Abstract:The present study was conducted to investigate the effects of chitosan (CS)-Zn on intestinal morphology, mucosal epithelial cell apoptosis and mucosal immune function in weanling pigs. A total of 150 weanling barrows with a body weight of 7 center dot 2kg were randomly allocated into five groups. A basal diet without Zn supplementation was used as the control and other four groups were fed the control diet supplemented with 50 or 100mg/kg of Zn as CS-Zn, 100mg/kg of Zn as ZnSO4 and 3000mg/kg of Zn as ZnO, respectively. The feeding trial lasted for 28d. The results showed that serum diamine oxidase activities, d-lactate levels and endotoxin contents were lower in pigs fed dietary 100mg/kg of Zn as CS-Zn or 3000mg/kg of Zn as ZnO than in pigs fed the control or 100mg Zn/kg as ZnSO4 diet. The ratios of the villus height:crypt depth of the duodenum, jejunum and ileum were higher in pigs that received 100mg/kg of Zn as CS-Zn or a high level of Zn as ZnO than in pigs fed the control diet. Moreover, terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate-biotin nick end labelling (TUNEL)-stained ileal epithelial cells were found in the control group, and apoptotic cells did not appear prominently in pigs that received the 100mg/kg of CS-Zn or ZnO diet. Secretory IgA concentration in ileal mucus was increased in the dietary group that received 100mg/kg of CS-Zn or ZnO. These results indicated that dietary 100mg CS-Zn/kg had similar biological effects to dietary 3000mg ZnO/kg on intestinal morphology, mucosal epithelial cell apoptosis and mucosal immune function.

Abstract:OBJECTIVE:To determine the antimicrobial effects of carboxymethyl chitosan zinc (CMC-Zn(+)) and CMC-Zn(+)-peptide (CMC-Zn(+)-P) on four kinds of periodontal pathogens. METHODS:Dilution method was used to determine the minimum inhibitory concentration (MIC) of CMC-Zn(+) for Porphyromonas gingivalis (Pg), Actinobacillusactinomycetemcomitans (Aa), Prevotella intermedia (Pi) and Actinomyces viscosus (Av). The antimicrobial characters of CMC-Zn(+) and CMC-Zn(+)-P on these four kinds of pathogens were evaluated by disk diffusion method. RESULTS:The MIC of CMC-Zn(+) for Pg, Aa, Pi and Av was 0.312 5%, 0.156 25%, 0.156 25% and 0.078 125% respectively. Significant antimicrobial effects were improved along with the increased concentration of CMC-Zn(+) and CMC-Zn(+)-P (P < 0.01), while the best antimicrobial concentrations of both CMC-Zn(+) and CMC-Zn(+)-P were 5%. The antimicrobial effect of CMC-Zn(+)-P was better than that of CMC-Zn(+) on the same kind of periodontal pathogen (P < 0.01). CMC-Zn(+)-P showed different antimicrobial effects on the four periodontal pathogens (Av > Aa > Pi or Av > Aa > Pg, P < 0.01). CONCLUSIONS:CMC-Zn(+) and CMC-Zn(+)-P have inhibition effect on Pg, Aa, Pi and Av, and the best antibacterial concentration was 5%. The CMC-Zn(+)-P has better antibacterial effect than CMC-Zn(+) on Pg, Aa, Pi and Av.
Modulation of Pro-Inflammatory Mediators in LPS-stimulated Human Periodontal Ligament Cells by Chitosan and Quaternized Chitosan

Abstract:The aim of this study was to evaluate the effects of chitosan and quaternized chitosan (HTCC) modulate IL-1β and TNF-α in LPS-stimulated human periodontal ligament cells (HPDLCs). MTT assay revealed that chitosan stimulated the proliferation of HPDLCs. However, HTCC inhibited the proliferation of HPDLCs at concentrations of 1000 and 100 μg/mL more than the control, especially after 5d (P<0.001). ELISA assay exhibited that chitosan inhibited the production of IL-1β and TNF-α at 24, 48 and 72 h. IL-1β and TNF-α secreted by HPDLCs with LPS and treated with 1000 μg/mL of HTCC significantly increased compared to both the control and the chitosan group (P<0.001). The bioactive role for bFGF in modulating the responses of HPDLCs cells to LPS via inhibiting IL-1β and TNF-α production was demonstrated. All results were necessary to enhance our understanding of the biomedical properties of chitosan and HTCC for modulation of pro-inflammatory mediators.
Influence of Aiyalin to IL-1 in Gingival Crevicular Fluid of Periodontitis Patients

