Abstract:To analyze the components of young Chinese eyes with special attention to differences in corneal power, anterior segment length and lens power. Cycloplegic refractions and ocular biometry with LENSTAR were used to calculate lens power with Bennett’s method. Mean refraction and mean values for the ocular components of five different refractive groups were studied with ANOVA and post-hoc Scheffé tests. There were 1889 subjects included with full data of refraction and ocular components. As expected, mean axial length was significantly longer in myopic eyes compared to emmetropes. Girls had steeper corneas, more powerful lenses and shorter eyes than boys. Lens power was lower in boys and also lower in myopic eyes. Lens thickness was the same for both genders but was lower in myopic eyes. Although cornea was steeper in myopic eyes in the whole sample, this was a gender effect (more girls in the myopic group) as this difference disappeared when the analysis was split by gender. Anterior segment length was longer in myopic eyes. In conclusion, myopic eyes have lower lens power and longer anterior segment length, that partially compensate their longer axial length. When analyzed by gender, the corneal power is not greater in low and moderate myopic eyes.

Abstract:PURPOSE: To determine corneal Q value and its related factors in Chinese subjects older than 30 years.DESIGN: Cross sectional study.METHODS: 1,683 participants (1,683 eyes) from the Handan Eye Study were involved, including 955 female and 728 male with average age of 53.64 years old (range from 30 to 107 years). The corneal Q values of anterior and posterior surfaces were measured at 3.0, 5.0 and 7.0mm aperture diameters using Bausch & Lomb Orbscan IIz (software version 3.12). Age, gender and refractive power were recorded.RESULTS: The average Q values of the anterior surface at 3.0, 5.0 and 7.0mm aperture diameters were -0.28±0.18, -0.28±0.18, and -0.29±0.18, respectively. The average Q value of the anterior surface at the 5.0mm aperture diameter was negatively correlated with age (B = -0.003, p<0.01) and the refractive power (B = -0.013, p = 0.016). The average Q values of the posterior surface at 3.0, 5.0, and 7.0mm were -0.26±0.216, -0.26±0.214, and -0.26±0.215, respectively. The average Q value of the posterior surface at the 5.0mm aperture diameter was positively correlated with age (B = 0.002, p = 0.036) and the refractive power (B = 0.016, p = 0.043).CONCLUSION: The corneal Q value of the elderly Chinese subjects is different from that of previously reported European and American subjects, and the Q value appears to be correlated with age and refractive power.

Abstract:Purpose To assess the distribution and factors related to intraocular pressure (IOP) in a Chinese children population in Northern China. Methods 1,238 offspring aged 6 to 18 years, residing in six villages, were recruited for this current study, the Handan Offspring Myopia Study. Participants underwent an interviewer-administered questionnaire and a complete ocular examination, including standardized measurement of IOP with Perkins applanation tonometry. Results 1,648 eyes of 828 children were included in the analysis. The mean IOP was 13.9 +/- 1.6 mmHg. The mean IOP for the study population increased from 13.0 +/- 1.5 mmHg for those 6 to 7 years of age to 14.2 +/- 1.4 mmHg for those 15 years of age or older (P < .001). The mean central corneal thickness (CCT) was 548.7 +/- 32.1 mu m, and had no difference among different age groups and gender. In univariate regression analysis, age (0.12 mmHg per 1 year old), height (0.09 mmHg per 5 cm), weight (0.02 mmHg per kg), body mass index (0.07 mmHg per 1 m/kg2), systolic blood pressure (0.06 mmHg per 5 mmHg), CCT (0.06 mmHg per 5 mu m), and SE (-0.11 mmHg per 1 D) were correlated with IOP. In multivariate regression analysis, higher IOP was only associated with older ages (p = .002) and thicker CCT (p = .001). Conclusion The mean IOP in healthy rural Chinese children aged 6-18 years is about 14 mmHg, which is lower than in adults in the same locality. The mean IOP is slightly increasing with age during childhood, which is opposite to the result among adults. Age and CCT are the major independent factors associated with IOP.

