Abstract:Benign paroxysmal positional vertigo (BPPV) is a disorder originating in the inner ear and is very uncomfortable for a patient, but it can be managed efficiently and effectively. It may be an uncommon complication after certain jaw bone related surgeries, such as an osteotome sinus floor elevation (OSFE). OSFE allows ridge augmentation and immediate implant placement to be completed concurrently. The procedure can occasionally cause membrane perforations or postoperative infections, but the rare complication, BPPV, happens if continuous malleting is applied. Immediate identification and proper management can eradicate discomforts resulting from BPPV. Possible preventive strategies, diagnostic examinations, and available treatment modalities for this rare sequela are discussed. Here we present the case of a patient who suffered from BPPV several hours later after OSFE and obtained relief through some medical aids and physiotherapeutic techniques.
Rhabdomyolysis After the Use of Percussion Massage Gun: A Case Report

Abstract:Objective: Percussion massage guns are commonly used by professional athletes and nonathletes worldwide for warm-up and physical recovery; however, there are no published clinical or evidence-based reports on percussion guns regarding their benefits, indications, contraindications, and even side effects. The purpose of this case report is to describe the first case of rhabdomyolysis as a severe and potentially life-threatening illness following use of a percussion gun. Methods: A young Chinese woman with untreated iron deficiency anemia presented with fatigue and pain in her thigh muscles for 3 days and tea-colored urine for 1 day, after cycling and subsequently receiving percussion gun treatment by her coach for the purpose of massage and relaxing tired muscles. Muscle tenderness and multiple hematomas were found on her thighs, and her urinalysis indicated hemoglobinuria. Her serum creatine kinase was reported as "undetectably high," a hallmark of serious muscle damage leading to a diagnosis of severe rhabdomyolysis. Aggressive intravenous fluid resuscitation, urine alkalinization via intravenous alkaline solution, assessment of urine output, and maintenance of electrolyte balance were administered during hospitalization. Results: The patient's clinical presentation gradually improved with the decline of creatine kinase, and she recovered well during follow-up. Conclusion: A case of severe rhabdomyolysis after percussion massage should alert caregivers, sports professionals, and the public to suspect and recognize the potentially serious adverse effects of percussion guns and to ensure that percussion massage guns be used appropriately and safely in rehabilitation therapy, especially in individuals with an underlying disease or condition. Research is needed to examine the benefits, indications, contraindications, and adverse reactions of percussion guns.
Benign Paroxysmal Positional Vertigo in Spontaneous Intracranial Hypotension.

Abstract:Objectives: To assess the prevalence and related factors of benign paroxysmal positional vertigo (BPPV) in patients with spontaneous intracranial hypotension (SIH). Methods: We retrospectively reviewed 156 consecutive inpatients with SIH, and collected the clinical and radiological data. These patients were divided into BPPV group and non-BPPV group according to the clinical manifestation and the results of Dix-Hallpike or supine roll tests during hospitalization period. We performed a univariate analysis and a further multiple logistic regression analysis to identify the related factors of the development of BPPV in SIH patients. Results: BPPV was detected in 18 patients among the total 156 SIH patients (11.54%). The univariate analysis showed a low cerebrospinal fluid (CSF) pressure (P = 0.018), a small pontomesencephalic angle (P = 0.012) and a positive venous distension sign (VDS) (P = 0.045) were associated with the presence of BPPV. But the multivariate analysis only demonstrated a low CSF pressure was related to the presence of BPPV (OR = 1.022, 95% CI: 1.001-1.043, P = 0.044). Conclusion: BPPV is common in SIH patients. SIH patients with low CSF pressure may be prone to develop BPPV.
Risk Factors for Residual Dizziness in Patients Successfully Treated for Unilateral Benign Posterior Semicircular Canal Paroxysmal Positional Vertigo.

