Abstract:Backgrounds Previous studies had revealed that sterol regulatory element-binding protein ( SREBP ) cleavage-activating protein ( SCAP ) rs12487736 polymorphism was associated with blood pressure (BP), but whether rs12487736 could interact with lifestyle behaviors on BP is unknown. Methods A case–control study with 1092 Chinese children was conducted. Results We found an interaction between rs12487736 and high calorie foods intake (fried chips/cakes/cookies) on systolic blood pressure (SBP) ( P interaction = 0.027), and rs12487736 was associated with SBP in the subgroup having high calorie foods at least once in the last week ( b = 2.19, P = 0.025), but not in the subgroup not having high calorie foods. Also, interaction between protein intake (meat/fish/soy beans/egg) and rs12487736 on diastolic BP (DBP) was identified ( P interaction = 0.049); rs12487736 was associated with DBP in the subgroup consuming protein (meat/fish/soy beans/egg) <twice/day ( b = 3.38, P = 0.014), but not in the subgroup ≥twice/day. There is combined effect between rs12487736 and physical activity on DBP. In the subgroup who were inactive (physical activity <1 h/day), rs12487736 was significantly associated with DBP ( b = 3.27, P = 0.046), but not in the active group (physical activity ≥1 h/day). Similar combined effect between rs12487736 and soft drink was found. Conclusions Interactions or combined effects between SCAP and lifestyle behaviors on BP support the importance of promoting a healthy lifestyle in the children genetically predisposed to higher BP.

The obesity-related polymorphism PCSK1 rs6235 is associated with essential hypertension in the Han Chinese population

Abstract:Proprotein convertase subtilisin/kexin-type 1 (PCSK1) is a prohormone convertase that has an important role in prohormone maturation including the process of prorenin to renin. We studied the association of the PCSK1 single-nucleotide polymorphism (SNP) rs6235 (encoding an S690T substitution) with essential hypertension (EH), obesity and related traits in the Han Chinese population. The rs6235 SNP in the PCSK1 gene was investigated using a case–control study design, with 1034 hypertension cases and 1112 normotensive controls. In this study, the rs6235 SNP was significantly associated with hypertension (OR=1.26, 95% CI (1.10–1.46), P =0.001); the odds ratios of GC vs GG and CC vs GG were 1.30 (95% CI (1.06–1.58), P =0.010) and 1.55 (95% CI (1.12–2.13), P =0.007), respectively. In the controls, the C-allele was associated with increased systolic ( P =0.010) and diastolic ( P =0.010) blood pressure levels. In all of the EH patients and EH patients without a history of renin-angiotensin-aldosterone (RAA) system-related antagonists, the C-allele was associated with increased plasma renin activity ( P =0.00004 and 0.002, respectively) and aldosterone levels ( P =0.018 and 0.005, respectively). The C-allele was also associated with increased body mass index (BMI) ( P =0.010) in the normotensive controls. In conclusion, the PCSK1 SNP rs6235 was associated with EH and blood pressure in the Han Chinese population, and this association may be mediated by the SNP’s effect on RAA levels. rs6235 was also associated with BMI in this population.
[Association of Rs2228314 Polymorphism in SREBP2 with Serum Lipid Levels and Obesity among Children and Adolescents].

Abstract:OBJECTIVE:To study the relationship between rs2228314 polymorphism in sterol regulatory element binding protein 2 gene (SREBP2) and obesity, serum lipid levels in children and adolescents.METHODS:In our study, 2 030 children and adolescents aged from 7 to 18 years participated. Anthropometric measurements, including height and weight, were performed. Their serum total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) were detected. The matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) was used to detect rs2228314 genotypes.RESULTS:The GC/CC genotypes of rs2228314 polymorphisms had lower HDL-C levels than GG genotype [(0.10 ± 0.35) mmol/L vs. (0.14 ± 0.36) mmol/L, P=0.020]. The rs2228314 polymorphism was associated with the abnormal HDL-C level under the dominant model after adjustment for study samples, sex and age (OR=1.400, 95% CI: 1.027-1.907, P=0.033). The rs2228314 polymorphism was not associated with obesity under the dominant model after adjustment for study samples, sex, age and HDL-C level (OR=1.178,95% CI: 0.971-1.430, P=0.096).CONCLUSION:The GC/CC genotype carriers of SREBP2 rs2228314 polymorphism have higher risk of abnormal HDL-C level than the individuals with GG genotype among children and adolescents.
Family-Based Associations Between the Angiotensin-Converting Enzyme Insertion/Deletion Polymorphism and Multiple Cardiovascular Risk Factors in Chinese

