Abstract:Introduction Safe, rapid, and efficacious drugs for acute cardioversion of paroxysmal AF (PAF) are needed. In an open-label, Phase 2 study (INSTANT), orally inhaled flecainide was safe and efficacious for acute conversion of PAF to sinus rhythm (SR).[1, 2] Purpose Evaluate the efficacy and safety of flecainide acetate inhalation solution (FlecIH-103) compared to placebo in patients with short-duration, symptomatic, newly diagnosed, or recurrent PAF. Methods RESTORE-1 was a multicentre, randomized, double-blind, placebo-controlled trial in patients with symptomatic PAF (≤48 hours). Patients received FlecIH-103 at an estimated total lung dose of 120 mg or placebo (3:1) over 8 minutes using a breath-actuated nebulizer and were continuously monitored for 90 minutes (observation period), using a single-lead patch ECG electrode. Results The study was stopped prematurely after 55 of the expected 400 patients had been enrolled due to lower-than-expected flecainide peak plasma levels (197.8 ng/mL; 1.85-fold lower than those observed with the same target dose in INSTANT) and modest efficacy. The Data Monitoring Committee confirmed that there were no safety risks. Fifty-four of the enrolled patients (42 active; 12 placebo) were in AF at the start of inhalation. Baseline characteristics were well-balanced between treatment groups; overall mean age was 59.6 years (SD: 13.5) and 31.5% of patients were female. Cardioversion rates during the observation period in the efficacy population (n=51; 39 active, 12 placebo) were 30.8% (95% CI: 18.5-46.5%) in the active group and 0% (95% CI: 0.0-28.2%) in the placebo group (p=0.04; figure 1). In the active group, the median time to conversion from start of inhalation was 12.8 minutes (IQR: 7.8-25.0 minutes), which includes an 8-minute inhalation period. Among the 12 patients whose AF converted to SR, only 1 (8.3%) still had AF-related symptoms compared to 71.8% of patients whose AF did not convert to SR (p<0.001). Time to discharge-eligible status was 1.6h for patients whose AF cardioverted and 3.3h for those whose AF did not (p=0.002; figure 2). The most common adverse events (AEs; ≥8%) occurring more frequently in the active than the placebo group were the following: dyspnoea (14.3%), dysgeusia (14.3%) and cough (11.9%). No serious AEs or cardiovascular events of special interest (hypotension, ventricular tachycardia, bradycardia, sinus pause, atrial flutter 1:1, other arrhythmias) were reported. Conclusions Orally inhaled flecainide, at approximately half of the targeted dose, was modestly effective and safe. The 1.5-fold lower conversion rate observed in this study, compared to the same dose in the INSTANT trial, is consistent with the lower plasma levels of flecainide achieved. Future studies will be designed to improve drug delivery to yield peak plasma levels of flecainide associated with higher conversion rates.

Intravenous Flecainide for Emergency Department Management of Acute Atrial Fibrillation

Abstract:Background: Atrial fibrillation (AF) is the most commonly encountered dysrhythmia in the emergency department, and its prevalence is increasing. A substantial proportion of these patients have recent-onset AF (<48 h). The poor prognosis associated with AF is being increasingly recognized, and there is some evidence for better outcomes in younger patients with recent-onset AF when sinus rhythm is restored. Flecainide is recommended in the latest international guidelines for cardioversion of recent-onset AF, but its safety and efficacy relative to other recommended agents are unclear. Objective: Our aim was to clarify the Level 1 evidence for the use of i.v. flecainide in acute AF. Methods: We performed a systematic review and meta-analysis of the literature. Medline, Ovid, Embase, and Cochrane Central databases were searched for relevant studies. Only randomized controlled trials (RCTs) of i.v. flecainide for acute conversion of recent-onset AF were selected for meta-analysis. Results: Four hundred and three studies were screened, of which 11 RCTs were eligible for meta-analysis. Flecainide had high efficacy for cardioversion within 2 h (number needed to treat [NNT] = 1.8). Efficacy was superior to propafenone, amiodarone, procainamide, ibutilide, and sotalol (NNT = 4.3). There was no statistically significant difference in pro-dysrhythmia compared to these anti-dysrhythmics or placebo. Conclusions: Intravenous flecainide cardioversion could be a safe and effective option for emergency physicians to restore sinus rhythm in selected patients with acute AF.

