Abstract:Objectives To explore the clinical and pathological characteristics of stage Ⅳ breast cancer and to analyze their relationship with the morbidity and prognosis.Methods The records of 66 patients presenting from January 2008 to December 2014 with stage Ⅳ breast cancer were reviewed.All of the patients were women and the median age was 57.5 (31 to 80) years,accounted for 3.01％ (66/2 189) among the breast cancer patients treated in the same period.Statistical methods were used to analyze the correlation between clinical and pathological characteristics such as T-stage,N-stage,immuno-histo-chemistry and the morbidity and prognosis of stage Ⅳ breast cancer.The influence of patients characteristics to metastasis were compared by x2 test.Kaplan-Meier curves were reported for overall survival (OS),and the Log-rank test was used to compare the difference in groups.Cox proportional models were fitted for multivariate analysis.Results The median survival time of stage Ⅳ breast cancer was 56.0months and the 5-year survival rate was 40％.To metastasis,the effects of age,subtypes,histological grade,hormone receptor (HR) and human epidermal growth factor receptor 2 (HER2) had no significant statistics differences.It was concluded that the expression of HER2 (P =0.003) and HR (P =0.001) as well as single metastasis (P =0.029) were the influencing factors of the survival by muhivariate Cox regression analysis.Primary tumor R0 surgery group and no surgery group had no significant statistics differences of overall survival and the 5-year survival rate (P =0.102).Conclusions Clinical and pathological characteristics have no effect on metastasis.The expression of HER2 and HR as well as single metastasis play important roles in survival.
Gastric Cancer with Breast Metastasis: Clinical Features and Prognostic Factors

Abstract:Metastatic spread of gastric carcinoma to the breast is rare. In previous decades, reports on this subject were minimal and primarily limited to case reports. At present, little is known on the clinicopathological features and prognosis of this condition, and breast metastasis remains a challenging clinical problem. A total of 54 cases of breast metastasis from gastric cancer were collected from databases between January 1960 and December 2016. The present study included 3 cases of gastric cancer with breast metastasis from Renji hospital and 51 additional cases from previous studies. The clinicopathological features of patients, including epidemiology, symptoms, macroscopic presentation, pathological diagnosis, imaging, treatment and overall survival time, were analyzed. The median survival time was 8.6 months. All but one of the patients were female, and the median age at diagnosis of breast metastasis was 43 years old (age range, 22-72 years). A majority of patients presented with Borrmann class III disease, signet ring cell carcinoma, T4 tumor types, lymph node involvement, initial stage IV gastric cancer, primary lesions in the gastric antrum, left breast metastasis and palpable breast nodules. The median interval between the primary gastric carcinoma diagnosis and presentation of breast metastasis was 1.25 months (range, 0-72 months). The expression of the estrogen receptor, progesterone receptor, human epidermal growth factor receptor-2 and gross cystic disease fluid protein-15 was negative in the patients with breast metastases. In univariate analysis, age, gastric tumor size, gastric lymph node involvement and breast metastasis histology were significantly associated with overall survival (OS) time (P=0.001, 0.039, 0.034 and <0.001, respectively). Therapeutically, gastric surgery and chemotherapy were not associated with OS (P=0.959 and 0.290, respectively). In further multivariate analysis, the time between occurrence (P=0.017), age (P=0.009), histology (P=0.045) and breast metastasis localization (P=0.043) were independent indicators of OS time. Although breast metastasis from gastric cancer is rare, physicians should be vigilant when patients with a history of gastric cancer present with newly developed mammary symptoms and signs.

Abstract:Background The features and survival of stage IV breast cancer patients with different metastatic sites are poorly understood. This study aims to examine the clinicopathological features and survival of stage IV breast cancer patients according to different metastatic sites. Methods Using the Surveillance, Epidemiology, and End Results database, we restricted our study population to stage IV breast cancer patients diagnosed between 2010 to 2015. The clinicopathological features were examined by chi‐square tests. Breast cancer‐specific survival (BCSS) and overall survival (OS) were compared among patients with different metastatic sites by the Kaplan‐Meier method with log‐rank test. Univariable and multivariable analyses were also performed using the Cox proportional hazard model to identify statistically significant prognostic factors. Results A total of 18,322 patients were identified for survival analysis. Bone-only metastasis accounted for 39.80% of patients, followed by multiple metastasis (33.07%), lung metastasis (10.94%), liver metastasis (7.34%), other metastasis (7.34%), and brain metastasis (1.51%). The Kaplan‐Meier plots showed that patients with bone metastasis had the best survival, while patients with brain metastasis had the worst survival in both BCSS and OS ( p < 0.001, for both). Multivariable analyses showed that age, race, marital status, grade, tumor subtype, tumor size, surgery of primary cancer, and a history of radiotherapy or chemotherapy were independent prognostic factors. Conclusion Stage IV breast cancer patients have different clinicopathological characteristics and survival outcomes according to different metastatic sites. Patients with bone metastasis have the best prognosis, and brain metastasis is the most aggressive subgroup.

