Abstract:BACKGROUND: Dobutamine stress echocardiography (DSE) is an established method of detecting myocardial ischemia. Its diagnostic accuracy solely depends on wall motion assessment. Clear visibility of the left ventricular endocardium is essential for reliable assessment of a wall motion abnormality. However, incremental benefits of contrast DSE for the detection of coronary artery disease (CAD) have not been demonstrated in overweight or obese patients.OBJECTIVES: The purpose of the present study was to test the incremental benefits of contrast DSE in detecting CAD in overweight or obese patients.METHODS: Sixty-two overweight or obese patients (body mass index 26 kg/m(2) to 33 kg/m(2)) underwent DSE with or without contrast and coronary angiography. Contrast-enhanced images were achieved at rest and during peak DSE after administration of SonoVue (Bracco Diagnostics Inc, Italy) or Optison (Mallinckrodt, USA). The endo, cardial border resolution for each myocardial segment was graded as 0, 1 or 2. A total of 992 segments from 62 subjects were analyzed. The results of DSE with or without contrast were compared with the findings on angiography.RESULTS;. The differences in the score grading between the two groups with or without contrast, at rest and during peak DSE were statistically significant (P < 0.001). The sensitivity, specificity and accuracy. of contrast DSE in detecting CAD, compared with the studies without contrast, were improved (82% versus 70%, 78% versus 67% and 8 1 % versus 69%, respectively).CONCLUSIONS: SonoVue and Optison can enhance left ventricular endocardial border delineation in overweight or obese patients, optimizing, the evaluation of wait motion both at rest and during peak stress. This increases the diagnostic value of DSE in detecting CAD.

Abstract:ObjectiveRegional diastolic function in CAD patients during DSE by DTI.Method14 healthy persons and 20 patients were performed standarded protocol of DSE. From DTI spectrum,peak early diastolic velocity (Ve) ,peak late diastolic velocity (Va) and the ratio of peak early diastolic velocity to peak late diastolic velocity(Ve/Va)were measured in each segment at baseline,10 μg/(kg·min~(-1)) and 40μg/(kg·min~(-1)) during dobutamine(Dob) infusion.ResultsDuring dobutamine infusion, Ve in CON group increased accompanied with the dose of Dob, Ve in CAD group at 10 μg/(kg·min~(-1)),but decreased at 40 μg/(kg·min~(-1)) compared with 10 μg/kg·min,but there were significant difference between the two group(P0.001).ConclusionDTI-DSE can quantificatively detect the changes of cardiac diastolic function during DSE noninvasively.
Value of Low Dose Dobutamine Real-Time Myocardial Contrast Echocardiography in the Assessment of Coronary Artery Disease

Abstract:OBJECTIVE:To evaluate the value of quantitative real-time myocardial contrast echocardiography (RT-MCE) combined with low dose dobutamine stress test in the detection of coronary artery disease (CAD), and to assess the contribution of collateral blood flow (CBF) to myocardial perfusion.METHODS:Twenty-six hospitalized patients referred for coronary angiography and subsequent revascularization underwent routine echocardiography, RT-MCE at baseline and after low dose dobutamine administration. The images of RT-MCE were analyzed quantitatively from microbubble replenishment curves for myocardial perfusion and its reserve by using the QLab software.RESULTS:At baseline, both beta and A x beta (but not A) were decreased with the increase of severity of coronary stenosis (P < 0.01). Under dobutamine stress, A, beta and A x beta values were decreased with the increase of severity of coronary stenosis (P < 0.01), Graded decreasing in the reserves of A, beta and A x beta were observed with increasing coronary stenosis severity (P < 0.01). Furthermore, significant differences in beta, A x beta, and WMS were observed between segments with CBF and those without.CONCLUSION:Quantitative RT-MCE in conjunction with dobutamine stress can be used as a sensitive measure to identify and stratify CAD as well as to assess the contribution of collateral blood flow.