Abstract:Objective The risk factors for residual dizziness (RD) after successful treatment of benign paroxysmal positional vertigo (BPPV) are poorly characterized. We determined the risk factors for RD in patients with benign unilateral posterior semicircular canal paroxysmal positional vertigo (pc-BPPV) after successful treatment. Methods We conducted a prospective study of patients diagnosed with unilateral pc-BPPV between March 2015 and January 2017. Bone mineral density (BMD) was measured by dual-energy X-ray bone mineral densitometry. Participants underwent bithermal caloric testing (C-test) using videonystagmography and a canalith repositioning procedure (CRP). The occurrence of RD was the primary outcome. The participants underwent follow-up 1 week, 1 month, and 1 year after successful CRP, consisting of outpatient visits, questionnaires, and telephone interviews. Results We assessed 115 participants with unilateral pc-BPPV (31 men and 84 women) who were 53.2 ± 8.8 years old. RD occurred in 60 (52.2%) participants. The participants who experienced RD were older, had vertigo for longer before treatment, and were more likely to show a positive C-test and significant BMD loss. Conclusions We found that a significant reduction in BMD (T-score < −1 standard deviation), a positive C-test, and older age are independently associated with RD in patients with pc-BPPV after successful CRP.

Abstract:Context: Handheld percussive massage devices (ie, massage guns) are a relatively new and under-researched recovery tool. These tools are intended to increase range of motion and reduce muscle soreness by delivering targeted vibration to soft tissues. Empirical knowledge about the potential influence of these devices on perceptual recovery and the recovery of performance characteristics after exercise is scarce. Objective: To investigate the effect of a 5-minute massage gun application, using a commercially available device, on physical and perceptual recovery after a strenuous bout of lower body exercise. Design: Controlled laboratory study. Setting: Physiology laboratory. Patients or other participants: A total of 65 active young adults (age = 21.3 ± 1.4 years; age range = 18-30 years; 34 women: height = 165.8 ± 6.1 cm, mass = 66.0 ± 7.4 kg; 31 men: height = 181.1 ± 6.0 cm, mass = 81.5 ± 11.8 kg). Intervention(s): Participants applied a massage gun on the calf muscles of 1 leg after strenuous exercise (massage gun recovery group) for 5 minutes and used no recovery intervention on the other leg (control group). Main outcome measure(s): Ankle range of motion, calf circumference, isometric strength, calf endurance, and perceived muscle soreness measures were collected at baseline and at various points after lower body exercise. Results: No significant group × time interactions were recorded for any of the performance or perceptual measures (P values > .05). Effect sizes were mostly unclear, except for a small increase in perceived muscle soreness in the massage gun recovery group compared with the control group immediately (d = -0.35) and 4 hours (d = -0.48) postrecovery. Conclusions: Massage guns appeared to have little effect on physical measures when applied for 5 minutes immediately after strenuous calf exercise. Given the small increase in muscle soreness up to 4 hours after their use, caution is recommended when using massage guns immediately after strenuous lower body exercise.

Abstract:Vertebral artery dissection (VAD) is a common cause of stroke in middle-aged individuals. Patients with VAD usually describe a trivial minor neck trauma preceding the event. Such traumas may be associated with spinal manipulation or sudden movements of the neck. Our case is a 43-year-old lady who presented with a history of sudden-onset dizziness, dysarthria, nausea/vomiting, tinnitus, and imbalance. Two days prior to her presentation, she experienced a new-onset moderate to severe intensity headache along with neck pain. The patient mentioned a first-time use of a home massage device three weeks prior to headache onset. After investigations, the patient was diagnosed with VAD, and treatment was initiated. She was discharged in stable condition. With the recent increased popularity of home massage devices, we report this case to raise awareness about the safe use of massage devices in order to prevent the occurrence of such injuries and complications.

Abstract:Benign paroxysmal positional vertigo of the posterior semicircular canal is a common vestibular disorder. Symptoms include brief, episodic, position-provoked vertigo with characteristic nystagmus on Dix–Hallpike testing. Benign paroxysmal positional vertigo is believed to be the result of free-floating otoconial debris in the posterior canal endolymph. Several repositioning maneuvers that attempt to correct these aberrant particles have been described. Currently, the particle repositioning maneuver is the most widely used of these techniques. Current evidence would suggest that mastoid vibration during repositioning and postmaneuver activity limitations are unnecessary adjuncts to the particle repositioning maneuver. Several investigators have demonstrated excellent initial resolution of vertigo after repositioning; however, long-term recurrences still limit this treatment.

