Abstract:Methods A total of 75 female patients with AIS, 86 healthy agematched girls and 40 age-matched patients with CS due to excrescent hemivertebra were included into this study. Long-cassette standing coronal radiographs of the spine and pelvis were obtained from the subjects in fiston-clavicle position. The radiographic parameters were measured including length of spine (LOS), height of spine (HOS), length of thoracic vertebrae (LOT), height of thoracic vertebrae (HOT), width of pelvis (WOP), height of pelvis (HOP) and width of thorax (WOT). SPI was defined as the ratio between length of the spine and height of the spine, namely LOS/HOP. Four age cohorts (10~11 yrs, 12~13 yrs, 14~15 yrs and 16~18 yrs) were determined in AIS patients and normal controls. In addition, the theoretical value of LOS in CS patients was calculated according to the normal SPI measured in healthy controls.
Ventilation/perfusion SPECT/CT in Patients with Severe and Rigid Scoliosis: an Evaluation by Relationship to Spinal Deformity and Lung Function

Abstract:Objective: Structural changes of the spine and chest wall associated with SRS result in poor cardiopulmonary function. Comprehensive assessment of pulmonary function is extremely important for patients with SRS before a correction operation. We explore the changes of distribution of lung ventilation and perfusion function in patients with SRS using SPECT/CT and describe the relation between these changes with Cobb angle and FVC%. Patients and methods: From March 2015 to August 2016, 16 consecutive SRS patients with a mean age of 20.1 years(range 11-36 years) were included in the study. Scoliotic parameters on radiographs were analyzed. FVC% were obtained by spirometry test. Lungs ventilation/perfusion single photon emission computed tomography scans was performed preoperatively in all patients to explore pulmonary ventilation and perfusion function changes. These changes were measured as the deviation form the normal perfusion and ventilation function distribution in right and left lung and correlated with the Cobb angle of main curve and FVC%. Results: The regional lung ventilation and perfusion function defects were not found in all SRS patients. Ventilation function deviation was a mean 5.7%(range, - 3.6% to 10.1%), significantly less than perfusion function deviation of 8.2%(range, - 0.3% to 22.2%) (P = 0.015, t = - 2.732). Lung ventilation and/or perfusion function deviation did not correlated with Cobb angle and FVC%, respectively. There was significant correlation between lungs ventilation and perfusion function deviation(P = 0.001, r = 0.753). Conclusion: The ventilation and perfusion function distribution were favourable in convex and concave side lung of SRS. Deformity bring about greater lungs perfusion function changes than ventilation function. The measurement of lung ventilation and perfusion function changes may represents an additional functional feature to assess pulmonary function of SRS more comprehensively.
Factors Affecting the Preoperative Pulmonary Function Test in Patients with Scoliosis: a Retrospective Evaluation of 108 Cases

Abstract:OBJECTIVES:To evaluate factors affecting the preoperative pulmonary function in patients with scoliosis.METHODS:Preoperative pulmonary function texts (PFTs) were performed on 108 patients with spinal deformities, 17 males and 91 females, aged (15.2 +/- 5.36) (7 - 27), 70 being with idiopathic scoliosis (IS) and 38 with congenital scoliosis (CS). The parameters affecting the preoperative pulmonary function were analyzed.RESULTS:The Cobb angle of scoliosis was negatively correlated with vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in one second (FEV1), peak expiratory flow (PEF), forced expiratory flow between 25% to 75% of forced vital capacity (FEF25-75), maxima mid-expiratory flow (MMEF), and maximal ventilatory volume (MVV), and not correlated with minute ventilation (MV) and ratio of forced expiratory volume in one second to forced vital capacity (FEV1/FVC%). The impairment of lung functions was more severe in the CS patients than in the IS patients. The degrees of impairment of lung functions in the groups of the Cobb angle > or = 90 degrees , with the number of vertebrae involved > or = 7, with the age of onset < 10, and of cephalad location of the curve were more severe than those of the groups with different corresponding parameters.CONCLUSION:The deterioration of pulmonary function mainly depends upon the age of onset, Cobb angle, location of the apex vertebra and the number of involved vertebrae.
Biplanar stereoradiography predicts pulmonary function tests in adolescent idiopathic scoliosis: a cross-sectional study

