Abstract:Myeloperoxidase (MPO) is a common target antigen of anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis and is recognized in one-third of patients with anti-glomerular basement membrane (GBM) disease. Our previous study identified over 60% of patients with anti-GBM disease recognizing linear peptides of MPO heavy chain. Here we tested whether aberrant glycosylation alters MPO antigenicity through exposure of neo-epitopes on MPO molecules. Atypical glycosylated MPO molecules, including all possible glycosylation types, were prepared by exoglycosidase and endoglycosidase treatments. Antibodies were detected from the sera of 40 patients with anti-GBM disease without the coexistence of MPO-ANCA. Circulating antibodies against aberrant glycosylated MPO existed in 21 of these patients. Non-glycan MPO and MPO with only N-acetylglucosamine had high frequencies of recognition (16 and 15 patients, respectively). Antibodies binding to aberrant glycosylated MPO could not be inhibited by intact MPO or GBM antigen. When applied to ethanol-fixed neutrophils from normal individuals, these antibodies yielded a typical cytoplasmic staining pattern (c-ANCA). Antigen specificity was detected in 90% of the antibodies using five peptides containing one of the five N-glycosylation sites each, mostly on N323, N355, and N391. The antibodies were restricted to IgG1 subclass, could activate complement, and induce neutrophil degranulation in vitro. Thus, aberrant glycosylated MPO exposed neo-epitopes and was recognized by half of the patients with anti-GBM disease. Their antibodies possessed pathogenic characteristics and may be associated with kidney injury.
Influence of Myeloperoxidase by Anti-Myeloperoxidase Antibodies and Its Association with the Disease Activity in Microscopic Polyangiitis

Abstract:Methods. MPO-ANCA was purified with MPO-affinity chromatography from plasma of 13 consecutive patients with MPO-ANCA-positive MPA. The titre, avidity, immunoglobulin G (IgG) subclasses and the ability to reverse the binding between ceruloplasmin and MPO of MPO-ANCA were tested using ELISA. The oxidation activity of MPO in the presence of MPO-ANCA was measured. The associations between the oxidation activity of MPO after binding MPO-ANCA and the clinical and immunological characteristics of MPO-ANCA were analysed.Results. The MPO activity without binding to its autoantibodies was 1.521 (0.12) (expressed by A-values at 10 min, using o-phenylenediamine as substrate). After binding affinity-purified MPO-ANCA, the MPO activity ranged from 1.083 to 1.642. Correlation analysis showed that the oxidation activity of MPO after binding MPO-ANCA positively correlated with the level of ceruloplasmin binding on MPO in presence of MPO-ANCA (r = 0.617, P = 0.025) and negatively correlated with initial sera creatinine levels (r = -0.564, P = 0.045), BVAS (r = -0.735, P = 0.004), avidity of MPO-ANCA (r = -0.575, P = 0.040), levels of IgG2 (r = -0.610, P = 0.027) and IgG3 subclasses (r = -0.695, P = 0.008) of MPO-ANCA.Conclusions. MPO-ANCA from different patients can influence MPO activity with different levels. The low oxidation activity of MPO after binding MPO-ANCA might be associated with more severe disease.
Influence of Myeloperoxidase-Catalyzing Reaction on the Binding Between Myeloperoxidase and Anti-Myeloperoxidase Antibodies

Abstract:In the current study, whether myeloperoxidase (MPO)–catalyzing reaction could influence the antigenicity of MPO was investigated. Hypochlorite acid, the main product of the catalytic reaction, could lower the binding between MPO–antineutrophil cytoplasmic antibodies (ANCA) and MPO when the available chlorine was higher than 0.031×10−3 g/l. After MPO-catalyzing reaction with H2O2 lower than 0.469 g/l, the binding level between MPO–ANCAcontaining plasma and MPO increased slightly. The peak binding level was 1.135 ± 0.205 (expressed by the absorbance value at 405 nm). However, with the existence of hydrogen donor (o-phenylenediamine) in the reaction system, the peak binding level between MPO-ANCA–containing plasma and post-catalyzing MPO was significantly higher (1.367 ± 0.321 vs 1.135 ± 0.205, p = 0.023). Moreover, at the approximately physical concentration of H2O2 (0.02 g/l), MPO-ANCA exhibited higher titer to post-catalyzing MPO than to pre-catalyzing MPO (3.91 ± 0.84 vs 3.57 ± 0.84, p < 0.001, expressed as the lgT). These data demonstrated that MPO-catalyzing reaction could potentially increase the antigenicity of MPO.