Abstract:OBJECTIVE To investigate the efficacy and safety of intravenous ibutilide for conversion of atrial fibrillation (AF) and flutter (AFL) to sinus rhythm. METHODS Ninety-nine consecutive patients aged 18-75 y with AF/AFL were included. The duration of arrhythmia was <90 d (1 h-90 d) and ventricular rate was >60 beats/min. Patients were assigned randomly into two groups: 49 patients in ibutilide group received ibutilide 1 mg, then repeated if AF/AFL was not converted after 10 min; 50 patients in propafenone group received propafenone 70 mg, then repeated if AF/AFL persisted after 10 min. Two drugs were diluted by 50 ml of 5% glucose and injected intravenously within 10 min. RESULTS Ventricular rates were decreased in both groups. AF/AFL were converted in 34 of 49 patients (69.4 % ) in ibutilide group and in 22 of 50 patients (44.0 %) in propafenone group (P <0.05). The converting time of ibutilide was significantly shorter than that of propafenone [(16.79 ± 12.31) min compared with (36.92 ± 11.38)min, P <0.01]. The most serious adverse effect of ibutilide was non-sustained monomorphic ventricular tachycardia (3/49,6.12 %). Transient hypotension and heart pause were the main adverse events in patients who received propafenone, acute left heart failure occurred in one patient of propafenone group. CONCLUSION Intravenous ibutilide is a safe and effective agent for cardioversion of recent-onset AF/AFL. Furthermore,strict processing under electrocardio-monitoring is important.
Effect of Oral Propafenone on Reversing Paroxysmal Atrial Fibrillation with No Rheumatic Disease

Abstract:Objective To evaluate the effects of orally administered propafenone on reversing paroxysmal atrial fibrillation and its safety in non valvular heart disease. Methods 78 patients with paroxysmal atrial fibrillation were divided into propafenone group and lanatoside C group. Subjects with congestive heart failure, serious myocardial ischemia, valvular heart disease, electrolyte disturbance, impairment of hepatic function and renal function were excluded. 39 patients were performed oral propafenone 300 -600 mg(6-8 mg/kg) in a single dose. 39 patients were given intravenous lanatoside C 0.4 mg as control. Close observations of cardiac rhythm, heart rate,electrocardiogram, blood pressure and symptoms under heart monitor were performed and the times from taking the drug to reversing the atrial fibrillation were recorded. Result 39 patients in oral propafenone group were reversed within 4 hours (53. 85% ) . In the group of intravenous lanatoside C, 10 patients were reversed within 4 hours(25. 64% ). In the first 4 hours, the success rate of reversing atrial fibrillation and average reversal time were significantly higher in the propafenone group than those in lanatoside C group (P<0.05). Conclusion Oral propafenone is a convenient safe and efficiency method for reversing paroxysmal atrial fibrillation with no congestive heart failure, serious myocardial ischemic and valvular heart disease.

Abstract:Objective: To compare converting efficacies of intravenous ibutilide and propafenone on atrial fibrillation(AF) and artial flutter(AFL) in Chinese patients in a prospective study.Methods: We investigated 200 AF and AFL patients treated with ibutilide(1 mg,or 0.01 mg·kg-1 if body weight 60 kg,iv) or propafenone(70 mg,iv,repeated if necessary) in the randomized and controlled study.A second infusion was performed 10 min later if first infusion failed.Results: Overall,ibutilide had a higher cardioversion rate(55%,55/100) than propafenone(21%,21/100).Ibutilide converted more patients to sinus rhythm in patients with either AF [50%(40/79) vs 24%(21/90)] or AFL [71%(15/21) vs 0%].The RR intervals were not statistically different between AF and AFL during conversion.One torsade de pointes(TdP,1%) developed in ibutilide group and was converted by direct current electrical conversion.Conclusion: Ibutilide is more effective than propafenone in converting AF or AFL in Chinese patients.Adverse events are not significantly different between ibutilide and propafenone.
Cardioversion of recent-onset atrial fibrillation using intravenous antiarrhythmics: A European perspective

