Abstract:BACKGROUND:Metformin has shown its effectiveness in reducing body mass index (BMI) in obese children and adolescents, but relevant evidence for improving insulin resistance in overweight or obese children and adolescents is inconclusive. OBJECTIVES:This study aimed to assess whether metformin could effectively and safely improve homeostasis model assessment insulin resistance index (HOMA-IR) and other related laboratory indicators including fasting glucose, fasting insulin, high-density lipoprotein cholesterol (HDL-C), and low density lipoprotein-cholesterol (LDL-C). METHODS:Searches were carried out in PubMed, CENTRAL, Web of Science, EMBASE, CBM, Chinese National Knowledge Infrastructure (CNKI), and WanFang from their inception until March 2018. Randomized controlled trials (RCTs) comparing metformin alone with placebo in overweight or obese children and adolescents were included. The Cochrane risk of bias tool was applied to assess the methodological quality of every study and Meta-analysis was carried out with a random effects model or a fixed effects model. Publication bias was evaluated by the Begg and Egger tests. RESULTS:A total of 11 trials with a total of 865 participants met the inclusion criteria. Participants were between 4 and 18 years old. The time span of these studies ranged from 2001 to 2017. The daily dose of metformin was from 1000 mg to 2000 mg and the duration of intervention was 8 weeks to 18 months. Compared with placebo, metformin with lifestyle intervention reduced the level of LDL-C (P = 008, MD = - 4.29, 95% confidence interval [CI]: -7.45, -1.12). However, there was no obvious differences in improving insulin resistance, fasting glucose, and HDL-C. CONCLUSION:Metformin may improve the level of LDL-C, but it has no significant effect on insulin resistance. The use of metformin may be a new approach to lipid metabolism management in overweight or obese children and adolescents. REGISTRATION NUMBER:CRD42018092059.

Abstract:Objective: Some studies have shown that metformin can reduce body weight. However, metformin has not been officially approved as a medicine for weight loss because its effect on different populations remains inconsistent. This meta-analysis aimed to summarize the weight loss effect of metformin quantitatively. Method: The randomized controlled and high-quality case-control trials of metformin monotherapy in obesity treatment were eligible. Baseline body mass index (BMI) was chosen as a self-control to compare the changes in BMI of different populations before and after treatment. All changes were calculated as differences between the final and initial BMI values (with negative values indicating a decrease). Results were presented as weighted mean difference (WMD) with a 95% confidence interval (CI 95%). Subgroup analysis was performed based on baseline BMI, age, daily dose, and duration of medication. Results: A total of 21 trials (n = 1004) were included, and the meta-analysis of metformin treatment in different populations showed that metformin has a modest reduction in the BMI of included participants (WMD -0.98; 95% CI, -1.25 to -0.72), and the reduction of BMI was most significant in the simple obesity population (WMD -1.31; 95% CI, -2.07 to -0.54). The subgroup analysis showed that metformin treatment significantly reduced BMI in obesity patients with a BMI >35kg/m(2) (WMD -1.12; 95% CI, -1.84 to -0.39) compared with before treatment. BMI in the high dose group decreased by 1.01 units (WMD-1.01; 95% CI, -1.29 to -0.73) and BMI did not continue to decrease significantly after treatment of more than 6 months. Conclusion: Patients treated with metformin experienced about a one-unit reduction in BMI at the end of treatment. But whether this decreased value produced enough weight loss (5% of baseline body weight) to qualify as a "weight loss drug" as current guidelines require, requires larger specific randomized control trials.
Influence of metformin on blood pressure and metabolism in 155 patients with obesity hypertensives

Abstract:AIM: To evaluate the effects of metformin on blood pressure and metabolism in obesity hypertensives. METHODS: The obesity hypertensives were randomized into two groups: therapy group (n=155) was administered with nitredipine( 32± s 5) mg·d -1, po ,atenolol (30±4) mg·d -1, po and metformin (745± 26 )mg·d -1, po. The control group (n=147 ) was administered with nitredipine (32± 5) mg·d -1, po and atenolol(30± 4) mg·d -1, po. Each subject had been followed for 1.5- 2 a. RESULTS: After treatment, blood pressure decreased significantly in control and therapy groups respectively. But there were no differences between the two groups concerning about SBP: (17.3±2.2) kPa vs (17.0±2.4) kPa,DBP: (11.1±0.8) kPa vs (11.2±0.8)kPa. The therapy group showed decrease in BMI, waist, free fatty acids and insulin sensitivity index more than those of the control group; demonstrating as BMI((26.2±1.1)kg·m -2 vs (28.3±1.5) kg·m -2,P0.05), waist((92±7 )cm vs (95±6)cm,P0.05), free fatty acids( (547±183)μmol·L -1 vs (619±197)μmol·L -1,P0.05 )and insulin sensitivity index( (51±12) vs (46±13),P0.05), respectively. Through multifactor progression analysis, BMI was the most important factor influencing the blood pressure. CONCLUSION: Metformin improved insulin sensitivity by decreasing triglyceride and free fatty acids. Metformin had the effect on reducing weight of obesity hypertensives, but no evidance of cooperation with other anti-hypertensive drugs. Metformin administration for abdominal obesity type of cardiovascular disease patients with high triglyceride and impaired glucose tolerance(IGT) is advisable and worth to be recommanded.
Effects of metformin and fenofibrate on peripheral insulin resistance and β-cell function in patients with simple obesity