Abstract:Purpose: Various spinal and rib cage parameters measured from complex examinations were found to be correlated with preoperative pulmonary function tests (PFT). The aim was to investigate the relationship between preoperative rib cage parameters and PFT using biplanar stereoradiography in patients with severe adolescent idiopathic scoliosis. Methods: Fifty-four patients, 45 girls and nine boys, aged 13.8 ± 1.2 years, with Lenke 1 or 2 thoracic scoliosis (> 50°) requiring surgical correction were prospectively included. All patients underwent preoperative PFT and low-dose biplanar X-rays. The following data were collected: forced vital capacity (FVC), forced expiratory volume in 1 s (FEV1), FEV1/FVC ratio, residual volume, slow vital capacity (SVC), total lung capacity (TLC), rib cage volume (RCV), maximum rib hump, maximum width, mean thoracic index, spinal penetration index, apical vertebral rotation, main curve Cobb angle (MCCA), T4-T12 kyphosis. The primary outcome was the relationship between rib cage parameters and PFT. The secondary outcome was the relationship between rib cage parameters and spine parameters. Data were analyzed using Spearman's rank test. A multivariable regression analysis was performed to compare PFTs and structural parameters. Significance was set at α = 0.05. Results: The mean MCCA was 68.7° ± 16.7°. RCV was highly correlated with all pulmonary capacities: TLC (r = 0.76, p < 0.0001), SVC (r = 0.78, p < 0.0001) and FVC (r = 0.77, p < 0.0001). RCV had a low correlation with FEV1/FVC (r = - 0.34, p = 0.014). SPI was not correlated with any pulmonary parameters. Conclusion: Rib cage volume measured by biplanar stereoradiography may represent a prediction tool for PFTs. Improvement of pulmonary function after posterior spinal correction in severe scoliosis patients

Abstract:Objective To assess the recovery of pulmonary function after posterior spinal correction in severe scoliosis patients.Methods From 2007 to 2009,30 severe scoliosis patients(Cobb's angle>70°) with complete pulmonary function data,accompanied by pulmonary dysfunction,were enrolled in this study.There were 11 males and 19 females with an average age of 17.0 years(ranging from 10 to 36 years).Pulmonary function was evaluated by pulmonary function test(PFT) before surgery,3 and 24 months after surgery.Results The average Cobb's angles decreased from preoperative 109.1° to postoperative 65.6° with a correction rate of 43.0%.All the parameters of the pulmonary function,such as vital capacity(VC),forced vital capacity(FVC),and forced expiratory volume in 1 s(FEV1),demonstrated a slight increase with no statistical significance 3 months postoperatively(P>0.05).Two years after surgery,the absolute values of VC,FVC and FEV1 increased by 23.8%,23.6% and 25.6%,respectively;the percent-predicted values of VC,FVC and FEV1 increased by 17.6%,17.1% and 21.9%,respectively;compared with preoperation,the difference of PFT parameters,except for percent-predicted values of FEV1,had statistical significance after operation(P<0.05).There was a negative correlation between the preoperative PFT parameters and the Cobb's angles,and a positive correlation between the postoperative improvement rate of PFT parameters and preoperative Cobb's angles.Conclusion The pulmonary function of the scoliosis patients can be effectively improved by posterior spinal correction,and the pulmonary function increases along with time after surgery.The more severe scoliosis,the better recovery of pulmonary function.
Perioperative Evaluation and Treatment Strategy for Severe Scoliosis with Respiratory Failure

Abstract:Objective To investigate the preoperative evaluation of pulmonary function and protocol for scoliotic patients with respiratory failure. Methods From September 2000 to June 2008, sixteen patients suffered from respiratory failure were recruited into this study. There were two males and three females who were diagnosed as idiopathic scoliosis with a mean age of 13.5 years (range, 10-16 years). The other eleven patients were diagnosed as congenital scoliosis, including five males and six females with a mean age of 12.4 years (range, 10-14 years). The mean preoperative Cobb angle was 126.6°±15.5° with a mean height of 137.6 cm (range, 120-160 cm) and a mean weight of 32.5 kg (range, 18-40 kg). Decision was made depending on the severity of pulmonary impairment and spinal deformity. Preoperative treatment included respiratory training, noninvasive positive pressure ventilation therapy and halo-gravity traction. Results All patients received corrective surgery; ten patients were extubated tracheal tubes successfully 1 hour after surgery and six patients had to stay in intensive care unit (ICU) for 24 hours. Pulmonaryedema occurred in two cases and pneumonia in one case. The Cobb angle had improved to 72.0°±13.2° after surgery with a correction rate of 43%. All patients recovered well after surgery with no major cardiac or pulmonary complications.Conclusion Patients with preexisting respiratory failure can tolerate deformity correction operation with few complications after receiving a set of preoperative respiratory training modalities.
Novel Surface Topographic Assessment of Lung Volume and Pulmonary Function Tests in Idiopathic Scoliosis: A Preliminary Study

