Abstract:Background: In the spring of 1918, the "War to End All Wars", which would ultimately claim more than 37 million lives, had entered into its final year and would change the global political and economic landscape forever. At the same time, a new global threat was emerging and would become one of the most devastating global health crises in recorded history. Main text: The 1918 H1N1 pandemic virus spread across Europe, North America, and Asia over a 12-month period resulting in an estimated 500 million infections and 50-100 million deaths worldwide, of which ~ 50% of these occurred within the fall of 1918 (Emerg Infect Dis 12:15-22, 2006, Bull Hist Med 76:105-115, 2002). However, the molecular factors that contributed to the emergence of, and subsequent public health catastrophe associated with, the 1918 pandemic virus remained largely unknown until 2005, when the characterization of the reconstructed pandemic virus was announced heralding a new era of advanced molecular investigations (Science 310:77-80, 2005). In the century following the emergence of the 1918 pandemic virus we have landed on the Moon, developed the electronic computer (and a global internet), and have eradicated smallpox. In contrast, we have a largely remedial knowledge and understanding of one of the greatest scourges in recorded history. Conclusion: Here, we reflect on the 1918 influenza pandemic, including its emergence and subsequent rapid global spread. In addition, we discuss the pathophysiology associated with the 1918 virus and its predilection for the young and healthy, the rise of influenza therapeutic research following the pandemic, and, finally, our level of preparedness for future pandemics.

Abstract:This year, in 2018, we mark 100 years since the 1918 influenza pandemic. In the last 100 years, we have expanded our knowledge of public health and increased our ability to detect and prevent influenza; however, we still face challenges resulting from these continually evolving viruses. Today, it is clear that influenza viruses have multiple animal reservoirs (domestic and wild), making infection prevention in humans especially difficult to achieve. With this report, we summarize new knowledge regarding influenza A, B, C and D viruses and their control. We also introduce how a multi-disciplinary One Health approach is necessary to mitigate these threats.

Abstract:Humanity has experienced four major pandemics since the twentieth century, with the 1918 Spanish flu, the 2002 severe acute respiratory syndrome (SARS), the 2009 swine flu, and the 2019 coronavirus disease (COVID)-19 pandemics having the most important impact in human health. The 1918 Spanish flu caused unprecedented catastrophes in the recorded human history, with an estimated death toll between 50 – 100 million. While the 2002 SARS and 2009 swine flu pandemics caused approximately 780 and 280,000 deaths, respectively, the current COVID-19 pandemic has resulted in > 6 million deaths globally at the time of writing. COVID-19, instigated by the SARS – coronavirus-2 (SARS-CoV-2), causes unprecedented challenges in all facets of our lives, and never before brought scientists of all fields together to focus on this singular topic. While for the past 50 years research have been heavily focused on viruses themselves, we now understand that the host immune responses are just as important in determining the pathogenesis and outcomes of infection. Research in innate immune mechanisms is crucial in understanding all aspects of host antiviral programmes and the mechanisms underpinning virus-host interactions, which can be translated to the development of effective therapeutic avenues. This review summarizes what is known and what remains to be explored in the innate immune responses to influenza viruses and SARS-CoVs, and virus-host interactions in driving disease pathogenesis. This hopefully will encourage discussions and research on the unanswered questions, new paradigms, and antiviral strategies against these emerging infectious pathogens before the next pandemic occurs.

Abstract:BACKGROUND:A severe seasonal influenza epidemic was observed during 2017-2018 in China, prompting questions on clinical characteristics and outcomes of severe cases with influenza. METHODS:We retrospectively collected clinical data and outcomes of laboratory-confirmed hospitalized patients (severe to critical) during Jan-2011 to Feb-2018 from five hospitals, followed by a systematic analysis of cases from 2017 to 2018 (n = 289) and all previous epidemics during 2011-2017 (n = 169). RESULTS:In-hospital fatality was over 5-folds higher during the 2017-2018 (p < 0.01) in which 19 patients died (6.6%), whereas only 2 mortalities (1.2%) were observed during 2011-2017. Of the 289 hospitalized in 2017-2018, 153 were confirmed with influenza B virus, 110 with A/H1N1pdm09, and 26 A/H3N2, whereas A/H1N1pdm09 was the predominant cause of hospitalization in previous seasons combined (45%). Fatal cases in 2017-2018 were exclusively associated with either influenza B or A/H1N1pdm09. Our results show that a significant lower proportion of patients aged 14 or greater were treated with oseltamivir, during the 2017-2018 epidemic, and exhibited higher levels of clinical severity. CONCLUSIONS:In-hospital fatality rate might be significantly higher in the 2017-2018 season in China. A sufficient supply of oseltamivir and antiviral therapy within 48 h from onset could reduce fatality rates.

