Abstract:[Objective] To evaluate the application of video-mediastinoscopy(VM)and video assisted thoracoscopy surgery(VATS)in diagnosis on mediastinal small cell cancer.[Method] 20 patients with mediastinal lesions undertook 16 VM biopsies and 4 VATS exploration form July 2000 to March 2006.Three patients with massive thoracic pleural effusion received talc pleurodesis and 1 patient with pericardial effusion received pericardial window after malignancy were confirmed.[Results] All patients recovered smoothly without mortality and severe postoperative complications.Small cell cancer was confirmed in all the 20 patients.One of them was hematogenic and lymphagenic,2 of them were from thymus and other 17 lesions were from lung.Both thoracic pleural effusion and pericardial effusion were well controlled.[Conclusion ] VM and VATS is a mini-invasive,safe and accurate method in diagnosis on mediastinal small cell cancer.It is the best way to make a histological diagnosis on such kind of lesions.Also,effective treatment on thoracic effusion and pericardial effusion could be performed through VATS.

Abstract:OBJECTIVES:We summarize our experiences of video-assisted thoracoscopic surgery (VATS) sleeve lobectomy with bronchoplasty for non-small-cell lung cancer and discuss the indications and technical details of the operation.METHODS:From September 2011 to December 2012, 15 patients underwent VATS sleeve lobectomy with bronchoplasty at our institution (right upper lobe 10, right middle and lower lobes 1, left lower lobe 2 and left upper lobe 2), with mediastinal lymphadenectomy. Three incisions were utilized. The utility incision was made at the fourth intercostal space, anterior axillary line. Simple continuous and simple interrupted suturing of the membranous and cartilaginous portions of the bronchus were performed for the anastomosis.RESULTS:All procedures were uneventful, with a median operative time of 165 min, a median bronchial anastomosis time of 44 min and a median blood loss of 150 ml. There were no conversions to thoracotomy. There were 14 squamous carcinomas and 1 adenocarcinoma. All patients recovered well, and 1 experienced a minor complication. The median duration of chest tube drainage was 5.4 days, and the median length of hospital stay was 7 days. All patients were followed postoperatively for a range of 1-16 months without tumour recurrence.CONCLUSIONS:VATS sleeve lobectomy with bronchoplasty is safe and effective. The utility incision placed at the fourth intercostal space, anterior axillary line, is convenient for the anastomosis, and the suturing technique is expeditious and secure. Preserving the azygos vein does not compromise exposure for the anastomosis. This technique is very suitable for centrally located lung cancers <3 cm in diameter, particularly when the cancers are located within the brachial lumen.

Abstract:Objective To discuss the security,effectiveness and risk factors of video-assisted thoracoscopic surgery for posterior mediastinal tumors.Methods We retrospectively analyzed the data of 59 patients including 36 men and 23 women who underwent thoracoscopic resection of posterior mediastinal tumors in People's Hospital of Peking University from May 2001 to July 2009.Their age ranged from 6 to 73 years old with an average age of 40.6 years old.The average maximum diameter of the tumors was 4.86 cm.All procedures were performed under general anesthesia and tumors were cut out with three ports.The anterior port was extended to 6 to 10 cm when conversion to thoracotomy was needed.After mediastinal pleura were opened,the tumor was stripped out along the outside of peplos and the vascular pedicle nerves were managed respectively.Results All surgeries were carried out successfully.The surgical duration,perioperative blood loss,postoperative chest tube duration and postoperative stay in hospital were respectively 45-300 min(125.80±57.40 min),10-1 000 ml(168.10±157.70 ml),1-10 d(2.50±1.74 d),and 2-14 d(5.24±2.24 d).There were 6 cases of conversion to open thoracotomy with a conversion rate of 10.2%.Postoperative pathology showed that there were 46 cases of neurogenic tumors,10 cases of cyst,2 cases of teratoma,and 1 case of lipoma.Follow-up was done on 51 cases for a period of 7-108 months(55.0±24.0 months) and 8(13.6%) cases were missed out during the period.No recurrence or death occurred during the follow-up.Logistic multi-variable analysis showed that maximum diameter of the tumor ≥6 cm was the independent risk factor for extending operative time(OR=1.932,P=0.004),increasing perioperative blood loss(OR=2.267,P=0.002),increasing conversion rate to thoracotomy(OR=3.123,P=0.004) and increasing postoperative complication rate(OR=1.778,P=0.013).Conclusion Video-assisted thoracoscopic surgery for posterior mediastinal tumor is safe and effective.Maximum diameter of the tumor ≥6 cm is an independent risk factor for increasing operation difficulty and risk.

