Abstract:Objective To evaluate the impact of different surgical approach on postoperative renal function in patients with synchronous sporadic bilateral renal cell carcinoma ( SSBRCC) .Methods The medical records of 28 patients with SSBRCC were reviewed.Of all these cases, 21 patients were male and 7 were female, and the mean age was 51 ( 25-63 ) years.None of these cases had family history of renal carcinoma.The treatment and other clinical data were analyzed retrospectively.Results All of the 28 patients underwent bilateral surgeries.Single procedures were performed on 16 patients, of which, one underwent bilateral radical nephrectomy ( RN) , 8 bilateral nephron-sparing surgery ( NSS) and 7 unilateral NSS combined with contralateral RN.Staged procedures were performed on 12 patients, of which bilateral NSS was on 5 patients (NSS-NSS group), NSS followed by RN (NSS-RN group) on 3 patients and RN followed by NSS ( RN-NSS group) on 4 patients.In term of postoperative renal function preservation for SSBRCC patients, the standard NSS was better than RN.Between different staged procedures, NSS-NSS was the preferred choice, and the NSS-RN was superior to the RN-NSS.Conclusions In term of preservation of postoperative renal function, the staged procedures were recommended, and NSS was preferred in the first stage.In deciding the second stage surgical approaches, the surgeons should take the renal function preservation and the tumor size into consideration.

Abstract:Objective: To investigate oncological outcomes in patients with bladder cancer who underwent minimally invasive radical cystectomy (MIRC) or open radical cystectomy (ORC). Methods: We identified patients with bladder cancer who underwent radical cystectomy (RC) in 13 centers of the Chinese Bladder Cancer Consortium (CBCC). Perioperative outcomes were compared between MIRC and ORC. The influence of surgical approaches on overall survival (OS) and cancer-specific survival (CSS) in the entire study group and subgroups classified according to pathologic stage or lymph node (LN) status was assessed with the log-rank test. Multivariable Cox proportional hazard models were used to evaluate the association among OS, CSS and risk factors of interest. Results: Of 2 098 patients who underwent RC, 1 243 patients underwent MIRC (1 087 laparoscopic RC and 156 robotic-assisted RC, respectively), while 855 patients underwent ORC. No significant differences were noted in positive surgical margin rate and 90-day postoperative mortality rate. MIRC was associated with less estimated blood loss, more LN yield, higher rate of neobladder diversion, longer operative time, and longer length of hospital stay. There was no significant difference in OS and CSS according to surgical approaches (p=0.653, and 0.816, respectively). Subgroup analysis revealed that OS and CSS were not significantly different regardless of the status of extravesical involvement or LN involvement. Multivariable Cox regression analyses showed that the surgical approach was not a significant predictor of OS and CSS. Conclusions: Our study showed that MIRC was comparable to conventional ORC in terms of OS and CSS.
Bilateral Synchronous Sporadic Renal Cell Carcinoma: Retroperitoneoscopic Strategies and Intermediate Outcomes of 60 Patients.

Abstract:OBJECTIVE:To evaluate the presentation, management, pathology, and functional and oncological outcomes of patients undergoing retroperitoneoscopic treatment of bilateral synchronous sporadic RCC at our institution.METHODS:We retrospectively evaluated the records of 60 patients with bilateral synchronous sporadic RCC who underwent retroperitoneoscopic treatment at the General Hospital of People's Liberation Army from 2008 to 2014. The estimated glomerular filtration rate was calculated and compared among different surgical procedures. The overall survival and recurrence free survival were assessed based on information from recent follow-up.RESULTS:Fifty-six patients underwent bilateral retroperitoneoscopic surgeries in staged procedures, and four patients underwent bilateral retroperitoneoscopic surgeries in simultaneous procedures. Among the former group of patients, 34 underwent bilateral partial nephrectomy, 12 underwent radical nephrectomy followed by partial nephrectomy, and 10 underwent partial nephrectomy followed by radical nephrectomy. Bilateral partial nephrectomy can better preserve renal function (p = 0.040) and the sequence of partial nephrectomy and radical nephrectomy did not affect functional outcomes (p = 0.790). One patient undergoing simultaneous procedures developed acute renal failure and required temporary hemodialysis. At 3 and 5 years, overall survival rates were 93.0% and 89.4%, and recurrence free survival rates were 90.5% and 81.6%. High nuclear grade (p = 0.014) was related to disease recurrence.CONCLUSIONS:Staged bilateral partial nephrectomy was efficient in preserving renal function. The survival of patients with bilateral synchronous sporadic renal tumors was similar to that of patients with unilateral nonmetastatic tumors. Nuclear grade was an independent prognostic factor of disease recurrence.

