Abstract:Objective To investigate the experience and value of retroperitoneal laparoscopic nephron-sparing excision.Methods Between September 2005 and May 2009,35 cases underwent the retroperitoneal laparoscopic nephron-sparing surgery.Results All procedures were technically successful.The mean operating time was 85 min,and mean ischemic time was 25 min.The mean estimated blood loss was 240 mL.The mean hospital stay after surgery was 9d.No intraoperative complications occurred.No local recurrence or trocar site metastasis was observed during a mean follow-up period of 1-30 months.Conclusion Retroperitoneal laparoscopic nephron-sparing surgery represents a feasible option for the patients with localized small renal tumors.It offers a new effective and minimally invasive treatment for selected patients.
Cumulative Sum Analysis of the Learning Curve for Modified Retroperitoneoscopic Living-Donor Nephrectomy

Abstract:Objectives: To introduce our modified retroperitoneoscopic living-donor nephrectomy (RPLDN) techniques and estimate the learning curve. Methods: We retrospectively evaluated 121 consecutive donors who underwent modified RPLDN performed by a single surgeon. While the surgeon controlled the renal vessels, one hand was inserted through an abdominal incision for assistance. The kidney was manually retrieved through the abdominal incision. The learning curve was evaluated using the cumulative sum (CUSUM) method, which was a graphical method showing changes in individual surgical performance. Results: The mean operating time and warm ischemic time (WIT) were 129.4 min and 154.5 s, respectively. The mean estimated blood loss (EBL) was 44.4 mL. Regarding intraoperative complications, no open conversions or blood transfusions were required. The CUSUM learning curve included the following 3 unique phases: phase 1 (the initial 32 cases), representing the initial learning curve; phase 2 (the middle 38 cases), representing expert competence; and phase 3 (the final 51 cases), representing mastery. Conclusions: Our modified method has the advantages of a short operating time, an optimized WIT, a low EBL, and acceptable complication rates. The surgeon completed the initial learning phase of RPLDN after 32 cases and could effectively perform RPLDN after 70 cases.
Retroperitoneal laparoscopic nephron-sparing surgery for renal tumor:report of 15 cases

Abstract:Objective To evaluate the clinical effect of retroperitoneal laparoscopic nephron-sparing surgery.Methods From March 2007 to May 2008,15 patients with local renal tumors underwent retroperitoneal laparoscopic partial nephrectomy by a single surgeon.Hilar control was achieved with bulldog clamps.Hook-cautery is used to circumscribe the tumor for resection by scoring the renal capsule circumferentially with a 2-10 mm margin around the tumor.Specimens are randomly extracted through tumor edges and immediately given to pathological frozen sections.The operative time,blood loss,postoperative stay,complications and the time of complete convalescence were recorded.Results Mean operative time was 110 minutes and mean time of hilar control was 26 minutes.Mean blood loss was 200ml.Mean postoperative stay was 9 days.Final pathological evaluation revealed negative positive margins in these cases.During 2-14 months follow-up there was no local recurrence.Conclusions Retroperitoneal laparoscopic partial nephrectomy for small renal tumors can be performed safely and effectively.Extended follow-up period is required to determine its long-term functional and oncological outcomes.

Abstract:Objective:To evaluate the efficacy and the learning-curve period of our modified retroperitoneoscopic live-donor nephrectomy.Methods:From December 2003 to May 2009, 138 consecutive retroperitoneoscopic live-donor nephrectomies were carried out at our institution. Donors were separated into four groups in consecutive sequence in order to determine the learning-curve period. Groups 1-3 included forty consecutive cases each, whereas group 4 included the last eighteen cases. The renal artery and vein were controlled with two plastic locking clips at proximal ends without any clips on the kidney side. The kidney was manually retrieved through lumbar incision.Results:Mean operative times were 160.5, 116.9, 101.4 and 109.2 min in groups 1-4, respectively (group 1 vs group 2, 3 or 4, P < 0.01). Mean warm ischemic time was 3.5 min. Mean estimated blood loss was 88.8, 73.0, 69.3 and 43.9 mL in groups 1-4, respectively (group 1 vs group 4, P < 0.01). No blood transfusion or open conversion was required. Mean hospital stay was 7.8, 6.9, 6.6 and 5.8 days in groups 1-4, respectively (group 1 vs group 4, P < 0.05). Eight donors and seven grafts suffered from complications. Complication rates were 22.5%, 7.5%, 5.0% and 6.0% in groups 1-4, respectively (group 1 vs group 3, P < 0.05).Conclusion:Our modified retroperitoneoscopic live-donor nephrectomy can be carried out safely with a learning-curve period of about 40 cases.