Abstract:Pharmacological cardioversion using intravenous antiarrhythmic agents is commonly indicated in symptomatic patients with recent-onset atrial fibrillation (AF). Except in hemodynamically unstable patients who require emergency direct current electrical cardioversion, for the majority of hemodynamically stable patients, pharmacological cardioversion represents a valid option and requires the clinician to be familiar with the properties and use of antiarrhythmic agents. The main characteristics of selected intravenous antiarrhythmic agents for conversion of recent-onset AF, the reported success rates, and possible adverse events are discussed. Among intravenous antiarrhythmics, flecainide, propafenone, amiodarone, sotalol, dofetilide, ibutilide, and vernakalant are commonly used. Antazoline, an old antihistaminic agent with antiarrhythmic properties was also reported to give encouraging results in Poland. Intravenous flecainide and propafenone are the only Class I agents still recommended by recent guidelines. Intravenous new Class III agents as dofetilide and ibutilide have high and rapid efficacy in converting AF to sinus rhythm but require strict surveillance with electrocardiogram (ECG) monitoring during and after intravenous administration because of the potential risk of QT prolongation and Torsades de Pointes, which can be prevented and properly managed. Vernakalant, a partial atrial selective was shown to have a high success rate and to be safe in real-life use.
Efficacy and speed of conversion of recent onset atrial fibrillation using oral propafenone versus parenteral amiodarone: A randomized controlled comparative study

Abstract:Background: Atrial fibrillation is the most commonly encountered arrhythmia. Several antiarrhythmic agents are effective in restoring and maintaining sinus rhythm. Aim of the work: To compare the efficacy and rapidity of conversion of recent onset atrial fibrillation using oral propafenone versus intravenous infusion of amiodarone. Methods: The study included 200 patients with recent onset atrial fibrillation. Patients were equally divided into 2 groups; group A where intravenous infusion amiodarone was given and group B where oral propafenone was administrated. The effectiveness and the time needed for conversion of atrial fibrillation to sinus rhythm were compared in both groups. Results: The success of conversion of atrial fibrillation to sinus rhythm was 83% in group A and 85% in group B, p-value = 0.699. The time elapsed from drug administration till conversion of atrial fibrillation was 9.07 ± 5.04 hours in group A versus 3.9 ± 1.54 hours in group B, p-value = 0.001. In both groups, patients who showed failed conversion had a significantly larger left atrial diameter and a significantly higher high sensitivity C-reactive protein (hsCRP) level. Conclusion: Oral propafenone was faster than parenteral amiodarone in the conversion of recent onset atrial fibrillation to sinus rhythm. Patients with failed conversion had a bigger left atrial diameter and a higher hsCRP when compared to patients with successful conversion.
Cardioversion of atrial fibrillation: the use of antiarrhythmic drugs

Abstract:Atrial fibrillation (AF) is the commonest atrial arrhythmia and represents a large burden on modern health services. Large multicentre randomised trials have demonstrated that a rhythm control strategy (using antiarrhythmic drugs and direct current (DC) cardioversion) has no morbidity or mortality advantage over rate control. Therefore, for most patients, attempts to cardiovert AF to sinus rhythm (SR) should be reserved for those patients who are symptomatic despite adequate rate control. For recent-onset AF (<24 h) the use of agents like flecainide can be highly successful to pharmacologically cardiovert AF, although caution should be exercised in patients who have the potential for structural or coronary artery disease because of the risk of proarrhythmia. If there any is doubt as to the suitability of a patient for pharmacological cardioversion then DC cardioversion is the safer option. Owing to the high recurrence rate of AF after cardioversion (71-84% at 1 year), the use of antiarrhythmic drugs to maintain SR is recommended. The irreversible side effects of amiodarone mean that it should be avoided whenever possible for long-term maintenance treatment, although it is useful in short courses (8 weeks-6 months), particularly for patients who had a successfully treated secondary cause for AF. Other agents like flecainide and sotalol are also useful but should not be used for patients with structural heart disease. Data supporting the use of newer agents like dronedarone are at present limited.

