Abstract:Background: The mortality rate of peritoneal dialysis (PD) patients is still high and controversies persist regarding the mortality predictor. This study was designed to identify the predictability of the extracellular water/intracellular water ratio (E/I) on mortality in PD patients. Methods: 227 incident PD patients were included. Time-dependent Cox proportional hazard regression was used to investigate the predictability of E/I on mortality. Results: The 2- and 3-year survival was 74 and 65%, respectively. Univariate Cox proportional hazard regression analysis showed that the significant predictors of mortality were age, sex, Charlson Comorbidity Index, total Kt/V, serum albumin, pulse pressure, presence of malnutrition, and E/I. However, the final Cox proportional hazard models revealed that E/I was the only significant predictor. For every increase of 0.1 in the E/I value, the relative risk of death was 1.368. Conclusions: E/I is a strong independent predictor of mortality in incident PD patients. Copyright (c) 2007 S. Karger AG, Basel.

Abstract:BACKGROUND:Mitochondrial DNA (mtDNA) released into extracellular subsequent to cell injury and death can promote inflammation in patients and animal models. However, the effects of peritoneal dialysate cell-free mtDNA on intraperitoneal inflammation and peritoneal solute transport rate (PSTR) in peritoneal dialysis (PD) patients remain unclear. METHODS:We select the incident patients who began PD therapy between January 1, 2009, and December 30, 2010. Peritoneal dialysate was collected at the time of peritoneal equilibration test. The cell-free mtDNA, IL-6, IL-17A, TNF-α and IFN-γ were measured. All patients were followed till December 2017. The results were compared with PSTR and patient survival. RESULTS:One hundred and eighty-nine patients were included in the study. The average age was 47.1 ± 13.5 years, 55.6% of the patients were males. The average PSTR was 0.66 ± 0.12, the median dialysate mtDNA levels were 4325 copies/ul. The median concentrations of IL-6, IL-17A, TNF-α and IFN-γ were 25.9, 10.8, 25.8 and 17.9 pg/ml, respectively. We found that dialysate mtDNA was significantly correlated with PSTR (r = 0.461, P < 0.001), IL-6 (r = 0.568, P < 0.001), TNF-α (r = 0.454, P < 0.001) and IFN-γ (r = 0.203, P = 0.005). After adjustment for multiple covariates, dialysate mtDNA levels were independently correlated with IL-6 and PSTR. Dialysate mtDNA levels were not associated with patient survival. CONCLUSIONS:We found that dialysate mtDNA levels correlated with the degree of intraperitoneal inflammatory status in PD patients. Peritoneal effluent mtDNA was an independent determinant of PSTR but did not affect patient survival.
The Associations of Uric Acid, Cardiovascular and All-Cause Mortality in Peritoneal Dialysis Patients.

Abstract:AIMS:To investigate whether uric acid (UA) is an independent predictor of cardiovascular (CV) and all-cause mortality in peritoneal dialysis (PD) patients after controlling for recognized CV risk factors.METHODS:A total of 2264 patients on chronic PD were collected from seven centers affiliated with the Socioeconomic Status on the Outcome of Peritoneal Dialysis (SSOP) Study. All demographic and laboratory data were recorded at baseline. Multivariate Cox regression was used to calculate the hazard ratio (HR) of CV and all-cause mortality with adjustments for recognized traditional and uremia-related CV factors.RESULTS:There were no significant differences in baseline characteristics between patients with (n = 2193) and without (n = 71) UA measured. Each 1 mg/dL of increase in UA was associated with higher all-cause mortality with 1.05(1.00∼1.10) of HR and higher CV mortality with 1.12 (1.05∼1.20) of HR after adjusting for age, gender and center size. The highest gender-specific tertile of UA predicted higher all-cause mortality with 1.23(1.00∼1.52) of HR and higher CV mortality with 1.69 (1.21∼2.38) of HR after adjusting for age, gender and center size. The predictive value of UA was stronger in patients younger than 65 years without CV disease or diabetes at baseline. The prognostic value of UA as both continuous and categorical variable weakened or disappeared after further adjusted for uremia-related and traditional CV risk factors.CONCLUSIONS:The prognostic value of UA in CV and all-cause mortality was weak in PD patients generally, which was confounded by uremia-related and traditional CV risk factors.