Abstract:Objective: Severe spinal deformity results in restrictive pulmonary disease from thoracic distortions and lung-volume limitations. Though spirometry and body plethysmography are widely accepted tests for pulmonary function tests (PFTs), they are time-consuming and require patient compliance. This study investigates whether surface topographic [surface topography (ST)] measurements of body volume difference (BVD) and torso volume difference between maximum inhale and exhale correlate to values determined on PFTs. Methods: This study included patients with idiopathic scoliosis and thoracic/thoracolumbar curves ≥40 degrees. Patients received ST scans, clinical examinations, and EOS biplanar radiographs on the same day. PFTs were performed within 3 months of ST/radiographic analysis. Univariate linear regression analysis was used to examine relationships between BVD, PFT values, and mean curves. Results: Sixteen patients (14.6 ± 2.2 y, 69% females) with idiopathic scoliosis and mean thoracic/thoracolumbar curves of 62 degrees ± 15 ̊ degrees (45 degrees to 93 degrees) were assessed. BVD displayed statistically high-positive positive correlations with forced vital capacity ( R = 0.863, P < 0.0001), forced expiratory volume in 1 second ( R = 0.870, P < 0.001), vital capacity ( R = 0.802, P < 0.0001), and TLC ( R = 0.831, P < 0.0001. Torso volume difference showed similarly high positive correlations to forced vital capacity, forced expiratory volume in 1 second, vital capacity, and TLC, but not residual volume. No correlations emerged between the mean thoracic/thoracolumbar curve and BVD or PFT values. Conclusion: This study strongly endorses further investigation into ST scanning as an alternative to traditional PFTs for assessing pulmonary volumes. The noncontact and noninvasive nature of ST scanning presents a valuable alternative method for analyzing thoracic volume, particularly beneficial for patients unable to cooperate with standard PFTs. Abstract:A self-control ratio, the spine-pelvis index (SPI), was proposed for the assessment of patients with adolescent idiopathic scoliosis (AIS) in this study. The aim was to evaluate the disproportionate growth between the spine and pelvis in these patients using SPI. A total of 64 female patients with thoracic AIS were randomly enrolled between December 2010 and October 2012 (mean age 13 years, standard deviation (sd) 2.17; 9 to 18) and a further 73 healthy female patients with a mean age of 12.4 years (mean age 12.4 years, sd 2.24; 9 to 18), were randomly selected from a normal control database at our centre. The radiographic parameters measured included length of spine (LOS), height of spine (HOS), length of thoracic vertebrae (LOT), height of thoracic vertebrae (HOT), width of pelvis (WOP), height of pelvis (HOP) and width of thorax (WOT). SPI was defined as the ratio LOS/HOP. The SPI and LOT/HOP in patients with AIS showed a significant increase when compared with normal girls (p < 0.001 and p < 0.001 respectively), implying an abnormal pattern of growth of the spine relative to the pelvis in patients with AIS. No significant difference in SPI was found in different age groups in the control group, making the SPI an age-independent parameter with a mean value of 2.219 (2.164 to 2.239). We also found that the SPI was not related to maturity in the control group. This study, for the first time, used a self-control ratio to confirm the disproportionate patterns of growth of the spine and pelvis in patients with thoracic AIS, highlighting that the SPI is not affected by age or maturity.
Evaluation of the Radiographic Risk Factors of Postoperative Shoulder Imbalance in Adult Scoliosis

Abstract:Objective This study aimed to evaluate the radiographic risk factors of postoperative shoulder imbalance (PSI) after adult scoliosis (AS) correction surgery. Methods Seventy-nine patients with AS undergoing correction surgery at a single institution were reviewed. The mean follow-up was 28 months. Patients were divided into two groups based on their radiographic shoulder height (RSH): (1) the balanced group (RSH <10 mm) and (2) the unbalanced group (RSH ≥10 mm). The preoperative and postoperative Cobb angles of the proximal thoracic (PT), main thoracic (MT), thoracolumbar/lumbar (TL/L) and upper instrumented vertebra (UIV) were measured. Results No significant difference was found between the balanced and unbalanced groups when the UIV was T1–2, T3–4, or below T4. Univariate analysis indicated that the unbalanced group had significantly higher postoperative RSH, lower percentage PT correction, and greater percentage MT correction. The classification and regression tree analysis revealed that when the correction percentage of PT curve was more than 55.3%, 84.4% of patients acquired shoulder balance. However, when the correction percentage of PT curve was less than 55.3%, and the correction percentage of MT curve was more than 56%, 65.7% of the patients developed PSI. Conclusions In AS correction surgery, a lower percentage correction of the PT curve and greater percentage correction of the MT curve were independent radiographic risk factors of PSI, regardless of the UIV level. Sufficient PT correction is required to achieve postoperative shoulder balance in AS correction surgery when the MT curve is overcorrected.