Abstract:Objective To investigate the clinicopathological features,the efficacy of surgical strategies and the prognostic factors of sporadic bilateral renal cell carcinoma.Methods A total of 79 patients treated from January 2006 to December 2013 were included in the retrospective study.Of the 79 patients,56 were males,23 were females; the mean age was 52 (25-74) years.Sixty-four were bilateral synchronous renal cell carcinoma and 15 were bilateral metachronous renal cell carcinoma,none of these cases had family history.Clinical stages were T1 in 68 cases,T2 in 8 cases,T3 in 3 cases.Results All of the 79 patients underwent bilateral surgeries,of which bilateral nephron-sparing surgery (NSS) was performed in 34 (43.0％) patients,NSS-radical nephrectomy (RN) in 37 (46.8％),RN-radio-frequency ablation (RFA) in 6 (7.6％),NSS-RFA in 1 (1.3％),and bilateral RN in 1 (1.3％).There were 184 tumors in the 79 patients.Of the 171 tumors with pathological results,158 (92.4％) tumors were clear cell subtype,7 (4.1％) papillary,4 (2.3％) chromophobe and 2 (1.2％) sarcoid.Bilateral clear cell renal cell carcinoma was found in 55 patients and bilateral papillary renal cell carcinoma in 2 patients.Fifteen patients presented with multifocal tumors in one kidney.Seventy-seven (97.5％) patients were followed up and the median follow up period was 49 (9-150) months.In the most recent follow up,59 (76.6％) patients survived without tumors,9 (11.7％) patients survived with tumors,9 (11.7％) patients died.The 3-year overall survival rate and 3-year progress free survival rate were 89.8％ and 87.1％,respectively.The higher nuclear grade and higher T stage were related to worse prognosis with the P=0.005 and P=0.022 in multivariate analysis,respectively.Bilateral NSS did not increase the risk of disease progress (P=0.157).Conclusions Clear cell carcinoma is the main subtype of bilateral sporadic renal carcinoma.NSS is the preferred choice for bilateral kidney tumors.Patients with sporadic bilateral renal tumors underwent active surgeries on both kidneys could get comparable oncological results with that of patients with unilateral renal tumors.

Abstract:OBJECTIVE:To evaluate the surgical management and prognostic characteristics of bilateral sporadic renal tumors (BSRTs).METHODS:Twenty-three cases of BSRTs were identified and retrospectively reviewed. The presenting clinical findings, surgical treatment, and prognosis were evaluated.RESULTS:Twelve patients had synchronous tumors and 11 had asynchronous tumors. BSRTs comprised 1.8% of all RCC cases with a male-to-female ratio of 6.67:1 in the same term. The mean age of patients was 56 years. Seventeen patients were preformed bilateral operation (10 had synchronous and 7 had asynchronous tumors), 4 patients were preformed unilateral operation (1 had synchronous and 3 had asynchronous tumors), and 2 patients were not performed operation (asynchronous tumors). Twenty-one patients were followed up (11 had synchronous tumors and 10 had asynchronous tumors, mean 27 months). One and 5-years cancer specific survival rate of BSRTs were 84.6% and 33.5% respectively. The rate of renal failure of synchronous BSRTs (18%) was lower than that of asynchronous BSRTs (70%) after operation (P=0.030).CONCLUSIONS:Prognosis of BSRTs is poor. The key in the treatment of BSRTs is to detect and resect tumor earlier. Our results indicate that nephron sparing surgery should be performed in order to obtain optimal functional preservation of the renal remnant with complete local excision of tumor.
Surgical Management for Early-Stage Bilateral Breast Cancer Patients in China

