Quantitative Imaging in Medicine and Surgery

Abstract:Following tremendous economic progress, society in China is also undergoing fundamental changes, as is the healthcare system. Currently the training of Chinese young doctors and their future work placement are all undergoing re-structuring. We compiled some thoughts and opinions on the topic of 'should clinicians in China engage in research?', and publish them as a special report in this issue of Quantitative Imaging in Medicine and Surgery (QIMS). The contributors included some editorial members of this journal, and a few personal friends. Besides a few minor linguistic corrections, opinions from the contributors have not been edited, as we want authors' to write their own independent views. However, it is possible there is a selection bias of the contributors of this paper; more likely those who are interested in the medical research are selected and therefore the views of the contributors may not be generalizable. To compare the structure and funding of China with other countries, authors from UK, The Netherlands, France, and USA are also invited.
Exploring the Health Narratives of Chinese Female Migrant Workers through Culture-Centered and Gender Perspectives

Abstract:Female migrant workers (FMWs) represent a marginalized group in China who not only face health disparities but also lack a voice in society. Drawing on the culture-centered approach (CCA) and gender perspective to co-construct health meanings with FMWs, this research aimed to answer the question of how FMWs narrate their health and to reveal the agency of the cultural members. The in-depth interview method was applied to record the health narratives of 30 participants from October 2016 to December 2016. The results showed that FMWs' understandings of health were beset by the cultural system of gender in poverty, exploited working conditions, and discrimination in the medical context. The participants identified minimum wage and medical insurance as the key structural obstacles to healthy lives in the city. Moreover, FMWs had the agency to exercise health practices that challenge the domination of biomedicine. With gender issue threaded throughout the contextualized health narratives, the role of the gender perspective could be viewed as a "contact zone" in CCA. FMWs' agency and its interactions with structures and culture indicate the significance of engaging the voices at the margins in health communication research and praxis.

Abstract:Unintended pregnancy is a global issue, with approximately 85,000,000 women around the world having unintended pregnancy annually. The contents of clinical application of women's contraceptive methods are very wide, involving multiple areas. This consensus deeply discusses the specific contraceptive needs at different statuses, combined with gynecological diseases, postabortion contraception, and postpartum family planning, ensuring the correct use of contraceptive methods under the corresponding status. The top priority of the consensus is the specific contraception consensus section for women combined with gynecological diseases because medical treatment effect as well as side effects should be weighed carefully. The consensus is to make high-efficiency and individual contraceptive strategy for different groups based on multidisciplinary (gynecology, obstetrics, and family planning) and multidimensional aspects, which can provide uniform guidance for medical and health organizations under the condition as relevant global guidance or consensus is still lacking.

Abstract:Traditional Chinese Medicine (TCM) has significant advantages in treating gynaecological disorders. The paper has provided a brief introduction on the current progress of treating some gynaecological disorders including endometriosis, infertility, dysmenorrhea, abnormal uterine bleeding, premenstrual syndrome, menopausal syndrome, uterine fibroids, chronic pelvic inflammation, polycystic ovarian syndrome (PCOS), cervicitis and vaginitis with Chinese Herbal Medicine (CHM) and acupuncture. The use of TCM in the field of assisted reproductive techniques (ART) has also been included in the review. In addition, thirty-two commonly used Chinese medicinal formulas in treating gynaecological disorders have been introduced.

Abstract:Chinese traditional medicine (CTM) is a health care system with an extensive history of practical clinical experience. The foundation of CTM, while relatively simple, is substantively different from much of allopathic medicine. Such differences are difficult to explain using a Western medical vocabulary, and extend beyond linguistic foundations. This proves challenging when trying to identify appropriate teaching and research methods that are sensitive to the CTM paradigm and yet relevant to a public health orientation. Given the increased use of CTM, it becomes important to address possibilities that would ensure successful integration of CTM into a public health framework. We propose a model in which both CTM and biomedical clinical services could be offered to provide diverse, yet truly integrative, therapeutic approaches. Within this model, it is critical to enhance reciprocal educational and research-directed opportunities for both CTM practitioners and allopathic clinicians. Considerable responsibility rests upon academic institutions in becoming proactive in developing and implementing educational curricula and research programs that illustrate more effectively the potential bilateral benefit(s), limitations, and, ultimately, roles that CTM and biomedical approaches may assume within an integrative system of care.