Abstract:Objective We investigated, in a sample of Chinese families, the associations of body mass index (BMI), blood pressure (BP), serum lipids, fasting plasma glucose, serum creatinine and uric acid with the insertion/deletion (I/D) polymorphism of the angiotensin-converting enzyme (ACE) gene.Methods We genotyped 902 subjects from 186 nuclear families recruited in Shanghai, China, via a specialized hypertension clinic. We performed family-based association analyses for continuous and dichotomous phenotypic measurements using the quantitative and sib-transmission/disequilibrium tests (QTDT and Sib-TDT), respectively.Results The study sample included 121 parents and 781 offspring from 25 two-parent families, 71 one-parent families, and 90 families without parental information. The median number of offspring was four (range from 2 to 10). Of the 654 (85.4%) hypertensive offspring, 458 took anti hypertensive drugs. The 354 male offspring were slightly younger than the 427 female siblings (48.1 versus 49.2 years, P = 0.03), but they had similar BMI (25.1 kg/m(2)). In 482 informative offspring, QTDT analyses demonstrated a significant association between BIVII and the transmission of the ACE D allele (regression coefficient 0.563, chi(2) = 4.02, P = 0.04). In 106 families with at least one hypertensive offspring and at least one normotensive sibling, Sib-TDT analyses showed that the ACE D allele was slightly over-transmitted from heterozygous parents to hypertensive offspring (P = 0.08).Conclusions Our family-based study suggests that in Chinese, the ACE I/D polymorphism might play a role in the development of obesity and hypertension, which are closely linked cardiovascular risk factors. Abstract:Background: Genetic variants in Scavenger receptor Class B Type 1 ( SCARB1 ) influencing high-density lipoprotein cholesterol (HDL-C) and coronary heart disease (CHD) risk were identified by recent genome-wide association studies. Further study of potential functional variants in SCARB1 may provide new ideas of the complicated relationship between HDL-C and CHD. Methods: 2000 bp in SCARB1 promoter region was re-sequenced in 168 participants with extremely high plasma HDL-C and 400 control subjects. Putative risk alleles were identified using bioinformatics analysis and reporter-gene assays. Two indel variants, rs144334493 and rs557348251, respectively, were genotyped in 5,002 CHD patients and 5,175 control subjects. The underlying mechanisms were investigated. Results: Through resequencing, 27 genetic variants were identified. Results of genotyping in 5,002 CHD patients and 5,175 control subjects revealed that rs144334493 and rs557348251 were significantly associated with increased risk of CHD [odds ratio (OR): 1.28, 95% confidence interval (CI): 1.09 to 1.52, p = 0.003; OR: 2.65, 95% CI: 1.66–4.24, p = 4.4 × 10 −5 ). Subsequent mechanism experiments demonstrated that rs144334493 deletion allele attenuated forkhead box A1 (FOXA1) binding to the promoter region of SCARB1 , while FOXA1 overexpression reversely increased SR-BI expression. Conclusion: Genetic variants in SCARB1 promoter region significantly associated with the plasma lipid levels by affecting SR-BI expression and contribute to the susceptibility of CHD.

Combined Effects of the Rs9810888 Polymorphism in Calcium Voltage-Gated Channel Subunit Alpha1 D (CACNA1D) and Lifestyle Behaviors on Blood Pressure Level among Chinese Children.