Abstract:Background The aim of this study was to investigate the current surgical management strategy for bilateral breast cancer (BBC) patients and to assess the changes in this strategy in China. Methods This is a retrospective review of all patients with early-stage BBC who underwent surgical treatment at the Fudan University Shanghai Cancer Center between June 2007 and June 2014. Results A total of 15,337 patients with primary breast cancer were identified. Of these patients, 218 (1.5%) suffered from synchronous bilateral breast cancer (sBBC), and 296 (2.0%) suffered from metachronous bilateral breast cancer (mBBC). Patients with a lobular carcinoma component, those with estrogen receptor-positive cancer, and those with an accompanying sclerosing adenosis in the affected breast tended to develop BBC. The rates of bilateral mastectomy, breast conserving therapy, reconstruction, and combined surgeries were 86.2%, 6.4%, 3.7%, and 3.7%, respectively, for patients with sBBC and 81.1%, 4.4%, 3.0%, and 11.5%, respectively, for patients with mBBC. The interval between bilateral cancers, age at first diagnosis of breast cancer, histopathological type, and stage have significant impacts on the choice of surgery for patients with BBC. Conclusions Bilateral mastectomy was the dominant surgical management for patients with BBC in China, despite the increased application of breast reconstruction surgery observed in recent years. Bilateral prosthetic breast reconstruction was the ideal choice for patients with sBBC. Chinese surgeons should take responsibility for patient education and inform their patients about their surgical options.
Experience in diagnosis and treatment of non-hereditary bilateral synchronous renal carcinoma

Abstract:Objective To investigate the principles of diagnosis and treatment of non-hereditary bilateral synchronous renal cell carcinoma.Methods This retrospective study analyzed 36 cases of non-hereditary bilateral synchronous renal cell carcinoma in our hospital from January 2008 to December 2016,including 30 males and 6 females.A total of 74 renal tumors were found,in which 34 patients had bilateral single kidney tumor and 2 patients had two tumors in one kidney.The diameter of tumors ranged from 1 cm to 11 cm,with an average of (6.8 ±4.1)cm.The patients that underwent nephron-sparing surgery(NSS) got 4-12 points,with an average of (6.1 ±3.4) points in R.E.N.A.L.score and 3-13 points,with an average of (6.9 ± 3.7) points in Zhongshan score.The patients are classified into 4 groups according to operation methods.In group A,16 patients underwent bilateral NSS,which the preoperative creatinine was 63-103 μmol/L with an average of (80.9 ± 11.4) μmol/L.In group B,7 patients underwent one side of NSS before contralateral radical nephrectomy (RN),which preoperative creatinine was 59-87 μmol/L with an average of (75.7 ± 8.9)μmol/L.In group C,7 patients underwent one side of RN before contralateral NSS,preoperative creatinine was 57-107 μmol/L,with an average of (77.6 ± 19.2) μmol/L.In group D,6 patients underwent one side of NSS or RN and spare the contralateral side,2 of which shifted from NSS to RN after finding tumor invaded pelvis and upper ureter during surgery.Of all the 16 patients with bilateral NSS,4 patients underwent surgery on the side where tumor had a higher score in the first phase and then the side where tumor had a lower score in the second phase,11 underwent surgeries in an opposite order.One patient underwent bilateral NSS simultaneously.Group A,B and C are taken into final analysis.Result All the 30 patients underwent surgery successfully.The operation time of NSS ranged from 60 to 110 min with an average of (88.6 ± 23.6) min and RN ranged from 40 to 90 min with an average of (72.3 ± 21.4) min.The warm ischemia time of NSS was 12-40 mins with an average of (29.5 ± 9.7)min.The creatinine of Group A was 62-117 μmol/L with an average of (89.4 ± 15.8) μmol/L and 57-392 μmol/L with an average of (129.6 ±74.9)μmol/L one month after the first and second surgery respectively.The creatinine of Group B was 64-115 μmol/L with an average of (94 ± 14.4) μmol/L and 93-453 μmol/L with an average of (190.4 ± 117.2)μ mol/L one month after the first and second surgery respectively.The creatinine of Group C was 84-113 μmol/L with an average of (90.1 ± 12.1) μ mol/L and 88-156 μmol/L with an average of (121.4 ± 24.8)μmol/L one month after the first and second surgery respectively.One patient in Group B and C developed lung metastases.One patient in Group B occurred oliguria after the second stage of surgery,and gradually improved after one week of hemodialysis.The creatine showed no significant difference among Group A,B and C before operation,after the first and second stage (P ＞ 0.05).Postoperative hospital stay after the first stage surgery was 3-16 days with an average of (6.7 ± 3.4) d,and 3-16 d with an average of (6.2 ± 3.2)d after the second stage,respectively.Conclusions In principle,bilateral renal tumors should be treated with NSS,wbich can protect renal functions as much as possible.Among patients who can undergo bilateral NSS,the first-stage surgery should be operated on the simpler and easier side to preserve the kidney of one side as much as possible to lay a good foundation for the second stage surgery.Among patients who undergo one side of RN and the other side of NSS,NSS is recommended for the first stage,and RN for the contralateral second stage after the renal function of the operated side was restored.
Oncological Efficacy and Safety of Retroperitoneal Laparoscopic Surgery for Bilateral Renal Cell Carcinoma