Abstract:Globally, public health interventions have resulted in a 30-year increase in women's life expectancy. However, women's health has not increased when socioeconomic status is ignored. Women's health has become a major public health concern, for those women from developing countries are still struggling with infectious and labor-related diseases, and their counterparts in developed countries are suffering from physical and psychological disorders. In recent years, complementary and alternative medicine has attracted wide attentions with regards to maintaining women's health. Acupuncture, a crucial component of traditional Chinese medicine, has been used to treat many obstetric and gynecological diseases for thousands of years due to its analgesic and anti-inflammatory effects and its effects on stimulating the sympathetic/parasympathetic nervous system. To fully understand the mechanism through which acupuncture exerts its effects in these diseases would significantly extend the list of available interventions and would allow for more reasonable advice to be given to general practitioners. Therefore, by searching PubMed and CNKI regarding the use of acupuncture in treating obstetric and gynecological diseases, we aimed to summarize the proven evidence of using acupuncture in maintaining women's health by considering both its effectiveness and the underlying mechanisms behind its effects.

Abstract:Background: Breast cancer poses a significant health threat to women, marked by high incidence and mortality rates. Although modern treatment primarily involves surgery and chemotherapy, which may reduce quality of life, limited analysis exists on the effects of traditional Chinese medicine (TCM) on this aspect. In this paper, bibliometric software was used to study the literature related to TCM in the treatment of breast cancer to provide researchers with new insights and directions for development. Methods: By using CiteSpace and VOSviewer, we conducted an analysis of literature on TCM in breast cancer treatment from the Web of Science Core Collection (2013-2023). The assessment encompassed countries, institutions, journals, authors, keywords, and references, providing a comprehensive overview of developments in the field. Results: Analyzing 7419 articles, this study revealed an increasing trend in TCM research on breast cancer from 2013 to 2023. China and its institutions, particularly Beijing University of Chinese Medicine, made significant contributions. Liu Zhishun emerged as the most prolific author, while Y. Macpherson received the highest number of citations. The journal Evidence-Based Complementary and Alternative Medicine received the highest number of citations. Research primarily centers on TCM approaches for enhancing breast cancer patients' quality of life and elucidating their underlying mechanisms. Conclusion: Numerous studies have investigated TCM in treating female breast cancer, revealing treatment trends, analyzing major research topics, currently focusing on acupuncture, breastfeeding, and TCM, which have a greater impact on positively ameliorating breast cancer. Influential authors and publications were also identified. These insights provide valuable guidance for future research, contributing to the foundation for developing effective TCM strategies for breast cancer.

Abstract:Purpose: Healthcare utilization and health-seeking behaviors of Chinese American immigrant women may be influenced by longstanding cultural perspectives of family roles and relationships. An understanding of Chinese immigrant women's perceptions of family social support in health and how these beliefs manifest in healthcare utilization and help-seeking behaviors is critical to the development of culturally appropriate health interventions. Focusing on a sample of Chinese women in Chicago's Chinatown, this qualitative study seeks to describe women's attitudes and beliefs about spouse and adult children's involvement in women's health and healthcare. Methods: We conducted six focus groups among 56 Chinese-speaking adult women in Chicago's Chinatown between July and August 2014. Focus groups were transcribed, coded, and analyzed for emergent themes. Results: Women reported that their adult children supported their health and healthcare utilization by helping them overcome language and transportation barriers, making and supporting decisions, and providing informational and instrumental support related to diet and nutrition. Women viewed these supports with mixed expectations of filial piety, alongside preferences to limit dependency and help-seeking because of concern and emotional distress regarding burdening adult children. Women's expectations of the spouse involvement in their healthcare were low and were shaped by avoidance of family conflict. Conclusion: Findings inform opportunities for the development of culturally appropriate interventions to enhance Chinese immigrant women's health and healthcare. These include patient navigation/community health worker programs to promote self-management of healthcare and family-centered strategies for enhancing family social support structures and reducing family conflict.