Quantitative evaluation of myocardial perfusion in coronary heart disease by myocardial contrast and dobutamine stress echocardiography

Abstract:Objective: We aimed to investigate how combining myocardial contrast echocardiography (MCE) with dobutamine stress echocardiography (DSE) could help evaluate myocardial perfusion in coronary heart disease patients and understand changes in microcirculation across different levels of coronary artery blockage. Methods: We conducted MCE and DSE tests on 53 coronary heart disease patients, categorizing ischemic myocardium into four groups: A (<50%), B (50%-69%), C (70%-89%), and D (≥90%). Dynamic myocardial perfusion images were captured during rest and peak dobutamine stress from various angles, analyzing parameters like plateau value A, slope β, and the product A × β, reflecting different aspects of myocardial blood flow. Results: Parametric values of myocardial perfusion (PVMPs) were significantly lower in group D at rest compared with other groups (p < 0.001). PVMPs increased notably at peak dobutamine stress in groups A, B, and C (p < 0.001). Groups A and B had higher PVMPs than groups C and D, with group D significantly lower (p < 0.001). β reserve values decreased gradually from group A to D, with significantly lower values of A and A × β in groups C and D compared with A and B (p < 0.001). Sensitivity and specificity for predicting >70% coronary artery blockage was 80%, 66%, and 74%, 80%, respectively, using specific thresholds. Conclusion: Combining MCE with DSE is highly sensitive and accurate in diagnosing obstructive coronary artery blockages. It also helps assess myocardial microcirculation perfusion and left ventricular reserve function, which decline with increasing severity of coronary artery blockage.
Nursing Care and Cooperation in Diagnosing Coronary Arterydisease with Dobutamine Myocardial Perfusion Imaging

Abstract:Objective To explore the role of nursingcare and cooperation in patients recieving dobutamine myocardial perfusion imaging (DMPI). Methods DMPI was performed in 32 patients with coronary artery disease (CAD), using an incremental protocol 5～40 μg·kg -1 ·min -1 in3 min stages. Nursing care and cooperation were done in pre stress, duration of DMPI and post stress, respectively. Results Of the 32 patients, 23 had positive results in DMPI. The sensitivity was 71.9%. With the active nursingcare and cooperation of nurses, the patients with CAD had only a few mild symptoms, such as palpitation(34 1%), chest pain(28 1%), swirl(25 0%), nausea(18 8%), headache(9%) and so on in dobutamine stress, but no severe complications, for example, ventricular tachycardia, myocardial infarction, generalized tetany and soon. Conclusion The technical cooperation and nursing care play an essential role in the successful completion of dobutamine stress test and for avoiding severe complications.

Abstract:Myocardial perfusion imaging (MPI) is an established diagnostic test that provides useful prognostic data in patients with known or suspected coronary artery disease. In more than half of the patients referred for stress testing, vasodilator stress is used in lieu of exercise. Unlike exercise, vasodilator stress does not provide information on exercise and functional capacity, heart rate recovery, and chronotropy, and ECG changes are less frequent. These non-perfusion data provide important prognostic and patient management information. Further, event rates in patients undergoing vasodilator MPI are higher than in those undergoing exercise MPI and even in those with normal images probably due to higher pretest risk. However, there are a number of non-perfusion variables that are obtained during vasodilator stress testing, which have prognostic relevance but their use has not been well emphasized. The purpose of this review is to summarize the prognostic values of these non-perfusion data obtained during vasodilator MPI.