Abstract:Objective: To study the influence of aiyalin(Minocycline HCL dental S.R.Pellicle) to IL-1 in gingival crevicular fluid and to clinical symptoms of periodontal patients.Methods: We choose medium to serious periodontitis patients,then test probing depth,attachment loss,bleeding on probing,gingival index,loosen degree,quantity of gingival crevicular fluid and IL-1 before and after using aiyalin.Results: The difference of all indices before and after using aiyalin are statistically significant except attachment loss and loosen degree.Conclusion: Aiyalin can reduce the clinical symptoms of periodontal and inhibit the production of IL-1.

Abstract:PURPOSE:To explore the effects of eicosapentaenoic acid (EPA) on biological activity and inflammatory factor expression of human gingival fibroblasts (HGFs). METHODS:The effects of EPA on the activity, morphology and cell cycle of HGFs were observed by living and dead cell staining, immunofluorescence staining and flow cytometry, respectively. HGFs were stimulated by lipopolysaccharides (LPS) of Porphyromonas gingivalis (P. gingivalis) or heat inactivated P. gingivalis, after which the effects of EPA on mRNA and protein expression of IL-6, IL-8 and IL-1β were observed by real-time PCR and ELISA, respectively. The gene and protein expression of heme oxygenase-1(HO-1) was also detected by real-time PCR and Western blotting, respectively. The data were analyzed with SPSS 22.0 software package. RESULTS:200 μmol/L EPA inhibited cell activity of HGFs; 100 μmol/L EPA did not affect cell activity and morphology of HGFs, and had no significant effect on cell cycle (P>0.05). EPA significantly downregulated gene expression of IL-6 and IL-1β, and protein expression of IL-6 stimulated by P. gingivalis LPS and heat-killed P.gingivalis(P<0.05), in a dose-dependent manner. EPA increased gene expression of HO-1 in a dose dependent manner(P<0.05), and upregulated HO-1 protein expression. CONCLUSIONS:EPA significantly inhibits the expression of inflammatory factors without affecting the biological activity of HGFs, which may be related to the induction of HO-1, suggesting the potential role of EPA in the prevention and treatment of periodontitis.
Effect of Ultrasonic Cleaning Combined with Antibacterial Polypeptide Periodontal Gel on Inflammatory Reaction and Incidence of Adverse Reactions in Patients with Chronic Gingivitis

Abstract:The purpose of this investigation was to evaluate the efficacy of ultrasonic subgingival curettage in conjunction with antibacterial polypeptide periodontal gel in the management of chronic periodontitis of moderate to severe severity. Methods included dividing 500 hospitalised patients with moderate to severe chronic periodontitis evenly between an observation group and a control group. Subgingival ultrasonic curettage was performed on the placebo group. The non-treatment group received ultrasonic subgingival curettage and a periodontal gel rinse containing polypeptides. Results were compared before and after treatment in terms of the periodontal index, inflammation in the gingival crevicular fluid, and occlusal and masticatory efficiency. Both groups saw significant reductions in occlusal duration and occlusal force balance after treatment compared to pre-treatment levels, though the observation group saw a more dramatic decrease in these indices than the control group with P ≤ 0.05. The treatment and observation groups both saw significant reductions in the masticatory efficiency standard deviation afterward, but the index in the observation group was significantly lower than that of the control group with P ≤ 0.05.The authors claim that moderate to severe chronic periodontitis can be effectively treated with a combination of polypeptide periodontal gel and ultrasonic subgingival curettage. Substantial decreases from pre-treatment levels for both groups, with the Observation Group's index being significantly lower than the Control Group's index (P ≤ 0.05). It is possible that this treatment will help reduce inflammation and improve your periodontal health. Biting strength and occlusion stability can both be improved at the same time to help patients improve their chewing efficiency. Therefore, this method can be used securely in real-world patient care settings.
Anti-inflammatory Effects of PMX205 in Mouse Macrophage Periodontitis Model.

