Abstract:Purpose: To report surgical outcomes of 25-gauge pars plana vitrectomy using air as an internal tamponade for patients with primary rhegmatogenous retinal detachment (RRD). Methods: A retrospective clinical study of 59 eyes of 59 consecutive patients presented with primary RRD at the Beijing Tongren Eye Center in China. From August 2016 to May 2018, medical records of the patients who underwent 25-gauge pars plana vitrectomy with air tamponade for RRD were reviewed. The main outcome measures were primary and final anatomical success (retinal re-attachment) rates, and postoperative complications. Results: Of the 59 patients, aged 54.47 ± 11.81 years, 31 (52.5%) were men. Vitrectomy was performed 3 to 40 (averaged 16.98 ± 10.17) days after the onset of symptoms, and the mean follow-up period was 12.90 ± 5.92 months (ranging 6.07-26.10 months). Forty-two eyes (71.2%) had RRD with retinal breaks in the superior half of the retina, and the mean number of retinal breaks was 1.75 ± 0.94. Three eyes (5.1%) had RRD with giant retinal tears. Of the 59 eyes, 35 (59.3%) had RRD with inferior quadrants involved. Proliferative vitreoretinopathy (PVR) gradings were C1 in 2 (3.4%) eyes and B or below in 57 (96.6%) eyes. The primary and final anatomical success rates were 94.9% (56/59) and 98.3% (58/59), respectively. Of the three eyes which developed re-detachment of the retina, one eye had postoperative progression of PVR and two eyes were RRD associated with macular hole in high myopia. Postoperative complications included 5 eyes (8.5%) with serous choroidal detachment within 3 days after surgery and 4 eyes (6.8%) with macular epiretinal membrane formation 1 to 8 months after surgery. Secondary cataract surgery was performed in 13 of the 53 phakic eyes (24.5%) during follow-up. Conclusion: Small-gauge pars plana vitrectomy with air tamponade may be effective in treating selected cases of relatively simple primary RRD. Additional studies are needed to verify the efficacy of this surgical approach for more complicated cases such as those with giant retinal tears.

Abstract:BACKGROUND AND OBJECTIVE: To compare the surgical outcome of 27-gauge pars plana vitrectomy (PPV) with 25-gauge PPV and evaluate the efficacy of air tamponade for the treatment of myopic foveoschisis (MF). PATIENTS AND METHODS: Sixty eyes of 60 consecutive patients were recruited to undergo vitrectomy, internal limiting membrane (ILM) peeling, and air tamponade for treatment of MF. These eyes were divided into two groups: the 27-gauge group (n = 29) and the 25-gauge group (n = 31). RESULTS: There were no significant differences in the mean operating time ( P = .32), the mean time for vitrectomy ( P = .20), and the suturing rate( P = .46) between the 27-gauge group and the 25-gauge group. At the final follow-up, anatomical resolution rate did not differ between the two groups ( P = .65). CONCLUSION: The 27-gauge vitrectomy system is as efficient and feasible as the 25-gauge system for the treatment of MF. Air tamponade can provide good postoperative anatomic resolution of MF. A Short-Time Real-World Study of Two Perfluoro Propane Tamponade Methods in Pars Plana Vitrectomy for Retinal Detachment

Abstract:Introduction: This real-world study evaluated the efficacy, safety, and operative parameters of two perfluoropropane (C3F8) tamponade methods combined with pars plana vitrectomy (PPV) for retinal detachment(RD). Methods: A retrospective study of 132 patients (132 eyes) with RD (pure C3F8 in 38 eyes, mixed C3F8 in 94 eyes). All eyes underwent PPV with C3F8 tamponade and were followed up for at least 3 months. Retinal reattachment rate, time of gas configuration and injection, C3F8 dosage, intraocular pressure (IOP),best corrected visual acuity (BCVA), postoperative ocular inflammation, and patients' complaints were evaluated. Results: The single-surgery retinal reattachment rates of the pure C3F8 group and mixed C3F8 group were 97.4% and 96.8%, respectively, with no significant difference (P=1.00). The final retinal reattachment rates of the two groups were 100 % and 97.2%, respectively, with no significant difference (P=1.00). The gas configuration time, gas injection time, and C3F8 dosage were significantly less in the pure C3F8 group (all P<0.001). Time, but not group, was the influencing factor of postoperative IOP changes in the two groups (P < 0.001, P=0.547, respectively). Compared with the baseline, the IOP estimates of the pure C3F8 group showed a significant increase immediately after surgery ( P<0.001), and the mixed C3F8 group showed a significant increase immediately and 1-2 days after surgery(all P<0.05). There was no statistical difference in ocular inflammation(P=0.339) and patients' complaints of discomfort (P=0.175) between the two groups. Conclusion: Both two methods of C3F8 tamponade combined with PPV in RD patients showed good efficacy and safety, but the clinical operation of pure C3F8 tamponade was more convenient and eco-friendly.