Pulmonary function after thoracoplasty and posterior correction in scoliosis

Abstract:Objective:To investigate the pulmonary function after thoracoplasty and posterior correction in scoliosis.Method:From January 2007 to December 2009,A total of 30(12 males and 18 females) consecutive scoliosis patients with the Cobb angle over 70 degrees were recruited for this study.All patients with the mean age of 18.8±6.2 years old underwent posterior correction and thoracoplasty on the convex side.The absolute and percent predicted values of vital capacity(VC),forced vital capacity(FVC),and forced expiratory volume in 1 second(FEV1) were evaluated by pulmonary function test.All these PFT parameters were collected before and 3 to 24 months after correction.Result:After surgery,the coronal Cobb angles decreased from 101.0°±19.9°(range,70°-140°) to 53.0°±20.9° with a correction rate of 49.2%.At the 3th month follow-up,the absolute values of VC,FVC and FEV1 decreased 15.0%,16.5% and 12.7% respectively,while the percent predicted values of VC,FVC and FEV1 decreased 15.8%,17.3% and 13.1% respectively.All the parameters of the pulmonary function demonstrated a slight increase.8 cases with 2-year follow-up had parameters a little bit higher than preoperative baseline,with no statistical significance(P>0.05).The increased absolute values and percent-predicted values of these parameters were associated with recovery time after operation.Conclusion:The short-term pulmonary function decreases after thoracoplasty and posterior correction in scoliosis patients,however gradually returns to baseline with time going.
Flexibility of the thoracic curve and three-dimensional thoracic kyphosis can predict pulmonary function in nonoperatively treated adult patients with adolescent idiopathic scoliosis

Abstract:Background: Although several radiographic predictors for pulmonary function in adolescent patients have been reported, those in adult patients remain unclear. Therefore, we aimed to investigate the associations between spinal deformity and pulmonary function in nonoperatively treated adult patients with adolescent idiopathic scoliosis (AIS). Methods: Of 319 patients treated nonoperatively for AIS, 90 (average age, 40.0 ± 6.5 years) underwent both full-length standing radiographs and pulmonary function test. Standard two-dimensional (2-D) radiographic measurements were performed. Three-dimensional thoracic kyphosis (3-D TK) was calculated from 2-D standing radiograph data using a validated formula: 3-D TK (°) = 18.1 + 0.81 × (2-D TK) + 0.54 × (Cobb angle of thoracic curve). 3-D TK was defined as the sum of segmental kyphosis between T5 and T12, which eliminates the overestimation of TK in 2-D measurements due to rotational deformity. Bivariable correlation analysis, followed by a stepwise multiple linear regression analysis, was performed. Results: The average Cobb angle of the thoracic curve at the time of survey was 49.4° ± 14.6° with flexibility of 37.5% ± 18.2%. Thoracic curve magnitude, flexibility, apical vertebral rotation and translation, and 3-D TK were significantly correlated with percent-predicted forced vital capacity (%FVC) and expiratory volume in 1 s (%FEV1.0). Stepwise multiple regression analysis showed that curve flexibility and 3-D TK were significant, independent predictors of %FVC (R2 = 0.358) and %FEV1.0 (R2 = 0.335), curve flexibility having a greater impact (standardized coefficient > 0.45) than 3-D TK (<0.32). Conclusions: Our results indicate that nonoperatively treated patients with AIS should be recommended to maintain flexibility of the thoracic curve to prevent future pulmonary impairment. Moreover, 3-D TK is another independent predictor of pulmonary function, which suggests that segmental sagittal alignment is a component of deformity correction to focus on.
Schroth exercises improve health-related quality of life and radiographic parameters in adolescent idiopathic scoliosis patients