Abstract:Background This study aimed to evaluate the distribution characteristics of corneal and lenticular parameters in Chinese children with myopia and explored their association with other ocular components. Methods In this cross-sectional study, all ocular biometric parameters were measured using a Zeiss IOLMaster 700 Biometry. Simulations of the corneal and lenticular curvature radii were implemented using a customised MATLAB program based on cross-sectional swept-source optical coherence tomography images obtained from the same device. The associations of the calculated and simulated refractive powers of the cornea and lens with other ocular parameters were evaluated. Results In total, 119 children with myopia were recruited. Boys had a deeper anterior chamber and longer axial length (AL) than girls, while girls had steeper anterior corneal and lenticular curvatures and greater corneal and lenticular power. Children aged 10 years and older showed a larger anterior lenticular radius of curvature (sR(al)) and less lenticular power (P-L,P-OCT) than younger participants. There was a significant positive correlation between AL and the anterior corneal radius of curvature, regardless of sex or age. The sR(al) exhibited a significant increasing trend, and P-L,P-OCT exhibited a declining trend with a longer AL only in children younger than 10 years. Conclusion AL is the most influential factor in the determination of spherical equivalent refractive error, while decreases in both corneal and crystalline lens power are significantly inversely correlated with axial elongation.
Corneal biomechanics, refractive error, and axial length in Chinese primary school children

Abstract:Purpose: Low corneal hysteresis is associated with longer axial length in Chinese secondary school children. The authors sought to explore this association in primary school children. Methods: LogMAR presenting visual acuity, cycloplegic refractive error, ocular biometry, central corneal thickness (CCT), and corneal hysteresis (CH) was assessed for children in grades 1 to 3 at an academically competitive urban school in Shantou, China. Results: Among 872 eligible children (mean age, 8.6 ± 2.1 years), 651 (74.7%) completed the examination. Among 1299 examined eyes, 111 (8.5%) had uncorrected vision ≤6/12. Mean spherical equivalent refractive error for all eyes was +0.26 ± 1.41 D, and axial length (AL) was 22.7 ± 0.90 mm. CH for the lowest (mean AL, 21.7 ± 0.39 mm), two middle (mean AL, 22.4 ± 0.15 and 22.9 ± 0.15 mm), and highest quartiles (mean AL, 23.7 ± 0.74 mm) of AL were 10.6 ± 2.1 mm Hg, 10.4 ± 2.1 mm Hg, 10.3 ± 2.3 mm Hg, and 10.2 ± 2.3 mm Hg respectively (age- and gender-adjusted Pearson's correlation coefficient r = -0.052; P = 0.001). In generalized estimating equation models adjusting for age, gender, and CCT, lower CH was significantly associated with longer AL (P < 0.001) and more myopic refractive error (P = 0.001). Conclusions: CH measurement is practical in young children because this is when myopia undergoes its most rapid progression. Prospective follow-up of this cohort at high risk for myopia is under way to determine whether low CH is predictive, or a consequence, of long AL.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3><p>To investigate the distribution characteristics of lens power in highly myopic Chinese children and adolescents and its association with age, axial length (AL) and spherical equivalent (SE).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>Cross-sectional study.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Method</h3><p>A total of 459 highly myopic (SE ≤-5D) children and adolescents aged 4 to 19 years were included. Participants underwent a series of ophthalmic examinations, which included AL, cycloplegic refraction and Pentacam measurements. Lens power was calculated using Bennett's formula with its distribution described by age, AL and SE. Multiple regression was conducted to analyze the associated factors of lens power.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Greater lens power was independently associated with younger age, girls, shorter AL and thicker lens thickness (LT) (standardized β=-0.203, 0.214, -0.379 and 0.492, respectively, all <em>p</em> <0.001). However, significant difference in lens power with age was only found in participants younger than 9 years, after which it reached a plateau (mean difference of 1.23 D and 0.084 D per age group, respectively). The lens power was negatively associated with AL only in participants with AL <27 mm. No correlation was observed between lens power and SE.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Among highly myopic children and adolescents, differences in lens power with age declined significantly since 9 years old, which was 1 year earlier than non-high myopes in previous studies, implying the differences in pathophysiological process between non-high myopia and high myopia. The decoupling of lens power and AL in eyes longer than 27 mm might represent the limited influence of AL on lens power.</p>

