Abstract:Early presentation is critical for receiving effective reperfusion therapy for acute ischemic stroke, therefore, we undertook a national survey of awareness and responses to acute stroke symptoms in China. We undertook a cross-sectional community-based study of 187 723 adults (age ≥40 years) presenting to 69 administrative areas across China between January 2017 and May 2017 to determine the national stroke recognition rate and the correct action rate. Multivariable logistic regression models were used to identify factors associated with stroke recognition and intention-to-avail emergency medical services. Estimates of stroke recognition rate and correct action rate were 81.9% (153 675/187 723) and 60.9% (114 380/187 723), respectively, but these rates varied widely by sociodemographic status, region, and stroke risk. Approximately one-third of participants who recognized a stroke failed to call emergency medical service. Low likelihood of emergency medical service use was associated with younger age (40–59 years), being male, rural location, (regions of east, south, and northwest China), high body mass index (≥24), low education (primary school or below), low personal income (<us $731="" (odds="" (≥2),="" 2.00–2.10;="" 2.05;="" 95%="" P <0.001). Substantial discrepancies exist between stroke recognition and correct action and not all stroke patients know the appropriate responses. Further, national stroke educational programs with specific plans targeting different groups are needed, which do not solely focus on stroke recognition, but also on the appropriate responses at the time of a stroke.

Abstract:Background: While FAST (Face, Arm, Speech, Time) has been used in China for stroke awareness and rapid response education, however, the effectiveness is unclear due to the language barrier, and the stroke related mortality remains very high. We recently engineered Stroke 1-2-0 for China based on FAST.[1] This investigation aimed to investigate its acceptance in community physicians. Method: This investigation protocol was approved by the Institutional Review Board of the Minhang Hospital affiliated to Fudan University. Questionnaire is distributed to community physicians or family doctors (total of 247 physicians). An educational session was planned to teach both FAST and Stroke 1-2-0 for the participants before and after educational session to evaluate the awareness of stroke, stroke recognition tool and the acceptance of the stroke recognition tool. Results: In educational sessions consisting of 247 family physicians (82 male and 165 female), 98.79% of the survey responders are at the age range of 21-60 years old in two major cities (Shanghai and Hangzhou). Only 10.53% of the participants know that the thrombolytic therapeutic window is 4.5 hours. 28.74% of the participants select time points more than 4.5 hours. Only small portion of physicians in the community know FAST (36.44%) or Stroke 120 (39.27%) as a stroke recognition tool. After the training session, majority of the participants understand that the therapeutic window for ischemic stroke is 4.5 hr as compared to a small portion before the educational session (10.53%). Majority of the participants mastered Stroke 1-2-0 (98.79%) and FAST (98.79%) immediately after the training session. However, 97.17% of the participants believe that Stroke 1-2-0 is the most suitable program in China in stroke education. Only seven participants believe that FAST is a still better tool for Chinese. Conclusion: While community physician play critical role in patient education, a significant knowledge deficiency for stroke awareness is identified. Despite both Stroke 120 and FAST can be mastered easily by community physician, they believe Stroke 1-2-0 should be used for stroke awareness education and rapid action for the public domain in China.

Abstract:Background In June 2015, the Chinese Stroke Association (CSA) initiated the Chinese Stroke Center Alliance (CSCA) to establish the national hospital-based stroke care quality assessment and improvement platform. This article outlines its objectives, operational structure, patient population, quality improvement (QI) intervention tools, data elements, data collection methodology and current patient and hospital data. Methods The CSCA is a national, hospital-based, multicentre, voluntary, multifaceted intervention and continuous QI initiative. This multifaceted intervention includes stroke centre development, written care protocols, workshops and a monitoring/feedback system of evidence-based performance measures. The data coordinating centre of the CSCA resides at the China National Clinical Research Center for Neurological Diseases, Beijing Tiantan Hospital. Results As of July 2017, 1576 hospitals in China have contributed detailed clinical information to serve as a benchmark for the stroke care quality of 433264 patients with acute stroke/transientischaemic attacks (TIA), including 352572 (81.38%) acute ischaemic stroke, 30362 (7.01%) TIA, 42080 (9.71%) spontaneous intracranial haemorrhage, 5505 (1.27%) subarachnoid haemorrhage and 2745 (0.63%) not specified stroke. Conclusion The CSCA programme is designed to establish a continuous national stroke registry and help healthcare providers develop stroke centres and treat patients in a consistent manner in accordance with accepted national guidelines and, ultimately, improve patient outcomes. It supports the CSA mission to reduce stroke burden in China.
Thrombolysis in Acute Ischemic Stroke: Issues and Options