Abstract:Objectives: The purpose of this study was to determine the prognostic value of strain-encoded magnetic resonance imaging (SENC) during high-dose dobutamine stress cardiac magnetic resonance imaging (DS-MRI) compared with conventional wall motion readings. Background: Detection of inducible ischemia by DS-MRI on the basis of assessing cine images is subjective and depends on the experience of the readers, which may influence not only the diagnostic classification but also the risk stratification of patients with ischemic heart disease. Methods: In all, 320 consecutive patients with suspected or known coronary artery disease underwent DS-MRI, using a standard protocol in a 1.5T MR scanner. Wall motion abnormalities (WMA) and myocardial strain were assessed at baseline and during stress, and outcome data including cardiac deaths, nonfatal myocardial infarctions ("hard events"), and revascularization procedures performed >90 days after the MR scans were collected. Results: Thirty-five hard events occurred during a 28 ± 9 month follow-up period, including 10 cardiac deaths and 25 nonfatal myocardial infarctions, and 32 patients underwent coronary revascularization. Using a series of Cox proportional-hazards models, both resting and inducible WMA offered incremental information for the assessment of hard cardiac events compared to clinical variables (chi-square = 13.0 for clinical vs. chi-square = 26.1 by adding resting WMA, p < 0.001, vs. chi-square = 39.3 by adding inducible WMA, p < 0.001). Adding visual SENC or quantitative strain rate reserve to this model further improved the prediction of outcome (chi-square = 50.7 vs. chi-square = 52.5, p < 0.001 for both). In a subset of patients (n = 175) who underwent coronary angiography, SENC yielded significantly higher sensitivity for coronary artery disease detection (96% vs. 84%, p < 0.02), whereas specificity and accuracy were not significantly different (88% vs. 94% and 93% vs. 88%, p = NS for both). Conclusions: Strain-encoded MRI aids the accurate identification of patients at high risk for future cardiac events and revascularization procedures, beyond the assessment of conventional atherogenic risk factors and resting or inducible WMA on cine images. Abstract:Objective:To evaluate the comparative value of dipyridamole 201Tl single photon emission computer tomgraphy(SPECT) cardiac perfusion imaging and dobutamine 99mTc-MIBI SPECT imaging in cardiac syndrome X(CSX).Methods:A cohort of seventy-four patients with known CSX who had undergone pharmacological stress myocardial perfusion imaging in our hospital from March 2005 to April 2012 were reviewed.The CSX patients were assigned to group A and group B according to different pharmacological stress agents.Group A underwent dipyridamole 201Tl SPECT cardiac imaging and group B underwent dobutamine 99mTc-MIBI SPECT imaging.Images were interpreted by more than two experienced nuclear medicine physicians.Comparisons of the categorical variables between the two groups were performed using χ2 test for positive rates.Results:In group A,reverse redistribution phenomena were observed in 45 cases,while the remaining 8 had normal cardiac imaging.In group B,only 3 cases had reversible defect.The positive rates were 84.91%(in group A,45/53)and 14.29%(in group B,3/21),respectively.There was statistically significant difference in positive rate between the two groups(χ2=29.89,P0.001).Conclusion:Reverse redistribution in dipyridamole 201Tl SPECT cardiac imaging may be of some value for the assessment of patients with CSX.

Abstract:BACKGROUND:In patients with acute anterior myocardial infarction (MI), sometimes an "apical ballooning" contractile dysfunction pattern that exceeds factual myocardial injury is identified in the ventriculography and bedside echocardiography. The hemodynamic consequences/sequela of this "Tako-tsobu effect" has not been well delineated. Of note, this anatomic imaging finding often misleads frontline physicians who assume reciprocal causation of persistent cardiac pump failure and ventricular pressure overload. METHODS AND RESULTS:Using real-time myocardial perfusion contrast echocardiography (MCE), we investigated myocardial (microvascular) perfusion in 60 patients after acute MI and coronary revascularization. Twenty-eight percent of the studied patients showed significantly mismatched myocardial perfusion and contractile defects. In these patients, an integrated imaging assessment with coronary angiography/ventriculography, deformation echocardiography, and MCE proved that the myocardial mechanic abnormalities significantly exceeded the defected perfusion areas. Compared with 72% of the patients without perfusion-contractility mismatch, apparently worse systolic functions (left ventricular ejection, wall motion score, and systolic longitudinal strain) in these patients did not change diastolic ventricular filling pressures (E/E' and E/A) or hemodynamic consequences/adverse events. Both systolic and diastolic functions in patients with perfusion-contractility mismatch appeared to be comparable with those in patients with Tako-tsubo syndrome. CONCLUSIONS:Real-time MCE identifies discrepant myocardial microvascular perfusion and mechanics in patients with acute MI. The "Tako-tsubo effect" in patients with perfusion-contractility mismatch does not cause diastolic filling pressure change or worse hemodynamic consequence/cardiac event.