Abstract:Backgrounds A recent GWAS Study found a new locus (rs9810888 in CACNA1D) was associated with blood pressure (BP) in Chinese adults. But whether the association exists in children is unknown. Whether lifestyle behaviors could interact with rs9810888 on BP is not clear. This study aimed to identify the association between rs9810888 and BP in children, and also explore the gene-lifestyle interaction. Methods A case-control study was conducted among 2030 Chinese children aged 7 to 18 years. Genotyping was conducted by using the matrix-assisted laser desorption/ionization time-of-flight mass spectrometry. Lifestyle behaviors were investigated with questionnaire. Results With adjustment for age, age square, sex, study group and body mass index (BMI), rs9810888 was significantly associated with diastolic BP (DBP) (b = 1.69, p = 0.021) and mean arterial BP (MAP) (b = 1.56, p = 0.010). Stratified analysis showed that the rs9810888 GG genotype carriers had higher DBP than GT/TT carriers (b = 3.78, p = 0.023) in the subgroup having protein intake (meat/fish/soybeans/egg) = twice/day. In addition, in the subgroup with screen time >= 2h/day, the rs9810888 GG genotype carriers had higher DBP than GT/TT carriers (b = 5.49, p = 0.012), but not in the subgroup with screen time<2h/day. For MAP, in the subgroup having either fruits or vegetables < twice/day, the rs9810888 GG genotype carriers had higher MAP than GT/TT carriers (b = 2.64, p = 0.037), but not in the subgroup with both fruits and vegetables intake >= twice/day. Additionally, in the subgroup with screen time >= 2h/day, the rs9810888 GG genotype carriers had higher MAP than GT/TT carriers (b = 4.80, p = 0.017), but not in those with screen time<2h/day. Conclusions The CACNAID rs9810888 polymorphism was significantly associated with DBP and MAP. In addition, unhealthy lifestyle behaviors and rs9810888 GG genotype had combined effects on BP level among Chinese children.
Family-based Analysis of Apelin and AGTRL1 Gene Polymorphisms with Hypertension in Han Chinese.

Abstract:OBJECTIVE:Apelin and AGTRL1 (angiotensin receptor-like 1), elements of a newly identified pathway with a role in counter regulating the renin-angiotensin system, have been implicated in blood pressure regulation. This study aims to assess whether the apelin and AGTRL1 genetic polymorphisms might contribute to essential hypertension or its related phenotypes.METHODS:We recruited 1015 Han Chinese from 248 families with essential hypertension. Each individual was genotyped for 6 single nucleotide polymorphisms (SNPs) in apelin and 6 SNPs in AGTRL1. Data were analyzed using the family-based association test (FBAT) and the haplotype-based association test (HBAT).RESULTS:FBAT analysis showed that two SNPs rs3761581 and T-1860C within apelin conferred significant association with hypertension and its related phenotypes even after correcting for age and gender. Three SNPs (rs7119375, rs10501367 and rs9943582) within AGTRL1 were found to be associated with hypertension, BMI and the onset age of hypertension, whereas after correction, only marginal associations were noted. Of the common haplotypes (with frequencies over 3%), haplotypes (A-T) and (C-C) comprising rs3761581 and T-1860C in apelin and haplotype (G-G) comprising rs10501367 and rs7119375 in AGTRL1 were shown to be significantly associated with hypertension, BMI and the onset age of hypertension, even after a permutation correction.CONCLUSIONS:Our study suggests that genetic variation within apelin and AGTRL1 likely contributes to essential hypertension, BMI and the onset age of hypertension. Future well designed epidemiological or functional studies would be warranted to validate this hypothesis.
The Gene-Gene Interaction of INSIG-SCAP-SREBP Pathway on the Risk of Obesity in Chinese Children.

Abstract:Background. Childhood obesity has become a global public health problem in recent years. This study aimed to explore the association of genetic variants in INSIG-SCAP-SREBP pathway with obesity in Chinese children. Methods. A case-control study was conducted, including 705 obese cases and 1,325 nonobese controls. We genotyped 15 single nucleotide polymorphisms (SNPs) of five genes in INSIG-SCAP-SREBP pathway, including insulin induced gene 1 (INSIG1), insulin induced gene 2 (INSIG2), SREBP cleavage-activating protein gene (SCAP), sterol regulatory element binding protein gene 1 (SREBP1), and sterol regulatory element binding protein gene 2 (SREBP2). We used generalized multifactor dimensionality reduction (GMDR) and logistic regression to investigate gene-gene interactions. Results. Single polymorphism analyses showed that SCAP rs12487736 and rs12490383 were nominally associated with obesity. We identified a 3-locus interaction on obesity in GMDR analyses (P=0.001), involving 3 genetic variants of INSIG2, SCAP, and SREBP2. The individuals in high-risk group of the 3-locus combinations had a 79.9% increased risk of obesity compared with those in low-risk group (OR=1.799, 95% CI: 1.475–2.193, P=6.61×10-9). Conclusion. We identified interaction of three genes in INSIG-SCAP-SREBP pathway on risk of obesity, revealing that these genes affect obesity more likely through a complex interaction pattern than single gene effect.
Genetic Variations in SEC16B, MC4R, MAP2K5 and KCTD15 Were Associated with Childhood Obesity and Interacted with Dietary Behaviors in Chinese School-Age Population