Abstract:AIM: To observe the therapeutic efficacy of metformin and fenofibrate for peripheral insulin resistance and β-cell function in patients with simple obesity. METHODS: Forty cases of simple obesity with hypertension, hypertriglyceridemia, hyperinsulinemia but normal sugar tolerance were divided into the metformin and fenofibrate treatment group and the metformin treatment group. The subjects were observed for their blood pressure, blood fat, body mass index (BMI) and insulin level before and after treatment for 24 weeks. RESULTS: Metformin and fenofibrate could decrease blood pressure, blood fat, BMI and fasting insulin level obviously, and post-meal insulin changes were ob-vious (P<0.05). In metformin treatment group, blood pressure and triglyceride, BMI were also decreased significantly (P<0.05), but fasting and post-meal insulin levels did not change so notably like those in metformin and fenofibrate treatment group (P<0.05). CONCLUSION: Metformin and fenofibrate can increase insulin sensitivity and recover β cell function in patients with simple obesity accompanied by insulin resistance, hypertension and hypertriglyceridemia, so the combination has an obvious therapeutic action in patients with simple obesity.
Metformin-based treatment for obesity-related hypertension: a randomized, double-blind, placebo-controlled trial.

Abstract:OBJECTIVES:Obesity and hypertension are associated with an adverse metabolic profile and systemic low-grade inflammation. Metformin reduces weight and inflammation in patients with diabetes, but it is unclear whether it has beneficial effects in patients without diabetes. The objective was to explore whether metformin-based treatment could benefit obesity-related hypertension without diabetes. METHODS:A randomized, double-blind, placebo-controlled factorial trial was conducted in 360 obese hypertensive patients without diabetes in Chongqing, China. After a 1-2-week run-in period, patients were randomly assigned to metformin (500 mg once per day) or placebo, as well as to an antihypertensive medication. Change in blood pressure, obesity measurements and metabolic profile were assessed at 24 weeks. RESULTS:The 180 participants randomized to metformin and 180 randomized to placebo were similar at baseline. At 24 weeks, metformin compared with placebo did not have significant effects on blood pressure, blood glucose, high-density or low-density lipoprotein cholesterol, but it did reduce total serum cholesterol (0.2 mmol/l, P = 0.038). Metformin also significantly reduced weight (-0.7 kg, P = 0.006), BMI (-0.2 kg/m, P = 0.024), waist circumference (-0.9 cm, P = 0.008), and both subcutaneous (-6.1 cm, P = 0.043) and visceral adiposity (-5.4 cm, P = 0.028) as measured by computed tomography, and lowered serum high-sensitivity C-reactive protein levels (-0.6 mg/dl, P < 0.001). There was no significant difference in adverse events (P = 0.785). CONCLUSIONS:Metformin has no effect on blood pressure and blood glucose levels, but it does reduce total cholesterol, abdominal obesity and C-reactive protein levels in obese hypertensive patients without diabetes.
Efficacy and Safety of Pharmacological Combination Therapies for the Treatment of Type 2 Diabetes in China: A Systematic Review and Meta-Analysis