Abstract:Background: Finding an optimal treatment strategy for adolescent idiopathic scoliosis (AIS) patients remains challenging because of its intrinsic complexity. For mild to moderate scoliosis patients with lower skeletal growth potential (Risser 3–5), most clinicians agree with observation treatment; however, the curve progression that occurs during puberty, the adolescent period, and even in adulthood, remains a challenging issue for clinicians. The aim of the study is to investigate the efficacy of Schroth exercise in AIS patients with lower skeletal growth potential (Risser 3–5) and moderate scoliosis (Cobb angle 20°–40°).Methods: From 2015 to 2017, data of 64 patients diagnosed with AIS in Peking University Third Hospital were reviewed. Forty-three patients underwent Schroth exercise were classified as Schroth group, and 21 patients underwent observation were classified as observation group. Outcomes were measured by health-related quality of life (HRQOL) and radiographic parameters. HRQOL was assessed using the visual analog scale (VAS) scores for back, Scoliosis Research Society-22 (SRS-22) patient questionnaire. Radiographic spinopelvic parameters were obtained from anteroposterior and lateral X-rays. The pre-treatment and post-treatment HRQOL and radiographic parameters were tested to validate Schroth exercise efficacy. The inter-rater reliability of the radiographic parameters was tested using the interclass correlation coefficient (ICC). The paired t test was used to examine HRQOL and radiographic parameters. Clinical relevance between C2-C7 sagittal vertical axis (SVA) and thoracic kyphosis was analyzed using Spearman correlation.Results: In Schroth group, VAS back score, SRS-22 pain, and SRS-22 self-image domain were significantly improved from pre-treatment 3.0 ± 0.8, 3.6 ± 0.5, and 3.5 ± 0.7 to post-treatment 1.6 ± 0.6 (t = 5.578, P = 0.013), 4.0 ± 0.3 (t = −3.918, P = 0.001), and 3.7 ± 0.4 (t = −6.468, P

Abstract:Background context: Pulmonary function in patients with scoliosis has been a topic of concern, with some reports of markedly decreased ventilatory function leading to disability and increased mortality in patients with severe scoliosis. Only limited data is available concerning pulmonary function in adult patients with scoliosis. Purpose: To report the long-term pulmonary function (PF) in patients diagnosed with idiopathic scoliosis (IS) compared with an age-matched population using extended pulmonary function testing (EPFT). Study design/setting: Retrospective clinical follow-up. Patient sample: A total of 177 patients seen at our institution from 1972 to 1983 for a pediatric spinal deformity were assessed for inclusion in the study. About 77/129 eligible patients with IS (60%) partook in a clinical examination including radiographs, and EPFT. Outcome measures: The EPFT values included forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV1/FVC ratio, vital capacity (VC), total lung capacity (TLC), residual volume (RV), RV/TLC ratio, diffusion capacity of carbon monoxide (DLco), carbon monoxide transfer coefficient (KCO) and alveolar volume (VA). Results were expressed with z-scores derived from height and arm span normative data. Z-scores were calculated as z-score=Measured PF-Predicted PF /Relative standard deviation (RSD). The limits of normal, are defined as the 5th and 95th percentile limits (z-score between -1.645 and 1.645), respectively. Methods: Patients underwent a clinical examination with full spine standing radiographs and EFPT. The results were compared between patients with thoracic and thoracolumbar/lumbar (TL/L) main curves, and overall compared with a background population. Results were expressed with z-scores derived from height and arm span normative data. Results: Of 77 included patients, 76 (99%) were females with a mean age of 54.6±2.5 years. The mean follow-up time was 40.8±2.8 years. Forty-four patients had thoracic main curves, and 33 had TL/L main curves. We found no pulmonary impairment based on z-scores in the total cohort or between groups, with only patients who were current or previous smokers, having z-scores below the normal limits Patients with main thoracic curves displayed significantly lower PF on mean absolute values and mean z-scores on FEV1, FVC, FEV1/FVC ratio, VC, TLC, and DLco compared with main TL/L curves. Patients with thoracic curves had significantly larger Cobb angles at follow-up; 52±17° compared with 40±22° (p-value <.05) in the TL/L group. We found no linear association between thoracic Cobb angle and degree of pulmonary impairment assessed with DLco, TLC, and FVC. Comparison of pulmonary z-scores based on arm span data, differed significantly on FVC and TLC, with the arm span measurements showing lower mean z-scores (p-value <.05). Conclusions: Using EPFT, no pulmonary impairment could be demonstrated compared to the age-matched population 40 years after a diagnosis of IS. However, patients with thoracic curves had decreased PF compared to patients with TL/L curves although within the normal range. Thus, when treated as current guidelines suggest, patients with idiopathic scoliosis can expect the same long-term pulmonary function as the general population.