Abstract:Objective To evaluate the efficacy and safety of 27-gauge sutureless vitrectomy with air tamponade for rhegmatogenous retinal detachment (RRD).Methods The clinical data of 35 consecutive eyes with primary RRD from 35 patients who received 27-gauge vitrectomy with intraocular air tamponade in Zhongshan Eye Center from April 2016 to January 2017 were retrospectively analyzed.The mean follow-up duration was 8.6 months.Best corrected visual acuity (BCVA) (LogMAR) and intraocular pressure (IOP) were examined before surgery,1 week and 3 months after surgery.The operative duration,sclerotomy sites,retinal reattachment rate,intraoperative and postoperative complications were recorded.Results The mean duration of vitreous removal was (15.3 ± 3.6) minutes,and the mean duration of operation was (34.5 ± 4.8) minutes.No suturing process was performed at sclerotomy sites in all eyes.The retinal reattachment rate following a single procedure was 100％.The mean BCVA was significantly different among before surgery,1 week and 3 months after surgery (F =64.12,P＜0.01),and the BCVA at 1 week and 3 months after surgery was evidently improved in comparison with before surgery (0.82±0.31 vs.1.01 ±0.40;0.68±0.30 vs.1.01 ±0.40) (both at P＜0.05).The mean IOP was (14.69±3.66),(17.37±2.32) and (16.69±2.45) mmHg (1 mmHg =0.133 kPa) before surgery,1 week and 3 months after surgery,showing a significant difference among them (F=14.82,P＜0.01),and the IOP 1 week and 3 months after surgery was evidently higher than that before surgery (both at P＜0.05).The complications included intraoperative iatrogenic retinal breaks in 2 eyes,postoperative hypotony in 1 eye and hypertension in 5 eyes.These complications were curable.Conclusions 27-Gauge vitrectomy and air tamponade for RRD is an effective and safe approach.
Intraoperative Perfluorocarbon Liquid Tamponade Technique for Treatment of Extensive Retinal Detachment Secondary to a Myopic Macular Hole

Abstract:Background/Purpose:To report an intraoperative perfluorocarbon liquid (PFCL) tamponade technique in treating extensive retinal detachment secondary to a myopic macular hole (MH) through pars plana vitrectomy.Methods:The technique was applied in nine eyes with MH-RD extending two quadrants or more areas. The procedures for pars plana vitrectomy included: 1). thorough drainage of subretinal fluid through the MH with fluid-air exchange; 2). PFCL tamponade on the macular area for more than 10 minutes; and 3). repairing the MH after PFCL was removed.Results:All nine eyes gained intraoperative retinal reattachment after PFCL tamponade for 22.22 +/- 8.01 minutes and removal of PFCL. Procedures for MH closure included internal limiting membrane peeling in eight eyes, with internal limiting membrane free flap insertion (four eyes), internal limiting membrane inverted flap insertion (two eyes), or lens capsular flap transplantation (three eyes). All eyes received C3F8 tamponade. During 9.11 +/- 3.89 months of follow-up, eight of the nine eyes (89%) achieved retinal reattachment and MH closure; one eye achieved anatomical success after reoperations. All eyes had vision improvement at the last follow-up.Conclusion:This new technique in pars plana vitrectomy may promote anatomical and functional recovery in the treatment of extensive retinal detachment secondary to a myopic MH.
Treatment of Early Giant Retinal Tears with Domestic Perfluorodecalin and Perfluoropropane.