Abstract:Background: Chinese guideline recommends using combination therapy of antidiabetic-drug plus metformin for type 2 diabetes (T2DM) patients failed to control glucose on metformin. Treatment effect varies between antidiabetic drugs, but there is no direct evidence on comparing the effect of each alternative drug plus metformin. This study aims to estimate and compare the efficacy and safety of seven classes of antidiabetic-drug added onto metformin for Chinese T2DM patients failed on metformin. Methods: Systematic review was performed by searching databases including CNKI, WanFang, CQVIP, PubMed, EMBASE, Web-of-Science, ScienceDirect, Cochrane-Library, to identify randomized controlled trials(1990-2016) comparing the treatment effect of 'antidiabetic-drug + metformin vs metformin' or 'metformin vs placebo/lifestyle-intervention' in Chinese T2DM patients. Based on Chinese guideline, three-line eight-class of antidiabetic-drugs were included: 1st-line-drug(metformin), 2nd-line-drug/oral-drug(α-glycosidase inhibitor, sulfonylurea, glinide, DPP-4 inhibitor, thiazolidinedione), 3rd-line-drug/injectable-drug(insulin, GLP-1 receptor agonist). Using metformin as common comparator, meta-analyses and indirect-treatment-comparisons were used to estimate the effect of 'antidiabetic-drug + metformin vs placebo/lifestyle-intervention'. Findings: 325 studies were finally included. Overall, compared with placebo/lifestyle-intervention, 'oral-drug + metformin' and 'injectable-drug + metformin' reduced HbA1c level by 1·89% and 2·28%, BMI level by 2·55kg/m2 and 3·10kg/m2, TC level by 1·03mmol/l and 1·50mmol/l, SBP level by 5·59mmHg and 8·51mmHg, while increased HDL-C level by 0·31mmol/l and 0·18mmol/l, respectively. In 'oral-drug + metformin', the top two treatments for reducing HbA1c level were 'glinide + metformin' (WMD:-2·03,95%CI[-2·70,-1·36]) and 'sulfonylurea + metformin' (WMD:-2·00,95%CI[-2·64,-1·36]); for reducing BMI level were 'glinide + metformin' (WMD:-4·25,95%CI[-5·70,-2·80]) and 'DPP-4 inhibitor + metformin' (WMD:-2·81,95%CI[-4·22,-1·40]); for reducing TC level were 'α-glycosidase inhibitor + metformin'(WMD:-1·41,95%CI[-2·15,-0·68]) and 'DPP-4 inhibitor + metformin' (WMD:-1·22,95%CI[-1·71,-0·73]); for increasing HDL-C level were 'α-glycosidase inhibitor + metformin'(WMD:0·51,95%CI[0·27,0·75]) and 'thiazolidinedione + metformin' (WMD:0·35,95%CI[0·10,0·60]). In 'injectable-drug + metformin', 'GLP-1 receptor agonist + metformin' was superior to 'insulin + metformin' in reducing HbA1c level (WMD:-2·42vs-2·04) and BMI level (WMD:-3·27vs-2·05), while inferior to 'insulin + metformin' in reducing TC level (WMD:-1·43vs-1·79). All combination therapies had no significant difference on risks of hypoglycemia or gastrointestinal-reaction vs placebo/lifestyle-intervention. Interpretation: Pharmacological combination therapies had better efficacy than placebo/lifestyle-intervention in Chinese T2DM patients. Funding: No funding was received. The Antiobesity Effect of GLP-1 Receptor Agonists Alone or in Combination with Metformin in Overweight /Obese Women with Polycystic Ovary Syndrome: A Systematic Review and Meta-Analysis

Abstract:Objectives: Both glucagon-like peptide-1 receptor agonists (GLP-1RAs) and metformin (MET) have markedly antiobesity effects in overweight/obese polycystic ovary syndrome (PCOS) patients. However, there was no literature to compare the antiobesity effects of these two medicines. Therefore, a systematic review and meta-analysis were conducted in our present study to evaluate the antiobesity effects of GLP-1RAs either as monotherapy or combined with MET in comparison with MET alone in overweight/obese PCOS patients. Methods: All randomized controlled trials (RCTs) which reported the efficacy of GLP-1RAs and MET in overweight/obese PCOS patients in Medline (from Pubmed), Embase, Cochrane Central Register of Controlled Trials, Web of Science, and Scopus databases were independently searched by two reviewers. The random-effect model was used to pool data extracted from the included literature. The weighted mean difference (WMD) and 95% confidence interval (CI) were used to present the meta-analysis results (PROSPERO registration number: CRD42020173199). Results: A total of eight eligible RCTs were finally enrolled in our meta-analysis from the 587 retrieved literature. The results showed that GLP-1RAs alone or combined with MET was associated with a greater weight loss (N = 318, WMD = -2.61, 95% CI: -3.51 to -1.72, P ≤ 0.001, I 2 = 77.5%), more obvious reduction of waist circumference (N = 276, WMD = -3.46, 95% CI: -4.36 to -2.56, P ≤ 0.001, I 2 = 0.0%), and body mass index (BMI) (N = 318, WMD = -0.93, 95% CI: -1.60 to -0.26, P=0.007, I 2 = 84.9%) in overweight/obese PCOS patients when compared with MET alone. Further sensitivity analysis demonstrated that the meta-analysis results of the efficacy differences in terms of body weight, waist circumference, and BMI were relatively stable and reliable. Conclusion: Our meta-analysis demonstrated that the antiobesity effect of GLP-1RAs alone or combined with MET was superior to MET alone in terms of weight loss, the reduction of waist circumference, and BMI. More large-scale, high-quality RCTs are needed to further confirm these results in PCOS patients.
Effects of Metformin on Blood Lipid Profiles in Nondiabetic Adults: a Meta-Analysis of Randomized Controlled Trials

