Abstract:【Objective】 To investigate the correlation of expression or overexpression and co-expression or co-overexpression of HER family proteins with the clinicopathological parameters of gastric cancer.【Method】 Expression of HER1–4 proteins were determined by immunohistochemistry in 124 gastric cancer tissues,the correlation of expression,overexpression,co-expression and co-overexpression of HER family proteins with clinicopathological parameters and prognosis of the patients were also analyzed.【Results】 The expression rates of HER family proteins in 124 gastric cancer were 41.1%(51/124),59.7%(74/124),82.2%(102/124),and 28.2%(35/124),respectively.The overexpression rates of HER 1 ～ 4 proteins in the patients with gastric cancer were 6.5%(8/124),26.6%(33/124),22.6%(28/124),and 12.1%(15/124),respectively.HER2 overexpression was associated with intestinal-type gastric cancer(P = 0.001);HER3 overexpression was associated with depth of invasion,lymph node metastasis,distant metastasis,TNM stage of gastric cancer and worse survival(all P 0.05);the co-expression of HER1,HER2,and HER3 were associated with distant metastasis of gastric cancer(P = 0.035).【Conclusion】 Overexpression of HER3 may be used to assess the malignant biological behavior and prognosis of gastric cancer.

Abstract:Background Gastric cancer (GC) is a digestive system malignancy. Trastuzumab (a HER2-targeted monoclonal antibody) is an important targeted drug for GC. However, the drug resistance limits its clinical efficacy. B7-H3 was suggested to be a promising target for cancer immunotherapy. This study aimed to investigate the clinical significance of B7-H3 and HER2 co-expression and the therapeutic value of combination treatment in GC. Methods We examined the expression of B7-H3 and HER2 in 268 GC patients by immunohistochemistry. Pearson test was used to analyze the correlation between categorical variables. Overall survival was assessed by Kaplan–Meier analysis. All in vitro experiments using HER2-positive GC cells were treated with small interfering RNA targeting B7-H3/HER2 or B7-H3 blocking antibody 3E8/trastuzumab to verify the antitumor efficacy of the combination therapy. GC xenograft mouse models were established to evaluate the in vivo anti-tumor effect of combined therapy. Results There was a significant correlation between B7-H3 and HER2 expression in GC tissues. High co-expression of B7-H3 and HER2 was associated with poor prognosis ( P = 0.007) and could be an independent risk factor for survival. In addition, knockdown or targeted therapies of B7-H3/HER2 significantly suppressed cell proliferation, migration, invasion and adhesion in vitro . Trastuzumab combined with 3E8 was significantly effective at reducing mice tumor growth than monotherapy. Conclusion High co-expression of B7-H3 and HER2 indicates a poor prognosis, and combination therapy targeting B7-H3 and HER2 could be an immunotherapeutic strategy for GC.

Abstract:Objective To investigate the expressions of HER2,HER3 in evaluating prognosis of patients with gastric cancer.Methods The expressions of HER2 and HER3 in cancer tissue were detected with immunohistochemistry in 67 patients with gastric cancer.Correlations with clinicopathological parameters and prognostic significance were tested statistically.Results High HER2 expression was seen in 21(31.3%) cases,high HER3 expression was seen in 38(56.7%) cases,and high expressions of HER2 and HER3 were seen in 18 (26.9%) patients.HER2 had no significant correlation with clinicopathological parameters,HER3 was only correlated with near part of the tumor(P0.05).HER2 expression was correlated with HER3 expression(P0.05).Neither HER2 nor HER3 or HER2-HER3 coexpression had prognostic significance.Conclusion Expression rate of HER2 or HER3 in gastric cancer patients is high,but has no significance in predicting the prognosis.