Abstract:Introduction/Background The aim of our study was to investigate whether metastatic patterns were associated with prognosis in patients with FIGO stage IV high-grade serous ovarian cancer (HGSC). Methodology We retrospectively investigated FIGO stage IV HGSC patients who underwent primary staging or debulking surgery between April 2005 and June 2013 at our institution. None of these patients received neoadjuvant chemotherapy. Metastatic patterns were defined as pleural effusion (stage IVA), parenchymal metastases (stage IVB), and extra-abdominal lymph node metastases (stage IVB). Clinical characters and metastatic patterns were compared with chi-squared or Fisher’s exact tests as appropriate. Overall survival was analyzed using Kaplan-Meier method and Cox proportional hazards model. Results A total of 83 consecutive stage IV HGSC patients were identified. 42 (50.6%) patients were sub-classified into stage IVA with pleural effusion. Among the rest stage IVB patients, 19 (22.9%) patients had parenchymal metastases and 22 (26.5%) had extra-abdominal lymph node metastases. Five-year overall survival of stage IVA and IVB patients was 39.7% and 39.4%, respectively (p=0.711). Besides, there were no survival differences among three metastatic patterns (p=0.701). Conclusion Our study demonstrated that neither the sub-classification into FIGO IVA and IVB nor metastatic patterns of FIGO stage IV provided additional prognostic information. Clinicopathologic characteristics and survival analysis in stage IVB cervical cancer with hematogenous metastasis

Abstract:Background: The purpose of this study was to analyze the clinical characteristics, treatment modalities and prognosis of stage IVB cervical cancer patients with hematogenous metastasis. Methods: Between March 2005 and December 2017, a total of 160 patients FIGO (International Federation of Gynecology and Obstetrics, version 2018) stage IVB cervical cancer patients with hematogenous metastasis were included in this retrospective study. Patients were excluded from this analysis if they had lymphatic metastasis alone. Survival analyses included the Kaplan-Meier method, log-rank test, and Cox proportional hazards model. Results: Among 160 patients with hematogenous metastasis, 78 patients (48.8%, 78/160) exhibited only hematogenous metastases, while 82 (51.3%, 82/160) exhibited distant organ and lymph node (LN) metastases. Bone (37.5%, 60/160) was the most frequent metastatic site compared to lung (23.1%, 37/160) and liver (12.5%, 20/160). The median duration overall survival (OS) was 26 months and the 3-year OS rate was 35.3%. In univariate analysis, a significant difference was found in prognostic factors related to OS including tumor size (P=0.030), histology (P=0.006), white blood cell (WBC) count (P<0.001), squamous cell carcinoma antigen (SCCA) level (P=0.026), numbers of distant metastasis (P=0.017) and primary treatment (P=0.015). In multivariate analysis, the WBC was reported as an independent risk factor of progression free survival (PFS) (P=0.012, HR =3.25, 95% CI, 1.30-8.14) and OS (P=0.000, HR = 6.18, 95% CI, 2.39-15.95). In addition, chemoradiotherapy was found to prolong the OS of stage IVB cervical cancer patients with hematogenous metastasis (32 vs. 24 months, P=0.049, HR = 0.46, 95% CI, 0.21-0.99) compared with chemotherapy alone. Conclusions: For stage IVB cervical cancer with hematogenous metastasis, the WBC count was an independent recurrence and death risk factor. Also, stage IVB cervical cancer patients with hematogenous metastasis should not be treated with systemic chemotherapy alone, palliative radiotherapy should also be integrated into the treatment plan. However, future large scale prospective clinical trial was desperately in need.
Effect of HER-2 Overexpression and Other Tumor Characteristics on Metastasis Sites of Breast Cancer Patients after Operation