Abstract:To evaluate the effects of metformin on serum lipid profiles in nondiabetic adults through a comprehensive meta-analysis. In the present meta-analysis, randomized and controlled trials were collected by searching PubMed, Embase, and Cochrane Libraries from inception to April 2019. Compared with placebos, the effects of metformin treatment on lipid profiles in nondiabetic adults were evaluated. Forty-seven studies from 45 articles including 5731 participants were enrolled. Pooled results showed that metformin had significant effects on total cholesterol (mean change −6.57 mg/dl; 95% CI −9.66, −3.47; P = 0.000) and LDL-c (mean change −4.69 mg/dl; 95% CI −7.38, −2.00; P = 0.001), but insignificant effects on HDL-c (mean change −4.33 mg/dl; 95% CI −9.62, 0.96; P = 0.109) and triglyceride (mean change −0.85 mg/dl; 95% CI −0.36, 2.06; P = 0.169). Significant heterogeneities were found for all lipid profiles (HDL-c = 85.5%; LDL-c = 59.9%; total cholesterol = 75.3% and triglyceride = 67.1%). Different from the pooled data, in a subgroup analysis, the effect of metformin on triglyceride in patients with polycystic ovarian syndrome (PCOS) was significant with a mean reduction of 8.15 mg/dl. In addition, sensitivity analysis showed that the pooled effects of metformin on serum lipid profiles were stable. Publication bias derived from funnel plots or Begg’s tests (P = 0.933, 0.860, 0.904, and 0.567 for HDL-c, LDL-c, total cholesterol, and triglyceride, respectively) was not significant. This meta-analysis revealed that metformin could reduce total cholesterol and LDL-c in nondiabetic adults. In addition, metformin might exert a triglyceride-lowering effect in nondiabetics with PCOS status.

Abstract:Obesity is becoming a health concern worldwide and metformin, a first line anti-diabetic drug, was associated with weight loss under different backgrounds. However, most researches focused on the anti-diabetic mechanism and less attention has been paid on the mechanism of weight loss of metformin. Therefore, we established a metabonomic method to evaluate metformin action in preventing obesity in a high fat diet-induced-obesity (DIO) mice model. 36 male C57BL/6 mice (8-week old) were randomly divided into control group (n=12, normal chow), model group (n=12, high fat chow) and metformin group (n=12, high fat chow and dosed with metformin) over 16 weeks. A urinary metabonomic study using UPLC-TOF/MS was performed in combination with multivariate statistical analysis. In addition, indices of body weight and food intake as well as fasting blood glucose, fed blood glucose, oral glucose tolerance test (OGTT) and plasma insulin were collected. Significant weight loss in metformin-treated mice was achieved and 21 potential biomarkers were identified. Decreased glucose, myristic acid, stearidonic acid, lysoPC (16:0), lysoPC (18:0), L-glutamic acid, L-methionine, L-threonine, L-phenylalanine, L-histidine, L-carnitine, L-malic acid and pantothenic acid in urine indicated that metformin may have exerted effects on energy metabolism. Further, based on the biomarkers, we cautiously propose that tricarboxylic acid cycle (TCA) may have been compromised by metformin and might contribute to the activation of adenosine monophosphate kinase (AMPK), then AMPK activation led to more β-oxidation of certain fatty acids and augmented lipolysis and thus induced weight loss. Related cellular and molecular studies are being considered to further investigate the underlying mechanism.
Effects of metformin on insulin sensitivity and islet beta cell secretion in obesity patients

