Abstract:Rationale and Objectives: To identify CT features for distinguishing grade 1 (G1)/grade 2 (G2) from grade 3 (G3) pancreatic neuroendocrine tumors (PNETs) using different machine learning (ML) methods. Materials and Methods: A total of 147 patients with 155 lesions confirmed by pathology were retrospectively included. Clinicaldemographic and radiological CT features was collected. The entire cohort was separated into training and validation groups at a 7:3 ratio. Least absolute shrinkage and selection operator (LASSO) algorithm and principal component analysis (PCA) were used to select features. Three ML methods, namely logistic regression (LR), support vector machine (SVM), and K-nearest neighbor (KNN) were used to build a differential model. Receiver operating characteristic (ROC) curves and precision-recall curves for each ML method were generated. The area under the curve (AUC), accuracy rate, sensitivity, and specificity were calculated. R esults: G3 PNETs were more likely to present with invasive behaviors and lower enhancement than G1/G2 PNETs. The LR classifier yielded the highest AUC of 0.964 (95% confidence interval [CI]: 0.930, 0.972), with 95.4% accuracy rate, 95.7% sensitivity, and 92.9% specificity, followed by SVM (AUC: 0.957) and KNN (AUC: 0.893) in the training group. In the validation group, the SVM classier reached the highest AUC of 0.952 (95% CI: 0.860, 0.981), with 91.5% accuracy rate, 97.3% sensitivity, and 70% specificity, followed by LR (AUC: 0.949) and KNN (AUC: 0.923). Conclusions: The LR and SVM classifiers had the best performance in the training group and validation group, respectively. ML method could be helpful in differentiating between G1/G2 and G3 PNETs. How Reliable is the Ki-67 Cytological Index in Grading Pancreatic Neuroendocrine Tumors? A Meta-Analysis

Abstract:ObjectiveTo investigate the accuracy of the cytological Ki-67 index in distinguishing intermediate and high-grade (G2 + G3) from low-grade (G1) pancreatic neuroendocrine tumors (PNETs).MethodsTwo investigators independently searched databases to identify eligible studies using the following term: (Ki-67') AND (pancreatic endocrine tumor' OR pancreatic neuroendocrine tumor' OR pancreatic endocrine tumour' OR pancreatic neuroendocrine tumour' OR pancreatic endocrine tumors' OR pancreatic neuroendocrine tumors' OR pancreatic endocrine tumours' OR pancreatic neuroendocrine tumours'), and meta-analysis was performed to calculate the pooled sensitivity, specificity, positive (PLR) and negative likelihood ratio (NLR), and diagnostic odds ratio (DOR).ResultsA total of 263 lesions from 13 studies were included in the study. The pooled sensitivity and specificity of Ki-67 (cut-off value: 2%) in the differential diagnosis of G2 + G3 from G1 PNETs were 64% and 87%, respectively. The pooled PLR, NLR and DOR were 3.96, 0.42 and 11.21, respectively. The area under the summary receiver operating characteristic curve (AUROC) was 0.8397. While the cut-off value of Ki-67 index was set as 5%, the sensitivity and specificity were increased up to 69% and 93%, respectively, and the AUROC was increased to 0.955.ConclusionThe cytological Ki-67 index is very useful in distinguishing intermediate and high-grade from low-grade PNETs, and a cut-off value of 5% had a better predictive value compared with that of 2%.

Abstract:Objective The World Health Organization (WHO) grading system for the stratification of G1 and G2 pancreatic neuroendocrine tumors (pNETs) using an optimal Ki-67 index cutoff is still controversial. The present study aimed at finding one optimal Ki-67 cutoff value that distinguishes G1 and G2 tumors by analyzing the prognosis of patients with pNET in our center. Methods Data from 84 patients with pNET undergoing surgical resection in The First Affiliated Hospital of Sun Yat-sen University between March 2003 and October 2015 were retrospectively analyzed. Results The 5-year overall survival rate was 74.2%. Univariate analysis revealed that functional secretion, WHO grade, and TNM stage were significantly associated with long-term survival (all P < 0.05). Multivariate analysis demonstrated that WHO grade (P = 0.023) and TNM stage (P = 0.040) were independent prognostic factors. The receiver operating characteristic curve showed that the Ki-67 index of 5% had the best predictive ability (76.7%) for 5-year survival with a hazard ratio of 44.7. The hazard ratio was only 8.14 when the Ki-67 index cutoff was 2%. Conclusions TNM stage and WHO grade were independent prognostic factors of pNETs. A Ki-67 index of 5% is better than 2% in stratifying G1 and G2 pNET tumors.
[Accurate Grading of Pancreatic Neuroendocrine Tumors with Ki-67 Index in Fine-Needle Aspiration Specimens: a Comparative Cytologic and Histologic Study].