Abstract:Background: C5areceptor antagonistPMX205 is a synthetic hexapeptidecapable of blocking C5a-C5a receptor (C5aR) axis by simulating C5a active C-terminal amino acid residues. This hexapeptide presents good anti-inflammatory effects in a myriad inflammation models. The anti-inflammatory effect of PMX205 on periodontitis is yet to be fully fathomed. Objective: To examine the anti-inflammatory effects of PMX205 on RAW264.7 murine macrophages exposed togingipain extracts and Porphyromonas gingivalis (P. gingivalis). Methods: MTT assay was carried out so as to specify the cytotoxicity of PMX205. RAW264.7 cells were co-cultured in vitro with gingipain extracts or P. gingivalis to simulate the periodontitis inflammatory milieu. Real-time quantitative PCR, ELISA and Griess assay were performed in order to detect tumor necrosis factor-alpha (TNF-alpha), IL-6, IL-23, nitric oxide (NO), IL-10, transforming growth factor-beta 1 (TGF-beta 1), andarginase-1 (Arg-1). Furthermore, phagocytosis assay was done to evaluate the phagocytic capacity of RAW 264.7 cells. Finally, western blot analysis was conducted to evaluate myeloid differentiation factor 88 (MyD88). Results: PMX205 increased the expression levels of bacteriostatic substances (NO and IL-23) and anti-inflammatory cytokines (TGF-beta 1, IL-10 and Arg-1); however, it reduced the expression levels of proinflammatory cytokines TNF-alpha and IL-6once RAW 264.7 macrophages were stimulated via gingipain extracts or P. gingivalis. In addition, PMX205 promoted the macrophage phagocytosis and down-regulated protein expression of MyD88. Conclusion: PMX205 has recognizable anti-inflammatory effects in RAW 264.7 cell inflammation model, a finding which probably opens doors to future investigations on new targets for the prevention and treatment of chronic periodontitis.
[Effect of ultrasonic curettage combined with sodium hyaluronate gel on chronic periodontitis and its effect on inflammatory factors hs-CRP, MCP-1 and MMP-13 in gingival crevicular fluid]

Abstract:Purpose: To investigate the effect of ultrasonic curettage combined with sodium hyaluronate gel in the treatment of chronic periodontitis (CP) and the effect on inflammatory factor hypersensitive C-reactive protein (hs-CRP), monocyte chemotactic protein 1 (MCP-1) and matrix metalloproteinase 13 (MMP-13) in gingival crevicular fluid. Methods: A total of 102 patients with CP from October 2021 to October 2022 were selected, divided into experimental group (n=51) and control group (n=51) by random number table method. The control group received ultrasonic subgingival curetage, and the experimental group received sodium hyaluronate gel adjuvant therapy on the basis of the control group. The periodontal rehabilitation indexes, clinical efficacy and the changes of gingival crevicular fluid hs-CRP, MCP-1 and MMP-13 were compared between the two groups. The periodontal pathogens, bone metabolism indexes and the occurrence of adverse events during treatment were compared between the two groups. The data were statistically analyzed using SPSS 22.0 software package. Results: After treatment, the sulcus bleeding index (SBI), gingival index (GI), plaque index (PLI), periodontal pocket depth (PD) and attachment level (AL) of the two groups were significantly lower than before treatment (P＜0.05), and even significantly lower(P＜0.05) in the experimental group. Total effective rate of the experimental group was significantly higher than that of the control group(P＜0.05). hs-CRP, MCP-1 and MMP-13 in both groups after treatment were significantly lower than before treatment(P＜0.05), and hs-CRP, MCP-1 and MMP-13 in the experimental group after treatment were significantly lower than those in the control group(P＜0.05). The detection rates of Porphyromonas gingivalis, Forsetanella and Treponema dentalis were significantly lower in both groups after treatment than before treatment (P＜0.05), and the detection rates of the above indexes in the experimental group after treatment were significantly lower than those in the control group(P＜0.05). After treatment, the C-terminal peptide(CTX) of type Ⅰ collagen was significantly lower than that before treatment, and bone gla protein(BGP) was significantly higher than that before treatment(P＜0.05). The CTX and BGP of the experimental group were significantly lower than that of the control group and significantly higher than that of the control group(P＜0.05). There was no significant difference in the incidence of total adverse reactions between the two groups(P＞0.05). CONCLUSIONS：Ultrasonic curettage combined with sodium hyaluronate gel in the treatment of CP can promote periodontal tissue rehabilitation, enhance short-term efficacy, inhibit synthesis of inflammatory factors in gingival crevicular fluid, kill periodontal pathogens, regulate bone metabolism, and is safe and reliable.
The Effect of Polyphenol-Containing Solutions on in Situ Biofilm Formation on Enamel and Dentin

