Abstract:OBJECTIVE:to summarize the complications of posterior vertebral column resection (pVCR) and pedicle screw fixation for the treatment of rigid thoracic and lumber spinal deformity.METHODS:fifty four patients from a single center from February 2000 to February 2009 were included in this study. There were 23 males and 31 females with an average age of 28.3 years (range, 16-58 years). Patients were divided into 4 diagnostic categories: severe scoliosis (n = 9, mean Cobb angle, 91.7°), kyphoscoliosis (n = 22, mean scoliosis, 101.5°, and mean kyphosis, 69.4°), angular kyphosis (n = 21, mean kyphosis, 72.2°), global kyphosis (n = 2, mean kyphosis, 93.6°). All of the patients received one stage pVCR combined correction with pedicle screws and circumferential fusion. Radiographs and hospital charts were reviewed to analyze the outcomes of correction.RESULTS:a mean of 1.4 vertebral levels were resected, the mean operative time was 470 min, and the mean blood loss was 4180 ml. All the patients were followed-up for an average time of 26 months (range, 12 - 66 months). At the latest follow-up, the major curve correction averaged: severe scoliosis 61.4° (67%), kyphoscoliosis 59.7° (56%)/kyphosis 42.3° (59%), angular kyphosis 48.5° (71%), global kyphosis 62.7° (67%). A total of 21 complications (38.9%) occurred in 17 patients, including 5 (9.3%) neurological complications happened in early stage and one case of delayed complete paraplegic. There were 11 (20.4%) non-neurological complications happened in early stage and 5 (9.3%) cases in late stage.CONCLUSIONS:one stage posterior-only vertebral column resection can achieve satisfactory correction of severe deformities with limited flexibility. Given its technically demanding and exhausting features, its potential risk of significant complications should be paid more attention.

Abstract:OBJECTIVE:To analyze the risk factors of neurological complications of posterior vertebral column resection in the treatment of severe rigid congenital spinal deformities.METHODS:The clinical data of 88 patients with severe rigid congenital spinal deformities who underwent PVCR in Department Of Orthopaedics, Xijing Hospital, Fourth Military Medical University from June 2007 to November 2012 were collected. There were 39 males and 49 females at the average age of 16.9 years (range 6-46 years). To measure the Cobb angle and balance at preoperative, postoperative and follow up, and to record the operation report, neurological complications and at follow up. The relevant factors of neurological complications were analyzed by one-way analysis, including: age, Cobb angle, operation time, body mass index, pulmonary function, blood volume loss, resection level, number of vertebrae fixed, number of vertebrae resected, usage of cage or titanium mesh, preoperative neurologic function, the type of deformity and combination of spinal canal deformity, and further analyzed by multiariable Logistic regression analysis.RESULTS:The average follow up was 42 months (range 19 to 83 months). The number of resected vertebrae average 1.3 (range 1 to 3), operative time average 502.4 min (range 165.0 to 880.0 min), estimate blood loss average 2,238 ml (range 100 to 11,500 ml) for an average 69.3% blood volume loss (range 9% to 299%). The average preoperative major coronal curve of 93.6° corrected to 22.2°, at the final follow-up, the coronal curve was 22.2° with a correction of 76.8%. The average preoperative coronal imbalance (absolute value) was 2.5 cm decreasing to 1.3 cm at the final follow-up. The average preoperative major sagittal curve of 88.2° corrected to 28.7°, at the final follow-up, the sagittal curve was 29.2°, average decrease in kyphosis of 59.0°. The average preoperative sagittal imbalance (absolute value) was 3.1 cm decreasing to 1.2 cm at the final follow-up. There were 12 patients (13.6%) developed a neurological complications. High rate of neurological complications was occurred in patients with operative time greater than 480 min, pulmonary dysfunction, blood volume loss greater than 50%, T7-T99 osteotomy and preoperative neurologic compromise (P=0.046, 0.000, 0.000, 0.033, 0.043).CONCLUSIONS:Posterior vertebral column resection can achieve satisfactory efficacy in treatment of severe spinal deformities. Pulmonary dysfunction and blood volume loss greater than 50% were significant risk factors of neurological complications.
Comparison of Anterior and Posterior Vertebral Column Resection Versus Anterior Release with Posterior Internal Distraction for Severe and Rigid Scoliosis

