Abstract:Hepatitis, cirrhosis and hepatocellular carcinoma caused by hepatitis B virus (HBV) infection has threatening human health seriously. As HBV is a kind of non-cytotoxic virus, host immune response plays a vital role in pathogenesis and clinical outcomes of hepatitis B. Multiple immune cells (e.g., including cytotoxic T lymphocytes, regulatory T cells, natural killer cells, dendritic cells) are important in the immune regulation of HBV infection. Therefore, focusing on the activation states of immune cells may provide new evidences and strategies for determining immune status of HBV infectors, monitoring progression of diseases and predicting efficacy of antiviral treatment. JAK/STAT Signaling is Involved in IL-35-induced Inhibition of Hepatitis B Virus Antigen-Specific Cytotoxic T Cell Exhaustion in Chronic Hepatitis B

Abstract:Aims: Interleukin-35 (IL-35) is a new member of the interleukin-12 family and is composed of the P35 and EB virus-inducible gene 3 subunits. The aims of this study were to examine the roles of IL-35 in the exhaustion of HBV-specific CTLs, as little as known on the subject. Main methods: The relative levels of serum HBV markers were detected using automated biochemical techniques. The HBV DNA copies were measured by RT-qPCR. The expression of inhibitory receptors and the cell cytokines on the surface of CTLs were determined by flow cytometry. The pSTAT1-pSTAT4 protein levels expression was determined by flow cytometry, confocal microscopy and Western blot. Key findings: Our results showed that IL-35 can activate the Janus kinase 1 (JAK1)/tyrosine kinase 2 (TYK2)/ signal transducer and activator of transcription 1 (STAT1)/STAT4 pathway in CTLs in vitro. Interferon-gamma and tumor necrosis alpha-a expression increased in CTLs in the presence of a JAK/STAT-pathway blocker. In addition, we evaluated the expression of the exhaustion-associated molecules programmed death-1, cytotoxic T lymphocyte-associated protein-4, and lymphocyte activation gene-3 in CTLs after adding the JAK-STAT inhibitor The results showed that the expression of exhaustion-associated molecules on the CTL surface decreased after blocking the JAK-STAT pathway. IL-35 inhibited the function of HBV-specific CTLs through the JAK1/TYK2/STAT1/STAT4 pathway, and the function of CTLs was recovered after blocking the JAK/STAT pathway. Significance: These data provide a new experimental basis for immunotherapy for chronic hepatitis B.
CXCL13-mediated Recruitment of Intrahepatic CXCR5+CD8+T Cells Favors Viral Control in Chronic HBV Infection.