Abstract:Objective To analyze the breast caner patients with different sites of recurrence,and to have knowledge of the regular pattern of metastatic breast cancer.Methods We retrospectively analyzed 210 women with metastatic breast cancer.The clinical and biological characteristics were compared between the patients with organ metastases(lung,liver,brain)and non-viscera metastases(lymph node,bone).The Cox regression analysis was used to test the risk factors.Results Univariate analysis indicated that the average of age was significantly lower(50.71 vs 46.69,P=0.012)in the patients with viscera metastases,and the rate of HER-2 overexpression was also significantly higher(10.6% vs 25.4%,P=0.05).Cox regression analysis showed that patients with elder age(RR=0.927 95%CI 0.877-0.981,P=0.008),HER-2 negative overexpression(RR=0.253 95%CI 0.076-0.836,P=0.024)tended to develop non-viscera metastases,while patients with larger tumors(RR=3.832 95%CI 1.073-13.687,P=0.039)tended to develop organ metastases.Conclusions Patients with different clinical and biological characteristics may develop recurrence in different sites.There may be different mechanisms of metastasis for recurrence in different sites.
The Effect of Distant Metastases Sites on Survival in De Novo Stage-Iv Breast Cancer: A SEER Database Analysis

Abstract:To investigate the effect of distant metastases sites on survival in patients with de novo stage-IV breast cancer. From 2010 to 2013, patients with a diagnosis of de novo stage-IV breast cancer were identified using the Surveillance, Epidemiology, and End Results database. Univariate and multivariate Cox regression analyses were performed to analyze the effect of distant metastases sites on breast cancer–specific survival and overall survival. A total of 7575 patients were identified. The most common metastatic sites were bone, followed by lung, liver, and brain. Patients with hormone receptor+/human epidermal growth factor receptor 2− and hormone receptor+/human epidermal growth factor receptor 2+ status were more prone to bone metastases. Lung and brain metastases were common in hormone receptor−/human epidermal growth factor receptor 2+ and hormone receptor−/human epidermal growth factor receptor 2− subtypes, and patients with hormone receptor+/ human epidermal growth factor receptor 2+ and hormone receptor−/human epidermal growth factor receptor 2+ subtypes were more prone to liver metastases. Patients with liver and brain metastases had unfavorable prognosis for breast cancer–specific survival and overall survival, whereas bone and lung metastases had no effect on patient survival in multivariate analyses. The hormone receptor−/human epidermal growth factor receptor 2− subtype conferred a significantly poorer outcome in terms of breast cancer–specific survival and overall survival. hormone receptor+/human epidermal growth factor receptor 2+ disease was associated with the best prognosis in terms of breast cancer–specific survival and overall survival. Patients with liver and brain metastases were more likely to experience poor prognosis for breast cancer–specific survival and overall survival by various breast cancer subtypes. Distant metastases sites have differential impact on clinical outcomes in stage-IV breast cancer. Follow-up screening for brain and liver metastases might be effective in improving breast cancer–specific survival and overall survival.
Clinicopathological Characteristics and Prognosis of HER2-Low Early-Stage Breast Cancer: A Single-Institution Experience

Abstract:Background: Human epidermal growth factor 2 (HER2)-low breast cancer, which is defined as HER2 1+ or 2+ in immunohistochemistry without gene amplification, accounts for a considerable part of all breast cancers. However, it remains controversial whether HER2-low breast cancer is a distinct entity. Our aim was to compare the clinicopathological features and survival outcomes between HER2-zero and HER2-low early breast cancer. Methods: The study was a retrospective analysis that enrolled 1,039 patients with available HER2 expression data in a single institute from 2013 to 2014, of whom 262 HER2-positive patients were excluded from the subsequent analysis. The remaining patients were divided into HER2-zero and HER2-low groups. Each group was further categorized into a hormone receptor (HR)-positive and an HR-negative subgroup. Clinicopathological characteristics were collected and compared between HER2-zero and HER2-low groups. The primary endpoint was disease-free survival (DFS) and overall survival (OS), which were analyzed using the Kaplan–Meier method with log-rank test, landmark analysis, and Cox proportional hazards model. Results: A total of 777 non-HER2-positive patients were included in this analysis, of whom 126, 552, 53, and 46 patients were HR-positive/HER2-zero, HR-positive/HER2-low, HR-negative/HER2-zero, and HR-negative/HER2-low, respectively. No significant difference in DFS and OS was detected between the HER2-zero group and the HER2-low group when paired by HR status. Landmark analysis with a time point set at 5 years indicated that HR-positive/HER2-low patients had a better DFS compared with HR-positive/HER2-zero patients after 5 years ( p = 0.0047). HER2-low status was an independent prognostic factor for DFS after 5 years [hazard ratio (HR) 0.31, 95% confidence interval (CI) 0.13–0.75, p = 0.01]. Conclusion: The clinicopathological characteristics and prognosis of HER2-zero and HER2-low breast cancer were similar regardless of HR status. Patients with HR-positive/HER2-low tumors tended to have a better DFS than their HR-positive/HER2-zero counterparts after 5 years.

