Abstract:Deep brain stimulation (DBS) has been used to treat disorders in clinical practice, such as Parkinson's disease of central nervous system. However, the mechanisms of DBS are not clear yet. In order to investigate the changes of action potential (i. e., unit spike) firings of individual neurons during the periods of electrical stimulation, we designed an interpolation method to remove stimulation artifacts together with evoked potentials with large amplitudes. Thresholds of signal slope were used to determine the end points of interpolation segment thereby minimizing the non-usable period (NP) of spike detection. The method was used to analyze the spike activity during either orthodromic or antidromic stimulation of hippocampal CA1 region in rat brain. Results showed that the NP immediately following stimulation could be shortened by ∼8 ms using the method. A further decrease of the influence was achieved by using spline interpolation instead of linear interpolation. The novel method can be used to detect spike signals during the periods of high frequency stimulation efficiently, providing a simple and practicable method to investigate the neuronal activity during DBS.

Abstract:Background/Objective: Non-invasive neuromodulation techniques, such as repetitive Transcranial Magnetic Stimulation (rTMS) and transcranial Direct Current Stimulation (tDCS), have increasingly been investigated for their potential as treatments for neurological and psychiatric disorders. Despite widespread dissemination of these techniques, the underlying therapeutic mechanisms and the ideal stimulation site for a given disorder remain unknown. Increasing evidence support the possibility of non-invasive neuromodulation affecting a brain network rather than just the local stimulation target. In this article, we present evidence in a clinical setting to support the idea that non-invasive neuromodulation changes brain networks. Method: This article addresses the idea that non-invasive neuromodulation modulates brain networks, rather than just the local stimulation target, using neuromodulation studies in tinnitus and major depression as examples. We present studies that support this hypothesis from different perspectives. Main Results/Conclusion: Studies stimulating the same brain region, such as the dorsolateral prefrontal cortex (DLPFC), have shown to be effective for several disorders and studies using different stimulation sites for the same disorder have shown similar results. These findings, as well as results from studies investigating brain network connectivity on both macro and micro levels, suggest that non-invasive neuromodulation affects a brain network rather than just the local stimulation site targeted. We propose that non-invasive neuromodulation should be approached from a network perspective and emphasize the therapeutic potential of this approach through the modulation of targeted brain networks.

[Design of a System for Real-Time Seizure Detection and Closed-Loop Electrical Stimulation].

Abstract:In order to investigate the effect of deep brain stimulation on diseases such as epilepsy, we developed a closed-loop electrical stimulation system using LabVIEW virtual instrument environment and NI data acquisition card. The system was used to detect electrical signals of epileptic seizures automatically and to generate electrical stimuli. We designed a novel automatic detection algorithm of epileptic seizures by combining three features of field potentials: the amplitude, slope and coastline index. Experimental results of rat epileptic model in the hippocampal region showed that the system was able to detect epileptic seizures with an accuracy rate 91.3% and false rate 8.0%. Furthermore, the on-line high frequency electrical stimuli showed a suppression effect on seizures. In addition, the system was adaptive and flexible with multiple work modes, such as automatic and manual modes. Moreover, the simple time-domain algorithm of seizure detection guaranteed the real-time feature of the system and provided an easy-to-use equipment for the experiment researches of epilepsy control by electrical stimulation.

Abstract:Deep brain stimulation (DBS) has shown wide applications for treating various disorders in the central nervous system by using high frequency stimulation (HFS) sequences of electrical pulses. However, upon the onset of HFS sequences, the narrow pulses could induce synchronous firing of action potentials among large populations of neurons and cause a transient phase of "onset response" that is different from the subsequent steady state. To investigate the transient onset phase, the antidromically-evoked population spikes (APS) were used as an electrophysiological marker to evaluate the synchronous neuronal reactions to axonal HFS in the hippocampal CA1 region of anesthetized rats. New stimulation paradigms with time-varying intensity and frequency were developed to suppress the "onset responses". Results show that HFS paradigms with ramp-up intensity at the onset phase could suppress large APS potentials. In addition, an intensity ramp with a slower ramp-up rate or with a higher pulse frequency had greater suppression on APS amplitudes. Therefore, to reach a desired pulse intensity rapidly, a stimulation paradigm combining elevated frequency and ramp-up intensity was used to shorten the transition phase of initial HFS without evoking large APS potentials. The results of the study provide important clues for certain transient side effects of DBS and for development of new adaptive stimulation paradigms.