Abstract:China is one of the countries with highest incidence of stroke in the world. However, the rate of intravenous thrombolysis for ischemic stroke is extremely low in China, which is related to traditional triage system and inefficient management, the potential of hemorrhagic transformation, and narrow therapeutic time window. In order to improve the clinical implementation of thrombolysis, it is necessary to look at the related issues and to refine the therapeutic protocol according to the international experiences. Careful screening of eligible patients is crucial to reduce the rate of symptomatic intracranial hemorrhage. The advanced penumbral imaging is believed to enhance patient selection and extend the therapeutic time window. In addition, endovascular therapy is increasingly utilized to raise the recanalization rate. All these measures would enhance the improvement of the clinical management for acute stroke patients.

Abstract:Objective. Most patients of acute ischemic stroke (AIS) receive treatments in the department of emergency in China. We aimed to examine the status of AIS diagnosis and treatment and the impact of green pathway operation in different regions of China. Methods. In this nationwide survey, information regarding the emergency care of AIS was collected from 451 hospitals in different regions of China, by interviewing 484 physicians from these hospitals. Structured questionnaire was used to explore the status of AIS care and impact of the green pathway. Results. 445 hospitals from 18 provinces, 4 municipalities, and 3 ethnic autonomous regions in China were included in the present study. Overall, the proportion of door-to-needle time (DNT) less than 60 min was 66.08% in the enrolled hospitals (n = 298). Stratified by regions, the results suggested that hospitals located in East regions had shorter DNT time (P=0.036), and more proportion of rtPA (P<0.001) than those in West regions. Further analysis suggested that hospitals with a green channel were more likely to shorten DNT and improve the proportion of rtPA (P<0.01). Conclusion. Considerable regional differences were observed in terms of DNT time and thrombolysis rates in the departments of emergency in China. Further studies are required to confirm the regional differences in AIS care in China.

Abstract:Background and Purpose: Patients with large vessel occlusion stroke (LVOS) need to be rapidly identified and transferred to comprehensive stroke centers (CSC). However, previous pre-hospital strategy remains challenging. We aimed to develop a modified scale to better predict LVOS. Methods: We retrospectively reviewed our prospectively collected database for acute ischemic stroke (AIS) patients who underwent CT angiography (CTA) or time of flight MR angiography (TOF-MRA) and had a detailed National Institutes of Health Stroke Scale (NIHSS) score at admission. Large vessel occlusion (LVO) was defined as the complete occlusion of large vessels, including the intracranial internal carotid artery (ICA), M1, and M2 segments of the middle cerebral artery (MCA), and basilar artery (BA). The Conveniently-Grasped Field Assessment Stroke Triage (CG-FAST) scale consisted of Level of Consciousness (LOC) questions, Gaze deviation, Facial palsy, Arm weakness, and Speech changes. Receiver Operating Characteristic (ROC) analysis was used to obtain the Area Under the Curve (AUC) of CG-FAST and previously established pre-hospital prediction scales. Results: Finally, 1,355 patients were included in the analysis. LVOS was detected in 664 (49.0%) patients. The sensitivity, specificity, positive predictive value, and negative predictive value of CG-FAST were 0.617, 0.810, 0.785, and 0.692 respectively, at the optimal cutoff (≥4). The AUC, Youden index and accuracy of the CG-FAST scale (0.758, 0.428, and 0.728) were all higher than other pre-hospital prediction scales. Conclusions: CG-FAST scale could be an effective and simple scale for accurate identification of LVOS among AIS patients.