Abstract:A total of 16 patients with severe and rigid idiopathic scoliosis treated by anterior and posterior vertebral column resection (APVCR) were retrospectively reviewed after a minimum follow-up of 2 years. The indication for APVCR was scoliosis more than 90° with flexibility less than 20%. The radiographic parameters were evaluated, and clinical records were reviewed. All patients underwent APVCR with posterior pedicle screw instrumentation in a two-stage surgery. The rib hump was reduced from 7.2 cm preoperatively to 1.8 cm at final follow-up (75% correction). Preoperative curves ranged from 93° to 110° Cobb angle. Coronal plane correction of the major curve averaged 67% with an average loss of correction of 1.4%. The apical vertebral translation of the major curve was corrected by 63.5%. The preoperative coronal imbalance of 0.9 cm (range 0–2.4) was improved to 0.8 cm (range 0.1–1.7) at the most recent follow-up. The preoperative sagittal imbalance of 1.0 cm (range −3.1 to 4.6) was improved to 0.9 cm (range −2.6 to 3.0) at the most recent follow-up. Complications were encountered in four patients. One patient required ventilator support for 12 h after anterior surgery. Malposition of one pedicle screw was found in one patient. Malposition of titanium mesh cage happened to two patients. There were no neurological complications, deep wound infections or pseudarthrosis. APVCR is an effective alternative for severe and rigid idiopathic scoliosis.
Unilateral Posterior Vertebral Column Resection For Severe Thoracolumbar Kyphotic Deformity Caused By Old Compressive Vertebrae Fracture: A Technical Improvement

Abstract:Severe thoracolumbar kyphotic deformity caused by old compressive vertebrae fracture remains a big challenge for spine surgeons. When symptoms related to significant deformities cannot be adequately managed conservatively, posterior vertebral column resection (PVCR) is required, but with long operating time and severe blood loss. We develop a UPVCR technique, which is done through a unilateral approach instead of a bilateral approach, vertebral body resection advancing to cross the midline in an abrasive way from an extreme oblique orientation enable the resection of most contralateral vertebral body. In the present study, the effects of UPVCR for severe thoracolumbar kyphotic deformity were investigated. We did find that satisfactory correction of sagittal deformity, functional improvement and pain relief can be achieved by UPVCR, and it has the advantage of shortening surgery time, reducing blood loss and incidence of nerve root impingement over PVCR.
Posterior Vertebral Column Resection with Intraoperative Manual Retraction for the Treatment of Posttubercular Kyphosis in Upper Thoracic Spine or Cervicothoracic Junction

Abstract:Study Design: This is a case-series. Objective: To evaluate the clinical and radiologic outcomes of posterior vertebral column resection (PVCR) for treatment of posttubercular kyphosis in upper thoracic spine and cervicothoracic junction (CTJ). Summary of Background Data: Surgical management of severe posttubercular kyphosis in upper thoracic spine or CTJ is challenging. A new technique that combines PVCR and intraoperative manual traction in a single procedure, was developed to maximize the correction rate and minimize the risk of dural buckling and spinal cord injury. However, the safety and effectiveness of the procedure is yet to be established. Materials and Methods: Ten patients with upper thoracic or CTJ posttubercular kyphosis, who underwent PVCR with intraoperative manual traction between December 2008 and 2013, were studied retrospectively. Data pertaining to operation time, blood loss, and perioperative complications were collected. Outcomes were evaluated using the visual analog scale; the 12-Item Short Form Health Survey (SF-12), Mental Component Summary, and SF-12 Physical Component Summary. Neurological status was assessed using American Spinal Injury Association (ASIA) impairment scale. Radiologic outcomes including fusion and deformity correction were assessed using plain radiography and computed tomography. Results: The average duration of follow-up was 53.6±7.2 months. Average operating time and blood loss were 263±35.3 minutes and 2490±569 mL, respectively. Visual analog scale score for back pain, SF-12 Mental Component Summary, and Physical Component Summary showed significant improvement postoperatively. Kyphosis improved from a preoperative average of 73.6±13.1 degrees to an average of 37.9±8.7 degrees at final follow-up (correction rate: 48.5%). No postoperative neurological deterioration was observed based on the ASIA score. As of the last follow-up, solid fusion was not achieved only in 1 patient. Conclusions: PVCR with intraoperative manual traction is a safe and effective procedure for treatment of upper thoracic or CTJ posttubercular kyphosis. Level of Evidence: Level 4.
Posterior vertebral column resection for the treatment of severe angular thoracolumbar kyphosis

