Abstract:Summary Background Agenesis of third molar agenesis has a higher incidence than other tooth development anomalies. Previous research identified a potential correlation between third molar agenesis and specific craniofacial morphology; however, no systematic review and meta-analysis on this topic currently exists. Objective The objective of this systematic review and meta-analysis was to evaluate the association between third molar agenesis and craniofacial sagittal and vertical morphology. Search methods An electronic search was conducted on PubMed, Embase, Web of Science, and the Cochrane Library without restrictions on publication year or language; this was supplemented by the manual retrieval of relevant literature. Selection criteria Cross-sectional studies that compared craniofacial morphology using angular and linear measurements obtained from lateral cephalography between patients with third molar agenesis and those without were included. Data collection and analysis The quality assessment of the enrolled articles was evaluated by the Joanna Briggs Institute critical appraisal tool. Meta-analysis and sensitivity analysis were performed by Review Manager software (The Cochrane Collaborative, version 5.4, Cochrane IMS). Results A total of seven studies were included. Meta-analysis demonstrated that the ANB (mean differences (MD) = –0.75, 95% CI: –1.31 to –0.19, P < 0.01), palate length (ANS–PNS, MD = –1.68, 95% CI: –2.24 to –1.11, P < 0.01), and mandibular length (Go-Pog, MD = –0.36, 95% CI: –0.59 to –0.13, P < 0.01) were smaller in patients with third molar agenesis. With regard to vertical craniofacial morphology, the mandibular plane angle (MP-FH; MD = –1.88, 95% CI: –3.45 to –0.31, P = 0.02), gonial angle (gonial angle; MD = –1.73, 95% CI: –2.69 to –0.77, P < 0.01) and lower face height (lower face heigh angle; MD = –1.36, 95% CI: –1.94 to –0.77, P < 0.01) were smaller in patients with third molar agenesis, indicating a flatter or brachyfacial skeletal pattern. Conclusions The results of this study suggest that third molar agenesis maybe associated with a reduced maxillary length and a flatter mandible. However, these findings need to be interpreted with caution due to inconsistencies in the certainty of evidence. Clinical Trial Registration PROSPERO (CRD42023448226).

Abstract:Abstract Objective The purpose of this study was to find the prevalence of dental abnormalities in patients with third molar agenesis, and a comparison was made for the prevalence of those anomalies within the north Indian natives. Materials and Methods Pretreatment records of 518 patients were collected. Out of 518 patients whose records were analyzed, 472 patient records were selected on the basis of inclusion and exclusion standards. Panoramic radiographs and dental casts were analyzed in patients who had an absence of at least one third molar in which absence of any other tooth, presence of supernumerary teeth, presence of impacted canines, and peg-shaped lateral incisors were further assessed. The prevalence of these anomalies was compared with a randomly selected group of patients with presence of third molars that served as controls. Statistical analysis was done using the chi-square test (p < 0.05) and odds ratio. Results The incidence of third molar nondevelopment was found to be 19.2%. The presence of other anomalies was 39.5% in patients with third molar nondevelopment, and difference was statistically significant. The occurrence of impacted canines in patients with absence of third molars was 22%, and presence of third molars was 6.6%. The presence of other dental anomalies, that is, supernumerary teeth and tooth agenesis, was 7.7% in the agenesis group, which was also higher than that in the control group (3.3%) but was not statistically significant. The incidence of peg-shaped lateral incisors was similar in both the groups. Conclusion Impacted canines and total dental anomalies are more often related with absence of third molars than with the presence of third molars.
Studies of the Chronological Course of Third Molars Eruption in a Northern Chinese Population.

Abstract:Dental age estimation is of great importance for individual identification in forensic medicine and many other fields of study. Among them, tooth eruption is a parameter developmental morphology that can be determined by clinical examinations or by dental X-rays. The purpose of present research is to study the chronological course of third molars eruption in a Chinese population and compare that with other ethnic population for age estimation. A total of 1135 conventional orthopantomograms from 506 male and 629 female northern Chinese subjects aged between 11 and 26 years were analyzed. The eruption status of the third molars was assessed using the developmental stages described by Olze et al. Results showed that the third molars 18, 28, 38 and 48 in the stage A showed significant younger average age in males than in females. The Olze's stage A could be used as a reference stage to determine whether a male or female northern Chinese is likely to be equal or above age 16, with 99.6-100% and 97.4-98.1% of correct predictions, respectively. The stage D was found to be a useful marker for diagnosing age under 16 years, with 98.9-100% and 100% of correct predictions in males and females, respectively. There were some significant differences of the chronological course of the third molars eruption in different ethnic groups, which indicated that population-specific standards could enhance the accuracy of forensic age estimation based on third molar eruption.

