Abstract:Objective To investigate the clinical efficacy,safety,predictability and stability of femtosecond laser small incision lenticule extraction (SMILE) for the treatment of myopia.Methods In this prospective observational study,94 patients (170 eyes) with myopia underwent SMILE surgeries.The average age of the patients was 26.8±4.7 years (range 18 to 44 years).The mean preoperative spherical equivalent (SE) was-6.20±1.37 diopters (D),from-3.25 to-9.13 D.The median myopic astigmatism was-0.75 (0.75) D,up to-3.25 D.The mean corneal thickness was 554.7±27.5 μm (range 484 to 600 μm).Routine ophthalmic examinations were performed.The patients were followed from 6 months to 2 years with uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA),manifest refraction,intraocular pressure,corneal topography,and slit lamp examinations.The results were analyzed with a paired t test and linear regression analysis.Results All procedures were completed successfully.At the long-term follow-up examination,97％ of eyes had a UCVA of 1.0 or better and 100％ of eyes had a UCVA of 0.8 or better.BCVA was equal to or better than the preoperative BCVA in 100％ of eyes.35％ gained one line and 10％ gained two lines of BCVA.The mean postoperative SE was-0.01±0.45 D and 87％ and 100％ were within ±0.50 and ±1.00 D of the attempted correction.The root mean square (RMS) values of total higher order aberrations (HOA),spherical aberrations and coma increased by a significant amount after surgery:0.15±0.12 μm,0.04±0.11 μm,0.14±0.14 μm (t=-6.861,-2.078,-5.672,P＜0.05).Conclusion The SMILE procedure is safe,effective,predictable and the results were stable for treating myopia.HOA,spherical and coma aberrations increased postoperatively but the change in spherical aberrations was small.
Visual Outcomes and Optical Quality After Femtosecond Laser Small Incision Lenticule Extraction: An 18-Month Prospective Study.

Abstract:PURPOSE: To investigate the long-term visual outcomes and optical quality after femtosecond laser small incision lenticule extraction (SMILE) for moderate to high myopia correction. METHODS: Fifty-four patients (37 women, 17 men) who underwent SMILE were enrolled in this prospective clinical study. Patient ages ranged from 18 to 40 years, with a mean spherical equivalent of -6.50 +/- 1.64 diopters. Data including refractive parameters, retinal image quality, intraocular scattering, and aberrations were collected at 6 and 18 months after surgery. Patient satisfaction was also evaluated. RESULTS: At 18 months after surgery, both the mean safety index and the efficacy index were 1.23 +/- 0.23 (range: 0.80 to 1.50); there was no significant difference with indices measured at 6 months (P > .05). No patient lost two or more lines of corrected distance visual acuity. Total higher-order aberrations (HOAs) and coma significantly increased after SMILE (P < .05), whereas total spherical aberrations and trefoil changed only slightly. The mean modulation transfer function cutoff frequency was 36.66 +/- 8.54 cycles per degree (cpd) before surgery and 37.81 +/- 6.89 cpd at 18 months postoperatively; the mean objective scatter index was 0.62 +/- 0.33 before surgery and 0.71 +/- 0.38 at 18 months postoperatively. No significant difference was found between the three time points (P > .05). Mean patient satisfaction was 9.31 +/- 0.64 (with a maximum score of 10). CONCLUSIONS: SMILE showed good safety, efficacy, and stability in correcting moderate to high myopia, and patients were highly satisfied. HOAs increased after SMILE, mainly due to the increase of coma, whereas retinal image quality and intraocular scattering barely changed.