Abstract:Objectives: To investigate the effects of Chinese gallnut extracts and pure tannic acid on in situ biofilm formation on enamel and dentin samples over 24 h. Methods: Bovine enamel and dentin samples were buccally fixed on maxillary splints. Six volunteers wore the splints for 24 h, and rinsed their mouths with tap water (control), 1% tannic acidand 1% Chinese gallnut extracts-containing solution twice a day, 3 min after the splints were placed in the mouth and before night sleep. Live/dead staining was used for fluorescence microscopic (FM) visualization and quantification of bacteria viability of biofilms formed on enamel and dentin samples. Biofilm coverage was evaluated and recorded by FM and scanning electron microscopy (SEM). In addition, biofilms were analyzed by transmission electron microscopy (TEM). The Kruskal-Wallis test was used to analyze biofilm data. Results: Rinsing with tannic acidand Chinese gallnut extracts-containing solutions significantly reduced in situ biofilm coverage on enamel and dentin samples (P < 0.05). The bacterial viability of biofilms formed on enamel samples was significantly reduced compared to the control (P < 0.05). TEM analysis revealed an increase in pellicle's electron density and thickness and only few or no bacteria adherent to the pellicle in the experimental samples. Conclusions: Rinsing with tannic acidand Chinese gallnut extracts-containing solutions can effectively inhibit in situ biofilm formation, modify the ultrastructure of biofilms on enamel and dentin surfaces and significantly reduce the bacterial viability of biofilm on enamel surfaces. Clinical significance: Tannic acidand Chinese gallnut extracts-containing solutions might be used for dental biofilm management.

Abstract:PURPOSE:To observe the effect of eicosapentaenoc acid (EPA) on the expression of inflammatory factors in human periodontal ligament fibroblasts (hPDLCs) induced by using lipopolysaccharides (LPS) of Porphyromonas gingivalis (P. gingivalis).METHODS:hPDLCs were cultured by using tissue block method, and the effects of different concentrations of EPA on the activity of hPDLCs cells were observed by MTT method. According to MTT results, the appropriate concentration of EPA was selected, and the expression of interleukin-6(IL-6), IL-8 and IL-1β in hPDLCs induced by P.gingivalis LPS was detected by real time PCR and ELISA. The data were evaluated by SPSS 10.0 software package.RESULTS:25-100 μmol/L EPA had no effect on the activity of hPDLCs cells, but could inhibit the expression of IL-6, IL-8 and IL-1β induced by P. gingivalis LPS in a dose-dependent manner.CONCLUSIONS:EPA can inhibit the expression of inflammatory factors induced by P. gingivalis LPS without affecting cell activity, indicating that EPA has the possibility of anti-inflammatory treatment of periodontitis.

Abstract:Objective: To prepare a functional chitosan thermosensitive hydrogel for minocycline-HCl gelatin microspheres and observe its effects on repairing experimental rats gingivitis. Methods: Minocycline-HCl gelatin microspheres were prepared by using gelatin as core material and emulsion crosslinking method, then the latter was incorporated into the chitosan thermosensitive hydrogel. Established an experimental gingivitis model in SD rats and to observe the effect of hydrogel on gingivitis. Results: Microspheres thermosensitive hydrogel had obviously sustained effects on gingivitis. Results showed that GI,PD in experiment group were significantly improved. Conclusion: The chitosan thermosensitive hydrogel loading of minocycline-HCl gelatin microspheres have antiphlogistic effects on experimental rats gingivitis.
Antibacterial Eﬀects and Microarray-Based Molecular Mechanisms of Trans-Cinnamaldehyde Against Porphyromonas Gingivalis.