Abstract:OBJECTIVE:This study was sought to compare the sensitivity, specificity and accuracy of (1) dual isotope simultaneous acquisition single-photon emission computed tomography (DISA SPECT) myocardial image with (99m)Tc-sestamibi/(18)F-fluorodeoxyglucose ((99m)Tc-MIBI/(18)FDG); (2) low dose dobutamine alone and combined with Isosorbide Dinitrate (ISDN: Isoket) stress two dimensional echocardiography (2DE) to predict regional movement recovery after revascularization (CRV) in patients with old myocardial infarction (OMI) and severe left ventricular dysfunction. METHODS:Twenty-six patients (mean age 51 +/- 8 years, male 25, female 1) with OMI and severe left ventricular dysfunction (mean left ventricular ejection fraction, LVEF (38.6% +/- 4.9%) underwent low dose dobutamine 10 microg x kg(-1) x min(-1) (Dob10 microg) and ISDN (286 +/- 31 microg/min) combined with Dob5 microg (ISDN-Dob 5 microg) 2DE and DISA SPECT within one week. In echocardiogram and DISA SPECT images: the left ventricle (LV) was divided into 16 segments. The semi-quantitative scoring system was used for both images. Myocardial viability was defined as an improvement of at least >or= 1 grade in at least two contiguous segments at rest 2DE after CRV. The viable segments detecting rate with stress 2DE and DISA SPECT were compared. Compared with the results of post-CRV, the sensitivity, specificity and accuracy of detecting viable segments of two methods were calculated. RESULTS:Among 272 abnormal segments in 26 patients, 156 (57.4%) segments showed contractile improvement after CRV. The viable segments detecting rate with DISA SPECT was 72.4% (134/254), which was significantly higher than the contractile improved rate after CRV (P < 0.001). During Dob10 microg 2DE and ISDN-Dob5 microg 2DE, the detecting rates were 65.5% (163/249) and 65.7% (176/268), respectively, which were both comparable to the improved rate after CRV (both P > 0.05). With DISA SPECT, the sensitivity, specificity and accuracy were 93.7%, 55% and 76.8%, respectively. Compared with DISA SPECT, Dob10 microg 2DE showed similar sensitivity (88.6%), specificity (64.2%) and the accuracy (77.9%). When ISDN combined with Dob5 microg, the sensitivity (91.4%), specificity (68.1%) and accuracy (81.4%)were comparable to those of Dob10 microg 2DE and DISA SPECT (all P > 0.05), while the specificity was even higher than DISA SPECT (P < 0.05). CONCLUSION:In identifying myocardial viability in patients with OMI and severe left ventricular dysfunction, DISA SPECT has higher sensitivity, lower specificity and better accuracy. Dob10 microg and ISDN-Dob5 microg 2DE are both equivalent to DISA SPECT in sensitivities, specificities and accuracies, and even higher in specificity in ISDN-Dob5 microg 2DE.
Advances in Myocardial Perfusion MR Imaging: Physiological Implications, the Importance of Quantitative Analysis, and Impact on Patient Care in Coronary Artery Disease

Abstract:Stress myocardial perfusion imaging (MPI) is the preferred test in patients with intermediate-to-high clinical likelihood of coronary artery disease (CAD) and can be used as a gatekeeper to avoid unnecessary revascularization. Cardiac magnetic resonance (CMR) has a number of favorable characteristics, including: (1) high spatial resolution that can delineate subendocardial ischemia; (2) comprehensive assessment of morphology, global and regional cardiac functions, tissue characterization, and coronary artery stenosis; and (3) no radiation exposure to patients. According to meta-analysis studies, the diagnostic accuracy of perfusion CMR is comparable to positron emission tomography (PET) and perfusion CT, and is better than single-photon emission CT (SPECT) when fractional flow reserve (FFR) is used as a reference standard. In addition, stress CMR has an excellent prognostic value. One meta-analysis study demonstrated the annual event rate of cardiovascular death or non-fatal myocardial infarction was 4.9% and 0.8%, respectively, in patients with positive and negative stress CMR. Quantitative assessment of perfusion CMR not only allows the objective evaluation of regional ischemia but also provides insights into the pathophysiology of microvascular disease and diffuse subclinical atherosclerosis. For accurate quantification of myocardial perfusion, saturation correction of arterial input function is important. There are two major approaches for saturation correction, one is a dual-bolus method and the other is a dual-sequence method. Absolute quantitative mapping with myocardial perfusion CMR has good accuracy in detecting coronary microvascular dysfunction. Flow measurement in the coronary sinus (CS) with phase contrast cine CMR is an alternative approach to quantify global coronary flow reserve (CFR). The measurement of global CFR by quantitative analysis of perfusion CMR or flow measurement in the CS permits assessment of microvascular disease and diffuse subclinical atherosclerosis, which may provide improved prediction of future event risk in patients with suspected or known CAD. Multi-institutional studies to validate the diagnostic and prognostic values of quantitative perfusion CMR approaches are required.