Abstract:BACKGROUND AND AIMS:Despite significant improvements in targeted therapies for patients with advanced gastric cancer (GC), the prognosis of those patients remains poor. This study explores the expression and clinicopathological significance of human epidermal growth factor receptor 2, 3 and 4 (HER2, HER3, HER4) in GC, in order to find more prognostic biomarkers of GC and putative targets of therapy.METHODS:Immunohistochemistry was performed for HER2, HER3 and HER4 in 498 patients with GC using tissue microarray. Correlations between the receptor expression and clinicopathological features, as well as prognosis of the patients were statistically analysed.RESULTS:The high expression rates of HER2, HER3 and HER4 proteins in the patients were 8.6% (43/498), 20.7% (103/498) and 13.3% (66/498), respectively. High expression of HER2 and HER3 was correlated with proximal GC of the cardia (p<0.05). High expression of HER3 was associated with the tumour depth, tumour node metastasis (TNM) stage and lymph node metastasis (p<0.05). High expression of HER4 was associated with TNM stage (p<0.05) only. According to a regression model, high expression of HER3 was significantly associated with patients' poor survival (p=0.004). High expression rates of HER2, HER3 and HER4 were correlated with each other, but they were all associated merely with histologically intestinal-type adenocarcinoma of GC (p<0.05).CONCLUSIONS:HER3 is correlated with the malignant biological behaviour of GC. Expression of HER3 is a significant predictor of poor survival in GC. Therefore, the development of HER3-targeted agents may provide new possibilities in the treatment of GC.

Abstract:Background and AimHuman epidermal growth factor receptor (HER) family plays an important role in gastric cancer (GC), especially HER2. Too much attention has been paid to HER2; however, the functions of HER3 and HER4 overexpression in GC are always ignored. The clinicopathological and prognostic roles of HER3 and HER4 in GC are controversial. In this study, a systematic review and meta-analysis was conducted to evaluate the use of HER3 or HER4 as a predictor of clinicopathology and survival time in GC patients.MethodsEligible studies were searched on PubMed, Ovid, Web of Science, and Cochrane databases through multiple search strategies. Data collection and statistical analysis were carried out by the Revman 5.3 software. The Newcastle-Ottawa scale was used to assess the quality of included studies.ResultsA total of 448 studies about HER3 overexpression and GC, and 398 studies about HER4 overexpression and GC were searched. Of these, 5 eligible studies about HER3 including 1016 GC patients and 3 eligible studies about HER4 including 793 GC patients met the inclusion criteria. The results showed that HER3 and HER4 overexpression were significantly associated with depth of tumor invasion (OR = 0.44, 95% CI 0.29-0.67, P = 0.0002 and OR = 0.50, 95% CI 0.38-0.86, P = 0.007) and lymph node metastasis (OR = 0.40, 95% CI 0.20-0.77, P = 0.007 and OR = 0.57, 95% CI 0.38-0.86, P = 0.007), and HER3 overexpression reveals a tendency of later tumor node metastases (TNM) stage (OR = 0.50, 95% CI 0.22-1.15, P = 0.10) and predicts a worse survival time (RR = 0.71, 95% CI 0.61-0.84, P<0.00001), while HER4 overexpression had no correlation with TNM stage (OR = 0.60, 95% CI 0.20-1.78) and survival time (RR = 1.09, 95% CI 0.91-1.30).ConclusionsThis meta-analysis indicated that HER3 plays an essential role in the clinicopathology and prognosis of GC. However, HER4 may not be an ideal prognostic factor for GC.
The Clinical and Prognostic Significance of HER2 and Ki67 Expression in 2062 Chinese Patients with Resectable Gastric Cancer: a Retrospective Study

Abstract:Abstract Background The purpose of this study was to assess the prognostic value of the expression of human epidermal growth factor receptor 2 (HER2), Ki67, and their combination levels in the prognosis of Chinese patients with resectable gastric cancer (GC). Methods A total of 2062 Chinese GC patients were recruited with HER2 and Ki67 expression being evaluated using immunohistochemistry. Patients were divided into four groups according to HER2 and Ki67 expression. The distributions between HER2 and Ki67 expression levels and clinicopathological characteristics were compared using the Chi-square test. The relationship between HER2 and Ki67 expression level and overall survival were evaluated with the univariate and multivariate Cox regression analysis. Results There was no statistical differences between the overall survival (OS) rate and the expression level of HER2 (P = 0.748) or Ki67 (P = 0.063), but there were significant relationships between the OS rates and the combining expression levels of HER2/Ki67 (P < 0.05). Further, Ki67, sex, T stage, N stage, TNM stage, and adjuvant chemotherapy were significant and independent risk factors for GC survival (P < 0.05). Conclusions Our study illustrated that Ki67, but not HER2 acted as an independent prognostic factor in Chinese resectable GC patients. The evaluation of the combining expression levels of HER2/Ki67 may be more useful to assess patient prognosis with resectable GC.
Expression of Human Epidermal Growth Factor Receptor-2 Status and Programmed Cell Death Protein-1 Ligand Is Associated With Prognosis in Gastric Cancer