Abstract:Objective:To explore the relationship between end-dialysis over-weight (edOW) in initial stage of hemodialysis and long-term prognosis in maintenance hemodialysis patients.Methods:The data of initial uremia patients receiving hemodialysis in the First Affiliated Hospital, College of Medicine, Zhejiang University from January 1, 2008 to April 30, 2017 were retrospectively analyzed. The end point of follow-up was death or until April 30, 2018. The general data including age, gender, body mass index, primary disease, complications and laboratory indicators of the patients and the related parameters of dialysis from four to twelve months were collected. Kaplan-Meier method was used to analyze survival rate. Cox multivariate regression was used to analyze the relationship between edOW and all-cause mortality and cardiovascular disease (CVD) mortality.Results:A total of 469 patients (300 males, 64.0%) were enrolled, with age of (56.9±17.1)years old. During the follow-up period of (4.1±2.4) years (1.0-10.3 years), 102 patients died. The main cause of death was cardiovascular and cerebrovascular events, accounting for 44.1% (45/102). The value of edOW was (0.28±0.02) kg. The patients were divided into edOW<0.28 kg group ( n=292) and edOW≥0.28 kg group ( n=177) according to the mean value of edOW. Kaplan-Meier survival analysis showed that the long-term survival rate in edOW<0.28 kg group was higher than that in edOW≥0.28 kg group (Log-rank χ2=4.134, P=0.043), and the CVD mortality in edOW≥0.28 kg group was significantly higher than that in edOW<0.28 kg group (Log-rank χ2=11.136, P=0.001). Cox multivariate regression analysis showed that high edOW was an independent influencing factor for all-cause death and CVD death in hemodialysis patients ( HR=1.541, 95% CI 1.057-2.249, P=0.025; HR=1.930, 95% CI 1.198-3.107, P=0.007). Conclusion:High edOW in early phase is an independent influencing factor of all-cause and CVD death in hemodialysis patients.

Abstract:BACKGROUND:Diabetic patients on peritoneal dialysis (PD) have lower survival and are more likely complicated with inflammation than their non-diabetic counterparts. Here, we explored the interaction effects between diabetes and inflammation on the survival of PD patients. METHODS:Overall, 2,264 incident patients were enrolled from a retrospective cohort study in China. Patients were grouped according to the baseline levels of high-sensitive C-reactive protein (hsCRP, ≤3 mg/L or >3 mg/L) or serum albumin (SA, ≥38 g/L or <38 g/L). Then, several multivariable adjusted stratified Cox regression models were constructed for these groups to explore the predicted role of diabetes on all-cause or cardiovascular death under inflammatory or non-inflammatory conditions. RESULTS:Diabetics on PD were more likely to have inflammation than non-diabetics on PD, and they presented with elevated hsCRP (52.7% vs. 47.3%, P = 0.03) or decreased SA (77.9% vs. 62.7%, P < 0.001) levels. After stratification by size of center and controlling for confounding factors, diabetes was found to predict all-cause death in patients with hsCRP >3 mg/L or SA <38 g/L but not in patients with hsCRP ≤3 mg/L or SA ≥38 g/L. Similarly, the presence of diabetes was an indication of cardiovascular death in patients with hsCRP >3 mg/L or SA <38 g/L. However, if further adjusted by baseline cardiovascular disease, the predicted role of diabetes on death related to cardiovascular disease in patients with SA <38 g/L disappeared. CONCLUSION:Diabetic patients could do as well as non-diabetic patients without inflammation on peritoneal dialysis. Active strategies should be implemented to improve inflammation status in diabetic patients on PD.

