Abstract:A healthy 61-year-old female patient presented for evaluation of a medial canthal pigmented lesion that had been growing larger for the past 2 years. She denied itching, bleeding, or pain. A dermatologist had noted brown and red papules on her chest thought to be seborrheic keratoses, benign nevi, and cherry angiomas. Examination revealed a flat 3 × 3-mm hyperpigmented lesion with irregular margins (Figure, A). Histopathology from a 2-mm punch biopsy demonstrated atypical keratinocytes spanning the entire thickness of the epidermis without invasion of the dermis, leading to a diagnosis of pigmented squamous cell carcinoma in situ (Figure, B). Pigmented squamous cell carcinoma is a rare variant of squamous cell carcinoma. It has been described on the face, conjunctiva, esophagus, and oral and genital mucosa. 1 -3 The patient is currently planned for Mohs surgery and oculoplastic reconstruction.

Abstract:A 69 year-old female patient presented with a reddish-brown, enlarging lesion on her right lower eyelid. Physical exam revealed a 4-5mm, oval-shaped, reddish-brown, shiny, exophytic papule on the right lower eyelid. Shave biopsy of the lower eyelid papule was performed, which revealed nests of cells with prominent cribriform arrangements, focally surrounded by small pools of blue stringy mucinous material and papillary architecture. Cells stained positive for EMA, synaptophysin, and chromogranin, which confirmed the diagnosis of endocrine mucin-producing sweat gland carcinoma (EMPSGC). The lesion was fully removed with negative margins via Mohs micrographic surgery. Endocrine mucin-producing sweat gland carcinoma (EMPSGC) is a rare, low-grade, cutaneous adenocarcinoma that typically arises on the eyelids of elderly women. EMPSGC may be associated with invasive mucinous carcinoma.

Abstract:We report a case of conjunctival intraepithelial neoplasia (CIN) in a patient presenting with the pigmented conjunctival lesion. This study involved a 56-year-old woman that presented with right eye irritation for 1 month. She noticed brownish pigmentation arising from her right nasal conjunctiva and growing slowly over time. Biomicroscopic examination showed a gelatinous pigmented conjunctival mass with feeder vessels. Conjunctival impression cytology (CIC) was done and reported as CIN. Treatment was started with 0.02% mitomycin-C eye drops. The conjunctival lesion responded well to medication. This report shows that CIN can manifest as a pigmented tumor, resembling melanoma. CIC plays a role in the diagnosis of this condition. This tumor responded well with 0.02% mitomycin-C eye drops.

Abstract:Backgrounds/aims: The authors describe the case of a 79-year-old Caucasian woman who presented an ocular adnexal lesion as the first clinical manifestation of a systemic follicular lymphoma, highlighting the clinicopathological features of this rare entity and its potential to be misdiagnosed as marginal zone lymphoma of the mucosa-associated lymphoid tissue. Methods: Conjunctival impression cytology was performed for a rapid initial diagnosis of lymphoma, and subsequent histopathological and immunohistochemical studies were carried out for its confirmation and to identify the best therapeutic regimen. Results: After the initial presentation and diagnosis, she was submitted to complete clinical evaluation; confluent retroperitoneal lymphadenopathy was detected through abdominal computed tomography, characterizing clinical stage III. Conclusion: Awareness of this lymphoma is important when making a diagnosis of ocular adnexal lymphoid neoplasms for its appropriate evaluation and management.

Abstract:We describe a 41-year-old Asian male who presented with a left upper eyelid mass for one year associated with multiple pigmented maculopapules on his face, who was found to have Epidermodysplasia verruciformis (EV). The ophthalmic examination was unremarkable. The patient spontaneously started to have disseminated, bean-sized, pinkish flat papules on his forehead since childhood and had history of multiple non-melanoma skin carcinoma. He had been treated as possible xeroderma pigmentosum (XP) by dermatologists. The oculoplastic surgeon resected the lesion and submitted tissue for histopathological and genetic analysis. Eyelid biopsy was consistent with a squamous cell carcinoma (SCC). Next-generation Sequencing (Sanger sequencing) suggested the homozygous mutation of TMC8 gene which is indicative of EV. Our case is a rare report in eyelid SCC in the setting of EV. This patient highlights the importance of considering systemic genodermatosis rather than individual eyelid malignancy and the function of molecular diagnosis in clinical challenge case.

