Abstract:Objective To explore the risk factors for the upgrading of Gleason score radical prostatectomy (RP).Methods The study cohort consisted of 175 men treated with RP.The patients' age,serum prostate specific antigen (PSA),prostate volume,positive rate of puncture,maximal duty ratio of cancerous tissue,and changes of Gleason score were analyzed.The risk factors of Gleason score upgrading were analyzed with Logistic regression.Results Of all 175 patients,44 had (25.1％) upgraded Gleason score.In multivariate logistic regression models,the prostate volume was independent predictor of Gleason score upgrading.Conclusion Small prostate volume may be the predictor of Gleason score upgrading after RP.

Abstract:Objective:To explore the risk factors of Gleason Score upgrading in low risk prostate cancer after radical prostatectomy.Methods:We retrospectively evaluated 121patients who were biopsy proved prostatic carcinoma and per- formed radical prostatectomy.All patients included in the study was diagnosed low risk prostate cancer according to D'Amico classification,they had complete information for clinical variables,including age,body mass index(BMI),preoperative PSA level,prostate volume(PV),number of biopsy cores obtained,percentage of positive biopsy,clinical stage,biopsy GS. Grading of NB Gleason Score was compared with their corresponding radical prostatectomy specimens and assess the discrep- ancy of NB and prostatectomy specimens GS.Univariable and multivariable logistic regression analysis was used to identify predictors of Gleason Score upgrading.Results:Of the 121 patients,the specimen GS was upgraded in 49 (40.5%)patients and remained no change in 72 (59.5%)patients.Univariate and multivariate regression analysis shows that prostate volume were independent predictors with postoperative upgrading of GS(P=0.001).Conclusions:Smaller prostate volume was an independent factor for Gleason Score upgrading in clinically low risk prostate cancer after radical prostatectomy.This fact should be kept in mind when deciding on therapy decisions for patients with prostate cancer.

Abstract:Objective:To investigate the risk factors for Gleason score upgrading after radical prostatectomy in clinical low-risk prostate cancer patients aged≥65 years.Methods:A total of 485 clinical low-risk prostate cancer patients aged≥65 years at five centers of the national multi-center PC-follow database from January 2015 to March 2019 were retrospectively analyzed.Data including age at diagnosis, prostate-specific antigen(PSA), MRI prostate imaging, puncture Gleason score, operation method, puncture method, positive incision margin and capsule penetration were collected.Differences in Gleason scores before and after operation were compared, and the risk factors for Gleason score upgrading after radical resection were evaluated by univariate and multivariate Logistic regression analysis.Results:Of 485 patients with a puncture Gleason score of 3+ 3=6, 261(53.8%)cases had postoperative pathological upgrading, in whom 228(87.4%)cases had Gleason score upgrading of 7, 22(8.4%)had Gleason score upgrading of 8, and 11(4.2%)had Gleason score upgrading of 9 or more.The rate of Gleason score upgrading was elevated with increased preoperative PSA levels, positive pelvic MRI, and higher positive rates of puncture biopsy.The incidences of postoperative capsule penetration(27.2% vs.12.5%, P<0.001)and positive incision margin(25.2% vs.17.4%, P=0.036)had statistically significant differences between the pathologically upgraded group and the pathologically non-upgraded group.Multivariate analysis showed that preoperative PSA level, percentage of positive puncture biopsies, biopsy Gleason score and pelvic MRI were independent predictors of prostate cancer. Conclusions:For clinical low-risk prostate cancer patients aged≥65 years with high risk factors for Gleason score upgrading, repeated biopsies should be carried out when necessary and the treatment plan should be adjusted accordingly.
Factors Predicting 2014 ISUP Pathology Grade Upgrading from Prostate Biopsy to Radical Prostatectomy Pathology

