36 (19.8%) of 182 patients who did survived. Conclusions: The estimated 5-year OS rate was only 24.4% ± 2.5% for all 390 relapsed cases in China. Of them, 59% patients, 230 cases, were re-treated with 5-year OS 35.5%. Abstract:Introduction: This retrospective study was conducted to assess the survival rates and prognostic factors in children with relapsed acute lymphoblastic leukemia (ALL) who were treated according to the Chinese Children's Cancer Group ALL-2015 protocol at the Children's Hospital of Chongqing Medical University. Methods: The study cohort involving 852 evaluable children with ALL reported a total of 146 relapses during a median follow-up period of 53 months. The primary outcomes measured were the second complete remission (CR2) rates, and 5-year event-free survival (EFS) and overall survival (OS) for patients who received re-treatment post-relapse. Patient data were stratified by ALL subtype (B-ALL vs. T-ALL), age at relapse, site of relapse, and timing of relapse. Univariate and multivariate analyses were performed to identify factors significantly associated with EFS and OS. Results: As of March 31, 2023, 146 relapses were observed, including 128 B-ALL and 18 T-ALL cases. The 8-year CIR was (19.8 ± 1.6)%, with no significant difference between B-ALL and T-ALL (P=0.271). Among the 105 patients who underwent re-treatment, 70 achieved CR2, resulting in a CR2 rate of 67.6%. The 5-year EFS and OS rates for re-treated patients were (45.0 ± 5.4)% and (56.9 ± 5.2)%, respectively. Significant differences in 5-year OS and EFS were found between B-ALL and T-ALL relapses (P < 0.001). The 5-year EFS and OS varied significantly with relapse timing and site of relapse. Factors significantly affecting EFS after relapse included the site of relapse, immunophenotyping, CR2 achievement, and hematopoietic stem cell transplantation (HSCT). Immunophenotyping, CR2 achievement, and HSCT were also identified as significant factors affecting OS after relapse. Discussion: Despite treatment with the CCCG-ALL-2015 protocol, a significant relapse rate was observed, with 72% of children opting for re-treatment post-relapse. The study highlights the importance of considering specific prognostic factors to inform tailored treatment strategies for relapsed childhood ALL. The findings emphasize the need for further research into improving therapeutic approaches for this patient population. Abstract:Background: Primary vaginal yolk sac tumor is a rare malignancy in the pediatric population, and a diagnostic challenge and appropriate initial treatment remains unsolved. The aim of this study was to investigate the clinicopathologic features, treatment and prognosis of this tumor. Materials and Methods: Eight cases of primary vaginal yolk sac tumor were reported with a literature review. Results: There were 4 pure yolk sac tumor cases and four mixed germ cell tumors containing yolk sac tumor element, including two cases with embryonal carcinoma and two cases with embryonal carcinoma and dysgerminoma. Partial vaginectomy was performed in four cases and all patients received chemotherapy. 85 cases in literatures were reviewed and 9 cases were misdiagnosed. Follow-up data was available in 77 cases and 5-year overall survival rate was 87.6%. 5-year survival rate of biopsy with chemotherapy, conservative surgery with chemotherapy and radical surgery with chemotherapy was 91.1%, 100% and 28.6%, respectively (p<0.001). Compared to cases without relapse or metastasis after initial treatment, patients with relapse or metastasis had a shorter overall survival (35.6% vs 96.6%, p<0.001). Conclusions: Mixed germ cell tumor containing yolk sac tumor element was not uncommon and partial vaginectomy may be a good choice for primary vaginal mixed yolk sac tumor type to eradicate local tumor cells and provide complete information for pathological diagnosis and postoperative adjuvant therapy.

Abstract:Objective: To synthesize the evidence on Sertoli-Leydig cell tumour (SLCT) relapses, and identify the clinicopathological characteristics and prognosis of patients with recurrent SLCT. Methods: A literature search was undertaken of all published cases of SLCT relapse found in PubMed, Embase and Web of Science databases between January 1998 and January 2021. All articles in English reporting at least one case of SLCT relapse and mentioning the relapse location or the follow-up data were included. All reported data on relapsed cases were extracted. Student's t-test and Chi-squared test were used for the descriptive analysis, and the Kaplan-Meier statistical method was applied for survival analysis. Results: Eighty-five patients from 33 articles were included in this review. The median age was 20 years (range 3-76 years) with a median time to relapse of 14 months (range 1-168 months). Forty-eight percent (36/75) of relapses were local and 52% (39/75) were distant. In the subgroup of conservative primary surgery, contralateral ovarian SLCT events (metachronous or recurrent) were more frequent in the paediatric population than in the adult population (58.3 vs 18.2%; p = 0.005). Eleven cases had multiple relapses. Twenty-one percent (12/57) of cases were treated with conservative surgery after recurrence, and 64.9% (37/57) of cases were treated with radical surgery which tends to have a better 2-year survival rate (78.5% vs 61.0%; p = 0.177). Overall median survival was 48 months after recurrence (95% confidence interval ±21.0 months) with overall 5-year survival of 38.9%. The mean survival time was significantly higher for patients diagnosed at an early stage (I and II) compared with patients diagnosed at an advanced stage (p = 0.003). Discussion: The results showed that SLCT relapses have a poor prognosis and occur mainly in young patients, soon after the initial diagnosis. The majority of SLCT relapses are located in the abdominopelvic region. Contralateral ovarian SLCT events (metachronous or recurrent) occurred more frequently in paediatric cases. Multi-modal treatment with surgery and chemotherapy appears to be the best approach. The best chemotherapeutic regimen has yet to be defined.
Clinical characteristics and long-term management of prepubertal testicular teratomas: a retrospective, multicenter study