Abstract:In China, stroke is a major cause of mortality, and long-term physical and cognitive impairment. To meet this challenge, the Ministry of Health China Stroke Prevention Project Committee (CSPPC) was established in April 2011. This committee actively promotes stroke prevention and control in China. With government financial support of 838.4 million CNY, 8.352 million people from 536 screening points in 31 provinces have received stroke screening and follow-up over the last seven years (2012–2018). In 2016, the CSPPC issued a plan to establish stroke centers. To shorten the pre-hospital period, the CSPPC established a stroke center network, stroke map, and stroke “Green Channel” to create three 1-h gold rescue circles, abbreviated as “1-1-1” (onset to call time <1 h; pre-hospital transfer time < 1 h, and door-to-needle time < 1 h). From 2017 to 2018, the median door-to-needle time dropped by 4.0% (95% confidence interval (CI), 1.4–9.4) from 50 min to 48 min, and the median onset-to-needle time dropped by 2.8% (95% CI, 0.4–5.2) from 180 min to 175 min. As of 31 December 2018, the CSPPC has established 380 stroke centers in mainland China. From 1 November 2018, the CSPPC has monitored the quality of stroke care in stroke center hospitals through the China Stroke Data Center Data Reporting Platform. The CSPPC Stroke program has led to a significant improvement in stroke care. This program needs to be further promoted nationwide.

Unconventional association between risk factors and knowledge of stroke among cerebrovascular disease survivors in China: A cross-sectional community-based study (FAST-RIGHT)

Abstract:Background: Cerebrovascular disease (CVD) survivors are at a risk of recurrent strokes, and early correct response to stroke is crucial to promote access to effective reperfusion therapy. This study aimed to investigate whether there is an association between increased risk factors and the intent to call emergency medical services (EMS) among CVD survivors. Methods: A cross-sectional community-based study was conducted from January 2017 to May 2017, including 187 723 adults (age ≥ 40 years) across 69 administrative areas in China. A CVD survivor population of 6290 was analyzed. Multivariable logistic regression models were used to identify the association between the number of modifiable risk factors and stroke recognition and EMS calling, respectively. Results: The estimated stroke recognition rate in CVD survivors with 0, 1-2, and 3-7 modifiable risk factors was 84.7% (321/379), 84.4% (2346/2780), and 85.6% (2673/3123), respectively. The rate of calling EMS in the 0, 1-2, and 3-7 modifiable risk factor groups was 66.0% (250/379), 62.7% (1744/2780), and 67.8% (2117/3123), respectively. The prevalence of cardiovascular diseases was higher among CVD survivors than the non-CVD population. The CVD survivors’ knowledge of recognizing stroke and intent to call EMS did not improve with an increasing number of modifiable risk factors, even after adjustment for multiple sociodemographic factors. Conclusions: Despite being at a higher risk of recurrent stroke, Chinese CVD survivors showed poor knowledge of stroke, and their intent to call EMS did not improve with stroke risk. Special, targeted, and enhanced secondary stroke prevention education is needed for CVD survivors.
Analysis of Time to the Hospital and Ambulance Use Following a Stroke Community Education Intervention in China