Carotid Dissection Associated with a Handheld Electric Massager

Abstract:The extracranial internal carotid artery (ICA) is susceptible to injury and dissection from external shear forces applied to the neck. Traumatic ICA dissection usually occurs in the setting of a sudden, high amplitude force causing significant distortion of surrounding soft tissues. Weaker, repetitive forces applied for longer intervals may also pose a risk for ICA dissection. A 38-year-old woman with no significant stroke risk factors had sudden onset of severe dysarthria and left hemiparesis several days after receiving an approximately 20-minute neck massage with a handheld electric massager. The moving elements consisted of two approximately 2-cm-diameter spheres that percuss the skin with low amplitude and high frequency. Magnetic resonance imaging and angiography demonstrated acute infarction in the right middle cerebral artery territory and dissection of the extracranial right ICA. Handheld electric massager units may cause ICA dissection and disabling stroke.

Abstract:Objective To investigate the clinic characteristics of post-traumatic benign paroxysmal positioning vertigo and find the causes of misdiagnosis and preventive measures. Methods The clinical data of 17 cases of post-traumatic benign paroxysmal positioning vertigo treated in the Department of Otolaryngology Head and Neck Surgery, Navy General Hospital from June 2013 to September 2016 were retrospectively analyzed. Results The average age of the patients was 38. 8 years. Ten cases (58. 82%) complained of dizziness and 7 cases (41. 18%) complained of vertigo. Twelve cases (70. 59%) were initially misdiagnosed. Five cases were misdiagnosed as posterior circulation ischemia. Seven cases were misdiagnosed as cere-bral hemorrhagic stroke. The average period of misdiagnosis was 12. 75 d. All of the 17 cases were confirmed by medical his-tory, physical examinations, positioning test, CT and MRI, and were treated by manual repositioning maneuvers. The symp-tom of vertigo was disappeared in 3 weeks after repositioning maneuver in all of the cases. Conclusion The post-traumatic be-nign paroxysmal positioning vertigo tends to be misdiagnosed in early stage for its lack of specificity. The post-traumatic benign paroxysmal positioning vertigo may be diagnosed through detailed inquiry, careful physical examination, positioning test and except other vertigo caused by cerebrovascular diseases.
Clinical Analysis of Diagnosis and Treatment of 82 Patients with Horizontal Canal Benign Paroxysmal Positional Vertigo

Abstract:Objectives: To explore the clinical features, diagnosis and treatment of horizontal canal benign paroxysmal positional vertigo (HC-BPPV). Methods: A total of 82 consecutive HC-BPPV patients were enrolled.The clinical baseline data of the patients were registered to analyze the clinical features,diagnosis and treatment of HC-BPPV patients.Results:①Of the 82 HC-BPPV patients,62(75.6%)were positional vertigo, 12(14.6%)were positional dizziness,and 8(9.8%)were walking unsteady.②Onset-to-visit intervals:39 cases (47.6%)within 3 days,14 cases(17.1%)in 4~7 days,9 cases(11.0%)in 7~14 days,20 cases over 14 days (24.4%).③Inducements:43 cases(52.4%)had fatigue,31 cases(37.8%)had sleep disorders,10 cases(12.2%) had emotional agitation,6 cases(7.3%)had head injuries,and 3 cases(3.7%)had precursor infection.④Attack duration:16 patients(19.5%)lasted for several seconds,35 patients(42.7%)lasted for 10 seconds to 1 minute, and 19 patients(23.2%)lasted for 1 to 2 minutes,2 patients lasted for 2 to 5 minutes and 10 patients(12.2%) lasted for more than 5 minutes. ⑤Vestibular function examination: 28 cases (35.0%) had unilateral peripheral vestibular dysfunction,22(78.6%)were consistent with the side of the affected semicircular canal.⑥In the Roll test, there were 46 cases (56.1%) had transient geotropic direction-changing positional nystagmus (DCPN), 6 cases (7.3%) had persistent geotropic DCPN, and 30 cases (36.6%) had persistent apogeotropic DCPN. Fifty cases (61.0%) were affected on the right side and 32 cases (39.0%) on the left side. ⑦The instantaneous cure rates for patients with transient DCPN, continuous DCPN, and persistent DCPN was 86.5% (45/52), 0%, and 73.3%(18/30),respectively.The recovery rate was 92.3%(48/52),33.3%(2/6),and 80.0%(24/30)respectively after 1 week. Conclusion: Repositioning maneuver is an effective way to treat patients with HC-BPPV, especially for the transient geotropic HC-BPPV and persistent apogeotropic HC-BPPV. Non-otolith mechanism may exist in persistent geotropic DCPN.