Abstract:Prepubertal testicular teratomas are rare tumors with limited practical guidance for their management. This study aimed to analyze a large multicenter database to establish the optimal management of testicular teratomas. We retrospectively collected data on testicular teratomas in children younger than 12 years who underwent surgery without postoperative chemotherapy in three large professional children's institutions in China between 2007 and 2021. The biological behavior and long-term outcomes of testicular teratomas were analyzed. In total, 487 children (with 393 mature teratomas and 94 immature teratomas) were included. Among mature teratomas, 375 cases were testis-sparing, 18 were orchiectomies, 346 were operated through the scrotal approach, and 47 underwent the inguinal approach. The median follow-up period was 70 months, and no recurrence or testicular atrophy was observed. Among the children with immature teratomas, 54 underwent testis-sparing surgery, 40 underwent orchiectomy, 43 were operated through the scrotal approach, and 51 were operated through the inguinal approach. Two cases of immature teratomas with cryptorchidism had local recurrence or metastasis within 1 year of the operation. The median follow-up duration was 76 months. No other patients had recurrence, metastasis, or testicular atrophy. Conclusion: Testicular-sparing surgery is the first treatment choice for prepubertal testicular teratomas, with the scrotal approach being a safe and well-tolerated strategy for these diseases. Additionally, patients with immature teratomas and cryptorchidism may have tumor recurrence or metastasis after surgery. Therefore, these patients should be closely followed up in the first year after surgery. What is Known: • There is a fundamental difference between testicular tumours in childhood and those in adulthood - not only in terms of the difference and incidence but also in terms of histology. • For surgical techniques, the inguinal approach is recommended for the treatment of testicular teratomas in children. What is New: • The scrotal approach being a safe and well-tolerated strategy for testicular teratomas in children. • Patients with immature teratomas and cryptorchidism may have tumor recurrence or metastasis after surgery. These patients should be closely followed up in the first year after surgery.
Risk Factors and Outcomes of Early Relapse After Curative Resection of Intrahepatic Cholangiocarcinoma

Abstract:Early relapse after hepatectomy for intrahepatic cholangiocarcinoma (ICC) has a tremendous influence on the long-term survival outcomes of ICC patients. The purpose of our study was to investigate risk factors for early tumor relapse and confirm whether early relapse was correlated with ICC patients' long-term survival outcomes. Three hundred and twenty-two consecutive ICC patients undergoing partial hepatectomy at Liver Surgery Department of Zhongshan Hospital (Fudan University, Shanghai, China) between January 2005 and December 2011 were included in this retrospectively study. The definition of early relapse had been described as tumor relapse within 24 months after hepatectomy in ICC patients. We identified a total of 168 ICC patients with early relapse and 23 ICC patients with late relapse after hepatectomy. From the time of relapse, the long-term survival outcomes were worse among patients who had early vs. late relapse (median OS 16.5 vs. 44.7 months, respectively; P < 0.0001). The overall survival of the early relapse group was lower than that of the late relapse group (P < 0.0001). Multivariate Cox regression analysis indicated that multiple tumors (hazard ratio [HR], 1.951; 95% CI, 1.382–2.755; P < 0.001), lymphonodus metastasis (HR, 1.517; 95% CI, 1.061–2.168; P = 0.022), and higher serum CA19-9 levels (HR, 1.495; 95% CI, 1.095–2.039; P = 0.011) were independent risk factors of early relapse. Moreover, multiple tumors (HR, 1.641; 95% CI, 1.120–2.406; P = 0.011), lymphonodus metastasis (HR, 2.008; 95% CI, 1.367–2.949; P < 0.001), elevated NLR (HR, 1.921; 95% CI, 1.331–2.774; P < 0.001) and higher serum CA19-9 levels (HR, 1.990; 95% CI, 1.409–2.812; P < 0.001) were independent predictors of overall survival for ICC patients with early relapse. Collectively, our findings demonstrated that multiple tumors, lymphonodus metastasis, and higher serum CA19-9 levels were associated with the increased risks of early relapse and worse prognoses of ICC after curative-intent resection.