Abstract:Objective To report our initial experience on the complications of retroperitoneoscopic live donor nephrectomy(RPLDN)and their managements in 117 cases.Methods From December 2003 to January 2009,117 cases of RPLDNs were carried out in our hospital.The operation was performed through 3 lumbar ports,after the kidney was liberated fully and the ureter was severed 7-8 cm under the lower pole of the kidney,the renal artery and vein were blocked with Endo-cut or Hem-o-lok and then cut off.Endo-cut was used in 3 patients and Hem-o-lok in 114 donors.Afterwards,the kidney was taken out quickly from the donor and infused with 4℃ kidney preserving fluid(HCA)immediately.We reviewed the intraoperative and postoperative complications in the donors and the grafts and the managements of the cases.Results All the 117 operations were successfully completed.No patients died during the operation or had delayed graft function recovery postoperation.No one was converted to an open surgery or needed blood transfusion during the procedure.Surgery-related complications occurred in 5 donors(4.3%)including intraoperative bleeding in 2(cured by hemostasis during the operation)and retroperitoneal hematoma in 3(cured by conservative management).Graft-related complications occurred in 8(6.8%)cases;among which extra arteries in the graft owning to endovascular stapler was found in 1 patient(who had a short common stem of the main renal artery,the three branches were anastomosed with the external iliac artery and 2 branches of internal iliac artery respectively);two graft kidneys were lacerated by laparoscopic instruments superficially and were repaired using absorbable sutures;renal subcapsular hematoma was noted in 2 grafts,capsulotomy was performed in one of them while the other received no treatment,both the cases had normal function of the transplanted kidneys;ureteral complications occurred in 3 grafts,in which vesico-urethral anastomotic leakage was developed in 10 days after withdrawal of the double J stents,they were cured by ureteral re-implantation.All the donors and recipients with complications were followed up for an average of 50 months(range 15-62 months).No other complications were found in the cases.During the follow-up,blood and urine routine,liver and renal functions,and blood glucose level are all in a normal range,abdominal B-ultrasonography revealed no abnormalities.B-ultrasonography of the implanted kidneys showed normal morphology and renal functions.Conclusions RPLDN is a safe and reliable method.Most of its complications complications may be attributed to the learning curve.Refinements in surgical techniques may decrease the rates of both donor and graft complications.

However, longer operation time and risks of abdominal complications indicate the need for improved techniques. Abstract:O Background. Laparoscopic donor nephrectomy (LDN) has gradually become the main approach to obtain live donor kidneys. However, the shorter right renal vein limits its wider application. The aim of this study was to compare the outcomes of left- and right-side retroperitoneal LDN. Methods. We reviewed the perioperative data of 527 consecutive donors receiving retroperitoneal pure LDN with a new method at our center between April 2009 and April 2014. The patients were divided into group A (the first 100 patients) and group B (the remaining 427 patients). A total of 423 cases of left donor surgery and 104 cases of right donor surgery were compared. The comparison of the laterality of LDN was also performed between group A and group B. Results. This is currently the largest case series of LDN in our country. Although right side LDN patients had longer operation time and a slightly higher incidence of intraoperative complications compared with left-side LDN patients, the operation time was shorter in both the groups compared with previous reports. In group B, patients undergoing right-side LDN had longer operation time and more frequent complications. Once the learning curve of 100 cases was completed, the incidence of complications and operation time were greatly reduced in both sides for LDN. There was no significant difference in the serum creatinine levels in recipients at 6 months of follow-up. Conclusions. Despite a slightly higher incidence of complications and longer operation time, right-side LDN can achieve equally safe and effective transplantation outcomes. This expands the source of potential donor kidneys.
Transumbilical laparoendoscopic single-site surgery: more than 1-year experience in radical nephrectomy and its learning curve study.