Abstract:Dental age estimation is of great importance for individual identification in forensic medicine and many other fields. The present study aimed to provide reference data for chronological aging based on mineralization of third molars in a northern Chinese population. A total of 3212 digital orthopantomograms from 1551 males and 1661 females of northern Chinese origin ranging in age from 5 to 25 years were assessed. The mineralization status of the third molars was assessed using the formation stages described by Demirjian et al. with two modifications. Results showed that left and right third molars were generally at similar stages of mineralization. The third molars 18 at stage D, 28 at stage D, 38 at stage D and 48 at stages B and G showed significantly lower mean ages in males than in females. In comparison to different ethnic groups there were some significant differences in the chronological course of third molar mineralization. As possible causes of these differences, ancestry, socioeconomic status, eating habits and statistical phenomena, such as age range and age distribution of samples are discussed.
Study on the Relationship Between the Third Molar and Malocclusion

Abstract:Objective To study the relationship between the third molar and malocclusion. Methods It was studied panoramic radiograph,cephalogram and record model of 350 patients (male 156, female194) from the orthodontic whose ages were 12～18 yearsdd. Results There was significant difference between the congenitally missing M3 and the crowding(P0.05). And the crowding was the most in malocclusions. There was significant difference between the congenitally missing M3 and the spacing (P0.05). There was no significant difference between the congenitally missing M3 and every skeletal classification(P0.05). But the patients of class Ⅲ were more congenitally missing upper M3. The patients of class Ⅱ were more congenitally missing lower M3. The ratio of classⅠ、Ⅱ、Ⅲ was in 58.3%、26.6%、15.1%. Conclusion The congenitally missing M3 was more in the spacing malocclusion than the other malocclusion. The crowding malocclusion was more in presence M3 than the congenitally missing M3. There were correction between class Ⅱ/Ⅲ and congenitally missing M3 in lower/upper.

Abstract:Objective To study the relation tetween the third molardevelopment and skeletal maturity.Methods The material for the present study,consisted of 589 panoramic radiographs accompanied with 589 left hand-wrist radiographs (aged-8～15).The size of samples was determined by staticstivians.Results There are low correlations between the level of skeletal maturation and the third molar development (male:r=0.3719,female:r=0.3958).Conclusion The result suggests that when we study the relationship between the dental age and the steletal maturity,we should not includ the third molar.
Relationship between Molar Dentoalveolar height and Vertical Growth Patterns in Normal Occlusion

Abstract:Objective: The aim of this study was to investigate the relationship between molar dentoalveolar height and vertical growth patterns in normal occlusion. Methods: The cephalometric radiographs of 100 Guang Dong persons with normal occlusion were measured, the relationship between molar dentoalveolar height and six angle measurements, three facial height measurements were analysed by statistic methods. Results: Correlation coefficients showed that the molar dentoalveolar heights were positively related to anterior facial height, anterior lower facial height, posterior facial height, and negatively related to the angle of SN-MP, ODI. Mandibular molar dentoalveolar height showed positive correlation with the angle of ANB and negative correlation with APDI. Conclusion: The molar dentoalveolar height is closely related with the craniofacial height. This relationship should not be ignored in the planning of orthodontics.

Abstract:Objective:To investigate the relationship between the structure of TMJ and the craniofacial pattern,occlusion in ClassⅡ1mandibular retrusion children. Methods:TMJ MRI,lateral cephalometric were taken respectively from 30 ClassⅡ1 patients (14 boys and 16 girls,the average age was 10.8±1.1 years). The condyle position and disc position were measured. The correlation of condyle and disc position with craniofacial pattern and occlusion were analyzed metrically and compared with pearson test.Results:There were low positive correlation between MP-SN angle and anterial space. No correlation between posterior-anterior ratio and other data were found. The posterior facial height and Dp-Cc/Cs-Cc angle had low positive correlation. The anterior facial height and Dp-Cc/Cs-Cc、Ca-Ca'、Ca-Cp showed low negative correlation,and little correlation was found between condyle position and overbite and overjet. Overbite had no correlation with disc-condyle-fossa relationship. However,The positive correlation was found between the overjet. and Ca-Dm and Dp-Cc/Cs-Cc,and Pearson coefficient is 0.420 and 0.460.Conclusion:Vertical craniofacial patter has relation to forward displacement of condyle and backward displacement of disc. A deeper overjet is related with a higher risk of abnormal disc-condyle-fossa relationship.
Retrospective study of 345 cases of young permanent molar development