Abstract:Objective:The interaction between neutrophils and activation of alternative complement pathway plays a critical role in the pathogenesis of ANCA-associated vasculitis (AAV). MPO, which can be released from ANCA-stimulated neutrophils, was recently demonstrated to be capable of activating the alternative complement pathway. Here we aimed to investigate the interaction between MPO and factor H (FH), a key regulator of the alternative pathway, and its effect on the functional activities of FH.Methods:Detection of FH and MPO on neutrophil extracellular traps (NETs) induced by serum from AAV patients and in kidney biopsies of AAV patients was performed by immunostaining. In vitro binding between MPO and FH was examined by ELISA and surface plasmon resonance. The influence of MPO on the complement regulatory activity of FH was further assessed.Results:FH deposited and co-localized with MPO in NETs. In kidney biopsies from AAV patients, MPO was closely adjacent to FH in glomerular capillaries. We demonstrated that MPO binds to FH with an apparent nanomolar affinity and identified short consensus repeats 1-4 of FH as the major binding sites. In terms of functional analysis, MPO inhibited the interaction between FH and C3b and the decay-accelerating activity of FH. The fluid phase and surface cofactor activities of FH upon C3b inactivation were inhibited by MPO.Conclusion:Our findings indicate that MPO binds to FH and influences the complement regulatory activity of FH. MPO-FH interaction may participate in the pathogenesis of AAV by contributing to activation of the alternative complement pathway.

Abstract:In patients with active anti-neutrophil cytoplasmic Ab (ANCA)-associated vasculitis (AAV), there are high levels of circulating C-reactive protein (CRP), which can inhibit the alternative complement pathway by binding factor H and triggering the classical complement pathway by binding C1q. However, the alternative, not the classical, complement pathway has been proven to play an important role in AAV. We found that both purified myeloperoxidase (MPO) and MPO released from ANCA-stimulated neutrophils could bind modified CRP (mCRP), but not pentameric CRP. Furthermore, MPO could block the binding between mCRP and factor H, as well as the binding between mCRP and C1q. Binding with mCRP did not influence the enzymatic activity of MPO. Binding with mCRP also did not influence the binding between MPO and its physical inhibitor, ceruloplasmin, as well as the binding between MPO and MPO-ANCA. The results indicated that MPO might be a complement regulator and inhibit the negative regulatory effect of CRP on the alternative complement pathway.
High Potential to Reverse the Inhibition of Myeloperoxidase by Ceruloplasmin of Anti-Myeloperoxidase Autoantibodies of IgG3 Subclass

