Childhood Atopic Dermatitis and Risk of Problematic Substance Use.

Abstract:1-year follow-up. Lymphomatoid contact dermatitis is a chronic ACD that forms part of the spectrum of pseudolymphomas. Continuous antigenic stimulation has been implicated in the induction of lymphoid cell proliferation.1Y3 Preservatives, dyes, rubber chemicals, metal compounds (gold sodium thiosulfate, cobalt naphthenate, and nickel sulfate), N-isopropyl-N-phenyl-pphenylenediamine, and phosphorous sesquisulfide are contactants that have been reported to elicit a pseudolymphoma response. Although it is considered a benign reaction, progression into cutaneous lymphoma or leukemia has been described, so patients should be followed up regularly. Allergen avoidance is the primary approach to the management, although topical immunosuppression may be prescribed. In our case, the diagnosis was made based on the patch test, the clinical resolution after total removal of the antigenic stimulus, and the histological findings of both the eyelid papule and the hydroxyethyl methacrylateYpatched area, which revealed a lymphocyte infiltration pattern consistent with type A LyP. Drugs have been reported to act as a true antigenic trigger inducing CD30 lymphomatoid reactions, but to date, no contact allergen has. However, because we cannot know for certain how LyP and ACD interact, we cannot determine whether acrylates are causing LCD LyP-like or inducing LyP itself. Furthermore, the number of cases reported of LCD overall is small, and etiopathogenesis is not well understood. This is, to our knowledge, the first case of ACD induced by acrylates mimicking LyP clinically and histologically. It is important to include LCD in the differential diagnosis of lesions with lymphomatoid histology, because this reaction is a diagnostic challenge and needs to be followed up for transformation to a true lymphoma.