Abstract:Background & Aims: Although CD8(+)T cell exhaustion hampers viral control during chronic HBV infection, the pool of CD8(+)T cells is phenotypically and functionally heterogeneous. Therefore, a specific subpopulation of CD8(+)T cells should be further investigated. This study aims to dissect a subset of CD8(+)T cells expressing C-X-C motif chemokine receptor 5 (CXCR5) in chronic HBV infection. Methods: The frequency of CXCR5(+)CD8(+)T cells and the levels of C-X-C motif chemokine ligand 13 (CXCL13), a chemokine of CXCR5, were measured in patients with chronic HBV infection. C57BL/6, interleukin (IL)-21 receptor- or B cell-deficient mice were hydrodynamically injected with pAAV-HBV1.2 plasmids. Phenotype and functions of peripheral and intrahepatic CXCR5(+) and CXCR5(-)CD8(+)T cells were assessed. Results: CXCR5(+)CD8(+)T cells were partially exhausted but possessed a stronger antiviral ability than the CXCR5(-) subset in patients with chronic HBV infection; moreover, CXCR5(+)CD8(+)T cells were associated with a favorable treatment response in patients with chronic hepatitis B (CHB). High levels of CXCL13 from patients with CHB facilitated the recruitment of intrahepatic CXCR5(+)CD8(+)T cells, and this subpopulation produced high levels of HBV-specific interferon (IFN)-gamma and IL-21. Notably, PD1 (programmed death 1) blockade and exogenous IL-21 enhanced the production of IFN-gamma. More strikingly, mice injected with CXCR5(+)CD8(+)T cells showed remarkably decreased expression of HBsAg. Additionally, an impaired production of HBV-specific IFN-gamma from intrahepatic CXCR5(+)CD8(+)T cells was observed in IL-21 receptor- or B cell-deficient mice. Conclusion: CXCL13 promotes the recruitment of CXCR5(+)CD8(+)T cells to the liver, and this subpopulation improves viral control in chronic HBV infection. The identification of this unique sub- population may contribute to a better understanding of CD8(+)T cell functions and provide a potential immunotherapeutic target in chronic HBV infection. Lay summary: Exhaustion of CD8(+)T cells is an important factor in the development of chronic hepatitis B virus (HBV) infection. CD8(+)T cells expressing the receptor CXCR5 are partially exhausted, but have potent antiviral activity, as they produce high levels of HBV-specific cytokines in chronic HBV infection. Increased expression of CXCL13 within the liver facilitates the recruitment of CXCR5(+)CD8(+)T cells and establishes effective immune control of HBV infection. Abstract:Chronic hepatitis B virus (HBV) infection remains a leading cause of hepatic inflammation and damage. The pathogenesis of chronic hepatitis B (CHB) infection is predominantly mediated by persistent intrahepatic immunopathology. With the characterization of unique anatomical and immunological structure, the liver is also deemed an immunological organ, which gives rise to massive cytokines and chemokines under pathogenesis conditions, having significant implications for the progression of HBV infection. The intrahepatic innate immune system is responsible for the formidable source of cytokines and chemokines, with the latter also derived from hepatic parenchymal cells. In addition, systemic cytokines and chemokines are disturbed along with the disease course. Since HBV is a stealth virus, persistent exposure to HBV-related antigens confers to immune exhaustion, whereby regulatory cells are recruited by intrahepatic chemokines and cytokines, including interleukin-10 and transforming growth factor β, are involved in such series of causal events. Although the considerable value of two types of available approved treatment, interferons and nucleos(t)ide analogues, effectively suppress HBV replication, neither of them is sufficient for optimal restoration of the immunological attrition state to win the battle of the functional or virological cure of CHB infection. Notably, cytokines and chemokines play a crucial role in regulating the immune response. They exert effects by directly acting on HBV or indirectly manipulating target immune cells. As such, specific cytokines and chemokines, with a potential possibility to serve as novel immunological interventions, combined with those that target the virus itself, seem to be promising prospects in curative CHB infection. Here, we systematically review the recent literature that elucidates cytokine and chemokine-mediated pathogenesis and immune exhaustion of HBV infection and their dynamics triggered by current mainstream anti-HBV therapy. The predictive value of disease progression or control and the immunotherapies target of specific major cytokines and chemokines in CHB infection will also be delineated.

Abstract:Patients with chronic HBV infection show poor immune response to HBV-specific CD8+ T cells. Several studies demonstrate that regulatory T cells (Treg) and dendritic cells (DC) are important to maintain peripheral immune tolerance. In this study, we investigated the effects of CD4+CD25+Treg and/or the adherent cells (AC) on the proliferation of HBc18-27-specific CD8+ T cells (c18-27-CD8Ts) in response to in vitro stimulation. The frequency of c18-27-CD8Ts in four different mixed leukocyte reactions (MLRs) were analyzed using an HLA-A2–HBc18-27 tetramer. The data indicated that the median percentage of c18-27-CD8Ts in four different MLRs were significant difference in patients with chronic HBV infection. Our results showed that Treg and/or AC might suppress the frequency of HBc18-27-specific CD8+ T cell proliferation in response to in vitro stimulation in chronic HBV patients, and AC might be more effective than Treg.
Interleukin 21 Reinvigorates The Antiviral Activity Of Hepatitis B Virus (Hbv)-Specific Cd8(+) T Cells In Chronic Hbv Infection