Abstract:Background/Aims: This study investigated the influence of clinicopathological characteristics and different therapy patterns on overall survival of gastric cancer patients with liver metastases. Methodology: One hundred and sixty two patients were enrolled in this study. The data of clinicopathological characteristics, treatment methods and overall survival were collected. Univariate and multivariate analyses were performed using log-rank tests and Cox's proportional hazard model, respectively. Results: Of all the patients enrolled, 27 (16.7%) had liver-only metastases and 48 patients (28.7%) received local treatment for liver metastases. The median overall survival of the 162 patients was 9.5 months (95%CI: 8.4-10.6). One- and two-year survival rates were 28.4% and 4.3%, respectively. There was no difference in the overall survival of patients who had received the different chemotherapy regimens. Multivariate analysis revealed that numbers of liver metastases 1-3, without extrahepatic metastases, receiving local treatment for liver metastases were better prognostic factors, and HR were 0.490, 0.479, 0.495. Conclusions: A smaller number of liver metastases, without extrahepatic metastases and local therapy for liver metastases are the independent survival factors for gastric cancer with liver metastases.
Characteristics and Survival in Bone Metastatic Breast Cancer Patients with Different Hormone Receptor Status: A Population-Based Cohort Study.

Abstract:BackgroundAccumulating preclinical evidence has uncovered the indispensable role of steroid hormone and their receptors, namely, estrogen receptor (ER) and progesterone receptor (PR), in the development of bone metastases in breast cancer. Limited data are available regarding the survival difference between different hormone receptor (HR) subgroups, and its prognostic significance is uncertain now. Such data are important for risk stratification and needed to formulate specialized regimen for bone metastatic breast cancer. MethodsFrom the year of diagnosis 2010 to 2018, 554,585 breast cancer patients, among which are 19,439 with bone metastasis and 10,447 with bone-only metastasis, were extracted from the Surveillance, Epidemiology, and End Results (SEER) database. Kaplan-Meier survival analysis was performed to compare the survival difference between the different HR status subgroups. Univariate and multivariate Cox proportional hazard regression was used to validate the prognostic role of HR status and identify other prognostic factors in bone metastatic breast cancer. ResultsER-positive/PR-positive breast cancer patients with bone metastasis showed the best breast cancer-specific survival (BCSS) and overall survival (OS) than those with other HR statuses, while single PR-positive bone metastatic breast cancers manifest similar survival with ER-negative/PR-negative ones. Adjusted Cox regression analysis demonstrated that patients with older age, male, black race, ILC, higher tumor grade, T3-T4, HER2-negative status, absence of surgery or adjuvant treatment, and HR status other than ER-positive/PR-positive tended to have worse outcomes. Further subgroup analysis based on HER2 status showed that within HER2-positive breast cancers, ER-positive/PR-positive ones still manifest better survival than the other three HR status subgroups, which are similar in survival outcomes. ConclusionAlthough collectively viewed as HR-positive breast cancers, certain distinctions exist between bone metastatic breast cancers with different HR statuses in survival outcome. Our findings indicate that despite metastasizing to the same location, the different survival rate is determined by the HR status of breast cancer. The selection and intensity of the regimen should consider HR status, and HER2 status occasionally, when treating bone metastatic breast cancer.