Abstract:Porphyromonas gingivalis (P. gingivalis) is one of the keystone pathogenic bacteria of periodontitis and peri-implantitis. This study aimed to investigate the antibacterial effects and molecular mechanisms of trans-cinnamaldehyde (TC), a safe extract from natural plants, on P. gingivalis. Minimum inhibitory and minimum bactericidal concentrations (MIC and MBC) of TC were determined, and scanning and transmission electron microscopies were used to assess the morphological changes. The overall biomass was estimated, and the metabolic activity of biofilms was determined at different TC concentrations. A microarray-based bioinformatics analysis was performed to elucidate the underlying molecular mechanisms of TC-inhibited P. gingivalis, and significant differences among groups were determined. TC showed an inhibitory effect on the proliferation and survival of planktonic P. gingivalis, of which the MIC and MBC were 39.07 μg/mL and 78.13 μg/mL, respectively. TC also significantly suppressed the formation and metabolic activity of P. gingivalis biofilm. The results of the significant pathways and gene ontology (GO) analyses revealed that TC treatment inhibited two metabolic pathways, accompanied by the downregulation of relative genes of nitrogen metabolism (NrfA, NrfH, and PG_2213) and starch and sucrose metabolism (PG_1681, PG_1682, and PG_1683). Thus, this study confirmed TC to be a natural antimicrobial agent against P. gingivalis and further demonstrated that TC suppressed the microbial activity on P. gingivalis through the disruption of physiological metabolism, which might inhibit the growth and the biofilm formation of P. gingivalis.
Effect of Porphyromonas gingivalis and Lactobacillus acidophilus on Secretion of IL1B, IL6, and IL8 by Gingival Epithelial Cells

Abstract:Porphyromonas gingivalis alters cytokine expression in gingival epithelial cells, stimulating inflammatory responses that may lead to periodontal disease. This study explored the effect of Lactobacillus acidophilus on the specific expressions of the interleukins (ILs) IL1B, IL6, and IL8 induced by the pathogen. Human gingival epithelial cells were co-cultured with P. gingivalis , L. acidophilus , or L. acidophilus + P. gingivalis ; the control group consisted of the cells alone. Protein and gene expression levels of the ILs were detected using ELISA and qRT-PCR, respectively. The supernatant from the P. gingivalis group held significantly higher protein and mRNA levels of IL1B, IL6, and IL8, compared to the control group. In the mixed bacterial group ( L. acidophilus + P. gingivalis ), the levels of all three ILs decreased with increasing concentrations of L. acidophilus and were significantly different from the P. gingivalis group. This suggests that in gingival cells, L. acidophilus offsets the P. gingivalis -induced secretion of these ILs in a dose-dependent manner.
Cytotoxic and anti-inflammatory effects of chitosan and hemostatic gelatin in oral cell culture

Abstract:Chitosan is a biopolymer with bactericidal/bacteriostatic effect, biocompatible and biodegradable. It has been used in tissue engineering to replace tissues partially or completely by releasing bioactive materials or influencing cell growth, usually in regenerative medicine and dentistry. The aim of this study was to evaluate the cytotoxic and anti-inflammatory effect of chitosan alone or with hemostatic gelatin (Spongostand®) in cultures of human pulp cells (HPC), human gingival fibroblasts (HGF) and mouse pre-osteoblasts (MC3T3-E1, ATCC). HPC and HGF were isolated from patients. Cells were subcultured in DMEM. Chitosan was inoculated at different concentrations (0-0.5%) and hemostatic gelatins impregnated with chitosan (0.19%) were placed directly in the presence of cells and incubated for 24 hours. Cell viability was determined by MTT method and mean cytotoxic concentration (CC50) was calculated from the dose-response curve. Anti-inflammatory effect was calculated from the in vitro gingivitis model induced with interleukin 1beta (IL-1β) in HGF and protein detection. The data were subjected to Shapiro-Wilk, Kruskal-Wallis and Mann-Whitney tests. Experiments were performed in triplicate of three independent assays. Cell viability of HPC, HGF and MC3T3-E1 in contact with chitosan decreased significantly (p<0.05). The HPC were the most sensitive (CC50= 0.18%), followed by HGF (CC50=0.18%) and MC3T3-E1 (CC50= 0.19%). The cytotoxicity of gelatins impregnated with chitosan decreased cell viability of HGF and HPC by 11% and 5%, respectively. The proinflammatory effect was reduced significantly in the gingivitis model. To conclude, chitosan induces moderate cytotoxic effects alone or with hemostatic gelatin at 0.19%, in dose-dependent manner, with anti-inflammatory effects on human gingival fibroblasts. The use of chitosan as a biomaterial can be an excellent choice for use in regenerative dentistry.
Effects of Essential Oil-Containing Mouthrinse in Reducing Plaque and Gingivitis