Abstract:Structured Background Context Preoperative supine radiographs are mandatory in the new AdIS classification. Supine radiographs are easily reproducible and highly predictive of side bending radiographs. However, few studies evaluated the use of supine radiographs in predicting postoperative curve correction after PSF in AdIS. Purpose To investigate the use of supine and side bending (SB) radiographs in predicting postoperative curve correction in Adult Idiopathic Scoliosis (AdIS) patients who underwent Posterior Spinal Fusion (PSF). Study Design Retrospective study Patient sample 93 AdIS patients who underwent PSF between 2022 and 2023 were included. Outcome Measures Demographic data were age, gender, height, weight, BMI, Risser grade, Lenke curve types and Cobb angles. Main outcome measures are pre-operative and immediate post-operative Cobb angle (proximal thoracic (PT), main thoracic (MT) and thoracolumbar/lumbar (TL/L) curves), Supine Cobb angle and Flexibility rate (PT, MT and TL/L), and Correction Rate (PT, MT and TL/L) Methods Correlation study was performed between supine Cobb angle vs. post-operative Cobb angle for PT, MT and TL/L curves. A predictive formula was derived from the correlation plots. Results A total of 93 subjects were included in our study with a median age of 24.7 years and comprised of 80 females (86.0%). Preoperative Supine Cobb angle ( r =0.835, r =0.881, r =0.767, p<0.001) and preoperative SB Cobb angle ( r =0.815, r =0.872, r =0.801, p<0.001) showed similar strong positive correlation with postoperative PT, MT and TL/L Cobb angle, but preoperative Supine Cobb angle had slightly stronger correlation in PT and MT, whereas preoperative SB Cobb angle had stronger correlation in TL/L curve. Using the derived predictive formulae, there was a significant, strong, positive correlation between the predicted value and actual value of postoperative standing Cobb angle, (r=0.852, P<0.001), with 71.0% of the patients had predicted postoperative Cobb angle from the supine radiographs within 5 degrees of the actual value. Conclusion Both supine radiographs and side bending radiographs had strong predictability of the postoperative Cobb angle for PT, MT and TL/L curves. In 71.0% of patients, the actual postoperative Cobb angle was within 5 degrees of the predicted postoperative Cobb angle using the predictive formulae.

Abstract:Study Design:Prospective clinical study.Objectives:To investigate the factors correlated with pulmonary function in adolescent idiopathic scoliosis (AIS) patients, and to determine the influence of body mass index (BMI) on the pulmonary function.Summary of Background Data:Impaired pulmonary function has been reported in patients with AIS. The factors reported to affect pulmonary function included the curve magnitude, location of the apex vertebrae, decrease in thoracic kyphosis, and the number of vertebrae involved. A low BMI has been well confirmed in AIS patients. However, the relationship between BMI and pulmonary function has never been investigated.Methods:One hundred twenty female patients with a single thoracic curve were enrolled in this study. Pulmonary function test and radiographic examination were performed. Several radiographic parameters, including the curve magnitude, the curve type, location of the apical vertebra, the number of vertebrae involved, and the degree of thoracic kyphosis, were recorded. BMI was calculated according to the measurements of standing height and body weight. Patients were divided into 2 groups according to BMI, (group A >17.5 kg/cm(2) and group B 17.5 kg/cm(2)), and the pulmonary function was compared between the 2 groups with students t test. Partial correlation analysis and stepwise multiple regression analysis were used to assess the association between BMI and the results of the pulmonary function test. The receiver operating characteristics curve was created to identify the best dividing point for diagnostic variables.Results:The average percentage of predicted vital capacity, forced vital capacity, and forced expiratory volume in 1 second was significantly decreased in patients with a lower BMI. A positive correlation was found between BMI and pulmonary function (r=0.41, P<0.001). The stepwise regression analysis showed that low BMI was significantly associated with pulmonary function impairment (R-2=0.206, P<0.001). According to the receiver operating characteristics curve, patients with a BMI <17.7 kg/cm(2) are prone to have abnormal pulmonary function.Conclusions:BMI was found to be an important indicator for pulmonary function in AIS patients. The decreased BMI could explain approximately one-fifth of the variation in pulmonary impairment in AIS patients, which might shed some light on the rehabilitation of preoperative pulmonary function.
Spine curvature and the cardiopulmonary exercise endurance of adolescents with idiopathic scoliosis