Abstract:Objective: To investigate the clinical and prognostic features of malignant germ cell tumor (MGCT) .Methods: Between January 1998 and January 2016, 138 patients with extracranial (all stages) MGCT were enrolled in Shanghai Children's Medical Center (SCMC) . The clinical characteristics, therapeutic efficacy and prognosis were retrospectively analyzed. Results: The 5-year overall survival (OS) of pediatric patients was determined as 96. 1％, in detail 100. 00％ at stage Ⅰ, 94. 43％ at stage Ⅱ, 96. 44％ at stage Ⅲ and 88. 73％ at stage Ⅳ. Multivariate analysis suggested that staging affected the survival significantly (P < 0. 01) . 13 cases through clinical observation on stage Ⅰ/Ⅱpediatric patients post surgery, 5 cases (38. 5％) were suffered from recurrence within 2 years and alleviated by chemotherapy treatment. Conclusion: The cisplatin-based combination chemotherapy can improve the outcome of childhood extracranial MGCT.

Abstract:The objective of this study was to analyze the clinical behavior and treatment policy of patients with primary testicular diffuse large B-cell lymphoma by retrospective analysis of 32 patients at our institute. All patients underwent orchidectomy. Anthracycline-based chemotherapy was administered to 27 patients (84.38%), six of whom also received rituximab; prophylactic intrathecal chemotherapy was given to seven patients (21.88%); and eight patients (25%) received prophylactic scrotal radiotherapy. Thirteen patients had relapse, among whom 12 cases were extranodal recurrences. Seven patients had central nervous system involvement, and four patients relapsed in the contralateral testis. The presence of B symptoms, poor Eastern Cooperative Oncology Group performance status, left testicular involvement, central nervous system involvement, and first relapse within 1 year were associated with worse progression-free survival using univariate analysis. Poor Eastern Cooperative Oncology Group performance status, left testicular involvement, and surgery alone were negative prognostic factors for overall survival.

Abstract:506 Background: In testicular germ cell tumor (GCT) staging, current guidelines lack consensus in the measurement of retroperitoneal lymph node metastasis, including the recommended plane and dimension. Therefore, our objective was to investigate current guideline recommendations for staging of retroperitoneal lymph nodes and their influence on clinical stage (cS), relapse rate and therapy. Methods: In this explorative study we retrospectively analyzed 154 cSI GCT patients who underwent orchiectomy without any adjuvant therapy and had a follow-up of ≥ 24 months. Here, cSI was defined as retroperitoneal lymph nodes <10mm in axial short-axis diameter (SAD). In initial staging imaging the largest retroperitoneal lymph node was measured considering the different dimensions (SAD and long-axis diameter (LAD)) in the three different radiological planes: axial, sagittal and coronal. Results: In the studied cohort overall survival (OS) was 100% and relapse free survival was 82% with a mean follow-up of 95 months. By using axial SAD (RECIST 1.1) all patients were classified as cSI. Based on axial LAD (German S3 guideline) or LAD in any plane (EAU, ESMO, onkopedia and AJCC), significantly more patients would be classified as cSIIA (0% vs. 38% vs. 52%) or even cSIIB (0% vs. 1% vs. 25%; p<0.001). Overtreatment would occur in 0%, 31%, and 61% ( p<0.001) for axial SAD, axial LAD and LAD in any plane, while undertreatment would affect 18%, 10%, and 2% ( p<0.001), respectively. A stage-appropriate therapy would hypothetically require the following number of chemotherapy cycles (BEP or Carboplatin): optionally 34 for axial SAD vs. up to 361 for LAD in any plane. Conclusions: We found a huge variety of clinical stages according to different lymph node staging recommendations in current guidelines. With a 100% OS rate in the entire cohort and considering the balance between the risks of overtreatment and undertreatment, with stage-adapted treatment at relapse, we strongly recommend adopting axial SAD in accordance with RECIST 1.1. Prospective validation is warranted to standardize guideline recommendations.

[Testicular involvement in pediatric lymphoid tumors: a review of the literature and a series of clinical observation]

Abstract:Lymphoid tumors with testicular involvement in childhood are rare and heterogeneous. The disease may manifest with uni- or bilateral scrotal enlargement. Comprehensive examination includes evaluation of all lymph nodes involvement, as well as ultrasound examination, magnetic resonance imaging and positron emission tomography. A diagnosis is made on basis of morphological and immunohistochemical verification. Determination of lymphoid tumor variant and stage, is recommended to perform chemotherapy according to prognostic risk group, and, in some cases, transplantation of hematopoietic stem cells is required as consolidation therapy. We present three rare clinical cases of follicular lymphoma with testicular involvement, T-lymphoblasti progenitor cell lymphoma, and B-lineage acute lymphoblastic leukemia (ALL) relapse. Different schemes of chemotherapy, combined with orchiectomy (in 2 of 3 cases) resulted in prolonged complete remission. In the first case, due to treatment-refractory B-lineage ALL, the disease was incurable. Our data on clinical, morphological, immunohistochemical and therapeutic features of lymphoid tumors with testicular involvement make it necessary to form multidisciplinary teams, including pediatric urologists, hematologic oncologists and surgeons for timely diagnosis and successful treatment.