Abstract:Objective To report on the technique and results of posterior vertebral column re-section (PVCR) for severe angular thoracolumbar kyphosis. Methods From January 2007 to Octo-ber 2012,17 patients with severe thoracolumbar kyphosis underwent posterior vertebral column re-section (PVCR) in our institute. They were 13 males and 5 females,with an average age of 26.4 years old (range,18to 39years). The preoperative Cobb angle was 64°-105°In these cases,kyphotic Cobb angle and ASIA grade were measured before operation,after operation and at final follow-up. Results All operations were performed successfully. The operation time ranged from 250 to 460min (average,318min); blood loss during the operation ranged from 1600 to 2600ml(average, 2120ml),The postoperative kyphosis ranged from 10° to 21°(average,16.6°),which showed signifi-cant difference (P<0.01). The average kyphosis correction rate was 77.8%. The follow-up ranged from 13 to 34 months(average,16.8months) and kyphosis at final follow-up ranged from 10° to 23° (average,17.5°). Among the 2 patients with preoperative ASIA grade D,5 cases improved to grade E,the kyphosis improved and the local back pain relieved very well. Conclusion Posterior verte-bral column resection is an effective and safe way to treat severe thoracolumbar kyphosis.

Abstract:We report a multilevel modified vertebral column resection (MVCR) through a single posterior approach and clinical outcomes for treatment of severe congenital rigid kyphoscoliosis in adults. Transpedicular eggshell osteotomies and vertebral column resection are two techniques for the surgical treatment of rigid severe spine deformities. The authors developed a new technique combining the two surgical methods as a MVCR, through a single posterior approach, for surgical treatment of severe congenital rigid kyphoscoliosis in adults. Thirteen adult patients with severe rigid congenital kyphoscoliosis deformity were treated by a single posterior approach using a MVCR technique. The surgery processes included a one-stage posterior transpedicular eggshell technique first, and then expanded the eggshell technique to adjacent intervertebra space through abrasive reduction of the vertebral cortices from inside out. All posterior vertebral elements were removed including the cortical vertebral bone around the neural canal. Range of resection of the vertebral column at the apex of the deformity included apical vertebra and both cephalic and/or caudal adjacent wedged vertebrae. Totally, 32 vertebrae had been removed in 13 patients, with 2.42 vertebrae being removed on average in each case. The average fusion extent was 7.69 vertebrae. Mean operation time was 266 min with average blood loss of 2,411.54 ml during operation. Patients were followed up for an average duration of 2.54 years. Deformity correction was 59% in the coronal plane (from 79.7° to 32.4°) postoperatively and 33.7° (57% correction) at 2 years follow-up. In the sagittal plane, correction was from preoperative 85.9° to 27.5° immediately after operation, and 32.0° at 2 years follow-up. Postoperative pain was reduced from preoperative 1.77 to 0.54 at 2 years follow-up in visual analog scale. SRS-24 scale was from 38.2 preoperatively to 76.9 at 2 years follow-up postoperative. Complications were encountered in four patients (30.7%) with transient neurology that spontaneously improved without further treatment within 3 months. MVCR technique through a single posterior approach is an effective procedure for the surgical treatment of severe congenital rigid kyphoscoliosis in adults.
Comparison Of Posterior Vertebral Column Resection And Anterior Corpectomy And Instrumentation For Correcting Late Post-Traumatic Thoracolumbar Kyphosis

