Abstract:T helper 17 (Th17) cells and related cytokines are important in multiple sclerosis (MS), which have become the new therapeutic targets of MS. In published studies, the effect of interferon-beta (IFN-beta) on the proportion of Th17 cells and related cytokines in MS patients remains controversial. We did a meta-analysis to assess the role of IFN-beta on Th17 cells and related cytokines in MS patients. Compared with control subjects, MS patients with IFN-beta treatment had a lower proportion of Th17 cells in CD4(+) T cells of peripheral blood [-1.49 (-2.96 to -0.02)], interleukin (IL)-17 level [-1.08 (-1.77 to -0.40)], and IL-23 level [-1.63 (-2.59 to -0.68)] in serum. The results showed that IFN-beta can reduce the proportion of Th17 cells, the levels of IL-17 and IL-23 in serum of MS.

Abstract:Multiple Sclerosis (MS) is an immune-mediated and neurodegenerative disease of central nervous system. Relapsing-remitting (RR)-MS occurring with acute attacks and remissions, is the most common clinical type of MS. There are different strategies applied in first-line treatment of RR-MS patients such as interferon-beta (IFN-β) and glatiramer acetate. In this study, activating and inhibitory receptor expressions and interleukin (IL)-22 levels of NK cells were investigated in RR-MS patients with or without IFN-β therapy. Activating receptor expression and IL-22 levels of NK cells were increased in RR-MS patients under IFN-β therapy. Elevated NK cells with activating profile and increased IL-22 under IFN-β therapy suggest that IFN-β treatment might direct NK cells toward a pro-inflammatory status.

Abstract:Background Systemic sclerosis (SSc) is a chronic inflammatory disease with complex pathogenesis. The role of regulatory T cells (Tregs) in the development of SSc has started being studied during the last decade with new aspects being disclosed continuously. Although there is a general agreement in the medical literature regarding the decreased functional capacity of circulating Tregs in SSc, the alteration of absolute number of Treg and Th17 cells as well as their associations with clinical characteristics of SSc are still unclear. Objectives The aim of the present study was to explore the relationship between absolute reduction of peripheral CD4 +regulatory T and Th17 cell subset and clinical characteristics in SSc patients. Methods The peripheral CD4 +T subsets from 54 patients with SSc and 30 healthy control subjects were analysed. The patients were divided into the untreated group (n=29) and treated group (n=25). The patients were also divided into group 1 (n=9) for the prominent pulmonary lesions, group 2 (n=5) for prominent esophageal involvement, group 3 (n=8) for both lung lesions and esophageal involvement, and group 4 (n=10) who had fingers swelling, sclerosis, ulceration prominent. Directly using the results from flow cytometry combined with internal standard beads, absolute number of peripheral Th17 and Treg cells from the subjects in each group were calculated. Results Although there were some changes among CD4 +T cell subsets in peripheral blood from these SSc patients, the major alteration was the reductions of Treg cell absolute number. Compared with the normal controls, the number of CD4+CD25+FOXP3+Treg cells were significantly decreased in untreated group (p=0.029) , in group 1 (p=0.014) and in group 2 (p=0.073) as compared with heathy controls. Interestingly, Tregs in group 2 were significantly lower than those in group 1 (p=0.006), in group 3(p=0.098) and in group 4(p=0.034). In addition, the number of Th17 cells were significantly higher in group 3 (p=0.003 and 4 (p Conclusions The absolute number of peripheral CD4 +CD25+FOXP3+Treg cells decreased in untreated patients, indicating that this reduction arising from the disease itself. Our findings suggest that the severity of clinical manifestations in patients with SSc is related to the absolute decrease of peripheral Treg cells. Although there was no statistically significant different, absolute number of the Treg cells trended to decrease in patients who had more severe clinical manifestations. With the involvement of vital organs, the absolute number of Treg cells decreased more significantly. Moreover, in patients with relatively mild clinical manifestations, the increase in the absolute number of Th17 cells is even more obvious, suggesting that Th17 also plays an important role in early and intermediate stages. Abstract:It has been reported that cytokines play an important role in the pathogenesis of multiple sclerosis (MS). The aim of this study was to evaluate the serum levels of interleukin (IL)-18, IL-23 and IL-17 in Chinese patients with MS. We compared the serum concentrations of pro-inflammatory cytokines IL-18, IL-23 and IL-17 in 39 patients with MS and 39 healthy controls matched with sex and age. Serum cytokines were measured by FlowCytomix, a kind of cytometric bead-based assay. Correlations between the serum levels of the three cytokines and disability (expanded disability status scale, EDSS), disease duration, current age and age at onset were examined. Serum concentrations of all IL-18, IL-23 and IL-17 were significantly higher in MS patients than healthy controls. There were no significant differences of the three cytokines' levels between female and male healthy controls, while the serum IL-18 level was observed significantly higher (P=0.049) in male MS patients than female MS patients. No significant correlations were observed between any of the three cytokines' levels and EDSS, disease duration and current age. However, IL-23 was found negatively correlated with age at onset in male MS patients (rs=−0.775, P=0.041). Our data suggest that all IL-18, IL-23 and IL-17 may be involved in the pathogenesis of MS. However, the relationships of the three cytokines and clinical characteristics of MS need to be further investigated in the future.