Abstract:Background: The real incidence of atrial arrhythmia (AA) after patent foramen ovale (PFO) closure and whether this complication can be prevented remain unknown. This study assessed if flecainide is effective to prevent AA during the first 3 months after PFO closure, and if 6 months treatment by flecainide is more effective than 3 months to prevent AA after PFO closure. Methods: AFLOAT is a prospective, multicentre, randomized, open-label, superiority trial with a blind evaluation of all the endpoints (PROBE design). Patients were randomized in a 1:1:1 ratio after PFO closure to receive flecainide (150 mg once a day in a sustained-release (SR) dose) for 3 months, flecainide (150 mg od SR dose) for 6 months, or no additional treatment (standard-of-care) for 6 months. The primary endpoint was the percentage of patients with at least one episode AA (≥30s) recorded within 3 months after PFO closure on long-term monitoring with an insertable cardiac monitor (ICM). The secondary endpoint was the percentage of patients with at least one episode of AA (≥30s) recorded with ICM during the 3-6 months period after PFO closure. Results: 186 patients were included (mean age 54 years, male 68.8%) and AA (≥30s) occurred in 53 (28.5%) patients during the 6-month follow-up; 86.8% of these AA events occurred in the first month after PFO closure. The primary outcome occurred in 33/123 (26.8%) and 16/63 (25.4%) patients receiving flecainide for at least 3 months or standard of care, respectively [Risk Difference (RD) 1.4%; 95% confidence interval (CI) -12.9% to 13.8%, NS]. The secondary endpoint occurred in 3/60 (5.0%), 4/63 (6.3%), and 5/63 (7.9%) patients receiving flecainide 6 months, 3 months or standard of care, respectively [RD -2.9%; 95% CI -12.7% to 6.9%, and RD -1.6%; 95% CI -11.8% to 8.6%, respectively]. Conclusions: In the first 6 months following successful PFO closure, AA (≥30s) occurred in 28.5% of cases, mostly in the first month after the procedure. Flecainide did not prevent AA after PFO closure.

Abstract:Flecainide is a class 1c antiarrhythmic used in the treatment of tachyrhythmias such as paroxysmal supraventricular tachycardia, Wolf-Parkinson-White syndrome, atrioventricular nodal reentry tachycardia, atrial fibrillation, and atrial flutter. Flecainide toxicity can present as milder symptoms such as nausea or vomiting to more severe symptoms such as syncope, coma, dysrhythmias, or even cardiac arrest. Mortality can be as high as 22.5% in severe cases and the half-life of flecainide is 20.5 hours. A 68 year old female with history of longstanding persistent atrial fibrillation on apixaban, hypertension, hyperlipidemia, NASH, and depression, with prior ejection fraction (EF) of 55% who presented to the emergency department after becoming unresponsive when with family. She went into cardiac arrest twice en route with successful resuscitation. In the emergency room, found to be alert but fatigued and normotensive with heart rate in the 60s. First EKG showed QRS 116ms and QTc 509ms. Notably, she had an outpatient cardioversion 10 days prior for her atrial fibrillation for which she was she was started on flecainide the night before the procedure. She was admitted to the cardiac intensive care unit with a lidocaine drip, bicarbonate drip, and intralipid therapy. Due to continued ventricular tachyarrhythmias and continued cardiac arrests despite overdrive pacing leading to refractory cardiogenic shock requiring sedation and vasopressors, she was started on extracorporeal membrane oxygenation (ECMO) support with an impella device for left ventricle unloading. Her ejection fraction was 5-9% on admission with improvement to 40-44% after 5 days of medical management and mechanical support. She had a coronary CT angiogram a month before her cardioversion with a calcium score of 0. Flecainide level came back at 1.59 (0.2-1.0 ug/mL). ICU course was complicated by acute limb ischemia of the impella cannulation limb requiring a bedside SFA perfusion catheter placement as well as a ventilator-associated pneumonia. She had an implantable cardioverter-defibrillator placed for secondary prevention given her probable history of long QT. Flecainide toxicity, although a known phenomenon, is rare. This is a severe case of toxicity in someone who has a likely existing long QT that is either congenital or secondary to her sertraline. Her QTc before and after outpatient cardioversion were 526ms and 530ms respectively. It was thought that flecainide being proarrhythmogenic exacerbated an already prolonged QT. This case highlights the importance of knowing when to get consultation for ECMO support. Veno-arterial ECMO was used here to help cardiopulmonary support in the setting of a likely drug induced and reversible ventricular electrical storm causing severe cardiomyopathy. ECMO in this case gave support through an acute period allowing elimination of flecainide to occur. In addition, this case highlights possible complications of cannulated devices as well as a reminder that underlying conditions can exacerbate the already known adverse effects of a medication.