Central And Peripheral Corneal Power Change In Myopic Orthokeratology And Its Relationship With 2-Year Axial Length Change

Abstract:PURPOSE. We assessed the relationship between axial length (AL) change and corneal refractive power change induced by orthokeratology (ortho-k) contact lens wear.METHODS. A total of 88 myopic children aged 7 to 12 years were fitted with ortho-k lenses for overnight wear. Axial length was measured before enrollment and every 6 months after initiation of lens wear up to 24 months. Corneal apical refractive power and the mean power of each concentric ring from 0.5 to 7.2 mm in diameter at 0.1-mm intervals were measured using the Pentacam HR, and compared before and after 3 months of ortho-k lens wear. Summed corneal power change (SCPC) was defined as the sum of relative corneal power change within the central 7.2-mm diameter corneal region. Age, baseline spherical equivalent refractive error (SERE), and SCPC were tested against 2-year AL growth in a stepwise multiple linear regression model.RESULTS. Of the 88 myopic children who were successfully fitted with ortho-k lenses, 64 completed the 2-year follow-up period. These completed test subjects experienced a mean axial growth of 0.37 +/- 0.27 mm. Spherical equivalent refractive error and corneal apical refractive power decreased from baseline values of -2.88 +/- 0.96 diopters (D) and 43.47 +/- 1.03 D to -0.20 +/- 0.85 D and 40.61 +/- 1.23 D, respectively, after 3 months of lens wear. The mean SCPC was 10.84 +/- 5.28 D.mm after 3 months. Axial elongation was significantly correlated with SCPC (standardized beta = -0.573, P < 0.001) and baseline age (standardized beta = -0.403, P < 0.001), but not with baseline SERE (beta = 0.081, P = 0.396). The regression model using SCPC and baseline age to predict axial elongation was acceptable (adjusted R-2 = 0.56) and statistically significant (F-2,F-61 = 41.09, P < 0.001).CONCLUSIONS. Axial elongation in children undergoing ortho-k therapy is negatively correlated with the age of the wearer and summed corneal power change from the central to the mid-peripheral cornea. Corneal power change is predictive of myopia progression in orthokeratology

Abstract:Purpose: This study aims to investigate the relationship between corneal refractive power change along three axes (nasal, temporal, and inferior) after orthokeratology (OK) treatment and 2-year axial growth in children. Methods: Thirty-two Chinese children aged from 9 to 14 were fitted with OK. When corneal reshaping process following OK treatment was completed and stabilized, the 3-month topographic outputs were taken as the post-OK data. Corneal refractive powers along the nasal, temporal, and inferior axes were collected over an 8-mm-diameter ring in 1-mm steps using the sagittal power map. The maximum power change along each axis was selected and divided into two subcategories, level 1 and level 2, depending on whether the value was below or above the average. Axial length (AL) was measured every 6 months during a 24-month period. The relationship between the maximum power changes and 2-year axial elongation were analyzed. Results: Twenty-seven subjects completed the 24-month study. After OK treatment, statistically significant steepening (p < 0.05) was observed at the nasal 2 mm and 3 mm; temporal 3 mm; and inferior 2 mm, 3 mm, and 4 mm locations compared with the apical center. AL increased significantly throughout the 24-month observation period (p < 0.001). Changes in corneal refractive power significantly affected axial elongation (nasal, p = 0.001; temporal, p = 0.011; inferior, p = 0.001). Two-year axial elongation in patients with larger corneal power changes (level 2) was reduced by 54% to 69% compared with those with smaller corneal power changes (level 1). Maximum power changes along the three axes were negatively correlated (p < 0.05) with 2-year axial growth. Conclusions: Subjects with larger magnitude of corneal relative peripheral power change along specific axes after OK treatment experienced slower axial elongation by the end of 24 months. This effect might be mediated by the induction of greater amount of relative myopic defocus on the peripheral retina. Our study lends weight to potential OK lens designs for myopia control in children.