Abstract:Objective To analyze the clinical features and misdiagnosed causes of benign paroxysmal positioning ver-tigo ( BPPV) secondary to sudden deafness in order to improve the final diagnosis rate. Methods Clinical data of 27 cases of BPPV secondary to sudden deafness treated in our department from March 2011 to June 2017 was retrospectively analyzed. Re-sults A total of 27 patients diagnosed as BPPV secondary to sudden deafness were included in this study. There were 8 male and 19 female with the age from 32 to 75 years old (average age 53. 4 years). Seventeen cases of sudden deafness accompa-nied by vertigo had missed diagnosis. In the 10 cases complained of vertigo after a period of sudden deafness, there were 9 ca-ses misdiagnosed as Meniere disease and one case was misdiagnosed as posterior circulation ischemia. All 27 cases were diag-nosed as BPPV secondary to sudden deafness in our department through careful physical examination and positioning test and treated by manual repositioning maneuvers. The vertigo or dizziness in all 27 cases was disappeared within 3 weeks after trea-ted by manual repositioning maneuvers. Conclusion BPPV secondary to sudden deafness are very easy to be misdiagnosed and missed diagnosis for non-specific manifestations, but the manual repositioning maneuvers can get high curative rate. Clini-cians should strengthen their understanding of the disease and improve the diagnosis and treatment ability.
Extreme Frequency of Benign Paroxysmal Positional Vertigo Recurrences Unexplained by Any Known Risk Factors: A Case Report

Abstract:Benign paroxysmal positional vertigo (BPPV) is a common vestibulopathy and involves failed dissolution and dislocation of calcium carbonate crystals into the semicircular canal. This causes short-lasting vertigo during changes in head position. Oftentimes, BPPV can be resolved within a single clinic visit, but secondary to many known risk factors, BPPV can recur. This case report follows a patient with extremely frequent recurrences despite a lack of known risk factors. A 55-year-old female experienced BPPV in December 2022, with successful canalith repositioning treatment from otolaryngology. On having a recurrence in March 2023, the patient underwent videonystagmography including caloric testing, and MRI, all of which showed normal findings besides left posterior-canal BPPV. From December 2022 to February 2024, the patient had 13 recurrences, each treated to resolution, confirmed by repeating positional tests and per subjective report for at least 2 weeks following. The incidence of BPPV recurrence is reported higher in females, however, this could not be interpreted as a causative factor. Though many other risk factors are documented in literature, this patient's history, demographics, imaging, and blood tests were all negative. This case report highlights a gap in knowledge of vestibular pathophysiology, as this patient's high rate of recurrence remains unexplained.
Acute Effects of Handheld Vibration Massage on Posterior Shoulder Soft Tissues

Abstract:Background: Interventions using vibration stimulation have been recognized for their potential for increasing range of motion (ROM) without compromising muscle strength. Handheld vibration massagers can efficiently deliver vibration therapy to the shoulder joint and may be a potential treatment. Purpose: To evaluate the effects of vibration massage using a handheld device on the soft tissues of the posterior shoulder joint, particularly on internal rotation (IR) passive ROM and external rotation (ER) muscle strength. Study design: Crossover study design. Methods: A crossover study with a 5-min vibration massage and passive control condition was conducted in healthy male volunteers (mean age 20.5 ± 1.7 years). Vibration massage was applied to the posterior shoulder soft tissues of the dominant arm, with no intervention under control conditions. IR-ROM (vertebral level and in abduction) and strength of the external rotators (isometric and isokinetic) were measured before and immediately after the intervention. Vertebral levels were calculated as a ratio of lengths (ratio decreases with increased mobility). IR-ROM in abduction, the angle was measured. Statistical analysis was performed with two-way repeated measures ANOVA and paired t-test (Bonferroni correction). Results: Vibration application decreased (improved) vertebral level IR ROM by -4.1% (p < 0.01, d = 0.445) and increased abduction position IR ROM by 11.4° (p < 0.01, d = 0.694). These changes exceeded the 95% confidence interval for the minimum detectable change. By contrast, the control condition produced no changes. IR-ROM (vertebral level and abduction) immediately after the intervention showed significant differences between the control and vibration conditions (p = 0.036, d = 0.273; p = 0.048, d = 0.483, respectively). Muscle strength did not show any interaction, time, or between-condition effects. Conclusions: A massage using a handheld vibration massager applied to the posterior shoulder soft tissues increased IR-ROM without negatively affecting muscle strength, suggesting its potential use as a means of warming up. Level of evidence: Level 3.
Resolution of Postsurgical Diplopia, Paresthesia, and Weakness Following Inpatient Massage Therapy: A Case Report