Abstract:BACKGROUND:Magnetic resonance imaging (MRI) is a versatile technique for examination of the cardiovascular system but only recently has assessment of myocardial ischaemia in coronary artery disease (CAD) become possible, for example by demonstrating abnormalities of regional ventricular contraction during stress. Global ventricular function during stress was assessed by MRI of aortic flow, which has not been previously attempted.DESIGN:Variables measured by MRI reflecting the effect of ischaemia on global ventricular function during dobutamine stress were correlated with thallium-201 myocardial perfusion tomography.PATIENTS:10 normal controls and 25 patients with CAD.SETTING:Tertiary cardiac referral centre.METHODS:Novel MRI sequences and analysis systems were used to measure the following variables during staged dobutamine infusion to 20 micrograms/kg/min: stroke volume, cardiac output, cardiac power output, peak flow, peak flow acceleration, aortic back flow, and flow wave velocity. Heart rate, blood pressure, double product, and maximum tolerated dobutamine dose were also measured. Multiple regression analysis was used to compare changes during stress with 201TI tomography.RESULTS:All parameters except for stroke volume and diastolic blood pressure increased in the controls. In the patients with CAD a significant relation was shown between the extent of reversible ischaemia and the change in peak flow acceleration (P < 0.00001), peak flow (P = 0.002), cardiac power output (P = 0.036), maximum dobutamine dose (P = 0.039), and systolic blood pressure (P = 0.04). Peak flow acceleration accounted for 58.4% of the variation in reversible ischaemia, and after allowing for this, only cardiac power output remained independently predictive adding a further 4.2% to the model (adjusted r2 = 0.626). A decrease in peak flow acceleration with an increase in dobutamine infusion indicated moderate or severe ischaemia (chi 2 = 10.2, P = 0.017).CONCLUSION:MRI may be used to assess variables of aortic flow during stress, which includes acceleration with high temporal resolution. Peak flow acceleration was the most sensitive indicator of the effect of ischaemia on global ventricular function.

Abstract:Determination of the severity of coronary artery disease (CAD) by single photon emission computed tomography (SPECT) imaging has previously been shown to have greater sensitivity, specificity, and accuracy when performed with pharmacologic stress using dobutamine than by standard dose dipyridamole (SDD) or exercise stress testing (EST) prior to SPECT imaging. The use of lung to heart (L:H) ratios has been shown to be valuable in determining the presence or absence of left main (LM) or triple vessel (3V) CAD. No such work has been previously reported for dobutamine. Twenty-one patients were studied using dobutamine (n = 7) or EST (n = 14). These results were compared with results from Part II of this series of studies using high-dose dipyridamole (HDD) pharmacologic stress. In this study, patients underwent L:H ratio analysis following injection of 3 mCi of Tl-201, this provides sufficient time for thallium clearance from the blood pool. Results of the L:H ratios were compared with the results of coronary arteriographic (CA) evaluation. Patients who were “stressed” via EST demonstrated statistically greater (p ≤ 0.001) L:H ratios in patients with LM/3V CAD when compared with patients who had 0–2 significantly stenosed coronary arteries. Patients stressed with dobutamine demonstrated lower L:H ratios (p=NS) in patients with LM/3V CAD than was seen for patients with 0–2 V CAD. Patients stressed with dobutamine had statistically (p ≤ 0.05) lower L:H ratios than did similar patients stressed with EST. Increased L:H ratios following EST and HDD, as shown previously in Part II of this series, provide excellent markers for LM/3V CAD following Tl-201 injection. The presence of “normal” L:H ratios in patients with LM/3V CAD following dobutamine stress may suggest the presence of “stunned” or “hibernating” myocardium. The presence of “decreased” L:H ratios following dobutamine after HDD or EST has already shown an increased L:H ratio, might suggest a marker for myocardial viability that deserves further investigation.