Abstract:was a positive correlation between age and axial length (r = -0.432, p << 0.0001) and vitreous chamber depth (r = 0.505, p << 0.0001). Mean corneal power (p < 0.001) and lens power (p < 0.001) were significantly greater in pre-school children than in grade school children, while axial length (p < 0.001) and vitreous chamber depth (p < 0.001) were significantly greater in grade school children. Mean lens thickness (p = 0.01) and lens power (p = 0.07) were higher in grade school children than in middle school children, while axial length (p = 0.024) and vitreous chamber depth (p < 0.001) were higher in the middle school children. Ocular components may play different roles in emmetropization. In pre-school children, decreases in corneal power and lens power compensate for the increased axial length. In the grade school and middle-school children, decreases in lens power and lens thickness compensate for the change. Naturally occurring accommodation can also be caused by vision intensive schoolwork which could potentially lead to increased eye size.

Abstract:BackgroundMyopia subjects usually have long axial lengths and are predisposed to deep anterior chamber depths and open angles. Jiangning eye study is a randomized, population based study in Shanghai to determine whether myopia has an effect on the prevalence of occludable angles.MethodsA total of 2478 residents aged 50 years and over were randomly selected, and 2044 (82.5%) individuals of them participated in ophthalmic examinations between November 2012 and February 2013. Eye examinations included autorefraction, noncontact tonometry, measurement of axial length, anterior chamber depth, lens thickness and spectral domain optical coherence tomography.ResultsThe crude prevalences of iris trabecular meshwork contact in Jiangning Chinese individuals with myopia, emmetropia, and hyperopia are 1.25%, 6.44%, and 7.43%, respectively. There is also iris trabecular meshwork contact and shallow anterior chamber depth in myopia subjects even with long axial lengths.ConclusionsThe prevalence of iris trabecular meshwork contact in myopia subjects is lower than their emmetropia or hyperopia counterparts. Axial length is an important factor associate with occludable angle.

Abstract:Abstract Objective This study aimed to profile ocular biometry parameters and predictors of spherical equivalent refraction (SER) among children with moderate to high hyperopia. Methods Individuals <18 years of age with moderate to high hyperopia were enrolled from November 2015 to November 2021. Participants underwent a series of comprehensive ocular examinations, and were classified as having low hyperopia, that is, SER +0.5 to < +2.0 D or moderate to high hyperopia, that is, SER ≥ +2.0 D. Results A total of 459 and 230 eyes with moderate to high hyperopia and low hyperopia, respectively, were included. Moderate to high hyperopic eyes had a shorter axial length, stronger lens power (24.78 ± 5.47 D vs. 18.74 ± 1.63 D, p < 0.001) and weaker corneal power (42.82 ± 1.75 D vs. 43.31 ± 1.55 D, p < 0.001) than low hyperopic eyes. When comparing values before and after 5 years of age, both lens power and axial length differed significantly in the moderate to high hyperopia group, whereas in the low hyperopia group, they only differed significantly after 9 years of age. Lens power was negatively associated with AL in eyes with axial lengths between 20 and 22 mm. A multiple linear regression model which included axial length (standardised β = −0.80, p < 0.001), corneal power (standardised β = −0.47, p < 0.001) and lens power (standardised β = 0.23, p < 0.001) explained 81.2% of the variance in SER. Conclusions Differences in lens power and axial length in moderate to high hyperopic eyes became significantly smaller after 5 years of age, at least 4 years earlier than for the low hyperopia. Lens power could offset the axial elongation in participants with axial lengths between 20 and 22 mm, suggesting that children with moderate to high hyperopia might have different ocular growth patterns. Axial length, corneal power and lens power were the main predictors of SER in moderate to high hyperopia.