Abstract:BACKGROUND AND OBJECTIVE:Chest computed tomography (CT), particularly thin-slice high resolution CT, has low sensitivity and specificity for detecting pulmonary nodules <10 mm in size. This limitation leads to challenges in clinical diagnosis and treatment of small pulmonary nodules. This study introduces the use of video-assisted thoracoscopic surgery (VATS) for the diagnosis and treatment of small pulmonary nodules.METHODS:From November 2009 to May 2012, 64 patients with small pulmonary nodules without prior preoperative pathologic diagnosis were treated by pulmonary wedge resection through VATS. The diagnosis of small pulmonary nodules was established from rapid frozen section. The type of operation depends on the pathology and the condition of the patients. Twenty patients with primary lung cancer were subjected to lobectomy and radical resection of the lymph nodes by complete thoracospic lobectomy or video-assisted thoracoscopic invasive lobectomy. Pulmonary wedge resection was performed in 44 patients, among whom 21 have benign nodule, 18 have precancerous lesion, 3 have metastatic nodule, and 2 have primary lung cancer for which lobectomy was not fit.RESULTS:Confirmative diagnosis is difficult to obtain among patients with small pulmonary nodules. VATS is effective in the diagnosis and treatment of small pulmonary nodules. With VATS, patients with benign small pulmonary nodules can be cured, and patients with primary lung cancer can receive definite diagnosis and effective treatment in time.CONCLUSIONS:CT-guided hook-wire fixation is useful in precise lesion localization for surgical resection.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>In this study we aimed to assess the clinical outcomes of performing video‐assisted thoracic surgery (VATS) to treat primary mediastinal cysts (PMCs) and investigate the clinical factors which increase the difficulties associated with VATS.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>The medical records of all consecutive PMC patients, who underwent surgical resection from April 2001 to July 2016, were reviewed and 282 patients were included. Clinical characteristics, imaging features, and surgical outcomes were analyzed. Follow‐up data were successfully obtained from 230 PMC patients by telephone or outpatient clinic annually. The latest follow‐up was July 2019.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>VATS was performed in 278 patients and four patients were converted into thoracotomy. The mean operation time and intraoperative bleeding were 102.4 ± 40.9 minutes (range 25–360 minutes) and 52.4 ± 75.1 mL (range 5–600 mL), respectively. The intra‐ and postoperative complication rates were 2.8 and 5.7%, respectively. Seven patients with bronchogenic cysts showed severe cyst adhesion to vital mediastinal structures and thus had incomplete resection. Multivariable logistic analysis revealed that a maximal cyst diameter greater than 5 cm was significantly associated with increased risks of operation time extension (OR = 2.106; 95% CI: 1.147–3.865, <i>P </i> = 0.016) and intraoperative blood loss increase (OR = 4.428; 95% CI: 1.243–16.489, <i>P </i> = 0.022). A total of 230 patients had follow‐up data. The median follow‐up time was 70 months (range, 36–210 months). No local recurrence was observed. </p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Surgical resection by VATS may be recommended for PMC management as a primary therapeutic strategy. Surgical Management of Primary Mediastinal Mature Teratoma: A Single-Center 20 Years' Experience.

Abstract:BackgroundThis study aims to investigate the clinical efficacy of video-assisted thoracic surgery (VATS) in treating mediastinal mature teratoma (MMT) and explore the clinical factors that increase the difficulties associated with VATS.MethodWe retrospectively reviewed 101 consecutive patients with MMT who underwent surgical excision between November 2001 and June 2021. Follow-up was done by telephone or at an outpatient clinic. The deadline for follow-up was February 2022.ResultsThe operative time, the chest tube indwelling time, and the hospital stay duration were significantly shorter in the VATS group compared with the thoracotomy group. The intraoperative and postoperative complication rates in the VATS group were lower than that of the thoracotomy group (P < .05). In thoracoscopic surgery, the clinical symptoms during the course of the disease were significantly associated with bleeding loss increasing [odds ratio (OR) = 3.562; 95% confidence interval (CI) 1.180–10.753, P = .024] and operation time extension (OR = 5.697; 95% CI 1.529–21.221, P = .010). The relationship between lesions and superior vena cava or innominate vein from preoperative CT imaging was significantly associated with bleeding loss increasing (OR = 4.629; 95% CI 1.463–14.639, P = .009). A maximal lesion diameter greater than 7 cm was significantly associated with increased risks of operation time extension (OR = 5.019; 95% CI 1.641–15.348, P = .005).ConclusionCompared with traditional thoracotomy surgery, VATS can be performed safely in selected patients with MMT. A surgical method for complete resection needs to be determined according to preoperative imaging and intraoperative conditions to reduce the unnecessary damage.