Abstract:Background: Peripheral arterial disease (PAD) is an important manifestation of systemic atherosclerosis and is common among dialysis patients. Cardiovascular disease (CVD) accounts for the leading cause of mortality in dialysis patients, and PAD has been found as a predictor for cardiovascular as well as overall mortality in general population. However, the study on the role of PAD in the prognosis of peritoneal dialysis patients is rather limited. Methods: Prevalent continuous ambulatory peritoneal dialysis patients over 60 years old were recruited in this study and were followed-up regularly to death or the end of the study. The diagnosis of PAD was based on ankle-brachial pressure index (ABI) < 0.9 or intermittent claudication. Univariate and multivariate Cox proportional hazard models were used to identify the risk factors for cardiovascular and overall mortality. Survival curves were estimated by the Kaplan-Meier method followed by log-rank test to compare the mortality rate between PAD and non-PAD patients. Results: One hundred and seventy-one patients were included and 62 (36%) had PAD complication. In the follow-up of 24.4 (median 34.6) months, 36 deaths were recorded: 19 from PAD group and 17 from non-PAD group. Twenty-one patients died due to CVD: 13 from PAD group and 8 from non-PAD group. The presence of PAD and serum albumin was found independently associated with cardiovascular and overall mortality using Cox proportional hazards model. Conclusion: PAD is very common in aged peritoneal dialysis patients and independently associated with both cardiovascular and overall mortality.
Volume markers in left ventricular diastolic dysfunction and adverse outcomes in peritoneal dialysis patients: a prospective cohort study

Abstract:Left ventricular diastolic dysfunction (LVDD) is an early event associated with cardiovascular complications and poor prognosis in chronic kidney disease patients undergoing dialysis. In this study, we investigated whether diastolic dysfunction, measured by the E/E' ratio, affects adverse outcomes in peritoneal dialysis (PD) patients (n = 148). Our results showed that patients with an E/E' ratio ≥ 15 were more likely to be female, have a longer dialysis vintage, have significantly higher left atrial volume index and left atrial kinetic energy levels, have lower E' levels and LV hypertrophy (LVH) degree, and have higher volume markers. Kaplan-Meier curves revealed that patients with a higher E/E' ratio had worse survival and a higher risk of heart failure than those with a lower E/E' ratio. Subgroup analysis demonstrated that non-diabetic patients with a higher E/E' ratio had a higher risk of heart failure than those with a lower E/E' ratio. Cox proportional hazard regression analysis indicated that the ECW/ICW ratio was strongly associated with LVDD and confirmed that the E/E' ratio was an independent risk factor for overall death. Our study suggests that monitoring the E/E' ratio in PD patients is important for improving their prognosis.
Association between arterial stiffness and peritoneal small solute transport rate.

Abstract:While cardiovascular disease accounts for 40-50% of the mortality in dialysis patients, and while a high peritoneal transport in continuous ambulatory peritoneal dialysis (CAPD) is an independent predictor of outcome, it is unclear if there are any links. Aortic stiffness has become established as a cardiovascular risk factor. We thus studied pulse wave velocity (PWV) in CAPD patients to explore the possible link between peritoneal small solute transport and aortic stiffness. CAPD patients (n = 76, 27 M/49 F) in our center were included in the present study. Aortic stiffness was assessed by brachial pulse pressure (PP) and carotid-femoral PWV. Patients' peritoneal small solute transport rate was assessed by D/P(cr) at 4 h. Extracellular water over total body water (E/T ratio) was assessed by means of bioimpedance analysis. C-reactive protein was also measured. Carotid-femoral PWV was positively associated with patients' age (r = 0.555; P < 0.01), time on peritoneal dialysis (r = 0.332; P < 0.01), diabetic status (r = 0.319; P < 0.01), D/P(cr) (r = 0.241; P < 0.05), PP (r = 0.475; P < 0.01), and E/T (r = 0.606; P < 0.01). In a multivariate regression analysis, carotid-femoral PWV was independently determined by E/T (P < 0.01), PP (P < 0.01), age (P < 0.01), and D/P(cr) (P < 0.05). D/P(cr), in addition to E/T, age, and PP, was an independent predictor of elevated carotid-femoral PWV in CAPD patients, suggesting that there might be a link between high aortic stiffness and increased peritoneal small solute transport rate.
Association of very Low-density Lipoprotein Cholesterol with All-cause and Cardiovascular Mortality in Peritoneal Dialysis.