Abstract:Background: The field of medicine dates back to ancient times. Early practices were adopted by subsequent societies. Cultures had significant influences over many aspects of life. Globally, there is a health care crisis of inadequate delivery and less-than-optimal outcomes despite all the advancements and evidence provided by modern biomedicine. North Atlantic Treaty Organization countries represent diverse cultures and societal experiences and, collectively, may offer new perspectives in the evolution of health care delivery. Objective: A retrospective review of the influences of ancient practices and cultures is provided. This can serve as a foundation for a prospective discussion about integrating the various current systems into a better paradigm of health care. Methods: This article discusses the impact of historical and cultural perspectives on medicine in general and on specifically integrative medicine (IM). The origins of some IM modalities are highlighted. Conclusions: History reveals that new medical paradigms were a blending of old traditions with new innovations. Changes occurred because society questioned old methods. Today the situation is not different. The field of IM is evolving as a result.

Abstract:Purpose: Traditional Chinese medicine (TCM) proposes the world as a holistic unity of energy. With a long history of use, TCM is generally accepted in the Chinese health care system. Despite growing global interest and use, TCM has received a mixed response from Western health care systems. This study aimed to describe the underpinnings of TCM, compare TCM and Western medicine, and suggest how key modalities might be applied to strengthen holistic nursing care in the United States. Design: A qualitative, descriptive exploratory investigation employing a constructivist grounded theory. Method: Interview of TCM healers in Hong Kong ( n = 5). Transcripts were reviewed to identify common themes and categories. Findings: Five themes were identified: improvement in energy flow and health, TCM as “a way of knowing,” curing source versus symptoms of disease, declining TCM practice in Hong Kong, and need for interprofessional education. Conclusions: TCM is considered useful in chronic illnesses, whereas those with acute illnesses often seek Western medicine. Failing to address the holistic care needs and preferences of individuals results in less than optimal health outcomes. Nurses need explicit TCM content and would benefit from a restructuring of curricula and clinical experiences to foster integration into caring practices.

Abstract:The Obstectrics and Gynecology Hospital attached to Fudan University has made great achievements in the clinical and experimental study of integrated traditional Chinese and western medicine in the past 45 years. Study on induction of ovulation with acupuncture revealed the relationship between circulating estrogen and central beta-endorphin, and suggested that the depletion of central beta-endorphin resulted in the release of GnRH and ovulation, and the main indication for acupuncture treatment was pubertal dysfunctional uterine bleeding. Acupuncture was also successfully used to treat anorexia nervosa. The successful treatment of polycystic ovary syndrome (PCOS) with Yu's Tonifying Recipe for tonifying the kidney and resolving phlegm was found to be related to the elevation of serum FSH which resulted in ovulation. Good result was obtained in hyperinsulinemia pattern of PCOS treated with Tiangui Recipe for replenishing the kidney and activating blood. In studies on patients and animal model, Tiangui Recipe was found to decrease peripheral androgen and insulin levels and lead to central regulation of proopiomelanocortin, neuropeptide Y and leptin receptor, as well as regulation of neuro-endocrine-metabolic network. Thus it can promote ovulation and reduce body weight. In the studies, a new way to classify PCOS was suggested. Gengnianchun Recipe for women with postmenopausal syndrome was found to be effective in increasing estrogen receptors (ERs) and ER mRNA in hypothalamus and peripheral organs and regulating neuro-endocrine-immune-metabolic network in patients and aging rats. Gongtai was effective for controlling menorrhagia under the mechanism of regulating endometrial prostaglandins. The researches mentioned above indicate that illness is the disorder of some key-links in the life network of the patient which may be affected by factors from the environment. And this idea is different from the ideas in traditional Chinese medicine and biomedicine. It indicates that woman in subclinical health state has already got something incontrollable in her life network, and it also gives a new impact on the treatment and prevention of women's diseases as well as the prevention and treatment of women's reproductive health.
Complementary and integrative healthcare communication in Chinese American patient / primary care visits: An observational discourse analysis

