Novel Strategy in Giant Cutaneous Squamous Cell Carcinoma Treatment: the Case Experience with a Combination of Photodynamic Therapy and Surgery

Abstract:The treatment for giant cutaneous squamous cell carcinoma (cSCC) is challenging. Here, we reported that a 90 year-old patient with giant cSCC was successfully treated through combination of surgery and photodynamic therapy (PDT). Due to the advanced patient age, large tumor area, and deep infiltration, radical surgery cannot be performed initially. After in situ resection, five rounds of topical 5-aminolevulinic acid PDT were continuously performed. The surgical wound was healed five months after resection. Seven months after initial resection, the residual tumor was resected by a radical surgery. The patient was followed up for six months after the radical surgical resection and showed no tumor recurrence. This case suggested that initial utmost resection for main body of tumor, decreasing the tumor boundary by PDT treatment and the sequential treatment regimen of radical surgery can be an effective therapeutic schedule for refractory and giant cSCC.