Abstract:Objective:To analyze the effects of clinical characteristics on the numbers of metastasis positions in metastatic prostate cancer patients.Methods:A total of 198 patients who received androgen deprivation therapy(ADT) from January 2005 to August 2011 were recruited.The relation between the clinical characteristics and the numbers of metastasis positions were analyzed by χ2 test and logistic regression model.The effects of the numbers of metastasis positions on prognosis were analyzed by survival analysis with Kaplan-Meier method and log-rank test.Results:There were significant relations between Gleason scores,serum prostate-specific antigen (PSA) levels,PSA levels after ADT,ECOG scores and number of metastatic sites.Compared with single metastasis,the biochemical progression free survival and overall survival were shortened in patients with multiple metastasis.Conclusion:Gleason scores,serum PSA levels,PSA levels after ADT,ECOG scores have an important effects on number of metastatic sites.The multiple metastasis predicted poor prognosis.
Clinical Characteristics and Survival Analysis of Breast Cancer Molecular Subtypes with Hepatic Metastases

Abstract:BACKGROUND: The liver is one of the most common metastatic sites of breast cancer, hepatic metastases developing in 6%-25% of patients with breast cancer and being associated with a poor prognosis. The aim of this study was to analyze the survival and clinical characteristics of patients with hepatic metastases from breast cancer of different molecular subtypes and to investigate the prognostic and predictive factors that effect clinical outcome.METHODS: We retrospectively studied the charts of 104 patients with breast cancer hepatic metastases diagnosed at Sun Yat-sen University Cancer Center from December 1990 to June 2009. Subtypes were defined as luminal A, luminal B, human epidermal growth factor receptor 2 (HER2) enriched, triple-negative (TN). Prognostic factor correlations with clinical features and treatment approaches were assessed at the diagnosis of hepatic metastases.RESULTS: The median survival time was 16.0 months, and the one-, two- three-, four-, five- year survival rates were 63.5%, 31.7%, 15.6%, 10.8%, and 5.4%, respectively. Median survival periods after hepatic metastases were 19.3 months (luminal A), 13.3 months (luminal B), 18.9 months (HER2-enriched), and 16.1 months (TN, P=0.11). In multivariate analysis, a 2 year-interval from initial diagnosis to hepatic metastasis, treatment with endocrine therapy, and surgery were independent prognostic factors. Endocrine therapy could improve the survival of luminal subtypes (P=0.004) and was a favorable prognostic factor (median survival 23.4 months vs. 13.8 months, respectively, P=0.011). Luminal A group of patients treated with endocrine therapy did significantly better than the Luminal A group of patients treated without endocrine therapy (median survival of 48.9 vs. 13.8 months, P=0.003).CONCLUSIONS: Breast cancer subtypes were not associated with survival after hepatic metastases. Endocrine therapy was a significantly favorable treatment for patients with luminal subtype.

Abstract:Objective To analyze clinical pathological characters of early gastric cancer with distant metastasis.Methods The clinical data of 4 420 patients with early gastric cancer admitted between April 1973 and November 1998 in 8 hospitals of Chinese and Japanese university were analyzed retrospectively.Among them,22 patients had distant metastasis.The sex ratio,average age,maximum tumor diameter,infiltrating depth,differentiation type,and long term survival were compared between the distant metastasis (DM) group and non-distant metastasis (control) group.Results There were very significant differences of sex ratio,maximum tumor diameter,infiltrating depth,and 5 year survival rate (DM group 34.7%,control group 91.3%) between DM group and control group (P<0.01).There were no significant differences of age and differentiation type between the two groups.Conclusion Even early gastric cancer can have distant metastasis and poor prognosis.Female,tumor infiltrating submucosa and maximum diameter >4cm can be risk factors of early gastric cancer with distant metastasis.

Abstract:Objective To analyze the clinical characteristics and prognosis of patients with breast cancer brain metastases in different subtypes.Methods Retrospective clinical and survival analyses were performed in 90 breast cancer patients with brain metastases.Patients were divided into estrogen receptor(ER) + and/or progesterone receptor(PR) +,human epidermal growth receptor-2(HER-2) +,and triple negative(ER-,PR-and HER-2-) subtypes.Results In the 90 patients,there were 51 patients(56.7%) with ER+ and/or PR+12 patients(13.3%) with HER-2+ and 27 patients(30%) with triple negative breast cancer.The median DFS(disease free survival) was 30.9,25.4 and 16.0 months(P=0.001)among ER+ and(or)PR+,HER-2+ and triple negative breast cancer.The median brain disease free survival was 36.0,38.0 and 22.0 months(P=0.008)among the above three types.The median OS(overall survival) was 52.0,25.5 and 36.6 months(P=0.075)among the three types.The median survival from brain metastasis was 11.0,5.5 and 8.0 months(P=0.829) among the three types.Conclusion The triple negative subtype was more prone to metastasize to brain with a shorter median disease free survival compared with other two subtypes.The median overall survival and survival from brain metastasis were similar among three subtypes.