Abstract:Objective To investigate the risk factors predicting pathology grade upgrading after radical prostatectomy using the 2014 International Society of Urologic Pathology (ISUP) grading system.Methods A total of 205 patients who underwent biopsy and radical prostatectomy from January 2017 to December 2018 were reviewed retrospectively.The median and range of the patients' age,PSA level,prostate volume,number of biopsy core examined,Gleason score and ISUP grade were 66 (45-81) years old,17.16(0.89-1254.00)ng/ml,36.4(4.1-152.1) rnl,10(1-15),7(6-10),and 3(1-5) respectively.The patients were divided into group of upgrading ISUP grade and group without upgrading ISUP grade.Multivariate Logistic regression analysis and receiving operating characteristic curve analysis were performed to identify predictors of ISUP upgrading and determine the optimal cut off value respectively.Result The median and range of Gleason score and ISUP grade after radical prostatectomy were 7 (6-10),and 3 (1-5) respectively.The radical prostatectomy ISUP grade upgraded in 73 (35.6％) out of 205 cases when compared with biopsy ISUP grade.Radical prostatectomy ISUP grades were concordant in 91 cases (44.4％) and downgraded in 41 cases(20.0％).Of 101 with biopsy ISUP grades less than or equal to 2,the ISUP grade of radical prostatectomy upgraded in 58 cases (57.4％),while radical prostatectomy ISUP grade upgraded in only 18 (26.9％) of 67 patients with biopsy ISUP grades of 3 or 4.Biopsy ISUP grades represent an independent predictor for ISUP grade upgrading after radical prostatectomy (OR =0.496,P ＜ 0.001).Conclusion Patients with biopsy ISUP grades less than or equal to 2 are at great risk of ISUP grade upgrading after radical prostatectomy.

Abstract:OBJECTIVE:To assess the discrepancy between preoperative needle biopsy (NB) Gleason score and pathological specimen Gleason score (GS) after radical prostatectomy, and to explore the risk factors of postoperative upgrading of GS.METHODS:We retrospectively evaluated 160 patients who suffered from biopsy proved prostatic carcinoma and performed radical prostatectomy. Age of the patients was 57-82 years, with the average age of 71.6; prebiopsy prostate specific antigen (PSA) was 0.31-40.32 μg/L,with the average PSA of 11.29 μg/L; body mass index (BMI) was 16.41-32.04 kg/m2, with the average BMI of 23.63 kg/m2; prostate volume (PV) was 9.52-148.46 mL, with the average PV of 40.19 mL. All the patients included in the study had complete information for clinical variables, including age, BMI, prebiopsy PSA level, PV, number of biopsy cores obtained, percentage, clinical stage, and biopsy GS. Grading of NB Gleason score was compared with their corresponding radical prostatectomy specimens, and the discrepancy between the NB and prostatectomy specimens GS assessed. Upgrading was defined as any increase in the pathological GS over that of the biopsy GS as a total sum of primary and secondary grades or a change in the order of primary and secondary grades towards higher ones. Univariable and multivariable Logistic regression analyses were used to identify predictors of pathological grading changes.RESULTS:Of the 160 patients, the specimen GS was upgraded in 49 (30.6%) patients and remained with no change in 82 (51.3%) patients. Univariate and multivariate regression analysis showed that prostate volume and biopsy GS were independent predictors with postoperative upgrading of GS. Age, BMI, PSA before needle biopsy, clinical stage and needle number showed no statistical significance (P>0.05).CONCLUSION:Lower biopsy GS and smaller prostate volume are increased risks for clinically upgrading of GS after radical prostatectomy. This fact should be kept in mind when deciding on therapy decisions for patients with prostate cancer.
[Influence of Clinical Factors on Gleason Score Upgrade in Patients Undergoing Radical Prostatectomy].

Abstract:OBJECTIVE:To evaluate clinical factors affecting Gleason score upgrade in patients receiving radical prostatectomy (RP).METHODS:A total of 322 patients with prostate cancer who received RP from January 2012 to December 2013 at Department of Urology at Fudan University Shanghai Cancer Center were included, and their data of age, body mass index (BMI), prostate-specific antigen (PSA), prostate volume, percentage core, clinical staging, pathological characteristics, biopsy Gleason score and RP Gleason score were analyzed. Differences in categorical variables and continuous variables were compared using χ² tests and Student's t-test, respectively. Unconditional multiple logistic regression was used to estimate OR and 95% CI of the association of Gleason score upgrade with clinical factors.RESULTS:Gleason score upgrade occurred in 107 of 322 (33.3%) patients. There was no difference in age, BMI and clinical staging between the two groups. Compared with patients without Gleason score upgrade, higher levels of PSA (χ² =6.740, P=0.034), smaller prostate volume (t=3.481, P=0.002) and elevated percentage core (t=-2.097, P=0.037) were observed in patients with Gleason score upgrade. In addition, lymph node metastasis (χ² =4.193, P=0.041) and extracapsular extension (χ² =4.747, P=0.029) were more common in patients with Gleason score upgrade. After adjusting for potential confounders, PSA levels (OR=2.451, 95% CI: 1.290-4.660), prostate volume (OR=0.982, 95% CI: 0.969-0.995) and percentage core (OR=2.756, 95% CI: 1.033-7.357) were independent predictors for Gleason score upgrade.CONCLUSION:Gleason score upgrade happens at a relatively high rate. PSA levels, prostate volume and percentage core are important factors affecting Gleason score upgrade.
Predictive Factors for Gleason Score Upgrading in Patients with Prostate Cancer after Radical Prostatectomy: A Systematic Review and Meta- Analysis