Abstract:Deep brain stimulation (DBS) has become one of the common treatments for movement disorders such as Parkinson's disease. It is also a promising treatment for other neurological and psychiatric disorders. However, the stimulation paradigms of regular DBS are not diversified enough to meet the needs for extending its applications, because current DBS therapy usually utilizes single-channel high-frequency stimulation of electrical pulses with a constant inter-pulse-interval. According to the desynchronization mechanisms of DBS, new stimulation paradigms have been developed by designing the temporal and spatial patterns of pulse sequences to improve the therapeutic efficacy, to extend its application and to save the electric energy of impulse generator. The new paradigms include: stimulations of varying-frequency sequences (including regular varying-frequency and random varying-frequency), multi-channel asynchronous stimulations at different spatial locations, as well as the combined stimulations of varying-frequency and multi-channels. These new paradigms may improve the efficacy of DBS and reduce the energy consumption thereby showing good prospects in treating Parkinson's disease and other brain diseases such as epilepsy, obsessive-compulsive and minimally conscious state. Notably, in addition to the better acute effect during the stimulations, the efficacy of multi-channel stimulations may last for a long time after the termination of stimulations, indicating a sustained effect or an after-effect. This performance breaks the limitation of regular DBS that has only acute effects and opens out a new prospect for DBS. Based on a summary of the stimulation paradigms of regular DBS and their underlying mechanisms, this review presents the development of new DBS paradigms on the stimulations of varying-frequency and multi-channels to generate asynchronous effects, thereby providing valuable information for the development of DBS.
Design and Application of Neural Electrical Stimulation System with Time-Varying Parameters

Abstract:Currently, commercial devices for electrical neural stimulations can only provide fixed stimulation paradigms with preset constant parameters, while the development of new stimulation paradigms with time-varying parameters has emerged as one of the important research directions for expanding clinical applications. To facilitate the performance of electrical stimulation paradigms with time-varying parameters in animal experiments, the present study developed a well-integrated stimulation system to output various pulse sequences by designing a LabVIEW software to control a general data acquisition card and an electrical stimulus isolator. The system was able to generate pulse sequences with inter-pulse-intervals (IPI) randomly varying in real time with specific distributions such as uniform distribution, normal distribution, gamma distribution and Poisson distribution. It was also able to generate pulse sequences with arbitrary time-varying IPIs. In addition, the pulse parameters, including pulse amplitude, pulse width, interphase delay of biphasic pulse and duration of pulse sequence, were adjustable. The results of performance tests of the stimulation system showed that the errors of the parameters of pulse sequences output by the system were all less than 1%. By utilizing the stimulation system, pulse sequences with IPI randomly varying in the range of 5~10 ms were generated and applied in rat hippocampal regions for animal experiments. The experimental results showed that, even with a same mean pulse frequency of ~130 Hz, for neuronal populations, the excitatory effect of stimulations with randomly varying IPIs was significantly greater than the effect of stimulations with fixed IPIs. In conclusion, the stimulation system designed here may provide a useful tool for the researches and the development of new paradigms of neural electrical stimulations.

