Firefighter Health: A Narrative Review of Occupational Threats and Countermeasures

Abstract:Structural firefighters are responsible for protecting properties and saving lives during emergency operations. Despite efforts to prepare firefighters for these hazardous occupational demands, the unfortunate reality is that the incidence of health morbidities is increasing within the fire service. Specifically, cardiovascular disease, cancer, and mental health disorders are among the most documented morbidities in firefighters. Pubmed and Google Scholar search engines were used to identify peer-reviewed English language manuscripts that evaluated firefighters' occupational health threats, allostatic factors associated with their occurrence, and evidence-based strategies to mitigate their impact. This narrative review provides fire departments, practitioners, and researchers with evidence-based practices to enhance firefighters' health.

Specifically, the article focuses on the following aspects: 1. **Summarize Major Physical and Mental Health Issues**: Including the prevalence and impact of cardiovascular diseases, cancer, depression, and post-traumatic stress disorder (PTSD) among firefighters. 2. **Identify Stress Load Factors Related to Health Issues**: Investigate how physiological and psychological stress negatively impacts firefighters through stress response systems (such as the hypothalamic-pituitary-adrenal axis and the sympatho-adrenal-medullary axis). 3. **Evaluate Evidence-Based Strategies to Reduce Stress Load**: Propose evidence-based methods to mitigate occupational health threats to enhance the health and well-being of firefighters. ### Research Background - Firefighters face various dangers while performing their duties, leading to increasing issues with cardiovascular diseases, cancer, and mental health problems. - The article employs a literature review method, searching relevant English literature through databases like PubMed and Google Scholar, selecting studies from 1984 to 2024, and analyzing the health issues and coping strategies of firefighters. ### Key Findings - Cardiovascular diseases are one of the most common health threats among firefighters, especially sudden cardiac events (SCE). - There is a high rate of obesity among firefighters, which further increases the risk of cardiovascular diseases. - Clinical assessments and fitness tests (such as cardiopulmonary function tests) are recommended to evaluate the risk of cardiovascular diseases. - Regular physical activity and strength training are recommended to improve cardiovascular health. - Dietary interventions are also important countermeasures, with recommendations to adopt a Mediterranean diet or time-restricted eating to reduce oxidative stress and inflammatory responses. ### Conclusion This paper aims to provide evidence-based practice guidelines for fire departments, practitioners, and researchers to improve the health levels of firefighters.
