Multiparametric Ultrasound for Focal Testicular Pathology: A Ten-Year Retrospective Review

Abstract:Conventional ultrasonography (US), including greyscale imaging and colour Doppler US (CDUS), is pivotal for diagnosing scrotal pathologies, but it has limited specificity. Historically, solid focal testicular abnormalities often led to radical orchidectomy. This retrospective study evaluated the utilisation of contrast-enhanced ultrasound (CEUS) and strain elastography (SE) in investigating intratesticular focal abnormalities. A total of 124 cases were analysed. This study underscored the superior diagnostic capabilities of CEUS in detecting vascular enhancement in all malignant cases, even those with undetectable vascularity by CDUS. It also highlighted the potential of CEUS in identifying distinctive vascular patterns in benign vascular tumours. Definitive confirmation of benignity could be obtained when the absence of enhancement was demonstrated on CEUS. While SE alone offered no distinctive advantage in differentiating between benign and malignant pathologies, we demonstrated that incorporating a combination of CEUS and SE into the evaluation of focal testicular abnormalities could improve diagnostic performance metrics over conventional CDUS. Our findings underscore the role of advanced ultrasound techniques in enhancing the evaluation of focal testicular abnormalities in clinical practice and could aid a shift towards testis-sparing management strategies.

The problem that this paper attempts to solve is that when evaluating focal testicular lesions, although traditional ultrasound examinations (such as grayscale imaging and color Doppler ultrasound) are of great value in diagnosing scrotal pathologies, their specificity is limited and it is difficult to accurately distinguish between benign and malignant lesions. This has led to the fact that for solid testicular lesions, especially palpable masses, radical orchiectomy is usually performed, and this surgical method may lead to unnecessary organ loss. To improve this situation, through a retrospective study, the paper explores whether the combined use of contrast - enhanced ultrasound (CEUS) and strain elastography (SE), two advanced ultrasound techniques, can improve the diagnostic accuracy of focal testicular lesions. Specifically, the study aims to evaluate whether these techniques can more accurately identify vascular enhancement patterns, thereby detecting vascular enhancement in malignant cases, even if these cases are not identified in conventional color Doppler ultrasound; at the same time, the study also explores the effectiveness of these techniques in confirming benign lesions, especially when CEUS shows no enhancement, whether it can more surely confirm the benign characteristics of the lesions. In addition, the study also examines whether the combined use of CEUS and SE can provide better diagnostic performance indicators than using these techniques alone or traditional ultrasound techniques. In summary, the main purpose of this study is to share the clinical experience of using multi - parameter ultrasound techniques (including CEUS and SE) to evaluate focal testicular lesions over the past ten years, and show how these advanced ultrasound techniques can help improve the accuracy of diagnosis, and then support the adoption of more conservative management strategies, such as organ - sparing treatment methods, rather than the traditional radical orchiectomy.
