Abstract:The formation of a traumatic arteriovenous fistula (AVF) between the extracranial middle meningeal artery (MMA) and the pterygoid plexus (PP) is very rare, and understanding of this condition is limited. This paper reports the case of an 8-year-old who suffered minor injuries after a high fall four months prior to admission and showed good recovery after one month. However, the child gradually developed exophthalmos of the left eye and conjunctival redness one month prior to admission. Auscultation revealed an intracranial murmur near the left side of the face, in the temporal region. A digital subtraction angiography (DSA) showed rupture of the left extracranial MMA and an AVF between the MMA and the PP. The blood drained toward the cavernous sinus, resulting in retrograde blood flow into the ophthalmic vein and the cortical vein. The diagnosis was an AVF between the MMA and the PP, and a combination of coils and Onyx liquid embolic agent was employed to perform AVF embolization. Follow-up six months later indicated no recurrence of the AVF, and the patient showed good recovery with a normal-appearing left eye. The AVF in this case drained toward the cavernous sinus, and symptoms of increased intracranial venous system pressure were apparent, similar to those produced by fistulas between the internal carotid artery and the cavernous sinus. This condition is very rare, and the use of coils in combination with Onyx for AVF embolization is novel, warranting the reporting of the current case.

Abstract:Background: While cerebral infarction in children is rare, its prognosis is poor, and this condition can seriously burden society and families. A correlation between patent foramen ovale (PFO) and ischemic stroke has not been found in pediatric patients. Case presentation: We report a 7-year-old boy who suffered from multiple cerebral infarctions. Subsequently, the patient was diagnosed with an abnormal shunt of PFO. He underwent PFO closure and was followed up for 1 year. The patient did not experience any further cerebral infarction. Conclusions: With this case report, we want to illustrate that although the incidence rate of ischemic cerebral infarction in adolescents is very low, we should not neglect the role of PFO. Therefore, after exclusion other causes of cerebral infarction, PFO should be considered in adolescent and adult stroke patients with adult closure criteria in the same way.

Abstract:Dural arteriovenous fistula (DAVF) is a rare type of cerebral arteriovenous malformation. The occurrence of parkinsonism together with progressive cognitive dysfunction caused by DAVF has been rarely reported. The probable underlying pathophysiology could be due to venous hypertension caused by DAVF which leads to basal ganglia and cortical dysfunction. Here, two DAVF cases were reported presenting with parkinsonism and progressive cognitive dysfunction. A 54-year-old man (case 1) and a 75-year-old man (case 2) presented with bradykinesia, gait disturbances and cognitive dysfunction who were initially misdiagnosed as having Parkinson's disease. Case 1 exhibited a characteristic type of subcortical calcification on CT scan, while case 2 had specific resting tremor and intracranial hypertension. Both cases showed transient response to the dopaminergic treatment. After 3D time-of-flight (TOF) magnetic resonance arteriography (MRA) or digital subtraction arteriography (DSA), they were diagnosed as having DAVF. This report suggests the possibility of DAVF in patients presenting with parkinsonism and progressive cognitive dysfunction, which requires further attention to be paid, especially in those with transient response to levodopa. Early 3D TOF MRA or DSA is recommended for the diagnosis of DAVF.

Abstract:Intracranial pial arteriovenous fistulas (AVFs) associated with multiple aneurysms of the main feeding arteries are very rare cerebrovascular lesions. We report a unique case of pial AVFs associated with four aneurysms of the feeding anterior cerebral artery (ACA) which presented as intracerebral hemorrhage (ICH), intraventricular hemorrhage (IVH) and spontaneous subarachnoid hemorrhage (SAH). CT angiography (CTA) and digital subtraction angiography (DSA) images demonstrated clearly the direct connection without nidus between the first and second segment of right ACA accompanied by four irregular aneurysms and an abnormally dilated draining vein into the superior sagittal sinus (SSS). Owing to the superficial-seated fistulas, the morphology of feeding arteries and associated four aneurysms and intracranial hemorrhage, the lesions were surgically treated. Postoperative cerebral angiography certified closure of the fistulas and exclusion of the four aneurysms as well as disappearance of early venous drainage. However, subsequent precontrast brain CT showed hydrocephalus underwent left ventriculoperitoneal shunt placement.

Abstract:A patent foramen ovale (PFO) carries a high risk of paradoxical embolism. This risk is higher in certain conditions, including acute pulmonary embolism (APE). Although most patients with a PFO are asymptomatic, various clinical manifestations may be associated with PFO. Concomitant APE and acute ischemic stroke (AIS) due to paradoxical embolism from a PFO are rare. We report a case of a 61-year-old man who presented with simultaneous PE and AIS in the presence of PFO, was treated successfully with anticoagulation, and was discharged from the hospital neurologically intact.