Abstract:Introduction: Previous studies have revealed that Gleason score upgrading (GSU) was closely related to an increased biochemical recurrence rate and adverse oncologic outcomes in patients with prostate cancer (PC). Therefore, we performed a meta-analysis to determine the predictive factors for GSU following radical prostatectomy (RP). Methods: We performed an extensive literature search using PubMed, Embase, and Cochrane in September 2022. In order to calculate the pooled odds ratio (OR), standardized mean difference (SMD), and 95% confidence intervals, a fixed effect or a DerSimonian and Laird random effect was applied. Results: Twenty-six studies included 18,745 PC patients that were available for further analysis. Our results revealed that GSU was significantly correlated with age (summary SMD = 0.13; p = 0.004), prostate volume (PV) (summary SMD = -0.19;p < 0.001), preoperative PSA (p-PSA) (summary SMD = 0.18; p < 0.001), PSA density (PSAD) (summary SMD = 0.40; p < 0.001), number of positive cores (summary SMD = 0.28; p = 0.001), percentage of positive cores (summary SMD = 0.36; p < 0.001), Prostate Imaging Reporting and Data System (PIRADS) scores (>3/<= 3) (summary OR = 2.27; p = 0.001), clinical T stage (>T2/<= T2) (summary OR = 1.73; p < 0.001), positive surgical margins (PSM) ( summary OR = 2.12; p < 0.001), extraprostatic extension (EPE) (summary OR = 2.73; p < 0.001), pathological T stage (>T2/<= T2) (summary OR = 3.45; p < 0.001), perineural invasion (PNI) (summary OR = 2.40; p = 0.008), and neutrophil to lymphocyte ratio (NLR) (summary SMD = 0.50; p < 0.001). However, we found that GSU was not significantly correlated with body mass index (BMI) (summary SMD = -0.02; p = 0.602). Moreover, our sensitivity and subgroup analyses showed that the findings were reliable. Conclusions: Age, PV, p-PSA, PSAD, number of positive cores, percentage of positive cores, PI-RADS score, clinical T stage, PSM, EPE, pathological T stage, PNI, and NLR are independent factors predicting GSU following RP. Upgrade in Gleason Score Between Biopsy and Radical Prostatectomy Pathology Indicates Poor Outcomes in Prostate Cancer

Abstract:Gleason score (GS) plays an important role in determining the biology of prostate cancer but prognostic information is scanty. A total of 966 patients with paired biopsy and radical prostatectomy histology were enrolled from 8 academic hospitals in China from January 2005 to March 2013, with median follow-up of 53 months. Kaplan-Meier curves and multivariate models were generated to compare the GS upgrade to those in whom the Gleason score remains the same on the risk of postoperative biochemical recurrence/progression and death. Overall, 331 patients (34.26%) experienced a GS upgrade post Radical Prostatectomy (RP). We found that patients with upgraded GS experienced a significantly higher rate of biochemical recurrence/clinical progression/death/cancerspecific mortality compared to those with concordant GS (P<0.001). According to the biopsy GS, patients were divided into 3 groups (biopsy GS≤6, GS=7, and GS≥8), patients with upgraded GS suffered a significantly higher biochemical recurrence (P<0.005) than those with concordant GS in the 3 groups. In multivariate models, a change in GS was an independent predictor of biochemical recurrence (2.01 (1.45-2.80), P<0.001), progression (1.77 (1.062.96), P=0.003) and death (1.83 (0.83-4.04), P=0.036) in the preoperative setting only. Patients experiencing an upgrade in their GS between biopsy and post RP exhibited significantly more aggressive pathological features than corresponding concordant tumors, and a higher risk of biochemical recurrence/progression and death post RP.
Does the Gleason Score 7 Upgrading Always Predict Worse Prognosis?