Abstract:BACKGROUND: Pulmonary metastasectomy via a traditional open thoracotomy is effective for the treatment of patients with pulmonary metastases. However, whether video-assisted thoracoscopic surgery (VATS) achieves similar clinical effects in terms of survival rates remains unclear. We conducted this meta-analytic study to compare VATS with open thoracotomy for pulmonary metastasectomy.METHODS: Relevant studies were identified by searching PubMed from inception to December 2014. The overall and recurrence-free survival rates were measured in terms of hazard ratios (HRs) with VATS and open thoracotomy as the main outcomes.RESULTS: Eight studies were included in our meta-analysis, which included 337 patients in the VATS group and 485 patients in the open thoracotomy group. The overall survival rate in the VATS group was statistically significantly higher than that in the open thoracotomy group [HR, 0.69; 95% confidence interval (CI), 0.55-0.87; P = 0.002]. The VATS group still had a higher overall survival rate after exclusion of the one low-quality study (HR, 0.78; 95% CI, 0.59-1.03), but the result was not statistically signiinot signcant (P = 0.075). No difference in the recurrence-free survival rate was found between the two groups (HR, 0.86; 95% CI, 0.69-1.08). The subgroup analysis showed no difference between the two groups in either the overall survival rate (HR, 0.72; 95% CI, 0.50-1.04) or the recurrence-free survival rate (HR, 0.79; 95% CI, 0.59-1.08) in patients with metastases from colorectal cancer.CONCLUSIONS: VATS is an alternative surgical approach for pulmonary metastasectomy in patients with resectable pulmonary metastases. Further large prospective studies are needed to identify the indications for VATS in patients with pulmonary metastases.

Abstract:Objective To summarize the clinical experiences of video-assisted thoracoscopic surgery(VATS) lobectomy performed on a series of 300 consecutive patients,and report the results of a 3-year follow-up.Methods We retrospectively analyzed the clinical data of 300 consecutive patients who underwent VATS lobectomy from September 2006 to December 2009 in the Department of Thoracic Surgery,People's Hospital of Peking University.Of the 300 patients,there were 159 males and 141 females with the age ranged from 18 to 86 years(58.30±13.90 years).Preoperative diagnosis showed that there were 266 patients of mass in the lung,22 of bronchiectasis,5 of cyst/abscess in the lung,3 of pulmonary sequestration,2 of fungus infection,and 2 of pneumothorax.We assessed the peri-operative variables by standard descriptive statistics and estimated the 3-year survival rate by Kaplan-Meier analyses.Results Sixty-six patients were diagnosed to have benign diseases and 234 patients were with malignancies.A percentage of 81.82%(54/66) of the benign patients had infectious diseases,and the majority of the malignancies was non-small cell lung cancer(213 patients),especially adenocarcinomas which comprised 73.08%(171/234) of all the malignancies.A total of 273 patients accomplished VATS lobectomy,of whom 27 patients required conversion to thoracotomy at a conversion rate of 9.00%(27/300).In the VATS lobectomy accomplished group,the mean operation time was 3.17±0.88 h,and the blood loss was 225.70±195.20 ml.Benign surgery took significantly less time(t=2.280,P=0.0032) and had shorter drainage time(t=1.392,P=0.0304) than those of malignancies.Dense adhesions between lymph nodes and blood vessels was the primary reason for conversion to thoracotomy in 17 patients at a percentage of 62.96%(17/27).Bleeding was the second reason for conversion in 5 patients at a percentage of 18.52%.The patients in the upper lobe lobectomy group showed significantly higher risk of conversion compared with those in the non-upper lobe surgery group(χ2=6.131,P=0.013),while gender(χ2=1.182,P=0.277),pathology(χ2=0.210,P=0.647) and the tumor located in left or right side(χ2=2.933,P=0.087) didn't influence the risk of conversion.The result of the 3-year follow-up showed that there was no reoccurrence of symptoms in patients with benign diseases;Non-small cell lung cancer patients had a 3-year survival rate of 0.87 with the 95% confidence interval(CI) from 0.77 to 0.96,and pathologic stage I patients at 0.91 with the 95%CI from 0.85 to 0.98.Conclusion VATS lobectomy is safe and effective.This research shows that domestic technologies of VATS lobectomy and its mid-term results have reached the international standard.