Abstract:Background Strategies that target functional recovery of exhausted hepatitis B virus (HBV)-specific CD8(+) T cells are beneficial for viral control, but the potential for interleukin 21 (IL-21) to rescue CD8(+) T-cell function is not well understood.Methods We investigated the effect of IL-21 on CD8(+) T-cell responses by phenotypic and functional analysis of samples from patients with chronic HBV infection and a mouse model with HBV expression.Results IL-21 promoted the proliferative capacity of HBV-specific CD8(+) T cells and down-regulated expression of the inhibitory receptors programmed death 1 and T-cell immunoglobulin domain and mucin domain 3. Additionally, IL-21 boosted the production of interferon-, granzyme B, and CD107a in HBV-specific CD8(+) T cells and enhanced the cytolytic activity of CD8(+) T cells against HepG2.2.15 cells. Notably, an HBV mouse model established from IL-21 receptor knockout mice showed significantly decreased frequency of HBV-specific CD8(+) T cells and increased levels of serum hepatitis B surface antigen (HBsAg). Meanwhile, administration of recombinant mouse IL-21 in an HBV mouse model established from wild-type mice resulted in enhanced functionality of HBV-specific CD8(+) T cells and accelerated HBsAg clearance.Conclusions IL-21 enhances the antiviral effect of HBV-specific CD8(+) T cells, suggesting that it may contribute to viral clearance in chronic HBV infection.Interleukin-21 is highly expressed in chronic hepatitis B patients with favorable treatment response, and it can reinvigorate antiviral activity of hepatitis B virus (HBV)-specific CD8(+) T cells by down-regulating expression of inhibitory receptors and boosting production of antiviral cytokines.
Changes of Costimulatory Molecule CD28 on Circulating CD8+ T Cells Correlate with Disease Pathogenesis of Chronic Hepatitis B.

Abstract:Costimulatory signals are critical for antiviral immunity. The aim of this study was to evaluate the change of costimulatory molecule CD28 on circulating CD8 + T cells in chronic hepatitis B patients (CHB). Seventy CHB patients and fifty-six healthy controls were included, and forty-eight CHB patients were recruited for 52 weeks of longitudinal investigation. The proportions of circulating CD8 + CD28 + and CD8 + CD28 − subpopulations were determined by flow cytometry, and the CD8 + CD28 + /CD8 + CD28 − T cells ratio was calculated. Compared with the subpopulation in healthy controls, high proportions of CD8 + CD28 − subpopulation were observed in CHB patients. Similarly, the CD8 + CD28 + /CD8 + CD28 − T cells ratio was significantly decreased in CHB patients compared with healthy controls and correlated significantly with hepatitis B virus (HBV) loads. High proportions of CD8 + CD28 − subpopulation and low CD8 + CD28 + /CD8 + CD28 − T cells ratio were observed in hepatitis B e antigen- (HBeAg-) positive individuals as compared with that in HBeAg-negative subjects. A significant decrease in CD8 + CD28 − subpopulation, increase in CD8 + CD28 + subpopulation, and CD8 + CD28 + /CD8 + CD28 − T cells ratio were seen in those patients who received efficient antiviral therapy. Thus, aberrant CD28 expression on circulating CD8 + T cells and the CD8 + CD28 + /CD8 + CD28 − T cells ratio reflect the dysregulation of T cell activation and are related to the pathogenesis of chronic HBV infection.
The function of circulating CD4+CD25+ regulatory T cells in patients with chronic hepatitis B

Abstract:Objective To investigate the induction and function of circulating CD4+ CD25+ regulatory T cells(CD4+CD25+ Treg cells)in patients with chronic hepatitis B,and their role in the progress of chronic HBV infection.Methods Twenty two patients with chronic hepatitis B(CHB),11 patients with acute hepatitis B(AHB)and 20 healthy blood donators were enrolled in the study.After stimulating with recombinant HBV antigen or anti-CD3 mAb,the fraction of CD4+ CD25+Treg cells in peripheral blood mononuclear cells(PBMC)from different subjects was analyzed using flow cytometry.CD4+CD25+ Treg cells and effector cells(PBMC-Treg)were sorted out and cultured alone or together in vitro,and the cell proliferation and cytokines production were measured. Serum HBV titers,HBV markers and liver function were also evaluated.Results The fraction of circulating CD4+CD25+ Treg cells induced in PBMC from CHB patients under the stimulation of recombinant HBsAg was much higher than that in AHB patients(F=3.47,P＜0.05)and normal controls(F=4.98,P＜0.05).With HBsAg stimulation,cell proliferation and cytokines production (IFN-γand IL-10)of CD4+ CD25+ Treg cells either from patients or from normal controls was very low when cultured alone,and there was no significant difference between two groups(P＞0.05).Cell proliferation and IFN-γproduction of effector cells from AHB patients were higher than those from CHB patients,however,when effector cells were cultured with CD4+Cd25+Treg cells at a ratio of 5:1,the cell proliferation and IFN-γ production were significantly inhibited (F=4.66,5.24,P＜0.05).Conclusion Circulating nificantly suppress the production and IFN-γ production of effector T Cells,which may be associated with the progress of chronic HBV infection.
Effects of Regulatory T Cells on Anti-Viral Immune Responses Impaired by Chronic Hepatitis B