Abstract:Objective: To study the cytomorphologic features and determine whether pancreatic neuroendocrine tumors (PanNET) sampled by fine-needle aspiration (FNA) can be accurately graded based on the Ki-67 index when compared to surgical samples. Methods: Corresponding intraoperative (19 cases) or endoscopic ultrasound-guided (3 cases) FNA cytology and surgical tissue specimens were obtained from 22 tumors, which were reviewed and stained for Ki-67 proliferation marker. The cytological samples included more than 200 tumor cells. Samples were graded by scoring the Ki-67 positive index in accordance with the 2010 WHO criteria. The grading scores assigned to the FNA cytology samples were compared with the scores assigned to the corresponding histological samples. Concordance was achieved by using 5% (instead of 2%) as a cut-off value for defining G2 tumors. One cytological sample included less than 500 tumor cells was excluded in the concordance calculation. Results: The cytological smears consisted of uniform, monotonous and isolated cells, loose cellular aggregates and rosette-like formations. Some tumor cells clustered around segments of capillaries. The cells demonstrated distinct cytoplasmic and nuclear features. Mitoses and necrosis were rarely seen. When traditional 2% Ki-67 index cut-off value were used to classify G2 tumors, the majority (86.4%, κ=0.812, P<0.01) of FNA cytology samples and corresponding surgical tissue specimens demonstrated concordance. When a 5% cut-off value was adopted, the concordance rate was 95.5% (21/22, κ=1.000, P<0.01). Similar concordance rates between the cytological and histological grades were achieved with threshold value of cytological assessment material set at more than 500 or 200 cells. Conclusions: The cytological Ki-67 index in adequate material (>200 tumor cells) is useful in grading pancreatic neuroendocrine tumors, and a cut-off value of 5% showed better predictive value compared with that of 2%. Accurate grading of PanNET is critical for predicting tumor biology, patient prognosis, and making informed decisions regarding patient management and treatment.
Concordance Between the Ki-67 Index Cutoff Value of 55% and Differentiation in Neuroendocrine Tumor and Neuroendocrine Carcinoma in Grade 3 Pancreatic Neuroendocrine Neoplasms.

Abstract:OBJECTIVE:In 2017 and 2019, the World Health Organization defined grade 3 neuroendocrine tumors (G3 NETs) and neuroendocrine carcinoma (G3 NEC) in the pancreas. The validity of this classification remains to be verified. METHODS:Clinical data were collected and analyzed for 39 G3 pancreatic neuroendocrine neoplasms (PanNENs) patients between 2009 and 2018. RESULTS:The tumor-node-metastasis stage (P = 0.0260), differentiation (P = 0.0115), and Ki-67 index (P = 0.0371) are prognostic factors for G3 PanNENs by Kaplan-Meier survival analysis. Among 39 patients, 18 had a Ki-67 index of less than 55% and well-differentiated morphology (G3 NET) and 16 had a Ki-67 index of 55% or greater and poorly differentiated morphology (G3 NEC). Grade 3 neuroendocrine tumor had a significant better prognosis than G3 NEC (median overall survival time, 25 months [95% confidence interval, 10.854-39.146 months] vs 12 months [95% confidence interval, 6.316-17.684 months], P = 0.0164). Based on Cox regression analyses, tumor-node-metastasis stage (P = 0.016) was identified as the independent prognostic factor for G3 PanNENs. CONCLUSIONS:The upper Ki-67 index cutoff of 55% might be the best cutoff value to define G3 NETs and G3 NECs for G3 PanNENs. The World Health Organization 2017 and 2019 classification system for G3 PanNENs can identify high-risk patients with G3 PanNENs.
Pancreatic neuroendocrine neoplasms at magnetic resonance imaging: comparison between grade 3 and grade 1/2 tumors.