Abstract:Purpose: The aim is to report our 1-year experience in transumbilical Laparoendoscopic single-site (LESS) radical nephrectomy (LESS-RN) and assess its learning curve. Patients and Methods: From September 2009 to December 2010, 18 consecutive patients with a solitary renal tumor underwent transumbilical LESS-RN. We grouped measured parameters on the basis of the first nine (group A) and last nine (group B) patients, and compared both subgroups with the control group (C), which was composed of the initial nine consecutive patients who underwent conventional laparoscopic RNs performed by the same surgeon. Results: There was one LESS patient converted to conventional laparoscopic surgery for intraoperative hemorrhage and transfused. The mean for measured parameters of the groups A, B, C were: age: 58.6, 57.2, and 59.2 years, respectively (P > 0.05); American Society of Aneshesiologists score: 1.9, 2.0, and 2.0, respectively (P > 0.05); body mass index: 25.2, 23.5, and 23.1 kg/m(2), respectively (P > 0.05); lesion size at CT scan: 5.0, 4.6, and 6.2 cm, respectively (P = 0.039); operative skin to skin time: 191.6, 123.9, and 122.2 minutes, respectively (P = 0.000); estimated blood loss: 172.2, 246.7, and 116.7 mL, respectively (P > 0.05); incision length: 5.6, 5.9, and 7.4 cm, respectively (P = 0.000); hospital stay: 5.8, 5.8, and 5.8 days, respectively (P > 0.05). At the latest follow-up, all patients did not present any local recurrence or metastasis. Conclusions: In selected patients, the transumbilical LESS-RN can be safely performed with favorable outcomes. While a learning curve certainly exists, it appears to be short for an experienced laparoscopic surgeon.
Hand-assisted retroperitoneal laparoscopic living donor nephrectomy with a modified technique

Abstract:Objective To evaluate the safety,feasibility and results of the hand-assisted retroperitoneal laparoscopic living donor nephrectomy ( HRPLDN ) with a modified technique. Methods Living donors (n =32) were divided into HRPLDN group (n =16) and open group (n =16) according to surgical technique.Operative data and postoperative outcomes including operative time,estimated blood loss,warm ischemia time,length of hospital stay and complication rate,were collected. Results All procedures were completed successfully.In HRPLDN group,the mean operative time was 101.3 ± 21.2 min (range from 70 to 150 min),with an estimated blood loss of 53.8 ±25.5 ml (range from 20 to 100 ml) and warm ischemia time of 2.4 ± 0.6 min ( range from 1.5 to 3.5 min).No living donor needed conversion to open surgery and the urine volume of transplanted kidney after first 24 hours was 5036 ml (range from 3500 -6500 ml).The mean postoperative on bed time were (2.8 ± 0.7 ) d (ranging from 2 -4 d).All parameters of HRPLDN were significantly better than that of open groups. Conclusion Living donor nephrectomy with HRPLDN is a safe and reliable surgical technique.

Abstract:OBJECTIVETo summarize our experience of retroperitoneal laparoscopic living donor nephrectomy, our continuous technical improvements and refinement of this skill and standardization of each procedure of this operation.METHODSHaving approved by hospital ethical committee and local government administration, a total of 193 living donors underwent retroperitoneal laparoscopic living donor nephrectomy from Dec.2003 to Feb. 2016 in our department. Under general anaesthesia, the operation was performed through 3 lumbar ports. After the kidney was liberated fully and the ureter was severed 7-8 cm under the lower pole of the kidney, the renal artery and vein were blocked with endo-cut or hem-o-lok separately and then severed. Then the kidney was taken out quickly and flushed with 4 °C kidney preserving fluid immediately, the donor kidneys were then preserved in iced saline until kidney transplantation. Clinical data about operation time, volume of blood loss, perioperative complications, renal function of both donors and recipients before and after operation were collected.RESULTSThe 193 retroperitoneal laparoscopic living donor nephrectomy operations were successful with only one operation was converted to open living donor nephrectomy because of hemorrhage and unclear operation field during the operation. The average operation time was 85 min (55-135 min), the average blood loss was 60 mL (20-200 mL), and no donor needed blood transfusion during or after operation. Three donors were found to have hematoma of renal fossa after operation and none of them required further treatment. The average hospital stay after operation was 5.7 days (4-9 days). In the study, 162 donors were followed up for an average of 42 months (1-58 months) and they were all healthy. Two kidney recipients had urinary bladder anastomosis leakage after operation and both needed surgical repair, a new anastomosis of ureter and bladder were made. Three kidney recipients had kidney subcapsular hematoma but required no further treatment. One kidney recipient had delayed graft function and recovered finally and the renal function of other recipients were all normal. Renal function of both donors and recipients during the follow up period were normal.CONCLUSIONRetroperitoneal laparoscopic living donor nephrectomy is a safe and reliable technique, it may become a standardized operation for living kidney transplantation after continuous technical improvement. Precautions must be taken to avoid complications and a skilled hand is necessary for success.
Clinical analysis of retroperitoneal living donor nephrectomy under 3D laparoscopy