Abstract:Objective To analyze the imaging manifestations of second molar( M2) and third molar( M3) in children aged 11-14 in Changzhou City,and to explore the relationship of development between M3 and M2. Methods A total of 345 adolescents with 11-14 years old,of Changzhou Stomatological Hospital in January-October,2013 were selected as the imaging database. The M3 and M2 on the panoramic radiographs were observed for the development degree in 345 cases. And then the correlation analysis was done. Results In the all adolescents,43. 26% of M2 was in stage G. The development of M2 appeared earlier in the mandibular than in the maxillary( Z =- 11. 69,P 0. 001) and earlier in female than in male( Z =- 3. 06,P = 0. 002); 21 teeth were impacted and tilted in the mandibular,it was 3. 04% of the mandibular M2. 33. 26% of M3 was in stage C. The development of M3 appeared earlier in female than male( Z =- 2. 06,P = 0. 040),and earlier in the mandibular than in the maxillary( Z =- 3. 42,P = 0. 001). M3 and M2 had positive correlation when calculated by Spearman correlation coefficient( r = 0. 869,P 0. 001). The difference was statistically significant. Conclusions M2 and M3 in adolescents was closely related to the development. It was necessary to consider fully the relationship between the two in orthodontic treatment.
Evolutionary trends in human mandibles and dentition from Neolithic to current Chinese

Abstract:Objectives: This study aimed to systematically compare Neolithic mandibles and dentition with modern Chinese, and thereby discern human evolutionary trends. Materials and methods: Neolithic remains of 45 adults unearthed at the Zhangqiu Jiaojia site, were compared with clinical records of 48 patients at Shandong University. All samples were scanned by cone beam computed tomography (CBCT) using identical parameters. Digital imaging and communications in medicine images were collected, three-dimensional models reconstructed, and morphology measurements obtained using Mimics software. Results: Neolithic mandibles were significantly larger in their vertical and sagittal dimensions (P < .05), but similar in horizontal width to modern humans. Their condyles had fewer bird beak and crooked finger shaped morphologies than modern mandibles (P < .05). Neolithic third molars were more often erupted than in modern mandibles, and their Position A, class I and II, and vertical impactions were more common (P < .05). Neolithic teeth were generally smaller in crown lengths and in cross-sectional areas, than their modern counterparts (P < .05). Conclusions: Neolithic mandibles were larger than modern humans, who have refined diets and mandibular atrophy. They had fewer abnormally shaped condylar morphologies, and much fewer third molar impactions than in modern humans. However, modern dentition particularly their crowns are larger, likely through genetic influx from migrations.
Component analysis of sagittal craniofacial morphology in Class Ⅲ malocclusion patients in Xi'an area

Abstract:Objective To analyze the sagittal craniofacial morphology composition in Class Ⅲ patients in Xi'an area.Methods The craniofacial type of the Class Ⅲ malocclusion in 1177 patients were analyzed.The features of Class Ⅲ malocclusion patients in mixed dentition,early permanent dentition and permanent dentition were studied by computer-aided cephalometric X-ray.Then the component analysis of sagittal craniofacial morphology was performed.Results In the mixed dentition,the craniofacial types in sagittal directions mainly manifested as normal maxilla with mandiblar hyperplasia followed by normal mandible with maxillary hypoplasia.Craniofacial types in sagittal directions were normal maxilla and mandible or normal maxilla with mandibular hyperplasia in early permanent dentition.In permanent dentition,craniofacial types in sagittal directions mainly showed normal maxilla with mandibular hyperplasia followed by maxillary hypoplasia with mandibular hyperplasia.Conclusions Class Ⅲ patient has different sagittal features in different ages,but mandibular hyperplasia is still the main manifestation.
Morphological Influence of the Mandibular Third Molar Extraction to the Angulusmandibulae Using Cone-Beam Computed Tomography

Abstract:Objective: To explore the influence of the mandibular third molar extraction on the morphological changes of gonia angle.Methods: 30 adult orthodontic patients whose mandibular third molars need to be extracted for treatment had taken CBCT imaging before the teeth extracted,then 6 months later all the patients received the second CBCT scanning.We measured the width,height and length of the alveolar bone of the mandibular third molar through Mimics 10.01 software and compared the data using SPSS18.0 statisti-cal software.Results: After the extraction of the mandibular third molar,the width and height of alveolar bone located in the gonial angle reduced,and the difference had statistical significance,while the length had no conspicuous change.Conclusion: Extraction of mandibular third molar can reduce the width and height of angulusmandibulae bone to some extent,but the saggital length has no significant changes.