Abstract:OBJECTIVE:To determine the clinical significance of Gleason score(GS) 7 upgraded on radical prostatectomy(RP) and its impact on the prognosis of patients. PATIENTS AND METHODS:We used the Surveillance, Epidemiology, and End Results (SEER) database to study 8832 men diagnosed with M0 GS 3+4/4+3 prostate cancer (PCa) from 2010 to 2015 treated by RP. Logistic regression was used to analyze the effect of clinicopathological characteristics on the Gleason sore upgraded. Cox hazards regression analysis was performed to find significant factors of overall survival (OS). RESULTS:A total of 6237 (70.6%) biopsy GS 3+4 patients and 2595(29.4%) biopsy GS 4+3 patients were included in the study. Univariate and multivariate logistic regression analysis found that prostate-specific antigen (PSA)>20ng/ml, T stage 3-4, lymph node metastasis are independent risk factors in predicting the incidence of GS upgraded after RP (all P<0.05). Through multivariate analysis, we found that black race, GS upgraded, chemotherapy played significant roles in predicting poor OS (all P<0.05). It was surprising to find that the biopsy GS upgraded in patients with PSA 0-4ng/ml and 4.1-10ng/ml had a significant association with poor OS (all P<0.05). Multivariate analysis showed that only in patients with PSA 4-10ng/ml, biopsy GS upgrade had a statistically important relationship with poor OS (P=0.046). CONCLUSIONS:Not all patients with GS 7 upgraded had a worse prognosis than those without GS upgraded. Only in patients with PSA 4.1-10ng/ml, biopsy GS 7 upgraded was an independent risk factor affecting OS.
Value of Prostate-Specific Antigen Density in Predicting an Upgrade in Gleason Score for Initial Prostate Biopsy

Abstract:Objective To investigate the performance of PSAD as a predictor of Gleason score upgrade between initial prostate biopsy and radical prostatectomy(RP)in the patients with low or intermediate risk prostate cancers(PCa).Methods This study comprised a retrospective analysis of data from 170 PCa patients(PSA≤20 μg/L,stage≤T2b,Gleason score≤7).The median age was 68 yrs,the median PSA was 10.2 μg/L,the median PSAD was 0.35 mg/L2 and the median prostate volume was 28.4 ml.There were 95 cases in cT1 and 75 cases in cT2.According to biopsy Gleason score,patients were divided into three groups:3+3(91 cases),3+4(42 cases)and 4+3(37 cases).Each group was further stratified into to subgroups according to whether their RP Gleason score was concordant or upgraded.Receiver-operating characteristic(ROC)curves for predictive power of PSAD were generated for each group,and the area under the curve(AUC)was calculated.Results Of the 170 patients,79(46.5％)had an upgrade in Gleason score,51(56.0％)in 3+3 group,17(40.5％)in 3+4 group,11(29.7％)in 4+3 group,respectively.In the 3+3 group,PSAD was significantly higher in subgroup with upgraded Gleason score compared with the subgroup of concordant Gleason score(0.37 mg/L2 versus 0.23 mg/L2,P＜0.01).In the other two groups,PSAD were not of significant differences between subgroup with upgraded Gleason score and the subgroup of concordant Gleason score(0.33 mg/L2 versus 0.36 mg/L2,0.49 mg/L2 versus 0.58 mg/L2,P＞0.05).ROC analysis showed a decline in AUC with increasing biopsy Gleason score.It was 0.762 for 3+3 group,0.529 for 3+4 group and 0.413 for 4+3 group.The pathologic stage in upgraded cases were more advanced in all the groups(P＜0.05).Conclusion PSAD has the ability of predicting Gleason score upgrade after RP in the biopsy Gleason score 3+3 PCa patients whose clinical risk stratification are low or intermediate.
Prognostic Significance of Modified Gleason Scoring System after Radical Prostatectomy