Abstract:Background PD-L1 and HER-2 are routine biomarkers for gastric cancer (GC). However, little research has been done to investigate the correlation among PD-L1, HER-2, immune microenvironment, and clinical features in GC. Methods Between January 2013 and May 2020, a total of 120 GC patients treated with chemotherapy were admitted to Henan Tumor Hospital. We retrospectively identified PD-L1, HER-2 level before chemotherapy and abstracted clinicopathologic features and treatment outcomes. Univariate and multivariate survival analyses were performed to assess the relationship between PD-L1/HER-2 levels and progression-free survival (PFS). The mRNA and tumor microenvironment of 343 patients with GC from The Cancer Genome Atlas (TCGA) were used to explore the underlying mechanism. Results We retrospectively analyzed 120 patients with gastric cancer, including 17 patients with HER-2 positive and 103 patients with HER-2 negative GC. The results showed that the expression of PD-L1 was closely correlated with HER-2 (P = 0.015). Patients with PD-L1/HER-2 positive obtained lower PFS compared to PD-L1/HER-2 negative (mPFS: 6.4 vs . 11.1 months, P = 0.014, mPFS: 5.3 vs . 11.1 months, P = 0.002, respectively), and the PD-L1 negative and HER-2 negative had the best PFS than other groups (P = 0.0008). In a multivariate model, PD-L1 status, HER-2 status, tumor location, and tumor differentiation remained independent prognostic indicators for PFS (P < 0.05). The results of database further analysis showed that the proportion of PD-L1+/CD8A+ in HER-2 negative patients was higher than that in HER-2 positive patients (37.6 vs 20.3%), while PD-L1−/CD8A− was significantly higher in HER-2 positive patients than HER-2 negative patients (57.8 vs . 28.8%). In addition, it showed that not only CD4+T cells, macrophages, and CD8+T cells, but also the associated inflammatory pathways such as IFN- γ /STAT1 were associated with HER-2. Conclusion HER-2 status could predict the efficacy of immune checkpoint inhibitors, and HER-2 status combined with PD-L1 level could predict the prognosis of GC patients.

Abstract:Objective To explore the clinicopathological features and prognostic significance of human epidermal growth factor receptor 2 (HER2) in gastric carcinoma. Methods Pathological data of 127 patients with gastric carcinoma were retrospectively analyzed. HER2 expressions of all patients were detected by immunohistochemistry (IHC). 119 (93.7 %) patients were undergone R0 dissection and 123 (96.9 %) cases received D2 lymph nodes dissection. 51 (40.2 %) patients received adjuvant chemotherapy. The proportional differences of clinicopathological features for patients between HER2-positive and HER2-negative were compared, including the patients' survival. Results HER2 overexpression rate was 8.7 % (11/127) in gastric carcinoma patients. For the patients with HER2-positive and HER-negative, the lymph node metastasis rates were 100.0 % (11/11) and 81.9 % (95/116), respectively (P= 0.041). The 3-year overall survival (OS) rates for HER2-positive and HER2- negative patients with gastric carcinoma were 32.7 % and 42.9 % (P=0.413), and the 5-year progression free survival (PFS) rates were 27.3 % and 42.2 % (P = 0.354), respectively. Among patients with HER2-negative, 3-year OS rate for patients with surgery plus adjuvant chemotherapy was 55.3%, compared with 35.4%for patients with surgery alone (P=0.015), and the 3-year PFS rates were 53.3 % and 35.3 % (P= 0.038), respectively. Among patients with HER2-positive patients, 3-year OS rate for patients with surgery plus adjuvant chemotherapy was 0, compared with 75.0%for patients with surgery alone (P=0.002), and the 3-year PFS rates were 0 and 60.0% (P=0.004). Conclusions HER2 is expressed in gastric carcinoma tissue, related to lymph node metastasis. HER2 status are not correlated with the prognosis for gastric carcinoma patients, however, it is likely to be a predictive marker for adjuvant treatment after surgery for patients with gastric carcinoma.