Abstract:Among the four subclasses, anti-neutrophil cytoplasmic antibodies (ANCAs) against myeloperoxidase (MPO) IgG, IgG3 subclass was associated with severe disease. This study aimed to investigate some aspects of the in vitro pathogenic potential of MPO-ANCA of IgG3 subclass. IgG from plasma of 10 consecutive patients with positive MPO-ANCA was divided into the IgG poor in IgG3 and the IgG rich in IgG3 with protein A. Then, the ability to influence the oxidation activity of MPO, the ability to induce respiratory burst of neutrophils, and the ability to reverse the binding between ceruloplasmin (CP) and MPO were compared between the two fractions. The avidity of the MPO-ANCA-positive IgG rich in IgG3 was higher than that of the MPO-ANCA-positive IgG poor in IgG3 in 7 out of 10 patients. At the same antigen-binding level, the level of respiratory burst of neutrophils induced by the MPO-ANCA-positive IgG rich in IgG3 was significantly higher than that induced by the MPO-ANCA-positive IgG poor in IgG3 (1429.70 ± 137.85 vs. 1321.70 ± 244.51, P = 0.049, expressed by the mean fluorescence intensity); the binding level between MPO and CP after adding the MPO-ANCA-positive IgG rich in IgG3 was significantly lower than after adding the MPO-ANCA-positive IgG poor in IgG3 (0.599 ± 0.214 vs. 0.777 ± 0.353, P = 0.010, expressed by A 405); there was no significant difference in the MPO activity between adding the MPO-ANCA-positive IgG poor in IgG3 and rich in IgG3 (P = 0.217). In conclusion, MPO-ANCA of IgG3 subclass might have stronger ability to reverse the inhibition of MPO by CP than other subclasses.
Platelet Activation is Induced Via Myeloperoxidase Production and Anti-Myeloperoxidase Antibodies Primed by Thrombin

Abstract:In addition to their central role in hemostasis and thrombosis, platelets involve in inflammatory responses. Anti-neutrophil cytoplasmic autoantibodies (ANCAs) directed to myeloperoxidase (MPO-ANCAs) are the main serological markers of ANCA-associated vasculitis. The current study demonstrated that MPO-ANCAs caused normal human platelets to activate, primed by low doses of thrombin. Purified immunoglobulin G, MPO-ANCAs and their F(ab')2 fragments evidently increased the CD62P expression of platelets compared with controls. The existence of MPO in platelets was confirmed with mass spectrometry technology, after analyzing the F(ab')2 fragment pull-downed proteins. After thrombin priming, platelets not only expressed MPO on the surface with an activation-dependent manner, but also released the enzyme to extracellular space. These effects markedly enhanced the activation of platelets. Additionally, there was no apparently increased CD62P-expression on platelets in MPO-ANCAs group compared with controls when adding MPO inhibitor, proving that primed platelets have ANCA antigen at their surfaces interacting with the autoantibodies. High Mobility Group Box-1 Contributes to Anti-Myeloperoxidase Antibody-Induced Glomerular Endothelial Cell Injury Through a Moesin-Dependent Route

Abstract:Background Our previous study found that circulating and urinary levels of high mobility group box-1 (HMGB1) were closely associated with disease activity in patients with antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV). Moreover, HMGB1 participates in ANCA-induced neutrophil activation. Cross-reactivity between moesin and anti-myeloperoxidase (MPO) antibody has been reported in both human and mouse. The current study investigated whether HMGB1 participated in MPO-ANCA-induced glomerular endothelial cell (GEnC) injury, which is one of the most important aspects in the pathogenesis of AAV. Methods The effects of HMGB1 on expression of moesin on GEnCs and anti-MPO antibody binding to GEnCs were measured. MPO expression on GEnCs was explored. The effects of HMGB1 in MPO-ANCA induced GEnC injury were measured, during which the role of moesin was explored. Antagonists for various relevant receptors were employed. Results Sera from AAV patients at the active stage could mediate GEnC injury, while this effect could be attenuated by preblocking HMGB1. HMGB1 could increase the expression of moesin on GEnCs and the binding of anti-MPO antibody to moesin. The colocalization of moesin expression and anti-MPO antibody binding can be detected. Little, if any, MPO was expressed in GEnCs. HMGB1 increased GEnC activation and injury in the presence of patient-derived MPO-ANCA-positive IgGs through moesin. The effects of HMGB1 on expression of moesin on GEnCs, anti-MPO antibody binding to GEnCs, GEnC activation and injury were mainly toll like receptor 4 (TLR4) dependent. Conclusions HMGB1 can increase the expression of moesin but not MPO on GEnCs, and can further participate in MPO-ANCA-induced GEnC activation and injury by cross-reactivity between moesin and anti-MPO antibody.