Abstract:Objective To analysis the modified Gleason scoring system for predicting the prognosis after radical prostatectomy.Methods A total of 242 patients who received radical prostatectomy from April,2006 to October 2011 were recruited.The patients who lost follow-up or had adjuvant radiation or hormonal therapy or had visceral or bone metastasis were excluded,the remaining 168 patients were evaluated in the present study.The patients' age ranged from 53 to 85 years old (mean age 69 years old).The mean PSA level was 13.31ng/ml (ranging from 4.59 to 36.12 ng/ml).According to the traditional Gleason scoring system,there were 50 patients in Gleason ≤ 6 group,86 patients in Gleason 7 group and 32 patients in Gleason≥8 group.Patients were divided in five groups according to the modified Gleason scoring system.There were 50 patients in Gleason ≤6 group,67 in Gleason 3 + 4 group,19 in Gleason 4 + 3 group,15 in Gleason 8 group and 17 in Gleason 9-10 group.The biochemical-free-survival curve was drawn by Kaplan-Meier method and the multivariate Cox regression models were used to evaluate the clinical and pathological variables for the development of biochemical recurrence.ROC curve analysis was used to determine the predicted value for 5-year BCR of modified and traditional Gleason scoring.Results Significant differences were noted between the modified Gleason scoring groups and traditional Gleason scoring groups in PSA value (P =0.005),pathological stage (P =0.002),extraprostatic extension (P =0.003),seminal vesicle invasion (P =0.004),lymph node involvement (P =0.049) and positive surgical margin (P =0.006).With a median follow-up of 68 months(ranging from 7 to 98 months),5-year BFS rates for men with Gleason grade ≤6,3 + 4,4 + 3,8 and 9-10 tumours on RP pathology were 84.0％ (42/50),76.1％ (51/67),57.9％(11/19),40.0％ (9/15),29.4％ (5/17),respectively.On multivariate analysis,the HR value of Gleason 3 + 4 group and Gleason 4 + 3 group were 1.736 and 2.075 (P ＜ 0.05).The area under the curve in modified and traditional Gleason scoring were 0.698 (95％ CI 0.609-0.788) and 0.674 (95％ CI O.584-0.764),respectively.Conclusions The modified Gleason scoring system is related to the prostate cancer grade and its survival rate.Therefore,it can predict prognosis accurately in patients with prostate cancer.It can potential to reduce overtreatment in patients with Gleason 3 +4 prostate cancer.

Abstract:Abstract Background The objective of this study is to investigate the clinical significance and risk factors of upgrading in the International Society of Urological Pathology (ISUP) Grade Group System in men undergoing robot-assisted radical prostatectomy (RARP) for prostate cancer. Methods A total of 583 patients diagnosed with prostate cancer by systematic biopsy were treated with RARP without neoadjuvant therapy from November 2011 to December 2018. Clinicopathological data were obtained from our clinical records. ISUP grade upgrading (IGU) was defined as ‘ISUP grade in prostatectomy specimen determined to be higher than that in the biopsy specimen’. Clinicopathological factors, including age, PSA, prostate volume at biopsy (PV), PSA density, clinical stage, body mass index (BMI), interval from biopsy to prostatectomy, maximum percentage of cancer involvement per core (%CI), total number of biopsy cores, percentage of cancer positive biopsy cores (%PC), and sampling density were analyzed to detect potential risk factors of IGU. Biochemical recurrence (BCR) rates were calculated to analyze the effect of IGU on cancer prognosis. Results In univariate analysis, BMI was a positive predictor of IGU, while %CI, %PC, and sampling density were negative predictors of IGU. BMI and %PC were statistically significant predictors of IGU in multivariate analysis. For cases diagnosed as ISUP grade group 2 or higher at biopsy, there was a significant difference in BCR rates between cases with and without IGU. Conclusions The results from our cohort showed that elements of both high-grade cancer risk (such as BMI) and sampling efficiency (such as %PC) contribute to IGU. Excluding cases diagnosed as ISUP grade group 1 at biopsy, BCR-free rates were significantly worse in cases with IGU, highlighting the need for more accurate pathological diagnosis at biopsy.