Abstract:Background This study compares the clinical outcomes of femtosecond laser small-incision lenticule extraction (SMILE) for the correction of myopia and myopic astigmatism greater than − 10 D, and − 10 D or less respectively. Methods 60 eyes/patients were equally selected into group 1 (myopia and myopic astigmatism of − 10 D or less) and group 2 (myopia and myopic astigmatism of over − 10 D), both of which were treated with SMILE. Visual and refractive outcomes, corneal higher-order aberrations, and Bowman’s layer micro-distortions were evaluated preoperatively, 3 months, and 6 months postoperatively. Results LogMAR corrected distance visual acuity (CDVA) of group 1 and group 2 was − 0.069 ± 0.047 and − 0.053 ± 0.073 6 months postoperatively ( P = 0.48). 100% eyes in group 1 and 97% in group 2 were within 1 D of targeted correction ( P = 0.45). Meanwhile, 100% eyes in group 1 and 97% in group 2 had an uncorrected distance visual acuity of 20/25 or better ( P = 0.20). Changes in corneal higher-order aberrations root mean square, coma, and trefoil were similar between the two groups but spherical aberration was higher in group 2 ( P < 0.01). Micro-distortions were observed in 53% in group 1 and 77% in group 2. More micro-distortions were observed in group 2 (3.40 ± 2.66) than in group 1 (2.07 ± 2.29) ( P = 0.041). The total number of micro-distortions was not correlated with postoperative CDVA ( P = 0.77). Conclusions Visual outcomes showed similar results of SMILE for myopic correction of > − 10 D and ≤ − 10 D. Refractive outcomes showed slightly under-correction in higher myopic eyes. Higher myopic treatment tends to induce more spherical aberrations. Micro-distortions had no impact in visual and refractive outcomes.

Abstract:PURPOSE: To investigate objective optical quality and changes in light scatter after femtosecond laser small incision lenticule extraction (SMILE) for moderate to high myopia correction. METHODS: Prospective clinical study of 66 eyes of 66 patients (23 males, 43 females) with a mean age of 28.67 ± 6.62 years, mean spherical refraction of −6.06 ± 1.57 diopters, and mean cylindrical refraction of −0.68 ± 0.46 diopters. Every patient had a 3-month follow-up after a standard SMILE procedure. An optical quality analysis system was used to measure modulation transfer function cutoff frequency (MTF cutoff ), Strehl2D ratio, and objective scatter index (OSI). RESULTS: At 3 months postoperatively, mean spherical equivalent was 0.03 ± 0.24 diopters and corrected distance visual acuities of all patients were equal to or better than preoperative values, with a mean safety index of 1.12 ± 0.17 (range: 0.80 to 1.50) and mean efficacy index of 1.18 ± 0.21 (range: 0.80 to 1.50). Mean OSI increased from 0.75 preoperatively to 1.09 at 20 days postoperatively ( P < .05), and gradually declined to 0.94 at 40 days and 0.82 at 3 months postoperatively ( P > .05). MTF cutoff and Strehl2D ratio did not change significantly compared with preoperative values ( P > .05). Multiple linear regression analysis revealed that preoperative OSI was significantly correlated with optical quality at 3 months. Patients with less preoperative OSI tended to acquire higher MTF cutoff (b = −8.61) and lower OSI (b = 0.65) ( P < .05). CONCLUSIONS: SMILE had little impact on retinal image quality after myopia correction. Postoperative OSI showed a temporary increase and then returned to normal. Abstract:The small incision lenticule extraction (SMILE) with femtosecond laser involves creating an intrastromal lenticule by scanning at two different depth in the corneal stroma and making side-cut incisions,followed by separation and complete removal of the lenticule from the small incision.This technique revolutionizes the traditional refractive surgery to achieve " all-in-one",with better security,effectiveness,predictability and stability.It has a good prospect in application but its long-term efficacy needs more samples to be further investigated.