Abstract:Objective To investigate the effects of metformin on insulin sensitivity and secretion in patients with obesity and insulin resistance. Methods This study enrolled 42 obesity patients with insulin resistance who were regularly followed-up in Peking Union Medical College Hospital from September 2012 to May 2016. They were divided into two groups according to their different status of glucose metabolism: normal glucose tolerance( NGT) and impaired glucose regulation( IGR) . Life style intervention and metformin were given to all these patients. The antropometric and metabolic data were collected before treatment, 3 and 6 months after treatment respectively. Results 42 patients, aged (23.6±6.5) years, including 11 males and 31 females were enrolled. 19 of them were NGT and 23 were IGR (8 of IGT and 15 of IFG) . Among all these patients, fasting insulin was significantly higher at 3 months after treatment(P＜0.05).The same results were shown in group-NGT(P＜0.05). Fasting insulin was significantly lower at 6 months after treatment than at baseline among all patients( P＜0.05) . HOMA-IR showed no significant difference between the baseline and 3 months after treatment, but significantly higher at baseline and 3 months after treatment than 6 months after therapy( P＜0.001) . HOMA-beta was significantly( P＜0.001) lower be-fore treatment and 6 months treatment the effect was more significant than 3 months after treatment among all pa-tients. HOMA-beta was significantly lower at baseline in group-IGR than at baseline in group-NGT ( P＜0.05) . Conclusions The effect of metformin on insulin secretion is earlier than that of improving the insulin sensitivity in patients with obesity and insulin resistance. Metformin is more likely to promote insulin secretion in patients with normal glucose tolerance than those with IGR within 3 months of intervention.
Treating Obesity Using the Combination of Metformin and Epigallocatechin-3-gallate

Abstract:Objective: This study aimed to investigate the anti-obesity effects of the combination of metformin and epigallocatechin-3-gallate (EGCG) in a diet-induced obese (DIO) model. Methods: Male C57BL/6J mice were fed with high-fat diet (HFD, 60% fat) to establish a DIO model. The DIO mice were administered intragastrically daily with vehicle, metformin (Met, 250 mg/kg), EGCG (200 mg/kg), or mixture of Met and EGCG (Met/EGCG; 250 and 200 mg/kg) for 32 days. Body weight was measured daily at a fixed time point. An intraperitoneal glucose tolerance test was performed on day 26 after treatments. Body composition was determined by using nuclear magnetic resonance technology. Adipose tissues and major organs were collected after culling. Gene expression in brown adipose tissue (BAT) and subcutaneous white adipose tissue (scWAT) were detected by quantitative polymerase chain reaction. Results: Compared with the vehicle group, the body weight was significantly reduced in DIO mice in the Met and Met/EGCG groups. The weight loss rate in the Met and EGCG groups were 9.1% and 1.0%, respectively. However, Met/EGCG induced a 14.0% body weight loss, suggesting that the combination of Met and EGCG has a synergistic effect on reversing the body weight in DIO mice. Met/EGCG demonstrated beneficial effects on the subcutaneous fat, BAT, and glucose homeostasis in DIO mice. Furthermore, Met/EGCG did not significantly increase the thermogenic gene expression in BAT, whereas in scWAT, Met/EGCG showed moderate effects on the expression of specific markers of browning fat or beige fat. Conclusion: Metformin and EGCG have combined treatment effect on the body weight of DIO mice. Their combination might be a promising treatment approach for obesity.

Abstract:Objective To observe effects of metformin on plasma adiponectin,leptin and resistin of hyperinsulinemic children and adolescents.Methods Fifty-four obese hyperinsulinemic children aged 6~17 years were enrolled in this trial and were divided into two groups:mild-moderate obesity and severe obesity group;forty-five healthy non-obese children aged 6.5~17 years were as control.The obese hyperinsulinemic children received metformin(175~250 mg twice or thrice daily)for 12 weeks.The plasma concentrations of plasma adiponectin,leptin and resistin were measured at baseline and after 12 weeks of metformin treatment for obesity groups.The fasting plasma glucose and fasting plasma insulin were also measured.Results Before metformin treatment,the HOMA-IR and the serum concentrations of insulin,leptin and resistin of the two obese hyperinsulinemic groups were significantly higher,while adiponectin was significantly lower compared with the contral group;there was no significant difference(P0.05)in serum fasting glucose between obese hyperinsulinemic children and the controls.After 12 weeks of treatment,serum insulin concentrations and insulin resistance index(HOMA-IR)decreased(P0.01);leptin levels decreased(mild and moderate obesity group 24.3±1.8μg/L vs 19.6±6.3μg/L,P0.05;severe obesity group 30.2±5.1μg/L vs 24.7±5.3μg/L,P0.01).The plasma resistin levels increased(mild and moderate obesity group 16.5±6.0 μg/L vs 22.0±5.1μg/L,P0.05;severe obesity group 22.3±5.2μg/L vs 30.6±11.7μg/L,P0.01).The plasma adiponectin didn't change greatly compared to the before(mild obesity and moderate obesity group 8.9±2.3 mg/L vs 8.4±3.2 mg/L,P0.05;severe obesity group 7.0±3.0 mg/L vs 6.5±1.2 mg/L,P0.05).Conclusion These data collectively suggest that metformin induces attenuation of hyperinsulinemia and insulin resistance in obese hyperinsulinemic children.The decrease of serum leptin levels may contribute to the insulin-sensitizing and antihyperglycemia function of metformin.On adipocytokines level,the effect of metformin is limited.