Abstract:Background/Aims: Cardiovascular disease (CVD) is the leading cause of death in dialysis patients. Little is known about the relationship between very low-density lipoprotein cholesterol (VLDL-C) and cardiovascular mortality in these patients. Methods: A total of 1324 incident patients who began continuous ambulatory peritoneal dialysis (CAPD) therapy at our hospital between January 1, 2005, and September 30, 2014, with baseline serum VLDL-C values were investigated. The associations of the VLDL-C levels with all-cause and cardiovascular mortality were assessed. Results: The mean age of the cohort was 50.2 +/- 14.8 years, and the mean VLDL-C level was 33.6 +/- 18.0 mg/dl. One hundred sixty-five (12.5%) patients died during the study period. Multivariable models revealed that the high VLDL-C group was associated with significantly higher all-cause (HR, 2.08, 95% CI, 1.13 to 3.29, P = 0.002) and cardiovascular mortality (HR, 1.92, 95% CI, 1.18 to 4.29, P = 0.013) compared with the low VLDL-C group even after adjusting for various covariates. Using the VLDL-C level as a continuous variable, the hazard ratios (HRs) of all-cause and cardiovascular mortality associated with a 10-mg/dl increase in VLDL-C level were 1.12 (95% CI, 1.02 to 1.26, P = 0.025) and 1.11 (95% CI, 1.02 to 1.22, P = 0.029), respectively. VLDL-C was associated more strongly to all-cause (e.g., Akaike information criteria of 1990.205 vs. 1994.451) and cardiovascular (e.g., Akaike information criteria of 984.146 vs. 985.634) mortality than triglyceride (TG) levels. Conclusions: An elevated VLDL-C level is an independent risk factor for all-cause and cardiovascular mortality in peritoneal dialysis (PD) patients. (C) 2017 The Author(s) Published by S. Volume Overhydration is Related to Endothelial Dysfunction in Continuous Ambulatory Peritoneal Dialysis Patients

Abstract:Objective In dialysis patients, volume overhydration is common and is related to increased risk of cardiovascular morbidity and mortality. However, it remains unclear whether volume overload imposes those detrimental effects through endothelial dysfunction. Methods In this cross-sectional study, 81 stable patients on continuous ambulatory peritoneal dialysis in a single center were recruited. Volume status was evaluated by extracellular water, assessed by bioimpedance analysis, and normalized to individual height (nECW). Endothelial function was estimated by endothelial-dependent flow-mediated dilatation (FMD) of the brachial artery and expressed as percentage change relative to baseline diameter. Results There were 37 male and 44 female patients (mean age 61 ± 12 years, dialysis vintage 20 ± 23 months). FMD in female patients was significantly higher than that in male patients (9.17% ± 6.23% vs 6.31% ± 5.01%, p < 0.05). FMD was negatively correlated with weight (r = -0.308, p < 0.01), body mass index (r = -0.242, p < 0.05), systolic blood pressure (r = -0.228, p < 0.05), ECW (r = -0.404, p < 0.001), and nECW (r = -0.418, p < 0.001). No correlation was found between FMD and other variables. In multiple stepwise regression analysis, calcium x phosphate product (² = 0.422, p < 0.001), nECW (² = -0.343, p < 0.01), and dialysis vintage (² = -0.237, p < 0.05) were independent determinants of FMD (adjusted R2 = 0.327 for this model). Conclusion There was independent correlation between index of volume status and FMD, and higher nECW was related to worse endothelial function. The results of this study may help us understand the underlying mechanism of volume overhydration leading to increased cardiovascular morbidity and mortality in dialysis patients.