Abstract:Influenza remains an important disease in humans and animals. In contrast to measles, smallpox and poliomyelitis, influenza is caused by viruses that undergo continuous antigenic change and that possess an animal reservoir. Thus, new epidemics and pandemics are likely to occur in the future, and eradication of the disease will be difficult to achieve. Although it is not clear whether a new pandemic is imminent, it would be prudent to take into account the lessons we have learned from studying different human and animal influenza viruses. Specifically, reconstruction of the genes of the 1918 pandemic virus and studies on their contribution to virulence will be important steps toward understanding the biological capabilities of this lethal virus. Increasing the availability of new antiviral drugs and developing superior vaccines will provide us with better approaches to control influenza and to have a positive impact on disease load. A concern is that the imposition of new rules for working with infectious influenza viruses under high security and high containment conditions will stifle scientific progress. The complex questions of what makes an influenza virus transmissible from one human to another and from one species to another, as well as how the immune system interacts with the virus, will require the active collaboration and unencumbered work of many scientific groups.

Abstract:Chinese government had adopted strict measurements in the earlier prevalence of novel H1N1 influenza virus. As with the coming of the pandemic, the control strategy had been modified. Many controversies had been aroused among the professionals, the officials and the publics. As influenza was mainly transmitted through the respiratory tract, it was very difficult to cut off the transmitting pathway. In addition, the transmissibility of the novel H1N1 virus was almost the same as that of seasonal influenza. The whole population was more susceptible to it and the complete control of the infective source was impossible. The aim of the present strategy was to inhibit the peak of the pandemic and retard the spread speed of the virus. The main task of medical service was to reduce the mortality rate of the pandemic so as to do the best to decrease the negative influence on the whole society.

Abstract:This paper described the epidemiology and controlling experiences of influenza H1N1 in Hangzhou in the past 1 year. A total of 2,078 cases confirmed by real‐time quantitative PCR till March 31, 2010, were analyzed by SPSS 12.0 software. During the early pandemic stage, a patient must be tested for H1N1 nucleic acid once he/she had influenza‐like symptoms with the epidemiological history in 7 days, and be diagnosed if it was positive. But in the pandemic peak, we made efforts to identify and save severe cases combined with pneumonia or hypoxemia or respiratory failure or septic shock or multiple organ dysfunctions and failure. In general, the prevalence was 2.77/100,000 (2,078/7,510,844); severity rate, 10.44% (217/2,078); fatality rate, 0.48% (10/2,078). The carrier and secondary attack rate were 9.52% (58/612) and 8.66% (53/612), respectively. About 50% of serious cases and 100% of deaths had the basic underlying diseases: cardiovascular diseases, 13.66% (25/217); chronic lung disease, 12.02% (22/217); pregnant, 7.1% (13/217). Of all cases aged from 1 month to 89 years, 52.99% (1,435/2,708) were in the 10–29 years, with most of them distributed in downtown area. The timeline showed two epidemic peaks occurred in September and November 2009, respectively. Furthermore, the hemagglutinin gene remained 99% identical with the American and vaccine strains, but only 70% with the 1947–2008 Chinese strains. In conclusion, Hangzhou pandemic influenza H1N1 was caused by the highly conserved virus, with low prevalence, transmission, and mortality, because we took efficient controlling methods. J. Med. Abstract:▪ Abstract Pandemics are the most dramatic presentation of influenza. Three have occurred in the twentieth century: the 1918 H1N1 pandemic, the 1957 H2N2 pandemic, and the 1968 H3N2 pandemic. The tools of molecular epidemiology have been applied in an attempt to determine the origin of pandemic viruses and to understand what made them such successful pathogens. An excellent example of this avenue of research is the recent phylogenetic analysis of genes of the virus that caused the devastating 1918 pandemic. This analysis has been used to identify evolutionarily related influenza virus genes as a clue to the source of the pandemic of 1918. Molecular methods have been used to investigate the avian H5N1 and H9N2 influenza viruses that recently infected humans in Hong Kong. Antigenic, genetic, and epidemiologic analyses have also furthered our understanding of interpandemic influenza. Although many questions remain, advances of the past two decades have demonstrated that several widely held concepts concerning the global epidemiology of influenza were false.