Abstract:Objective:To quantify any correlation between the severity of spinal curvature of an adolescent with idiopathic scoliosis and their cardiopulmonary exercise endurance.Methods:The cardiopulmonary exercise test (CPET) results and the full-length spinal X-rays in a standing position of 64 adolescents with idiopathic scoliosis were reviewed retrospectively. Independent t-tests were used to compare the two datasets obtained from those with left or right thoracic scoliosis. The correlation between the Cobb angle and cardiopulmonary exercise endurance was analyzed using Pearson correlation coefficients, multiple factor linear regression and two-stage linear regression.Results:After adjusting for gender, age, height and weight, the multiple linear regression analysis showed that the Cobb angle was significantly negatively correlated with maximum tidal volume (β=-0.013) and significantly positively correlated with the rate of respiration (β=0.421). The relationship between the Cobb angle and cardiopulmonary exercise endurance was non-linear. With a Cobb angle > 34°, a 1° increase reduces cardiopulmonary exercise endurance by a factor of 1.4 on average. At smaller Cobb angles the corresponding increase is about 0.87 times.Conclusions:The Cobb angle is a negative predictor of ventilation during exercise among adolescents with idiopathic scoliosis. The more severe a patient′s spinal curvature, the lower the cardiopulmonary exercise endurance is likely to be.
Radiographic analysis of spino-pelvic sagittal alignment and prediction of lumbar lordosis in Chinese asymptomatic subjects

Abstract:Introduction In the past 10 years, the importance of the sagittal shape of human pelvis has been pointed out by the orthopedists. However, most studies were defined in Western and Korean populations. It is unclear whether these characteristics of Chinese spinopelvic sagittal alignment differ from those of western and Korean populations. In addition, the predictive equation of lumbar lordosis remains unclear. This study described normal types of spinopelvic sagittal alignment in Chinese asymptomatic subjects, and investigated a prediction equation of lumbar lordosis from selective spinal and pelvic sagittal parameters. Material and Methods This was a prospective radiological analysis using full-spine standing lateral radiographs of Chinese volunteers. A total of 171 volunteers (94 males and 77 females; mean age, 23 years [range, 18–28 years]) participated. Pelvic and spinal parameters were measured according to geometric characteristics. The Pearson correlation test was calculated, regression analysis was running with the lumbar lordosis (predicted variable), and the other spinal and pelvic parameters (predictor variables), to determine the best sets of predictors to include in the model. Results Mean pelvic incidence was 44.6 ± 9.5 degrees, significantly less than that reported in Western subjects. The volunteers were classified into four types according to the location of their apex of total lumbar lordosis. As the apex of total lumbar lordosis moved cranially, lordosis tilt decreased, whereas lower lumbar lordosis, pelvic incidence, sacral slope, and the number of vertebral bodies included in the lower lumbar lordosis increased. On the basis of the pelvic incidence, total lumbar lordosis predictive equation was 0.639 PI + 20.593 degrees. Preliminary prediction of lower lumbar lordosis was PI-10 degrees. Conclusion The sagittal alignment of Chinese population could be classified into four types, and was different from that of Caucasian. Lower lumbar lordosis comprised most of total lumbar lordosis, the role of pelvic incidence in determining the sagittal curve was mainly through the adjustment of lower lumbar lordosis. Using regression analysis, total lumbar lordosis could be precisely predicted in asymptomatic subjects. To optimally plan surgery for spinal deformity, the prediction of postoperative lower lumbar lordosis curvature was equally important as that of total lumbar lordosis.
Assessment of Lung Morphology in Adolescent Idiopathic Scoliosis Following Posterior Spinal Fusion by Three-Dimensional Computed Tomography