Abstract:Massage therapy is utilized in hospitals for patients experiencing pain, anxiety, sleeplessness, among other symptoms. Postsurgical pain is one of the most common reasons massage therapy is ordered. We present a case of a 45-year-old woman who underwent ventral hernia repair. Postoperative day 2, she began to experience multiple symptoms, including double-vision, left-sided facial numbness, tremors, pain, and weakness in her left fingers, arm, and leg. A static magnetic resonance image of the brain and cervical spine were obtained, which revealed disc protrusions at C3/C4, C5/C6, C6/C7, and mild deformity of the spinal cord. The patient’s pain was difficult to control and she was unable to be weaned from intravenous pain medication. Massage therapy was ordered on postoperative days 6 and 7. During both massage sessions, as upper neck muscle tension was reduced, the therapist and patient observed several audible “pops” of the cervical spine with immediate relief of symptoms. In this particular case, massage therapy, though requested to address pain, had a secondary benefit in relieving her diplopia, left-sided facial numbness, tremors, and weakness. Although the mechanism of action is not clear, this case highlights the significant secondary beneficial effects that often occur with massage therapy.
Low vitamin D and uric acid status in patients with benign paroxysmal positional vertigo

Abstract:Science Progress, Volume 106, Issue 4, October-December 2023. PurposeGrowing evidence reported that vitamin D and uric acid metabolism played roles in the occurrence of benign paroxysmal positional vertigo, an otoconia-related vestibular disorder. We aimed to investigate the serum 25-hydroxy vitamin D (25(OH)D) and uric acid in patients with benign paroxysmal positional vertigo and to determine the risk factor for benign paroxysmal positional vertigo.MethodsThis case-control study comprised 182 patients with benign paroxysmal positional vertigo and 182 age- and gender-matched controls. All subjects' age, body mass index, systolic blood pressure, diastolic blood pressure, 25-hydroxyvitamin D (25(OH)D), uric acid and serum calcium measurements were analyzed.ResultsWe found a female preponderance of benign paroxysmal positional vertigo patients, with a median of 60 (52–66) years old. The results showed low vitamin D status both in benign paroxysmal positional vertigo and controls, with no significant difference of 25(OH)D levels between benign paroxysmal positional vertigo patients and controls (P > 0.05). Compared with the control group, patients with benign paroxysmal positional vertigo had a higher prevalence of vitamin D deficiency and a lower prevalence of vitamin D sufficiency (P < 0.05). Uric acid was significantly lower in the benign paroxysmal positional vertigo groups (P < 0.05). Logistic regression analysis revealed that age and uric acid were considered higher risk predictors for benign paroxysmal positional vertigo.ConclusionOur study observed low vitamin D status in patients with benign paroxysmal positional vertigo, with no significant differences of the 25(OH)D level in patients with benign paroxysmal positional vertigo and controls. Elderly, vitamin D deficiency and low uric acid levels may be risk factors for benign paroxysmal positional vertigo occurrence.

Abstract:Objective: This study analyzed the characteristics of patients with idiopathic sudden sensorineural hearing loss(ISSNHL) accompanied by benign paroxysmal positional vertigo(BPPV) and the impact of residual dizziness on their lives. Design: Retrospective observational study Setting: Tongde Hospital of Zhejiang Province between January 2023 and January 2024. Patients: A total of 54 adult inpatients with ISSNHL and BPPV were included in this study. Interventions: All patients received 50 mg of intravenous prednisolone daily for five consecutive days and hemodilution agents for ten days. Concurrently, BPPV was treated with repositioning maneuvers by the same therapist using the SRM-IV vertigo diagnostic and treatment system. Different repositioning methods were used for different types of otolithiasis. Patients were grouped based on the presence or absence of residual dizziness upon resolution of nystagmus at discharge. Main outcome measures: Pure tone audiometry, dizziness handicap inventory, and Hamilton anxiety scale assessments were performed before and after treatment. Results: .A total of 54 subjects were evaluated. The residual symptom group comprised 24 subjects (58.3% female, mean age 46.75±13.8 years). The non-residual group included 30 subjects (56.7% female, mean age 45.77±11.86 years)。 There were no significant differences between the two groups in pre-treatment hearing status and DHI scores. However, the Hamilton Anxiety Rating Scale (HAMA) scores before treatment showed a significant statistical difference.