Change in Corneal Power Distribution in Orthokeratology: A Predictor for the Change in Axial Length

Abstract:PURPOSE: To investigate the correlation between the change in spatial corneal power distribution and axial length (AL) elongation during orthokeratology (Ortho-k) treatment using mathematical methods.METHODS: Seventy-six subjects aged from eight to 13 years were fitted with Paragon CRT ortho-k lenses. Manifest refraction and corneal topography were checked at baseline and one day, one week, two weeks, one month, three months, six months, nine months, and one year after lens wear. AL was measured at baseline and the six-month and one-year follow-up visits. Relative corneal refractive power change (RCRPC) was calculated by a polynomial function and a monomial function. Factors including age, baseline spherical equivalent refractive error (SER), power exponent and RCRPC were tested against one-year AL growth in a stepwise multiple linear regression model.RESULTS: A total of 67 subjects completed the one-year study, with nine dropouts. The SER significantly reduced after the first month of lens wear (P < 0.001). AL significantly changed over time (P = 0.0003) with the annual growth being 0.32 ± 0.18 mm. Power exponent and RCRPC were stable throughout the follow-up visits (all P > 0.05). Change of AL was significantly correlated with baseline age (standardized β = -0.292, P < 0.001) and power exponent (standardized β = 0.691, P < 0.001), but not with the other factors being analyzed. The regression equation using baseline age (X1) and power exponent (X2) as functions for 1-year AL change (Y) was Y = 0.438-0.034X1 + 0.309X2, with R2 being 0.752.CONCLUSIONS: The asphericity of the treatment zone may affect axial elongation in children undergoing ortho-k therapy.TRANSLATIONAL RELEVANCE: Because the ortho-k lens design may affect myopia control effect in children undergoing ortho-k therapy, future ortho-k lenses should consider applying these designs to obtain a better myopia control effect in children.

Regional Summed Corneal Refractive Power Changes in Myopic Orthokeratology and Their Relationships With Axial Elongation

Abstract:Purpose: To determine the relationship between regional summed corneal refractive power changes (CRPCs) in myopic orthokeratology and axial elongation. Design: This retrospective study included 70 eyes of 70 patients aged 8 years to 13 years who underwent orthokeratology lens (OK lens) treatment, and all patients underwent regular follow-ups at 1 week, 1 month, 6 months, and 12 months at Zhongshan Ophthalmic Center between January 2019 and May 2021. Methods: Axial length (AL) was measured at baseline and 12 months by an IOLmaster 5.0. Refractive error power was measured using cycloplegia. Regional summed CRPCs were calculated by MATLAB software using difference tangential power maps at the sixth month acquired by corneal topography measurements (Medmont E300 Corneal Topographer; Medmont Pty, Victoria, Australia) and defined as changes in corneal refractive power at the sixth month from baseline. The regional summed CRPCs were then subdivided into 4-mm diameter circles, 4- to 5-mm diameter chords, and 5- to 6-mm diameter chords according to the distance from the central of the pupil and into negative, positive, and total according to the values. Pearson correlation, multiple linear regression analysis, and stepwise multiple linear regression analysis were performed to analyze the relationships among these parameters. Results: Axial elongation had a negative relationship with positive regional summed corneal refractive power in the central 4-mm diameter circle and age (r=-0.282, P=0.018; r=-0.473, P<0.001, respectively) and a positive relationship with negative regional summed corneal refractive power in the 5- to 6-mm diameter chord (r=0.361, P=0.002). Stepwise multiple linear regression analysis identified age (standardized β=-0.434, P<0.001) and negative regional summed corneal refractive power in the 5- to 6-mm diameter chord (standardized β=0.305, P=0.004) as factors influencing AL elongation. Conclusion: Negative regional summed corneal refractive power in a 5- to 6-mm diameter chord after OK lens treatment may be an important index for evaluating the control effects of axial elongation.
Effects of Age on Ocular Anterior Segment Dimensions Measured by Optical Coherence Tomography.