Abstract:Hypereosinophilic syndrome (HES), characterized by an increased number of eosinophils in tissues and/or blood, presents with heterogeneous clinical manifestations. Studies have shown that HES can affect the nervous system and may be associated with cerebral infarction. Patent foramen ovale (PFO) is the most common congenital intracardiac defect that can cause right-to-left shunting and contribute to the paradoxical embolization of venous emboli, and even lead to stroke. We report the case of a young man who presented with cerebral infarction accompanied by both HES and PFO. The patient underwent thorough evaluation to determine the source of emboli and the potential pathogenesis. In this case, HES was confirmed and glucocorticoid treatment was conducted. Direct imaging using optical coherence tomography (OCT) confirmed that the embolus originated from the PFO. Therefore, we performed PFO occlusion. The patient recovered well, and no new cerebral infarction was observed at 6-month follow-up. Based on the results of our study, we conclude that it is important to consider unusual etiologies of cerebral infarction, particularly in younger patients.

Abstract:Patent foramen ovale (PFO) is a normal atrial blood flow pathway during fetal life. It is also a necessary passageway for the fetus to survive. The PFO will anatomically close 2 months after birth and reach anatomically complete closure within 1 year. [ 1 ] However, PFOs also remain in about 25% to 30% of adults, and when right heart chamber pressures increase (e.g., Valsalva maneuver), PFOs can open, causing right-to-left shunts in the atria. [ 2 ] Arterial ischemic stroke (AIS) is a general term for necrosis of brain tissue due to insufficient blood supply to the brain from a variety of causes. About 30% of AIS cases have an unknown cause, called cryptogenic stroke (CS). Some patients with CS have coexisting PFOs, and it is currently believed that there is a correlation between CS and PFOs. [ 3 , 4 ] The mechanism by which a PFO triggers CS has been controversial, and 3 potential mechanisms by which a PFO triggers CS in stroke are currently thought to be possible. Three possible mechanisms include paradoxical embolization (PE), "insitu" thrombosis, and arrhythmias. [ 5 ] PE is the most common cause of CS. PE is when a PFO establishes a direct connection between right and left heart blood flow, and emboli from the veins or right atrium may enter the arterial circulation directly and reach the cerebral arteries to obstruct blood flow, leading to AIS. In addition to cerebral embolism, it can lead to myocardial infarction, gastrointestinal ischemia, renal infarction, and peripheral arterial embolism. [ 6 ] The in situ thrombus hypothesis refers to the formation of a thrombus in the atrial septum, the mechanism of which may be related to the stagnation of blood flow in the atria and the pressure difference between the atria. [ 7 ] Another hypothesis suggests that PFO may cause abnormal left atrial electrical activity, which in turn causes arrhythmias such as premature atrial contractions. Abnormal left atrial electrical activity in a child can lead to thrombosis, which can cause CS. [ 8 ]

Abstract:Cervical artery dissection (CAD) and Patent Foramen Ovale (PFO) are important causes of stroke in young patients. Although PFO is considered an independent risk factor for cerebral infarction in young adults with cryptogenic stroke, other concomitant causes may be necessary to cause brain injury. PFO could be a predisposing factor of stroke through several mechanisms including paradoxical embolism from a venous source, thrombus formation in atrial septum, or atrial arrhythmias causing cerebral thromboembolism. The pathophysiology of CAD is poorly understood and includes both constitutional and environmental factors. A causal association is often difficult to establish, as other predisposing factors may also play a role in CAD etiopathogenesis. We present a family with ischemic stroke (a father and his three daughters), in which the two different stroke causes are present. We hypothesized that a paradoxical embolism caused by PFO, associated with arterial wall disease, in the presence of a procoagulant state, could produce arterial dissection and then stroke.

Paradoxical Brain Embolism Caused by Isolated Pulmonary Arteriovenous Fistula Successfully Treated with Recombinant Tissue Plasminogen Activator

Abstract:Pulmonary arteriovenous fistula (PAVF), a vessel malformation connecting the pulmonary circulation to the systemic circulation while bypassing the pulmonary capillaries, can cause paradoxical cerebral infarction. It is often associated with hereditary hemorrhagic telangiectasia (HHT), a genetic disease characterized by multiple dermal, mucosal, and visceral telangiectasia causing recurrent bleeding. Paradoxical cerebral embolism caused by PAVF without HHT is rare. Here, we report a patient with isolated PAVF who experienced an ischemic stroke caused by a paradoxical embolism from deep venous thrombosis; the patient was successfully treated with recombinant tissue plasminogen activator. She presented with a decrease in arterial oxygen saturation to 91%, and lung disease was suspected. A PAVF was subsequently found in the right S6 region using contrast computed tomography. Interventional radiologists successfully occluded the shunt using 6 microcoils. PAVF should be considered when determining the pathogenesis of cerebral ischemia in patients with hypoxia, which can be the only symptom of PAVF.