Abstract:Both genetic predisposition and lifestyle factors are associated with the risk for obesity. Multiple obesity loci have been identified using genome-wide association studies mainly in European populations. The aims of this study were to examine the associations of these loci with obesity and gene×dietary behavior interactions among Chinese children and adolescents. Nineteen candidate SNPs were genotyped using Sequenom technology in the Chinese children (N=2977, 853 obese and 2124 controls, aged 7–17). Dietary behaviors were assessed using self-administered questionnaires. After adjusting for age, sex and multiple testing, MC4R rs17782313, SEC16B rs543874, MAP2K5 rs2241423 and KCTD15 rs11084753 were associated with obesity and obesity-related traits (all P<0.005), with odd ratios ranging from 1.22 to 2.15. Dose–response association was significant between genetic risk score, which was calculated by summing the risk alleles, and the risk of obesity (P<0.001). Multiplicative interaction was found between rs543874 and salt preference on obesity with an OR of 4.40 (95% CI, 1.12–17.30). Additive interactions with salt preference were found in rs17782313 and rs11084753. Our findings indicated that rs17782313, rs543874, rs2241423 and rs11084753 were associated with the risk for children obesity in China, and interaction of genetic variants with diet behaviors on obesity.
Dietary Cholesterol Interacts with SREBF1 to Modulate Obesity in Chinese Children

Abstract:SCOPE:Sterol regulatory element binding protein 1 gene (SREBF1) is an important candidate gene for obesity that could be affected by cholesterol. Different SREBF1 gene variants may have distinct responses to cholesterol, leading to different risks for obesity and obesity-related metabolic traits. Thus, we performed a gene-by-diet correlation analysis to test whether SREBF1 gene variation modulate the relationship between cholesterol and obesity.METHODS AND RESULTS:A total of 642 school-aged children in Jinan, China, were selected by stratified cluster nested sampling. Anthropometric and biochemical measurements, as well as genotyping of tag single nucleotide polymorphisms (SNPs) of SREBF1, were performed in this sample. Nutritional intake assessments were completed using a 24-h dietary recall for three consecutive days. Multilevel mixed-effects linear regression was used to test interactions between SREBF1 SNPs and cholesterol intakes for obesity. Results showed that SREBF1 rs2236513/rs2297508/rs4925119 strongly modulated the relationship between cholesterol intake and serum LDL-cholesterol/total cholesterol levels (p < 0.001). While SREBF1 rs4925118 modulated the relationship between cholesterol intake and homeostasis model assessment of insulin resistance related characteristics (p < 0.05).CONCLUSION:These results suggest that cholesterol intake recommendation may need to account for SREBF1 variation.
Common Variants in Serum/Glucocorticoid Regulated Kinase 1 (SGK1) and Blood Pressure Responses to Dietary Sodium or Potassium Interventions: A Family-Based Association Study

Abstract:Background/Aims: Serum/Glucocorticoid Regulated Kinase 1 (SGK1) plays a significant role in regulating renal Na+ reabsorption, K+ secretion, and blood pressure (BP). This study aimed to assess the association of common genetic variants in the SGK1 gene with BP responses to controlled dietary sodium or potassium interventions. Methods: A total of 334 subjects from 124 families were recruited from the rural areas of northern China. After a three-day baseline observation, they were sequentially maintained a seven-day low-sodium diet (3g/day of NaCl or 51.3mmol/day of sodium), a seven-day high-sodium diet (18g/day of NaCl or 307.8 mmol/day of sodium) and a seven-day high-sodium plus potassium supplementation intervention (4.5g/day of KCl or 60mmol/day of potassium). Six single-nucleotide polymorphisms (SNPs) in the SGK1 gene were selected. Results: After adjustment for multiple testing, SNP rs9376026 was significantly associated with diastolic BP (DBP) and mean arterial pressure (MAP) responses to low-sodium intervention (P = 0.018 and 0.022, respectively). However, the associations between selected SNPs in the SGK1 gene and BP responses to high-sodium or high-sodium plus potassium-supplementation intervention did not reach statistical significance. In addition, SNP rs9389154 and two other SNPs (rs1763509 and rs9376026) were associated respectively with systolic BP (SBP) and DBP at baseline (P = 0.040, 0.032, and 0.031, respectively). SNP rs3813344 was significantly associated with SBP, DBP, and MAP (P = 0.049, 0.015 and 0.018, respectively). Conclusion: Our study indicates that the genetic polymorphism in the SGK1 gene is significantly associated with BP responses to dietary sodium intervention.
Variants of 11β-hydroxysteroid dehydrogenase ( HSD11B ) gene type 1 and 2 in Chinese obese adolescents