Abstract:Background Older subjects tend to have smaller ocular anterior segment. The present study aimed to measure anterior segment dimensions with optical coherence tomography (OCT) and quantitatively assess the effect of age and other factors.Methods Anterior segment OCT images were obtained in normal subjects residing in the greater Los Angeles area. Four line scans were acquired at the 90, 45, 0 and 135 meridians of each eye. Computer calipers acquired anterior segment dimensions of corneal diameter, anterior chamber width, corneal vault and anterior chamber depth on OCT images. Measurements from 4 meridians were averaged. Axial length and corneal power were measured by partial coherence interferometry. Univariate and multivariate analyses were performed to assess correlations.Results Sixty-six eyes of 33 normal subjects (aged 22-65 years, 19 Asians, 14 Caucasians) were enrolled. For every 1 year of age, corneal diameter was 0.033 mm narrower (P <0.01), anterior chamber width was 0.031 mm narrower (P <0.01), corneal vault was 0.016 mm lower (P <0.01), and anterior chamber depth was 0.025 mm lower (P <0.01). Asian eyes had smaller corneal diameter (P=0.035) and anterior chamber width (P=0.015) compared with those of Caucasian eyes. Body height showed positive correlation with corneal diameter (0.039 mm per centimeter of height, P <0.01) and corneal vault (0.024 mm per centimetre of height, P <0.01). Gender did not have an independent effect on anterior segment dimensions.Conclusions Anterior segment dimensions were smaller in older subjects. Age-related changes may affect the tolerability of long-term implants such as phakic intraocular lens. Chin Med J 2011;124(12):1829-1834
The Effect of Corneal Power Distribution on Axial Elongation in Children Using Three Different Orthokeratology Lens Designs

Abstract:SIGNIFICANCE:This study will enhance our understanding of the effects of orthokeratology lens design on corneal profile, the results of which may be useful in developing future orthokeratology lens designs. PURPOSE:This study aimed to evaluate the effect of lens design on corneal power distribution after orthokeratology using mathematical methods. METHODS:Sixty-five subjects were enrolled in this prospective study and assigned to four groups: Euclid with 6.2-mm back optic zone diameter (aged <14 years), Euclid with 6.2-mm back optic zone diameter (aged ≥14 years), double tear reservoir lens with 5.0-mm back optic zone diameter, and double tear reservoir lens with 6.0-mm back optic zone diameter. Manifest refraction and corneal topography were checked at baseline and 1 day, 1 week, 2 weeks, and 1 month after lens wear. Relative corneal refractive power change was calculated by a polynomial function and a monomial function. The maximum relative corneal refractive power change (Ymax) and the corresponding distance from the corneal center (Xmax) were analyzed. Relative corneal refractive power change over time and between groups was compared using repeated-measures analysis of variance. RESULTS:Refractive reduction and central corneal flattening were seen at all follow-up visits after orthokeratology lens wear, being fastest in the 5.0-mm back optic zone diameter group (P < .001). The cornea steepened in an aspheric way toward the midperiphery and peaked at approximately 2 to 3 mm off the apex. Overall, Ymax was not different among the four groups, but Xmax was smallest in the 5.0-mm back optic zone diameter group (P < .001). At 1/2 Xmax, relative corneal refractive power change of the 5.0-mm back optic zone diameter design was significantly higher than that of the other three groups (P < .001). The power exponent of the monomial of the 5.0-mm back optic zone diameter design was greater than that of the other three groups (P < .001). CONCLUSIONS:An orthokeratology lens design with smaller back optic zone diameter might yield a faster myopic reduction and a smaller aspheric treatment zone.
Distribution of Ocular Biometry in 7- and 14-Year-old Chinese Children.