Abstract:Objective: Chinese-American patients use CIH at high rates but disclosure of CIH use to clinicians is low. Further, the content of CIH talk between patients and their clinicians is not well described. We aimed to characterize CIH talk between Chinese-American patients and their primary care clinicians. Methods: Discourse analysis of 70 audio-recordings of language concordant and discordant-interpreted visits. Results: Nearly half of all visits (48.6%) had some form of CIH communication. 'Simple CIH talk' focused on a single CIH topic resulting in a positive, neutral, or negative response by clinicians. 'CIH-furthering talk' was characterized by clinicians and patients addressing more than one CIH topic or including a combination of orientations to CIH by both clinicians and patients. CIH-furthering talk characterized by clinician humility could enhance rapport, cultural understanding, and open communication. CIH-furthering talk also led to miscommunication and retreat toward biomedicine. Conclusion: CIH communication occurred frequently during language concordant and discordant-interpreted visits with Chinese-American patients. Both patients and clinicians used CIH-furthering talk as a conversational resource for managing care. Innovation: This discourse analysis of visits between Chinese-American patients and their clinicians advances understanding of CIH communication beyond disclosure, illustrating the complexity of linguistic and cultural nuances that affect patient care.
Delivery of patient-centered care in complementary medicine: Insights and evidence from the Chinese medical practitioners and patients in primary care consultations in Hong Kong

Abstract:Introduction: Traditional Chinese Medicine (TCM) has become increasingly popular around the world, and has been accepted by people not only in China and Southeast Asia, but also in Western countries. Despite its historic role in the Chinese society, there has been limited research on exploring the nature of TCM practitioner-patient interactions in the Chinese context. As indicated by a major study regarding the Hong Kong context1, there is a need to investigate the role of TCM practitioner and promote interdisciplinary research to ensure safety and synergy of TCM and Western medicine in primary care. This study aims to address this gap by investigating the nature of TCM consultations and their communication patterns in Hong Kong. Methods: Based on 10 h of conversations (in Cantonese) between TCM practitioners and their patients in the diagnostic interviews, the study explored how the doctor-patient relationship was negotiated in the course of the consultation, while both the TCM practitioners and the patients were constantly trying to manage and maintain common ground. Particular attention had been paid to the identification of specific linguistic and discourse strategies that TCM practitioners had employed to establish doctor-patient rapport, so that a better understanding of patient-centred care in the TCM context could be obtained. The participants were recruited from a local university operated clinic which shared the characteristic of TCM practitioners in Hong Kong. Results: A range of linguistic strategies that TCM practitioners used to deliver patient-centred care have been identified. These strategies are also helpful in shaping a joint decision-making process that will lead to better patient understanding and compliance with the doctors' treatments. Conclusions: This study demonstrates empirically how TCM practitioners utilize a range of linguistic resources and communication strategies to shape the ongoing discourse so that their patients can have a better understanding of their illnesses. For an example, it is found that TCM practitioners and their patients were constantly trying to manage and maintain common ground by using a range of grammatical markers, including sentence-final particles (SFPs) and discourse markers (DMs), to negotiate the epistemic commitment so that the patient would have good compliance with the practitioner's suggested treatment. It is also observed that various types of interrogatives have been used by the TCM practitioners to elicit information from the patients as well as to encourage them to talk and make a response. Furthermore, it is found that TCM practitioners would deliberately enquire about the patients' everyday experiences because what they eat, do, and encounter all have an important impact on their body conditions. By exploring into the patients' daily routines in the social talk, the practitioner can help maintain and promote the overall balance of the patient's body, and help them monitor and enhance their health conditions by modifying their daily habits and behaviours. With the adoption of these linguistic and communication strategies, the TCM practitioners are shown to have placed the patients' needs as their top priority. Previous studies in the field have already proved that co-construction of the treatment plan between the doctor and the patient is extremely important, and that a patient-centred approach can largely reduce adverse events leading to avoidable patient harm. The specific strategies identified in the current study can enhance the TCM practitioners' communication with patients, creating an environment that will surely optimise safety for both patients and clinicians.