Abstract:Purpose: To compare the clinical and radiological outcomes between anterior corpectomy & instrumentation (ACI) and posterior vertebral column resection (PVCR) in correcting late post-traumatic kyphosis (LPTK) of the thoracolumbar spine. Methods: Between January 2007 and December 2012, twenty patients underwent ACI, and twenty-three patients underwent PVCR for LPTK were prospectively reviewed. The clinical records were reviewed and compared for surgical time, blood loss, functional improvement (Oswestry Disability Index, ODI), pain relief (Visual Analog Scale, VAS). The radiological records were reviewed and compared for correction of kyphotic deformity, correction loss, fusion rate, cage subsidence and cage tilting. Results: The surgery time was shorter in PVCR group than that in ACI group (156.3 +/- 32.9 min vs. 188.3 +/- 37.1 min, P=0.006), the blood loss was less in PVCR group than that in ACI group (806.5 +/- 174.5 ml vs. 974.5 +/- 146.1 ml, P=0.021), the difference of deformity correction was statistically significant in favor of the PVCR group (37.3 +/- 5.2 vs. 22.3 +/- 4.4, P<0.001). At 24 months follow-up, no statistical difference was found between the two groups in cage subsidence (2.1 +/- 0.7 mm in PVCR vs. 1.9 +/- 0.8 mm in ACI, P=0.796), anterior-posterior cage tilting (2.0 +/- 0.5 in PVCR vs. 2.4 +/- 0.7 in ACI, P=0.111), lateral cage tilting (2.9 +/- 0.8 in PVCR vs. 3.2 +/- 0.9 in ACI, P=0.342) and correction loss (2.8 +/- 1.8 in PVCR vs. 2.5 +/- 1.7 in ACI, P=0.536). All of the patients got ODI improvement and VAS relief, and no statistical difference was found between the two groups in ODI improvement (44.2 +/- 5.7% in PVCR vs. 42.9 +/- 5.9% in ACI, P=0.444) and VAS relief (5.2 +/- 0.9 in PVCR vs. 5.3 +/- 1.3 in ACI, P=0.815). Conclusions: The posterior vertebral column resection have the advantage over anterior corpectomy & instrumentation in shorter surgery time, less blood loss, and more kyphotic correction for correcting late post-traumatic kyphosis of the thoracolumbar spine.

Abstract:Objective To evaluate the corrective efficacy and clinical risk of single posterior approach with multi-level vertebra column resection(VCR) in severe kyphosis and scoliosis.Methods From 2003 Jan to 2006 Jan,38 patients of severe rigid kyphosis and scoliosis treated by single posterior approach with multi-level VCR were reviewed.Long cassettes X-ray and 3D CT scan and MRI images were applied for preoperative evaluation.Operating time,blood loss and spine shorten during operation were recorded.Cobb angle of pre-or postoperation,trunk balance were also measured.Results Mean 1.5 to 3 vertebra were resected and 42mm was shortened(ranged 31-62mm) during operation in this group.Mean operating time was 348 minutes(ranged 280-510 min) with average blood loss of 2010 ml(range 1400-3200ml).All patients were followed up 1 to 2.5 years.Deformity correction was 56.3%(51.2%-64.2%) in the coronal plane and 75.6(67.8%-81.4%) in the sagittal plane.All patients got good trunk balance with no decompensation.Neurologic complication included 3 transient spinal cord injury symptoms and 6 radicular pain.They recovered with no special treatment in follow-up.Superior mesenteric artery syndrome were observed in 4 patients.They were healed with gastrointestinal decompression.Conclusion Posterior multi-levels VCR is an effective alternative for severe rigid kyphoscoliosis patients.

Abstract:Background Severe post-tubercular kyphosis with late-onset neurological deficits is difficult to treat, with high risk of neurological complications. This study retrospectively evaluates the efficacy and safety of posterior vertebral column resection (PVCR) for treating severe post-tubercular kyphosis with late-onset neurological deficits. Methods From January 2012 to December 2015, 13 patients with severe post-tubercular kyphosis underwent PVCR. All these patients were of late-onset neurological deficits. The operative time, blood loss, preoperative and postoperative kyphotic angles, sagittal vertical axis (SVA), neurological status, and complications were recorded. The preoperative and postoperative Oswestry Disability Index (ODI) scores and visual analog scale (VAS) scores for back pain were compared. The American Spinal Injury Association (ASIA) grading system was used to evaluate neurological function. Results The mean postoperative follow-up period was 28.6 months. The mean operative time was 388 ± 46 min. The mean blood loss was 2554 ± 1459 ml. The mean preoperative and postoperative kyphotic angles were 93.7 ± 14.4° and 31.7 ± 7.3°, respectively, with a mean correction of 62.0 ± 13.8°. The mean preoperative and postoperative SVA were 43.2 ± 44.4 mm and 17.8 ± 16.2 mm, respectively. The mean ODI score improved from 56.3 ± 5.1 preoperatively to 18.3 ± 18.5 at last follow-up. The mean VAS score improved from 6.4 ± 1.8 preoperatively to 1.8 ± 0.8 at last follow-up. Two cases had spinal cord injuries, including one complete paraplegia and one incomplete paraplegia, and a total neurological complication rate of 15.4%. The risk factors for neurological complications were summarized. Conclusions Severe post-tubercular kyphosis with late-onset neurological deficits can be corrected by PVCR carefully and properly to prevent neurological complications. In many cases with stenosis adjacent to the angular kyphosis, sufficient decompression of the spinal cord at the segments with stenosis is necessary before correcting the kyphosis.