Abstract:Multiple sclerosis(MS)is a predominantly T-cell-mediated auto-immune inflammatory disease of the central nervous system(CNS)characterized by multifocal areas of immune cell infiltration,demyelination,axonal damage,and disruption of neuronal signal.1,2 It is now widely accepted that MS is a heterogeneous,multifactorial disease influenced by genetic,epigenetic,and environmental factors.3 Now,subsequent studies have identified and highlighted an important pathogenic role for Th17 cells in autoimmunity,a kind of CD4+T-cell subset that secrete signature cytokine interleukin IL-17A.4,5 However,the treatment for MS still remains a great challenge.Thus,the current prompted us to find a more effective disease-modifying treatment.Here,we found that small-molecule TPN10456 potently suppressed Th17 cell differentiation.Interestingly,TPN10456 inhibited pathogenic Th17 cells differentiation,but not conventional Th17 cells.Meanwhile,TPN 10456 also inhibited human Th17 cells differentiation in vitro.Moreover,compared with the vehicle group,TPN 10456-treated EAE mice showed less severe EAE symptoms,including lower clinical scores,reduced inflammatory cells infiltration,and less extensive demyelination in CNS.These results suggest that TPN 10456 may represent a promising therapeutic agent for the treatment of MS.

Objectives The aim of the present study was to explore the absolute number status of CD4 +T subsets in peripheral blood of patients with SSc using our modified flow cryometric method and investigate whether the imbalance of Th17 and Treg cells can be corrected by supplementing low dose interleukin −2 (IL-2). Methods The peripheral CD4 +T subsets from 54 patients with SSc and 30 healthy control subjects were analysed. The patients were divided into the untreated group (n=29) and treated group (n=25). The patients were also divided into group A (n=28) who only used low-dose glucocorticoids (GCS), and group B (n=26) who also used immunosuppressant cyclophosphamide (CYC), methotrexate (MTX), leflunomide (LEF) or mycophenolate mofetil (MMF). In addition, 21 patients from 54 patients, given a small dose of IL-2 (50WIU) treatment for a 5 day course, divided into pre-treatment group and post-treatment group. Directly using the results from flow cytometry combined with internal standard beads, absolute number of peripheral CD4 +T subsets from the subjects in each group were calculated. Results Although there were some changes among CD4 +T cell subsets in peripheral blood from these SSc patients, the major alteration was the reductions of Treg cell absolute number. Compared with the normal controls, the number of CD4+CD25+FOXP3+Treg cells were significantly decreased in all patients (p=0.001), in untreated group (p=0.029), in treated group (p=0.029), in group A (p=0.006) and in group B (p=0.004). The ratio of Th17/Treg in total patients (p=0.008) and group B (p=0.001) were significantly higher than that in normal control group, and the group B was significantly higher than the group A (p=0.032). Moreover, the number of Th17 cells in group B was significantly higher than that in group A (p=0.023). After IL-2 treatment, the absolute number of CD4+CD25+FOXP3+Treg cells (p 0.05). Since Th17 cells increases significantly lower than Treg cells, so their ratio decreases significantly (p=0.001) to get re-balance. Conclusions The absolute number of peripheral CD4 +CD25+FOXP3+Treg cells decreased in untreated patients, indicating that this reduction arising from the disease itself. Our findings suggest that SSc progression is associated with the absolute decrease of peripheral Treg cells. Although there was no statistically significant different, absolute number of the Treg cells trended to decrease in patients treated with