Abstract:PURPOSE: To compare the visual quality, accommodation, and stereoacuity between patients with a myopia magnitude of-3.00 to-8.50 diopters (D) who underwent ICLV4c (STAAR Surgical) implantation and small incision lenticule extraction (SMILE). METHODS: The visual quality parameters, amplitude of accommodation (AMP), and stereoacuity were compared between and within the ICLV4c and SMILE groups (40 eyes/ group) with a mean spherical equivalent (SE) of-6.14 D. Differences in these parameters before and after surgery were analyzed using SPSS 22.0 software (SPSS, Inc).RESULTS: Modulation transfer function (MTF) cut-off increased significantly by 3.24 cycles/degree (t =-2.111, P =.041) and AMP decreased significantly by 1.66 D (t = 7.519, P < .001) from baseline to 3 months after surgery in the ICLV4c group. However, the MTF cut-off was significantly lower (t = 2.123, P = .037) and the AMP was significantly better (t =-3.605, P = .001) in the SMILE group than in the ICLV4c group at 3 months postoperatively. Other parameters such as Objective Scatter Index, Strehl ratio, and Optical Quality Analysis System (Visiometrics) under different contrast values did not show significant changes (P > .05). Stereoacuity did not improve significantly, except in a patient with anisometropia (2.50 D) in the ICLV4c group.CONCLUSIONS: The ICLV4c implantation yielded a better visual quality but worse AMP than did SMILE in patients with a myopia magnitude of-3.00 to-8.50 D in the short term.
Small Incision Lenticule Extraction (SMILE) for Moderate and High Myopia: Seven-Year Outcomes of Refraction, Corneal Tomography, and Wavefront Aberrations

Abstract:Purpose . To investigate the long-term outcomes of refraction, corneal tomography, and wavefront aberrations after small incision lenticule extraction (SMILE) for moderate and high myopia. Methods . Prospective, nonconsecutive case series. A total of 26 patients (26 eyes) who underwent SMILE from May 2010 to March 2013 at the Fudan University Eye and ENT Hospital (Shanghai, China) were enrolled. The periods of follow-up were 1 month, 1 year, 5 years, and 7 years after surgery. The routine eye examinations included uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA), manifest refraction, and corneal tomography. Results . All surgeries were executed without any complications. At the final visit, an UDVA of 20/20 or better was achieved in 26 eyes (100%) and 11 eyes (42%) exhibited no change in CDVA. 9 eyes (35%) gained one line, 6 eyes (23%) gained two lines, and no eyes lost CDVA. 24 eyes (92%) and 26 eyes (100%) were within ±0.5 D and ±1.00 D of the target refraction, respectively. A mean refractive regression of −0.17 D was observed between 1 month and 7 years postoperatively. Mean corneal front curvature (MCFC) was significantly decreased between pre- and post-SMILE surgery ( P < 0.0001 ). Higher-order aberrations (HOAs) and vertical coma were significantly increased after SMILE compared to those measured before surgery (all P < 0.001 ). There were no significant differences in trefoil and spherical aberration between pre- and post-SMILE surgery (all P > 0.05 ). Conclusion . SMILE is an effective, safe, and stable procedure for moderate and high myopia, with relatively constant corneal stability and wavefront aberrations. Five-Year Follow-Up of Visual Outcomes and Optical Quality after Small Incision Lenticule Extraction for Moderate and High Myopia

Abstract:This study investigated the long-term visual and optical quality changes after small incision lenticule extraction (SMILE) for the correction of moderate to high myopia. Forty-one eyes of 41 patients with a mean spherical equivalent (SE) of − 6.74 ± 1.45 D undergoing SMILE were enrolled. Preoperative and 1-month, 3-month, 1-year and 5-year postoperative follow-up examinations were performed including visual acuity, manifest refraction, corneal aberrations and contrast sensitivity. An optical quality analysis system was used to measure objective scatter index (OSI), Strehl ratio (SR) and modulation transfer function cutoff frequency (MTFcutoff). The safety and efficacy indices were 1.09 ± 0.12 and 1.03 ± 0.17, respectively, at the 5-year follow-up. No significant changes in SE from 1 month to 5 years postoperatively were found. The OSI significantly increased at 1 and 3 months postoperatively (p < 0.01 and p < 0.01) and declined at 1 and 5 years (p = 0.81 and p = 0.87). MTFcutoff and SR showed similar trends. Coma, spherical aberration and total higher-order aberration significantly increased after SMILE (p < 0.01) and remained stable from 1 month to 5 years after surgery (p > 0.05). Contrast sensitivity in the photopic and mesopic condition showed no significant changes during 5-year follow-up (p > 0.05). Comparison of visual results and higher-order aberrations after small incision lenticule extraction (SMILE): high myopia vs. mild to moderate myopia