Abstract:Objective To evaluate the prognoses and the prognosis factors of protatic cancer patients with Gleason score ≥ 8 after radical prostatectomy.Methods We retrospectively reviewed the data of 321 patients who underwent radical prostatectomy between 2002 and 2008.The Inclusion criteria were: Gleason score ≥ 8;receiving no neoadjuvant therapy before surgery;and the patients were followed up regularly after operation.Kaplan-Meier Logrank and Cox regression model were used for univariable and multivariable analyses to assess the factors influencing their prognoses.Results Totally 29 patients were included and they were followed up for a mean of(51.7±25.8) months.Sixteen patients had biochemical recurrence(55.2%) and 5-year biochemical recurrence-free survival rate was 40.2% in our group.The 5-year cancer-specific survival rate and overall survival rate was 95.8% and 84.8%,respectively.Univariate and multivariate analyses showed that the adjuvant therapy type was the only significant predictor of biochemical recurrence-free survival(P <0.01).Adjuvant androgen deprivation therapy plus radiotherapy significantly improved the biochemical recurrence-free survival in patients with Gleason score ≥8 after radical prostatectomy.Conclusion For patients with Gleason score ≥ 8,tumor staging,seminal vesicle invasion,positive surgical margin,and postoperative adjuvant therapy are the factors influencing the prognoses of prostatic cancer patients.Multi-modal treatment like adjuvant androgen deprivation therapy plus radiotherapy can greatly improve the biochemical recurrence-free survival.
Predictors of Gleason Grading Group Upgrading in Low-Risk Prostate Cancer Patients From Transperineal Biopsy After Radical Prostatectomy

Abstract:Rationale and Objectives To investigate the predictors of Gleason Grading Group (GGG) upgrading in low-risk prostate cancer (Gleason score = 3 + 3) from transperineal biopsy after radical prostatectomy (RP). Materials and Methods The clinical data of 160 patients who underwent transperineal biopsy and RP from January 2017 to December 2022 were retrospectively analyzed. First, univariate and multivariate logistic regression analysis were used to obtain independent predictors of postoperative GGG upgrading. Then receiver operating characteristic curve was used to evaluate the diagnostic efficacy of predictors. Finally, Linear-by-Linear Association test was used to analyze the risk trends of patients in different predictor groups in the postoperative GGG. Results In this study, there were 81 cases (50.6%) in the GGG concordance group and 79 cases (49.4%) in the GGG upgrading group. Univariate analysis showed age, free/total prostate-specific antigen (f/tPSA), proportion of positive biopsies, positive target of magnetic-resonance imaging (MRI) and positive target of contrast-enhanced ultrasound had significant effects on GGG upgrading (all P < .05). In multivariate logistic regression analysis, age (odds ratio [OR] = 1.066, 95%CI = 1.007-1.127, P = .027), f/tPSA (OR = 0.001, 95%CI = 0-0.146, P = .001) and positive target of MRI (OR = 3.005, 95%CI = 1.353-76.674, P = .007) were independent predictors. The prediction model (area under curve = 0.751 P < .001) had higher predictive efficacy than all independent predictors. The proportion of patients in exposed group of different GGG increased with the level of GGG, but decreased in nonexposed group, and the linear trend was significantly different (all P < .001). Conclusion Age, f/tPSA, and positive target of MRI were independent predictors of postoperative GGG upgrading. The predictive model constructed had the best diagnostic efficacy.

Low Serum Testosterone Predicts Upgrading and Upstaging of Prostate Cancer after Radical Prostatectomy

Abstract:Often, pathological Gleason Score (GS) and stage of prostate cancer (PCa) were inconsistent with biopsy GS and clinical stage. However, there were no widely accepted methods predicting upgrading and upstaging PCa. In our study, we investigated the association between serum testosterone and upgrading or upstaging of PCa after radical prostatectomy (RP). We enrolled 167 patients with PCa with biopsy GS ≤6, clinical stage ≤T2c, and prostate-specific antigen (PSA) <10 ng ml−1 from April 2009 to April 2015. Data including age, body mass index, preoperative PSA level, comorbidity, clinical presentation, and preoperative serum total testosterone level were collected. Upgrading occurred in 62 (37.1%) patients, and upstaging occurred in 73 (43.7%) patients. Preoperative testosterone was lower in the upgrading than nonupgrading group (3.72 vs 4.56, P< 0.01). Patients in the upstaging group had lower preoperative testosterone than those in the nonupstaging group (3.84 vs 4.57, P= 0.01). In multivariate logistic regression analysis, as both continuous and categorical variables, low serum testosterone was confirmed to be an independent predictor of pathological upgrading (P = 0.01 and P= 0.01) and upstaging (P = 0.01 and P = 0.02) after RP. We suggest that low serum testosterone (<3 ng ml−1 ) is associated with a high rate of upgrading and upstaging after RP. It is better for surgeons to ensure close monitoring of PSA levels and imaging examination when selecting non-RP treatment, to be cautious in proceeding with nerve-sparing surgery, and to be enthusiastic in performing extended lymph node dissection when selecting RP treatment for patients with low serum testosterone.
Risk Factors of under Staging and under Grading of Patients Undergoing Radical Prostatectomy