Abstract:OBJECTIVE:To explore the methods of lens-sparing (without lensectomy), non-silicone oil tamponade and no scleral buckling for treatment of early giant retinal tears.METHODS:Thirteen cases of early retinal detachment of proliferative vitreoretinopathy (PVR) grade C2-D1, with giant tear extending from 135 degrees to 270 degrees were chosen. Transscleral cryotherapy was first applied to treat each end of the giant tear up to the oral serrate. And then conventional three-port pars plana vitrectomy was performed. Perfluorodecalin liquid was injected to manipulate the flap of the retinal tear. The flap with no cryotherapy before was treated with endolaser or cryotherapy under optimal visual condition, then air-fluid was exchanged completely and C3F8 was injected properly.RESULTS:With a mean follow-up of 8.2 months, the retina was reattached completely in 12 eyes. Success rate was 92.3%, and visual acuity improved. Most postoperative complications were slight. The retina failed to reattach only in 1 case with severe vitreous blood.CONCLUSIONS:In treating early retinal giant tears without severe PVR, the lens-sparing, non-silicone oil tamponade and no scleral bucking are helpful to simplify operation and to improve the success rate of the retinal surgery and to yield satisfactory outcome of visual acuity. Besides, it can also avoid the initial side effects of silicone oil tamponade and diopter irregularity after lensectomy.

Abstract:Objective To evaluate the effect of vitreoretinal surgery with lens sparing technique in treating the detachment with giant retinal tear(GRT) associated with proliferative vitreoretinopathy(PVR). Methods Thirty one consecutive eyes with GRT underwent vitrectomy were analysed retrospectively. Operative techniques included peeling of pre retinal membrane, injection of perfluorodecalin liquid, retinotomy and retinectomy,endolaser,and silicon oil or C 2F 6 gas tamponade. Lens sparing vitrectomy was performed in 28 phakic eyes. Follow up period ranged from 11 to 34 months. Results Anatomic retinal attachment was achieved intraoeratively in 29 eyes. In 16 eyes of 28 eyes with postoperative cataract formation,3 eyes underwent cataract surgery with or without intraocular lens implantation. The corrected final visual acuity ranged from 0.4 to 0.01. Conclusion Most phakic eyes of retinal detachment with GRT PVR can be successfully operated on with an outcome of improving the visual acuity by using techniques of lens sparing vitrectomy, perfluorodecalin liquid and silcone oil tamponade.
Air Versus Perfluoropropane Gas in Pars Plana Vitrectomy for Primary Rhegmatogenous Retinal Detachment: A 3-Year Retrospective Study.

Abstract:Introduction: This study aimed to compare anatomical outcomes of air and perfluoropropane gas (C3F8) tamponade in pars plana vitrectomy for the treatment of rhegmatogenous retinal detachment (RRD). Methods: In this retrospective study, data were gathered from 578 patients (578 eyes) with RRD. The follow-up records of all 578 patients that underwent primary vitrectomy for RRD with air or C3F8 were examined and analyzed. Surgical outcomes of the two groups were compared.Results: A total of 342 eyes were treated with air and 236 with C3F8. The mean follow-up period was 37.65 +/- 2.33 months. Baseline and preoperative clinical characteristics were similar between groups, but the period to intraocular bubble disappearance (P < 0.0001), intraocular pressure on the first postoperative day (P < 0.0001), number of cases with intraocular pressure > 21mmHg within three days post-surgery (P < 0.0001) and the number with intraocular pressure > 21mmHg during follow-up (P = 0.0002) differed significantly between groups. Primary reattachment rates for air and C3F8 groups were 95.03% and 95.34%, respectively. Clinical characteristics were similar in those with and without successful reattachment, and the frequency of new or unclosed breaks was similar between the two groups. There was no significant difference in two groups according to the presence or absence of inferior retinal breaks and inferior detached quadrants. Univariate and multivariate logistic regression identified no risk factor for surgical failure. Conclusions: Air showed equivalent effects to C3F8, with a shorter period to intraocular bubble disappearance, less risk of post-operative intraocular hypertension, and less expense.
Pars Plana Vitrectomy with Partial Tamponade of Filtered Air in Rhegmatogenous Retinal Detachment Caused by Superior Retinal Breaks