Abstract:Background To compare the refractive results and higher-order aberrations (HOAs) after small incision lenticule extraction (SMILE) in high myopia and mild to moderate myopia patients. Methods This prospective study included 165 eyes (86 patients) undergoing SMILE. According to the preoperative spherical equivalent (SE), treated eyes were divided into two groups: the high myopia group (more than -6.0 D, group-H) and the mild to moderate group (less than -6.0 D, group-M). Follow-up intervals were at 1 day, 10 days, 1 month and 3 months postoperatively. We obtained the following parameters: uncorrected (UDVA) and corrected distance visual acuity (CDVA), SE, efficacy and safety index, and HOAs. Results Preoperative SE was -7.16 ± 0.93 D in group-H and -4.34 ± 0.97 D in group-M. At 3 months postoperatively, the SE in group-H and group-M was -0.20 ± 0.37 D and 0.01 ± 0.19 D ( t = - 4.11, P <0.05), respectively. It was found that 77% and 98% had an UDVA of 20/20, 98% and 99% had a CDVA of 20/20 in group-H and group-M, respectively, while 87% and 95% had a SE within ±0.5 D and ±1.0 D in group-H, and 98% and 100% in group-M. The efficacy indexes were 0.98 ± 0.18 in group-H and 1.05 ± 0.10 in group-M ( t = - 3.084, p < 0.05). The safety indexes were 1.06 ± 0.09 and 1.06 ± 0.09 ( t = 0.153, p > 0.05), respectively. There were significant increases in total HOAs, 3 rd -order coma, and 4 th -order spherical aberrations. Conclusions SMILE is an effective and safe surgery for correcting myopia. But the target correction amount in high myopia patients should be adjusted to avoid undercorrection and acquired more satisfaction. SMILE induced increases of HOAs. Comparison of long-term changes in the effective optical zone following SMILE and FS-LASIK for moderate and high myopia

Abstract:Background: Visual quality after corneal refractive surgery is linked to the postoperative effective optical zone (EOZ). This study aims to compare long-term changes in the EOZ following small incision lenticule extraction (SMILE) and femtosecond laser-assisted in-situ keratomileusis (FS-LASIK) for moderate and high myopia. Methods: This study included 42 patients (72 eyes) who underwent either SMILE (36 eyes) or FS-LASIK (36 eyes). A custom software program based on the tangential curvature difference map of the Pentacam HR (Oculus Optikgeräte GmbH) was used to define the EOZ at 3 and 7 years postoperatively. The EOZ, its chronological changes compared to the programmed optical zone (POZ), and the corneal wavefront aberrations following SMILE and FS-LASIK were analyzed. Correlations between the EOZ changes and relevant parameters were evaluated. Results: Three years postoperatively, EOZ following SMILE and FS-LASIK were 5.13 ± 0.27 mm and 4.70 ± 0.24 mm (P < 0.001), respectively. Seven years postoperatively, EOZ following SMILE and FS-LASIK decreased to 5.03 ± 0.28 mm and 4.63 ± 0.23 mm (P < 0.001), respectively. At postoperative 7 years, the percentages of EOZ/POZ were negatively correlated with Q-value changes (β = -5.120, P = 0.009) following SMILE and positively correlated with the cylinder correction (β = 1.184, P = 0.004) following FS-LASIK. The induced spherical aberrations in the SMILE group were less than those in the FS-LASIK group (P < 0.05) and were negatively correlated with the EOZ/POZ (β = -16.653, P < 0.001). Conclusions: The EOZ following SMILE was larger than that following FS-LASIK in the long postoperative term for moderate and high myopia. Furthermore, a continual reduction in the EOZ was noted after both surgical modalities.
Early Visual Outcomes and Optical Quality after Femtosecond Laser Small-Incision Lenticule Extraction for Myopia and Myopic Astigmatism Correction of Over-10 Dioptres