immunosuppressive agents. The use of IL-2 can effectively up-regulate Treg cells as well as increase Th17 to a certain extent, which restores the balance of Th17/Treg cells, but the long-term effect needs further investigation. Abstract:Objective Multiple sclerosis (MS) is a mainly cell-mediated autoimmune demyelinating disease in central nervous system (CNS), characterized by inflammatory demyelinating and infiltration of mononuclear cells around microvessels in CNS. It has been shown that MS is caused by the imbalance between T helper cell 1 (Th1) and Th2 or between inflammatory cytokines and anti-inflammatory cytokines. However, the profile of cytokine according to the published data is contradictory. This study is to evaluate the status of cytokines from mononuclear T cells in MS patients and try to provide clues for clinical diagnosis and treatment. Methods Enzyme-linked immunospot assay (ELISPOT) was used to test the spontaneous and antigen-specific [concanavalin A (ConA), myelin basic protein (MBP) and acetyleholine receptor (AChR)] Th1-related cytokine interferon-γ (IFN-γ) and Th2-related cytokines interleukin-4 (IL-4), IL-10 in the peripheral blood mononuclear cells in MS patients, who had not received any immunological treatment over the last 3 months. Results Compared with normal controls and patients with non-immune neurological diseases, MBP specific IL-4, IL-10 and IFN-γ of MS patients increased significantly (P = 0.000, for all). In addition, MBP specific IFN-γ level of MS patients increased signicantly in acute or exacerbating phase when compared with that in stable phase (P = 0.002), while MBP specific IL-4 and IL-10 levels did not differ significantly (P > 0.05, for all). Causal association between the peripheral immunity and the risk and disease severity of multiple sclerosis

Abstract:Background: Growing evidence links immunological responses to Multiple sclerosis (MS), but specific immune factors are still unclear. Methods: Mendelian randomization (MR) was performed to investigate the association between peripheral hematological traits, MS risk, and its severity. Then, further subgroup analysis of immune counts and circulating cytokines and growth factors were performed. Results: MR revealed higher white blood cell count (OR [95%CI] = 1.26 [1.10,1.44], P = 1.12E-03, P adjust = 3.35E-03) and lymphocyte count (OR [95%CI] = 1.31 [1.15,1.50], P = 5.37E-05, P adjust = 3.22E-04) increased the risk of MS. In further analysis, higher T cell absolute count (OR [95%CI] = 2.04 [1.36,3.08], P = 6.37E-04, P adjust = 2.19E-02) and CD4 + T cell absolute count (OR [95%CI] = 2.11 [1.37,3.24], P = 6.37E-04, P adjust = 2.19E-02), could increase MS risk. While increasing CD25 ++ CD4 + T cell absolute count (OR [95%CI] = 0.75 [0.66,0.86], P = 2.12E-05, P adjust = 1.72E-03), CD25 ++ CD4 + T cell in T cell (OR [95%CI] = 0.79[0.70,0.89], P = 8.54E-05, P adjust = 5.29E-03), CD25 ++ CD4 + T cell in CD4 + T cell (OR [95%CI] = 0.80[0.72,0.89], P = 1.85E-05, P adjust = 1.72E-03), and CD25 ++ CD8 + T cell in T cell (OR [95%CI] = 0.68[0.57,0.81], P = 2.22E-05, P adjust = 1.72E-03), were proved to be causally defensive for MS. For the disease severity, the suggestive association between some traits related to CD4 + T cell, Tregs and MS severity were demonstrated. Moreover, elevated levels of IL-2Ra had a detrimental effect on the risk of MS (OR [95%CI] = 1.22 [1.12,1.32], P = 3.20E-06, P adjust = 1.34E-04). Conclusions: This study demonstrated a genetically predicted causal relationship between elevated peripheral immune cell counts and MS. Subgroup analysis revealed a specific contribution of peripheral immune cells, holding potential for further investigations into the underlying mechanisms of MS and its severity.