Abstract:Non-syndromic permanent tooth agenesis affects a significant proportion of the population, especially if third molars are considered. Although tooth agenesis has been linked to a smaller craniofacial size, reduced facial convexity and a shorter skeletal face, the occlusal characteristics of individuals with tooth agenesis remain largely unexplored. Therefore, this study investigated potential associations between tooth agenesis and metric occlusal traits in 806 individuals (491 with 4.1 missing teeth per subject, including third molars, and 315 without any tooth agenesis). Dentoskeletal morphology was defined through anatomical landmarks on pre-treatment cephalometric radiographs. Multivariate regression models, adjusted for sex and age, showed that tooth agenesis was significantly associated with a reduced overjet, an increased interincisal angle, and shorter upper and lower dental arch lengths, but not with overbite. Moreover, apart from reduced tooth length and dentoalveolar effects, as the number of missing teeth increased the upper front teeth were progressively retruded according to the craniofacial complex and to the face. Thus, tooth agenesis has a substantial influence on dental and occlusal characteristics, as well as on the sagittal position and inclination of anterior teeth. These findings emphasize the necessity for personalized, multidisciplinary approaches in individuals with multiple agenesis to successfully meet treatment goals.

THE STUDY OF THE RELATIONSHIP BETWEEN CRANIAL BASE AND ANGLE CLASS II AND III MALOCCLUSION

Abstract:Objective:To investigate the relationship between cranial base structure and maxillofacial morphology of malocclusion.Methods:Sixty-four patients aged from 12 to 14,were divided into 2 groups,namely Angle Class Ⅱ and Ⅲ.All the cephalographs were traced,then digitized and statisticed using a computer.Results:The cranial base angle of class Ⅱ was significantly larger than that of class Ⅲ,and was negatively correlated with articulare angle,gonion angle,and positively correlated with SN-palatal angle.Anterior cranial base length was positively correlated with facial length.Posterior cranial base length was postitvely correlated with mandibular body length,facial length,upper facial length,palatal breadth,and negatively correlated with lower facial length.Conclusion:The structure of cranial base is in close relation with the maxilloficial morphology.The larger the cranial base angle,the closer class Ⅱ face pattern is.Conversely the smaller the cranial base angle,the closer class Ⅲ face pattern is.Posterior cranial base has more effects on maxillofacial morphology than anterior cranial base.
Relationship Between Root Morphology of the Mandibular Third Molars and Crowding in the Posterior Segment

Abstract:Objective To investigate the root morphology of the mandibular molars and the relationship between root morphology and crowding in the posterior segment.Methods One thousand and fifty-six dental panoramic radiographs were evaluated.The age range of the patients when the X-rays were taken was from 22 to 35 years.The roots of the mandibular first,second and third molars were evaluated.The relationship between root morphology of the mandibular molars and crowding in the posterior segment was assessed.Results The root tended to become convergent from mandibular first to third molar.No significant differences were found in third molar root morphology between impacted molars and normally positioned ones.Conclusions The root morphology of the mandibular third molars was variable.No significant correlation was found between root morphology and crowding in the posterior segment.
Study of Cranioficial Structure by Anteroposterior Cephalometric Analysis in Skeletal Class Ⅲ Patients

Abstract:Objective To investigate the characteristics of cranioficial bone structure on anteroposterior cephalograms in skeletal Class Ⅲ patients.Methods Posteroanterior cephalograms were taken for Fouty-One patients with skeletal class Ⅲ malocclusion.All posteroanterior cephalograms were hand-traced.Eight linear measurements and 12 ordinates of certain points on dentoalveolar structures,jaws and other craniofacial regions were abtained using a digital vernier caliper.One sample t-test was performed to determine the difference of the measurements between samples and subjects with normal occlusion. Results Compared with normal values,the sample of this study showed a wider mid-face width and a narrower lower face width(P0.01].The ramus height and the distance from the most superior points of condyles to Me point were larger than norms.The ordinates of point Sc,Cdl,Me and CMDI were different significantly from normal values.Conclusion There were significant differences in crainoficial width proportion and ordinates of mandible and lower teeth between the sample and the subjects with normal occlusion.
Craniofacial Asymmetry Analysis in Adolescents with Skeletal Class Ⅲ Malocclusion-a 3D CT Study