Abstract:Background. Bronchiectasis is one of the common diseases diagnosed in the world. No major improvement for the treatment approaches and limited efficacy promote a big challenge for management of this disease. Video-assisted thoracoscopic surgery (VATS) offers a new choice for the treatment of bronchiectasis. The purpose of this study was to present our experience of VATS for bronchiectasis and to compare this with thoracotomy in our institution. Methods. We reviewed the medical records of patients who underwent VATS lobectomy and general lobectomy for bronchiectasis between January 2005 and December 2009. Results. A total of 279 patients underwent thoracotomy, 52 patients underwent attempted VATS lobectomy. Fifty-two patients from 279 patients for thoracotomy were selected and compared with the VATS group. Pleural adhesion was observed in 15 patients (28.8%) in VATS. The VATS lobectomy was converted to open thoracotomy in 7 patients. There was no difference in the blood loss and median operative time between the two groups, but the patients with VATS had shorter length of stay in hospital (p = 0.045), fewer complications (p = 0.039) than those with thoracotomy. Forty-nine (94%) and 46 (88%) patients fully recovered after operation by VATS and thoracotomy, respectively. Conclusions. Video-assisted thoracoscopic lobectomy in localized bronchiectasis is a safe and more efficient procedure in selected patients with better recovery. Abstract:BackgroundThe role of video-assisted thoracoscopic surgery (VATS) in mediastinal lymph node dissection (MLND) for non-small cell lung cancer (NSCLC) following neoadjuvant therapy remains controversial. The aim of this study was to demonstrate the sufficiency of VATS by evaluating perioperative and long-term outcomes. MethodsPatients with locally advanced NSCLC and treated with radical surgery after neoadjuvant therapy were identified in our database. The thoroughness of MLND was compared by approach. Multivariable logistic regression analysis was used to evaluate predictors of sufficient MLND. Propensity score matching was performed. Kaplan-Meier and Cox proportional hazard analyses were used to assess long-term survival. ResultsOf the 127 enrolled patients, 56 underwent attempted VATS and 71 underwent thoracotomy. Multivariable logistic regression analysis revealed that approach was not a predictor of sufficient MLND (odds ratio 0.81, 95% confidence interval [CI] 0.364-1.803; P =0.606). After matching, 28 pairs of patients were selected from the two groups. There was no significant difference between the numbers of dissected lymph nodes (15 vs. 20; P =0.191) and nodal stations (7 vs. 7; P =0.315). Recurrence-free (log-rank P =0.613) and overall survival (log-rank P =0.379) was similar in both groups. Multivariable Cox proportional hazards model analysis indicated that VATS was not an independent predictor of recurrence-free (hazard ratio 0.955, 95% CI 0.415-2.198; P =0.913) or overall survival (hazard ratio 0.841, 95% CI 0.338-2.093; P =0.709). ConclusionCompared to thoracotomy, VATS is a sufficient approach for MLND to treat locally advanced NSCLC following neoadjuvant therapy without compromising long-term survival.

Abstract:Objective To appraise the technique and the indication of the Video Assisted Thoracic Surgery(VATS) in particular, the lung cancer. Methods Twenty patients successfully underwent anatomical lobectomy onVATS with 7 cm minithoractomy. The 7 cm minimal thoractomy was made at the third or fourth intercostal space justbehind the anterior axillary line for using conventional or dedicated endoscopic instruments. Mediastinal lymph nodeswere routinely removed through the minithoractomy. Forty four staple-liner(ENDOPATH) were used in these 20 ca-ses. Three operations were performed on VATS with incision extended to 12～14 cm due to pleural adhesion, absenceof fissure and bleeding during hilum dissection. Results There were no operative death. Complication included delayedwound healing in one and air leak for 4～5 days in two. Eight patients with benign lesions recovered uneventfully. Themean follow-up period is 15 months, one patient with adenocarcinoma developed recurrence of tumor at the site of chestdrainage tube with pleural effusion 3 months after operation, the others were well. Conclusion The VATS is a mini-mal invasive procedure and the anatomical lobectomy using VATS is safe and efficient in benign lesions, the mediastinalnodes can be resected with minithoractomy in primary lung cancer but it is important to adhere to the indication, andlonger follow-up is also necessary. Clinical Analysis of Single Utility Port Video-Assisted Thoracic Surgery in Surgical Treatment of Non-Small Cell Lung Cancer