Abstract:In order to exert electrical stimulation at a given phase of brain electric signal waveforms, a closed-loop system was developed using a general NI data acquisition card together with LabVIEW virtual instrument environment. One of the A/D channels on the card is used to record the field potential signals from the hippocampal region in a rat brain. A customized LabVIEW program analyzes the amplitude and frequency features of the signals, and then predicts the upcoming peak or trough of the theta rhythms in the field potentials. Finally, stimulation waveforms are outputted from a digital I/O port or a D/A channel of the card exactly at the expected time phase to realize the closed-loop stimulation. Experimental results show that the percent prediction accuracies are 92% and 86% for peak and trough predictions, respectively. The electrical stimulation system is versatile and flexible, and can be used for predicting various rhythm phases and other features appeared in neural signals. It provides a novel closed-loop control method for neural stimulation both in neuroelectrophysiology researches and in electrical stimulation therapy of brain diseases.

Abstract:With the development and characterization of biomarkers that may reflect neural network state as well as a patient's clinical deficits, there is growing interest in more complex stimulation designs. While current implantable neuromodulation systems offer pathways to expand the design and application of adaptive stimulation paradigms, technological drawbacks of these systems limit adaptive neuromodulation exploration. In this paper, we discuss the implementation of a phase-triggered stimulation paradigm using a research platform composed of an investigational system known as the CorTec Brain Interchange (CorTec GmbH, Freiburg, Germany), and an open-source software tool known as OMNI-BIC. We then evaluate the stimulation paradigm's performance in both benchtop and in vivo human demonstrations. Our findings indicate that the Brain Interchange and OMNI-BIC platform is capable of reliable administration of phase-triggered stimulation and has the potential to help expand investigation within the adaptive neuromodulation design space.

An Operational Approach for Optimizing Transcranial Direct Current Stimulation

Abstract:Transcranial direct current stimulation (tDCS) is a non-invasive brain stimulation technique that has been utilized for treating brain disorders and improving cognitive function. In order to achieve targeted tDCS, many optimization methods of montages and electric currents have been proposed. However, these methods have some limitations. Most of them were proposed for single-objective optimization (focality or intensity) and have no constrain with the number of electrodes (Most devices only have less than 8 electrodes currently). In this study, we proposed an operational optimization approach for well-targeted tDCS, which aims to optimize for two objectives of electric field (EF) intensity and focality with constraints on the number of electrodes. Compared with traditional tDCS in our cohort (10 subjects), our method significantly improves the EF focality. When compared to commonly used 4×1 high-definition tDCS (HD-tDCS), our method can achieve higher EF intensity in the target region with less than 8 electrodes. Our method can balance the two objectives of EF and shorten optimization time, which is convenient for practical application.

Abstract:The ability to modulate brain states using targeted stimulation is increasingly being employed to treat neurological disorders and to enhance human performance. Despite the growing interest in brain stimulation as a form of neuromodulation, much remains unknown about the network-level impact of these focal perturbations. To study the system wide impact of regional stimulation, we employ a data-driven computational model of nonlinear brain dynamics to systematically explore the effects of targeted stimulation. Validating predictions from network control theory, we uncover the relationship between regional controllability and the focal versus global impact of stimulation, and we relate these findings to differences in the underlying network architecture. Finally, by mapping brain regions to cognitive systems, we observe that the default mode system imparts large global change despite being highly constrained by structural connectivity. This work forms an important step towards the development of personalized stimulation protocols for medical treatment or performance enhancement.

Abstract:Transcranial temporal interference stimulation (tTIS) has been proposed as a new neuromodulation technology for non-invasive deep-brain stimulation (DBS). However, few studies have detailed the design method of a tTIS device and provided system validation. Thus, a detailed design and validation scheme of a novel tTIS device for animal brain stimulation are presented in this study. In the proposed tTIS device, a direct digital synthesizer (DDS) was used to generate a sine wave potential of different frequencies, which was converted to an adjustable sine wave current. A current transformer was used to produce electrical isolation of different channels, which eliminated the current crosstalk between channels and greatly increased the load capacity by amplifying the output voltage. Several in vitro experiments were first conducted to validate the tTIS device. Our results indicated that the error percentages of the stimulation currents were within ±2%. Current crosstalk between channels was almost completely eliminated. Then, in vivo electric field measurement shows that the 2-pole arrangement may provide better cortical targeting than the 4-pole mode. A pilot animal experiment was conducted in which evoked motion and electromyographic activation of the contralateral forelimb were observed, which indicated that the 2-pole tTIS had successfully activated the primary motor cortex in a rat. Motor activation induced by the 2-pole tTIS demonstrated the feasibility and safety potential when applying our tTIS device for neuromodulation.