Abstract:IMPORTANCE Prehospital delay (time from symptom onset of stroke to the door of a hospital) in patients with stroke is long in China. With the goal of improving public awareness and knowledge of stroke recognition, Stroke 1-2-0 was developed in China as an education program to prompt rapid response to the onset of stroke based on clinical practice in China, and examination of its outcomes is needed. OBJECTIVE To investigate the association of the Stroke 1-2-0 educational campaign with prehospital delay for patients with ischemic stroke. DESIGN, SETTING, AND PARTICIPANTS In a population-based cross-sectional study, all patients with ischemic stroke events were admitted to the Minhang Hospital, which is the only tertiary care hospital with a stroke center that provides acute stroke care in Xinzhuang county, Shanghai, China. The study period was from January 1, 2016, to December 31, 2019, and data analysis was performed from January 1 to July 31, 2021. EXPOSURES A multifaceted Stroke 1-2-0 educational campaign comprising slides, videos, brochures, and posters distributed in the community. MAIN OUTCOMES AND MEASURES Proportion of patients with hospital arrival within 3 hours and use of an ambulance to seek medical care, as well as the odds of seeking medical attention within 3 hours after the stroke before vs after initiation of the multifaceted educational campaign. RESULTS A total of 2857 patients (1774 men [62.1%]; mean [SD] age, 69.83 [12.66] years) with stroke were identified, including 503 in the precampaign period and 2354 in the postcampaign period. Following the multifaceted campaign, the median (IQR) prehospital delay time decreased from 18.72 (7.44-27.84) hours to 6.00 (2.00-16.35) hours (P < .001). After the implementation of the Stroke 1-2-0 campaign, the proportion of patients with hospital arrival time within 3 hours increased from 5.8% to 33.4% (P < .001) and use of an ambulance increased from 3.2% to 30.6% (P < .001). In an interrupted time series analysis, the initiation of the Stroke 1-2-0 campaign was associated with significantly increased odds of arriving at the hospital within 3 hours (odds ratio, 8.01; 95% CI, 7.17-8.95; P < .001) and use of an ambulance (odds ratio, 9.41; 95% CI, 8.24-10.74; P < .001). CONCLUSIONS AND RELEVANCE The persistent multifaceted campaign using the Stroke 1-2-0 program was associated with reduced prehospital delay and improved timely arrival rate and ambulance arrival rate for patients with stroke. These findings suggest that Stroke 1-2-0 can be adopted in other regions of China to possibly improve health outcomes and reduce clinical burdens for all patients with stroke.
Value of the stroke 1-2-0 prehospital stroke education system: the experience of a general practitioner team

Abstract:Background: Stroke is one of the leading causes of death worldwide, especially in developing countries. In China, there is an urgent need to educate people about stroke awareness and the importance of using emergency medical services (EMS) quickly after a stroke has occurred. Objective: We sought to explore the effects of the Stroke 1-2-0 Prehospital Stroke Education System based on the experience of a general practitioner team. Method: We prospectively enrolled 119 community general practitioners to be trained in the procedures advocated by the Stroke 1-2-0 Prehospital Stroke Education System. The training content included early detection of ischemic stroke, first aid for stroke, and intravenous thrombolysis; The effects of the training were later evaluated via a before-and-after comparison. The 119 enrolled physicians formed a Stroke 1-2-0 lecturer group and taught stroke knowledge to community residents. The group remained active for 6 months, during which the medical treatment data of stroke patients (i.e., stroke onset time, prehospital delay, whether an ambulance was called, and whether thrombolytic therapy was performed) in each of 5 jurisdictions were recorded for the month before (January 2021) and that after (August 2021) the 6-month community education program. Finally, the effects of the community education program were evaluated. Results: The participants' understanding of intravenous thrombolysis in the treatment of acute ischemic stroke improved significantly after the training as compared with their earlier understanding (96% vs. 78.99%; P < .001), and their understanding of the time window for intravenous thrombolysis increased from 26.05% before to 72% (P < .001) after the training. Most of the participants (90% vs. 67.23%; P < .001) said that they would immediately call the 120 emergency number of China's emergency phone system if they encountered individuals who appeared to be victims of acute stroke. A total of 82 stroke patients were seen before and 67 after the community education program. As for the use of the emergency call system, more patients with stroke activated that system after the program versus before (21.95% vs. 37.31%; P = .04). The 3-hour arrival rate after the program was nearly three times higher than that before the program (62.69% vs. 19.51%; P < .001). Also, regarding receiving thrombolysis after the occurrence of a stroke, the program triggered a substantial increase compared with the total earlier (19.4% vs. 6.1%; P = .013). Conclusion: We found that the Stroke 1-2-0 Prehospital Stroke Education System significantly improved community residents' knowledge regarding stroke.