Abstract:Objective To explore the feasibility and clinical application value of single utility port video-assisted thoracic surgery(VATS) pulmonary lobectomy and systematic mediastinal lymphadenectomy in surgical treatment of non-small cell lung cancer(NSCLC). Methods We reviewed the clinical data of 42patients with NSCLC received single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy from March 2011 to March 2013. The utility port for observation was about 1.5 cm long and located at the 7th and 8th rib in the middle axillary line. The port for operation was about 4.0-5.0 cm long and located at the 3rd and 4th rib in the outer edge of pectoralis major. Results All of the 42 patients were successfully subjected to single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy, none of them needed converse to transit-assisted incision or thoracotomy. Operative duration was 90-200 min; intraoperrative blood loss was 50-400 ml;lymph node dissection number was 9-16. After the surgery, the patients received chest drainage for 5-8 days. They were discharged from hospital in 6-10 days after the surgery without severe complications. Conclusions Based on the conventional three utility ports VATS, the single utility port VATS pulmonary lobectomy and systematic mediastinal lymphadenectomy caused less injury in surgical treatment of non-small cell lung cancer(NSCLC). It has certain clinical advantages and was a minimally invasive surgical treatment for selected patients with NSCLC.

Abstract:Minimally invasive techniques have been widely applied in general thoracic surgery. Compared with video-assisted thoracoscopic surgery (VATS), due to its theoretic superiority, robotic surgery is challenging the traditional position of VATS. With its unique advantages, including 3D vision and a high-freedom endowrist, it leads to easier lymph node dissection, more convenient blood vessel dissection, a shorter learning curve and competence for the completion of complex surgery. However, as a new surgical technology, the safety and efficacy of robotic-assisted thoracoscopic surgery (RATS) still need to be further verified. Thus, in this article, we review and summarize the application of RATS versus VATS in general thoracic surgery.

Abstract:Video-assisted thoracoscopic surgery (VATS) for a relatively large mediastinal tumor (5.0–10.0 cm) remains controversial. In addition, few studies have focused on comparing different surgical approaches for large mediastinal tumors. Therefore, this study aimed to compare the short-term outcomes between subxiphoid approach VATS, intercostal approach VATS, and traditional sternotomy for large-sized anterior mediastinal tumors.

[The Clinical Practice of Video-Assisted Thoracic Surgery (VATS)].

Abstract:From November 1992 to March 1994, thirty five patients were performed using VATS in our hospital. The procedures included blebectomy with pleurodesis 14 cases; wedge resection 10 cases; malignant pleural effusion, pleural biopsy and pleurodesis 4 cases; pleural biopsy 3 cases; lung tumor biopsy 2 cases; lung biopsy 2 cases. Converted into thoracotomy 2 cases because of the tumors could not be resected thoracoscopically. No death and severe complication. The procedures were successful for diagnosis and treatment in our series. Compared with standard thoracotomy, the operative trauma, postoperative pain, drainage volume, tube time, postoperative hospitalization were remarkably less in this group (P < 0.05).