Abstract:Ocular adnexal lymphoma may involve the eyelids, conjunctiva, orbital tissue, or lacrimal structures. The majority are non-Hodgkin's B-cell lymphomas of mucosa-associated lymphoid tissue (MALT) lymphoma type. Follicular lymphomas represent a small percentage of ocular adnexa lymphomas, particularly in Japan. We report a 68-year-old female patient who presented with a salmon pink patch-like lesion of the left conjunctiva, suspected of being (MALT) lymphoma. However, histologic and immunohistologic examinations were consistent with follicular lymphoma. This case demonstrates the importance of considering such rare lymphomas when making a diagnosis of ocular adnexal lymphoid neoplasms. [J Clin Exp Hematop 53(1): 49-52, 2013].
Pseudo-pigmented choroidal schwannoma with extraocular extension in an elderly patient

Abstract:Purpose: To describe a rare case of clinically pigmented choroidal schwannoma with extraocular extension in an elderly patient. Observations: We report a case of a 79-year-old Black male who presented with a clinically pigmented, juxtapapillary choroidal tumor with concern for extraocular extension on standardized ocular echography. The lesion was concerning for choroidal melanoma with extraocular extension. He had no history of cancer or known genetic disease. After an extensive discussion of management options, he underwent enucleation of the left eye. Histopathology of the tumor showed non-pigmented cells with bland, spindle-shaped nuclei and areas of Antoni A pattern along with immunostaining consistent with choroidal schwannoma with extraocular extension. Conclusion: Choroidal schwannoma can mimic choroidal melanoma and can have a clinically pigmented appearance particularly in darkly complected individuals. This case highlights the importance of including choroidal schwannoma on the differential diagnosis for choroidal neoplasms, particularly in populations of individuals in whom melanoma is less common. Though more common in younger patients, choroidal schwannoma can present in elderly patients.

Unilateral amelanotic conjunctival malignant melanoma: a case report

Abstract:Introduction: Cutaneous malignant melanomas rarely occur in the eye, usually in the eyelids or the conjunctiva. Conjunctival malignant melanomas are even rarer. Most melanomas are dark in color as they are pigmented. However, amelanotic conjunctival malignant melanomas, a scarce variant of the cancer, can be challenging to diagnose accurately. Case presentation: We present two cases of white Caucasian Swedish-born women who were diagnosed with unilateral amelanotic malignant melanoma in the conjunctiva of the eye. In the first case, the patient was an 81-year-old woman who was suffering from redness and foreign body sensation in the left eye. The initial diagnosis was blepharitis. Three biopsies were taken, which showed malignant melanoma in the eyelid and the conjunctiva. Unfortunately, the eye and the rest of the orbit could not be saved, and the patient had to undergo an orbital exenteration. In the second case, the patient was a 50-year-old woman, and the tumor was localized in the temporal conjunctiva of the left eye. The initial diagnosis was pinguecula, but at the time of surgery, the physician suspected conjunctival intraepithelial neoplasia. The tumor was not completely removed, so adjuvant brachytherapy and local chemotherapy were used. The eye was preserved. No neck and/or lung metastasis was detected in either case at the time of diagnosis. Conclusions: Conjunctival amelanotic malignant melanomas should be suspected when tumors are present in the eye and/or the eyelids. By suspecting amelanotic malignant melanoma, the delay in treatment can be shortened. Treating them as soon as possible is essential to minimize the risk of metastasis.