Abstract:Background: Surveys of acupuncture practitioners worldwide have shown an increase in the use of acupuncture to treat women’s health conditions over the last ten years. Published studies have explored the effectiveness of acupuncture for various conditions such as period pain, fertility, and labor induction. However, it is unclear what role, if any, peer-reviewed research plays in guiding practice. Methods: Acupuncturists with a significant women’s health caseload were interviewed online in three small groups to explore factors that contribute to acupuncturists’ clinical decision made around treatment approaches and research. Results: Eleven practitioners participated in the focus groups. The overarching theme that emerged was one of ‘Not mainstream but a stream.’ This captured two themes relating to acupuncture as a distinct practice: ‘working with what you’ve got’ as well as ‘finding the right lens’, illustrating practitioners’ perception of research needing to be more relevant to clinical practice. Conclusions: Acupuncture practitioners treating women’s health conditions reported a disconnect between their clinical practice and the design of clinical trials, predominantly due to what they perceived as a lack of individualization of treatment. Case histories were popular as a learning tool and could be used to support increasing research literacy.

Chinese medical education, residency training, obstetric care structure, and management of common obstetric complications are briefly reviewed and compared to the United States. Maternal-fetal medicine (MFM) is rapidly developing in China, but the development of MFM may not follow the same trajectory as in the West. Abstract:Our Bodies,Ourselves,as a women's self-help handbook,is a major achievement of the American Women's Health Movement in the 1970s.From its first edition to the latest version of 2011,it has undergone nine major revisions in 40 years and has been translated into 30 languages,which also had a profound impact on the development of the global women's health.This article selects three representative editions and divides them into two parts:"sexuality and childbearing" and "taking care of ourselves",exploring the changes in the depth and breadth of health issues in different parts of the world,in order to outline the changes of contemporary women's health concept from the physical level to the psychological level,from the family level to the social level,from the focus of childbearing period to the focus of whole life time.
Challenges and opportunities of integrating traditional Chinese medicine into mainstream medicine: A review of the current situation

Abstract:IntroductionAlthough Traditional Chinese medicine (TCM) has been practiced for centuries, its acceptance within an integrative framework is hindered by conventional scientific research methodology. How can we optimize research to support the integration of TCM into mainstream medicine? The aim of this paper was to propose priority areas for future TCM research by characterizing the literature contributing to published articles resulting from the three year project “Good Practice in TCM Research (FP7 GP-TCM) in the Post-genomic Era”.MethodsAll references were extracted from the 20 papers published as a direct result of the GP-TCM project. The main themes of the papers were categorized and the distribution of articles referring to each research topic calculated.ResultsThe nine main themes that were identified included: Chinese materia medica (CMM) quality, CMM toxicity/safety, in silico, in vivo, in vitro, clinical trials (CMM), clinical trials (acupuncture), regulation and future direction. In addition, three extra research topics (authorization, research methodology and others) were identified. Of these, the most frequently reported themes were CMM toxicity/safety, quality and in vivo research.ConclusionWithin this framework, the quality control of CMM products, network pharmacology, biomedical studies using systems biology and the adaptation of Western clinical research methods were supported by advances in research and technology. Combined with international collaboration, these provide vital directions for research and development in TCM to validate integrative approaches for mainstream medicine and health care models for the future.This article belongs to the Special Issue: Traditional and Integrative Approaches for Global Health.