Abstract:Objective To investigate the effects of an essential oil-containing mouthrinse in reducing plaque and gingivitis.Methods After examination,67 subjects were selected for the study,whose modified gingival index was(MGI)≥1.5 and modified Quigley and Hein plaque index(MPI)≥2.0.All the subjects were instructed to rinse their mouths twice a day,using 20ml essential oil-containing mouthrinse for 30 seconds per use,at 12 hour interval,besides tooth brushing.After having used the mouthwash for 13 weeks continuously,MGI,MPI and gingival bleeding index(GBI)of the subjects were evaluated again and compared with the baseline.Results After using this essential oil-containing mouthrinse continuously for 13 weeks,compared with the baseline,MGI,MPI and GBI of the subjects decreased significantly(P0.01).MGI decreased by 26.52%,MPI decreased by 61% and GBI decreased by 30.65%.Conclusion Besides regular tooth brushing,using an essential oil-containing mouthrinse provides an adjunctive benefit in reducing plaque and gingivitis.
Effectiveness of a triclosan/copolymer dentifrice on microbiological and inflammatory parameters

Abstract:According to the US Surgeon General's report, "Oral Health in America," published in 2000, most adults in the United States show some degree of periodontal pathology, with severe periodontal diseases affecting about 14% of middle-aged adults. Periodontal diseases are polymicrobial-induced inflammatory diseases, and they vary from mild gingival inflammation to severe deterioration of the periodontium, ie, loss of periodontal supportive tissues and, ultimately, tooth loss. New evidence shows that periodontal diseases may impact systemic health. For this reason, the maintenance of a healthy mouth is becoming increasingly important for the overall health of the body. This article summarizes laboratory research conducted during the development of a novel, multibenefit, oral-care technology based on triclosan--a broad-spectrum antibacterial agent--and a polyvinylmethylether/maleic acid copolymer. This unique combination of agents is found in Colgate Total, a clinically proven efficacious dentifrice for control of dental plaque and gingivitis. Data are presented that demonstrate the unique antibacterial properties of this dentifrice: (1) a broad-spectrum antimicrobial profile; (2) the long-lasting retention of triclosan on hydroxyapatite and epithelial cells; and (3) molecular evidence of antibacterial activity against specific pathogens in clinical dental plaque. In addition, data are presented that demonstrate the anti-inflammatory effects of triclosan on specific cytokines, the interruption of inflammatory pathways, and the inhibition of bone resorption. Overall, these data support the multibenefit clinical effects of Colgate Total and suggest a plurality of mechanisms of action.
Integrated effects of ultrasonic scaling and subgingival irrigation with 0.12% chlorhexidine by a newly designed ultrasonic scaler tip in chronic periodontitis

Abstract:Objective: To evaluate the integrated efficacy of completely simultaneous ultrasonic scaling and subgingival irrigation with chlorhexidine in chronic periodontitis. Method and materials: This was a split-mouth randomized controlled trial including 19 patients with moderate to severe chronic periodontitis. After calculus removal, the test side received simultaneous ultrasonic scaling and subgingival irrigation with 0.12% chlorhexidine, and the control side received simultaneous ultrasonic scaling and subgingival irrigation with distilled water. A newly designed ultrasonic scaler tip with a liquid outlet on the terminal was used. Clinical parameters were assessed, and gingival crevicular fluid was collected before treatment at baseline and 1.5, 3, and 6 months after baseline. Results: On follow-up, both sides showed significant reductions in clinical parameters and concentration of inflammatory mediators in gingival crevicular fluid. Adjunct application of CHX resulted in an additional peri odontal pocket reduction (0.27 to 0.29 mm, P < .05) compared to the control side, in sites with initial probing depth of 4 to 5 mm. Within the initial probing depth >= 6 mm, the additional probing depth reduction was 0.44 to 0.60 mm (P < .05), with clinical attachment loss, concentration of interleukin-6, and concentration of matrix metalloproteinase-8 being 0.32 to 0.38 mm, 2.64 to 3.40 mu g/L, and 19.78 to 22.39 ng/L, respectively (all P < .05). Conclusion: In this study, treatment outcomes of chronic periodontitis could be improved by treating the root surface with simultaneous ultrasonic scaling and chlorhexidine irrigation. The adjunctive use of 0.12% chlorhexidine with a newly designed ultrasonic scaler tip in the treatment of moderate to severe chronic periodontitis demonstrated significant clinical benefits and decrease in inflammatory mediator when compared with scaling and root planing plus placebo.