Abstract:The prognostic value of extravascular lung water (EVLW) has been widely investigated; however, a wide range of its predictive accuracy has been reported.A meta-analysis of diagnostic test studies was conducted.Various patient populations in the intensive care unit were included, such as burned patients and patients with acute lung injury/acute respiratory distress syndrome and sepsis.A computerized search of PubMed, Current Contents, CINAHL, and EMBASE from inception until March 1, 2011, was performed to identify potentially relevant articles. The inclusion criteria were studies investigating the prognostic value of EVLW in critically ill patients. There was no language restriction in the searching.The EVLW index (EVLWI) was used.The outcome was mortality (including in-hospital mortality, intensive care unit mortality, and 28-day mortality).We analyzed data from 11 studies and 9 countries involving 670 patients. Overall, the EVLWI was significantly higher in nonsurvivors than in survivors, with a mean difference of 5.06 mL/kg (95% confidence interval, -7.53 to -2.58). The heterogeneity was significant with I(2) = 90%. The pooled statistics of diagnostic accuracy together with relevant 95% confidence interval were as follows: sensitivity, 0.81 (0.72-0.88); specificity, 0.66 (0.55-0.76); diagnostic odds ratio, 8.84 (3.83-20.4), positive likelihood ratio, 2.44 (1.69-3.52); negative likelihood ratio, 0.28 (0.16-0.46).The sample sizes of included studies were small.The EVLWI appears to be a good predictor of mortality in critically ill patients.
The regularity of intracellular and extracellular water changes in maintenance hemodialysis patients during dialysis

Abstract:Objective This study aimed to explore the regularity of intracellular and extracellular water changes in maintenance hemodialysis (MHD) patients during dialysis.Methods We selected 55 MHD patients as the subjects and measured their total body water (TBW),intracellular water (ICW) and extracellular water (ECW) before dialysis and every hour during dialysis,i.e.,a total of 5 measurements for a patient.Resuits ①Both ICW and ECW decreased during dialysis as compared with those before dialysis,and ICW decreased more than ECW.②The ratio of ICW/ECW was 2∶1 in the first two hours during the dialysis,without significant difference with that before dialysis (t=-5.620,P=0.580;t=-2.048,P=0.053);the ratio increased slightly to 2.1∶1 in the last two hours during the dialysis (t=-3.269,P=0.030;t=-4.696,P ＜0.001).③The ratios of △ICW/△ECW were 1.9,1.8,1.5 and 1.4 at the 1st,2nd,3rd and 4th hour respectively during the dialysis,and this ratio decreased significantly at the 4th hour as compared with that at the 1 st hour (t=2.279,P=0.036).④After dialysis,the reduction rate of ICW and ECW were 5.4％ and 8.4％ respectively (t=3.994,P＜0.001).Conclusion ①Dialysis by ultrafiltration decreased the water retention in both extracellular and intracellular spaces,and the intracellular water decreased more than the extracellular water,resulting in slight increase of ICW/ECW ratio at the end of dialysis.②The ratio of △ICW/△ECW decreased gradually along with the hour of the dialysis,and the reduction rate of ICW was less than that of ECW,suggesting that the excessive water in intracellular space moves to extracellular space through osmotic gradient and removes from the body with ECW during dialysis.
The Associations of Blood Pressure Parameters with All-Cause and Cardiovascular Mortality in Peritoneal Dialysis Patients: a Cohort Study in China.

Abstract:BACKGROUND:It remains controversial to claim blood pressure (BP) as a leading risk factor for high risk of death in peritoneal dialysis patients, and less is known about the relationship between BP and mortality in Chinese peritoneal dialysis patients. METHODS:From Zhejiang Renal Data System in China, we collected data on patients treated and followed up at 98 peritoneal dialysis centres from 2008 to 2016. The associations of BP parameters [SBP, DBP, mean arterial pressure (MAP) and pulse pressure (PP)] with all-cause and cardiovascular mortality were examined. We fitted Cox models for mortality with penalized splines using nonparametric smoothers. Several sensitivity analyses were performed to confirm the robustness of our primary findings. RESULTS:A total of 7335 Chinese peritoneal dialysis patients were included. During a median follow-up of 35.8 months, 1281 (17.5%) patients died. SBP, DBP, MAP follow a U-shaped pattern of both all-cause and cardiovascular mortality. PP presents a reverse L-shaped association with all-cause mortality. Either a higher (SBP >141, DBP >85 or MAP >102 mmHg) or lower (SBP <119, DBP <67 or MAP <88 mmHg) BP tends to have a significantly higher all-cause and cardiovascular mortality risk. Higher PP (>60 mmHg) is related to a higher risk of all-cause mortality, but not cardiovascular mortality. These associations remain the same in our competing risk analysis and subgroup analyses. CONCLUSION:These data indicate U-shaped associations of SBP, DBP and MAP with all-cause mortality and cardiovascular mortality, respectively, and a reverse L-shaped association of PP with all-cause mortality. Further studies are needed to reliably establish the optimal BP targets for better hypertension control in peritoneal dialysis patients.
The Association between Baseline Serum Lipids and Mortality in Peritoneal Dialysis Patients