Abstract:Backgroud:The efficacy of trastuzumab for treating human epidermal growth factor receptor 2(HER2)-positive gastric cancer(GC)has been confirmed by phaseⅢclinical trial.Investigation on correlation of HER2 score with clinical outcome will be helpful for determining which patient might be benefit from trastuzumab therapy.Aims:To study the correlation of HER2 gene amplification and protein expression with clinicopathological characteristics and prognosis of GC.Methods:One hundred and seventy-seven GC specimens were collected.HER2 gene amplification and protein expression were determined by using the Food and Drug Administration(FDA)-approved kits for fluorescence in situ hybridization(FISH)and immunohistochemistry(IHC),and were scored according to the consensus panel recommendations on HER2 scoring for GC.The difference in HER2 positivity rate among GC with different clinicopathological characteristics was analyzed,and the correlation between HER2 and prognosis was estimated by Kaplan-Meier survival curve.Results:Thirty-one cases of GC(17.5%)were positive for HER2.The positivity rates in intestinal type GC and well-differentiated GC were higher than those in diffuse/mixed type GC and poorly-differentiated GC,respectively(28.1% vs.12.5%,P=0.0109;37.0%vs.10.7%,P<0.0001),but were not correlated with gender,age,tumor location and TNM stage.The overall survival was not significantly different between HER2-positive and HER2-negative GC,but in well-differentiated cases,the survival of HER2-positive GC was worse than that of HER2-negative GC(P=0.0084). Conclusions:Patients with intestinal type GC and well-differentiated GC may be the potential responders to trastuzumab therapy.HER2 can not yet be served as an independent predictor for prognosis of GC.
Relationship Between Overexpression of HER3 and Malignant Phenotype of Gastric Cancer

Abstract:Objective To study the relationship between overexpression of human epidermal growth factor receptor 3(HER3) and malignant phenotype of gastric cancer.Methods The expression of HER3 was detected by immunohistochemistry in 161 patients with gastric cancer underwent gastrectomy.Clinicopathological characteristics and the expression of HER3 were retrospectively analyzed.HER3 siRNA was constructed and transfected to human gastric cancer cell with HER3 expression.The expression of HER3 was detected by Western blot assay,the proliferation of the cells by MTT assay,the invasion by wound-healing assay,and the apoptosis by TUNEL assay.ResultsThe high expression of HER3 was associated with tumor invasive depth,lymph node metastasis,stage,the invasion of nerve and vessel,and the 4-year overall survival(P0.05 or P0.01).The independent prognostic factors were the stage(HR:4.044,95%CI:2.149-7.609,P=0.000) and overexpression of HER3(HR:1.922,95%CI:1.081-3.416,P=0.026).In vitro,expression of HER3 was significantly inhibited by HER3 siRNA.The proliferation(72 h:MNK45 vs.MNK45 siRNA,SGC7901 vs.SGC7901 siRNA:P0.05,96 h:MNK45 vs.MNK45 siRNA:P0.05,SGC7901 vs.SGC7901 siRNA: P0.01) and the invasion(P=0.0071) of gastric cancer cells were down-regulated and the apoptosis(MNK45 vs.MNK45 siRNA:P=0.0019,SGC7901 vs.SGC7901 siRNA:P=0.0061) of the cells was significantly increased after treated with HER3 siRNA.Conclusion In gastric cancer,the overexpression of HER3 is associated with the proliferation,invasion,and apoptosis of gastric cancer.High expression of HER3 predicts a poor survival in the patients with gastric cancer.
Expression of HER-2/neu,PTEN and CD44V6 in Gastric Cancer Tissue and Its Correlation with the Prognosis of Gastric Cancer

Abstract:Objective To study the expression of HER-2/neu,PTEN and CD44V6 proteins in gastric cancer and its correlation with the prognosis of gastric cancer.Methods Expressions of HER-2/neu,PTEN and CD44V6 proteins in 30 normal tissue samples and 65 gastric cancer tissue samples were detected with immunohistochemical staining.Results The positive rate of HER-2/neu,PTEN and CD44V6 expression was 21.5%,36.9%and 58.5%,respectively,in gastric cancer tissue samples,and 10.0%,90.0%and 10.0%,respectively,in normal tissue samples(P0.05) .The expression of HER-2/neu,PTEN and CD44V6 in gastric cancer was closely related with tumor cell differentiation,invasion depth and lymph node metastasis(P0.05) but not with age,sex and pathological stages of patients(P0.05) .The 5-year survival rate for patients with positive or negative expression of CD44V6 and HER-2/neu was 7.14%and 22.2%,respectively(P0.05) .Cox regression analysis showed that the expression of HER-2/neu and CD44V6 proteins was a risk factor for gastric cancer.Conclusion Expression of HER-2/neu,PTEN and CD44V6 proteins in gastric cancer can be used as an indicator for predicting the prognosis of gastric cancer.
Overexpression of Grb2/HER2 Signaling in Chinese Gastric Cancer: Their Relationship with Clinicopathological Parameters and Prognostic Significance