Abstract:decrease contrast enhancement at rest without changes after ADN was considered a fixed perfusion defect (FD).Results: Akinesis was observed in 278 segments.NP, RD and FD were found in 18 (6.5%),54 (19.5%) and 206 (74%) territories respectively.All 18 akinetic segments and N P were observed in pts submitted to a successful revascularization procedure.37/54 (68.5%) akinetic segments and RD, and 28/206 (13.5%) akinetic segments were also related to successful revascularization procedures.Conclusion: ACE is a feasible method to study myocardial perfusion in humans and the additional information about the different types of perfusion defects could be useful in the management of myocardial viability.
Relation Between Diastolic Perfusion Time and Coronary Artery Stenosis During Stress-Induced Myocardial Ischemia

Abstract:Background Experimental studies have demonstrated that during stress-induced myocardial ischemia, coronary obstruction and diastolic perfusion time are factors that limit subendocardial perfusion and correlate to degree of myocardial dysfunction. The relation between these two factors has not yet been investigated in humans. The aim of the present study was to assess the relation between diastolic perfusion time and degree of coronary stenosis during different types of stress tests. Methods and Results Nine patients with isolated and proximal stenosis of the left anterior descending coronary artery were selected. Patients underwent three different randomized stress tests (upright, supine bicycle stress test, and transesophageal atrial pacing). Diastolic perfusion time, heart rate (RR interval), and systolic and diastolic pressures were measured during the test and at the ischemic threshold (0.1-mV ST-segment depression). Angiographic measurements of coronary stenosis were evaluated by quantitative coronary angiography. At the ischemic threshold, significant differences among tests were found in heart rate ( P <.05), systolic pressure ( P <.001), and diastolic pressure ( P <.05). In each stress test, diastolic perfusion time at the ischemic threshold was closely correlated with minimal stenosis diameter ( r =.97; P <.001) and percent diameter stenosis ( r =.92; P <.001) with no difference among the tests. In contrast, heart rate, rate-pressure product, and time to ischemic threshold were not significantly correlated with percent diameter stenosis and minimal stenosis diameter. No significant correlation was observed at the ischemic threshold between diastolic perfusion time and corresponding values of heart rate, despite the close correlation at rest ( r =.95; P <.001). Conclusions Despite differences in associated hemodynamic responses to various stress tests, a close relation exists between stenosis severity and diastolic perfusion time at the onset of stress-induced myocardial ischemia. Therefore, diastolic perfusion time at the ischemic threshold may be an indirect estimate of the hemodynamic significance of coronary stenosis.

Abstract:Mental stress can trigger myocardial ischemia, but the prevalence of mental stress-induced ischemia in congestive heart failure (CHF) patients is unknown. We characterized mental stress-induced and adenosine-induced changes in myocardial perfusion and neurohormonal activation in CHF patients with reduced left-ventricular function using SPECT to precisely quantify segment-level myocardial perfusion.METHODS: Thirty-four coronary artery disease patients (mean age±SD, 62±10 y) with CHF longer than 3 mo and ejection fraction less than 40% underwent both adenosine and mental stress myocardial perfusion SPECT on consecutive days. Mental stress consisted of anger recall (anger-provoking speech) followed by subtraction of serial sevens. The presence and extent of myocardial ischemia was quantified using the conventional 17-segment model.RESULTS: Sixty-eight percent of patients had 1 ischemic segment or more during mental stress and 81% during adenosine. On segment-by-segment analysis, perfusion with mental stress and adenosine were highly correlated. No significant differences were found between any 2 time points for B-type natriuretic peptide, tumor necrosis factor-α, IL-1b, troponin, vascular endothelin growth factor, IL-17a, matrix metallopeptidase-9, or C-reactive protein. However, endothelin-1 and IL-6 increased, and IL-10 decreased, between the stressor and 30 min after stress. Left-ventricular end diastolic dimension was 179±65 mL at rest and increased to 217±71 after mental stress and 229±86 after adenosine (P<0.01 for both). Resting end systolic volume was 129±60 mL at rest and increased to 158±66 after mental stress (P<0.05) and 171±87 after adenosine (P<0.07), with no significant differences between adenosine and mental stress. Ejection fraction was 30±12 at baseline, 29±11 with mental stress, and 28±10 with adenosine (P=not significant).CONCLUSION: There was high concordance between ischemic perfusion defects induced by adenosine and mental stress, suggesting that mental stress is equivalent to pharmacologic stress in eliciting clinically significant myocardial perfusion defects in CHF patients. Cardiac dilatation suggests clinically important changes with both conditions. Psychosocial stressors during daily life may contribute to the ischemic burden of CHF patients with coronary artery disease.