Abstract:Objective To summarize the experience in retroperitoneal living donor nephrectomy under 3D laparoscopy and to investigate its clinical effect and safety. Methods The clinical data of 19 cases who underwent retroperitoneal living donor nephrectomy under 3D laparoscopy were collected.Operation duration,intraoperative blood loss, renal warm ischemia time,length of renal artery,renal vein and ureter,incision size and operative complications were recorded.Postoperative renal function of donors and recipients was observed. Results Nineteen cases of living donor nephrectomy were successfully completed under the 3D laparoscopy,and there were no conversion into conventional laparoscopy and open surgery.The operation duration of living donor nephrectomy under 3D laparoscopy was 80.5﹣125.2 (with an average of 102.3)min;intraoperative blood loss was 40.6﹣90.4 (60.8)ml;renal warm ischemia time was 100﹣230 (161)s.Length of renal artery,renal vein and ureter was 2.6﹣3.2 (2.9)cm,2.2﹣3.0 (2.6)cm and 8﹣13 (10)cm, respectively.The incision size was about 5﹣6 cm,and the wound healed.Urine volume of the donors at 24 h after operation was 2 000﹣2 500 ml.Serum creatinine increased slightly in 1 case at 3 d after operation,and returned to normal through reexamination after 7 d and 1 month.The donors stayed in the hospital 5﹣7 (6)d after operation.All transplant operations were performed successfully,and delayed graft function did not occurred. Conclusions Accuracy of operation can be improved effectively by 3D laparoscopic surgery system.It is safe and feasible to perform retroperitoneal living donor nephrectomy under 3D laparoscopy.

Abstract:OBJECTIVES To establish a modified cost-effective and safe approach of retroperitoneal laparoscopic live donor nephrectomy (RPLLDN). METHODS Between December 2003 and December 2008, total of 109 living related renal donors underwent consecutive three-port RPLLDNs. The initial retroperitoneal space was created by insertion of a rubber catheter attached to a saline-filled midfinger of a glove. The renal hilum and ureter were circumferentially mobilized. The tributaries of renal vessels were divided by harmonic scalpel without any clips. In laparoscopy, the ureter was sheared with scissors. A longitudinal 6-8 cm skin incision was enlarged form the primary trocar distally to retrieve the graft. The main renal vessels were controlled, using 2 Hem-o-lok clips placed at each proximal ends. The graft was retrieved manually through the skin incision. RESULTS All the 109 RPLLDNs were carried out successfully. The mean operation time and mean warm ischemic time was 129.7 +/- 42.6 and 3.6 +/- 1.2 minutes, respectively. The mean blood loss was 73.6 +/- 53.7 mL. No blood transfusion or open conversion was required. No major complication occurred in the donors, and only 5% of the donors suffered from minor complications. A total of 3% and 4% of the recipients developed major and minor complications, respectively, and 12.8% of the donors required analgesics. The mean level of postoperative serum creatinine of the donors was 1.31 +/- 0.22 mg/dL and the mean level of postoperative serum creatinine of the recipients at the first month was 1.59 +/- 0.91 mg/dL. Abstract:Objective To summarize our experience on retroperitoneal laparoscopic living donor nephrectomies. Methods A total of 151 donors underwent retroperitoneal laparoscopic living donor nephrectomy in Peking University 3rd Hospital from December 2003 to December 2012. The operation was performed through 3 lumbar ports. After the kidney was fully liberated and the ureter was severed 7- 8 cm under the lower pole of kidney. The renal artery and vein were blocked with Endo- cut (in 3 patients) or hem- o- lok (in 148 patients) separately and then severed; and then the kidney was quickly taken out from the donor and was immediately infused with 4 ℃ kidney preserving fluid. Results The 151 operations were successful. The average operation time was (86.0±13.6) min (54-180 min) and blood loss was (57.1±12.3) mL (20-200 mL) in average. No patient required blood transfusion. The mean warm ischemia time was (3.6±1.2) min (2-8 min). Three patients had hematoma of renal fossa after operation and required no further treatment. Hospital stay after operation was (4.8±1.7) d (3.5-9 d). No delayed graft function occured in all recipients who received these allogratfs. Conclusion Retroperitoneal laparoscopic living donor nephrectomy is a safe and reliable method. The modified techniques can be easier to learn and inerease the quality of the donor kidney.