Abstract:Abstract There has been a resurgence of the H7N9 bird-flu virus in China since last winter, resulting in over 460 human infections—the largest number since the first outbreak in 2013—raising serious concerns about its further spread and the effectiveness of existing anti-viral drugs. This is just the latest example of the increasing threat from emerging infectious diseases. Due to a combination of factors related to farming practices, human behaviour, international travel, globalization and climate change, there has been a succession of such pandemics in recent years, such as Severe Acute Respiration Syndrome (SARS), Nipah, Middle East Respiratory Syndrome (MERS), Ebola and Zika, posing an unprecedented challenge to scientists and health workers worldwide. Abstract:Influenza is an acute respiratory disease in mammals and domestic poultry that emerges from zoonotic reservoirs in aquatic birds and bats. Although influenza viruses are among the most intensively studied pathogens, existing control options require further improvement. Influenza vaccines must be regularly updated because of continuous antigenic drift and sporadic antigenic shifts in the viral surface glycoproteins. Currently, influenza therapeutics are limited to neuraminidase inhibitors; novel drugs and vaccine approaches are therefore urgently needed. Advances in vaccinology and structural analysis have revealed common antigenic epitopes on hemagglutinins across all influenza viruses and suggest that a universal influenza vaccine is possible. In addition, various immunomodulatory agents and signaling pathway inhibitors are undergoing preclinical development. Continuing challenges in influenza include the emergence of pandemic H1N1 influenza in 2009, human infections with avian H7N9 influenza in 2013, and sporadic human cases of highly pathogenic avian H5N1 influenza. Here, we review the challenges facing influenza scientists and veterinary and human public health officials; we also discuss the exciting possibility of achieving the ultimate goal of controlling influenza's ability to change its antigenicity.

Abstract:Infection with either influenza A H5N1 virus in 1997 or avian influenza A H7N9 virus in 2013 caused severe pneumonia that did not respond to typical or atypical antimicrobial treatment, and resulted in high mortality. Both viruses are reassortants with internal genes derived from avian influenza A H9N2 viruses that circulate in Asian poultry. Both viruses have genetic markers of mammalian adaptation in their haemagglutinin and polymerase PB2 subunits, which enhanced binding to human-type receptors and improved replication in mammals, respectively. Hong Kong (affected by H5N1 in 1997) and Shanghai (affected by H7N9 in 2013) are two rapidly flourishing cosmopolitan megacities that were increasing in human population and poultry consumption before the outbreaks. Both cities are located along the avian migratory route at the Pearl River delta and Yangtze River delta. Whether the widespread use of the H5N1 vaccine in east Asia-with suboptimum biosecurity measures in live poultry markets and farms-predisposed to the emergence of H7N9 or other virus subtypes needs further investigation. Why H7N9 seems to be more readily transmitted from poultry to people than H5N1 is still unclear.

Abstract:The world is still in heightened awareness of the potential threat of another influenza pandemic, although it has been 5 years since the 2009 influenza A H1N1 pandemic. Evaluation of the adopted intervention strategies for handling the 2009 H1N1 pandemic is helpful for dealing with future outbreaks. In this paper we developed a hybrid model combining meta-population and agent-based models to evaluate the containment and mitigation strategies e.g., contact tracing and quarantine of contacts at assembly sites in the early phase, more medical institutions to detect cases and treat serious patients in the developing phase, and rapid vaccines delivered to students first for an H1N1 pandemic that were adopted in China. We find that the presented model can retrospectively fit relatively well to the spreading progress through comparison of the simulation results with actual infections data, and it can be used for practical application in evaluating the containment and mitigation strategies for an influenza pandemic if model validation and parameter estimation can be conducted by using actual data. The results will contribute to understanding the spread of viruses and the control of infectious diseases, and to helping government officials create policies on handling an influenza pandemic, especially beneficial to a large and diverse country such as the People's Republic of China.