Abstract:In the chronic, organ-specific autoimmune disorder known as multiple sclerosis (MS), the myelin sheath is attacked by immune cells, leading to damage to the central nervous system (CNS). It has been discovered that miRNAs are important in the etiology of MS, since deregulation of miRNAs can lead to defects in immune tolerance. In this study, we sought to investigate the involvement of miR-155 in MS disorder through examination of its altered expression in peripheral blood mononuclear cells (PBMCs) of patients with MS in compare with healthy controls. Furthermore, we investigated the frequency of T helper 17 cells (Th17) in MS patients and analyzed not only the expression of inflammatory cytokines including IL-6, IL-17 and IL-21 in patients' PBMCs, but also their secreted levels in serum of patients suffering from MS. Subsequently, we assessed the correlation between miR-155 expression with Th17 frequency and levels of released cytokines in serum. Upregulated expression of miR-155 was detected in PBMCs of MS patients and the positive correlation between its expression with increased frequency of Th17 cells and their related inflammatory cytokine profile augmented secretion in serum were identified. In conclusion, our study revealed the significant association between Th17 frequency, increased level of cytokines related to Th17 differentiation and function with miR-155 augmented expression in PBMCs. So, our findings suggested that miR-155 and especially its expression in immune cells including effector T cells can be the target of future therapeutic strategies for the management and prevention of MS progression, however, further research is requisite before this approach can be utilized in clinical practice.
Let-7f-5p Suppresses Th17 Differentiation Via Targeting STAT3 in Multiple Sclerosis.

Abstract:T helper 17 (Th17) cells are regarded as key factors in the pathogenesis of multiple sclerosis (MS). Although the involvement of certain microRNAs (miRNAs) in the development of MS has been reported, their roles in Th17 cell differentiation and MS pathogenesis remain elusive. In this study, we identified that let-7f-5p expression is significantly downregulated in CD4+ T cells from MS patients and during the process of Th17 differentiation. The overexpression of let-7f-5p suppressed Th17 differentiation, whereas the knockdown of let-7f-5p expression enhanced this progress. We then explored the molecular mechanism through which let-7f-5p suppressed Th17 differentiation and identified signal transducer and activator of transcription 3 (STAT3), a pivotal transcription factor of Th17 cells, as a direct target of let-7f-5p. In contrast to the downregulated expression of let-7f-5p, STAT3 and p-STAT3 protein levels were dramatically upregulated and inversely correlated with let-7f-5p in peripheral blood CD4+ T cells from MS patients. In conclusion, let-7f-5p functions as a potential inhibitor of Th17 differentiation in the pathogenesis of MS by targeting STAT3 and may serve as a new therapeutic target.
Relationships of Peripheral Blood Th17 and Regulatory T Cells with Disease Activity in Patients with Systemic Sclerosis

Abstract:Objective To evaluate the relationship of peripheral blood Th17 and regulatory T (Treg) cells with disease activity in patients with systemic sclerosis (SSc).Methods This study recruited 21 patients with active SSc,24 patients with inactive SSc and 24 normal human controls with informed consent.Peripheral blood samples were obtained from these subjects.Flow cytometry was used to detect the percentages of Th17 and Treg cells in peripheral blood CD4+ cells,a fluorescence-based quantitative PCR to determine the levels of interleukin (IL)-17A,retinoid-related orphan receptor gamma t (RoRγt),forkhead box P3 (FoxP3) mRNA in peripheral blood mononuclear cells (PMBCs),and enzyme linked immunosorbent assay to measure the serum level of IL-17.Results Increased percentage of Th17 cells in peripheral blood CD4+ cells was observed in patients with active SSc compared with those with inactive SSc and normal human controls (2.34％ ± 1.19％vs.0.68％ ± 0.39％ and 0.57％ ± 0.49％,respectively,both P ＜ 0.05).No statistical difference was noted in the percentage of Treg cells in CD4+ cells or the mRNA expression levels of FoxP3 between the patients with active SSc,inactive SSc and normal human controls (all P ＞ 0.05).There was a significant increase in the mRNA expression of IL-17A,RoRγt in PBMCs and serum levels of IL-17 in patients with active SSc compared with patients with inactive SSc and normal human controls ( 11.73 ± 0.80 vs.9.77 ± 1.30 and 10.79 ± 0.74,respectively,both P ＜ 0.05; 18.48 ± 1.09 vs.15.89 ± 1.48 and 17.77 ± 1.64,respectively,both P ＜ 0.05; 53.60± 9.90 pg/ml vs.15.18 ± 3.24 pg/ml and 15.53 ± 4.12 pg/ml,respectively,both P ＜ 0.05).The percentage of Th17 cells in CD4+ cells and serum IL-17 levels were both positively correlated with disease activity in patients with active SSc (r =0.675,0.644,respectively,both P ＜ 0.05).Conclusions Th17 cells are highly proliferative in patients with active SSc,which may be closely correlated with the activity of SSc.
Meta-analysis identifies common gut microbiota associated with multiple sclerosis