Abstract:Myeloperoxidase (MPO) is a heme-containing peroxidase, mainly expressed in neutrophils and, to a lesser extent, in monocytes. MPO is known to have a broad bactericidal ability via catalyzing the reaction of Cl− with H2O2 to produce a strong oxidant, hypochlorous acid (HOCl). However, the overproduction of MPO-derived oxidants has drawn attention to its detrimental role, especially in diseases characterized by acute or chronic inflammation. Broadly speaking, MPO and its derived oxidants are involved in the pathological processes of diseases mainly through the oxidation of biomolecules, which promotes inflammation and oxidative stress. Meanwhile, some researchers found that MPO deficiency or using MPO inhibitors could attenuate inflammation and tissue injuries. Taken together, MPO might be a promising target for both prognostic and therapeutic interventions. Therefore, understanding the role of MPO in the progress of various diseases is of great value. This review provides a comprehensive analysis of the diverse roles of MPO in the progression of several diseases, including cardiovascular diseases (CVDs), neurodegenerative diseases, cancers, renal diseases, and lung diseases (including COVID-19). This information serves as a valuable reference for subsequent mechanistic research and drug development.

Expression of Myeloperoxidase in Patient-Derived Endothelial Colony-Forming Cells—Associations with Coronary Artery Disease and Mitochondrial Function

Abstract:Background and Aims: Myeloperoxidase (MPO) plays a critical role in the innate immune response and has been suggested to be a surrogate marker of oxidative stress and inflammation, with elevated levels implicated in cardiovascular diseases, such as atherosclerosis and heart failure, as well as in conditions like rheumatoid arthritis and cancer. While MPO is well-known in leukocytes, its expression and function in human endothelial cells remain unclear. This study investigates MPO expression in patient-derived endothelial colony-forming cells (ECFCs) and its potential association with CAD and mitochondrial function. Methods: ECFCs were cultured from the peripheral blood of 93 BioHEART-CT patients. MPO expression and associated functions were examined using qRT-PCR, immunochemistry, flow cytometry, and MPO activity assays. CAD presence was defined using CT coronary angiography (CACS > 0). Results: We report MPO presence in patient-derived ECFCs for the first time. MPO protein expression occurred in 70.7% of samples (n = 41) which had nuclear co-localisation, an atypical observation given its conventional localisation in the granules of neutrophils and monocytes. This suggests potential alternative roles for MPO in nuclear processes. MPO mRNA expression was detected in 66.23% of samples (n = 77). CAD patients had a lower proportion of MPO-positive ECFCs compared to non-CAD controls (57.45% vs. 80%, p = 0.04), a difference that persisted in the statin-naïve sub-cohort (53.85% vs. 84.62%, p = 0.02). Non-CAD patients with MPO expression showed upregulated mitochondrial-antioxidant genes (AIFM2, TXNRD1, CAT, PRDX3, PRDX6). In contrast, CAD patients with MPO gene expression had heightened mROS production and mitochondrial mass and decreased mitochondrial function compared to that of CAD patients without MPO gene expression. Conclusions: MPO is present in the nucleus of ECFCs. In non-CAD ECFCs, MPO expression is linked to upregulated mitochondrial-antioxidant genes, whereas in CAD ECFCs, it is associated with greater mitochondrial dysfunction.

Inhibition of Oxidative Activity of Myeloperoxidase by Anti-Myeloperoxidase Antibodies from Patients with Microscopic Polyangiitis