Abstract:Methods by which to achieve non-invasive deep brain stimulation via temporally interfering with electric fields have been proposed, but the precision of the positioning of the stimulation and the reliability and stability of the outputs require improvement. In this study, a temporally interfering electrical stimulator was developed based on a neuromodulation technique using the interference modulation waveform produced by several high-frequency electrical stimuli to treat neurodegenerative diseases. The device and auxiliary software constitute a non-invasive neuromodulation system. The technical problems related to the multichannel high-precision output of the device were solved by an analog phase accumulator and a special driving circuit to reduce crosstalk. The function of measuring bioimpedance in real time was integrated into the stimulator to improve effectiveness. Finite element simulation and phantom measurements were performed to find the functional relations among the target coordinates, current ratio, and electrode position in the simplified model. Then, an appropriate approach was proposed to find electrode configurations for desired target locations in a detailed and realistic mouse model. A mouse validation experiment was carried out under the guidance of a simulation, and the reliability and positioning accuracy of temporally interfering electric stimulators were verified. Stimulator improvement and precision positioning solutions promise opportunities for further studies of temporally interfering electrical stimulation.

Abstract:Deep brain stimulation (DBS) has been applied widely in clinic to treat movement disorders such as Parkinson's disease. It also shows great prospects in the treatments of intractable epilepsy, refractory obsessive-compulsive disorder, and other diseases of the central nervous system in brain. Over the past three decades, the mechanisms of DBS have emerged gradually based on multiple lines of evidence in clinical applications, in animal experiments, and in simulations of computational models. Important advances in DBS mechanisms have been achieved although final conclusions are still under debate. This review analyzes and summarizes the development of DBS theories from electrophysiological perspectives: from the original theory of inhibitions or excitations to the recent prevalent theory of modulations; from focusing on neuronal activity locally at stimulation sites to discovering the decoupling of somatic and axonal responses, and further to discovering the intermittent depolarization block of axons induced by high frequency stimulations, together with de-synchronous activity presumably caused by axonal activity in population neurons in the projection brain regions. The series of advances indicate that DBS has complex mechanisms in modulating the neuronal networks. Understanding the mechanisms of DBS has significance for improving DBS therapies, for developing new stimulation modes, and for extending its clinical applications.
Neuroimaging evaluation of deep brain stimulation in the treatment of representative neurodegenerative and neuropsychiatric disorders

Abstract:Brain stimulation technology has become a viable modality of reversible interventions in the effective treatment of many neurological and psychiatric disorders. It is aimed to restore brain dysfunction by the targeted delivery of specific electronic signal within or outside the brain to modulate neural activity on local and circuit levels. Development of therapeutic approaches with brain stimulation goes in tandem with the use of neuroimaging methodology in every step of the way. Indeed, multimodality neuroimaging tools have played important roles in target identification, neurosurgical planning, placement of stimulators and post-operative confirmation. They have also been indispensable in pre-treatment screen to identify potential responders and in post-treatment to assess the modulation of brain circuitry in relation to clinical outcome measures. Studies in patients to date have elucidated novel neurobiological mechanisms underlying the neuropathogenesis, action of stimulations, brain responses and therapeutic efficacy. In this article, we review some applications of deep brain stimulation for the treatment of several diseases in the field of neurology and psychiatry. We highlight how the synergistic combination of brain stimulation and neuroimaging technology is posed to accelerate the development of symptomatic therapies and bring revolutionary advances in the domain of bioelectronic medicine.