Abstract:A complex network of chemokines and pro- and anti-inflammatory mediators is involved in the initiation and progression of chronic periodontitis. Matrix metalloproteinases (MMPs), the main enzymes responsible for matrix degradation, are important for periodontal tissue destruction, but their activity can be inhibited by tissue inhibitors of metalloproteinases (TIMPs). Interleukin-12 (IL-12) and granulocyte-macrophage colony-stimulating factor (GM-CSF) have been shown to be involved in inflammatory and autoimmune diseases. Until now, no studies have reported on serum levels of MMP-2 and GM-CSF in chronic periodontitis patients and periodontally healthy subjects. Therefore, the aim of the present study was to determine the serum levels of MMP-2, TIMP-1, IL-12 and GM-CSF in chronic periodontitis patients, compared with periodontally healthy subjects. The test group of the study comprised 40 chronic periodontitis patients, whereas the control group included 108 periodontally healthy individuals. Blood samples were collected from all participants and examined using enzymelinked immunosorbent assay (ELISA) analysis. Clinical periodontal parameters (bleeding on probing, clinical attachment level and probing pocket depth) and the serum levels of MMP-2, TIMP-1, IL-12 and GM-CSF were statistically significantly higher in the test group than in the conptrol group. These results may indicate that MMP-2, TIMP-1, IL-12 and GM-CSF could be involved in the initiation and progression of chronic periodontitis. Abstract:OBJECTIVE:To observe the effect of periodontal initial therapy together with Chinese herbal compound for clearing heat and detoxification (Abbr. TCM) on the levels of tumor necrosis factor-alpha (TNF-alpha) and interleukin-1 beta (IL-1 beta) in gingival crevicular fluid (GCF) of patients with chronic periodontitis (CPD).METHODS:Thirty-four teeth of 24 patients with CPD were randomly assigned to the control group (17 teeth of 11 patients) and the treated group (17 teeth of 13 patients), they were treated for 8 weeks with periodontal initial therapy, but TCM was given additionally to the treated group. The GCF of the observed teeth was collected at the time before initial therapy (datum line) and at 2 and 8 weeks after treatment. At the same time, the clinical parameters, including probing depth (PD), clinical attachment loss (CAL), and bleeding on probing (BOP) were recorded, and the levels of TNF-alpha and IL-1 beta in GCF were measured by radioimmunoassay.RESULTS:The levels of TNF-alpha and IL-1 beta in GCF after treatment were significantly lowered in all patients as compared with those before treatment (P<0.01), while PD, CAL and BOP showed significant reduction (P<0.01). After treatment, TNF-alpha and IL-1 beta levels in the treated group were lower than those in the control group (TNF-alpha: 2.5551 +/- 1.0157 microg/L vs 3.4341 +/- 1.0762 microg/L, IL-1 beta: 0.1335 +/- 0.0216 microg/L vs 0.1471 +/- 0.0169 microg/L; P<0.05).CONCLUSION:TCM could enhance the effect of periodontal initial therapy in lowering TNF-alpha and IL-1 beta levels, which was attributed to reducing the inflammatory reaction of periodontal tissues.

Abstract:Periodontitis initiated by periodontopathic bacteria is associated with several systemic diseases. Porphyromonas gingivalis (P. gingivalis) is one of the major pathogens causing periodontal diseases, and is thought to also play a critical role in possible mechanisms linking periodontitis with other systemic disorders. We explored whether the production of pro-inflammatory cytokines interleukin-6 (IL-6), interleukin-1β (IL-1β) and tumour necrosis factor-α (TNF-α) in the liver and spleen changed as a result of oral administration of P. gingivalis in mice. mRNA expression of pro-inflammatory cytokines was analysed by Real-time Quantitative polymerase chain reaction (RT-qPCR) and cytokine protein levels were measured by Enzyme-Linked Immunosorbent Assay (ELISA) and immunohistochemistry. In addition, histological changes of liver and spleen tissues were monitored using haematoxylin and eosin staining. The results showed that liver and spleen tissue of P. gingivalis treated mice had higher mRNA and protein levels of pro-inflammatory cytokines compared to the control group. The production of proinflammatory cytokines in the liver and spleen was therefore suggested to increase as a result of oral administration of P. gingivalis in mice, and may provide further understanding of the mechanisms linking periodontitis and systemic disorders.