Abstract:OBJECTIVE:To investigate the inhibitory effects on myeloperoxidase (MPO) oxidation activity by affinity-purified anti-MPO antibodies from patients with microscopic polyangiitis (MPA) and to further investigate the interaction between MPO, ceruloplasmin and anti-MPO antibodies.METHODS:Human IgG fractions were purified from plasma of 11 patients with anti-MPO antibody positive MPA and sera of 12 normal controls. Anti-MPO antibodies were further purified from anti-MPO antibody containing IgG fractions using MPO affinity chromatography. The enzyme activity of MPO was measured, in the presence of anti-MPO antibodies and normal IgG preparations, using a classical MPO oxidation assay. Interaction between ceruloplasmin, MPO and anti-MPO antibodies was further investigated using ELISA.RESULTS:Anti-MPO antibodies from 7/11 patients with MPA could inhibit the MPO activity as non-competitive inhibitors in a dose-dependent manner. Ceruloplasmin could competitively inhibit the oxidation activity of MPO in a dose-dependent and time-dependent manner. Anti-MPO antibodies could inhibit the binding between MPO and ceruloplasmin to a maximum of (75.4+/-11.6)%.CONCLUSION:Anti-MPO antibodies, from the majority of patients with MPA, could inhibit the oxidation activity of MPO and interfere with the binding between MPO and ceruloplasmin.
Released Myeloperoxidase Attenuates Neutrophil Migration and Accumulation in Inflamed Tissue

Abstract:Neutrophil (PMN) recruitment to sites of insult is critical for host defense, however excessive PMN activity and tissue accumulation can lead to exacerbated inflammation and injury. Myeloperoxidase (MPO) is a PMN azurophilic granule enzyme, which together with H2O2, forms a powerful antimicrobial system designed to kill ingested bacteria. Intriguingly, in addition to intracellular killing of invading microorganisms and extracellular tissue damage due generation of ROS, soluble MPO has been directly implicated in modulating cellular responses and tissue homeostasis. In the current work, we used several models of inflammation, murine and human PMNs and state-of-the-art intravital microscopy to examine the effect of MPO on PMN migration and tissue accumulation. We found that in the absence of functional MPO (MPO knockout, KO mice) inflammatory PMN tissue accumulation was significantly enhanced. We determined that the elevated numbers of PMNs in MPO knockout mice was not due to enhanced viability, but due to increased migratory ability. Acute PMN migration in models of zymosan-induced peritonitis or ligated intestinal loops induced by intraluminal administration of PMN-chemokine CXCL1 was increased over 2-fold in MPO KO compared to wild type (WT) mice. Using real-time intravital imaging of inflamed mouse cremaster muscle and ex vivo PMN co-culture with inflamed endothelial cells (ECs) we demonstrate that elevated migration of MPO KO mice was due to enhanced adhesive interactions. In contrast, addition of soluble recombinant MPO both in vivo and ex vivo diminished PMN adhesion and migration. Although MPO has been previously suggested to bind CD11b, we found no significant difference in CD11b expression in either resting or activated PMNs and further showed that the MPO binding to the PMN surface is not specific to CD11b. As such, our data identify MPO as a novel regulator of PMN trafficking in inflammation.
Influence of variable domain glycosylation on anti-neutrophil cytoplasmic autoantibodies and anti-glomerular basement membrane autoantibodies