Abstract:AIM: To evaluate the proportion of CD4(superscript +)CD25(superscript +) regulatory T cells (Tregs) in the immunopathogenesis of patients with chronic hepatitis B (CHB), and to evaluate their potential application in CHB immunotherapy. METHODS: Peripheral blood mononuclear cells (PBMCs) of 34 patients with CHB and 18 healthy donors were evaluated for the CD4(superscript +)CD25(superscript +) Treg phenotype and the proportion of CD4(superscript +)CD25(superscript +) Tregs as a percentage of the total number of CD4(superscript +) cells using flow cytometry with triple-color/fourth-color staining. CD4(superscript +)CD25(superscript +) Tregs were isolated from CHB patients by magnetic beads. Then PBMCs of Treg-depleted and non-Treg-depleted CHB patients were stimulated with HBV core18-27 peptide. The percentage of HBV corel8-27-specific cytotoxic lymphocytes (CTLs) and interferon (IFN)-γ-secreting CTLs was determined in PBMCs of Treg-depleted and non-Treg-depleted CHB patients by HLA-pentamer and Elispot (enzyme-linked immunospot assay) analysis. RESULTS: The percentage of CD4(superscript +)CD25(superscript +) Tregs in CHB patients was reflected by detecting the proportion of CD4(superscript +)CD127(superscript lo)CD25(superscript hi-int) Tregs (4.40%±2.11%) and CD4(superscript +)CD25(superscript +)CD45RO(superscript +)CTLA4(superscript +) Tregs (3.78%±1.87%), which were significantly higher than the proportion of CD4(superscript +)CD1271(superscript lo)CD25(superscript hi-int) Tregs (2.11%±1.26%) and CD4(superscript +)CD25(superscript +)CD45RO(superscript +)CTLA4(superscript +) Tregs (1.58%±0.76%) in healthy donors (P＜0.01). The frequency and IFN-γ-secreting spots of NY-ESO-1-specific CD8(superscript +) T cells was higher in CD4(superscript +)CD25(superscript +)- depleted PBMCs from CHB patients than in the total PBMC population of all patients tested (0.94%±0.38%, 26±13 vs 0.20%±0.18%, 119±30, t=5.25, t=9.886, P＜0.01). CONCLUSION: The proportion of CD4(superscript +)CD25(superscript +) Tregs in CHB patients seems to be greater than that in healthy donors. The elimination of Tregs can significantly improve the anti-viral CTL responses in CHB patients, compared with stimulation with HBV core18-27 peptide alone. This may contribute to immunotherapy in CHB patients.
Circulating and Liver Resident CD4+CD25+ Regulatory T Cells Actively Influence the Antiviral Immune Response and Disease Progression in Patients with Hepatitis B.