Abstract:Rationale: A dural arteriovenous fistula (DAVF) refers to an abnormal direct connection between an intracranial artery and a dural venous sinus. A DAVF presenting with parkinsonism is rare, and is therefore easily misdiagnosed. Therefore, early consideration of DAVF in the differential diagnosis of reversible parkinsonism is necessary. Patient concerns: We present the case of a 51-year-old male with progressive parkinsonism. Diagnoses: He was diagnosed as straight sinus occlusion. Imaging studies revealed a DAVF associated with cerebral hypoperfusion of the lenticular nuclei and frontal lobe white matter. Interventions: Endovascular embolization was performed through his left occipital artery. Outcomes: Treatment resulted in marked clinical improvement that a major improvement of parkinsonism was observed concomitant with no evidence of early venous drainage of this patient. Lessons: DAVF should always be considered as a potential cause of progressive parkinsonism on account of its potential reversibility. Our case suggests a concomitant role of basal ganglia degeneration and frontal white matter hypoperfusion in the pathology of parkinsonism due to DAVF. However, the precise pathophysiology remains to be investigated.
Clinical study of transcatheter closure of patent foramen ovale with cryptogenic stroke

Abstract:AIM: To evaluate the therapeutic efficacy and safety of transcatheter closure of patent foramen ovale(PFO) with cryptogenic stroke.METHODS: Infarcted lesions were confirmed by CT or MRI in 26 patients with PFO with ischemic stroke(aged 35±12 years,range 7-52 years).Other identifiable causes such as aorta and cerebral vascular disease or left ventricular system disease were excluded.TCD displayed right to left shunting through PFO during Valsalva and TTE/TEE confirmed atrial septum aneurysm(ASA).All patients underwent transcatheter closure of patent foramen ovale.RESULTS: TEE showed that the average diameter of PFO in the 26 patients was(3±1) mm(range: 1-5 mm) and five cases were defined as PFO with ASA.TCD and bubble test assessed the degree of shunting,including six cases of grade 1,nine cases of grade 2,and 11 cases of grade 3 or higher.Twenty-four cases were successfully occluded,with the success rate of 92.3%.The size of the occluder was from 18-35 mm,including six cases of 18 mm,seven cases of 25 mm,eight cases of 30 mm and three cases of 35 mm.The main cause for the two unsuccessful cases was the failure of the wire to go through the atrial septum.No displacement of occluder and no residual shunt were observed by TTE during the 6-month to 1-year follow-up.No recurrence of ischemic cerebral infarction occurred.CONCLUSION: Transcatheter interventional occlusion of PFO is safe and effective in reducing recurrence of ischemic stroke.

Retrograde cerebral embolism and pulmonary embolism caused by patent ductus arteriosus: a case report

Abstract:Background: Although rare, paradoxical embolism sometimes occurs with patent ductus arteriosus (PDA). This study presents a case of PDA-associated paradoxical embolism with acute ischemic stroke (AIS) and pulmonary embolism (PE) following thoracoscopic surgery. Case presentation: A 65-year-old woman developed acute-onset aphasia and right hemiparesis on the third day following thoracoscopic resection for a right lung tumor. Brain magnetic resonance imaging revealed multiple infarcts, and lower extremity venous Doppler ultrasound revealed deep vein thrombosis. The patient subsequently developed dyspnea, tachycardia, and hypoxemia. PE was confirmed by percutaneous transfemoral venous selective pulmonary angiography, which meanwhile demonstrated a PDA lesion. The patient, after receiving catheter-directed thrombolysis and inferior vena cava filter placement, improved in both neurological and respiratory status. Conclusion: For an uncommon but potentially fatal case with PDA-induced paradoxical embolism causing AIS and PE, early recognition and treatment are vital. Further studies are warranted to determine the optimal management and prognosis of patients with PDA-related embolic events.

Abstract:Patent foramen ovale(PFO) is a congenital cardiac lesion present in >25% of the adult population.Diagnosis,evaluation and treatment of PFO have attracted increasing interest as it has been suggested that PFO may be associated with various pathological conditions such as cryptogenic stroke,platypnea-orthodeoxia syndrome,decompression sickness and migraine.Management of PFO remains controversial and,although not yet approved by the U.S.Food and Drug Administration,transcatheter approach to PFO closure continues to develop.However,its potential complications include device embolization,thrombus formation,atrial arrhythmias,residual shunting and device erosion or perforation.As with any cardiac intervention,transcatheter closure may cause serious complications and drug therapy thus seems to be a safe and effective option.This paper reviews the drug therapy of PFO with cryptogenic stroke to provide a theoretical basis of its drug treatment for clinicians.