Abstract:Brain stimulation is a critical technique in neuroscience research and clinical application. Traditional transcranial brain stimulation techniques, such as transcranial magnetic stimulation (TMS), transcranial direct current stimulation (tDCS), and deep brain stimulation (DBS) have been widely investigated in neuroscience for decades. However, TMS and tDCS have poor spatial resolution and penetration depth, and DBS requires electrode implantation in deep brain structures. These disadvantages have limited the clinical applications of these techniques. Owing to developments in science and technology, substantial advances in noninvasive and precise deep stimulation have been achieved by neuromodulation studies. Second-generation brain stimulation techniques that mainly rely on acoustic, electronic, optical, and magnetic signals, such as focused ultrasound, temporal interference, near-infrared optogenetic, and nanomaterial-enabled magnetic stimulation, offer great prospects for neuromodulation. This review summarized the mechanisms, development, applications, and strengths of these techniques and the prospects and challenges in their development. We believe that these second-generation brain stimulation techniques pave the way for brain disorder therapy.

Multi-Point Temporal Interference Stimulation by Using Each Electrode to Carry Different Frequency Currents

Abstract:Recently, a novel technology for noninvasive deep brain stimulation (NDBS) with temporally interfering electric fields was developed. This noninvasive technology is able to perform one-point temporal interference (TI) stimulation and stimulates the hippocampus without affecting the overlying cortex in mice. In this study, we introduce the concept of multi-point temporal interference (MTI) stimulation, which can simultaneously stimulate multiple nodes in the brain network to modulate its function. For the sake of realizing MTI stimulation, we proposed the scheme with each electrode carrying different frequency currents, which has higher usability with respect to the scheme by adding more electrode pairs. Additionally, to optimize the MTI stimulation, we selected the proper current frequencies and amplitudes, which were verified by geometrical model, magnetic resonance imaging (MRI) human head model, and tissue phantom. Finally, we tested the independence between the two stimulation points in MTI stimulation. The MTI stimulation can be generated by our method with proper parameters in geometrical model, MRI human head model, and tissue phantom. The stimulation points in MTI stimulation are all steerable, and furthermore can be controlled independently. Our results suggest that MTI stimulation can be used to simultaneously stimulate multiple target nodes of the brain network in deep brain areas noninvasively, which paves the way for the modulation of the brain in research and clinical neurobiology.
A Noninvasive Deep Brain Stimulation Method Via Temporal-Spatial Interference Magneto-Acoustic Effect: Simulation and Experimental Validation

Abstract:Deep brain transcranial stimulation is used in both research and for the treatment of neuropsychological diseases. Ideally, such stimulation should be noninvasive and precisely controlled. We propose a temporal-spatial interference magneto-acoustic stimulation (TIMAS) method combining transcranial magneto-acoustic stimulation (TMAS) and temporal interference stimulation (TIS) to achieve such stimulations, using the characteristics of the response of brain neurons to modulated low-frequency oscillation. Ultrasonic waves with two frequencies can interfere with each other to produce a modulated low-frequency signal. The modulated signal with difference frequency characteristics can be used for deep brain electrostimulation by means of magneto-acoustic coupling effect. A focused difference frequency electric field with a millimeter focal spot, a lateral resolution of 1.2 mm, an axial resolution of 6.4 mm, and a frequency of 4.13 kHz was achieved in the experimental system. These parameters are much better than previously reported magneto-acoustic coupling stimulation parameters. The measured electric field intensity for nerve stimulation was 137.2 mV/m, which meets the stimulation standard and reaches the threshold for effective nerve stimulation. Simulation and experimental results showed that TIMAS has superior penetration and temporal-spatial resolution and can generate a low-frequency envelope modulated electric field with a definite direction. TIMAS can be used as a new noninvasive low-frequency envelope modulated electrical stimulation, which can obtain high spatial resolution and high focus even at deep stimulation depths, and has low envelope frequency compared with the traditional TMAS. The method proposed in this article provides a new direction for the development of TMAS and is expected to be applied in brain science research and the treatment of major neuropsychiatric diseases related to deep brain regions.
A Design of Raster Plot for Illustrating Dynamic Neuronal Activity During Deep Brain Stimulation