Abstract:AIM: To investigate the clinical characteristics and prognosis of patients with malignant eyelid tumors.METHODS: This was a retrospective, non-randomized, clinical reviews. Between January, 2002 and December, 2011, 75 cases with histologically confirmed malignant eyelid tumors were evaluated. Patients' charts were reviewed for clinical information, treatment procedure, and disease course. Survival analysis in terms of recurrence-free survival was performed using age, sex, location of tumor and histopathological type. The follow-up ranged from 1 to 78 months (mean=21 months).RESULTS: The 75 eyelid tumors included 35 basal cell carcinoma (BCC, 46.7%), 22 sebaceous gland carcinoma (SGC, 29.3%), 7 squamous cell carcinoma (SCC, 9.3%), 10 malignant melanoma (MM, 13.3%), and 1 Merkel cell carcinoma (MCC, 1.3%). Recurrence developed in 17 cases (22.7%). The recurrence rate of BCC (4/35, 11.4%) was significant lower than MM (6/10, 60.0%, P<0.001). The mean interval of recurrence was 21 months (range 362) for all eyelid tumors. Tumor located at canthus had higher recurrence rate (50%) compared with those located at eyelid (19%, P<0.05). Histological type was independent variable for recurrence by Cox regression analysis.CONCLUSION: It is important to achieve a negative tumor margin in canthus located malignant eyelid tumor. Clinicians should have a high level of suspicion for recurrence according to histological type when treating patients with eyelid tumor.
A rare case of unilateral diffuse melanocytic proliferation

Abstract:A 67-year-old woman presented with metamorphopsia in the right eye. Leopard mottling was seen temporal to the fovea oculus dexter with corresponding hyper- and hypo-autofluorescent lesions on fundus autofluorescence. Spectral domain-optical coherence tomography revealed hyperreflective dots in the retinal pigment epithelium and choroid with subretinal fluid (SRF). Intravitreal bevacizumab was administered with which SRF resolved, albeit with increase in the areas of mottling. The patient was diagnosed to have metastatic ductal carcinoma of the right breast. It is important to bear in mind that the well-known entity of bilateral diffuse uveal melanocytic proliferation can rarely present unilaterally.

Abstract:Purpose: The demonstration of a rare case of bilateral diffuse uveal melanocytic proliferation (BDUMP) due to a lung carcinoma with unusual dermal lesions. Case description: A 76-year-old man with painless bilateral vision loss revealed leopard or giraffe spot chorioretinopathy and bilateral serous retinal detachment. Ultrasound biomicroscopy revealed uveal swelling expanding into the anterior chamber angle. Dermal and mucosal lesions were present on the lip, breast, groin, scrotum, and penis. Screening analyses revealed a non-small cell lung carcinoma. Conclusions: The diagnosis of BDUMP, a rare paraneoplastic syndrome, was made. The ophthalmological diagnosis led to screening investigations and revealed the underlying malignant disease. Uncommonly, multiple dermal and mucosal lesions could be detected and were analyzed histopathologically.

Abstract:A 16-year-old girl born from a nonconsanguineous marriage presented to the dermatology out-patient department with innumerable hyperpigmented macules predominantly over the photoexposed parts of her body involving the face, neck, chest, back, of forearms, hands, and legs with a history of photosensitivity and photophobia (Figure 1). These were interspersed with multiple depigmented macules over her arms and legs since, she was three years old. She also had a small pigmented mass involving the left eye present for the last 3 months. She had a family history of similar cutaneous lesions in her elder sister. There was no history of hearing loss, seizures, spasticity, or cognitive impairment.
Advances in multimodal imaging for diagnosis of pigmented ocular fundus lesions

