Abstract:With the wide use of computers in China [1], more and more people use the internet to get information [2]. This information is usually accepted as accurate and is used in discussion with their doctors. In the past 5 years, we have observed that the majority of patients and risk populations managed in our HIV/AIDS clinic had searched AIDS on the internet before the first physician visit or telephone consultation. Some individuals only consulted with physicians to discuss information found on the internet because some of the information was not consistent between different websites. We thus decided to evaluate the quality and content of the information pertaining to HIV/AIDS on the internet most accessed by Chinese patients. To our knowledge this is the first such investigation in China. Ten specific topics (transmission, diagnosis, natural history, usefulness of CD4 cell counts and HIV viral load detection, treatment in China, opportunistic infections, pregnancy, breast feeding, prevention, and Chinese state policies pertaining to HIV/AIDS cases), which were the most frequently asked questions were evaluated. Three search engines (Yahoo, Baidu and Sina) were used to find general information about AIDS provided in simplified Chinese characters. Searches were performed using the following three phrases: (aizibing meaning AIDS patients), HIV, and AIDS. Results were compiled from all three searches and duplicates removed. The first 100 results returned for each search engine were reviewed. Websites that contained only anecdotal stories or personal accounts of AIDS were excluded, as were websites that contained only superlinks to other sites without content of their own or not written in simple Chinese figures. Sites that provided only AIDS information and contained multiple pages or sections giving general information about AIDS were included for study. Thirteen websites met these criteria and were studied (see Table 1).Table 1: Summary of AIDS information contained in 13 selected websites.The coverage of the subject and accuracy of the information with respect to 10 AIDS topics were evaluated for the 13 websites. The generally accepted information was based on statements by the World Health Organization and Chinese Centre for Disease Control and Prevention. Based on the copyright of the website, of the 13 websites examined, three were provided by government, two by hospitals, five by non-commercial organizations, and three were unknown. Information was categorized as ‘accurate’, ‘inaccurate’, ‘insufficient’, or ‘not addressed/not found'. The content of the websites was evaluated on three separate occasions (November 2005, January 2006 and May 2006). Only four websites provided general information on more than five topics, and 10 websites contained no information on more than four topics. None provided special literature, journals, or their superlinks. Only one, www.sh-aids.com, provided relatively sufficient information in this study. In our opinion, all of the 13 websites were aimed at the public. Although all the websites did not contain sufficient information for all 10 topics, the content provided was accurate. Information with same origin occurred frequently. Almost all websites contained enough information concerning transmission routes and HIV prevention. Most websites expatiated on the progress of HIV infection and the window period, including the clinical manifestations and laboratory tests. With regard to the topic of HIV infection diagnosis, 11 websites elaborated on the procedure and the results of the screening test, only one website (www.sh-aids.com) gave a particular explanation of the confirmatory test. Five websites contained information on CD4 cell counts and HIV-RNA viral load detection. Almost all websites provided some information on HIV treatment, but only three gave the full data we evaluated. Very little information was provided with respect to mother-to-child transmission, opportunistic infections, and the state policies pertaining to HIV/AIDS. Every website contained some information not included in the 10 topics, such as HIV risk behaviour, HIV high-risk populations, pictures and news related to HIV/AIDS, the side effects of antiretrovial medications, etc. Although the information on HIV risk behaviour was not evaluated in this study, five websites provided information that ‘sharing razor and toothbrush’ was a high risk. It is a hypothetical risk, early studies have shown a lack of transmission of HIV infection by close interpersonal casual contact [3]. In addition, ‘homosexual relationship’ is considered a high-risk behaviour, but is not specified as a male homosexual relationship. HIV/AIDS in China is considered to be in its rapidly increasing phase [4]. As HAART is not available for most patients, the most effective method is to minimize risk through education and information to the population [5]. It is important to take advantage of the internet, because of its mass appeal and increased use in China [1]. The present study has shown that there is insufficient and inaccurate information regarding HIV/AIDS on Chinese websites. Considering the successful use of the internet in HIV/AIDS prevention in Europe and the United States [6–8], we suggest that more attention should be paid to the development of adequate AIDS websites in China.