Mal de Debarquement Syndrome: A Case Presentation of a Vestibular Enigma

Abstract:Mal de Debarquement (MdD) is a rare vestibular disorder characterized by a rocking or swaying sensation following passive motion exposure, such as boat or airplane travel. The etiology and pathogenesis are unknown. Due to a lack of clinical awareness and research, it is underrecognized and misdiagnosed. We present a case of a 37-year-old male with classic MdD symptoms following an amusement park boat ride. Along with normal evaluations, including neurological and vestibular tests, the patient experienced relief with clonazepam, aligned with current management strategies. MdD's impact extends beyond physical symptoms, with studies showing significant economic and psychological burdens associated with the condition. Therefore, it is important to recognize MdD as a missed diagnosis that can potentially result in prolonged and debilitating symptoms that may require management with selective serotonin reuptake inhibitors (SSRIs), transcranial magnetic stimulation, or vestibular rehabilitation.
Consumer Perceptions of Home-Based Percussive Massage Therapy for Musculoskeletal Concerns: Inductive Thematic Qualitative Analysis

Abstract:Background: Musculoskeletal pain is a prevalent concern among diverse populations, from the average individual to the elite athlete. Handheld percussive massage therapy devices like massage guns have gained much popularity in both medical and athletic settings. Its application has been prominently recognized in injury prevention and rehabilitation. The expansion of the market to provide handheld percussive therapy devices with varying features and price points has encouraged professional and novice use. While percussive therapy holds similarities to more studied therapeutic modalities, like vibration therapy and soft tissue mobilization, there is limited evidence-based information on the indications and contraindications. Objective: This study aims to use a qualitative analysis of consumer perceptions to understand the perceived therapeutic potential of percussive massage therapy as a home-based intervention for musculoskeletal concerns of everyday users and elite athletes. Additionally, we aim to gain insight on valuable characteristics supporting its therapeutic potential as well as pertinent limitations. Methods: The TOLOCO massage gun (TOLOCO) was identified as the best-selling percussive massage therapy device on Amazon. We performed an inductive thematic qualitative analysis on the top 100 positive comments and the top 100 critical comments of the device between June 2020 and April 2023 to determine 4 relevant themes. Results: The 4 themes identified upon qualitative analysis were pain management, versatility, accessibility, and safety and user education. Consumer reviews indicated use for this percussive therapy device in adolescents, adults, and older people across a spectrum of activity levels. Consumers reported the therapeutic potential of percussive massage therapy in managing wide-ranging musculoskeletal concerns like acute pain, chronic pain, nonsurgical injury rehabilitation, postsurgical injury rehabilitation, and injury prevention. Consumers highlighted the versatility of the device to address person-specific needs as a key feature in supporting its perceived therapeutic benefits. Additionally, consumers frequently commented on the affordability and availability of this device to increase accessibility to home-based care. Some critical reviews emphasized a concern for the quality of the device itself. However, this concern did not translate to the overall modality of percussive massage therapy. Of note, despite strong approval for its therapeutic potential, consumer reviews lacked evidence-based insights on appropriate usage. Conclusions: Home-based percussive massage therapy holds value with its perceived efficacy in pain management for acute and chronic conditions, as well as in injury prevention and rehabilitation. As a low-cost and readily available device for everyday users and high-performing athletes, percussive massage therapy works toward establishing increased health care accessibility and optimizing health care usage. This home-based intervention can serve to reduce the significant personal and economic burden of prevalent musculoskeletal concerns. However, the limited scientific research on percussive massage therapy raises concerns about the lack of evidence-based care and indicates the need for future studies.