Abstract:Purpose Growth factor receptor-bound 2 (Grb2)-mediated HER2 signaling is thought to play a critical role in gastric cancer development, progression and metastasis. However, little is known about their expression in gastric cancer. In this study, we try to explore their relationship with clinicopathological parameters and prognostic significance in gastric cancer patients. Materials and methods We examined the expression of Grb2 and HER2 in normal gastric mucosa, primary gastric cancers, and lymph node metastases using immunohistochemical analysis of tissue microarrays containing specimens obtained from 1,143 patients with gastric cancer. Results Grb2 was overexpressed in 48% (553/1,143) of primary tumors and 59% (155/262) of lymph node metastases. We observed significant differences in Grb2 expression between the primary tumors and the lymph node metastases ( P < 0.01). Also, HER2 was overexpressed in 28% (321/1,143) of the primary tumors and 30% (79/262) of the lymph node metastases. Overexpression of Grb2 and Her2 was associated with age (>60 years), tumor location (cardia of stomach), adenocarcinoma, and high/moderate differentiation. A significant relationship was found between Grb2 and HER2 expression using Chi-Square Tests and Spearman Correlation. Overexpression of Grb2 correlated significantly with poor survival rates in both univariate and multivariate analysis. Conclusions Our data demonstrated a progressive amplification of Grb2 and HER2 expression in gastric carcinogenesis, suggesting the importance of Grb2 and HER2 as positive biomarkers for gastric cancer development and progression.
Overexpression of the chemokine receptor CXCR3 and its correlation with favorable prognosis in gastric cancer

Abstract:Chemokine receptor, CXCR3, has been increasingly reported to be involved in tumorigenesis and tumor progression, but limited data are available regarding the expression of CXCR3 in gastric cancer (GC). In the present study, the expressions of CXCR3 and its variants were detected in 96 GC and corresponding nontumor gastric tissues by immunohistochemical staining, in 40 freshly frozen GC and nontumor gastric tissues by reverse-transcription polymerase chain reaction and quantitative real-time polymerase chain reaction, and in 10 freshly frozen GC and nontumor gastric tissues by Western blotting. Results revealed that an overexpression of CXCR3 occurs in GC tissues as compared to the nontumor gastric tissues. High level of CXCR3 expression was found to be inversely associated with invasion depth and metastasis (P = .030 and P = .019, respectively) and directly associated with improved overall survival (log-rank test, P < .001). Furthermore, multivariate analysis showed that high CXCR3 expression acts an independent prognostic factor for GC patients (hazard ratio, 0.379 [0.196-0.734]; P = .004). The messenger RNA expression of both the CXCR3 variants, CXCR3-A and CXCR3-B, were up-regulated in GC tissues (P = .006 and P = .002, respectively), although CXCR3-B messenger RNA expression was significantly higher than CXCR3-A, with an average CXCR3-B to CXCR3-A ratio of 1.80. CXCR3-B protein expression was also up-regulated in GC tissues (P = .023). In conclusion, our study suggested a potential use of CXCR3 overexpression as a prognostic marker for GC and involvement of the up-regulation of CXCR3-B in favorable prognosis of GC patients.