Abstract:BACKGROUND:The grading of pancreatic neuroendocrine neoplasms (PanNENs) is associated with the choice of treatment strategy. The aim of this study is to identify the magnetic resonance imaging (MRI) features in differentiating pancreatic neuroendocrine tumors (PanNETs) grade 1/2 (G1/G2) and pancreatic neuroendocrine carcinoma grade 3 (PanNEC G3). PATIENTS AND METHODS:A total of 59 patients with histologically proven PanNENs and who underwent pretreatment MRI were retrospectively analyzed. Tumor location, size, boundary, cystic or solid appearance, enhancement degree, pancreatic duct dilatation, metastases and MRI signal were evaluated. Apparent diffusion coefficients (ADCs) were measured on ADC maps. Receiver operating characteristic curve was used to determine the cut off values and the sensitivity and specificity of prediction. Spearman correlation and logistic regression analysis were adopted to identify the association between MRI features and pathological parameters. RESULTS:A total of 47 lesions were PanNETs G1/G2 and 12 lesions were PanNEC G3. G1/G2 tumors were more common with well-circumscribed border compared with PanNEC G3. Ill- defined boundary, big size, necrosis, low-moderate enhancement, pancreatic duct dilatation, metastases and high diffusion-weighted imaging (DWI) intensity were more common in PanNEC G3 than in PanNETs G1/G2. The ADC values of PanNEC G3 were also significantly lower compared with the PanNETs G1/G2 and normal pancreatic parenchyma. The cut off value of ADC was 0.95×10-3 mm2/s for differentiating PanNEC G3 from PanNETs G1/G2 with 72.3% sensitivity and 91.6% specificity, respectively. Ki-67 index and mitosis count positively correlated with tumor size, pancreatic duct dilatation and metastases (P<0.05) and negatively correlated with ADC values (P<0.01), respectively. Regression analysis further showed that metastases and ADC value were associated with PanNENs grade. CONCLUSION:Metastases and ADC value may have potential for differentiating PanNEC G3 from PanNETs G1/G2.
Enhanced computed tomography features predict pancreatic neuroendocrine neoplasm with Ki-67 index less than 5%

Abstract:BACKGROUND: Several studies have suggested that patients with pancreatic neuroendocrine neoplasm (pNEN) with the Ki-67 index of < 5% are more likely to show better prognosis after clinical intervention. Moreover, the Ki-67 index at 5% has also been suggested as a potential threshold by the 2016 European Neuroendocrine Tumor Society guidelines.OBJECTIVE: Based on preoperative enhanced computed tomography (CT), this study aimed to investigate imaging characteristics eligible to discriminate the ≤ 5% Ki-67 group from the > 5% Ki-67 group of patients with nonmetastatic pNEN.METHODS: Patients with pathologically diagnosed pNEN and preoperative multiphase CT were enrolled. Their Ki-67 index was calculated and grouped according to the 5% cutoff value. The following CT imaging characteristics and some serum biomarkers were assessed between the two groups: the diameter, location, tumor margin, calcification, pancreatic atrophy, distal pancreatic duct dilation, vessel involvement, and enhancement pattern characteristics of both arterial phase (AP) and portal vein phase (PVP).RESULTS: A total of 142 patients with pNEN were enrolled in this study, comprising 104 in the low (Ki-67, 1%-5%) and 38 in the high index group (Ki-67, >5%). Alpha fetoprotein and cancer antigen 125 were significantly different between the two groups (P-values, 0.030 and 0.049, respectively). The diameter (P < 0.0001), margin (P = 0.003), distal main ductal dilation (P = 0.021), vessel involvement (P = 0.002), AP hypoenhancement (P < 0.0001), PVP hypoenhancement (P = 0.003), AP ratio (P = 0.0001), and PVP ratio (P = 0.0003) were significantly different between the low and high index groups. The area under the curve of the multivariate logistic regression model was 0.853.CONCLUSION: Nonmetastatic pNENs with larger diameter, ill-defined margin, distal main ductal dilation, and tumor hypoenhancement in AP in preoperative enhanced CT tend to have a Ki-67 index of > 5%.The results of this study provide an alternative method to clinicians to decide whether surgery is appropriate.