Abstract:Background: The aim of this study was to clarify the features and prognostic significance of isolated para-aortic lymphatic metastasis of endometrial cancer. Methods: A retrospective study of patients with stage IIIC endometrial cancer was performed based on the Surveillance, Epidemiology, and End Results (SEER) database. A total of 2767 patients were divided into three groups according to the lymphatic metastasis patterns: isolated pelvic lymphatic metastasis, isolated para-aortic lymphatic metastasis and dual lymphatic metastasis. The clinic-pathological characteristics and prognosis of patients were compared among the three groups. Result: The proportion of patients with isolated para-aortic lymphatic metastasis was 13.70%. Patients with isolated pelvic lymphatic metastasis or isolated para-aortic lymphatic metastasis shared similar histological characteristics, except that patients with isolated para-aortic lymphatic metastasis had a lower proportion of tumors over 5 cm in diameter than patients with isolated pelvic lymphatic metastasis (35.1% versus 45.7%, p = 0.001). Compared with patients with dual lymphatic metastasis, isolated para-aortic lymphatic metastasis was more common in patients with endometrioid tumors (78.6% versus 67.3%, p < 0.001), grade 1–2 cancers (53.3% versus 36.3%, p < 0.001) and negative peritoneal cytology (76.2% versus 61.1%, p < 0.001). Dual lymphatic metastasis was an independent predictive factor for the poor outcomes of patients at stage IIIC. However, in stage IIIC endometrioid tumors, patients with isolated pelvic lymphatic metastasis and those with isolated para-aortic lymphatic metastasis shared similar prognosis. Patients at stage IIIC with nonendometrioid tumors and patients at stage IV could not be further divided into subgroups according to lymphatic metastasis patterns in terms of prognosis. Conclusion: Endometrioid patients with isolated pelvic lymphatic metastasis and isolated para-aortic lymphatic metastasis share similar clinical pathological characteristics and prognoses.

Abstract:Objective To analyze the association between the primary metastatic sites of HER2-positive breast cancer and the molecular subtypes, and to establish the treatment and follow up strategy for HER2-positive breast cancer. Meth-od The clinical data of 176 patients with HER2-positive metastatic breast cancer were analyzed. HER2-positive was de-fined as (+++) in immunohistochemistry or FISH amplification, and was pathologically classified as invasive ductal carci-noma, and then the distribution of primary recurrence and metastasis in those patients were analyzed. Result The medi-an age was 50 (range:31-76) in the 176 patients;and primary liver metastasis was observed in 38.1%, including 18.8%of single liver metastasis;29.0%of patients had lung metastasis, with 11.4%of single lung metastasis;and a significant dif-ference was observed between liver and lung metastasis (P=0.045);there were 42.6%of patients had lymph node metasta-sis (including supraclavicular, abdominal and mediastinal lymph nodes, etc.), in which 19.9%had single lymph node in-volvement;25.1%had bone metastasis, including 6.3%of single bone;chest wall recurrence rate was 14.3%, and 8.0%had sole chest wall involvement;4 cases (2.3%) were with brain metastases; other rare metastases includes: ovarian (1 case) and uterine metastasis (1 case). Primary visceral metastasis rate was 59.7%, while 34.7%of patients had multiple sites involvement. Conclusion The metastasis of HER2-positive breast cancer is unusual in nature, with more visceral and lymph nodes metastasis, especially in liver. In the follow-up setting after surgery and adjuvant therapy, more attention should be paid to lymph nodes and visceral examination, especially for liver and lung. Meanwhile, the results also indi-rectly explain the different metastasis pattern of various molecular subtypes, so corresponding medications are required in postoperative adjuvant chemotherapy when metastatic sites are considered.