Abstract:Background: Previous studies have identified a diverse group of microbial taxa that differ between patients with multiple sclerosis (MS) and the healthy population. However, interpreting findings on MS-associated microbiota is challenging, as there is no true consensus. It is unclear whether there is gut microbiota commonly altered in MS across studies. Methods: To answer this, we performed a meta-analysis based on the 16S rRNA gene sequencing data from seven geographically and technically diverse studies comprising a total of 524 adult subjects (257 MS and 267 healthy controls). Analysis was conducted for each individual study after reprocessing the data and also by combining all data together. The blocked Wilcoxon rank-sum test and linear mixed-effects regression were used to identify differences in microbial composition and diversity between MS and healthy controls. Network analysis was conducted to identify bacterial correlations. A leave-one-out sensitivity analysis was performed to ensure the robustness of the findings. Results: The microbiome community structure was significantly different between studies. Re-analysis of data from individual studies revealed a lower relative abundance of Prevotella in MS across studies, compared to controls. Meta-analysis found that although alpha and beta diversity did not differ between MS and controls, a higher abundance of Actinomyces and a lower abundance of Faecalibacterium were reproducibly associated with MS. Additionally, network analysis revealed that the recognized negative Bacteroides-Prevotella correlation in controls was disrupted in patients with MS. Conclusions: Our meta-analysis identified common gut microbiota associated with MS across geographically and technically diverse studies.
Study on memory T cell subsets in peripheral blood of patients with multiple sclerosis

Abstract:Objective:To explored the mechanisms that lead to the changes of the memory T cell subsets.Methods:By using of flow cytometry and enzyme-linked immunosorbent assay(ELISA), we detected the percentage of memory T cell subsets and plasma concentration of interleukin-15(IL-15) in peripheral blood of MS patients and healthy individuals. Furthermore, MS patients were subdivided into both relapse-remission MS(RRMS) and chronic progressive MS(CPMS) group according to the clinical features.Results:Compared to healthy controls, there was an increase and decrease in CD8~+T_ CM and terminal effector memory CD8~+T cell in MS patients(P<0.05 and P<0.01). After the clinical subdivision, the RRMS patients showed an increased terminal effector memory CD8~+T cell compared with healthy controls(P<0.05). In addition, MS patients showed a higher level of plasma IL-15 than those of control group(P<0.05).Conclusion:The upregulated CD8~+T_ CM in MS patients may reflect a persistent chronic inflammatory response that may have been induced during early stages of the disease. We speculate that this derangement may play an important role in the maintenance of chronic inflammation, while IL-15 may participate in the immunoregulatory process of promoting T_ CM differentiation.
Altered Plasma Metabolic Profiles in Chinese Patients With Multiple Sclerosis