Abstract:Background The accurate estimation of the Ki-67 index of well-differentiated pancreatic neuroendocrine tumors (PanNET) not only guides curative management but also avoids unnecessary treatment. Purpose To investigate whether magnetic resonance imaging (MRI) features could preoperatively discriminate the Ki-67 index of well-differentiated G1/G2 PanNETs and to construct an individualized predictive model through developing a MRI-based nomogram. Material and Methods Ninety-nine patients with G1/G2 PanNETs were divided into Ki-67 index <5% (n = 58) and Ki-67 index >= 5% groups (n = 41). Multiparametric MRI features of two groups were assessed via uni- and multivariate logistic regression models. The MRI-based nomogram was then developed based on multivariable logistic analysis; the effectivity of the nomogram was validated according to the C-index, calibrations, and a decision curve analysis. Results MRI features, including tumor size (odds risk [OR] = 7.791; P = 0.003) and the ADC values (OR = 0.245, P = 0.018) were significant independent risk factors for the high Ki-67 index (>= 5%) of G1/2 PanNETs at multivariable analysis. The performance of the MRI-based nomogram (with a C-index of 0.825) was improved compared with that based on tumor size and ADC alone (with C-index values of 0.709 and 0.716, respectively). The calibration curve of the nomogram exhibited good consistency between evaluated and observed outcomes. The decision curve showed that the nomogram incorporating two MRI features had better clinical utilization than single features. Conclusion The preliminary MRI-based nomogram could be used to discriminate the Ki-67 index in well-differentiated G1/2 PanNETs.
Automated Quantification Of Ki-67 Index Associates With Pathologic Grade Of Pulmonary Neuroendocrine Tumors

Abstract:Background: Classification of the pulmonary neuroendocrine tumor (pNET) categories is a step-wise process identified by the presence of necrosis and number of mitoses per 2 mm(2). In neuroendocrine tumor pathology, Ki-67 was first described as a prognostic factor in the pancreas and incorporated into the grading system of digestive tract neuroendocrine neoplasms in the 2010 WHO classification. However, the significance of Ki-67 in pNETs was still a controversial issue. This study was to investigate the potentially diagnostic value of Ki-67 in pNETs.Methods: We retrieved 159 surgical specimens of pNETs, including 35 typical carcinoids (TCs), 2 atypical carcinoid (ACs), 28 large-cell neuroendocrine carcinomas (LCNECs), 94 small-cell lung cancers (SCLCs). Manual conventional method (MCM) and computer-assisted image analysis method (CIAM) were used to calculate the Ki-67 proliferative index. In CLAM, 6 equivalent fields (500 x 500 mu m) at 10 x magnification were manually annotated for digital image analysis.Results: The Ki-67 index among the 4 groups with ranges of 0.38% to 12.66% for TC, 4.34% to 29.48% for AC, 30.67% to 93.74% for LCNEC, and 40.71% to 96.87% for SCLC. The cutoff value of Ki-67 index to distinguish low grade with high grade was 30.07%. For the univariate survival analyses in pNETs, both the overall survival and progression-free survival correlated with Ki-67 index. In addition, the Ki-67 index performed by CIAM was proved to be of great positive correlation with MCM.Conclusions: Ki-67 index counted by CIAM is a reliable method and can be a useful adjunct to classify the low- and high-grade NETs.
Correlations of CT features,pathological findings and expression of Ki-67 in cystic-solid pancreatic neuroendocrine tumors

Abstract:Objective To study the correlations between CT,pathology features and expression of Ki-67 in cystic-solid pancreatic neuroendocrine tumors(CPNETs).Methods CT images of 19 cases with pathologically proved CPNETs were retrospectively analyzed.Ki-67 expression were determined by immunohistochemical method in all patients.The correlation between expression of Ki-67 and CT features in CPNETs were studied.Results Of the 1 9 cases,20 lesions were revealed including 18 cases with single lesion and one case with two lesions in the duodenum and pancreatic head.The diameters of the masses ranged from 1.8 cm to 10.0 cm.The lesions were divided into 3 groups according to the expressions of tumor Ki-67,inclduing well differentiated CPNETs (Ki-67<3%,6 cases);median differentiated CPNETs(Ki-67 was 3% ~20%,8 cases)and low differentiated CPNETs(Ki-67 > 20%,5 cases).The expressions of tumor Ki-67 were not correlated with patient’s age,sex and occurring place(P >0.05).There were positive correlation between Ki-67 and tumor diameter,dilatation of pancreatic duct,vessels invasion,lymph lode metastasis and distant metastasis(P<0.05).With the increased expression of Ki-67 tumor tends to be irregular lobulated shape,meanwhile the cystic wall tends to be more irregular and the wall nodule could be seen (P<0.01).Negative correlation were observed between Ki-67 and the cystic proportion of tumor(P<0.05).The expressions of tumor Ki-67 were not correlated with the blood supply(P >0.05).Conclusion Some CT features of CPNETs are associated with the expression of Ki-67,which would be helpful in staging and making prognosis for CPNETs.
Gene Expression Profiling of Pancreas Neuroendocrine Tumors with Different Ki67-Based Grades