Abstract:Background To investigate the anatomic and functional outcomes of pars plana vitrectomy (PPV) with partial tamponade of filtered air for rhegmatogenous retinal detachment (RRD) caused by superior retinal breaks. Methods Retrospective, comparative, consecutive case series study. Patients with RRD caused by superior retinal breaks undergone PPV with partial tamponade (Group A) and whole tamponade (Group B) of filtered air were included. The main outcomes were primary and final success rates, best corrected visual acuity (BCVA), and rate of postoperative cataract surgery. Results Forty-one patients (41 eyes) were included in Group A and 36 patients (36 eyes) were included in Group B. There were no significant differences in primary or final success rates between Groups A and B ( P = 0.618 and P = 1.000, respectively). The patients in Group A experienced quicker postoperative vision improvement (from the Week 1 follow-up) than the patients in Group B (from the Month 3 follow-up). The postoperative cataract surgery rate of Group A (7/31) was lower than that of Group B (13/26) ( P = 0.031). Conclusions PPV with partial tamponade of air is effective in achieving a high anatomic success rate, quicker postoperative vision improvement, and lower rate of postoperative cataract surgery in RRD caused by superior retinal breaks.
AIR VERSUS GAS TAMPONADE IN RHEGMATOGENOUS RETINAL DETACHMENT WITH INFERIOR BREAKS AFTER 23-GAUGE PARS PLANA VITRECTOMY: A Prospective, Randomized Comparative Interventional Study.

Abstract:Purpose: To compare the efficacy of air and octafluoropropane (C3F8) in treating rhegmatogenous retinal detachments with inferior breaks after 23-gauge pars plana vitrectomy.Methods: A prospective, randomized comparative interventional study. Sixty-four patients with rhegmatogenous retinal detachment with inferior breaks underwent pars plana vitrectomy with air (32 eyes) or gas (32 eyes) tamponade. Anatomical and visual outcomes of the two groups were compared.Results: The mean follow-up period was 13.09 +/- 1.90 months. Significant differences were identified regarding prone positioning period (P < 0.01), intraocular pressure (P < 0.01), and gas volume (P = 0.03) on the first postoperative day. The single-operation success rates for the air and gas groups were 84.38% and 78.13% (P = 0.522), and the final surgery success rates increased to 100% and 96.88% (P = 0.313), respectively. The single-operation success rate between the groups was not statistically significant, even after adjustment for confounding factors. Multivariate logistic regression also indicated that the number of involved retinal quadrants (odds ratio = 19.88, P = 0.01) was an independent predictor of surgery failure. The only postoperative complication observed was new or missed breaks, which occurred in 12 patients (18.75%).Conclusion: Air had equivalent tamponade effects to C3F8, with a shorter prone positioning period, fewer complications, and less expense, in the surgical management of rhegmatogenous retinal detachment with inferior breaks.
Traumatic Retinal Detachment Associated with Giant Retinal Tear Treated with 25-Gauge Pars Plana Vitrectomy

Abstract:Objective To evaluate the efficacy of 25 G micro-invasive vitrectomy for the treatment of traumatic retinal detachment associated with giant retinal tear. Methods The data of 13 eyes of 13 patients with tarumatic retinal detachment associated with giant retinal tear underwent 25 G micro-invasive vitrectomy were analyzed retrospectively. The follow-up time was 6 months after operation or 2-3 months after removal of silicone oil. Visual acuity, anatomical success rate and complications were observed. Results At the last follow-up, the postoperative visual acuity was improved in 11 cases (84. 62%), unchanged in 1 case (7. 69%) and deteriorated in 1 case (7. 69%). The difference was statistically significant between before and after surgery (t=3. 604,P=0. 004). During the follow-up time, retinal reattachment was in 12 cases(92. 31%) and recurrent retinal detachment after silicone oil removal in 1 case (7. 69%). Two cases (15. 38%) complicated cataract and 2 cases ( 15. 38%) associated with proliferative vitroretinopathy ( PVR ) . No intraocular hypertension happened. Conclusion The surgery of 25-gauge micro-invasive vitrectomy for the treatment of giant retinal tear associated with retinal detachment can achieve excellent attachment rates and with less complications.
Vitrectomy with air tamponade and cryotherapy for retinal detachment repair without perfluorocarbon use: A UK 12-month prospective case series