Abstract:PURPOSE:To describe distributions of ocular biometry and their associations with refraction in 7- and 14-year-old children in urban areas of Anyang, central China.METHODS:A total of 2271 grade 1 students aged 7.1 ± 0.4 years and 1786 grade 8 students aged 13.7 ± 0.5 years were measured with ocular biometry and cycloplegic refraction. A parental myopia questionnaire was administered to parents.RESULTS:Mean axial length, anterior chamber depth, lens thickness, central corneal thickness, corneal diameter, corneal radius of curvature, axial length/corneal radius of curvature ratio, and spherical equivalent refraction were 22.72 ± 0.76 mm, 2.89 ± 0.24 mm, 3.61 ± 0.19 mm, 540.5 ± 31 μm, 12.06 ± 0.44 mm, 7.80 ± 0.25 mm, 2.91 ± 0.08, and +0.95 ± 1.05 diopters (D), respectively, in 7-year-old children. They were 24.39 ± 1.13 mm, 3.42 ± 0.41 mm, 3.18 ± 0.24 mm, 548.9 ± 33 μm, 12.03 ± 0.43 mm, 7.80 ± 0.26 mm, 3.13 ± 0.14, and -2.06 ± 2.20 D, respectively, in 14-year-old children. Compared with 7-year-old children, the older group had significantly more myopia (-3.0 D), longer axial length (1.7 mm), deeper anterior chamber depth (0.3 mm), thinner lens thickness (-0.2 mm), thicker central corneal thickness (10 μm), and greater axial length/corneal radius of curvature ratio (0.22) (all p < 0.001), as well as smaller corneal diameter (-0.03 mm, p = 0.02) and similar corneal radius of curvature. Sex differences were similar in both age groups, with boys having longer axial length (0.5 mm), deeper anterior chamber depth (0.1 mm), shorter lens thickness (0.03 mm), greater central corneal thickness (5 μm), greater corneal diameter (0.15 mm), and greater corneal radius of curvature (0.14 mm) than girls (all p < 0.01). The most important variables related to spherical equivalent refraction were vitreous length, corneal radius of curvature, and lens thickness.CONCLUSIONS:The 14-year-old group had larger parameter dimensions than the 7-year-old group except for corneal radius of curvature (unchanged) and lens thickness and corneal diameter (both smaller). Boys had large parameter dimensions than girls except for lens thickness (smaller). Axial length, corneal radius of curvature, and lens thickness were the most important determinants of refraction.

Abstract:Abstract To examine the refractive lens power (RLP) and lens thickness and their associated factors in children from North-Western China. Children from two schools (primary school and junior high school) in the North-Western Chinese province of Qinghai underwent a comprehensive ophthalmic examination including biometry and cycloplegic refractometry. The RLP was calculated using Bennett’s equation. The study included 596 (77.9%) individuals (mean age: 11.0 ± 2.8 years; range: 6–16 years) with a mean axial length of 23.65 ± 1.24 mm (range: 20.02–27.96 mm). Mean lens thickness was 3.30 ± 0.16 mm (range: 2.85–3.99 mm) and mean RLP was 24.85 ± 1.98D (range: 19.40–32.97). In univariate analysis, girls as compared to boys had a significantly thicker lens and greater RLP, shorter axial length, smaller corneal curvature radius and shorter corneal curvature radius (all P < 0.001). Both sexes did not differ significantly in refractive error ( P = 0.11) and corneal thickness ( P = 0.16). RLP was positively associated with refractive error (correlation coefficient r = 0.33; P < 0.001) and lens thickness (r = 0.62; P < 0.001) and negatively with axial length (r = − 0.70; P < 0.001). In univariate analysis, RLP decreased significantly with older age in the age group from age 6–13, while it plateaued thereafter, with no significant difference between boys and girls. In multivariate regression analysis, a higher RLP was associated with younger age ( P < 0.001; standard regression coefficient β = − 0.07), female sex ( P < 0.001; β = − 0.08), shorter axial length ( P < 0.001; β = − 0.48) and higher lens thickness ( P < 0.001; β = 0.42). In Chinese children, RLP with a mean of 24.85 ± 1.98D decreases with older age, male sex, longer axial length, and thinner lens thickness. Changes in RLP and axial length elongation are important players in the emmetropization and myopization.