Abstract:Objective To investigate the relationship between 11β-hydroxysteroid dehydrogenase ( HSD11B ) gene type 1 and 2 and obesity in Chinese children. Methods A total of 400 obese and 200 healthy adolescents were enrolled as obese and control groups. Seven SNPs in HSD11B1 (rs4393158, rs2235543, rs10082248, rs10863782, rs2236903, rs2298930, rs4545339) and four variants in HSD11B2 gene (rs28934592, rs28934591, rs28934594 and rs28934593) were measured by automated platform MassArray. Results The rs28934592 in HSD11B2 and rs10863782 in HSD11B1 were excluded as false positive or HWE P < 0.05. Moreover, one allele type was found in the other three locations of HSD11B2 . The minor allele frequency of rs2235543 and rs10082248 was higher in patients than that in controls ( P = 0.045, P = 0.041, respectively). The rs10082248, rs2298930 and rs4545339 were associated with the risk of obesity in the recessive model ( P < 0.05, respectively). Moreover, the total cholesterol in patients with GG or AG genotype was significantly higher than that in patients with AA genotype in rs10082248. The rs4393158 was associated with the hypertension in log-additive model test ( P = 0.037), and glucose abnormal and hypercholesteremia in dominant model test ( P < 0.05, respectively), while the rs2235543 was associated with hypercholesteremia in overdominant model test ( P = 0.017). Conclusions The polymorphism of HSD11B1 may be a cause of childhood obesity, or even associated with the complication of childhood obesity. However, variants of HSD11B2 may be not a cause of obesity.
Variant Near FGF5 Has Stronger Effects on Blood Pressure in Chinese with a Higher Body Mass Index

Abstract:BACKGROUND The objective of this study was to investigate the genetic association of 4 candidate variants with blood pressure and test the modifying effects of environmental factors including age, sex, and body mass index (BMI). METHODS We used a linear mixed-effects model to test for variant main effects and variant interactions with age, sex, and BMI on systolic (SBP) and diastolic (DBP) blood pressure in 7,319 Chinese adults from the China Health and Nutrition Survey (CHNS). We attempted to replicate our significant interaction findings in 1,996 Chinese men from the Fangchenggang Area Male Health and Examination Survey (FAMHES). RESULTS Two variants (rs11105378 near ATP2B1 and rs1458038 near FGF5) were significantly associated (P < 0.00625 = 0.05/8) with both SBP and DBP in CHNS. Variant rs1378942 near CSK was nominally associated with SBP (P = 0.01). The signal at rs1458038 exhibited a genotype-by-BMI interaction affecting blood pressure (Pinteraction = 0.0018 for SBP; Pinteraction = 0.049 for DBP), with the strongest variant effects in those with the highest BMI. In FAMHES, rs1458038 also showed stronger effects on SBP and DBP among men with the highest BMI. CONCLUSIONS Our findings suggest high BMI increases the effect of the blood pressure-increasing allele at rs1458038 near FGF5, further highlighting the importance of obesity prevention in reducing hypertension risk.