Abstract:Introduction: Rhegmatogenous retinal detachment (RRD) is commonly managed by pars plana vitrectomy (PPV). Gas tamponade has significant clinical and quality of life factors that affect the patient. Using shorter acting air tamponade, intuitively leads to surgical concern for higher rate of primary detachment. To circumvent this, surgeons advocate the use of perfluorocarbon to maximise sub-retinal-fluid drainage when using air tamponade and/or avoid using cryotherapy due to longer duration for scar formation relative to laser. These steps are a deviation from traditional technique for otherwise routine primary RRD repair and discourages popularisation of using air tamponade despite the benefits to patients. Method: Prospective 12-month, single centre, single surgeon, consecutive study from January to December 2019. Inclusion criteria was as per based on the pneumatic retinopexy versus vitrectomy for retinal detachment: (i) single retinal break or group of breaks, no larger than 1-clock-hour (30°), in detached retina; (ii) all breaks in detached retina lie above the 8 to 4 o’clock meridian; and (iii) breaks or lattice degeneration in attached retina at any location. We excluded PVR formation and RRD in the inferior 4 o’clock hours. Results: We report primary and final success rate of 96% and 100%, respectively in 23 eyes (22 macular-on and one macular-off) with RRD eyes in a 12-month period. Cryopexy was utilised in 65% patients and in isolation in 23% with no perfluorocarbon use in all eyes. Conclusion: We show cryotherapy can be used safely in AT-RRD without PFCL in RRD fulfilling PIVOT trial criteria with minimal cataract formation and IOP spikes and faster post-operative visual rehabilitation.

[Application of Perfluorocarbon Liquids in Vitrectomy].

Abstract:OBJECTIVE:To popularize the use of perfluorocarbon liquids in vitrectomy and increase the cure rate of complicated retinal detachment.METHODS:The clinical application of perfluorocarbon liquids during vitrectomy for complicated retinal detachment of 76 eyes has been summarized. The postoperative retinal detachment rate and visual acuity of 54 eyes with retinal detachment complicated with severe proliferative vitreoretinopathy and 11 eyes with giant retinal tears were analyzed statistically.RESULTS:The retina was reattached in 71 of 76 eyes (93.4%), in 50 of 54 eyes with severe PVR (92.6%) and in 9 of 11 eyes with giant retinal tears (81.8%). The postoperative visual acuity was above 0.02 in 69 eyes (90.8%) and > 0.1 in 29 eyes (38.2%) of 76 eyes, respectively in 50 (92.6%) and 18 (33.3%) of 54 eyes with severe PVR and 9 (81.8%) and 4 (36.4%) of 11 eyes with giant retinal tears. During surgery, perfluorocarbon entered through tears into subretinal space and was removed immediately in 6 of 76 eyes. Postoperatively, a small portion of residual perfluorocarbon liquid was observed in the anterior chamber of 17 of 76 eyes and in the vitreous cavity of 2 phakic eyes of 76 eyes, no complication was observed except one case with mild corneal opacification.CONCLUSION:Perfluorocarbon liquids are the useful adjunct to the hydrokinetic manipulation in vitreous surgery.
Non-Buckled Vitrectomy for Retinal Detachment with Inferior Breaks and Proliferative Vitreoretinophathy

Abstract:AIM:To investigate the efficacy of non-buckled vitrectomy with classical endotamponade agents in the treatment of primary retinal detachment (RD) complicated by inferior breaks and proliferative vitreoretinophathy (PVR).METHODS:A retrospective, consecutive and case series study of 40 patients with inferior break RD and PVR ≥C1 was conducted. All patients underwent a standard 3-port 20-gauge pars plana vitrectomy (PPV) with gas or silicone oil tamponade without supplementary scleral buckling. The vitreous and all proliferative membrane were completely removed, and retinectomy was performed when necessary. The mean follow-up was 12.5 months. The primary and final anatomic success rate, visual acuity and complications were recorded and analyzed.RESULTS:Primary anatomic success rate was achieved in 35 of 40 eyes (87.5%) and the final anatomic success rate was 100%. The most common cause of redetachment was recurrent PVR. The best-corrected visual acuity (BCVA) at final follow-up was improved in 34 eyes (85%), remained stable in 1 eye (2.5%), and worsened in 5 eyes (12.5%). The mean visual acuity at final follow-up was improved significantly (P=0.000).CONCLUSION:This retrospective study provides evidence that vitrectomy without scleral buckling seemed to be an effective treatment for inferior break RD with PVR. With complete removal of vitreous and proliferative membranes and timing of retinectomy, the inferior breaks which complicated with PVR could be closed successfully without additional scleral buckling.