Abstract:CD4+CD25+ regulatory T cells (Treg) have been shown to maintain immune tolerance against self and foreign Ags, but their role in persistent viral infection has not been well-defined. In this study, we investigated whether and where CD4+CD25+ Treg contribute to the development of chronic hepatitis B (CHB). One hundred twenty-one patients were enrolled, including 16 patients with acute hepatitis B, 76 with CHB, and 29 with chronic severe hepatitis B. We demonstrated that in chronic severe hepatitis B patients, the frequencies of CD4+CD25+ Treg in both PBMC and liver-infiltrating lymphocytes were significantly increased and there was a dramatic increase of FoxP3(+)-cell and inflammatory cell infiltration in the liver compared with healthy controls. In CHB patients, circulating CD4+CD25+ Treg frequency significantly correlates with serum viral load. In acute hepatitis B patients, circulating CD4+CD25+ Treg frequency was initially low and with time, the profile reversed to exhibit an increased number of circulating Treg in the convalescent phase and restored to normal levels upon resolution. In PBMC taken from infected patients, depletion of CD4+CD25+ Treg led to an increase of IFN-gamma production by HBV-Ag-stimulated PBMC. In addition, CD4+CD25+ Treg were capable of suppressing proliferation of autologous PBMC mediated by HBV Ags, which probably reflects the generation of HBV-Ag-specific Treg in circulation and in the liver of HBV-infected patients. Together, our findings suggest that CD4+CD25+ Treg play an active role not only in modulating effectors of immune response to HBV infection, but also in influencing the disease prognosis in patients with hepatitis B.
Inhibition of CD4+ CD25+ Regulatory T Cells in Chronic Hepatitis B Patients

Abstract:OBJECTIVE:To evaluate the inhibition of CD4+ CD25+ regulatory T cells (Treg) in the chronic hepatitis B patients.METHODS:Peripheral blood samples were collected from 22 patients with chronic hepatitis B (CHB) and 18 healthy blood donors to isolate the peripheral blood mononuclear cells (PBMCs). Flow cytometry was used to analyze the proportion of CD4+ CD127(lo)CD25(hi-int) Tregs in the CD4+ T cells so as to calculate the proportion of CD4+ CD25+ Tregs in the CD4+ T cells. BrdU incorporation method was used to evaluate the immune inhibition of the CD4+ CD25+ Tregs. CD4+ CD25- cells were isolated by magnetic bead sorting technique. The CD4- T cells and CD4+ CD25- T cells ere mixed and stimulated by HBVcore 18-27 peptide. The PBMCs of the CHB patients with the Treg depleted and Treg not depleted underwent detection of HBVcore18-27 specific cytotoxic T lymphocytes (CTLs). The IFN-gamma secretion of the CTLs in the PBMCs of CHB patients with Treg depleted and Treg not depleted was detected by HLA-pentamer and enzyme-linked immunospot assay (Elispot).RESULTS:The proportion of CD4+ CD127(lo)CD25(hi-int) Treg in the CD4+ T cells used to reflect the percentage of CD4+CD25+ Tregs in the CD4+ T cells of the CHB patients was 4.3% +/- 2.4%, significantly higher than that of the healthy controls (2.1% +/- 1.3%, t = 3.74, P <0.01). There was no significant difference in the inhibition of CD4+ CD25- T cells by autogenous CD4+ CD25+ T cells between the CHB patients and healthy controls. The frequency of CTLs induced by HBV core 18-27 of the CHB patients with their CD4+ CD25+ cells in circulation depleted was 0.74% +/- 0.31%, significantly higher than that of the patients whose CD4+ CD25+ cells in circulation were not depleted (0.17% +/- 0.08%, t = 4.75, P <0.01). The frequency of IFN-gamma secreting spots of HBVcore18-27-specific CD8+ T cells of the CHB patients with their CD4+ CD25+ cells depleted was (112 +/- 33), significantly higher than that of the CHB patients whose CD4+ CD25+ cells in circulation were not depleted [(23 +/- 14), t =7.828, P<0.01)].CONCLUSION:The proportion of CD4+ CD25+ Treg in CHB patients is increased compared to the healthy blood donor. The proliferative capacity of CD4+ CD25- T cells is inhibited by the presence of CD4+ CD25+ Treg dose-dependently, and the inhibition of CD4+ CD25+ Tregs in the CHB patients is similar to the inhibition of CD4+ CD25+ Tregs in healthy donors. The elimination of Treg cells followed by stimulation with HBVcore18-27 peptide significantly improves the antivirus CTL responses in CHB patients.