Abstract:Objective:To explore craniofacial asymmetric characteristics among the nonsyndromic skeletal Angle Class III adolescent patients based on 3-dimensional model reconstruction and measurements by CT data for early diagnosis,treatment and prediction in these children.Methods:20 patients with skeletal class III malocclusion aged 7-21 years were selected.Craniofacial asymmetry was examined in 3D using CT.We undertook image reconstruction and volumetric measurements(21 indices) by scanning DICOM images into Mimics software.Statistical significance was assessed by paired-samples t test.Results:The differences between the left and right structures were statistically significant in the linear measurements of zygomatic arch,jugal bone,and zygomaticofrontal suture toward sagittal plane.And all these three values of the right sides were bigger than those of the left sides.Conclusion:As to the skeletal Class III individuals in this study,some anatomy structures around jugal bone on the right side are located significantly farther to the sagittal plane than those on the left side.It's feasible to employ Mimics to quantify craniofacial morphology and displacement with growth by using 3D CT images.
3-Dimensional Craniofacial Asymmetry Analysis in Adolescence with Skeletal Class Ⅲ Malocclusion

Abstract:Objective:To evaluate craniofacial asymmetry based on 3-dimensional model reconstruction of CT data for early diagnoses,treatment and prediction in skeletal class Ⅲ children.Method:19 patients with skeletal class Ⅲ malocclusion aged 7～16 years were selected.3-dimensional CT examination was done on them respectively and then the data were processed by the software Mimics 10.01 to reconstruct images and to analyze asymmetry by 21 indices.Statistical significance was assessed by paired-samples t test.Result:Three indices,including AZ(ZA)-sagittal,J-sagittal,and Z-sagittal,showed statistical significances(P0.05) between right and left sides in the craniofacial structure.Conclusions:Mimics 10.01 made it facilitated to quantify the craniofacial asymmetry through CT data.Bone structures related to zygoma showed that the right side was prior to the left side in adolescences with skeletal class Ⅲ malocclusion.
Craniofacial Morphology of Chinese Patients with Bilateral Temporomandibular Joint Osteoarthrosis

Abstract:OBJECTIVE:To evaluate the morphological features of craniofacial structures in young patients affected by bilateral temporomandibular joint osteoarthrosis (TMJOA).METHODS:Forty-three males and 189 females aged 15 to 25 years who had lateral cephalograms and temporomandibular joint (TMJ) radiological examinations were included. They were classified into a TMJOA group and a control group, according to TMJ bony status. Thirty-two variables from lateral cephalogram were analysed by t test to evaluate the cranial and dentofacial differences between the two groups.RESULTS:TMJOA group showed a shorter posterior ramus height and shorter condyles, smaller SNB angle and larger ANB angle, smaller facial plane angle and larger angle of convexity, steeper mandibular plane angle and more vertical y-axis.CONCLUSION:Bilateral TMJOA is associated with dentofacial alterations characterised by a tendency toward retrognathism and shorter mandibles. However, the cause-and-effect relationship is yet to be clarified.
Component Analysis of Craniofacial Type of Xi'an Chinese with Class III Malocclusion in the Early Permanent Dentition

Abstract:Objective:To analyze craniofacial type composition of Class Ⅲ malocclusion in early permanent dentition in the Xi'an region. Methods:We performed a retrospective study on craniofacial type of the Class Ⅲ malocclusion patients admitted to the Department of Orthodontics,Stomatology Hospital,the Fourth Military Medical University,during 2003-2008. The computer-aided cephalometric X technology had been used and the features of class III malocclusion patients in early permanent dentition were studied. Results:In early permanent dentition,craniofacial types in sagittal directions were mainly manifested as maxillary and mandibular normal or maxillary normal with mandibular hyperplasia. Craniofacial types in vertical direction were mainly manifested as average angle,and seldom as low-angle and high-angle. The cross-analysis of sagittal and vertical craniofacial morphology was performed. Both maxillary and mandibular normal and maxillary normal with mandibular hyperplasia in average angle patients were predominant,and followed by maxillary hypoplasia with mandibular normal combined with average angle. Conclusion:It was demonstrated that class III malocclusion patient in early permanent dentition has either sagittal or vertical features. The component analysis can play a very important role in understanding the detailed features of class III malocclusion. And more important,it is helpful for the future study on the early treatment of class III malocclusion.