Abstract:Pancreatic neuroendocrine tumors (PanNETs) display variable aggressive behavior. A major predictor of survival is tumor grade based on the Ki67 proliferation index. As information on transcriptomic profiles of PanNETs with different tumor grades is limited, we investigated 29 PanNETs (17 G1, 7 G2, 5 G3) for their expression profiles, mutations in 16 PanNET relevant genes and LINE-1 DNA methylation profiles. A total of 3050 genes were differentially expressed between tumors with different grades (p < 0.05): 1279 in G3 vs. G2; 2757 in G3 vs. G1; and 203 in G2 vs. G1. Mutational analysis showed 57 alterations in 11 genes, the most frequent being MEN1 (18/29), DAXX (7/29), ATRX (6/29) and MUTYH (5/29). The presence and type of mutations did not correlate with the specific expression profiles associated with different grades. LINE-1 showed significantly lower methylation in G2/G3 versus G1 tumors (p = 0.007). The expression profiles of matched primaries and metastasis (nodal, hepatic and colorectal wall) of three cases confirmed the role of Ki67 in defining specific expression profiles, which clustered according to tumor grades, independently from anatomic location or patient of origin. Such data call for future exploration of the role of Ki67 in tumor progression, given its involvement in chromosomal stability.

Abstract:Summary This study investigated the accuracy of MRI features in differentiating the pathological grades of pancreatic neuroendocrine neoplasms (PNENs). A total of 31 PNENs patients were retrospectively evaluated, including 19 cases in grade 1, 5 in grade 2, and 7 in grade 3. Plain and contrastenhanced MRI was performed on all patients. MRI features including tumor size, margin, signal intensity, enhancement patterns, degenerative changes, duct dilatation and metastasis were analyzed. Chi square tests, Fisher’s exact tests, one-way ANOVA and ROC analysis were conducted to assess the associations between MRI features and different tumor grades. It was found that patients with older age, tumors with higher TNM stage and without hormonal syndrome had higher grade of PNETs (all P <0.05). Tumor size, shape, margin and growth pattern, tumor pattern, pancreatic and bile duct dilatation and presence of lymphatic and distant metastasis as well as MR enhancement pattern and tumor-topancreas contrast during arterial phase were the key features differentiating tumors of all grades (all P <0.05). ROC analysis revealed that the tumor size with threshold of 2.8 cm, irregular shape, pancreatic duct dilatation and lymphadenopathy showed satisfactory sensitivity and specificity in distinguishing grade 3 from grade 1 and grade 2 tumors. Features of peripancreatic tissue or vascular invasion, and distant metastasis showed high specificity but relatively low sensitivity. In conclusion, larger size, poorlydefined margin, heterogeneous enhanced pattern during arterial phase, duct dilatation and the presence of metastases are common features of higher grade PNENs. Plain and contrast-enhanced MRI provides the ability to differentiate tumors with different pathological grades.
Pancreatic Neuroendocrine Tumor: Prediction of Tumor Grades by Radiomics Models Based on Ultrasound Images