The repertoire of TCR beta chain variable (TCRBV) regions of peripheral Tregs in ACLF patients is not well understood. Methods Human PBMCs were separated and sorted into CD4 + CD25 + Treg subsets using density gradient centrifugation and magnetic activated cell sorting (MACS). The CD4 + CD25 high Treg frequency in peripheral blood of ACLF and chronic hepatitis B (CHB) patients was measured by flow cytometry. The molecular profiles of TCRBV CDR3 were determined using gene melting spectral pattern (GMSP) analysis. TCRBV gene families were cloned and sequenced when the GMSP profiles showed a single-peak. Results CD4 + CD25 high Treg prevalence in peripheral blood of ACLF patients is increased significantly compared to healthy donors (HDs) ( P < 0.01) and CHB patients ( P < 0.01). The prevalence of CD4 + CD25 high Tregs in ACLF or CHB patients is positively correlated with HBV DNA load. The TCRBV11, BV13.1, BV18, BV20 are the most prevalent TCRBV in CD4 + CD25 + Tregs in ACLF and CHB patients. In addition, the CDR3 motifs were relatively conserved in these four TCRBV gene families. Conclusions The CD4 + CD25 high Tregs prevalence in peripheral blood is indicative of disease severity in ACLF or CHB patients. The relatively conserved TCRBV20 CDR3 motif “TGTGHSPLH” and TCRBV11 CDR3 motif “VYNEQ” may be used in helping diagnosis and treat patients with ACLF.

Abstract:The frequency and T-cell receptor beta-chain variable (TCRBV) patterns of peripheral CD4(+) CD25(+) regulatory T-cells (Tregs) are ambiguously altered in chronic hepatitis B (CHB) patients following tenofovir disoproxil fumarate (TDF) treatment. Moreover, the clinical significance of these parameters in relation to hepatitis B e antigen (HBeAg) seroconversion (SC) is largely unknown. In this study, the circulation of Tregs in HBeAg-positive CHB patients was determined by flow cytometry, and the molecular profiles of frequent TCRBV patterns of Tregs were analyzed using a gene melting spectral pattern. The parameters, such as Treg frequency, the number of skewed TCRBV patterns, hepatitis B virus (HBV) DNA levels, and alanine aminotransferase (ALT) levels, were analyzed by comparing their associations in seroconverting and non-seroconverting patients following TDF treatment. The Treg frequency was significantly correlated with the ALT level in seroconverting but not in non-seroconverting patients. Similarly, skewed TCRBV patterns were remarkably associated with HBV DNA levels in the SC group. Six TCRBV families (BV3, BV11, BV12, BV14, BV20, and BV24) were more prevalent than other TCRBV members in seroconverting patients pretreated with TDF, while BV12, BV15, and BV22 were predominant in non-seroconverting patients during TDF treatment. Taken together, the preferential TCRBV patterns may be associated with immune responses related to SC. The dynamic frequency and skewed TCRBV patterns of peripheral Tregs could contribute to predicting SC in CHB patients. Moreover, the conserved TCRBV complementarity-determining region (CDR3) motif may be targeted to develop personalized immunotherapy for CHB patients.

Abstract:Objective To analyze the expression of four important molecules of CD4+CD25+ regulatory T cells(Tregs) in peripheral blood of chronic hepatitis B patients and asymptomatic HBV carriers,and to investigate their potential functions.Methods 50 patients with chronic hepatitis B(CHB),28 asymptomatic HBV carriers(ASC) and 24 healthy blood donators were enrolled in this study.Peripheral blood mononuclear cells(BMC) from CHB,ASC and healthy blood donators were isolated by Ficoll-Hypaque density gradient centrifugation.CD4+CD25+ Tregs were isolated by using CD4+CD25+ regulatory T cell isolation kit,and auto MACS separator(Miltenyi Biotec Inc.).Then their total RNA was extracted.After reverse transcription,we developed and evaluated a rapid and reliable real-time PCR approach using Applied Biosystems 7500 Real-Time PCR technology for validation of this partial results of hybridization.Results The results of real-time PCR demonstrated that the expression of ICAM-1,neuropilin-1 and TLR5 showed no significant difference in circulating CD4+CD25+ Tregs from different groups.ASC presented a lower expression of TLR8 of circulating CD4+CD25+ Tregs than those in CHB patients and normal controls(P<0.05),but there was no significant difference of TLR8 expression in circulating CD4+CD25+ Tregs between CHB and normal controls(P>0.05).Conclusion The results of real-time PCR demonstrated that there is difference of TLR8 expression in circulating CD4+CD25+ Tregs among different groups.TLR8 may play an important role in dominant immunological self-tolerance maintained by CD4+CD25+ Tregs after HBV infection.
Circulating Cd4(+) Cd25(+) Regulatory T Cells Correlate With Chronic Hepatitis B Infection