Abstract:PurposeTo investigate early visual and refractive outcomes, corneal stability and optical quality after femtosecond laser small-incision lenticule extraction (SMILE) for treating myopia and myopic astigmatism over -10D. MethodsThirty eyes (30 patients) with myopia and myopic astigmatism of over -10D were treated with VisuMax((R)) femtosecond laser (version 3.0; Carl Zeiss Meditec AG, Jena, Germany). Six months postoperative safety, efficacy and predictability were evaluated. Corneal Scheimpflug topography was measured preoperatively, 1day, 3months and 6months postoperatively. Wavefront aberrations were measured preoperatively, 3months and 6months postoperatively. ResultsSix months postoperatively, LogMAR uncorrected and corrected distance visual acuity (CDVA) were -0.0130.086 and -0.073 +/- 0.069, respectively. 73% (97%) of eyes were within 0.5 (1) D of target refraction. No eyes lost CDVA, 43% (13 eyes) gained one line and 7% (two eyes) gained two lines. Mean corneal back curvature (KMB) and posterior central elevation (PCE) did not change significantly comparing preoperative and 6months postoperative data (p=0.91 and 0.77, respectively). Comparing 1day with 6months postoperative data, central corneal thickness (CCT), mean corneal front curvature (KMF), KMB and PCE did not change significantly (p=0.27, 0.07, 0.52, 0.71, respectively). Total higher-order aberration (HOA), spherical aberration and coma increased significantly (p<0.01) but trefoil remained stable (p=0.49). ConclusionOur results indicate that SMILE can correct myopia and myopic astigmatism of over -10D predictably. No early ectasia was observed. Long-term changes in visual quality and corneal stability require further investigation.
Long-Term Observation of Higher-Order Aberrations and Microdistortions in Bowman’s Layer after Small Incision Lenticule Extraction for the Correcting Myopia with Spherical Equivalent Higher Than −9.0 Diopters

Abstract:PurposeTo evaluate the outcomes in corneal higher-order aberrations (HOAs) and microdistortions in the Bowman’s layer after femtosecond laser small incision lenticule extraction (SMILE) for correcting extremely high myopia.MethodsThis prospective study included patients with myopia with spherical equivalent ≥ -9.0 Diopters (D). SMILE was performed in forty eyes of 40 patients. Pentacam was used to evaluate HOAs before and at 1 day, 3 months, 6 months, and 2 years after surgery. Fourier-domain optical coherence tomography was used to evaluate microdistortions at 2 years postoperatively. Thirty-two eyes of 32 patients receiving femtosecond laser-assisted in situ keratomileusis (FS-LASIK) were enrolled as the control group. HOAs were measured before, at 1 day and at least 1 year postoperatively.ResultsAfter SMILE, the long-term safety and effectiveness index was 1.25 and 0.85, respectively. Microdistortions were observed in 73.5% of the eyes at 2 years, with an average number of 1.20 ± 1.22 microdistortions and an average width of 287.37 ± 259.00 μm. We detected more microdistortions in the horizontal meridian than in the vertical meridian (p = 0.035). The average number and width of microdistortions were both higher in the central region (≤4 mm) than in the peripheral region (4–8 mm) (both p < 0.001). With the exception of horizontal trefoil in the SMILE group and vertical trefoil in the FS-LASIK group, significant changes over time were observed in all other HOAs (all p < 0.05). Meanwhile, we detected significant increases in the total corneal HOA, spherical aberration (SA), and coma at all time-points after both surgeries (all p < 0.01). Compared with FS-LASIK, SMILE induced less SA (p < 0.001) and more horizontal coma (p = 0.036). In the SMILE group, the HOA, SA, and trefoil were more in the small optical zone (≤6.0 mm) than in the large optical zone (>6.0 mm) (all p < 0.05). The increase in SA and most trefoil correlated with the mean number of central microdistortions number (all p < 0.05).ConclusionFor myopia over −9.0D, the microdistortions in the Bowman’s layer were still detectable in most eyes long-term after SMILE. Both SMILE and FS-LASIK induced more HOAs, mainly HOA, SA, and coma. The small optical zone and microdistortions may affect postoperative aberrations.
Refractive Outcomes Comparing Small-Incision Lenticule Extraction And Femtosecond Laser-Assisted Laser In Situ Keratomileusis For High Myopia