Abstract:Deep brain stimulation (DBS), which usually utilizes high frequency stimulation (HFS) of electrical pulses, is effective for treating many brain disorders in clinic. Studying the dynamic response of downstream neurons to HFS and its time relationship with stimulus pulses can reveal important mechanisms of DBS and advance the development of new stimulation modes (e.g., closed-loop DBS). To exhibit the dynamic neuronal firing and its relationship with stimuli, we designed a two-dimensional raster plot to visualize neuronal activity during HFS (especially in the initial stage of HFS). Additionally, the influence of plot resolution on the visualization effect was investigated. The method was then validated by investigating the neuronal responses to the axonal HFS in the hippocampal CA1 region of rats. Results show that the new design of raster plot is able to illustrate the dynamics of indexes (such as phase-locked relationship and latency) of single unit activity (i.e., spikes) during periodic pulse stimulations. Furthermore, the plots can intuitively show changes of neuronal firing from the baseline before stimulation to the onset dynamics during stimulation, as well as other information including the silent period of spikes immediately following the end of HFS. In addition, by adjusting resolution, the raster plot can be adapted to a large range of firing rates for clear illustration of neuronal activity. The new raster plot can illustrate more information with a clearer image than a regular raster plot, and thereby provides a useful tool for studying neuronal behaviors during high-frequency stimulations in brain.
On temporal scale-free non-periodic stimulation and its mechanisms as an infinite improbability drive of the brain's functional connectogram

Abstract:Rationalized development of electrical stimulation (ES) therapy is of paramount importance. Not only it will foster new techniques and technologies with increased levels of safety, efficacy, and efficiency, but it will also facilitate the translation from basic research to clinical practice. For such endeavor, design of new technologies must dialogue with state-of-the-art neuroscientific knowledge. By its turn, neuroscience is transitioning-a movement started a couple of decades earlier-into adopting a new conceptual framework for brain architecture, in which time and thus temporal patterns plays a central role in the neuronal representation of sampled data from the world. This article discusses how neuroscience has evolved to understand the importance of brain rhythms in the overall functional architecture of the nervous system and, consequently, that neuromodulation research should embrace this new conceptual framework. Based on such support, we revisit the literature on standard (fixed-frequency pulsatile stimuli) and mostly non-standard patterns of ES to put forward our own rationale on how temporally complex stimulation schemes may impact neuromodulation strategies. We then proceed to present a low frequency, on average (thus low energy), scale-free temporally randomized ES pattern for the treatment of experimental epilepsy, devised by our group and termed NPS (Non-periodic Stimulation). The approach has been shown to have robust anticonvulsant effects in different animal models of acute and chronic seizures (displaying dysfunctional hyperexcitable tissue), while also preserving neural function. In our understanding, accumulated mechanistic evidence suggests such a beneficial mechanism of action may be due to the natural-like characteristic of a scale-free temporal pattern that may robustly compete with aberrant epileptiform activity for the recruitment of neural circuits. Delivering temporally patterned or random stimuli within specific phases of the underlying oscillations (i.e., those involved in the communication within and across brain regions) could both potentiate and disrupt the formation of neuronal assemblies with random probability. The usage of infinite improbability drive here is obviously a reference to the "The Hitchhiker's Guide to the Galaxy" comedy science fiction classic, written by Douglas Adams. The parallel is that dynamically driving brain functional connectogram, through neuromodulation, in a manner that would not favor any specific neuronal assembly and/or circuit, could re-stabilize a system that is transitioning to fall under the control of a single attractor. We conclude by discussing future avenues of investigation and their potentially disruptive impact on neurotechnology, with a particular interest in NPS implications in neural plasticity, motor rehabilitation, and its potential for clinical translation.
