Abstract:Background: In 2012, the “ShangHai At Risk for Psychosis (SHARP)” study was launched at the Shanghai Mental Health Center (SMHC), the largest outpatient mental health clinic in China. This study identified individuals at Clinical High Risk (CHR) for psychosis and evaluated clinical, neurocognitive, and biological features. We address the question of whether baseline clinical and neurocognitive characteristics (such as higher levels of unusual thought content [UTC], greater decline in functioning and poorer neurocognitive performance) predict subsequent conversion to psychosis similar to Western samples. Methods: CHR status was determined with the Structured Interview for Prodromal Syndromes (SIPS) at participants’ first visit to the SMHC. A cohort of 300 CHR and 150 Healthy Control (HC) participants was recruited between 2012 and 2015 and will be followed up for at least 1 year. At baseline, 226 participants received the MATRICS Consensus Cognitive Battery (MCCB). The primary longitudinal outcome was conversion to psychosis, which was determined using the Presence of Psychotic Symptoms from SIPS. Analyses will examine baseline clinical and neurocognitive predictors of psychosis. Survival analysis methods will be performed to illustrate the relationship of baseline characteristics to conversion outcome over time. The association of baseline clinical factors with risk for conversion was evaluated with logistic regression. Results: The final CHR case for 1-year follow-up will be completed on January 1, 2017. We present preliminary data on the first 100 CHR subjects (91 who completed a 2-year follow-up) that showed a 25% conversion rate. Several factors such as P1 symptoms (UTC, OR = 2.67, P = .032, 95% CI: 1.088–6.546) and decline in functioning at baseline (OR = 1.07, P = .027, 95% CI: 1.008–1.136) account for increasing the risk of conversion to full psychosis. Comparisons of baseline neurocognitive performances were made between converters (n = 20) and nonconverters (n = 63) among those had completed the 2-year follow-up. Converters showed significantly impaired MCCB performance on speed of processing (P = .038), visual learning (P = .01), and the composite score (P = .028). Conclusion: Results from this preliminary analysis of the first 100 subjects suggest that clinical predictors are similar to the findings from western samples. MCCB performance was similarly impaired. Analysis of the complete baseline data will be carried out after follow-ups are complete.