Abstract:Previous genome-wide association studies (GWAS) have identified and validated variants at more than 1000 loci with modest effects on population blood pressure (BP), explaining in aggregate ~6% of the trait variance and highlighting genetic differences in BP regulation in ethnically diverse populations. We performed an exome-wide association study (EWAS) in Han Chinese population (N=5,954) from Beijing and identified 4 EWAS single nucleotide polymorphism (SNP)-trait associations with 3 SNPs, including two novel associations (rs2165468-peripheral systolic BP and rs33975708-central systolic BP (cSBP). rs33975708 is a coding variant in the coiled-coil domain containing 93 ( CCDC93 ) gene, c.535C>T, p.Arg179Cys (MAF=0.15%) and was associated with substantially increased cSBP (β=29.3 mmHg, P =1.23E-7) in Han Chinese population. CRISPR/Cas9 was used to introduce this variant in mice to investigate vascular molecular mechanisms. While this did not yield a viable mouse with the specific SNP, we did generate mice with an out of frame deletion making a gene knock-out. Ccdc93 deletion ( Ccdc93 -/- ) in its homozygous form was embryonic lethal (<E10.5). Ccdc93 +/- heterozygous mice were viable and morphologically normal and displayed 1.31-fold lower aortic Ccdc93 protein expression (P=0.0041, N=6). Systolic BP was higher in Ccdc93 +/- mice (110±8 mmHg vs 125±10 mmHg, P=0.016, N=5-7) and showed impaired acetylcholine (Ach)-induced relaxation (max Ach 86% vs 68%, P<0.03) and enhanced phenylephrine (PE)-induced contraction (max PE 123% vs 198% normalized to KCL response, P<0.03, N=5) in thoracic aorta ex vivo . Transcriptome analysis by RNA-seq of thoracic aorta identified significantly enriched biological processes in Ccdc93 +/- among the pathways altered in fatty acid metabolic process (adjusted P=2.9E-9) together with mitochondrial metabolism (P=1.3E-14). Plasma free fatty acid levels were higher in Ccdc93 +/- mice (96±7 μM vs 124±13 μM, P=0.0031, N=6) and aortic parkin protein expression, a marker of mitochondrial dysfunction, was 1.63-fold higher (P=0.028, N=6). In conclusion, our genetic and functional studies indicate a potential novel role of CCDC93 on blood pressure regulation through its effects on vascular mitochondrial function.

Association of Common Variants Identified by Recent Genome-Wide Association Studies with Obesity in Chinese Children: a Case-Control Study

Abstract:BACKGROUND:Large-scale genome-wide association studies have identified multiple genetic variants that are associated with elevated body mass index (BMI) or the risk of obesity in Caucasian or Asian populations. We examined whether these variants are individually associated with obesity in Chinese children, and also assessed their cumulative effects and predictive value for obesity risk in Chinese children.METHODS:We genotyped 40 single nucleotide polymorphisms (SNPs) and conducted association analyses for 32/40 SNPs with an estimated minor allele frequency >1% in 2 030 unrelated Chinese children, including 607 normal-weight, 718 overweight, and 705 obese individuals from two cross-sectional study groups. Logistic regression and linear regression under the additive model were used to examine associations, and the area under the receiver operating characteristic curve (AUCROC) was reported as prediction summary.RESULTS:We identified obesity association for 6 SNPs near SEC16B, RBJ, CDKAL1, TFAP2B, MAP2K5 and FTO (odds ratios (ORs) ranged from 1.19 to 1.41, nominal two-sided P-values < 0.05). Association (Bonferroni corrected) of rs543874 near SEC16B and rs2241423 near MAP2K5 had presumably stronger effects on obesity in Chinese children than in Caucasian populations. Their risk alleles were also associated with BMI standard deviation score (BMI-SDS) variability. We demonstrated the cumulative effects of the 32 SNPs on obesity risk (per risk allele: OR = 1.06, 95 % CI: 1.03-1.11, P = 4.84 × 10(-4)) and BMI-SDS (β = 0.04, 95% CI: 0.02-0.06, P = 3.69 × 10(-7)). The difference in AUCROC for a model with covariates (age, age square, sex and study group) and the model including covariates and all 32 SNPs was 2.8% (P = 0.0002).CONCLUSION:While six SNPs were individually associated with obesity in Chinese children, the 32 common variants identified by recent GWA studies had cumulative effects and resulted in a limited increase in the AUCROC predictive value for childhood obesity.

Abstract:Although multiple genetic markers associated with blood pressure have been identified by genome-wide association studies, their aggregate effect on risk of incident hypertension and cardiovascular disease is uncertain, particularly among East Asian who may have different genetic and environmental exposures from Europeans. We aimed to examine the association between genetic predisposition to higher blood pressure and risk of incident hypertension and cardiovascular disease in 26 262 individuals in 2 Chinese population-based prospective cohorts. A genetic risk score was calculated based on 22 established variants for blood pressure in East Asian. We found the genetic risk score was significantly and independently associated with linear increases in blood pressure and risk of incident hypertension and cardiovascular disease ( P range from 4.57×10 –3 to 3.10×10 –6 ). In analyses adjusted for traditional risk factors including blood pressure, individuals carrying most blood pressure–related risk alleles (top quintile of genetic score distribution) had 40% (95% confidence interval, 18–66) and 26% (6–45) increased risk for incident hypertension and cardiovascular disease, respectively, when compared with individuals in the bottom quintile. The genetic risk score also significantly improved discrimination for incident hypertension and cardiovascular disease and led to modest improvements in risk reclassification for cardiovascular disease (all the P <0.05). Our data indicate that genetic predisposition to higher blood pressure is an independent risk factor for blood pressure increase and incident hypertension and cardiovascular disease and provides modest incremental information to cardiovascular disease risk prediction. The potential clinical use of this panel of blood pressure–associated polymorphisms remains to be determined.
Two Obesity Susceptibility Loci in LYPLAL1 and ETV5 Independently Associated with Childhood Hypertension in Chinese Population.