Abstract:There are conflicting data on the weight‐reducing potential of metformin (MTF) in nondiabetic patients with obesity. The purpose of this systematic review and meta‐analysis was to evaluate the effect of MTF on weight and cardiometabolic parameters in adults with overweight/obesity with or without nonalcoholic fatty liver disease (NAFLD) (CRD42018085512). We included randomized controlled trials (RCTs) in adults without diabetes mellitus, with mean body mass index (BMI) ≥ 25 kg/m2, with or without NAFLD, comparing MTF to placebo/control, lifestyle modification (LSM) or a US Food and Drug Administration‐approved anti‐obesity drug, reporting on weight or metabolic parameters, and extending over at least 3 months. We conducted a systematic search in MEDLINE, EMBASE, PubMed and the Cochrane Library without time limitation (until March 2022). We screened and selected eligible articles, abstracted relevant data, and assessed the risk of bias. All steps were in duplicate and independently. We conducted a random‐effects model meta‐analysis using Review Manager version 5.3, with prespecified subgroup analyses in case of heterogeneity. Effects of metformin and rosiglitazone on peripheral insulin resistance and β-cell function in patients with simple obesity

Abstract:Objective: To observe the therapeutic efficacy of metformin and rosiglitazone for insulin resistance and β-cell function in patients with simple obesity. Methods: Forty cases of simple obesity with hypertension, hypertriglyceridemia, hyperinsulinemia but normal sugar tolerance were divided into metformin and rosiglitazone treatment group and metformin treatment group, and then cured for 6 months and observed for their blood pressure, blood fat, body mass index (BMI) and insulin level before and after treatment. Results: Metformin and rosiglitazone could decrease blood pressure, blood fat, body mass index (BMI) and fasting insulin level obviously, and post-meal insulin changes were obvious (P < 0.05). In metformin treatment group , blood pressure and triglyceride and BMI were also decreased significantly (P < 0.05), but fasting and post-meal insulin level did not change so notably as those in metformin and rosiglitazone treatment group (P < 0.05). Conclusion: Metformin and rosiglitazone can increase insulin sensitivity and recover β-cell function in patients with simple obesity accompanied by insulin resistance, hypertension and hypertriglyceridemia, so the combination has an obvious therapeutic action in patients with simple obesity.

Abstract:Objective To evaluate the effect of metformin on adipose tissue distribution,hepatic lipid content,glucose and lipid metabolism,and insulin sensitivity in patients with newly-diagnosed type 2 diabetes. Methods A self-control clinical trial was conducted with 9 newly-diagnosed patients with type 2 diabetes.The patients were given oral metformin 750-1 500mg/d for 24 weeks.The body weight(Wt),body mass index(BMI), waist(W),hip(H),and waist hip ratio(WHR)were determined;the adipose tissue masses in abdomen and thigh were quantified by using magnetic resonance imaging(MRI);hepatic lipid content was measured by using ultrasound histogram;B-cell function was evaluated by using arginine stimulating test;meanwhile,FPG,2hPG, HbA_(1c),HOMA-IR and serum lipids were also assessed.Then paired t-test was performed to analyze the changes of parameters before and after treatment.Results Compared with before treatment,FPG,2hPG,and HbA_(1c), visceral adipose tissue(VAT),total abdominal adipose tissue(TAAT),and adipose tissue beneath the fascia (SFAT),Wt,and WHR were all significantly decreased 24 weeks after treatment(P<0.05).VAT decreased by 6.7% and TAAT by 7.9%.The decrease of hepatic lipid contents was not significant after treatment. Conclusion Metformin can not only improve glucose metabolism,but also decrease VAT,TAAT and SFAT in patients with type 2 diabetes.(Shanghai Med J,2007,30:676-679)