Abstract:The current study reported that combination of HER2 amplification and PD‐L1 expression in Chinese patients with gastric cancer could stratify the total populations into several subgroups with distinctive genomic and immune landscapes, which could be sensitive to different therapeutic regimens, indicating that both HER2 status and PD‐L1 expression level should be considered when making personalized treatment decisions for patients with advanced gastric cancer. Background Anti‐PD1/PD‐L1 antibody plus human epidermal growth factor receptor 2 (HER2) antibody and chemotherapy have become the new first‐line therapy for HER2 overexpression‐positive advanced gastric cancers (GC), suggesting that HER2 and PD‐L1 play a vital role in guiding systemic treatment for patients with GC. This study aimed to depict the genomic and immune landscapes of Chinese patients with GC and investigate their correlations with HER2 amplification and PD‐L1 expression. Patients and Methods Next‐generation targeted sequencing and PD‐L1 immunohistochemistry were performed on tumor samples from 735 patients with pathologically diagnosed GC. The genomic and immune landscapes and their correlations with HER2 amplification and PD‐L1 expression were analyzed. Results The most commonly mutated genes in Chinese GC were TP53 (64%), CDH1 (20%), ARID1A (18%), HMCN1 (15%), KMT2D (11%), and PIK3CA (11%). Seventy‐six (10%) patients were HER2 amplification, and 291 (40%) had positive PD‐L1 expression. Classifying the total population based on HER2 amplification and PD‐L1 expression level, 735 patients were divided into four subgroups: HER2+/PD‐L1+ (4.5%), HER2+/PD‐L1− (5.9%), HER2−/PD‐L1+ (35.1%), and HER2−/PD‐L1− (54.5%). The HER2+/PD‐L1− and HER2+/PD‐L1+ subgroups exhibited dramatically higher rate of TP53 mutations, CCNE1 and VEGF amplifications. The HER2+/PD‐L1− subgroup also had a markedly higher rate of MYC amplification and KRAS mutations. The HER2−/PD‐L1+ subgroup had significantly higher rate of PIK3CA mutations. HER2+/PD‐L1− subgroup had the highest TMB level and HER2−/PD‐L1+ subgroup had the highest proportion of patients with microsatellite instability‐high than other subgroups. Furthermore, we observed that different HER2 amplification levels had distinct impacts on the correlations between PD‐L1 expression and therapeutic genomic alterations, but no impact on the prognosis. Conclusion The combination of HER2 amplification and PD‐L1 expression in Chinese patients with GC could stratify the total populations into several subgroups with distinctive genomic and immune landscapes, which should be considered when making personalized treatment decisions.

TFF3 and HER2 expression and their correlation with survival in gastric cancer

Abstract:The molecular biomarkers human epidermal growth factor receptor-2 (HER2) and trefoil factor 3 (TFF3) are reported to play important roles in the pathogenesis of gastric cancer (GC). In this study, we investigated the clinicopathological and prognostic significance of TFF3 and HER2 expression in GC and explored the correlation between these two biomarkers. Ninety-two patients who were diagnosed with GC were enrolled. TFF3 and HER2 expression was determined on tumor tissues. The results showed that TFF3 and HER2 were positively expressed in 42.7 and 10.9 % of the cases, respectively. There were significantly higher rates of TFF3 positivity in patients with deep invasive tumors and advanced stage ones. Patients with negative TFF3 staining survived longer than those with the presence of TFF3, with 5-year overall survival (OS) rates of 57.1 ± 7.1 and 39.5 ± 7.5 %, respectively ( P = 0.033). However, HER2 positivity was not significantly associated with OS ( P = 0.262). Multivariate analysis demonstrated TFF3 expression to be an independent indicator for short-term survival, with a hazard ratio of 2.327 (95 % confidence interval (CI), 1.202–4.507, P = 0.012). There was a trend that the expression of TFF3 was more frequent in HER2 negative tumors than in HER2 positive ones (positive rates: 16.3 vs. 4.7 %, P = 0.098). Patients with HER2-negative/TFF3-negative GC presented higher OS than those with other phenotypes ( P = 0.009). This study suggests that TFF3 is an independent indicator for survival in GC, while HER2 is not associated with the outcome. Patients with HER2-negative/TFF3-negative GC have the best outcome.
Relationship Between Four Tumor-Associated Bio-Markers and Prognosis of Gastric Cancer