Abstract:Objective To analyze the clinicopathological characteristics and prognosis of different subtypes of breast cancer patients with bone metastasis.Methods For this study, we recruited 300 primary breast cancer patients with bone metastasis treated at the Department of Oncology, the First Affiliated Hospital of Xi`an Jiaotong University, between September 1, 2007 and September 1, 2011.We also retrospectively analyzed their clinical and follow-up data.Results The percentage of Luminal A, Luminal B, human epidermal growth factor receptor-2 (HER-2) overexpression and triple negative subtypes in all the bone metastatic breast cancer patients was 59.0％, 16.0％, 13.7％ and 11.3％, respectively.Age, tumor size and histologic grade significantly differed among the four subtypes (P<0.05).However, there were no significant differences in menopausal status, lymph node metastasis, histological type or lymphovascular invasion among different subtypes (P>0.05).The median survival time of Luminal A breast cancer patients with bone metastasis was 28.6 months, longer than Luminal B (26.9 months), HER-2 overexpression (20.9 months) and triple negative breast cancer patients (12.0 months) with bone metastasis.The overall survival significantly differed among the patients with four subtypes of breast cancer.Conclusion Different subtypes of breast cancer patients with bone metastasis have different clinical characteristics and prognosis.Luminal A breast cancer patients with bone metastasis have better prognosis whereas triple negative subtype has poorer prognosis.

Abstract:Objective:To explore the clinicopathologic features of breast cancer and prognosis.Methods:The survival was studied for 146 breast cancer patients retrospectively.Results:Lymph nodes stage,clinical stage and radiotherapy were obvious correlated with the overall survival in the univariate analysis;clinical stage,tumor stage and endocrine therapy were obvious correlated with the disease-free survival within 5 years in the univariate analysis.Clinical stage was both correlated with both overall survival and disease-free survival within 5 years in the univariate analysis.Conclusion:Clinical stage is important for the disease-free survival and overall survival,therefore,early discovery and early treatment can improve the survival rate in breast cancer patients.
Clinicopathologic characteristics and prognostic significance of HER2-low expression in patients with early breast cancer: A systematic review and meta-analysis

Abstract:Background: HER2-low expression breast cancer (BC) accounts for approximately 45%-55% of all BC cases. The purpose of this study was to investigate the prognostic difference between patients with HER2-low expression and HER2-zero BC. Methods: An electronic search of Pubmed, Embase, Cochrane Library, and Web of Science databases was performed to screen studies that included prognostic comparisons between HER2-zero and HER2-low expression groups. A total of 14 studies involving 52106 patients were included. Results: Our results indicated that HER2-low expression was associated with a significant benefit in OS among all patients with early BC (HR, 0.83; 95% CI, 0.78–0.88), patients with hormone-receptor positive BC (HR, 0.83; 95% CI, 0.77–0.89), and patients with TNBC (HR, 0.78; 95% CI, 0.70–0.87). HER2-low expression was associated with a significant benefit in DFS among all patients (HR, 0.81; 95% CI, 0.71–0.93), patients with hormone receptor-positive BC (HR, 0.81; 95% CI, 0.72–0.90), but no significant difference in DFS was found in patients with TNBC (HR, 0.87; 95% CI, 0.65–1.17). HER2-low expression was associated with a significant benefit in RFS among all patients (HR, 0.90; 95% CI, 0.85–0.95), patients with hormone receptor-positive BC (HR, 0.90; 95% CI, 0.84–0.96), but no significant difference in RFS was found in patients with TNBC (HR, 0.80; 95% CI, 0.55–1.16). Conclusions: Among patients with early-stage BC, patients with HER2-low expression BC had better OS in the overall population, hormone receptor-positive and TNBC subgroups. Besides, favorable DFS and RFS were observed in both the overall population and hormone receptor-positive subgroup. Abstract:Objective:To explore the features of pelvic lymph node metastasis,the related prognostic factors and the influence to outcome in stage Ⅰ,Ⅱcervical cancer patients.Methods:We retrospectively analyzed 32 cervical cancer patients with lymph node metastasis and 32 cases without lymph node metastasis,who had undertaken extensive hysterectomy and radical pelvic lymphoadenectomy.Results:The lymph node metastasis rate was 23.35%,and the most common metastasis of cervical carcinoma occurred at the obturator node(59.37%).Pelvic lymph node metastasis was associated with deep cervical stromal invasion,which was not correlated with age,histological classification,the grade of differentiation,parametrial involvement,and lymphovascular invasion.The relevant prognostic factor for cervical carcinoma was number of lymph node metastasis,and was not the depth of cervical stromal invasion,histological classification,the grade of differentiation and lymphovascular invasion.Conclusion:The stromal invasion ≥l/2 is a risk factor of pelric lymph node metastasis in cervical cancer.The prognosis of patients with ≥3 positive pelvic lymph node are quite poor,which is helpful for clinical therapy.
