Abstract:Purpose: Osteoarthritis (OA) is a multifaceted disease that involves pathological changes of multiple tissues in the joint. Most studies on cartilage matrix in OA have focused on changes of glycosaminoglycan and collagen content; very little is known about the alteration of collagen architecture in OA. However, the complex organization of collagen fibers in all layers of articular cartilage requires highly quantifiable imaging tools for visualization and analysis. One such tool is Second Harmonic Generation (SHG) imaging, which is a powerful two-photon confocal imaging technique ideal for imaging non-centrosymmetric molecules, such as collagen fibers. The purpose of this study is to use the SHG imaging method to evaluate collagen fiber thickness and orientation changes in early mouse experimental OA and to correlate these changes with synovial inflammation and joint destruction. Methods: OA was induced in NFκB-luciferase reporter mice (Taconic) via the destabilization of the medial meniscus (DMM) surgery. Mice were then subjected to three types of analysis between 1–10 weeks post-surgery: 1) SHG imaging. Collagen thickness and orientation were visualized using SHG imaging on cryosections. Fiber thickness was calculated as the ratio of forward to backward SHG signal (FSHG/BSHG), and directional orientation was calculated as variance of fiber orientation with respect to each other. 2) Histological analysis. Cartilage damage revealed by safranin O staining was assessed by the modified Mankin system. IHC was used to assess collagen content and macrophage presence. Synovitis in the anterior and posterior compartments of the joint were scored. 3) In vivo imaging of bioluminescence signals for NFκB activity to assess whole joint inflammation. Luciferin (25 μg) was injected intraarticularly, and mice were imaged with the IVIS Biophotonic 200 imager. As controls, sham and non-surgery mice were analyzed. Non-parametric data from Mankin scoring and synovitis analysis were evaluated via Kruskal-Wallis analysis followed by Dunn’s multiple comparison test. All other experiments were evaluated with ANOVA followed by Bonferroni’s multiple comparison test. Spearman’s correlation was performed for correlation analysis. P < 0.05: considered statistically significant. Results: Zonal differences in collagen fiber thickness and organization were observed as soon as OA initiated after DMM surgery, which persisted over time (Fig. A–C). Even at 1 week-post surgery, the DMM joint showed a decrease in collagen fiber thickness in the radial zone and an increase in collagen fiber disorganization in the superficial zone. Interestingly, in the radial zone, where the DMM joint showed decreased directional variance relative to the Sham joint, suggesting that the collagen fibers in the radial zone became more organized in OA. Bioluminescence imaging data showed that DMM and Sham control joints had a similar surge of inflammation at 1-week post-surgery, but the DMM joint exhibited a delay in resolution of inflammation in subsequent weeks. A similar trend was observed with synovitis, which we found to be mainly driven by synovial cell density and inflammatory infiltration rather than synovial thickness (data not shown). Interestingly, we found a correlation between the trend of collagen fiber thickness changes in the deep zone with that of synovitis in both the DMM and Sham knees; however, DMM surgery leads to an overall shift toward higher synovitis and thinner collagen fibers. Additionally, the trend of collagen directional variance in the superficial zone followed that of the Mankin score during OA progression (Fig. D). Conclusions: SHG imaging is a powerful approach that can be used to quantitatively analyze collagen landscape throughout articular cartilage. Combining SHG imaging with bioluminescence imaging and histological approaches, we uncovered a coordinated change between different components of cartilage matrix and inflammation in OA progression.