Abstract:Purpose: To investigate the relationship between prognosis of gastric cancer (GC) and the expression of P53, Epidermal growth factor receptor (EGFR), Human epidermal growth factor receptor-2 (HER-2), and Vascular endothelial growth factor (VEGF).Methods: One hundred and forty-seven patients admitted to People's Liberation Army General Hospital (Beijing, China) with diagnosis of locally advanced GC were enrolled in the study. Follow-up data were obtained by outpatient review or telephone follow-up. Expressions of P53, EGFR, HER-2 and VEGF were determined by immunohistochemical staining. The relationship between protein expression, clinico-pathological factors, disease-free survival time (DFS) and overall survival (OS) were analyzed.Results: The expressions of EGER, HER-2, P53 and VEGF in GC were 17.7, 17.0, 41.0 and 55.9%, respectively. The expressions of EGFR and P53 were positively correlated (r = 0.306, p < 0.05), while the expressions of VEGF and HER-2 were negatively correlated (r = -0.2, p < 0.05). The expressions of EGFR, HER-2 and VEGF were not related to the clinico-pathological factors (p > 0.05) while expression of P53 was related only to histological grade (p < 0.05). Univariate analysis showed that OS and DFS were longer (p < 0.05) when P53 was lowly expressed. Multiple-factor analysis revealed that histological grade, infiltration depth and P53 expression were independent factors that influenced DFS.Conclusion: These results indicate that the expression of P53, EGFR, HER2 and VEGF can be used to predict prognosis of GC and screening of patients benefits from adjuvant chemotherapy.
Prognosis Significance of Her2/Neu Overexpression and Amplification in Patients with Curatively Resected Gastric Cancer

Abstract:e14539 Background: HER-2/neu targeted therapy has been successfully used in advanced gastric cancer, but the role of HER-2/neu inprognosis of gastric cancer is not yet clear. To investigate the correlation between HER-2/neu expression and amplification and their associations with clinicopathologic outcomes and prognosis in patients with curatively resected gastric cancer. Methods: We constructed tissue microarray blocks containing >70% of gastric cancer tissue and matched adjacent normal gastric tissue for 229 patients. Expression of the HER-2/neu protein in these specimens was analyzed using immunohistochemical (IHC) staining. Amplification of HER-2/neu was also analyzed for the same samples using fluorescence in situ hybridization (FISH). Kaplan Myers curve and Cox proportional hazards model were used to assess the survival. Results: Of the 229 gastric cancer samples, 14.85% were positive for HER-2/neu protein expression that was closely correlated to the Lauren type, degree of differentiation, tumor size, lymph node metastasis and overall survival. Overall, 11.79% of the 229 gastric cancers were positive for amplification of HER-2/neu, which was also closely correlated to the Lauren type, degree of differentiation, tumor size and overall survival. Moreover, Cox proportional hazards model showed that positive expression of HER-2/neu, Lauren type, tumor size, vessel invasion, TNM stage were independent prognostic factor. Both IHC and FISH assays provide an important prognostic factor for gastric cancer with a concordance rate of 96.9%, but IHC was a stronger independent prognostic factor. Conclusions: Expression of the HER-2/neu protein, compared with its gene amplification, was an independent and more sensitive prognostic factor for curatively resected gastric cancer patients.
Impact of HER2 on Prognosis and Benefit from Adjuvant Chemotherapy in Stage II/III Gastric Cancer Patients: a Multicenter Observational Study

Abstract:Background: Human epidermal growth factor receptor 2 (HER2) is a well-developed therapeutic target in breast and gastric cancer (GC). However, the impact of HER2 on survival and benefit from fluorouracil-based adjuvant chemotherapy remains unclear in patients with GC. Materials and Methods: This multicenter cohort study involved 5622 consecutive stage II/III GC patients. HER2 expression was assessed prospectively via immunohistochemistry (IHC). The staining intensity was graded on a scale of 0 to 3+. An IHC score of 2+or 3+was defined as high expression, and a score of 3+was defined as overexpression. Results: HER2 overexpression was independently associated with a lower 5-year overall survival (OS) in stage II [hazard ratio (HR), 2.10; 95% CI: 1.41–3.11], but not in stage III GC (HR, 1.00; 95% CI, 0.82–1.20). Further analysis revealed that stage II patients with high HER2 expression showed a poorer response to chemotherapy than stage II patients with low HER2 expression (P interaction=0.024). The HRs for 5-year OS were 0.51 (95% CI, 0.38–0.70) for stage II patients with low HER2 expression, 0.58 (95% CI, 0.51–0.66) for stage III patients with low HER2 expression, 1.13 (95% CI, 0.61–2.09) for stage II patients with high HER2 expression, and 0.47 (95% CI, 0.36–0.61) for stage III patients with high HER2 expression. Conclusions: Fluorouracil-based adjuvant chemotherapy is insufficient for stage II GC patients with high HER2 expression, indicating that prospective trials are required to validate alternative HER2-targeted adjuvant therapies in the individuals above.