Abstract:Objective To determine the relevant factors of under staging and under grading in patients undergoing radical prostatectomy and to define important predictors of under staging in this population. Methods The clinical T stages and biopsy Gleason scores were compared with the pathological T stages and prostatectomy Gleason scores in 55 patients. Results Among the 55 cases,50 were preoperatively classified as clinical stages T 1～T 2,of whom 21(42%) were postoperatively classified as pathological stages T 3～T 4.Of the 26 cases who preoperatively had the Gleason scores of 2 to 6,11(42%) had postoperatively pathological grades of 7 to 10.Logistic regression analysis revealed that PSA( P =0.0159)at diagnosis and the percentage of positive biopsies of prostate ≥50%( P =0.0013) were significant predictors for preoperative clinical staging lower than postoperative pathological staging. Conclusions The results demonstrate that patients who were classified as clinical stages T 1～T 2 with PSA≥20 ng/ml and percentage of positive biopsies≥50% may have high possibility of under staging.
MRI-based PI-RADS score predicts ISUP upgrading and adverse pathology at radical prostatectomy in men with biopsy ISUP 1 prostate cancer

Abstract:Introduction: Most men diagnosed with very-low and low-risk prostate cancer are candidates for active surveillance; however, there is still a misclassification risk. We examined whether PI-RADS category 4 or 5 combined with ISUP 1 on prostate biopsy predicts upgrading and/or adverse pathology at radical prostatectomy. Materials and methods: A total of 127 patients had ISUP 1 cancer on biopsy after multiparametric MRI (mpMRI) and then underwent radical prostatectomy. We then evaluated them for ISUP upgrading and/or adverse pathology on radical prostatectomy. Results: Eight-nine patients (70%) were diagnosed with PI-RADS 4 or 5 lesions. ISUP upgrading was significantly higher among patients with PI-RADS 4-5 lesions (84%) compared to patients with equivocal or non-suspicious mpMRI findings (26%, p < 0.001). Both PI-RADS 4-5 lesions (OR 24.3, 95% CI 7.3, 80.5, p < 0.001) and stage T2 on DRE (OR 5.9, 95% CI 1.2, 29.4, p = 0.03) were independent predictors of upgrading on multivariate logistic regression analysis. Men with PI-RADS 4-5 lesions also had significantly more extra-prostatic extension (51% vs. 3%, p < 0.001) and positive surgical margins (16% vs. 3%. p = 0.03). The only independent predictor of adverse pathology was PI-RADS 4-5 (OR 21.7, 95% CI 4.8, 99, p < 0.001). Conclusion: PI-RADS 4 or 5 lesions on mpMRI were strong independent predictors of upgrading and adverse pathology. Incorporating mpMRI findings when selecting patients for active surveillance must be further evaluated in future studies.

Abstract:OBJECTIVE:To investigate the prognostic factors for clinically significant increase in post-prostatectomy Gleason score (pGS) in patients with biopsy Gleason score (bGS) ≤7.METHODS:This retrospective study included 170 cases of prostate cancer treated by radical prostatectomy in our hospital from January 2010 to December 2017. We analyzed the clinical and pathological data on the patients, including the age, preoperative serum tPSA, fPSA, fPSA / tPSA, prostate volume, PSA density (PSAD), and positive puncture rate of the patients with clinically significant elevation of pGS, as well as the possible factors for clinically significant pGS increase in patients with bGS = 7 and those with bGS ≤ 6.RESULTS:The pGS was found consistent with the bGS in 95 (55.9%) of the 170 patients, decreased in 11 (6.5%) and increased in 64 (37.6%). Among those with elevated pGS, 55 (32.4%) were shown with and the other 9 (5.3%) without clinical significance. Clinically significant escalation of pGS was markedly correlated with the positive puncture rate in the patients with bGS = 7 (P = 0.021) and with the age (P = 0.018) and PSAD (P = 0.033) of those with bGS ≤ 6. ROC curve analysis further showed the positive puncture rate > 0.528 in the patients with bGS = 7 and a higher risk of clinically significant pGS increase in those aged > 64.5 years with bGS ≤ 6 and PSAD > 0.267 μg/(L·g).CONCLUSIONS:Clinically significant elevation of pGS is correlated with the rate of positive punctures in prostate cancer patients with bGS = 7 and with age and PSAD in those with bGS ≤ 6.