Abstract:Objective To introduce the technical improvements in retroperitoneal laparoscopic radical nephrectomy and discuss their roles in decreasing the perioperative complication. Methods Sixty-one patients with localized renal cell carcinoma were treated with retroperitoneal laparoscopic radical nephrectomy from December 2006 to March 2008. Pre-operative CT scan was performed to evaluate the renal vessel status. Thirty degree laparoscope was used together with self-made balloon dilator to establish the retroperitoneal space. Harmonic scalpel was used in dissection and removal of the extra-peritoneal fat. Cautions were taken during the separation of peritoneum. The renal artery and vein were separated and ligated with Hem-o-lok clips. Complications such as vessel split and intes-tinal injury were recorded. Operation time, estimated blood loss, drainage volume, and length of hos-pital stay were analyzed as well. Results The operation time was 50-135 min, estimated blood loss during operation was 20-170 ml, post-operative drainage volume was 20-210 ml, mean post-opera-tive length of hospital stay was 5 d. Two complications occurred during the operation: one was a vena eava injury, and another was a minor duodenum wall perforation. Conclusions Retroperitoneal lapa-roscopic radical nephrectomy is an effective and safe surgical option for localized renal cell carcinoma. Improvements in this surgical technique bring more safety and convenience and could decrease perio-perative complications relatively.

Endo-cut was used in 3 patients and hem-o-lok in 112 donors. Then the kidney was taken out quickly from the donor and infused with 4℃ kidney preserving fluid (HCA) immediately. Results The 115 operations were successful. The mean operative time was 90 min (60-180 min) and blood loss was 60 ml (20-200 ml). No patient needed blood transfusion. Warm ischemia time was 4.5 min (3-8 min). 3 patients had hematoma of renal fossa after operation and required no further treatment. Hospital stay after operation was 5.7 days (4-9 days). 95 of 115 donors were followed up for 42 months (1-58 months). Conclusion Retroperitoneal laparoscopic living donor nephrectomy is a safe and reliable procedure and it can replace the traditional open surgery.

Abstract:Purpose: To compare the intra-operative and post-operative outcomes of Single-Port Robotic Donor Nephrectomies (SP RDNs) and Laparoscopic Donor Nephrectomies (LDNs) Materials and Methods: We retrospectively reviewed our institutional database for patients who received LDN or SP RDN between September 2020 and December 2022. Donor baseline characteristics, intra-operative outcomes, post-operative outcomes, and recipient renal function were extracted and compared between LDN and SP RDN. SP RDN learning curve analysis based on operative time and graft extraction time was performed using Cumulative Sum Analysis. Results: 144 patients underwent LDN and 32 patients underwent SP RDN. LDN and SP RDN had similar operative times (LDN: 190.3 ± 28.0 min., SP RDN: 194.5 ± 35.1 min., p=0.3253). SP RDN patients had significantly greater extraction times (LDN: 83.2 ± 40.3 sec., SP RDN: 204.1 ± 52.2 sec, p<0.0001) and warm ischemia times (LDN: 145.1 ± 61.7 sec., SP RDN: 275.4 ± 65.6 sec., p<0.0001). There were no differences in patient subjective pain scores, inpatient opioid usage, or Clavien-Dindo II+ complications. Short- and medium-term post-operative donor and recipient renal function were also similar between the groups. SP RDN graft extraction time and total operative time learning curves were achieved at case 27 and 13, respectively. Conclusion: SP RDN is a safe and feasible alternative to LDN that minimizes post-operative abdominal incisional scars and has a short learning curve. Future randomized prospective clinical trials are needed to confirm the findings of this study and to identify other potential benefits and drawbacks of SP RDNs.