Abstract:Purpose: To compare the long-term refractive effects of smallincision lenticule extraction (SMILE) and femtosecond laserassisted laser in situ keratomileusis (FS-LASIK) on the correction of high myopia and astigmatism.Setting: The Eye and ENT Hospital of Fudan University, Shanghai, China.Design: Retrospective case series.Methods: Patients had SMILE or FS-LASIK for high myopia. Subgroup analyses of high myopia (-6.00 to -8.75 diopters [D]) and extremely high myopia (>=-9.00 D) were performed. The main outcome measure was refractive predictability, compared between SMILE and FS-LASIK groups. Secondary outcomes included efficacy, safety, and residual astigmatism.Results: This study included 121 patients (121 eyes: 75 in the SMILE group and 46 in the FS-LASIK group). No differences were found in terms of refractive predictability between SMILE and FS-LASIK in eyes with highmyopia: 56% vs 58.7% achieved +/- 0.50 D of attempted correction (P =.771) and 81.3% vs 76.1% achieved +/- 1.00 D of attempted correction (P =.489). Efficacy indices of the SMILE and FS-LASIK groups were 1.02 +/- 0.24 and 1.03 +/- 0.24 (P =.742), respectively; safety indices were 1.23 +/- 0.22 and 1.20 +/- 0.24 (P =.324), respectively. Logarithmof the minimumangle of resolution uncorrected distance visual acuity and spherical equivalent in the high myopia subgroup were better than in the extremely high myopia subgroup after both SMILE and FS-LASIK (P <.01).Conclusions: SMILE and FS-LASIK were both effective in correcting high myopia and myopic astigmatism. Abstract:PURPOSE:The aim of this study was to compare visual outcomes after small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for high myopia.METHODS:In this prospective, comparative study, a total of 52 eyes of 34 consecutive highly myopic patients with spherical equivalent within the range of -8.00 to -10.00 diopters were recruited. Twenty-three eyes of 16 patients were in the FS-LASIK group and 29 eyes of 18 patients were in the SMILE group. Visual outcomes and wavefront aberrations were analyzed preoperatively and 6 months postoperatively.RESULTS:At the postoperative 6-month visit, 96.6% in the SMILE group and 91.3% in the FS-LASIK group achieved unchanged or better corrected distance visual acuity (CDVA). As for uncorrected distance visual acuity (UDVA), 96.6% in the SMILE group and 95.7% in the FS-LASIK group achieved UDVA of 20/20 or better. As for wavefront aberrations, high-order aberrations (HOAs) and spherical aberrations increased significantly after surgery in both groups relative to corresponding preoperative values (p < 0.001), and vertical coma increased after SMILE (p < 0.001). No statistically significant differences in changes of HOAs (p =0.90), spherical aberrations (p = 0.07), horizontal coma (p = 0.56), coma (p =0.08), horizontal trefoil (p =0.19), vertical trefoil (p = 0.90), and trefoil (p = 0.45) were detected between the 2 groups, except for vertical coma (p < 0.01).CONCLUSIONS:SMILE is as effective as FS-LASIK in correcting high myopia, but attention should be paid to the induction of vertical coma in highly myopic patients following a SMILE procedure.

Abstract:Purpose: To compare the effective optical zone (EOZ) and centration in eyes with high myopia after small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) using a novel method. Methods: Forty eyes of 40 consecutive patients with high myopia scheduled for SMILE or FS-LASIK were enrolled in the study. The EOZ, optical zone decentration, and corneal aberrations were analyzed using Scheimpflug imaging. These values were then analyzed and compared between the two procedures 6 months after surgery. Results: The mean EOZ diameter for SMILE (4.41 ± 0.14 mm) was larger than that for FS-LASIK (4.24 ± 0.28 mm; P = .002), corresponding to reductions of 1.60 ± 0.11 and 1.71 ± 0.21 mm, respectively, compared with the programmed optical zone (POZ) (P = .007). Moreover, the total decentration for SMILE (0.33 ± 0.12 mm) was greater than that for FS-LASIK (0.27 ± 0.15 mm; P = .020). The induction of spherical aberration (SA) was lower with SMILE than with FS-LASIK (P = .007). Abstract:The femtosecond laser has a number of advantages, such as short pulse time, high instantaneous power, high repetition rate, low monopulse energy, and small thermal effect. Femtosecond laser-assisted small incision lenticule extraction (SMILE) is becoming the new direction in refractive surgery, and the ocular surface changes after SMILE are attracting increasingly more attention. This article reviews adverse effects, including dry eye, injury of corneal nerves, and ocular surface inflammation, occurring after SMILE.
Differences in ocular high order aberrations before and after small incision lenticule extraction for correction of myopia: a systematic review and meta-analysis