Abstract:Introduction: Stroke is the leading cause of death and disability in China. The median time of stroke pre-hospital delay is more than 15 hours, mainly due to the lack of awareness on stroke symptoms and calling emergency services. We developed Stroke 1-2-0 recognition tool in China, by adapting Face, Arm, Speech and Time. Our preliminary findings suggested that Stroke 1-2-0 can improve public's knowledge of the stroke symptoms, but its impact on the prehospital delay is still unclear. Furthermore, these findings were mainly obtained from Shanghai, one of the largest metropolises in China. However, more than half of population in China lives in the rural area. Given the striking disparities in socioeconomic status and quality of stroke care across the nation, a multicentre trial is warranted. Methods and analysis: Stroke 1-2-0 education programme will adopt a multicentre, cluster-randomised controlled design. We aimed to recruit 32 communities from 16 counties across China. Each county includes two communities having more than 100 000 residents. The two communities sampled in the same county will be randomly assigned to receive either Stroke 1-2-0 education programme or usual care. The primary objective of this study is to evaluate the impact of Stroke 1-2-0 public education programme in reducing stroke prehospital delay among adults residing in the community, compared with the usual care. The intervention will be implemented for 1 year. The primary outcomes are the symptom onset to hospital arrival time ('onset-to-door time', ODT) and 3-hour hospital arriving rate. We will use an intention-to-treat approach. A linear mixed model will be used to control for potential cluster effects. Ethics and dissemination: This study is approved by the Shanghai Minhang District Central Hospital Institutional Review Board (Shanghai, China). The findings will be disseminated via peer-reviewed publications and conference presentations. Trial registration number: ChiCTR2000040782.

Abstract:Background and Purpose— Low rates of thrombolysis for ischemic stroke in China have mainly been attributed to delays in presentation to the hospital. This study aimed to evaluate factors associated with these delays. Methods— Data were from a prospective, multicenter, hospital-based registry of patients with acute stroke (ChinaQUEST [Quality Evaluation of Stroke Care and Treatment]), which involved 62 hospitals across a variety of economic and geographic regions in China during 2006. Univariate and multivariate analyses were undertaken to determine associations between variables of interest and delays to hospital presentation. Results— Median time to hospital presentation was 15.0 hours for 6102 cases (interquartile range, 2.8–51.0 hours). A total of 1546 (25%) patients arrived within 3 hours and 2244 (37%) patients arrived within 6 hours after symptom onset. Factors that prolonged time to presentation were: visiting a local doctor before presenting at emergency (OR, 0.48; P <0.001), symptom onset at home (OR, 0.62; P <0.001), transfer to a large (Level III) hospital for management (OR, 0.70; P =0.04), and history of diabetes (OR, 0.78; P =0.01). In contrast, factors that accelerated presentation to the hospital were hemorrhagic stroke (OR, 2.25; P <0.001), history of atrial fibrillation (OR, 1.94; P <0.001), unconsciousness at presentation (OR, 1.91; P <0.001), transfer by ambulance (OR, 1.91; P <0.001), and history of coronary artery disease (OR, 1.20; P =0.04). Conclusions— Health promotion strategies to improve community awareness of early symptoms of stroke, establishment of an alert system to cater for patients likely to experience stroke at home, and wider availability and use of ambulance services are promising methods to help expedite presentation to hospital poststroke and thereby improve the management of stroke in China.
Age and Geographic Disparities in Acute Ischaemic Stroke Prehospital Delays in China: a Cross-Sectional Study Using National Stroke Registry Data