Abstract:Objective We aimed to investigate whether the radiomics analysis based on B - mode ultrasound (BMUS) images could predict histopathological tumor grades in pancreatic neuroendocrine tumors (pNETs). Methods A total of 64 patients with surgery and histopathologically confirmed pNETs were retrospectively included (34 male and 30 female, mean age 52.4 +/- 12.2 years). Patients were divided into training cohort (n = 44) and validation cohort (n = 20). All pNETs were classified into Grade 1 (G1), Grade 2 (G2), and Grade 3 (G3) tumors based on the Ki- 67 proliferation index and the mitotic activity according to WHO 2017 criteria. Maximum relevance minimum redundancy, least absolute shrinkage and selection operator were used for feature selection. Receiver operating characteristic curve analysis was used to evaluate the model performance. Results Finally, 18 G1 pNETs, 35 G2 pNETs, and 11 G3 pNETs patients were included. The radiomic score derived from BMUS images to predict G2/G3 from G1 displayed a good performance with an area under the receiver operating characteristic curve of 0.844 in the training cohort, and 0.833 in the testing cohort. The radiomic score achieved an accuracy of 81.8% in the training cohort and 80.0% in the testing cohort, a sensitivity of 0.750 and 0.786, a specificity of 0.833 and 0.833 in the training/testing cohorts. Clinical benefit of the score also exhibited superior usefulness of the radiomic score, as shown by the decision curve analysis. Conclusions Radiomic data constructed from BMUS images have the potential for predicting histopathological tumor grades in patients with pNETs. Advances in knowledge The radiomic model constructed from BMUS images has the potential for predicting histopathological tumor grades and Ki- 67 proliferation indexes in patients with pNETs.

Abstract:In 1983, a monoclonal antibody, Ki67, was generated, that labeled the nuclei of proliferating non-neoplastic and neoplastic cells. The name Ki67 derived from the city of Kiel (Ki) where the antibody was produced in the university department of pathology and refers to the number of the original clone (67). Systematic assessment of the proliferative activity of tumors using Ki67 started in the 1990s, when Ki67, which only worked on frozen tissue, was complemented by the antibody MIB-1 that also worked in formalin-fixed tissues. Pancreatic neuroendocrine neoplasms (PanNENs) were the first endocrine tumors whose proliferative activity was assessed with Ki67. This approach was so successful that Ki67 was included as prognostic marker in the 2000 and 2004 WHO classifications of gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs). In 2010, the WHO classification of GEP-NENs introduced a three-tiered grading, originally proposed by ENETS in 2006 that was mainly based on the Ki67 index. As it has subsequently been shown that the Ki67 index is the most reliable factor in the prognostic evaluation of GEP-NENs, especially of PanNENs, the 2017 WHO classification of PanNENs requires its use and strongly recommends exact assessment of the proportion Ki67-labeled cells as basis for the calculation of the Ki67 index. Problems in assessing the Ki67 index include intertumoral and intratumoral staining heterogeneity and counting methods. Despite such problems, the Ki67 index has emerged as indispensable for the prognostic and therapeutic stratification of the majority of GEP-NENs and can barely be replaced by counting mitoses. In future, however, it can be anticipated that the Ki67 cut-offs experience refinement in relation to the type of tumor, its location, and its response to therapy. It is also possible that the prognostic risk of an individual tumor is calculated for each Ki67 unit and not for an “a priori” fixed Ki67 class.

Abstract:Pulmonary neuroendocrine tumors (PNETs) can be classified as four variants: typical carcinoid (TC), atypical carcinoid (AC), small cell lung cancer (SCLC) and large cell neuroendocrine carcinoma (LCNEC). Classification of the four tumor categories is a step-wise process identified by the presence of necrosis and number of mitoses per 2mm2. In NE tumor pathology, Ki-67 was first described as prognostic factor in the pancreas and was incorporated into the grading system of digestive tract NE neoplasms in the 2010 World Health Organization (WHO) classification. However, the significance of Ki-67 has not been fully studied in PNETs. The purpose of our research is to investigate the potentially diagnostic value of Ki-67. We retrieved 160 surgical specimens of pulmonary neuroendocrine tumors (37 TC, 2 AC, 30 LCNEC, 91 SCLC) based on mitotic rate and histologic features. A nuclear quantitation algorithm was used to calculate the Ki-67 proliferative index (PI). Six equivalent fields (500 × 500μｍ) at 10× magnification were manually annotated for digital image analysis. The mitotic activity was manually quantitated in the most cellular areas on whole-slide images, eight 40× high power fields were calibrated as 2 mm2. The median mitotic count and Ki-67 index for TC, AC, LCNEC, and SCLC were 1 and 3.96%, 6 and 28.91%, 58 and 57.62%, 118 and 79.3%, respectively. However, the Ki-67 index overlapped among the four groups, with ranges of 0.38-13.49% for TC, 16.34-41.48% for AC, 30.67-93.74% for LCNEC, and 12.43-96.87% for SCLC. The Ki-67 index was positively associated with mitotic number (P<0.001). Neuroendocrine carcinomas can be separated into two groups, high grade (Ki-67＞10％) and low grade (Ki-67≤10％). However, there is no adequate threshold to separate pulmonary AC from TC and no statistic difference of Ki-67 index between LCNEC and SCLC. For the univariate survival analyses in PNETs, both the Overall Survival (OS) and Progression Free Survival (PFS) correlated with Ki-67 index, mitotic number and nodal metastasis. But none of them was proved to be an independent prognostic factor. In high grade PNETs, multivariate analysis revealed that tumor size has a trend of being an independent prognostic factor, but did not reach significant level (P=0.085). Our results indicate that software-automated quantitation of Ki-67 index is a reliable method to predict the prognosis of PNETs. In addition, Ki-67 index can be a useful adjunct to pathologic grade in pulmonary neuroendocrine carcinoma.