Abstract:Objective:To present the experience of retroperitoneal laparoscopic surgery for bilateral renal cell carcinoma. Method:From January 2004 to june 2010,11 cases of bilateral renal cell carcinoma(BRCC)were retrospectively analyzed.Among the 11 csaes,6 had synchronous BRCC and 5 had asynchronous BRCC.The mean tumor size was(2.8±1.8) cm in diameter.All of the 11 patients received two-stage retroperitoneal laparoscopic operation.8 patients underwent bilateral retroperitoneal laparoscopic nephron-sparing surgery(LNSS),3 patients underwent unilateral retroperitoneal laparoscopic nephron-sparing surgery plus contralateral retroperitoneal laparoscopic radical nephrectomy(LRN). Result:11 cases were operated successfully without conversion to open surgery.The mean operation time was(102±27) min,and mean warm ischemia time(WIT)was(23.0±7.7) min,and mean blood loss was(80.0±43.5) ml,and mean postoperative hospital stay was(10.6±2.0) days.In a mean follow up for(3.5±2.0) years,9 patients' renal function remains well without tumor recurrence and metastasis,1 patient serum creatinine up to abnormal level immediately after second surgery,1 had lung metastasis but still alive with targeted therapy. Conclusion:retroperitoneal laparoscopic surgery is safe、feasible and effective for BRCC with less trauma and faster recovery of its advantages,but its long-term effects still need to study with large samples compare and long-term follow-up.