Electrophysiological and therapeutic effects of amiodarone in patients with preexcited atrial fibrillation

Abstract:Background: Atrial fibrillation (AF) with fast ventricular response over an overt accessory pathway (AP) (preexcited AF) with a short anterograde refractory period is a potentially malignant arrhythmia. This study aimed to evaluate the safety and efficacy of amiodarone for preexcited AF management. Materials and methods: This study enrolled 103 patients with evidence of AP in electrocardiography. Patients with preexcited AF were included in the study. Intravenous amiodarone (300 mg) was infused for 30 min for all patients in the AF rhythm. Electrophysiological parameters were evaluated before amiodarone injection and 2 h after pharmacological or electrical cardioversion. Results: Antegrade and retrograde refractory periods of the atrioventricular node (AVN) and AP, as well as antegrade and retrograde Wenckebach points of AVN, were increased significantly after amiodarone infusion. Furthermore, the mean of the shortest preexcited RR interval was increased during the monitoring period. Comparing the preexcited index at the beginning of the study and before cardioversion (2 h later) revealed that the QRS complexes changed to a wider pattern as the preexcitation index changed from 80.61 to 92.26 (P < 0.001). Nineteen (18.4%) patients converted to the sinus rhythm with amiodarone infusion. No ventricular arrhythmia was detected during monitoring. Conclusion: Amiodarone could be considered a safe drug in patients with preexcited AF for rate control despite its relatively low efficacy in conversion to the sinus rhythm.

Abstract:Flecainide is a class Ic antiarrhythmic agent that has an important role as part of rhythm control strategies in patients with atrial fibrillation (AF). Early clinical data on the use of flecainide showed an increase in arrhythmias and mortality compared with placebo in patients with a previous myocardial infarction and asymptomatic or mildly symptomatic ventricular arrhythmias. These findings only apply to a specific group of patients with left ventricular dysfunction and ischaemic heart disease, but had a negative impact on the use of class Ic antiarrhythmics across all indications and patient groups. The aim of this review was to evaluate the available safety data for flecainide in the literature and to assess its current use in patients with AF. Current European guidelines now recommend the use of flecainide in carefully selected groups of patients with AF who do not have structural heart disease. This includes for the cardioversion of recent-onset AF, pretreatment prior to direct current cardioversion, out-of-hospital acute oral therapy ('pill-in-the-pocket' approach) and for the ongoing maintenance of sinus rhythm. Potential cardiac adverse effects of flecainide include proarrhythmia, conduction abnormalities and negative inotropic effects. Dizziness is the most frequent non-cardiac side effect, followed by blurred vision and difficulty focusing; these are almost all mild, transient and tolerable. Data from recent clinical trials in patients with supraventricular arrhythmias suggest that flecainide has a good tolerability profile in groups of appropriately selected patients. Caution is required when using flecainide in patients with renal dysfunction, and there are a number of drug interactions, but these are well documented and manageable. Overall, flecainide is a good choice for the pharmacological management of AF. It has a good safety record and low incidence of adverse effects, rare end-organ toxicity and a low risk of ventricular proarrhythmia. To ensure that the benefits of treatment outweigh any potential risks, careful patient selection and monitoring is required.

Abstract:Flecainide, similar to encainide and propafenone, is IC class antiarrhythmic, inhibiting Nav1.5 sodium channels in heart muscle cells and modulates cardiac conduction. Despite its over 40-year presence in clinical practice, strong evidence and well-known safety profile, flecainide distribution in Europe has not always been equal. In Poland, the drug has been available in pharmacies only since October this year, and previously it had to be imported on request. Flecainide can be used successfully in both the acute and chronic treatment of cardiac arrhythmias. The main indication for flecainide is the treatment of paroxysmal supraventricular tachycardias, including atrial fibrillation, atrioventricular nodal re-entrant tachycardia, atrioventricular re-entrant tachycardia and ventricular arrhythmias in patients without structural heart disease. Beyond that, it may be used in some supraventricular tachycardia in children and for sustained fetal tachycardia. Many studies indicate its efficacy comparable to or better than previously used drugs such as propafenone and amiodarone, depending on the indication. This review aims to highlight the most important clinical uses of flecainide in the light of the latest scientific evidence and to provide an overview of the practical aspects of treatment, including indications, off-label use, contraindications, areas of use, monitoring of treatment and most common complications, taking into account special populations: children and pregnant women.

Abstract:Flecainide is a medication used to treat supraventricular and ventricular tachyarrhythmias. Cases of overdoses are rare, however, can lead to significant cardiac effects. In previous cases of flecainide toxicity, treatment with sodium bicarbonate, intravenous lipid emulsion and amiodarone have been reported to be effective in preventing cardiovascular collapse and reestablishing baseline rhythm. Here, we present a case of a man in his 40s presented with flecainide overdose with wide-complex tachycardia that was treated with intravenous sodium bicarbonate following failure of amiodarone to normalise QRS interval.