Abstract:AimsThe mammalian target of rapamycin (mTOR) and phosphorylated mTOR (p-mTOR) occurring downstream in the PI3K/Akt/mTOR pathway, are regarded as potential prognostic markers for gastric cancer (GC). However, the prognostic value of mTOR/p-mTOR expression remains controversial. In this study, we determined the expression of mTOR, p-mTOR, p70S6k, and p-p70S6K in GC, and investigated the correlation between their overexpression, clinicopathological parameters, and overall survival (OS).MethodsThe expression of mTOR, p-mTOR, p70S6k, and p-p70S6K was examined in 120 GC patients by immunohistochemistry (IHC). The association of protein expression with clinicopathological features and OS was explored. The p-mTOR expression was detected in normal, adjacent, and GC tissues using Western blot. Eligible studies retrieved from PubMed, Ovid, Web of Science and Cochrane databases, were reviewed in this meta-analysis.ResultsIHC showed that the rates of expression of the signal transduction molecules mTOR, pmTOR, p70S6k and p-p70S6K in GC were 60.8%, 54.2%, 53.3% and 53.3%, respectively. Overexpression of mTOR and p70S6K showed no significant association with clinical variables. Expression of p-mTOR was significantly associated with differentiation (P < 0.01), depth of invasion (P < 0.01), lymph node metastasis (P = 0.04) and TNM stage (P = 0.02). Expression of p-p70S6K was associated with differentiation (P = 0.006), depth of invasion (P < 0.001), and TNM stage (P = 0.02). In survival analysis, differentiation, depth of invasion, lymph node metastasis and TNM stage were not related to OS (all P > 0.05). Furthermore, p-mTOR and p-p70S6K expression, but not mTOR and p70S6K, were tightly associated with OS of GC patients (P = 0.006 and P < 0.001, respectively). In Western blot, p-mTOR was significantly higher in GC tissues than in normal and adjacent tissues. In the present meta-analysis, mTOR overexpression showed no relationship with any clinicopathological variables. However, p-mTOR was correlated with depth of invasion, and TNM stage (all P < 0.05), and its overexpression was associated with a shorter survival time (P < 0.001).ConclusionThe results suggest that p-mTOR is a more valuable prognostic factor than mTOR in GC.
Association of HER2 Status with Prognosis in Gastric Cancer Patients Undergoing R0 Resection: A Large-Scale Multicenter Study in China

Abstract:AIM:To determine whether the positive status of human epidermal growth receptor 2 (HER2) can be regarded as an effective prognostic factor for patients with gastric cancer (GC) undergoing R0 resection.METHODS:A total of 1562 GC patients treated by R0 resection were recruited. HER2 status was evaluated in surgically resected samples of all the patients using immunohistochemical (IHC) staining. Correlations between HER2 status and clinicopathological characteristics were retrospective analyzed. Hazard ratios (HRs) and 95% confidence intervals (CIs) were estimated using Cox proportional hazard model, stratified by age, gender, tumor location and tumor-node-metastasis (TNM) stage, with additional adjustment for potential prognostic factors.RESULTS:Among 1562 patients, 548 (positive rate = 35.08%, 95%CI: 32.72%-37.45%) were HER2 positive. Positive status of HER2 was significantly correlated with gender (P = 0.004), minority (P < 0.001), tumor location (P = 0.001), pathological grade (P < 0.001), TNM stage (P < 0.001) and adjuvant radiotherapy (74.67% vs 23.53%, P = 0.011). No significant associations were observed between HER2 status and disease free survival (HR = 0.19, 95%CI: 0.96-1.46, P = 0.105) or overall survival (HR = 1.19, 95%CI: 0.96-1.48, P = 0.118) using multivariate analysis, although stratified analyses showed marginally statistically significant associations both in disease free survival and overall survival, especially among patients aged < 60 years or with early TNM stages (I and II). Categorical age, TNM stage, neural invasion, and adjuvant chemotherapy were, as expected, independent prognostic factors for both disease free survival and overall survival.CONCLUSION:The positive status of HER2 based on IHC staining was not related to the survival in patients with GC among the Chinese population.