Abstract:OBJECTIVES:To investigate the incidence and treatment strategies for bilateral upper tract urothelial carcinoma (UTUC) and to compare the characteristic and oncologic outcomes of bilateral UTUC with those of unilateral tumors. METHODS AND MATERIALS:The study included 892 consecutive patients with UTUC. Bilateral UTUC was defined as synchronous bilateral carcinoma on preoperative imaging before confirmation by pathology or positive urine cytology result plus direct visualization. Radical nephroureterectomy (RNU) or nephron-sparing surgery (NSS) or both were carried out. RESULTS:A total of 39 patients (4.37%) suffered from bilateral disease. Discordant histological grade of bilateral tumor was found in 39.3% cases. Bilateral tumors were associated with female sex (P<0.001), preoperative renal insufficiency (P<0.001), previous or concomitant bladder tumors (P = 0.013), lower tumor stages (P = 0.020), papillary architecture (P = 0.001), and smaller-sized tumors (P = 0.020). Patients with worse renal function (P<0.001) or large-sized tumors (P = 0.039) tended to be treated with bilateral RNU. Most patients (67.6%) were treated with unilateral RNU plus unilateral NSS, with NSS being performed on tumors that only extended to the ureter (P = 0.003) and had a smaller size (P = 0.005). The median follow-up duration was 56 months. The 5-year cancer-specific survival and bladder recurrence-free survival rates were 81.2% and 64.5%, respectively, similar to those of unilateral tumors. Male sex (hazard ratio = 11.535) and higher tumor stage (hazard ratio = 3.386) were independent worse prognostic factors. CONCLUSIONS:The prevalence of bilateral UTUC is rare. Female patients, patients with renal insufficiency, and those with bladder tumor tended to suffer from bilateral disease and were less likely to present with worse pathological outcomes in the Chinese population. The tumor characteristics and renal function were informative in treatment selection. The oncologic outcomes were similar to those in unilateral UTUC, and male sex and a higher tumor stage were poor prognostic factors for these patients.
[Prognostic and Long Term Follow-Up Analysis of Sporadic Bilateral Renal Cell Carcinoma].

Abstract:OBJECTIVE To investigate the pathological feature, therapy and prognosis of bilateral sporadic renal cell carcinoma. METHODS The data of 59 bilateral sporadic renal cell carcinoma patients diagnosed from Apr. 1986 to Dec. 2009 were collected. We retrospectively analyzed the treatment, pathological features, long term survival and prognosis factors of the disease. RESULTS We found 59 patients with bilateral sporadic renal cell carcinoma (RCC) in the database diagnosed from Apr. 1986 to Dec. 2009. The median age was 56 years. Of all the cases, 37 were bilateral synchronous sporadic renal cell carcinoma and 22 bilateral metachronous sporadic renal cell carcinoma. Forty-three patients underwent bilateral surgeries, 11 unilateral surgery, and 5 no treatment. There were 122 masses in the 59 patients and 109 masses had pathological reports. All the 59 cases were RCC, and clear cell carcinomas was the main subtype (96.6%). The median follow-up time was 62.1 months (range 4-277 months). Thirty-nine patients (66.1%) survived without tumor recurrence, 4 survived with tumors recurrence, and 16 (27.1%) died. Kaplan-Meier curve revealed that the 3-year and 5-year overall survival rates were 83.5% and 81.1%, respectively; the 3-year and 5-year cancer special survival rates were 73.1% and 64.8%, respectively. The disease-free survival rates of bilateral synchronous and bilateral metachronous RCC were similar. Multivariate regression suggested that post-operative application of interferon-α and bilateral surgeries were related with a better prognosis. CONCLUSION The main subtype of bilateral sporadic renal cell carcinoma was clear cell carcinoma (96.6%). Bilateral synchronous and bilateral metachronous RCC both had a long time disease-free survival. Interferon-α application after surgery and bilateral surgeries were related with a better prognosis.

Abstract:Objective:To evaluate the efficacy of diagnosis,treatment,prognosis of bilateral renal cell carcinoma (BRCC).Methods:From January 1999 to January 2006,six cases of BRCC were treated.The mean ages of the patients at presentation were 53 years(35～74).Among the 6 cases,3 had synchronous BRCC and 3 had metachronous BRCC.6 tumors were at the upper pole of kidney,7 at the middle,1 at the inferior pole.7 tumors were on the left kidney,8 on the right.The mean tumor size was 4.6 cm in diameter(3～7 cm).Of three synchronous BRCC,2 cases underwent first stage operation,1 underwent two stage operation.The treatment included unilateral radical nephrectomy plus contralateral nephron sparing surgery(NSS) for 2 cases.Unilateral radical nephrectomy plus contralateral NSS after two weeks for 1 case.3 patients with metachronous BRCC underwent two stage operation.The treatment 3 cases included unilateral radical nephrectomy plus contralateral NSS.Results:The follow-up was 12～156 months(mean 84.5).Among 2 cases with tumor metastasis,1 case died of lung metastasis,1 case died of bone metastasis;2 cases survived with local tumor recurrence. 2 cases survived free of tumor.Conclusions:NSS is a safe and reasonable method for BRCC.The goal of management is to complete tumor resection and maintain renal function.