Abstract:Introduction: Lipid disturbances are common in ESRD patients. In peritoneal dialysis (PD) patients, dyslipidemia is even more common. This study aimed to examine whether serum lipids were associated with prognosis of PD patients. Methods: Patients from a multicenter retrospective cohort were used for the present study. The primary endpoint was all-cause mortality. Cox regression was used to analyze the association between serum lipids including total cholesterol, low-density lipoprotein (LDL), high-density lipoprotein, and triglycerides and the prognosis. Results: The results showed that lower total cholesterol and LDL levels at the initiation of PD predicted higher all-cause mortality in PD patients. Multivariate analysis reveal that the association disappeared after adjusting for age, gender, albumin, prealbumin, protein catabolic rate normalized to body weight, C-reactive protein, and residual renal function. Further analysis showed that patients with lower total cholesterol/LDL had a higher mortality only during the first 24 months of follow-up. In the patients who survived >2 years after PD, lower total cholesterol/LDL was not associated with higher long-term all-cause mortality any more. Conclusion: Lower total cholesterol/LDL levels at the initiation of PD were associated with overall mortality in PD patients. The association could be potentially modified by malnutrition, inflammation, and residual renal function or disappeared after 24 months.

Abstract:Objective: To determine if the association between high peritoneal protein clearance (PrC) and increased all-cause mortality is explained by inflammation and volume overload in continuous ambulatory peritoneal dialysis patients. Subjects and Methods: A total of 216 incident patients were enrolled. Demographics, biochemistry, inflammatory and volume status, peritoneal transport rate, fluid and solute removal were collected at baseline. Results: The median PrC was 57.2 ml/day. A high PrC was associated with more severe inflammation and volume overload. Using a multivariate regression model, for every 1-ml/day increase in PrC, the adjusted HR was 1.06 (1.00–1.12; p = 0.046) for all-cause death adjusted for age, diabetes, hemoglobin and D/Pcr. The predictability of PrC for all-cause death remained the same or mildly changed after additionally adjusted for inflammatory and volume overload markers with HR of 1.07 (1.00–1.14; p = 0.043) and 1.05 (0.99–1.12; p = 0.08), respectively. Conclusions: Peritoneal PrC, although correlated with volume overload and inflammation, is largely an independent predictor of mortality.

Abstract:Aim To research the assosiation between E/T(extracellular water to-total body water ratio) and pulse pressure(PP) in patients on continuous ambulatory peritoneal dialysis(CAPD).MethodsClinical stable CAPD patients(n=74,32M/42F) were included.Brachial blood pressure was measured twice in sitting position after patients had rested more than ten minutes.PP was calculated as systolic blood pressure(SBP) minus diastolic blood pressure(DBP) and it was used as an index of large arterial stiffness.Multiple-frequency bioelectrical impedance analysis was used to record the values for ECW,intracellular water(ICW),and total-body water.Based on these data,E/T also was calculated.In addition,some biochemical indices were determined with standard methods.ResultsPP was strongly associated with E/T(extracellular water/total body water)(r=0.691,P<0.001),E/I(extracellular water/intracellular water)(r=0.456,p=0.001),SBP(r=0.78,P<0.001),DBP,(r=-0.402,P<0.001),age(r=0.427,P<0.001),serum albumin(r=-0.36,P=0.002),C-reactive protein(CRP)(r=0.367,P=0.008) and diabetes(r=0.25,P=0.031),respectively.Multiple regression analysis showed that PP was independently determined by E/T(β=0.67,P<0.001)and CRP(β=0.253,P=0.016).They accounted for 55.7% of the total variance and E/T alone reprensented 50.4% of the explained variance.The regression equation:Y=-111.81+326.361X1+0.584X2(Y=PP;X1=E/T;X2= CRP).Conclusion E/T was closely associated with PP in peritoneal dialysis patients.E/T,in addition to CRP,was an independent risk factor for elevated PP in CAPD patients,suggesting that increased arterial stiffness might be the link between fluid overload and cardiovascular events and mortality in dialysis patients.