Abstract:Background The pathophysiological significance of variable region glycosylation of autoantibodies is still unclear. In the current study, the influence of the variable region N-linked oligosaccharides on the reactivity of three autoantibody specificities was investigated with Sambucus nigra agglutinin (SNA), which mainly binds to oligosaccharides with terminal α2, 6-linked sialic acid on the variable region of IgG. Methods Twenty-seven patients with serum positive anti-neutrophil cytoplasmic autoantibodies (ANCA) against myeploperoxidase (MPO) or proteinase 3 (PR3), or autoantibodies against glomerular basement membrane (GBM) were included. Total IgG was isolated and separated into non-SNA-binding and SNA-binding fractions with SNA affinity chromatography. Antigen-specific IgG was purified by immunoaffinity chromatography. Results At the same concentration of IgG, the antigen binding level of non-SNA-binding IgG was significantly lower than that of SNA-binding IgG for MPO-ANCA (absorbance value at 405 nm, 0.572 ± 0.590 vs . 0.962 ± 0.670, P < 0.001) and for PR3-ANCA (0.362 ± 0.530 vs . 0.560 ± 0.531, P = 0.003). The antigen binding level of non-SNA-binding IgG was significantly higher than that of SNA-binding IgG for anti-GBM antibodies (1.301 ± 0.594 vs . 1.172 ± 0.583, P = 0.044). The level of variable region glycosylation of total IgG was significantly lower than that of affinity-purified MPO-ANCA (1.021 ± 0.201 vs . 1.434 ± 0.134, P = 0.004). The level of variable region glycosylation of total IgG was significantly higher than that of affinity-purified anti-GBM antibodies (1.034 ± 0.340 vs . 0.734 ± 0.333, P = 0.007). The SNA-binding fraction of MPO-ANCA-containing IgG and PR3-ANCA-containing IgG induced higher levels of neutrophil oxygen radical production than the corresponding non-SNA-binding fractions (P < 0.001 and P = 0.043, respectively). The level of variable region glycosylation of affinity-purified MPO-ANCA was higher in active AAV than the same patients in remission (P = 0.001). Conclusion Characteristics of variable region glycosylation of ANCA and anti-GBM antibodies were different from that of total IgG, which might influence the antigen-binding ability of these antibodies. Variable region glycosylation of ANCA might influence the effect of ANCA-induced neutrophils respiratory burst.

Abstract:Background: It is demonstrated that levels of protein-bound chlorotyrosine, nitrotyrosine and myeloperoxidase (MPO), a protein that catalyzes generation of chlorinating and nitrating oxidants, serve as independent predictors of cardiovascular disease. Methods: Immunoprecipitation and Western blot were used to analyze protein concentration, nitration and chlorination. LC-MS/MS was used to identify nitrated and chlorinated sites of Tyr from immunoprecipitated serum proteins. Results: Apolipoprotein A-I (apoA-I), the primary protein constituent of high density lipoprotein (HDL), was identified as a selective target for MPO-catalyzed nitration and chlorination in patients with type 2 diabetes. The serum proteins from diabetic subjects showed that the levels of apoA-I nitration and chlorination were clearly increased, whereas apoA-I concentration and cholesterol efflux activity were significantly decreased. MPO as a likely mechanism for oxidative modification of apoA-I in vivo was apparently facilitated by MPO binding to apoA-I. Subsequently, it was found that Tyr 192 was the major nitration and chlorination site in apoA-I from diabetic serum. Further studies in vitro revealed that besides the classic inhibition in cholesterol efflux activities, MPO-catalyzed oxidation could result in a loss of anti-apoptotic activity of lipoprotein. Conclusions: ApoA-I undergoes MPO-mediated oxidation in serum from diabetic patients compared to non-diabetic subjects and MPO-catalyzed modification may impair the anti-apoptotic properties of HDL in vitro.

Abstract:OBJECTIVE:Antineutrophil cytoplasmic antibody (ANCA) is an important serological diagnostic tool for certain vasculitides, such as Wegener's granulomatosis and microscopic polyangiitis. Several studies suggested that ANCA titers correlated with disease activity. However, in some patients, ANCA still remained positive when patients were in clinical remission. The major isotype of ANCA is IgG, which has four subclasses. This study is to investigate the relationship between anti-MPO IgG subclasses and vasculitis activity.METHODS:Serum samples, taken from 30 anti-myeloperoxidase (MPO) antibody positive patients both in active phase and in remission, were analysed by ELISA for their anti-MPO IgG subclass distribution and the change of anti-MPO IgG subclasses was compared with the change of anti-MPO IgG antibodies.RESULTS:All four anti-MPO IgG subclasses were positive in active phase, IgG(4) being the highest. In remission, the level of anti-MPO IgG(1) and IgG(3) subclasses decreased the most and the percentage of negative change of IgG(3) was the highest, even more significant than that of the total IgG.CONCLUSION:anti-MPO IgG(3) antibody is more closely related with clinical disease activity than the total IgG. Higher ratio of IgG(4) may suggest that chronic repeated antigen stimuli may play a role in the production of ANCA.