Abstract:To compare the efficacy between intravitreal gas injection alone and pars plana vitrectomy (PPV) with intraocular gas tamponade in treating retinal detachment due to macular hole in high myopes.Prospective, randomized controlled, multicenter study.We included 231 eyes of 231 patients with retinal detachment owing to myopic macular hole from 14 study sites in China.The patients were randomly assigned to undergo intravitreal perfluoropropane (C3F8) gas injection (group A) or vitrectomy with intraocular C3F8 gas tamponade (group B).Complete ocular examination included best-corrected visual acuity, applanation tonometry, slit-lamp biomicroscopy, color fundus photography, ultrasound examination, and optical coherence tomography at baseline and every 3 months after the surgery.At 12 months after surgery, the retinal reattachment rates was 59.8% (52 eyes) in group A, which was significantly lower than group B (79 eyes, 74.5%; P = 0.029). Logistic regression showed that PPV (P<0.001) and low height of retinal detachment at baseline (P = 0.068) were associated with a greater retinal reattachment rate. Subgroup analysis revealed that in group B, concomitant internal limiting membrane (ILM) peeling showed a slightly higher anatomic success rate (79.2%) than no ILM peeling (70.0%). The postoperative best-corrected visual acuity of the eyes with attached retina between group A and B was not significantly different (P>0.05).Pars plana vitrectomy with C3F8 endotamponade resulted in a higher anatomic success rate than intravitreal C3F8 gas injection alone in treating retinal detachment owing to macular hole in high myopes.The authors have no proprietary or commercial interest in any materials discussed in the article.
Pars Plana Vitrectomy and Wide Internal Limiting Membrane Peeling with Perfluoropropane Tamponade for Highly Myopic Foveoschisis-Associated Macular Hole

Abstract:Purpose: To evaluate the outcomes of pars plana vitrectomy and wide internal limiting membrane peeling with perfluoropropane tamponade (C3F8) for highly myopic foveoschisis-associated macular hole.Methods: Prospective consecutive cases with highly myopic foveoschisis-associated macular hole were recruited. All the patients underwent pars plana vitrectomy and indocyanine green-aided internal limiting membrane peeling, the range of internal limiting membrane peeling was broadened beyond the vascular arcades and to the nasal side of the optic disc. With tamponade of 16% C3F8, all the patients kept a face-down position post-operatively. The main outcomes were best-corrected visual acuity and primary anatomical success rate.Results: There were 25 patients (25 eyes) included, with a mean age of 54.8 +/- 7.2 years. The mean follow-up duration was 14.2 +/- 3.9 months. The mean preoperative logarithm of the minimum angle of resolution best-corrected visual acuity (Snellen equivalent) was 1.489 +/- 0.558 (20/617). The mean postoperative logarithm of the minimum angle of resolution best-corrected visual acuity at the final follow-up was 0.882 +/- 0.522 (20/152), the difference being statistically significant with the preoperative one (P < 0.001). At the final follow-up, 17 eyes (68.0%) had vision improvement, and the primary anatomical success rate was 84.0% (21/25).Conclusion: Pars plana vitrectomy and wide internal limiting membrane peeling with C3F8 tamponade is effective and safe to achieve a high anatomical success rate of highly myopic foveoschisis-associated macular hole closure and regain visual function.

Abstract:Introduction Rhegmatogenous retinal detachment (RRD) with extensive choroidal detachment (CD), suprachoroidal fluid (SCF), and lower intraocular pressure (IOP) markedly increase the difficulty of pars plana vitrectomy (PPV) because it makes the insertion and placement of the trocar cannulas for PPV time-consuming and technically challenging. In this study, we developed a systemic approach that avoids the risks of sclerotomy or using a longer trocar to help in the safe placement of the trocar cannulas in order to prevent inadvertent fluid/air perfusion during PPV in RRD eyes with extensive CD, SCF, and hypotony. Methods This is a prospective case series study. Consecutive patients diagnosed with RRD with extensive CD, SCF, and hypotony were recruited. The main steps included injecting transconjunctival intravitreal fluid to increase IOP and enable the passive egress of SCF into the subconjunctival space, creating and broadening the inner opening for the trocar cannulas, transillumination with an endoillumination light pipe, and indentation of the infusion cannula to create maximum visual exposure of the inner segment and help peripheral shaving around the infusion tip. Main outcomes: the success rate of the infusion placement into the vitreous cavity, surgical duration, intra- and postoperative complications, and functional and anatomical outcomes. Results Thirty-eight patients (38 eyes) were included. The first-time success rate of cannula placement into the vitreous cavity was 100%, with a mean surgical duration of 8.5 ± 1.8 min. No intra- or postoperative complications related to the approach were observed. Significant vision improvement was noted in 68.4% (26/38) of the patients at the final follow-up. The primary and final retinal reattachment rates were 94.7% (36/38) and 100% (38/38), respectively. Conclusion This systemic approach is a feasible option to ensure the safe placement of trocar cannulas and prevent inadvertent perfusion during small-gauge PPV in RRD eyes with extensive CD, SCF, and hypotony, without the need for a draining sclerotomy, a longer trocar, or a sharp clearing of the cannula tip.