Abstract:PURPOSE:To evaluate whether accommodation has effects on eye elongation.METHODS:Two hundred seventy-three eyes with a refractive error between +1.0 and -0.5 diopters were divided into three groups: pre-school children, grade-school children, and middle-school children. Ocular structures were measured using autorefraction and A-scan ultrasonography.RESULTS:There was a negative correlation between age and corneal power (r = -0.227, P = .0001), lens thickness (r = -0.263, P = .00001), and Gullstrand lens power (r = -0.452, P << .0001). There was a positive correlation between age and axial length (r = -0.432, P << .0001) and vitreous chamber depth (r = 0.505, P << .0001). Mean corneal power (P < .001) and lens power (P < .001) were significantly greater in pre-school children than in grade-school children, whereas axial length (P < .001) and vitreous chamber depth (P < .001) were greater in grade-school children. Mean lens thickness (P = .01) and lens power (P = .07) were higher in grade-school children than in middle-school children, whereas axial length (P = .024) and vitreous chamber depth (P < .001) were higher in middle-school children.CONCLUSION:Ocular structures may play different roles in maintaining emmetropia. Accommodation during near work and its potential attendant hyperopic defocus could potentially drive eye elongation.
Ocular Biometry Features and Their Relationship with Anterior and Posterior Segment Lengths among a Myopia Population in Northern China

Abstract:Objectives: The study aims to explore the ocular biometry of a myopic population in Northern China, focusing specifically on anterior and posterior segment lengths. Methods: This is a cross-sectional study. The medical records of 3458 myopic patients who underwent refractive surgery were evaluated. Axial length (AL), anterior chamber depth (ACD), lens thickness (LT) and other biometric parameters were measured using the IOL Master 700. The study determined the anterior segment length (ASL = ACD + LT), the posterior segment length (PSL = AL − ASL) and the ratio of ASL to PSL (ASL/PSL). Results: This study included 3458 eyes from 3458 myopic patients (1171 men and 2287 women). The mean age was 27.38 ± 6.88, ranging from 16 to 48 years old. The mean ASL was 7.35 ± 0.27 mm, and the mean PSL was 18.39 ± 1.18 mm. The ASL and PSL trends demonstrate an age-related increase for both genders, with notable gender-specific variations. Across most age groups, males typically exhibited higher ASLs and PSLs than females, with the exception of the 35–40 and 40–45 age groups. The ASL and PSL consistently increased with a rising AL. The AL strongly correlates with the PSL and negatively correlates with the ASL/PSL ratio. The ACD and LT moderately correlate with the ASL, but an increased LT does not imply a longer posterior segment. The CCT and SE show little correlation with axial eye parameters. Conclusions: Among Chinese myopic patients, a longer ASL and PSL were correlated with older age and the male gender. The AL strongly correlates positively with the PSL and negatively correlates with the ASL/PSL ratio. An elongation of the posterior segment may primarily account for an eyeball's lengthening.

Abstract:Objective to investigate the aspherity characteristics of corneal anterior surface in myopic and hyperopia children. Methods Using computerized videokeratography (Humphrey ALTAS Corneal Topography System), the topographic parameters in 291 eyes of 164 nomal children were analyzed, which included the shape factor (SF) , the average power of zone of apex, 3mm, 5mm, 7mm in diameter. Refraction was measured with phoroptor with cycloplegia. The relations between the shape factor (SF) and refractive error, the corneal power and the refractive error were analyzed. Results The SF of the children with myopia was lower a little,The SF of the children with hyperopia was higher a little. The average power of zone of 7mm of the children with myopia was higher than hyperopia or emmetropia. A little but statistically significant positive correlation was found between corneal shape factor (SF) and spherical equivalent refractive error (r=0.16 P=0.003). The regression coefficient between corneal power and the refractive error increased from apex to peripheral area. Conclusions A tendency for cornea to flatten less rapidly in the periphery with increasing myopia or decreasing hyperopia was shown.
