Calculating nursing workload based on patients’ severity of illness and activities of daily living

Abstract:Objective To establish a convenient,scientific and reasonable formula for calculating nursing workload.Methods The nursing time of 32 indirect nursing items were recorded for 24 hours in ten wards for one week using the principle of stratified sampling.Totally 152 direct nursing items in 5322 cases of patients were recorded,and the patients’ severity of illness and activities of daily living(ADL) were assessed.The times of direct nursing items within 24 hours among 1139 patients were recorded,and the patients’ severity of illness and ADL were assessed.Then,the direct and indirect nursing time within 24 hours among different levels of illness severity and ADL was calculated.A formula of nursing workload in normal wards within 24 hours was developed according to the nursing time among different levels of illness severity and ADL as well as the single nursing time of admission,discharge,transfer and surgical care.Results The daily direct and indirect nursing time in a ward equals to the summation of nursing time for different levels of patients(A-Ⅰ,A-Ⅱ,A-Ⅲ,A-Ⅳ,B-Ⅰ,B-Ⅱ,B-Ⅲ,B-Ⅳ,CⅠ,C-Ⅱ,C-Ⅲ-C-Ⅳ) multiplied by the number of different level patients respectively,as well as the summation of single nursing time of admission,discharge,transfer and surgical care multiplied by the number of patients in each single item respectively.Conclusions The formula for calculating nursing workload based on patients’ severity of illness and ADL as well as the nursing time on admission,discharge,transfer and surgical care has good representative and feasibility.