Bilateral UTUC was defined as synchronous bilateral carcinoma on preoperative imaging before confirmation by pathology or positive urine cytology result plus direct visualization. Radical nephroureterectomy (RNU) or nephron-sparing surgery (NSS) or both were carried out.A total of 39 patients (4.37%) suffered from bilateral disease. Discordant histological grade of bilateral tumor was found in 39.3% cases. Bilateral tumors were associated with female sex (P<0.001), preoperative renal insufficiency (P<0.001), previous or concomitant bladder tumors (P = 0.013), lower tumor stages (P = 0.020), papillary architecture (P = 0.001), and smaller-sized tumors (P = 0.020). Patients with worse renal function (P<0.001) or large-sized tumors (P = 0.039) tended to be treated with bilateral RNU. Most patients (67.6%) were treated with unilateral RNU plus unilateral NSS, with NSS being performed on tumors that only extended to the ureter (P = 0.003) and had a smaller size (P = 0.005). The median follow-up duration was 56 months. The 5-year cancer-specific survival and bladder recurrence-free survival rates were 81.2% and 64.5%, respectively, similar to those of unilateral tumors. Male sex (hazard ratio = 11.535) and higher tumor stage (hazard ratio = 3.386) were independent worse prognostic factors.The prevalence of bilateral UTUC is rare. Female patients, patients with renal insufficiency, and those with bladder tumor tended to suffer from bilateral disease and were less likely to present with worse pathological outcomes in the Chinese population. The tumor characteristics and renal function were informative in treatment selection. The oncologic outcomes were similar to those in unilateral UTUC, and male sex and a higher tumor stage were poor prognostic factors for these patients.

Abstract:OBJECTIVE:To evaluate the efficacy of nephron sparing surgery for bilateral renal cell carcinoma.METHODS: Ten cases of bilateral renal cell carcinoma were retrospectively analyzed.Ten patients undergone open operation.Of 7 synchronous BRCC,1 case undergone first-stage operation,6 cases undergone two-stage operation.The treatment included bilateral tumor enucleation(3 cases),bilateral partial nephrectomy plus right adrenalectomy(1 case),unilateral radical nephrectomy plus contralateral tumor enucleation(2 cases),unilateral radical nephrectomy plus thrombectomy and contralateral tumor enucleation(1 case).Three patients with asynchronous BRCC undergone two-stage operation.The treatment included bilateral tumor enucleation(2 cases) and unilateral radical nephrectomy plus contralateral tumor enucleation(1 case).All of patients received biological therapy for 3 months.Ten patients were followed up of 3 months to 8 years,and the mean time was 19 months.RESULTS:Seven patients were alive without tumors recurrence or meatastasis.One had lung meatastasis at 6 months after surgery but was still alive for 3 months after that.One had local recurrence 1 year after operation but was still alive for 3 months after the biological therapy.One died of renal function failure after operation.After 9 months,2 years and 6 years,tumors on contralateral kidney appeared in patients who had undergone radical nephrectomy of asynchronous BRCC.CONCLUSIONS: The nephron sparing surgery is a safe and reasonable method at present with the bilateral renal cell carcinoma.The Prognosis of the carcinoma relates to the stage and grade.It is no difference between the bilateral renal cell carcinoma and the single one.