Abstract:Objective To investigate the effects of metformin and pioglitazone on plasma ghrelin in patients with newly diagnosed abnormal glucose metabolism and abdominal obesity.Methods From April to September 2010,39 patients with abnormal glucose metabolism (including impaired glucose tolerance and newly diagnosed type diabetes mellitus,glycated hemoglobin A1c(HbA1c) ＜7％ ) and obesity were enrolled in the study.They were randomly assigned to metformin group(500 mg tid,n =20,15 females and 5 males,aged (51±9) years,MET group) or pioglitazone group (30 mg/d,n =19,11 females and 8 males,aged (53 ±7) years,PIO group) on the basis of diet and exercise therapy for3 months.Three males in PIO group with drawed because of itching and edema.At baseline and after 3 months,plasma glucose,serum insulin and ghrelin levels during oral glucose tolerance test( OGTT,0 min,0.5,1.0,2.0,3.0 h),body weight,body mass index(BMI),HbA1c,etc were measured.The t test or Wilcoxon rank-sum test was used for statistical analysis. Partial correlation and multiple regression analysis were performed to examine the association between serum ghrelin levels and influencing factors.Results ( 1 ) In PIO group,serum ghrelin levels after treatment were OGTT 0 min (1069 ±467)ng/L,0.5 h (898 ±407)ng/L,1.0 h (812 ±371) ng/L,2.0 h (705 ± 328) ng/L and 3.0 h ( 1059 ± 606 ) ng/L,respectively.All increased than those before treatment ( (905 ±449),(688 ±378),(614 ±358),(572 ± 334)and(720 ±403)ng/L,Z value was from -2.354 to - 3.351,respectively,all P ＜ 0.05 ).(2) In MET group,serum ghrelin levels at OGTT 0 min,1.0,2.0,3.0 h after treatment were (731 ± 241 ),(642 ± 208 ),( 525 ± 195 ),(462 ± 146 ) and ( 611 ± 267 ) ng/L,respectively.Compared with the ghrelin levels before treatment( (731 ± 241 ),(642 ± 208 ),(525 ± 195 ),(462 ± 146) and (611 ± 267)ng/L),only the difference at 0 min,2.0,3.0 h was statistically significant (all P＜0.05).Serum ghrelin levels of MET group at OGTT 0.5,1.0 and 3.0 h were lower than that of PIO group ( Z value was - 2.012, - 2.006,- 2.226,respectively,all P ＜ 0.05 ).Conclusions Compared with those in PIO group,patients treated with metformin lose more weight,hut their serum ghrelin levels only partially increase at different point OGTT.This may be one of the reasons why metformin controls weight better.

Abstract:Background and aims To analyze the effect of oral metformin on changes in gut microbiota characteristics and metabolite composition in normal weight type 2 diabetic patients. Methods T2DM patients in the cross-sectional study were given metformin for 12 weeks. Patients with unmedicated T2DM were used as a control group to observe the metrics of T2DM patients treated with metformin regimen. 16S rDNA high-throughput gene sequencing of fecal gut microbiota of the study subjects was performed by llumina NovaSeq6000 platform. Targeted macro-metabolomics was performed on 14 cases of each of the gut microbiota metabolites of the study subjects using UPLC-MS/MS technology. Correlations between the characteristics of the gut microbiota and its metabolites, basic human parameters, glycolipid metabolism indicators, and inflammatory factors were analyzed using spearman analysis. Results Glycolipid metabolism indexes and inflammatory factors were higher in normal-weight T2DM patients than in the healthy population (P<0.05), but body weight, BMI, waist circumference, and inflammatory factor concentrations were lower in normal-weight T2DM patients than in obese T2DM patients (P<0.05). Treatment with metformin in T2DM patients improved glycolipid metabolism, but the recovery of glycolipid metabolism was more pronounced in obese T2DM patients. None of the differences in α-diversity indexes were statistically significant (P>0.05), and the differences in β-diversity were statistically significant (P <0.05). Community diversity and species richness recovered after metformin intervention compared to before, and were closer to the healthy population. We found that Anaerostipes/Xylose/Ribulose/Xylulose may play an important role in the treatment of normal-weight T2DM with metformin by improving glycemic lipids and reducing inflammation. And Metformin may play a role in obese T2DM through Romboutsia, medium-chain fatty acids (octanoic acid, decanoic acid, and dodecanoic acid). Conclusion Gut microbial dysbiosis and metabolic disorders were closely related to glucose-lipid metabolism and systemic inflammatory response in normal-weight T2DM patients. Metformin treatment improved glucose metabolism levels, systemic inflammation levels in T2DM patients, closer to the state of healthy population. This effect may be mediated by influencing the gut microbiota and microbial host co-metabolites, mainly associated with Anaerostipes and xylose/Ribulose/Xylulose. Metformin may exert its effects through different pathways in normal-weight versus obese T2DM patients.