Abstract:Purpose: To analyze the visual and anatomical outcomes for eyes with rhegmatogenous retinal detachment (RRD) and advanced proliferative vitreoretinopathy (PVR) undergoing giant peripheral retinotomy (GPR) using 25-gauge pars plana vitrectomy (PPV). Methods: In this retrospective multi-center study, patients with RRD with either anteroposterior or circumferential retinal shortening and advanced PVR requiring more than 90-degree GPR with/without relaxing retinotomy were included. Subjects of either gender, any age group, and with complete surgical notes were included. Outcome measures of the study included anatomical success (i.e. complete retinal re-attachment) at 6 months using survival analysis, visual outcomes, and post-operative complications. Results: Forty-one eyes of 41 patients (33 males) with a mean age of 44.9 ± 21.4 years were included. At 6 months follow-up, anatomical success was seen in 29 eyes (70.7%) with a cumulative re-attachment rate of 66% (95% confidence interval = 48 = 79%). All re-detachments occurred at ≤6 months with a peak at 4–6 months (n = 9). Twenty-three eyes (56%) achieved ambulatory vision (5/200) or better. Direct perfluorocarbon liquid-silicone oil exchange was performed in 20 eyes. Intra-operative complications included persistent retinal folds (2 eyes), subretinal air (1 eye), and subretinal bleed (1 eye). Eleven eyes (26.8%) developed secondary glaucoma (2 eyes required a drainage device), and hypotony of ≤6 mmHg was noted in 3 eyes (7.3%). Corneal decompensation was noted in 8 eyes (19.5%), and 3 eyes (7.3%) underwent re-surgery for re-RRD. Conclusion: After GPR using small gauge PPV, two-thirds achieve anatomical success, and over half have ambulatory vision, but overall post-operative complications can occur in more than half of the eyes.

Abstract:Objective To observe the outcomes of 23G pars plana vitrectomy (PPV) and air tamponade for non-inferior rhegmatognous retinal detachment (RRD).Methods A prospective case series study.From August 2017 to April 2018,39 consecutive RRD patients (39 eyes) in Department of Ophthalmology of Subei People's Hospital Affiliated to Yangzhou University were enrolled in this study.There were 20 males (20 eyes) and 19 females (19 eyes),23 right eyes and 16 left eyes,with the mean age of 55 ± 11 years.There were 30 eyes with lens and 9 eyes without lens or IOL.There were 21,14 and 4 eyes with 1,2 and equal or greater than 3 retinal tear respectively.All patients underwent 23G PPV which performed preretinal proliferative membranes and vitreous cortex removal,photocoagulation around the breaks with 3-5 rows,and filtered air tamponade.The follow-up was more than 2 months.The retinal reattachment,visual acuity and complications were observed.Pearson correlation analysis was used to analyze the correlation between BCVA and disease course.Chi-square test was performed for comparison among retinal reattachment rate and different clinical factors before operation.Results At 2 months after the PPV,35 eyes' retina reattached,the rate of reattachment was 89.8％.In 2-3 weeks,4 eyes were re-detached,all of them performed silicone oil tamponade.One eye was secondary to pre-macular membrane.The logMAR BCVA before and after PPV were 1.15 ± 0.78 and 0.41 ± 0.31,respectively (t=6.589,P=0.0001).Correlation analysis results showed that BCVA after surgery was positively correlated with BCVA before surgery (r=0.544,P=0.001).Twelve of 30 eyes with crystalline lens suffered cataract.The rate of reattachment vary in the number of the breaks (x2=9.181,P=0.010).Conclusion PPV with air tamponade may be an optimal treatment of non-inferior RRD with better success rate and security.