Abstract:ObjectiveWe have previously reported that myeloperoxidase-deficient (MPO−/−) neutrophils produce greater amounts of macrophage inflammatory protein-2 (MIP-2) upon in vitro stimulation with zymosan than wild-type neutrophils. This study aimed to examine the effect of MPO deficiency on the expression of other cytokines and chemokines.MethodsWild-type and MPO−/− neutrophils isolated from peritoneal cavity were stimulated with zymosan in vitro. Secretion of MIP-1α, MIP-1β, interleukin (IL)-1α, IL-1β, and tumor necrosis factor (TNF)-α by neutrophils was quantified by ELISA. mRNA expression in the neutrophils was analyzed by real-time reverse transcription-PCR, and the phosphorylation of extracellular-signal regulated kinase (ERK) 1/2 and p38 mitogen activated protein kinase (MAPK) in neutrophils was analyzed by western blot. For in vivo studies, mice were inoculated with zymosan intranasally, and the levels of these cytokines and chemokines were measured in the lungs.ResultsThe MPO−/− neutrophils stimulated by zymosan expressed and secreted significantly higher levels of MIP-1α, MIP-1β, IL-1α, IL-1β, and TNF-α than the stimulated wild-type cells. Expression of all of these inflammatory mediators was blocked by pre-treatment with BAY11-7082, U0126, and SB203580, which are inhibitors of nuclear factor (NF)-κB, ERK1/2, and p38 MAPK, respectively. Enhanced expression of these inflammatory mediators is associated with elevated activation of ERK1/2 in stimulated MPO−/− neutrophils. In vivo, MPO−/− mice had significantly higher numbers of alveolar neutrophils and increased production of MIP-1α, MIP-1β, IL-1α, IL-1β, and TNF-α relative to the responses seen in wild-type mice within 24 h of zymosan administration.ConclusionMPO deficiency upregulates the expression of several proinflammatory cytokines and chemokines in mouse neutrophils.

Abstract:Introduction: Myocardial infarction (MI) is a significant contributor to morbidity and mortality worldwide. Many individuals who survive the acute event continue to experience heart failure (HF), with inflammatory and healing processes post-MI playing a pivotal role. Polymorphonuclear neutrophils (PMN) and monocytes infiltrate the infarcted area, where PMN release high amounts of the heme enzyme myeloperoxidase (MPO). MPO has numerous inflammatory properties and MPO plasma levels are correlated with prognosis and severity of MI. While studies have focused on MPO inhibition and controlling PMN infiltration into the infarcted tissue, less is known on MPO's role in monocyte function. Methods and results: Here, we combined human data with mouse and cell studies to examine the role of MPO on monocyte activation and migration. We revealed a correlation between plasma MPO levels and monocyte activation in a patient study. Using a mouse model of MI, we demonstrated that MPO deficiency led to an increase in splenic monocytes and a decrease in cardiac monocytes compared to wildtype mice (WT). In vitro studies further showed that MPO induces monocyte migration, with upregulation of the chemokine receptor CCR2 and upregulation of inflammatory pathways identified as underlying mechanisms. Conclusion: Taken together, we identify MPO as a pro-inflammatory mediator of splenic monocyte recruitment and activation post-MI and provide mechanistic insight for novel therapeutic strategies after ischemic injury.