Abstract:Between March and April, 2009 a new strain of influenza capable of causing severe disease and mortality in humans was identified in North America, leading to the declaration by the World Health Organization (WHO) of the first pandemic of the 21st Century. The prompt intervention of World Health authorities allowed the early identification of the virus as triple assortment A(H1N1) derived from avian, swine and human viruses, and a descendant of the A(H1N1) virus that caused the 1918 influenza pandemic. International reports reflect a homogeneous disease that affects mainly the young, with severe cases in chronic patients, small children, the elderly, and pregnant women. Currently, the whole world, under the leadership of the WHO, is implementing pharmacological and non-pharmacological interventions to try and stop viral transmission. There are still many questions unanswered, particularly about the future behavior of the virus. Nevertheless, until now the world has demonstrated the capacity to successfully face, in a coordinated fashion, one of the greatest known threats to international public health.

Abstract:COVID-19 has been a topic of interest since a pandemic struck in 2019. The morbidity of influenza tended to decrease due to the measures to prevent COVID-19. Indeed, influenza seems to be "ignored" in this era of COVID-19. However, influenza has not disappeared from the scene. Presented here are two examples of recent influenza epidemics in China and Australia. Possible interactions between COVID-19 and influenza are discussed. Measures against COVID-19 may reduce contact with influenza, subsequently reducing adaptive immunity against influenza in the general population. Influenza might not be center stage right now, but insufficient adaptive immunity in the population may potentially trigger a future influenza pandemic. Coinfection with COVID-19 and influenza might potentially be a thorny problem. Hence, influenza cannot be ignored. Governments around the world should take measures to prepare for a possible influenza pandemic in the future.

Abstract:Respiratory infectious diseases have long been a serious public health problem. This study explores the significance of respiratory infectious disease prevention and control systems worldwide, particularly during and after the COVID-19 pandemic. The epidemiology of many respiratory diseases such as influenza changed over the past two years, and the incidence of pathogens such as Mycoplasma pneumoniae and Bordetella pertussis has also increased. Based on influenza surveillance data in China, the influenza season in 2023 was notably delayed and extended, with A(H1N1) pdm09 being the predominant strain. This editorial also reviewed the gradual establishment of China's infectious disease prevention and control system following the 2003 SARS outbreak, highlighting the role of medical facilities in managing public health emergencies, conducting infectious disease pre-screening, and reporting cases to networks in real time. In the future, China will further develop an intelligent, multi-trigger infectious disease surveillance and early warning system to increase the early detection of unknown infectious diseases and optimize the allocation of medical resources. A robust infectious disease control system is crucial to addressing future pandemic threats.

Abstract:During April 2009, a novel H1N1 influenza A virus strain was identified in Mexico and the USA. Within weeks the virus had spread globally and the first pandemic of the 21st Century had been declared. It is unlikely to be the last and it is crucial that real lessons are learned from the experience. Asia is considered a hot spot for the emergence of new pathogens including past influenza pandemics. On this occasion while preparing for an avian, highly virulent influenza virus (H5N1 like) originating in Asia in fact the pandemic originated from swine, and was less virulent. This discrepancy between what was planned for and what emerged created its own challenges. The H1N1 pandemic has tested national health-care infrastructures and exposed shortcomings in our preparedness as a region. Key health challenges include communication throughout the region, surge capacity, access to reliable information and access to quality care, healthcare worker skills, quality, density and distribution, access to essential medicines and lack of organizational infrastructure for emergency response. Despite years of preparation the public health and clinical research community were not ready to respond and opportunities for an immediate research response were missed. Despite warm words and pledges efforts to engage the international community to ensure equitable sharing of limited resources such as antivirals and vaccines fell short and stockpiles in the main remained in the rich world.