Abstract:Background Stroke has been the leading cause of death in China for decades. The extremely low intravenous thrombolysis rate is largely due to the prehospital delays that make patients ineligible for the time-sensitive therapy. Limited studies evaluated prehospital delays across China. We investigated prehospital delays in the stroke population across China and the associated age, rurality, and geographic disparities. Methods A cross-sectional study design was employed using the Bigdata Observatory platform for Stroke of China in 2020, the nationwide, prospective, multicentre registry of patients with acute ischaemic stroke (AIS). Mixed-effect regression models were used to account for the clustered data. Findings The sample contained 78,389 AIS patients. The median onset-to-door (OTD) time was 24 h, with only 11.79% (95% confidence interval [CI]: 11.56-12.02%) patients arriving at hospitals within 3 h. About 12.43% (95% CI: 12.11-12.74%) of patients 65 years or older arrived at hospitals within 3 h, which was significantly higher than the young and middle-aged patients (11.03%; 95% CI: 10.71-11.36%). After controlling for potential confounders, young and middle-aged patients were less likely to present to hospitals within 3 h (adjusted odds ratio: 0.95; 95% CI: 0.90-0.99) compared to patients 65 years or older. The 3-h hospital arrival rate was the highest in Beijing (18.40%, 95% CI: 16.01-20.79%), which was almost 5 times higher than that in Gansu (3.45%, 95% CI: 2.69-4.20%). The arrival rate in urban areas was almost 2 times higher than that in rural areas (13.35% versus. 7.66%). Interpretation We found that the low rates of timely arrival at hospitals after a stroke is more salient in the younger population, rural settings, or those residing in less developed geographic regions. Stroke Prevention and Control in China:Achievements, Challenges and Responses

Abstract:Stroke is one of the main causes of mortality, long-term physical and cognitive impairment in China. From the 1980s and 1990s, Chinese experts in the field of cerebrovascular disease established the prevention and control concepts for stroke and carried out related work. In 2009, the former Ministry of Health launched the Stroke Prevention and Treatment Project of the National Health Commission (SPTPC) to meet the stroke challenge. SPTPC actively promoted the establishment of the prevention and control system of stroke in China around various levels. SPTPC also carried out standardized training and promotion of prevention and treatment technology for cerebrovascular diseases, and resulted in significantly improved ability of stroke treatment. SPTPC also initiated a health management model for stroke, emphasized the importance of standardized follow-up, and all these efforts led to reduced recurrence and mortality of stroke within one year. Stroke prevention and treatment still face many difficulties, such as inadequate prevention and control system, lack of professional talents and insufficient promotion of prevention and control technology. In the next step, SPTPC will continue to participate in the screening and intervention of high-risk populations of stroke, improve the construction of regional stroke prevention and control system, vigorously promote the construction of stroke center and standardize the algorithm for the stroke diagnosis and increase the treatment efficacy, consolidate quality control and standardization work, and improve the mode of mobile stroke and remote diagnosis and treatment. Focusing on the work of “Reducing a Million New Disability Projects”, SPTPC will comprehensively promote the prevention and treatment of stroke in China to a new level and escort the national health.

Abstract:BACKGROUND: Delay time to hospital arrival may be influenced by lack of recognition of stroke signs and the necessity to seek emergency medical, which in turn is influenced by language barriers to, a modifiable risk factor, stroke awareness education. The objective was to determine the comprehension and satisfaction of a Spanish stroke awareness acronym, RÁPIDO, among community-living, Hispanic and Latino, Spanish-reading adults. METHODS: A 33-item survey was completed by 166 adults. Data on sociodemographics, language preferences, stroke education, and comprehension and satisfaction with RÁPIDO were collected. Descriptive characteristics were calculated. Fisher exact tests were performed to determine whether reading language (group 1, only or predominantly reads in Spanish; group 2, reads in Spanish and English equally or reads predominately in English) influenced survey responses. Responses to open-ended questions were categorized. RESULTS: Sixty-nine percent of the participants were born outside of the United States, 82% currently resided in the United States, 34% read only or predominately in Spanish, and 7% had a stroke. Most participants thought RÁPIDO was informative, eye-catching, and easily remembered. Significant differences were found between reading language preference groups for correctly identifying RÁPIDO images for facial drooping (group 1, 80%; group 2, 95%; P ≤ .001) and dizziness/loss of balance (group 1, 54%; group 2, 73%; P = .027). Eighty percent or more of all participants were able to correctly interpret RÁPIDO images for facial drooping, blurry vision, impaired speech, and call emergency services. Adding "911" to the RÁPIDO image of the clock was a common suggestion. CONCLUSIONS: RÁPIDO was well received among the participants. Modifications to RÁPIDO images representing dizziness/loss of balance and arm weakness, and the addition of "911" may improve its usefulness. Obtaining more extensive feedback across the United States and testing the effect of RÁPIDO on increasing knowledge of stroke signs and retention of that knowledge are necessary next steps.