Abstract:Circulating CD4(+) CD25(+) regulatory T cells (Tregs) have been demonstrated to maintain immunotolerance and suppress the antigen-specific or antigen-non-specific T-cell responses, but their role in chronic hepatitis B (CHB) infection in humans has not been well characterized. In this study, we analysed the frequency and phenotypic characteristics of CD4(+) CD25(+) Tregs in patients of different hepatitis B virus (HBV) infection status, and investigated the effect of Tregs on antiviral immune responses in CHB patients, and the mechanism of this effect. A total of 137 subjects, including 79 CHB patients, 26 asymptomatic HBV carriers (ASCs), 12 acute hepatitis B (AHB) patients and 20 healthy controls, were enrolled in the study. We found that the frequency of CD4(+) CD25(high) Tregs in AHB patients was comparable to that in healthy controls, while it was significantly increased in CHB patients. CD4(+) CD25(+) Tregs produced interleukin (IL)-10 but little or no interferon (IFN)-gamma under anti-CD3 stimulation. In CHB patients, the frequency of CD4(+) CD25(high) Tregs positively correlated with serum viral load, and the Tregs were capable of suppressing the proliferation and IFN-gamma production of autologous peripheral blood mononuclear cells (PBMC) mediated by HBV antigen stimulation in vitro. However, combined administration of anti-programmed death-1 (PD-1) and anti-cytotoxic lymphocyte antigen-4 (CTLA-4) monoclonal antibody slightly enhanced the cellular proliferation and significantly increased the IFN-gamma production of PBMC cocultured with Tregs at a ratio of 2 : 1. Thus, the frequency of circulating CD4(+) CD25(+) Tregs is increased in patients with CHB, and this may play an important role in viral persistence by modulating virus-specific immune responses.

Abstract:Objective To explore the percentages of CD4+CD25+Treg in peripheral blood from patients with chronic hepatitis B(CHB) and detect the levels of IL-2,IL-4,IL-10,IFN-γ and TGF-β in serum and investigate the roles of CD4+CD25+Treg and cytokines in blood on the chronicity of hepatitis B.Methods Selecting 24 cases of CHB as CHB group,8 cases of acute hepatitis B(AHB) as AHB group and 12 healthy persons as the normal control group.Detect the percentage of CD4+CD25+Treg in peripheral blood of the three groups with flow cytomerty and detect the levels of the five cytokines above in CHB group and the normal control group with double antibody sandwich method of ELISA.Results The percentages of CD4+CD25+Treg in CHB group is obviously higher than that of AHB group and that of the normal control group(both P<0.01).There is no correlation between CD4+CD25+Treg and HBV-DNA or ALT both in the groups of CHB and AHB(P>0.05).The levels of IL-2,IL-4,IL-10,IFN-γ and TGF-β in CHB group are higher than that of the normal control group(P<0.01).There is a positive correlation between ALT and IL-2 or IFN-γ in CHB group,respectively(P<0.05).Conclusion There exists immunologic derangement in patients with chronic hepatitis B.CD4+CD25+Treg and Th1/Th2 imbalance play an active role in the chronicity of hepatitis B.

Abstract:Background Accumulating evidence supports the theory that expression of CD127 on CD8 T cells during the process of antiviral immune response indicates a subset of effect CD8 T cells that successfully develop into fully protective memory. CD8 T cells expression of CD127 may be used as a predictor to evaluate disease status in chronic viral infection. The aim of this study was to investigate the CD127 expression level on different subsets of CD8 T cell and explore the relationship between CD127 expression on CD8 memory T cells and serum hepatitis B virus (HBV) DNA and hepatitis B e antigen (HBeAg) levels in patients with chronic hepatitis B (CHB). We also aimed to investigate the CD127 expression pattern on CD8 memory T cells of CHB patients who were treated with Telbivudine. Methods/Results Twenty HBeAg-positive CHB patients were selected and treated with telbivudine 600 mg/day for 48 weeks. The memory CD8 T cells were characterized by expression of CD45RA and CD27 markers. CD127 expression on the CD8 T-cell surface was measured by four-colour flow cytometry. Our results showed that CD127 expression on memory CD8 T cells was reduced in CHB patients. There was a strong negative correlation between CD127 expression on memory CD8 T cells and serum HBV DNA and HBeAg levels in CHB patients. Moreover, successful antiviral therapy increased CD127 expression on CD8 memory T cells as well as on HBV-specific CD8 T cells in CHB patients. Conclusion These results suggest that diminished CD127 expression on CD8 memory T cells of CHB patients is a potential mechanism explaining cellular immune function impairment in CHB infection, and that CD127 expression on CD8 memory T cells is a useful indicator for evaluating the effects of anti-HBV therapy.