Abstract:Pigmented ocular fundus lesions can range from benign to malignant. While observation is reasonable for asymptomatic benign lesions, early recognition of tumours that are vision or life threatening is critical for long-term prognosis. With recent advances and increased accessibility of multimodal imaging, it is important that providers understand how to best use these tools to detect tumours that require early referral to subspecialty centres. This review aims to provide an overview of pigmented ocular fundus lesions and their defining characteristics using multimodal imaging. We cover the spectrum of pigmented ocular fundus lesions, including freckle and focal aggregates of normal or near-normal uveal melanocytes, retinal pigment epithelium (RPE) hyperplasia, congenital hypertrophy of the RPE, RPE hamartoma associated with familial adenomatous polyposis, congenital simple hamartoma of the RPE, combined hamartoma of the retina and RPE (congenital hypertrophy of the RPE), choroidal nevus, melanocytosis, melanocytoma, melanoma, adenoma, and RPE adenocarcinoma. We describe key diagnostic features using multimodal imaging modalities of ultra-widefield fundus photography, fundus autofluorescence, optical coherence tomography (OCT), enhanced-depth imaging OCT, ultrasonography, fluorescein angiography, indocyanine green angiography, and OCT angiography (OCTA), with particular attention to diagnostic features that could be missed on fundus examination alone. Finally, we review what is on the horizon, including applications of artificial intelligence. Through skilled application of current and emerging imaging technologies, earlier detection of sight- and life-threatening melanocytic ocular fundus tumours can lead to improved patient prognosis.

Abstract:European Journal of Ophthalmology, Ahead of Print. PurposeTo report an atypical presentation of an epibulbar simple cartilaginous choristoma with a unique pigmented multicystic component.Case DescriptionA 69-year-old African American female presented for evaluation of a right nasal epibulbar lesion that had progressed over the prior year. Slit-lamp evaluation revealed an immobile, mildly pigmented multicystic lesion measuring 6.0 × 4.5 mm that involved the nasal bulbar conjunctiva and the plica semilunaris. The lesion appeared benign, without feeder vessels or features of epithelial dysplasia. Given its recent growth and the patient's cosmetic concerns, the lesion was excised with ocular surface reconstruction. Histopathological evaluation disclosed a well-circumscribed nodule of well-differentiated cartilage in the substantia propria, consistent with a simple cartilaginous choristoma. The overlying conjunctival stroma contained multiple cysts lined by focally pigment epithelium. The patient recovered well from surgery, with satisfactory cosmetic results.ConclusionsOur case of epibulbar simple cartilaginous choristoma includes a prominent superficial component of pigmented epithelial cysts, which has not been previously reported in the literature. This augments our knowledge on the spectrum of presentations of cartilaginous choristomas and underscores the importance of histopathological evaluation for definitive diagnosis.

UNILATERAL DIFFUSE UVEAL MELANOCYTIC PROLIFERATION

Abstract:Purpose: To report a unilateral case of what is named bilateral diffuse uveal melanocytic proliferation and consider the consequences of this finding. Methods: The ocular findings were investigated with multimodal imaging to include color fundus photography, fluorescein angiography, autofluorescence imaging, and enhanced depth imaging optical coherence tomography. Results: A 66-year-old woman had a history of breast cancer 23 years previously that was treated and the patient was free of disease since. She developed a recent decrease in visual acuity in her left eye prompting referral. She was seen to have an alteration in the pigmentation of the posterior pole of the left eye with dispersed red placoid spots. Autofluorescence imaging showed nummular areas of absent autofluorescence signal, which corresponded to areas of hyperfluorescence during fluorescein angiography. The placoid spots were hyperautofluorescent and hypofluorescent in autofluorescence and fluorescein angiography, respectively. She had diffuse thickening and infiltration of the choroid in the left eye. Because of the ocular findings, the patient underwent a systemic evaluation and was found to have widely metastatic disease with an unknown primary cancer. No progression of disease was seen in the left eye over a 6-month follow-up, and the right eye never showed any abnormality, except for a modest cataract, by any means of examination. Conclusion: Bilateral uveal melanocytic proliferation has been attributed to a paraneoplastic process, allegedly from a factor in the IgG fraction of the serum. However, unilateral involvement suggests that there are other factors involved in disease manifestation.