Abstract:Purpose: The purpose of this study was to evaluate the performance of magnetic resonance imaging (MRI) findings and texture parameters for prediction of the histopathologic grade of pancreatic neuroendocrine tumors (PNETs) with 3-T magnetic resonance.Patients and methods: PNETs are classified into Grade 1 (G1), Grade 2 (G2), and Grade 3 (G3) tumors based on the Ki-67 proliferation index and the mitotic activity. A total of 77 patients with pathologically confirmed PNETs met the inclusion criteria. Texture analysis (TA) was applied to T2-weighted imaging (T2WI) and diffusion-weighted imaging (DWI) maps. Patient demographics, MRI findings, and texture parameters were compared among three different histopathologic subtypes by using Fisher's exact tests or Kruskal-Wallis test. Then, logistic regression analysis was adopted to predict tumor grades. ROC curves and AUCs were calculated to assess the diagnostic performance of MRI findings and texture parameters in prediction of tumor grades.Results: There were 31 G1, 29 G2, and 17 G3 patients. Compared with G1, G2/G3 tumors showed higher frequencies of an ill-defined margin, a predominantly solid tumor type, local invasion or metastases, hypo-enhancement at the arterial phase, and restriction diffusion. Four T2-based (inverse difference moment, energy, correlation, and differenceEntropy) and five DWI-based (correlation, contrast, inverse difference moment, maxintensity, and entropy) TA parameters exhibited statistical significance among PNETs (P<0.001). The AUCs of six predicting models on T2WI and DWI ranged from 0.703-0.989.Conclusion: Our data indicate that MRI findings, including tumor margin, texture, local invasion or metastases, tumor enhancement, and diffusion restriction, as well as texture parameters can aid the prediction of PNETs grading.
Comparison of Tumor Markers for Predicting Disease-Free Survival in Surgically Resected Pancreatic Neuroendocrine Tumors

Abstract:BACKGROUND:Identification of biomarkers of pancreatic neuroendocrine tumors (PNETs) is important for stratification of the prognosis. Ki-67 index was the significant prognostic factor for PNETs. Recently, MMP-9, DJ-1, and α-1-B glycoprotein (A1BG) were shown to be the prognostic markers in some malignant tumors except for PNETs. The aim of this study was to compare these tumor markers for predicting disease-free survival in surgically resected PNETs. METHODS:A retrospective review of patients pathologically diagnosed with PNETs at our institution from January 2012 to January 2014 was conducted. Tumor specimens were stained by immunochemistry for MMP-9, DJ-1, A1BG, and Ki-67. Clinicopathologic findings and these 4 tumor markers of patients with PNETs were analyzed. Prognostic factors were determined by univariate and multivariate analyses. RESULTS:A total of 40 patients were selected. Of 40 tumors, 19 (47.5%) were positive for MMP-9, 21 (52.5%) for DJ-1, 18 (45%) for A1BG, and 24 (60%) for Ki-67 > 2%. Four (10%) tumors expressed none of the markers. MMP-9 and DJ-1 expression were more frequently observed in distant metastasis, higher WHO grade, and lymph node metastasis. Similarly, those with Ki-67 > 2% had larger tumor, higher WHO grade, lymph node invasion and larger surgical procedures. A1BG had no substantial effect on distant metastasis. Univariate analyses revealed that grade 2/3 histology, distant metastasis, lymph nodes metastasis, high expression of MMP-9, high expression of DJ-1, and Ki-67 > 2% were predictive of inferior disease-free survival for PNETs (all p < 0.05). In multivariable analysis, the significant poor factors associated with disease-free survival were grade 2/3 histology, distant metastasis, lymph node metastasis, and Ki-67 > 2% (all p < 0.05). CONCLUSIONS:Of the 4 markers studied, only Ki-67 was the independent prognostic factor. Expression of MMP-9 and DJ-1 correlated with invasion, metastasis, disease progression, and poor prognosis in PNETs, while A1BG had no prognostic value in PNETs. It is anticipated that these findings can serve as useful clinical survival predictors, especially in the setting of resected disease.