Abstract:Objective:To evaluate exercise-induced post-ischemic myocardial stunning by gated 99Tcm-MIBI SPECT perfusion imaging. Methods:A total of 108 patients with suspected coronary artery disease(CAD) and 30 patients with low likelihood CAD were examined by gated 99Tcm-MIBI SPECT perfusion imaging.Three data acquisitions were performed as early stage(5~15 min after exercise),delayed stage(about 1.5 h after exercise) and resting stage(next day of exercise).Left ventricular ejection fraction(EF) and volumes were calculated by QGS software,and the segmental perfusion was analyzed using a 5-point scale.According to myocardial perfusion imaging,108 suspected CAD patients were divided into Myocardial ischemia group,n=70 and Normal perfusion group,n=38,together with Low likelihood CAD group,we compared left ventricular EF,the end-diastolic volume(EDV) and end-systolic volume(ESV)at three stages in three groups. Results:In Myocardial ischemia group,post-exercise left ventricular EF at early stage and delayed stage were lower than that in resting stage,while the post-exercise ESV was higher than that in resting stage.The stress/rest ratios were 1.28±0.43 at early stage,and 1.25±0.31 at delayed stage.No significant differences in EDV at three stages among three groups.There was good correlation between decreased left ventricular EF and ESV dilation.Multivariate regression analysis indicated that summel defect score was the only independent predictor for left ventricular EF decreasing and ESV dilation.The same phenomenon was not found in Normal perfusion group and Low likelihood CAD group. Conclusion:The myocardial stunning is frequently found in ischemia patients by post-ischemic gated SPECT perfusion imaging as left ventricular EF decreasing and ESV dilation.The severity of myocardial stunning depends on the extent of ischemia.

Abstract:Introduction: Adenosine and regadenoson are commonly used vasodilators in myocardial perfusion imaging for assessment of suspected coronary artery disease. Hypothesis: The aim of this study is to determine the difference between the different vasodilators by quantifying stress and rest myocardial perfusion using cardiovascular magnetic resonance (CMR). Methods: Subjects with known or suspected CAD from 10 centers undergoing invasive coronary angiography or CT angiography were enrolled for rest and stress first-pass perfusion images using dual sequence. First-pass stress perfusion images were acquired on 1.5T or 3.0T GE scanner during adenosine or Regadenoson. Fully quantitative perfusion values were determined using Fermi deconvolution. Significant CAD was defined by: presence of≥50% stenosis in the left main coronary artery or ≥70% in the one major vessel. Diagnostic performance of stress myocardial blood flow (MBF) and myocardial perfusion reserve (MPR) was measured using receiver operating characteristics curves. Results: A total of 89 subjects were recruited with a median age of 66 yrs, 69% men, 55% significant CAD, history of hypertension in 78%, diabetes in 48%, and hyperlipidemia in 74%. 36 subjects used adenosine, while 53 used regadenoson as the vasodilator agent. Stress MBF had a good area under the curve for adenosine vs. regadenoson [ 0.89 (0.78-1.00) vs. 0.82 (0.66-0.98), p=0.43], sensitivity (100% vs. 84%), and specificity (80% vs. 80%). MPR had a higher area under the curve for adenosine than regadenoson [0.87 (0.75-0.98) vs. 0.68 (0.52-0.84), p=0.06], sensitivity (100% vs. 61%), and specificity (68% vs. 73%). (Figure) Conclusions: Our study showed that fully quantitative perfusion using CMR, regadenoson, and adenosine have similar good diagnostic accuracy when stress MBF was used as the diagnostic parameter. However, adenosine outperformed regadenoson for the detection of significant CAD when MPR was used as the diagnostic parameter.