Application of Retroperitoneal Laparoscopic Technique in Nephrectomy

Abstract:Objective:To explore the clinical application value of retroperitoneal laparoscopic nephrectomy.Methods:17 cases of retroperitoneal laparoscopic nephrectomy from May 2006 to October 2009 were retrospectively discussed.Among them,11 cases were male and 6 cases were female and their average age was 62 years old.The resections were in the left for 10 cases and in the right for 7 cases,which were completed by retroperitoneal laparoscope.Results:Complete resections of the ill kidney were performed in 17 cases,among which a conversion to open surgery was needed in 2 cases.The surgeries lasted 110 min to 210 min and the average time was 100 min.The blood loss during operation was 50ml to 150ml and the average was 100ml without blood transfusion during all surgeries.Peritoneal damage was in 2 cases,but no pneumothorax.Hemorrhage of puncture hole after operation was in 1 case,which was healed by processing without wound infection.The median length of hospital stay was 7-8d.There was no dead case after operation.With all patients followed up for 5-24 months,no further complication was found.Conclusion:Retroperitoneal laparoscopic surgery for nephrectomy could be fast for blood vessel exposing with clear layer of anatomy.It has the advantages of less trauma,less hemorrhage and rapid recovery,so it is worth expanding clinically.

Abstract:Objective To evaluate the technique and effect of laparoscopic live donor nephrectomy via retroperitoneal approach. Methods Thirty-two donors (15 males and 17 females) underwent laparoscopic donor nephrectomy at our institution. Twenty-nine left and 3 right procedures were performed. Under general anesthesia, all donors were placed in lateral decubitus. Three transretroperitoneal trocars were used through different ports. The first 12 mm port was placed 2 cm inferior to the edge of rib and 1 cm lateral to the edge of sarcospinous muscle. The second 10 mm port was about 8 to 10 cm anterior to the first, or on the anterior axillary line and 2 cm inferior to the edge of rib. The third 5 mm port was also on the anterior axillary line but 2 cm superior to the iliac crest. The surgeon created the retroperitoneal working space with a constructed catheter balloon device, and made the donor kidney and ureter dissociated. Renal arteries and veins were cut with endoscopic articulating linear stapler for the first 6 patients and then with Hemo-Lok, a plastic clip with a lock, for the others. Finally, the dissected donor kidney and ureter were taken out through an incision extended from the first trocar port. Results All the 32 donor grafts were dissected successfully. The duration of procedure was ranged from 60 to 180 min, and the volume of blood loss was from 20 to 200 ml. The range of warm ischemia time was 3 to 8 min, and 25 to 50 min of cold ischemia time. Short renal veins were found in 3 cases, 1 cm, 1.5 cm and 2 cm respectively. And one case of renal endoarterial injury occurred. On the day after transplantation, the urine volume of recipients was from 2800 to 10 100 ml. There was no delayed graft function found in recipients. Conclusion Laparoscopic donor nephrectomy via retroperitoneal approach is available and safe to renal transplantation with minimal invasion to donors. It can help to obtain donor grafts with higher quality, though skillful laparoscopic technique is demanded.

Abstract:Objective To evaluate the safety and effectiveness of the Hem-o-lok in retroperitoneal laparoscopic nephrectomy.Methods A total of 178 patients underwent retroperitoneal laparoscopic nephrectomy from November 2003 to November 2006.The operation was performed through 3 lumbar ports.The renal artery and veins were blocked with the Hem-o-lok and then severed.The ureter was cut at the site near the iliac blood vessel.For living donor nephrectomy,the kidney was taken out and infused with 4℃ kidney preserving fluid immediately.Results The operation was successfully completed in all the 178 cases and there was no conversion to open surgery.No Hem-o-lok ligating clips became dislodged.The mean operation time was 100 min(60-200 min),and the mean blood loss was 95 ml(20-200 ml).Complications included renal vein injury in 1 patient and pancreatic tail injury in 1 patient.The mean length of postoperative hospital stay was 5 days(4-8 days).Pathological results showed renal clear cell carcinoma in 65 patients,renal cystadenocarcinoma in 3 patients,renal leiomyosarcoma in 1 patient,renal lymphoma in 1 patient,renal oncocytoma in 2 patients,renal hamartoma in 2 patients,ureteral transitional cell carcinoma in 12 patients,ureteral squamous cell carcinoma in 2 patients,renal pelvis transitional cell carcinoma in 31 patients,renal pelvis xanthogranuloma in 1 patient,renal tuberculosis in 4 patients,accessory kidney in 1 patient,renal calculus in 4 patients,nephrohydrosis in 4 patients,and renal abscess in 1 patient.A total of 136 patients were followed for 1-36 months(mean,25 months),involving 64 patients followed for < 1 year,53 patients for 1-2 years,and 19 patients for 2-3 years.No metastasis(kidney bed,port site,or distant organs)was found.Conclusions The Hem-o-lok can be used safely and reliably in retroperitoneal laparoscopic nephrectomy,especially in living donor nephrectomy requiring long donor blood vessels.