Abstract:Objective To determine the clinical value of contrast enhanced MRI in differentiating benign and malignant pancreatic endocrine tumors (PNETs).Methods MRI findings on 13 cases who were pathologically diagnosed as PNETs were retrospectively analyzed , including the tumor volume , border , MRI signal and adjacent organ involvement of Grade 1~3 tumors, and the sensitivity, specificity and accuracy of MRI for diagnosing PNETs were calculated .Results A total of 18 tumors were detected , including 12 benign (Grade 1) and 6 malignant (Grade 2~3) tumors.Eight Grade 1 tumors′(66.7%) maximal diameters were≤2 cm, while 100%(6 /6) Grade 2 ~3 tumors′maximal diameters were ≥2 cm.All of Grade 1 tumors (100%) had clear boundary , while 50%(2/4) Grade 2 and 100%(2/2) Grade 3 tumors had unclear boundary.Ten Grade 1 tumors (83.3%) had intratumoral homogeneous signal , while 100%(6/6) Grade 2~3 tumors had heterogeneous intratumoral signal .Two Grade 3tumors ( 100%) had organ infiltration and retroperitoneal lymph node metastasis .The sensitivity ,specificity and accuracy of MRI for differentiating benign and malignant PNETs were 833.%, 85.7%and 84.6%, respectively.Conclusi ons MRI had higher sensitivity, specificity and accuracy in differentiating benign and malignant PNETs , but it was still difficult to differentiate Grade 1 from Grade 2 tumors by MRI.Tumors size could not be considered to be a reliable indicator for differentiating benign and malignant PNETs .
Value of Diffusion-Weighted Magnetic Resonance Imaging in Predicting World Health Organization Grade in G1/G2 Pancreatic Neuroendocrine Tumors

Abstract:The present study aimed to investigate the value of diffusion-weighted magnetic resonance imaging (DWI) in the grading of well-differentiated pancreatic neuroendocrine tumors (PanNETs). A total of 44 patients with histologically proven well-differentiated PanNET [grade 1 (G1) and grade 2 (G2) according to the World Health Organization (WHO) criteria] underwent pretreatment magnetic resonance imaging (MRI), which was retrospectively analyzed. The location, size, cystic or solid appearance, boundary, presence or absence of tumor contrast enhancement, and MRI signal of the tumor were assessed. Apparent diffusion coefficients (ADCs) within the primary tumor were measured on ADC maps. Receiver operating characteristic curves were used to determine ADC cut-off values and the sensitivity and specificity of the grade prediction. Spearman's rank correlation was utilized to probe the association between ADC value and PanNET grade or pathological parameters. G1 tumors exhibited a well-circumscribed border more often than G2 tumors. No marked differences were observed between PanNET G1 and PanNET G2 for cystic or solid appearance, enhancement, and T1- and T2-weighted imaging signals. Marked hyperintensities were more common in PanNET G2 tumors than in PanNET G1 tumors (P<0.01). The mean ADC values were statistically different between the normal pancreas and G1 and G2 tumors (P<0.01), and between PanNET G1 and PanNET G2 (P<0.05). Correlation analysis showed that ADC value was negatively correlated with PanNET grade, mitotic count and Ki-67 proliferation index (P<0.05). The cut-off ADC value was 0.930×10-3 mm2/sec, which identified G2 tumors with 82.4% sensitivity and 79.5% selectivity. ADC value therefore represents a non-invasive and valuable imaging parameter in predicting the WHO grade of tumors in well-differentiated PanNET.