Therapeutic Advances in Respiratory Disease

Abstract:Background: Pediatric bronchiectasis is a common respiratory disease in children. The use of video-assisted thoracoscopic surgery (VATS) for its treatment remains controversial. Objectives: The objective of our study was to compare and analyze the clinical efficacy of thoracoscopic surgery and thoracotomy in the treatment of pediatric bronchiectasis and summarize the surgical treatment experience of VATS in children with bronchiectasis. Design: Retrospective single-center cohort study. Methods: A retrospective analysis was conducted on the clinical data of 46 pediatric patients who underwent surgery with bronchiectasis at the Children’s Hospital of Chongqing Medical University from May 2015 to May 2023. The patients were divided into two groups: the VATS group (25 cases) and the thoracotomy group (21 cases). Comparative analysis was performed on various parameters including basic clinical data, surgical methods, operation time, intraoperative blood loss, transfusion status, postoperative pain, postoperative mechanical ventilation time, chest tube drainage time, length of hospital stay, incidence of complications, and follow-up information. Results: There were no statistically significant differences between the two groups of patients in terms of age, weight, gender, etiology, duration of symptoms, site of onset, and comorbidities ( p > 0.05). The operation time in the VATS group was longer than that in the thoracotomy group ( p < 0.001). However, the VATS group had better outcomes in terms of intraoperative blood loss, transfusion status, postoperative pain, postoperative mechanical ventilation time, chest tube drainage time, and length of hospital stay ( p < 0.05). The incidence of postoperative complications in the VATS group was lower than that in the thoracotomy group, although the difference was not statistically significant ( p = 0.152). Follow-up data showed no statistically significant difference in the surgical treatment outcomes between the two groups ( p = 0.493). Conclusion: The incidence of complications and mortality in surgical treatment of bronchiectasis is acceptable. Compared with thoracotomy surgery, VATS has advantages such as smaller trauma, less pain, faster recovery, and fewer complications. For suitable pediatric patients with bronchiectasis, VATS is a safe and effective surgical method.

Abstract:To explore clinical advantages of video-assisted thoracopic surgery(VATS) in thoracic surgery.The clinical datas of 106 patients with chest surgical diseases undergoing VATS From Mar. 2004 to Aug. 2008 were retrospectively analyzed. In the 106 cases, including 59 of spontaneous pneumothorax, 24 of pulmonary Inflammatory Pseudotumor、tuberculoma etc. benign pulmonary diseases, 6 of pulmonary cancer, 13 of mediastinal tumor ,2 of hemopneumothorax, 1 of pericardial effusion and 1 of pulmonary metastatic tumor. 56 cases VAMT were accomplished successful, while 7 cases converted to open surgery. the operating time ranged from 25 to 185 minutes, average 82.2 minutes. 5 postoperative complications occurred, in which 3 of persistent air leakage, 1 of thoracic cavity effusion and 1 of pneumonic infection, There was no operative death in the group. Therefore,thoracic disease by VATS has the advantages of micro-traumatic ,recover quickly, less postoperative pain, fewer complication and coincident cosmetology , so it is safe and reliable to select suitable patients
[Complete video-assisted thoracoscopic anatomic segmentectomy for pulmonary diseases: the early experiences]

Abstract:Objective: To evaluate the safety and feasibility of video-assisted thoracic surgery (VATS) anatomic segmentectomy for pulmonary diseases. Methods: Between November 2009 and July 2011, 20 patients received consecutive VATS anatomic segmentectomy by single surgical group. There were 3 male and 17 female, aging from 32 to 81 years with a mean of 53 years. The patients included 12 cases of non-small cell lung cancer (NSCLC) (5 cases of bronchioloalveolar carcinoma, 6 cases of adenocarcinoma, and 1 case of squamous cell carcinoma), 7 cases of benign diseases (3 cases of bronchiectasis, 2 cases of inflammatory pseudotumor, 1 case of tuberculosis, and 1 case of sclerosing hemangioma) and 1 case of metastasis tumor. The locations of resected segments included 1 anterior segment, 3 posterior segments, 4 apical segments, and 2 superior segments in the right side; and 5 lingular segments, 3 trisegments, and 2 superior segments in the left side. Simultaneously, 3 patients with bronchiectasis underwent segmentectomy and lobectomy, 1 of 6 patients with adenocarcinoma underwent lingulectomy and thymectomy. The pathological TNM stages of 12 NSCLC patients were 9 cases of T1aN0M0, 1 case of T1bN0M0 and 2 cases of T2aN0M0. Results: Of these 20 patients, the median operative time was 155 minutes (range, 120 to 235 minutes), the median blood loss was 50 ml (range, 10 to 600 ml), the median drainage duration was 3 d (range, 1 to 6 d), and the median hospital stay was 6 d (range, 3 to 9 d). One patient who had undergone lingulectomy had a 600 ml intraoperative bleeding from lingular artery, and the bleeding was controlled by suturing the rupture under VATS. Bloody sputum occurred in 2 patients, prolonged air leak occurred in one patient for 5 days, and one patient developed subcutaneous emphysema that spontaneously resolved. No mortality was observed for 30 days after the surgery. Conclusions: VATS anatomic segmentectomy is a feasible and safe technique with acceptable operative time, less blood loss, fewer complications, and shorter hospital stay.