Abstract:Objective To evaluate the changes in the lung volume,lung height,convex to concave lung volume ratio and pulmonary cross-sectional area of apical vertebral plane after posterior spinal fusion and to further analyse the relation between postoperative lung volume and pulmonary function.Methods From April 2009 to August 2013,a total of 30 patients were involved in this study.There were 9 males and 21 females,whose age ranged from 11.3 to 18.0 years old,with an average age of 15.7 years.The Risser sign of the subjects were 11 Risser Ⅲ,7 Risser Ⅳ,12 Risser Ⅴ,while the Lenke's classification were 19 Lenke Ⅰ,4 Lenke Ⅱ and 7 Lenke m.All scans were obtained during deep inspiration breath-hold in supine position and performed both preoperatively and a week after operations.Then,we used Syngo software to manage the computed tomography scan imaging and to calculate the lung volume,the lung height and the pulmonary cross-sectional area of apical vertebral plane.Results Left lung height increased from (21.4±2.6) cm to (22.6±2.5) cm postoperatively.Right lung height was (21.9±2.1) cm while it increased to (22.7±2.7) cm postoperatively.The pulmonary cross-sectional area of apical vertebral plane decreased to (223.1 ±38.4) cm2 postoperatively.Left lung volume,right lung volume,total lung volume and convex to concave lung volume ratio showed no statistically significant difference.The changes in left and right lung height and the pulmonary cross-sectional area of apical vertebral plane showed no significant correlation with patients' sex,age,Lenke's classification,Risser sign,and the correction of predominant thoracic curve or kyphosis angle from T5 to T12.Conclusion The current study demonstrates the changes in lung morphology in AIS patients a week after posterior spinal fusion.It shows that lung height in AIS patients immediately increases postoperatively.However,the postoperative lung volume shows no statistical difference. Abstract:AbstractPurposeEOS imaging system is accessible to clinicians since 2007, allowing 3D spinal reconstructions in a functional standing position with reduced radiation. However, numerous ongoing research protocols continuously help implementing the dedicated software. The main principle and applications of the EOS device are discussed here, with an emphasis on future projects. In particular, the authors studied the postoperative modification of the rib cage and spinal morphology after posteromedial correction, in a consecutive series of adolescent idiopathic scoliosis (AIS) patients.Methods49 thoracic AIS patients underwent low-dose stereoradiography preoperatively, postoperatively and at latest examination, with a minimum 2-year follow-up. Spinal and rib cages 3D reconstructions were obtained using dedicated software, and the postoperative modification of thoracic parameters was reported.ResultsAll parameters were significantly improved after surgery. Mean thoracic volume increase was 8.4 % (±8), influenced by the postoperative derotation of the apical vertebra. No difference was found in thoracic volume increase in patients who gained more than 10° of thoracic kyphosis. A significant correlation was found between spinal penetration index at the apex and thoracic sagittal alignment (p = 0.02).ConclusionsEOS imaging device now reliably provides a global 3D quantitative analysis of scoliotic deformities in a context of routine clinical use. This innovative tool will help in the future to better understand scoliosis physiopathology and to evaluate treatment strategies.

Dynamic 3D Reconstruction of Thoracic Cage and Abdomen in Children and Adolescents With Scoliosis: Preliminary Results of Optical Reflective Motion Analysis Assessment

Abstract:Background: In patients with untreated scoliosis or in those with posterior spinal instrumented fusion (PSF), the movements of neither the thoracic cage (ThC) nor the abdomen (ABD) during quiet and deep breathing have been well defined in the literature. The purpose of this study was to evaluate kinematic variations in the ThC and ABD during quiet and deep breathing by optical reflective motion analysis (ORMA) in children with scoliosis. Methods: The study included 6 healthy children (group A), 7 subjects with untreated scoliosis over 50 degrees (group B), and 8 patients with scoliosis treated by PSF (group C). After anthropometric measurements (standing height, sitting height, arm span, chest perimeter, body weight, body mass index, T1-T12, and L1-L5 length) were obtained, the movements of subjects during quiet and deep breathing were measured with a 10-camera 3-dimensional ORMA system (82 markers) with the subjects in a standard standing position. Results: No significant differences were observed in sex, age, weight, height, or arm span (P>0.05). Significant differences were observed in the chest perimeter, Cobb angle, and body mass index (P<0.05). ThC and ABD movements during quiet and deep breathing decreased significantly in group B and C when compared with group A (P<0.05). Group B showed decreased expansion of the ThC (-52.4% to -58.3%) and relatively increased motion of the ABD compared with groups A and C (P<0.001). However, ABD expansion remained lower in group B than in groups A and C (-32.8% and -5.7%). PSF does not completely eliminate transverse plane kinematics, although a greater reduction was observed at instrumented than noninstrumented levels (-60.8% vs. -35.1%; P<0.05). Conclusions: ORMA is a useful tool for assessing alterations in the kinematics of the ThC and ABD caused by severe scoliosis and/or PSF. Compared with normal subjects, patients with severe scoliosis had poorer and less effective kinematics of the ThC and ABD. In contrast, operated subjects had better and more effective kinematics of the ThC and ABD, breathing curves, thoracic expansion, and abdominal movements closer to normal compared with patients with severe, untreated deformity.