Abstract:Objective To present the experience of retroperitoneal laparoscopic surgery for bilateral synchronous renal cell carcinoma. Methods A patient with bilateral synchronous renal cell carcinoma (tumor in right kidney is 8 cm, and tumors in left kidney are 4.5 cm and 1 cm) received unilateral retroperitoneal laparoscopic nephron-sparing surgery and treatment of sunitinib after the first operation. 4 weeks later, the patient underwent contralateral retroperitoneal laparoscopic radical nephrectomy. Results Retroperitoneal laparoscopic surgery was performed successfully without conversion to open surgery. Operative time was 100 minutes and 110 minutes, respectively. Blocking time of renal artery branch was 30 minutes in the first operation. The level of serum creatinine raised to 252μmol/L and decreased slowly to 168μmol/L after 6 months. Conclusion Bilateral synchronous renal cell carcinoma is very rare. Retroperitoneal laparoscopic surgery is safe, effective and feasible for this disease with faster recovery. Two-stage operation is more favorable for the recovery of renal function. And targeted therapy has satisfied efficacy in the period.

Abstract:Objectives To introduce our successful experience on treating synchronous bilateral renal cell carcinoma( BRCC)with operation and multi - target drug.Methods From Jan 2011 to Dec 2011,3 cases of BRCC were treated by multi - target drug(Sunitinib) combined with operation in our institute.There were 2 males and 1 females with the average age of 48 years old( ranging from 40 to 59 years old).Two cases were found bilateral renal tumor during annual physical examination,while 1 case complained of sore waist.All cases were confirmed by ultrasounography and CT scan pre - operatively.Among the 3 cases,3 tumors were located at the upper pole of kidney,1 at the middle and 2 at the lower pole.Three tumors were on the left kidney,3 on the tight side.The mean tumor size was 3.5 cm in diameter( 2.0 ～ 5.5 cm).All of the 3 cases underwent bilateral nephron sparing surgery(NSS) with an interval of 8 to 10 weeks.Sunitinib were administrated during the intervals.Results NSS was successfully performed in all 3 cases with no blood transfusion.The estimated blood loss ranged from 200 ～ 400ml.The pathologic findings of all 6 tumors were clear cell carcinoma.After taking sunitinib during intervals,all tumors volume decreased at a certain degree.No case suffered urinary fistula,bleeding,or infection postoperatively.Local recurrence or distal metastasis were not observed during a mean follow - up period of 9 months(5 ～ 11 months).Conclusions Targeted drug - assisted NSS treatment for the synchronous bilateral renal cell carcinoma could reduce the tumor stage and increase the success rate of NSS.Furthermore,this method could allow NSS treatment in patients deemed to receive radical nephrectomy.

Abstract:OBJECTIVE:To retrospectively review the safety and efficacy of ultrasound (US)-guided percutaneous microwave ablation (MWA) of bilateral renal cell carcinomas (BRCCs).PATIENTS AND METHODS:In this study, eight patients of BRCCs with 20 lesions seen from November 2012 to October 2018 were enrolled retrospectively. The patients were treated with US -guided percutaneous MWA and were followed up with contrast-enhanced US and computed tomography or magnetic resonance imaging.RESULTS:Technical success was achieved 20 tumors. The 18 lesions were performed to ablate 90%; 2 lesions were repeated because of detecting residual ablated tumor. Except six patients were noted fever, no severe complications occurred. The median follow-up time was 24 months (range 7-59 months). Among the eight patients, there was no local tumor recurrence in 6 of 8 completely ablated tumor lesions. One patient was retreated by MWA 6 months after ablation, and another was retreated after 19 months due to local tumor recurrence. No patients have obvious change of renal function and lost their life.CONCLUSION:US-guided percutaneous MWA is a beneficial treatment for BRCC.
Treatment Options for Signet Ring Cell Carcinoma of the Urinary Bladder: A Population-Based Study