Abstract:Objective To analyze the neurological complications in treatment of severe thoracolumbar spinal deformity with one stage posterior vertebral column resection (pVCR) and discuss the related risk factors. Methods There were 67 patients with severe thoracolumbar spinal deformity who underwent one-stage pVCR from February 2000 to September 2010.There were 29 males and 38 females at an average age of 31.4 years old(range,14-62 years).There were 21 patients at age less than 18 years old and 46 at age more than 18 years old.Patients were divided into four pathological types:severe scoliosis group(n=11,mean Cobb angle 90.4°),kyphoscoliosis group(n=25,mean scoliosis 94.5°,and mean kyphosis 65.5°),angular kyphosis group(n=28,mean kyphosis 74.3°)and global kyphosis group(n=3,mean kyphosis 91.1°).of all the patients,59 patients underwent primary surgery and eight underwent revision surgery.Surgical methods included posterior apex vertebral column resection,segemental pedicle screw fixation and correction as well as 360° bone fusion.Neurological complication was statistically analyzed. Results The average follow-up was 14 months (range,3-69 months),which showed severe neurologic complication in eight patients(11.9%)after surgery.Severe neurologic complication occurred in three patients (4.5%),among whom one patient presented delayed complete paraplegia 23 hours after surgery.Five patients had mild neurologic deficits(7.5%),the incidence of which was higher than 23.1%for thoracic osteotomy (P<0.05).Multilevel pVCR had high rate of neurological complications (P<0.05).The incidence rate was 33.3% for patients with preoperative neurologic compromise and 7.3%for patients mthom preoperative neuroiogic compromise (P<0.05).The incidence rate was increased in the revision surgery (P<0.05).Eight patients with neurological deficits had kyphotic angle of raore than 60°although there was no statistical difference (P>0.05). Conclusions pVCR is an effective surgical method for the correction of severe thoracolumbar spinal deformity.The neurological complications,however,should be paid attention to the surgeons.The risk factors for neurologic complications include improper manipulation,massive blood losing,preoperative neurologic compromise,osteotomy at thoracic rein,multi-level vertebrectomy,revision surgery and severe kyphosis.

Abstract:In the past decades, an increasing number of surgeons started using posterior vertebral column resection (PVCR) to treat severe, rigid and angular spinal deformities. Little high-level evidence is available to guide surgical treatment. The aims of our study were to identify important surgical strategies and key technical points of Chinese experts who have extensive experience in the management of severe, rigid and angular spinal deformities using PVCR, and to standardize and unify the current core concepts. Workgroups of consensus were formed by selecting nationwide representing experts and comprehensive consultations. Eight task forces for major issues were established, then retrieval of literature, collection of expert opinions and writing of review articles were carried out. A modified Delphi process was chosen in round-table forum with three face-to-face meetings. Consensus was reached with items graded more than seven points including: indications and contraindications of PVCR; review PVCR in the evolution of spinal osteotomies; The corrective mechanism and safety of spinal cord; monitoring and responses of spinal cord crisis; characteristics and therapeutic outcome of pulmonary function; management of bleeding during PVCR; relationship of pedicle screw insertion and spinal cord safety; and analysis of non-neurologic complications and prevention strategies. In conclusion, The essential properties regarding PVCR procedure are tightly linked with various factors such as medical and surgical indication, range and level of vertebral column resection, strategies of correction, corrective efficiency and control of neurological risk. PVCR is used mainly for severe, rigid spinal deformity that is not manageable by other osteotomy techniques.
Posterior Vertebral Column Resection For Correction Of Thoracolumbar Kyphosis After Failed Anterior Instrumented Fusion