Abstract:Objective: To examine the causes and factors that lead to high order aberration (HOA) during the treatment of myopia using small incision lenticule extraction (SMILE), as well as the differences between SMILE and other corneal refractive surgeries through a systematic review and meta-analysis. Methods: A systematic search was conducted from January 2015 to February 2023 in Pubmed, Embase, Web of Science, and Google Scholar databases to gather relevant studies on SMILE and HOA. Studies meeting specific criteria were chosen, and clinical data was retrieved for analysis. Results: This meta-analysis resulted in the inclusion of 19 studies involving 1,503 eyes. Pooled results showed significant induction of total HOA (tHOA, d = -0.21, p < 0.001), spherical aberration (SA, d = -0.11, p < 0.001) and coma aberration (CA, d = -0.18, p < 0.001) after SMILE compared to pre-SMILE, while no significant change in trefoil aberration (TA) was observed (d = -0.00, p = 0.91). There was a significantly lower induction of tHOA after SMILE compared to femtosecond laser-assisted in situ keratomileusis (FS-LASIK, d = 0.04, p < 0.001), and no significant difference was observed compared to wavefront aberration-guided (WFG) refractive surgery (d = 0.00, p = 0.75). There was also a significant association between different levels of myopia and astigmatism, duration of follow-up, lenticule thickness, and preoperative central corneal thickness (CCT) on the induction of tHOA after SMILE (p < 0.05), while the higher preoperative myopia group (sphere > -5D), lower preoperative astigmatism group (cylinder ≤ -1D), larger lenticule thickness group (lenticule thickness > 100 μm), shorter follow-up group (follow-up 1 month postoperatively) and the thicker CCT group (CCT > 550 μm) brought a significant induction of tHOA compared to the opposite comparison group (p < 0.001). Conclusion: While SMILE can induce HOA significantly, it induces less HOA than FS-LASIK. Postoperative HOA following SMILE can be affected by factors such as myopia, astigmatism, lenticule thickness, CCT, and duration of follow-up. SMILE for Hyperopia With and Without Astigmatism: Results of a Prospective Multicenter 12-Month Study

Abstract:To investigate the safety and effectiveness of small incision lenticule extraction (SMILE) in patients who have hyperopia with or without astigmatism.This was a prospective multicenter trial including 374 eyes of 199 patients treated by SMILE for hyperopia using the VisuMax femtosecond laser (Carl Zeiss Meditec AG). Inclusion criteria were sphere up to +6.00 diopters (D), cylinder up to 5.00 D, and maximum hyperopic meridian up to +7.00 D, with preoperative corrected distance visual acuity (CDVA) of 20/25 or better. The optical zone was 6.3 mm with a transition zone of 2 mm. The minimum lenticule thickness was set at 25 µm in the center and at 10 µm at the edge. Patients were examined at 1 day, 1 week, and 1, 3, 6, 9, and 12 months after surgery. Standard refractive surgery outcomes analysis was performed.The preoperative spherical equivalent was +3.20 ± 1.48 D (range: +0.25 to +6.50 D). At the 12-month follow-up visit, 81% of eyes treated were within ±0.50 D and 93% of eyes were within ±1.00 D of intended correction. A total of 1.2% of eyes lost two or more lines of CDVA at the 12-month follow-up visit, and 83% were at least 20/20, corresponding to a safety index of 1.005 at 12 months. Of the 219 eyes with plano target, 68.8% had an uncorrected distance visual acuity of 20/20 or better and 88% were at least 20/25 uncorrected at 12 months. There were no statistically significant changes in contrast sensitivity.SMILE was found to be an effective treatment method for the correction of compound hyperopic astigmatism, demonstrating a high level of efficacy, predictability, safety, and stability.