Abstract:Background Both interleukin (IL)-17-secreting CD4 + T (Th17) and CD4 + CD25 + Foxp3 + T regulatory (Treg) cells have been shown to be associated with disease progression or liver damage in chronic hepatitis B (CHB) patients. However, the relationship between Treg cells and IL-17-secreting T cells in hepatitis B virus (HBV) infections is unclear. Methods The frequencies of Treg cells and IL-17-secreting T cells in hepatitis B e antigen (HBeAg)-positive CHB patients and healthy subjects were measured by flow cytometric analysis. The role of Treg cells on the differentiation of Ag-specific IL-17-secreting T cells was determined by removing the Treg cells from peripheral blood mononuclear cells (PBMCs) in HBeAg-positive CHB patients. Results The frequencies of both Th17 (1.71 ± 0.58 vs. 1.08 ± 0.36 %; P < 0.01) and Treg cells (8.92 ± 4.11 vs. 6.45 ± 1.56 %; P < 0.01) were increased in the peripheral blood of HBeAg-positive CHB patients compared with healthy controls, but in not the IL-17-secreting CD8 + T (Tc17) cells. The frequency of Treg cells was significantly associated with that of Th17 cells ( r = 0.625, P = 0.001) in CHB patients. Spearman analysis showed a positive correlation between the frequency of Treg cells and HBV DNA load ( r = 0.508, P = 0.008), as well as between the frequency of Th17 cells and serum alanine aminotransferase level ( r = 0.516, P = 0.007). The deletion of Treg cells significantly enhanced both Th17 and Tc17 cell development in PBMCs following recombinant HBV core antigen stimulation. Conclusions Our data indicate a clear inverse relationship between Th17 cells and Treg cells and that Treg cells can inhibit Th17 cell development in CHB patients.

Abstract:Interleukin (IL)-35 is an inhibitory cytokine consisting of IL-12A and Epstein-Barr virus-induced gene 3 (Ebi3) and is required by regulatory T-cells (Tregs) for maximal activity. During chronic hepatitis B virus (HBV) infection, Tregs have immunosuppressive effects on HBV-specific T helper (Th) cells, yet little is known about the complex regulation of Tregs and their contribution to the inadequate immune system response to the virus. In the present study, we investigated whether IL-35 is involved in HBV-related cellular immune responses. Cluster of differentiation (CD)4(+) T-cells from peripheral blood were derived from healthy volunteers, resolved HBV individuals and chronic active hepatitis B patients and stimulated with CD3/28-conjugated beads. We analysed mRNA and protein levels of IL-35 and assessed the inhibitory effect of IL-35 on HBV core antigen-specific cytotoxic T lymphocytes (CTLs), dendritic cells (DCs) and effector T-cells (Teffs). Correlation analyses between liver inflammation and HBV DNA load were conducted. Results show that chronic HBV patients harbour significantly higher levels of Ebi3 mRNA and protein in CD4(+) T-cells compared with healthy volunteers and resolved HBV individuals. IL-35 suppressed the proliferation of HBV antigen-specific CTLs and interferon (IFN)-γ production in vitro. Ex vivo, IL-35 decreased the proliferation of CD4(+)CD45RA(+) naïve T-cells, especially in CD4(+)CD25(-)CD45RA(+) naïve Teffs. IL-35 inhibited the expansion of CD11c(+) DCs. Our data indicate that IL-35 is highly expressed in chronic HBV CD4(+) T-cells and plays an important role in the inhibition of the cellular immune response in chronic HBV.