Abstract:To explore the effectiveness of posterior vertebral column resection for failed thoracolumbar anterior instrumented fusion. Ten patients with anterior fusion with refractory pain, progressive neurological deficits, and kyphosis were recruited. Anterior removal of the implant and posterior vertebral column resection were performed. The mean operating time, intraoperative blood loss, kyphosis angle, visual analog scale pain score, Oswestry disability index, bone fusion time, and complications were assessed in a minimum 18-month follow-up. The mean operating time was 323.5 +/- 63.6 minutes, with a mean blood loss of 1189 +/- 253.4 mL. The mean preoperative kyphosis angle of 54.6 degrees +/- 8.0 degrees immediately decreased to 4.8 degrees +/- 1.5 degrees after revision surgery and eventually to 6.8 degrees +/- 1.3 degrees at the final follow-up. The mean bone fusion time was 6.8 +/- 1.2 months. All patients had satisfactory sagittal and coronal balance with no implant failure at the last follow-up. The average visual analog scale score was 6.2 +/- 1.0 preoperatively, and it decreased to 2.6 +/- 0.5 at the last follow-up. No patient suffered from neurological deterioration. The Oswestry disability index decreased from 39.8% +/- 4.6% preoperatively to 24.5% +/- 4.7% at the final follow-up. Complications occurred in 4 patients: 3 experienced tearing of the lung, and 1 had a superficial wound infection. Anterior removal of the implant and posterior vertebral column resection constituted a safe and effective revision surgery for patients with prior anterior fusion with rigid postsurgical deformities.
Modified Posterior Vertebral Column Resection for the Treatment of Osteoporotic Fractures with Neurological Deficits in Elderly Patients

Abstract:Introduction The purpose of this retrospective study was to evaluate the results of spinal canal decompression and anterior column support via modified posterior vertebral column resection (PVCR) (Unilateral hemilaminectomy, pediculectomy, subtotal vertebrectomy) performed for eliminating the disadvantages of the anterior or combined A + P approaches, in the management of osteoporotic vertebral fractures with neurological deficits in elderly patients. Patients and Methods A total of 55 patients (40 females and 15 males) with more than 2 years of follow-up were included in this study. The fractures were in the thoracic spine in 16 patients, and thoracolumbar spine in 39 patients. Overall, 11 patients had ASIA grade C and 41 patients had ASIA grade D neurological deficits. Clinical and radiological outcome and complications were also evaluated. Surgical technique included placement of fenestrated titanium pedicle screws, followed by hemilaminectomy, unilateral pediculectomy, and decompression of the spinal canal by subtotal vertebrectomy, and support of anterior column by titanium cage. Contralateral facet joint fusion and grafting was made. Prophylactic vertebroplasties were made in one level above and one level below in all patients. Results Mean age was 74.2 years (range, 66–92), and mean follow-up was 43.5 months (range, 28–102). Mean operation time was 252 minutes (range, 160–340), and blood loss was 580 mL (range, 420–800). Mean preoperative local kyphosis angle was 18.5 degrees and improved to 4.2 degree after the operation and 4.8 degree at the final follow-up. Visual analog scale (VAS) score was eight before the operation and improved to two at final follow-up. Full neurological recovery was achieved in all patients. There were no pseudoarthroses. The major complication was adjacent segment fracture was made three patients (5.4%). The minor complications were superficial wound infections in five patients and dural tear in four patients (16.3%). Conclusion Treatment of osteoporotic vertebral fracture with neurological deficit is a challenging procedure in elderly patients. Modified PVCR provides effective decompression of neural structures. Contralateral intact lamina, pedicle, and cortex of the vertebra increase primary stability and provides more bony surface for fusion.