Abstract:Oxidative stress plays a fundamental role in abdominal aortic aneurysm (AAA) formation. Activated polymorphonuclear leukocytes (or neutrophils) are associated with AAA and express myeloperoxidase (MPO), which promotes inflammation, matrix degradation, and other pathological features of AAA, including enhanced oxidative stress through generation of reactive oxygen species. Both plasma and aortic MPO levels are elevated in patients with AAA, but the role of MPO in AAA pathogenesis has, heretofore, never been investigated. Here, we show that MPO gene deletion attenuates AAA formation in two animal models: ANG II infusion in apolipoprotein E-deficient mice and elastase perfusion in C57BL/6 mice. Oral administration of taurine [1% or 4% (wt/vol) in drinking water], an amino acid known to react rapidly with MPO-generated oxidants like hypochlorous acid, also prevented AAA formation in the ANG II and elastase models as well as the CaCl2 application model of AAA formation while reducing aortic peroxidase activity and aortic protein-bound dityrosine levels, an oxidative cross link formed by MPO. Both MPO gene deletion and taurine supplementation blunted aortic macrophage accumulation, elastin fragmentation, and matrix metalloproteinase activation, key features of AAA pathogenesis. Moreover, MPO gene deletion and taurine administration significantly attenuated the induction of serum amyloid A, which promotes ANG II-induced AAAs. These data implicate MPO in AAA pathogenesis and suggest that studies exploring whether taurine can serve as a potential therapeutic for the prevention or treatment of AAA in patients merit consideration.NEW & NOTEWORTHY Neutrophils are abundant in abdominal aortic aneurysm (AAA), and myeloperoxidase (MPO), prominently expressed in neutrophils, is associated with AAA in humans. This study demonstrates that MPO gene deletion or supplementation with the natural product taurine, which can scavenge MPO-generated oxidants, can prevent AAA formation, suggesting an attractive potential therapeutic strategy for AAA.

Abstract:Neutrophil-myeloperoxidase (MPO) is a heme-containing peroxidase which produces excess amounts of hypochlorous acid during inflammation. While pharmacological MPO inhibition mitigates all indices of experimental colitis, no studies have corroborated the role of MPO using knockout (KO) models. Therefore, we investigated MPO deficient mice in a murine model of colitis. Wild type (Wt) and MPO-deficient mice were treated with dextran sodium sulphate (DSS) in a chronic model of experimental colitis with three acute cycles of DSS-induced colitis over 63 days, emulating IBD relapse and remission cycles. Mice were immunologically profiled at the gut muscoa and the faecal microbiome was assessed via 16S rRNA amplicon sequencing. Contrary to previous pharmacological antagonist studies targeting MPO, MPO-deficient mice showed no protection from experimental colitis during cyclical DSS-challenge. We are the first to report drastic faecal microbiota shifts in MPO-deficient mice, showing a significantly different microbiome profile on Day 1 of treatment, with a similar shift and distinction on Day 29 (half-way point), via qualitative and quantitative descriptions of phylogenetic distances. Herein, we provide the first evidence of substantial microbiome shifts in MPO-deficiency, which may influence disease progression. Our findings have significant implications for the utility of MPO-KO mice in investigating disease models.

Abstract:Objective Recent studies have shown that myeloperoxidase (MPO) is involved in the pathogenesis of acute coronary syndrome (ACS). However, there were different results in various studies as to whether MPO could predict the long-term adverse outcomes in patients with ACS. The purpose of this article was to perform a meta-analysis in order to compare the long-term prognosis of ACS patients with high MPO levels and low MPO levels. Methods The literature was scanned by formal searches of electronic databases (PubMed, EMBASE, Medline, OVID and web of knowledge) from inception to July 2010. Studies grouped by MPO level and followed up for at least 30 days were included in this article. The authors decided to use the median MPO level in the studies included as a cut-off to divide the patients into two groups (high/low MPO group). Exclusion criteria were: (1) ongoing studies and (2) irretrievable data. Two investigators independently assessed all titles and abstracts for eligibility, with divergences resolved by consensus. The authors chose Mantel-Haenszel as statistical method, random effects as analysis model, risk ratio (RR) as effect measurement for dichotomous variables. Formal test for publication bias had been carried out using a funnel plot. Results A total of eight clinical trials were included in this meta-analysis, involving 3909 patients. The mean follow-up duration was 2 years. The high MPO level group showed a worse outcome than the low group (14.3% vs 10.4%, risk ratio (RR) 1.83, 95% CI 1.41 to 2.39, p<0.00001). Conclusion ACS patients with high MPO level had a poor long-term prognosis.
