Abstract:1421 Background: Follicular lymphoma (FL) is the common subtype of non-Hodgkin lymphomas and is FDG-avid in more than 95% of the cases. It is the most common indolent lymphoma with quite heterogeneous clinical course and different treatment options ranging from watch and wait to chemotherapy and/or autologous stem cell transplant. It is essential to accurate evaluate the patient at baseline, including histological grade, clinical staging and treatment indications to make the right treatment decision. A number of scientific reports focusing on the role of 18F-Fluro-deoxy-glucose positron emission tomography (FDG PET) combined with computed tomography (CT) in FL staging, restaging and prognostication in recent years. However, there are few reports on treatment decisions making. Objectives: Our aim was to evaluate the effect of 18F-FDG PET / CT on treatment decisions in patients with FL. Methods: Forty-seven histologically proven untreated FL patients were enrolled in the study, including 23 males and 24 females, with a median age of 49 years. There were 34 cases of grade 1-2 FL, 13 cases of grade 3. There were 14 cases of stage I and II, 8 cases of stage III, 25 cases of stage IV. All patients underwent 18F-FDG PET / CT. The size, number and SUVmax of all the lesions was observed and recorded respectively. The heterogeneous of FDG uptake was observed and extremely high uptake lesions, which may suggest histological large cell transformation, were recorded. The following should also be recorded if exist: involvement of at least three nodal areas, each of which with a diameter larger than 3 cm, any nodal or extranodal mass with a diameter larger than 7 cm, substantial splenic enlargement and pleural effusion or ascites. Record the number of patients whose treatment decisions have changed after the PET / CT examination and the reasons for the changes. Results: Among the patients of stage I and II, 9 changed their treatment decision after PET/CT. 2 were because of PET / CT prompted large cell transformation and confirmed by re-biopsy. 7 were because of up-regulated from stage II to III, and the treatment decision were changed from local radiotherapy to watch and wait since they did not match the treatment indications for III ~ IV patients. Among the patients of stage III and IV, 29 changed their treatment decision after PET/CT. 10 were found more than three nodal areas of which larger than 3 cm in diameter, 16 patients were found larger than 7 cm mass in diameter, 17 were found substantial splenic enlargement, 4 were found pleural effusion. Among them, 13 patients had one treatment indication, 14 patients had two, and two patients had three. Conclusions: 18F-FDG PET / CT helps to make FL patient’s treatment decisions by suggesting histological large cell transformation, clarifying clinical staging and finding treatment indications.