Abstract:Abstract Background Pulmonary arteriovenous fistula (PAVF) is a rare disease, which can lead to the direct return of unoxidized venous blood to pulmonary veins and left heart, resulting in right-to-left shunt leading to hypoxia. Long term, the right-to-left shunt will cause severe pathophysiological changes in the patient’s body and pulmonary circulation, and the prognosis will be poor if PAVF is not treated timely. Case presentation Here, we report the case of a 71-year-old man who presented with chest tightness and shortness of breath. After a series of examinations, PAVF and giant hemangioma were diagnosed, which are difficult to operate.Transcatheter interventional therapy was initiated. The patient recovered on the third day after operation and was discharged smoothly. During the long-term follow-up of nearly 4 years after discharge, the general condition and quality of life of the patient basically returned to normal. Conclusions PAVF is rare but very important clinical problem. When the clinical manifestations of persistent unexplained hypoxia appear, it is necessary to fully consider the possibility of PAVF. Once the diagnosis of PAVF is clear, timely treatment is recommended to avoid deterioration of the disease and affecting the prognosis.

Diffuse Pulmonary Arteriovenous Fistulas: Report of 2 Cases and Literature Review

Abstract:Objective To describe the clinical characteristics of diffuse pulmonary arteriovenous fistulas (PAVFs) and to evaluate the methods of diagnosis and treatment.Methods Retrospective analysis of two patients with diffuse PAVFs who were diagnosed in Peking University First Hospital and the literatures on the subject were reviewed.Results Both patients presented with the triad of dyspnea on exertion,cyanosis,and clubbing.Case 1 was a woman with hereditary telangiectasia disease (HHT),who also presented with repeated epistaxis and facial telangiectasia.Case 2 was a man with hepatic cirrhosis,who presented with hepatopulmonary syndrome.Echocardiographies of 2 patients were both normal,peripheral vein contrast echocardiography showed abnormal filling of the left atrium with echo contrast material.Chest CT showed bilateral intrapulmonary artery dilatation,subpleural small nodular,reticular and patchy opacities.Pulmonary angiography showed that pulmonary circulation time was both shortened.Pulmonary angiography also showed that lower left dorsal segment artery and pulmonary vein were directly connected in case 2,and then tanscatheter embolization was carried out.However,oxygenation of this patientwasn't improved.Case 1 was alive and case 2 died of liver cirrhosis after 5-year follow up,neither of them had brain complications.Conclusions Diffuse PAVFs are clinically rare and present with the triad of dyspnea on exertion,cyanosis,and clubbing.Pulmonary angiography and contrast echocardiography are helpful for diagnosis.
Assessment of the Characteristics of Patent Foramen Ovale Associated with Cryptogenic Stroke

Abstract:Objective: This study aims to comprehensively assess the characteristics of patent foramen ovale (PFO) in relation to Cryptogenic Strok (CS) by utilizing transesophageal echocardiography (TEE) and contrast transthoracic echocardiography (c-TTE) and to identify high-risk factors associated with PFO-related CS. Background: Transcatheter PFO closure has demonstrated its effectiveness in preventing PFO-related CS. Therefore, understanding the specific structural attributes of PFO associated with CS is imperative. Methods: Enrollment comprised 113 test patients who experienced CS in conjunction with PFO and 117 control patients diagnosed with migraine with PFO but without a history of stroke. The characteristics of the PFO were observed by TEE and c-TTE. A comparative analysis was undertaken to assess the variations in PFO characteristics between the test patients and controls, and to uncover the independent factors relevant to CS. Results: The patients in the test group were older than the controls. Both the height and length of the PFO during Valsalva exhibited greater dimensions in the test group when contrasted with controls. Notably, the test group presented higher incidence rates of low-angle PFO (defined as an angle between the inferior vena cava (IVC) and PFO ≤ 10°) and atrial septal aneurysm (ASA) as contrasted with the control group. Right-to-left shunt (RLS) III during Valsalva demonstrated a significantly elevated occurrence within the test group as opposed to the controls. Conversely, RLS II during Valsalva exhibited a significantly higher frequency in the controls in contrast to the tests. No significant disparities were observed between the two groups with respect to RLS I during Valsalva and all grades of RLS at rest. Multivariate analysis revealed that the length of the PFO during Valsalva, the presence of ASA, RLS III during Valsalva and low-angle PFO were independent relevant factors associated with CS. Conclusions: The length of the PFO tunnel, low-angle PFO, RLS III during Valsalva and the presence of ASA were independent risk factors for CS. The combined utilization of TEE and c-TTE may prove valuable in identifying PFO patients at a heightened risk of CS and in facilitating the screening process for transcatheter PFO closure.