Abstract:AIMS:Genome-wide association studies have identified novel obesity-associated susceptibility loci. Associations of these variants with childhood obesity have been studied in our previous research. The purpose of this study is to investigate if these loci are associated with hypertension being independent of obesity in Chinese children and adolescents.METHODS:Nineteen candidate SNPs were genotyped using Sequenom MassARRAY platform among Chinese children (N=2954, 514 hypertension and 2440 controls, aged 7-17years). Dietary behaviors were assessed through face to face investigations.RESULTS:Of the nineteen obese related SNPs, ten SNPs were found to be associated with systolic blood pressure (SBP) or diastolic blood pressure (DBP) in Chinese children. After adjusting for age, sex and WHtR, rs2605100 in LYPLAL1was found to be associated with high blood pressure (HBP) under dominant model (P=0.024) with the OR of 1.274 (95% CI =1.033-1.572, effect genotype=GG). The distribution of genotype of rs7647305 in ETV5 showed significant difference between HBP and non-HBP subjects under dominant model (P=0.011) with the OR of 0.654 (95% CI=0.471-0.909, effect genotype=CC). Using rs2605100 and rs7647305, the genetic risk score (GRS) analysis showed that, after adjusted for age, sex and WHtR, subjects carrying one or two risk alleles had the risks of hypertension with the ORs 1.797 (95% CI, 1.168-2.765), 2.149 (95% CI, 1.375-3.357) comparing with the subjects with non-risk-allele.CONCLUSIONS:Genetic variations of obesity-associated loci, LYPLAL1 rs2605100 and ETV5 rs7647305 independently associate with the risk of childhood hypertension in China.

Abstract:Objectives: Two large-scale genome-wide association studies (GWAs) have identified multiple variants associated with blood pressure (BP) or hypertension. The present study was to investigate whether some variations were associated with BP traits and hypertension or even prehypertension in adult She ethnic minority of China.Methods: The population of the present study comprised 4460 (1979 males and 2481 females, respectively) unrelated she ethnic minority based on a cross-sectional study from Ningde City in Fujian province of China. There were 1692 hypertensives, 1600 prehypertensives and 1168 normotensive controls, respectively. We genotyped 7 variants in CYP17A1, PLEKHA7, CACNB2, ATP2B1, TBX3-TBX5, CSK-ULK3 and 5H2B3 reported by the previous GWAs on Europeans. All analyses were performed in an additive genetic model.Results: As the minor allele of rs653178 in/near 5H2B3 was very rare with the frequency of 0.018, we excluded this single nucleotide polymorphism (SNP) in the further analyses. Of the other 6 loci, linear regression analyses revealed that rs11191548 in CYP17A1 and rs11014166 in CACNB2 were significantly associated with systolic BP (beta = -1.17, P=0.002 and beta = -0.50, P=0.006, respectively), while only SNP rs11191548 was significantly associated with diastolic BP (beta = -0.56,P=0.002) after adjusted by age, sex and BMI. Two variants in CACNB2 and PLEKHA7 were found to be significantly related to hypertension (odds ratios [OR] and (95% confidence interval [CI]): 0.79 (0.65-0.97) and 1.19 (1.01-1.41), respectively) in logistic regression analyses after adjusted by age, sex and BMI. In addition, we found that combined risk alleles of the 6 SNPs increased risk of hypertension in a stepwise fashion (P for trend <0.001). However, none of the 6 SNPs was significantly associated with BMI or prehypertension status. While logistic analysis showed that subjects with cumulative risk alleles more than 9 had significantly higher risk for prehypertension (adjusted OR: 3.10, P < 0.001) compared with those with risk alleles less than 4.Conclusions: We replicated that variations in CYP17A1, CACNB2 and PLEKHA7 were related to BP traits and/or hypertension in She population. In addition, although we failed to observe single gene associated with prehypertension, we first found that conjoint effect of multiple risk alleles on BP might increase the risk of progressing to prehypertension.