Abstract:Background. We aim to explore if disease severity score (DSS) at onset is associated with dialysate white cell counts, the severity of causative organisms and the risk for treatment failure of peritoneal dialysis (PD)-associated peritonitis in an adult PD cohort. Methods. Our prospective cohort study recorded all peritonitis episodes between 2008 and 2010. The DSS, demographic data and clinical characteristics were recorded at the onset of peritonitis. The dialysate cells were counted at regular intervals and organism culture were examined too. Treatment failure of peritonitis was defined as peritonitis-associated death and transfer to haemodialysis. Results. A total of 219 episodes of peritonitis in 146 PD patients were recorded, 21.9% of which resulted in treatment failure. There were no significant differences in dialysate white cell counts on the fifth and seventh day and the distribution of causative organism between groups with varied DSS level. DSS could not predict treatment failure including peritonitis-related death and transfer to haemodialysis after adjusting for age, gender, diabetes, dialysis duration, dialysate white cell count on the third day, the presence of Staphylococcus aureus, gram-negative organisms and polymicrobial organisms. Conclusion. Our study demonstrated that DSS at onset was not associated with prolonged elevation of dialysate white cell counts, severity of causative organisms and outcome of peritonitis episodes in adult PD patients.
Extracellular Fluid/Intracellular Fluid Volume Ratio as a Novel Risk Indicator for All-Cause Mortality and Cardiovascular Disease in Hemodialysis Patients

Abstract:BACKGROUND: In hemodialysis patients, fluid overload and malnutrition are accompanied by extracellular fluid (ECF) expansion and intracellular fluid (ICF) depletion, respectively. We investigated the relationship between ECF/ICF ratio (as an integrated marker reflecting both fluid overload and malnutrition) and survival and cardiovascular disease (CVD) in the context of malnutrition-inflammation-arteriosclerosis (MIA) complex.METHODS: Seventy-seven patients from a single hemodialysis unit were prospectively enrolled. The ECF/ICF volume was measured by segmental multi-frequency bioimpedance analysis. MIA and volume status were measured by serum albumin, C-reactive protein (CRP), pulse wave velocity (PWV) and plasma B-type natriuretic peptide (BNP), respectively.RESULTS: The mean ECF/ICF ratio was 0.56±0.06 and the cut-off value for maximum discrimination of survival was 0.57. Compared with the low ECF/ICF group, the high ECF/ICF group (ratio≥0.57, 42%) had higher all-cause mortality, CVD, CRP, PWV, and BNP, but lower serum albumin. During the 5-year follow-up, 24 all-cause mortality and 38 CVD occurred (18 and 24, respectively, in the high ECF/ICF group versus 6 and 14 respectively in the low ECF/ICF group, P<0.001). In the adjusted Cox analysis, the ECF/ICF ratio nullifies the effects of the MIA and volume status on survival and CVD and was an independent predictor of all-cause mortality and CVD: hazard ratio (95% confidence interval); 1.12 (1.01-1.25) and 1.09 (1.01-1.18) for a 0.01 increase in the ECF/ICF ratio. The degree of malnutrition (albumin), inflammation (CRP), arteriosclerosis (PWV), and fluid overload (BNP) were correlated well with the ECF/ICF ratio.CONCLUSIONS: Hemodialysis patients with high ECF/ICF ratio are not only fluid overloaded, but malnourished and have stiff artery with more inflammation. The ECF/ICF ratio is highly related to the MIA complex, and is a major risk indicator for all-cause mortality and CVD.