Abstract:Objective To investigate the awareness and influencing factors of early recognition and emergency care for stroke in the stroke patients and their family members. Methods A total of 1 904 patients and their family members from six hospitals in Shanghai were selected by convenient sampling from November to December 2016. With the aid of WeChat official account of " KangFu Zhushou" ,the participants were surveyed about the knowledge of early stroke recognition and first aid. Results The patients and their families had a low level of knowledge about early stroke recognition and first aid. The knowledge level was related to gender,education level,occupation and stroke contact history (P＜0.05). The main ways to acquire stroke knowledge were TV (53.6%)and Internet (54.7%). Conclusion It is imperative to carry out health education about early stroke recognition and emergency care in the stroke patients and their family members,especially in male,less-educated,physical labor,and no history of stroke contact ones.
Improving timely treatment with a stroke emergency map: The case of northern China

Abstract:Objective The Chinese stroke emergency map (SEM) was implemented in 2017 to reduce prehospital and hospital delays for acute ischemic stroke (AIS) patients suitable for intravenous recombinant tissue plasminogen activator (rt-PA) thrombolysis. However, data on the time delay following the implementation of anSEMin China are limited. Methods Data for suspected stroke patients from theSEMregistry center of Taiyuan, Shanxi Province, from August 2017 to July 2019, patients' characteristics, thrombolysis rate, and functional outcome at 90 days were analyzed. Results One thousand seven hundred and eighty six patients who arrived at hospitals within 4.5 hr of onset were included; 35.9% arrived by emergency medical services (EMSs), and 1,207 (67.6%) of the population received intravenous rt-PA. As a result of theSEM, the number of patients treated with rt-PA increased from 63.9% in phase 1 (August 2017 to July 2018) to 70.5% in phase 2 (August 2018 to July 2019). The median onset-to-door and onset-to-needle times decreased by five minutes (100 [IQR: 62-135] vs. 105 [IQR: 70-145],p = .005) and nine minutes (158 [IQR: 124-197] vs. 167 [IQR: 132-214],p = .001), respectively. Patients in phase 2 achieved greater independent function outcome at 90 days (79.9% vs. 72.1%; adjusted odds ratio, 2.010; 95% confidence interval, 1.444-2.798). The binary logistic regression models revealed that shorter onset-to-needle time (OR: 0.994; 95% CI: 0.992-0.997;p < .001) and lower baseline NIHSS scores (OR: 39.120; 95% CI: 23.477-65.188;p p p < .001) were significant predictors for the independent function outcome. Conclusion The implementation of a stroke emergency map is more likely to reduce prehospital delays and improve function outcomes. Future efforts should attempt to increase EMS usage.

Abstract:On June 29, 2016, a 34-year-old man fainted and fell on a subway platform. His sudden death increased public calls to make critical first aid equipment available in public places. The live video showed that several passengers who tried to give cardiopulmonary resuscitation (CPR) didn't press the man's chest fast enough and for long enough. No automatic external defibrillators (AEDs) were available in the subway station and a subway employee only made calls to emergency services and took no other action. The man's death showed an absence of a public emergency response capacity in China.