Abstract:Multiple sclerosis (MS) is an autoimmune disease that leads to the demyelination of nerve axons. An increasing number of studies suggest that patients with MS exhibit altered metabolic profiles, which might contribute to the course of MS. However, the alteration of metabolic profiles in Chinese patients with MS and their potential roles in regulating the immune system remain elusive. In this study, we performed a global untargeted metabolomics approach in plasma samples from 22 MS-affected Chinese patients and 21 healthy subjects. A total of 42 differentially abundant metabolites (DAMs) belonging to amino acids, lipids, and carbohydrates were identified in the plasma of MS patients and compared with those in healthy controls. We observed an evident reduction in the levels of amino acids, such as L-tyrosine, L-isoleucine, and L-tryptophan, whereas there was a great increase in the levels of L-glutamic acid and L-valine in MS-affected patients. The levels of lipid and carbohydrate metabolites, such as sphingosine 1-phosphate and myo-inositol, were also reduced in patients with MS. In addition, the concentrations of proinflammatory cytokines, such as IL-17 and TNF-α, were significantly increased, whereas those of several anti-inflammatory cytokines and chemokines, such as IL-1ra, IL-7, and MIP-1α, were distinctly reduced in the plasma of MS patients compared with those in healthy subjects. Interestingly, some DAMs, such as L-tryptophan and sphingosine 1-phosphate, showed an evident negative correlation with changes in the level of TNF-α and IL-17, while tightly positively correlating with altered concentrations of anti-inflammatory cytokines and chemokines, such as MIP-1α and RANTES. Our results revealed that altered metabolomic profiles might contribute to the pathogenesis and course of MS disease by modulating immuno-inflammatory responses in the peripheral system, which is essential for eliciting autoimmune responses in the central nervous system, thus resulting in the progression of MS. This study provides potential clues for developing therapeutic strategies for MS in the near future.

Abstract:Background: Since September 2012, when teriflunomide was approved as a disease-modifying treatment for relapsing multiple sclerosis, real-world observational studies on teriflunomide in Chinese patients are limited. Methods: We collected demographic characteristics and peripheral blood samples at different time points. Clinical symptoms, magnetic resonance imaging data, and concentrations of neurofilament light chains and multiple cytokines at different time points were compared to assess the efficacy. Moreover, the safety was assessed by blood routine, liver and kidney function, and a questionnaire to report adverse reactions. Results: Teriflunomide significantly reduced serum levels of neurofilament light chains and several inflammatory cytokines. After accepting teriflunomide treatment, many clinical symptoms improved, scores of the expanded disability status scale decreased from 2.0 to 1.75, and annualized relapse rates decreased from 1.45 to 0.31. 29 (80.56%) and 15 (78.95%) patients achieved the no evidence of disease activity-3 status after 6 months and 12 months treatment, respectively. Teriflunomide was associated with mild or moderate discomfort, and discontinuation rates due to adverse events were low. Conclusion: Serum neurofilament light chain protein is sensitive to teriflunomide treatment, suggesting that it has the potential to be used as an indicator to assess the efficacy of teriflunomide. Teriflunomide can significantly reduce the concentrations of inflammatory cytokines, indicating that teriflunomide may regulate neuroinflammation through the inhibitory effect on a variety of immune cells and their cytokines. Teriflunomide can improve clinical symptoms and disease severity in MS patients in southern China, and patients have good compliance.

Abstract:IL-17–producing Th17 cells have gradually become considered as key factors in the pathogenesis of many autoimmune diseases, including multiple sclerosis (MS). Although the involvement of certain microRNAs in the development of MS has been reported, their role in Th17-driven autoimmunity is still poorly understood. In this study, we identified microRNA (miR)-15b as an important factor in Th17-associated effects and determined that the expression of miR-15b is significantly downregulated in MS patients and in mice with experimental autoimmune encephalomyelitis. Overexpression of miR-15b alleviated experimental autoimmune encephalomyelitis, whereas knockdown of miR-15b aggravated it. We demonstrated that miR-15b suppressed Th17 differentiation both in vivo and in vitro. We also found that O-linked N-acetylglucosamine transferase is a potential target of miR-15b, enabling it to affect the transcriptional regulation of retinoic acid–related orphan receptor γT through O-linked N-acetylglucosamine glycosylation of NF-κB. These results contribute to the importance of miR-15b in Th17 differentiation and the pathogenesis of MS.