Abstract:Objectives To investigate the impact of ultrasound shear wave elastography (USWE) and multiparametric magnetic resonance imaging (mpMRI) in predicting a change in biopsy-assigned Gleason Score (GS) after radical surgery for localised prostate cancer (PCa). Method A total of 212 men opting for laparoscopic radical prostatectomy (LRP) between September 2013 and June 2017 were recruited into this study. All the participants had 12-core transrectal ultrasound (TRUS) biopsies and imaging using USWE and mpMRI before radical surgery. The predictive accuracy for imaging modalities was assessed in relation to upgrading and downgrading of PCa GS between the biopsies and radical prostatectomy using Student’s t-test and multivariable logistic regression analyses. A decision analysis curve was constructed assessing the impact of nomogram on clinical situations using different thresholds of upgrading probabilities. Results Most GS 6 diseases on biopsies were upgraded on radical surgery (37/42, 88.1%). Major downgrading was seen in GS 8 category of disease (14/35; 37.1%), whereas no alteration was observed in GS 7 on biopsies in most men (55/75; 73.3%). In univariate analysis, higher preoperative prostate-specific antigen (PSA) (p = 0.001), higher prostate-specific antigen density (PSAD) (p = 0.002), stiffer USWE lesions (p = 0.009), and higher prostate imaging–reporting and data system (PIRADS) (p = 0.002) on mpMRI were significant predictors of upgrading. In multivariate logistic regression analyses, only PSA (p = 0.016) and USWE-measured tissue stiffness (p = 0.029) showed statistical significance in predicting upgrading. Conclusions Measurement of tissue stiffness using USWE in clinically localised PCa can predict upgrading of GS and has the potential to improve patient management options.

Abstract:Objective: To analyze the risk factors for postoperative pathological upgrade of prostate cancer patients with single core positive biopsy, and to attempt to build a mathematical model for predicting postoperative pathological upgrade in these cancer patients with single core positive biopsy. Methods: A retrospective analysis was conducted on 1 349 patients diagnosed with prostate cancer and undergoing radical prostatectomy at Peking University First Hospital from January 2015 to August 2020. The patients' age, body mass index, clinical stage, prostate imaging reporting and data system (PI-RADS) scores, prostate volume in magnetic resonance imaging (MRI), Gleason score of biopsy, serum prostate specific antigen (PSA) before biopsy and operation, surgical method and pathological stage were inclu-ded in the analysis. The variables with P < 0.1 in univariate analysis were included to construct multi-variate Logistic regression and the nomogram was drawn. The model was evaluated using the receiver operating curve. Results: A total of 71 patients were included in this research, with 34 patients in the upgraded group and 37 patients in the non-upgraded group. There were no significant differences in the patients' age (P=0.585), body mass index (P=0.165), operation method (P=0.08), prostate volume in MRI (P=0.067), clinical stage (P=0.678), PI-RADS score (P=0.203), difference of PSA density (P=0.063), Gleason score in biopsy (P=0.068), PSA before puncture (P=0.359) and operation (P= 0.739) between the two groups. However, there were significant differences in the proportion of tumor tissue (P=0.007), postoperative pathological stage (P < 0.001) and postoperative Gleason score (P < 0.001) between the two groups. The preoperative variables with a P value of less than 0.1 (prostate volume in MRI, difference of PSA density, proportion of tumor tissue and Gleason score in biopsy) in univariate analysis were included in the Logistic regression, and the nomogram was drawn. Only the prostate volume in MRI had a P value of less than 0.05. The area under the curve of the model was 0.773. Conclusion: In patients with single core positive biopsy, if the prostate volume is small or the proportion of tumor in positive core is small, clinicians should be alert to the possibility of postoperative pathology upgrading, preoperative risk stratification should be carefully considered for patients with possible pathological upgrading. This model can be used to predict the pathological upgrade of patients with single core positive biopsy.