Abstract:BACKGROUND CONTEXT: Many different correction methods have been reported to treat severe and rigid scoliosis. In the past, anterior and posterior spinal fusion (APSF), which included an anterior release followed by posterior instrumented fusion, was widely applied. In recent years, anterior/ posterior vertebral column resection (APVCR) is used to treat severe and rigid scoliosis. PURPOSE: We aimed to compare the clinical results of APSF and APVCR for severe and rigid scoliosis. STUDY DESIGN: This is a retrospective, one-center, institutional review board-approved study. PATIENT SAMPLE: A total of 48 patients with severe and rigid scoliosis treated by APSF or APVCR were enrolled. OUTCOME MEASURES: Comparisons between groups were made regarding the following variables: age at surgery, gender, etiology, flexibility of main curve. anterior release length, posterior fusion length, screw number, operation time, estimated blood loss, hospitalization time, follow-up duration, different radiological parameters. complication rate, and Scoliosis Research Society (SRS)-22 scores. METHODS: According to the operating technique, 48 patients with severe and rigid scoliosis were divided into two groups. In the first group. 26 patients were treated by APSF. In the second group, 22 patients were treated by APVCR. All patients bad a minimum 2-year follow-up. The radiographic parameters as well as anterior release length, posterior fusion length, screw number, operation time, estimated blood loss, hospitalization time, complication rate, and demographic data were analyzed. RESULTS: There was no significant difference in age, gender, etiology, flexibility of main curve, anterior release length. posterior fusion length, screw number, and follow-up between the two groups. The APVCR group had longer operation and hospitalization time, and more blood loss. There was no significant difference in the preoperative main curve between the two groups, but the APVCR group had smaller main curve at postoperation and final follow-up, and higher correction rate at immediate postoperation and final follow-up. There was no significant difference in the preoperative, postoperative, and final follow-up thoracic kyphosis, and coronal and sagittal balance between the two groups. There was no significant difference in complication rate between the two groups. Analysis of the preoperative SRS-22 questionnaire revealed no difference between the two groups. At final follow-up. APSF and APVCR groups had similar scores in the function, pain, self-image, mental health, and satisfaction with the treatment domains. CONCLUSIONS: Compared with APSF, treating severe and rigid scoliosis by APVCR means longer operation and hospitalization time, and more blood loss, but it allows better correction rate of main curve. Furthermore, SRS-22 questionnaire improvement scores were similar for both correction methods. (C) 2017 Elsevier Inc. All rights reserved.

Abstract:Objective To describe a successful method by which vertebral column resection is performed through a unilateral posterior approach for old thoracolumbar compressive fracture accompanied with kyphotic deformity.Methods From February 2009 to January 2013,49 patients with old thoracolumbar compressive fracture accompanied with kyphotic deformity were treated at our department.Twenty-three of them were treated by posterior unilateral vertebral column resection (PUVCR),and 26 by posterior vertebral column resection (PVCR).The 2 groups were compatible in general clinical data (P ＞ 0.05).Their clinical records were reviewed and compared in terms of operation time,intraoperative blood loss,postoperative drainage,correction of kyphotic deformity,neural functional improvement by Oswestry disability index (ODI),and pain by visual analogue scale (VAS).Results The operation time,intraoperative blood loss and postoperative drainage in the PUVCR group were significantly less than in the PVCR group (P ＜ 0.05).There were no significant differences between the 2 groups in terms of correction of kyphotic angle (P =0.523) postoperatively or loss of correction (P =0.146) at one-year follow-up (P ＞ 0.05).No significant differences were observed between the 2 groups either in terms of improved ODI or decreased VAS at one-year follow-up (P ＞ 0.05).Conclusions In treatment of old thoracolumbar compressive fracture accompanied with kyphotic deformity,PUVCR can achieve satisfactory correction of sagittal deformity,neural functional improvement and pain relief as well as conventional osteotomy,but it has advantages of shorter operation time,reduced blood loss and decreased incidence of nerve root impingement.

Abstract:Background: The surgical technique of hemivertebrae excision varies from anteroposterior procedures to posterior-alone resections according to the experience and preference of surgeons. Both the approaches are reliable and give relatively good results. This study aims to evaluate and compare the clinical and radiological results of these two approaches for hemivertebrae resection. Materials and Methods: Sixty patients were retrospectively enrolled between 2006 and 2009. The subjects included 32 women and 28 men, with a mean age of 12.9 years (range: 5–24 years). Thirty patients who underwent one-stage anteroposterior hemivertebrae resection (the AP group) were followed for 38.5 months, and the other 30 patients who underwent posterior resection (the P group) were followed for 20.6 months. Clinical and radiological assessments were performed preoperatively, 1 week postoperatively, and at the final follow-up. The operation time, blood loss, degree of correction of the main curve/segmental curve/kyphosis, the average hospital stay, and complications were reviewed and compared between the two groups. Results: The mean operation time, blood loss, and hospital stay of the AP group and the P group were 451 min vs 248.5 min, 1290 ml vs 910 ml, and 21.93 days vs 18.97 days, respectively ( P <.05). The average correction rate of the main curve/segmental curve/kyphosis of the AP group and the P group was 68.5% vs 66.2%, 71.5% vs 69.6%, and 57.4% vs 56.1%, respectively ( P >.05). Overall complication rate was 6.7% in the AP group vs 10% in the P group ( P >.05). Conclusion: Posterior hemivertebrael resection is a promising approach for congenital scoliosis in terms of relative safety, degree of correction achieved, reduced operative time and blood loss.