Abstract:Objective: Multiple Sclerosis (MS) is an autoimmune disorder characterized by demyelination occurring within the white matter of the central nervous system. While its pathogenesis is intricately linked with the body's immune response, the precise underlying mechanisms remain elusive. This study aims to explore potential immune-related genes associated with MS and assess the causal relationship between these genes and the risk of developing MS. Methods: We retrieved expression datasets of peripheral blood mononuclear cells from MS patients from the Gene Expression Omnibus (GEO) database. Immune-related differentially expressed genes (IM-DEGs) were identified using the ImmPort database. GO and KEGG analyses were subsequently performed to elucidate the functions and pathways associated with the IM-DEGs. To visualize protein–protein interactions (PPIs), we used STRING, Cytoscape, and Cytohubba to construct networks of PPIs and hub genes. The diagnostic efficacy of hub genes was assessed using the nomogram model and ROC curve. The correlation of these hub genes was further validated in the mouse EAE model using quantitative PCR (qPCR). Finally, Mendelian randomization (MR) was performed to ascertain the causal impact of hub genes on MS. Results: Twenty-eight IM-DEGs were selected from the intersection of DEGs and immune genes. These genes are involved mainly in antigen receptor-mediated signaling pathways, B cell differentiation, B cell proliferation, and B cell receptor signaling pathways. Using Cytoscape software for analysis, the top 10 genes with the highest scores were identified as PTPRC, CD19, CXCL8, CD79A, IL7, CR2, CD22, BLNK, LCN2, and LTF. Five hub genes (PTPRC, CD19, CXCL8, CD79A, and IL7) are considered to have strong diagnostic potential. In the qPCR validation, the relative expression of these five genes showed significant differences between the control and EAE groups, indicating that these genes may play a potential role in the pathogenesis of MS. The MR results indicate that elevated levels of CD79A (OR = 1.106, 95% CI 1.002–1.222, p = 0.046) are causally positively associated with the risk of developing MS. Conclusion: This study integrated GEO data mining with MR to pinpoint pivotal immune genes linked to the onset of MS, thereby offering novel strategies for the treatment of MS.

Abstract:Multiple Sclerosis (MS) is a complex auto-inflammatory disease affecting the brain and spinal cord, which results in axonal de-myelination and symptoms including fatigue, pain, and difficulties with vision and mobility. The involvement of the immune system in the pathology of MS is well established, particularly the adaptive T cell response, and there has been a particular focus on the IL-17-producing subset of Th17 cells and their role in driving disease. However, the importance of innate immune cells has not been so well characterised. Here we focused on neutrophils, which are innate immune cells and rapid responders to inflammation, and which have recently been linked to other chronic autoimmune conditions. Multiple strands of evidence in patients with MS and in mice with the experimental autoimmune encephalomyelitis MS model suggest neutrophils may play a role in driving MS inflammation. Here, we performed proteomic analysis on neutrophils from patients with MS and healthy donors, revealing striking differences. In particular, granule proteins were significantly more abundant in the MS neutrophils compared to the healthy controls, with a particular over-abundance of proteins in primary and secondary granules. In addition, members of the MAVS signalling pathway were differently regulated compared to healthy donor cells. Finally, we find that MS neutrophils do not suppress T cell activation equivalently to healthy neutrophils, and in particular are unable to suppress expression of CD161 on the T cells, indicative of Th17 differentiation. We propose that neutrophil dysregulation in MS may contribute to dysfunctional T cell responses.

Abstract:Objective To investigate the expression and significance of intracellular interleukin(IL)-13 and IL-5 in peripheral blood CD4+ and CD8+ T cells from patients with multiple sclerosis(MS).Methods The intracellular production of IL-13 and IL-5 in peripheral blood CD4+ and CD8+ T cells was examined by flow cytometry from 23 patients with MS and 20 healthy subjects as control.Results Patients with MS showed a significantly higher percentage of CD4+IL-13+ and CD8+IL-13+cells than that in the healthy control group(P<0.05).The patients with MS during the relapse phase also showed a significantly higher percentage of CD4+IL-13+ and CD8+IL-13+cells than those during the remission phase(P<0.05).However, CD4+IL-5+ and CD8+IL-5+ T cells did not vary between the healthy control group and patients with MS during the relapse phase and the remission phase (P>0.05).Relationship analysis indicated that CD4+IL-13+ and CD8+IL-13+cells were correlated positively with MS(r=0.803,r=0.805,respectively,P<0.05),but CD4+IL-5+ and CD8+IL-5+ cells had no correlation with MS(r=0.361,r=0.349,respectively,P>0.05).Conclusion Certain kinds of Th2 and Tc2 cytokines are associated with both the onset and the clinical phase of MS.The relapse of MS is related to IL-13,but not to IL-5 secreted by Th2 and Tc2.