Abstract:Abstract Introduction Atrial arrhythmia is the most common complication of patent foramen ovale (PFO) closure. The real incidence of post-PFO closure atrial arrhytmia and whether this complication can be prevented is unknown. Methods/Design The Assessment of Flecainide to Lower the PFO closure risk of Atrial fibrillation or Tachycardia (AFLOAT) trial is a prospective, national, multicentre, randomized, open-label, superiority trial with a blind evaluation of all the endpoints (PROBE design). A total of 186 patients are randomized in a 1:1:1 ratio immediately after PFO closure to receive Flecainide (150 mg per day in a single sustained-release (SR) dose) for 6 months (Group 1), Flecainide (150 mg per day in a single SR dose) for 3 months (Group 2), or no additional treatment (standard of care) for 6 months (Group 3). The primary endpoint is the percentage of patients with at least one episode of symptomatic or asymptomatic atrial arrhythmia episode (≥30 s) recorded within 3 months after PFO closure on long-term monitoring with an insertable cardiac monitor. Whether 3 months of treatment is sufficient compared to 6 months will be analysed as a secondary objective of the study. Conclusion AFLOAT is the first trial to test the hypothesis that a short treatment with oral Flecainide can prevent the new-onset of atrial arrhythmia after PFO closure. Abstract:In recent years, the profile of patients who can undergo ambulatory surgery procedures in independent ambulatory surgical centers has changed. It is very important to consider the availability, skills and experience of the care staff, as well as the integral study of the patient, the resolution capacity of these centers and evaluations with quality care indicators to obtain optimal results. The present case was of a 34-year-old male, first presentation of rapid and symptomatic atrial fibrillation documented in the immediate postoperative period. Patient care, diagnostic strategy and treatment were described, since the patient required electrical cardioversion.

Abstract:Background: The Pill-in-the-Pocket (PiP) approach may be used in highly selected patients to achieve acute pharmacological cardioversion into sinus rhythm. Flecainide toxicity is rarely reported, especially with patients who take flecainide as PiP, and only limited evidence exists in its management. We present a case of accidental flecainide overdose for a patient who is on PiP and the acute management strategy. Case summary: A 78-year-old female with persistent atrial fibrillation (AF), previously underwent pulmonary vein isolation and maintained on verapamil 240 mg twice daily, presented to the electrophysiology clinic following a recent hospital admission for recurrent AF. Due to infrequent recurrent episodes of symptomatic AF, the patient preferred to avoid both repeat ablation and additional daily medications. After an initial trial on telemetric monitoring, a PiP approach with flecainide 300 mg was adopted. Unfortunately, palpitations and dyspnoea in the context of chronic obstructive pulmonary disease exacerbation led the patient to self-medicate with multiple doses of albuterol and flecainide. Twelve-lead electrocardiogram showed slow AF with a wide QRS complex. The patient received 1 g of calcium gluconate with a 180 mEq bolus of sodium bicarbonate 8.4% and was started on continuous sodium bicarbonate infusion at 150 mL/h. Over a 12 h period, the QRS complex narrowed down, and the rhythm returned to normal sinus rhythm with a QRS interval of 136 ms. Discussion: The PiP strategy with flecainide is safe and effective. Reinforcement of medication dosing and frequency with patient read back is key to avoid accidental toxicity, which could be life-threatening. Treatment with sodium bicarbonate is quick and highly effective.
Pharmacologic cardioversion of recent-onset atrial fibrillation: a systematic review and network meta-analysis