Abstract:Objectives: Signet ring cell carcinoma (SRCC) of the urinary bladder is a rare and highly aggressive form of bladder cancer, with no widely agreed-upon treatment strategy. The aim of this study was to identify important factors influencing patient prognosis and to assess how various treatment approaches affect survival outcomes. Methods: A retrospective study was conducted using data from the Surveillance, Epidemiology, and End Results (SEER) Program, including patients with bladder primary SRCC who were presented between 2000 and 2017. Univariate and multivariate Cox regression models were used to examine the impact of various factors on cancer-specific survival (CSS) and overall survival (OS). Propensity score matching (PSM) and inverse probability of treatment weighting (IPTW) were applied to homogenize both groups. The impact of different treatment regimens on patient CSS and OS was analyzed using the Kaplan-Meier method. Results: A total of 33 cases of non-muscular invasive SRCC and 210 cases of muscular invasive SRCC were included in this study. Multivariate analysis identified race, TNM stage, and surgical method as independent variables influencing both OS and CSS. In non-muscle invasive bladder SRCC patients, radical cystectomy showed no CSS benefit compared to transurethral resection of bladder tumors (P = 0.304). For muscle invasive SRCC, patients who underwent partial cystectomy had better OS and CSS compared to those who underwent radical cystectomy (P = 0.019, P = 0.024). However, after conducting a PSM analysis, the differences between the two surgical outcomes were not statistically significant (P = 0.504, P = 0.335). Lymphadenectomy, chemotherapy, and radiation did not show any benefit to the prognosis of patients. Conclusion: This study identified race, TNM stage, and surgical approach as significant independent predictors for SRCC outcomes. Simple radical cystectomy and partial cystectomy proved to be effective treatments for SRCC. The optimal treatment option still needs to be supported by a number of prospective research trials.
Treatment and survival of non-metastatic small cell carcinoma of the bladder from multiple centers in China

Abstract:Small cell carcinoma of the bladder (SCCB) is a rare, highly malignant neuroendocrine tumor. This study attempted to analyze tumor characteristics, treatments and clinical outcomes in China. We conducted a retrospective analysis of patients diagnosed with non-metastatic SCCB at multi-institutions between January 2007 and January 2022. The Kaplan-Meier method was used to calculate survival. A total of 20 patients were included. 10 had localized disease (T1-2N0), and 10 had locally advanced disease (≥ T3 or N+). 13 received local treatment (partial cystectomy or transurethral resection of the bladder tumor) and 7 received radical treatment (radical cystectomy or radiotherapy). A total of 18 patients (90%) received chemotherapy (CT), either neoadjuvant CT ( n = 5) or adjuvant CT ( n = 13). The median OS for the receiving local treatment was 65.3 months (95% CI 0 to 138 months) and the corresponding 1-year, 2-year, and 3-year OS was 77%, 54%, and 54%, respectively. The median OS for the receiving radical treatment was not reached and the corresponding 1-year, 2-year, and 3-year OS was 100%, 100%, and 75%, respectively. The median PFS for receiving local treatment was 13.8 months (95% CI 9.3 to 18.3 months) and the corresponding 1-year, 2-year, and 3-year PFS was 46%, 31%, and 31%, respectively. The median PFS for the receiving radical treatment was not reached and the corresponding 1-year, 2-year, and 3-year PFS was 83%, 56%, and 56%, respectively. This study reported the largest cohort of non-metastatic SCCB among Chinese population. Given its metastatic potential, CT remained an essential part of the treatment. The survival outcomes of radical cystectomy and RT in non-metastatic SCCB were encouraging.

Abstract:BACKGROUND:Renal cell carcinomas are usually unilateral. However, they are bilateral in 2% to 4% of sporadic cases and is considerably more common in familial cases. Synchronous sporadic bilateral multiple chromophobe renal cell carcinoma (CHRCC) with different subtypes is rare.CASE SUMMARY:In this case report, we describe a case of synchronous bilateral CHRCC with two histological variants, accompanied by a clear cell carcinoma and a cyst in a 50-year-old male. The patient underwent retroperitoneal laparoscopic bilateral nephron-sparing surgery and there was no serious postoperative renal dysfunction.CONCLUSION:We report a rare case of synchronous bilateral CHRCC with two histological variants associated with a clear cell carcinoma and a cyst.