Abstract:Non-alcoholic fatty liver disease (NAFLD) is closely associated with obesity and insulin resistance. To better understand the pathophysiology of obesity-associated NAFLD, the present study examined the involvement of liver and adipose tissues in metformin actions on reducing hepatic steatosis and inflammation during obesity. C57BL/6J mice were fed a high-fat diet (HFD) for 12 weeks to induce obesity-associated NAFLD and treated with metformin (150 mg/kg/d) orally for the last four weeks of HFD feeding. Compared with HFD-fed control mice, metformin-treated mice showed improvement in both glucose tolerance and insulin sensitivity. Also, metformin treatment caused a significant decrease in liver weight, but not adiposity. As indicated by histological changes, metformin treatment decreased hepatic steatosis, but not the size of adipocytes. In addition, metformin treatment caused an increase in the phosphorylation of liver AMP-activated protein kinase (AMPK), which was accompanied by an increase in the phosphorylation of liver acetyl-CoA carboxylase and decreases in the phosphorylation of liver c-Jun N-terminal kinase 1 (JNK1) and in the mRNA levels of lipogenic enzymes and proinflammatory cytokines. However, metformin treatment did not significantly alter adipose tissue AMPK phosphorylation and inflammatory responses. In cultured hepatocytes, metformin treatment increased AMPK phosphorylation and decreased fat deposition and inflammatory responses. Additionally, in bone marrow-derived macrophages, metformin treatment partially blunted the effects of lipopolysaccharide on inducing the phosphorylation of JNK1 and nuclear factor kappa B (NF-κB) p65 and on increasing the mRNA levels of proinflammatory cytokines. Taken together, these results suggest that metformin protects against obesity-associated NAFLD largely through direct effects on decreasing hepatocyte fat deposition and on inhibiting inflammatory responses in both hepatocytes and macrophages.

Abstract:Background: Metformin has been reported to possess anti-cancer properties in addition to glucose-lowering activity and numerous systematic reviews and meta-analyses have studied the association between metformin use and cancer incidence or survival outcomes. We performed an umbrella review to assess the robustness of these associations to facilitate proper interpretation of these results to inform clinical and policy decisions. Methods: We searched PubMed and Embase systematic reviews and meta-analyses investigating the effect of metformin use on cancer incidence or survival outcomes published from inception to September 2, 2018. We estimated the summary effect size, the 95% CI, and the 95% prediction interval, heterogeneity, evidence of small-study effects, and evidence of excess significance bias. Results: We included 21 systematic reviews and meta-analyses covering 11 major anatomical sites and 33 associations. There was strong evidence for the association between metformin use and decreased pancreatic cancer incidence. The association between metformin use and improved colorectal cancer overall survival (OS) was supported by highly suggestive evidence. Seven associations (all cancer incidence, all cancer OS, breast cancer OS, colorectal cancer incidence, liver cancer incidence, lung cancer OS, and pancreatic cancer OS) presented only suggestive evidence. The remaining 24 associations were supported by weak or not-suggestive evidence. Conclusions: Associations between metformin use and pancreatic cancer incidence or colorectal cancer OS are supported by strong or highly suggestive evidence, respectively. However, these results should be interpreted with caution due to the poor methodological quality of the systematic reviews and meta-analyses.

Abstract:ABSTRACT Objectives The present study aimed to investigate the effects of metformin on body weight in polycystic ovary syndrome (PCOS) patients by model-based meta-analysis (MBMA). Methods Randomized controlled trial (RCT) studies of metformin in treatment of PCOS patients were collected and efficacy indicators were the body mass index (BMI) change rates from baseline. Results 28 RCT studies, 1631 PCOS patients were included for analysis. Model of metformin monotherapy, and model of metformin combination therapy were established, respectively. After deducting control group effects, the Emax of metformin on BMI were −5.88% and −11.8% in metformin monotherapy and metformin combination therapy, respectively. In order to play better curative effects, for metformin monotherapy, 1000 mg/day metformin needs for at least 25.5 weeks; For metformin combination therapy, 1000 mg/day metformin needs for at least 58.6 weeks. Conclusion It was the first time to quantify the effects of metformin on body weight and recommend dose and duration for metformin monotherapy and metformin combination therapy in PCOS patients.