Abstract:Aims: We sought to identify the most effective antidysrhythmic drug for pharmacologic cardioversion of recent-onset atrial fibrillation (AF). Methods and results: We searched MEDLINE, Embase, and Web of Science from inception to March 2019, limited to human subjects and English language. We also searched for unpublished data. We limited studies to randomized controlled trials that enrolled adult patients with AF ≤ 48 h and compared antidysrhythmic agents, placebo, or control. We determined these outcomes prior to data extraction: (i) rate of conversion to sinus rhythm within 24 h, (ii) time to cardioversion to sinus rhythm, (iii) rate of significant adverse events, and (iv) rate of thromboembolism within 30 days. We extracted data according to PRISMA-NMA and appraised selected trials using the Cochrane review handbook. The systematic review initially identified 640 studies; 30 met inclusion criteria. Twenty-one trials that randomized 2785 patients provided efficacy data for the conversion rate outcome. Bayesian network meta-analysis using a random-effects model demonstrated that ranolazine + amiodarone intravenous (IV) [odds ratio (OR) 39.8, 95% credible interval (CrI) 8.3-203.1], vernakalant (OR 22.9, 95% CrI 3.7-146.3), flecainide (OR 16.9, 95% CrI 4.1-73.3), amiodarone oral (OR 10.2, 95% CrI 3.1-36.0), ibutilide (OR 7.9, 95% CrI 1.2-52.5), amiodarone IV (OR 5.4, 95% CrI 2.1-14.6), and propafenone (OR 4.1, 95% CrI 1.7-10.5) were associated with significantly increased likelihood of conversion within 24 h when compared to placebo/control. Overall quality was low, and the network exhibited inconsistency. Probabilistic analysis ranked vernakalant and flecainide high and propafenone and amiodarone IV low. Conclusion: For pharmacologic cardioversion of recent-onset AF within 24 h, there is insufficient evidence to determine which treatment is superior. Vernakalant and flecainide may be relatively more efficacious agents. Propafenone and IV amiodarone may be relatively less efficacious. Further high-quality study is necessary.
Efficacy and safety in pharmacological cardioversion of recent-onset atrial fibrillation: a propensity score matching to compare amiodarone vs class IC antiarrhythmic drugs

Abstract:The acute management of recent-onset (<48 h) atrial fibrillation (AF) is still debated. Aim of our study was to compare efficacy and safety of intravenously administered class IC antidysrhythmic agents vs amiodarone in a propensity score matched series of patients acutely treated for AF in the emergency department. During a 3-year period, we retrospectively evaluated all episodes of recent-onset (<48 h) AF pharmacologically treated for sinus rhythm restoration in the emergency department. By means of a propensity score matching considering the main statistically different covariates, we selected two accurately matched treatment groups. We analysed the differences between amiodarone and class IC group in terms of efficacy and safety that is conversion to sinus rhythm rates within 12 and 48 h after starting treatment, time to conversion, and adverse drug effects. An overall number of 817 episodes of recent-onset AF were collected (amiodarone group = 406, class IC group = 411). After matching, we obtained 358 episodes equally divided (amiodarone group = 179 and class IC group = 179). Conversion rates within 12 h were 139 (53.1 %) in amiodarone group and 95 (72.6 %) in class IC group (p < 0.05). Median time for cardioversion was 420 min (331.6-508.3 CI 95 %) in amiodarone and 55 min (44.9-65.1 CI 95 %) in class IC group (p < 0.05). The incidence of adverse events in both groups was very low and equally distributed (p = ns). Intravenously administration of class IC agents, when compared with amiodarone, proved to be more rapid and effective, and equally safe in the acute management of recent-onset AF.

Abstract:The relative efficacy of antiarrhythmic drugs (AADs) after electrical cardioversion are not well established. This study aimed to investigate the efficacies of different AADs for maintaining sinus rhythm (SR) after electrical cardioversion for atrial fibrillation (AF). We selected patients from a retrospective registry including patients admitted for cardioversion between January 2012 and June 2016. The primary outcome was time to AF recurrence during the first year after cardioversion. The secondary outcomes included AF recurrence within 1 month, and first readmission due to heart failure, stroke, or additional non-pharmacological rhythm control. A total of 265 patients were divided into the 4 groups according to AAD type: flecainide (n = 33), propafenone (n = 64), amiodarone (n = 128), and dronedarone (n = 40). During the first year after cardioversion, the AF recurrence-free survival was similar between all AAD groups (69.7% vs. 67.2% vs. 71.9% vs. 80.0%, p = 0.439). About half of all recurrences occurred during the first month. There was no difference in any of the secondary outcomes, although the amiodarone group showed a trend toward more non-pharmacological rhythm control. AAD type was not associated with recurrence in multivariate analysis. In this study, half of all patients received amiodarone after electrical cardioversion. Flecainide, propafenone, amiodarone, and dronedarone showed similar efficacies for maintaining SR after electrical cardioversion. Thus, it might be reasonable to reconsider amiodarone use after cardioversion, since it did not show superior efficacy to the other drugs considered and is associated with potential side effects.
