The Value of MRI for Discogenic Lumbar Pain

Abstract:Objective:To investigate the role of magnetic resonance imaging(MRI) findings including the high intensity zone(HIZ) in posterior annulus and the vertebral endplate changes for assessment of discogenic lumbar pain by correlating with the results of pain provocation computed tomography(CT) discography.Methods:Sixty-six patients who were suspected discogenic lumbar pain were studied with MRI and pain provocation discography.The presence of the high intensity zone and the vertebral endplate changes on MRI were correlated the results of discography by in predicting a painful disc.Results:106 disks were studied with pain provocation discography and MRI.Grade 0,grade 1,grade 2 and grade 3 with annular tears were 37,16,32,21 disks respectively,and 50 disks with concordant pain.Chi-square test shows that the correlation between the degree of annular tears and the pain provocative CT discography was significant(2=64.994,r=0.617,P0.01).21 disks had HIZs,which as predictor of concordant pain: sensitivity 28%,specificity 88%,positive predictive value 67%,negative predictive value 58%.There was a poor correlation between HIZ and the pain provocative CT discography(2=3.995,r=0.191,P0.05).The vertebral end plate changes in 16 disks as predictor for concordant pain: sensitivity 30%,specificity 98%,positive predictive value 94%,negative predictive value 61%.Chi-square test shows that the correlation between the end plate changes and the pain provocative CT discography(2=16.407,r=0.366,P0.01).Conclusion:CT discography is a very important imaging for pain evaluation tool in discogenic lumbar pain.MRI is good for depicting internal disc morphology,the HIZ and the end plate changes were associated with discogenic lumbar pain,which is important sign for predicting a painful disc lesion.
