Abstract:Purpose: To evaluate the long-term safety, efficacy, predictability, and stability of small incision lenticule extraction (SMILE) for the treatment of high myopia and myopic astigmatism >-10.0 D. Methods: This was a prospective study that incorporated 35 consecutive patients (35 eyes) undergoing SMILE from September 2015 to March 2016. These patients had a mean preoperative spherical equivalent refraction of -10.06 +/- 0.64 D. Patients were followed over a 4-year period and assessed for outcomes including uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refraction, and corneal topography. Results: At 4 years post-SMILE, respective efficacy and safety indices were 1.01 +/- 0.19 and 1.07 +/- 0.15. In total, 97% of operated eyes achieved an UDVA of 20/25 or better. >= 1 line was gained for 9 eyes (26%), with 25 eyes (71%) remaining stable. Twenty-four (69%) and 33 (94%) eyes, respectively, were within +/- 0.50 D and +/- 1.0 D of target refraction. From 3 months to 4 years postoperatively, a mean refractive regression of -0.22 D (-0.06 D per year) was detected, whereas no significant changes in mean corneal back curvature or posterior central elevation were detected (P = 0.617 and 0.754, respectively). We detected significant increases in higher-order aberrations (HOAs) of the anterior and total cornea (all P < 0.001), with spherical aberrations and vertical coma being particularly common, whereas posterior corneal HOA remained fairly stable (all P < 0.05). Conclusion: SMILE is a safe, effective, predictable, and stable means of correcting high myopia and myopic astigmatism over a 4-year postoperative period.
Five-Year Follow-Up of Visual Outcomes and Optical Quality after Small Incision Lenticule Extraction for Moderate and High Myopia

Abstract:This study investigated the long-term visual and optical quality changes after small incision lenticule extraction (SMILE) for the correction of moderate to high myopia. Forty-one eyes of 41 patients with a mean spherical equivalent (SE) of − 6.74 ± 1.45 D undergoing SMILE were enrolled. Preoperative and 1-month, 3-month, 1-year and 5-year postoperative follow-up examinations were performed including visual acuity, manifest refraction, corneal aberrations and contrast sensitivity. An optical quality analysis system was used to measure objective scatter index (OSI), Strehl ratio (SR) and modulation transfer function cutoff frequency (MTFcutoff). The safety and efficacy indices were 1.09 ± 0.12 and 1.03 ± 0.17, respectively, at the 5-year follow-up. No significant changes in SE from 1 month to 5 years postoperatively were found. The OSI significantly increased at 1 and 3 months postoperatively (p < 0.01 and p < 0.01) and declined at 1 and 5 years (p = 0.81 and p = 0.87). MTFcutoff and SR showed similar trends. Coma, spherical aberration and total higher-order aberration significantly increased after SMILE (p < 0.01) and remained stable from 1 month to 5 years after surgery (p > 0.05). Contrast sensitivity in the photopic and mesopic condition showed no significant changes during 5-year follow-up (p > 0.05). The SMILE procedure is satisfactory in terms of optical quality for correcting moderate and high myopia in the long-term period. ChiCTR-ONRC-13003114.
Visual Outcomes and Optical Quality After Femtosecond Laser Small Incision Lenticule Extraction: An 18-Month Prospective Study.

Abstract:PURPOSE: To investigate the long-term visual outcomes and optical quality after femtosecond laser small incision lenticule extraction (SMILE) for moderate to high myopia correction. METHODS: Fifty-four patients (37 women, 17 men) who underwent SMILE were enrolled in this prospective clinical study. Patient ages ranged from 18 to 40 years, with a mean spherical equivalent of -6.50 +/- 1.64 diopters. Data including refractive parameters, retinal image quality, intraocular scattering, and aberrations were collected at 6 and 18 months after surgery. Patient satisfaction was also evaluated. RESULTS: At 18 months after surgery, both the mean safety index and the efficacy index were 1.23 +/- 0.23 (range: 0.80 to 1.50); there was no significant difference with indices measured at 6 months (P > .05). No patient lost two or more lines of corrected distance visual acuity. Total higher-order aberrations (HOAs) and coma significantly increased after SMILE (P < .05), whereas total spherical aberrations and trefoil changed only slightly. The mean modulation transfer function cutoff frequency was 36.66 +/- 8.54 cycles per degree (cpd) before surgery and 37.81 +/- 6.89 cpd at 18 months postoperatively; the mean objective scatter index was 0.62 +/- 0.33 before surgery and 0.71 +/- 0.38 at 18 months postoperatively. No significant difference was found between the three time points (P > .05). Mean patient satisfaction was 9.31 +/- 0.64 (with a maximum score of 10). CONCLUSIONS: SMILE showed good safety, efficacy, and stability in correcting moderate to high myopia, and patients were highly satisfied. HOAs increased after SMILE, mainly due to the increase of coma, whereas retinal image quality and intraocular scattering barely changed.
Small Incision Lenticule Extraction (SMILE) for Moderate and High Myopia: Seven-Year Outcomes of Refraction, Corneal Tomography, and Wavefront Aberrations

Abstract:Purpose . To investigate the long-term outcomes of refraction, corneal tomography, and wavefront aberrations after small incision lenticule extraction (SMILE) for moderate and high myopia. Methods . Prospective, nonconsecutive case series. A total of 26 patients (26 eyes) who underwent SMILE from May 2010 to March 2013 at the Fudan University Eye and ENT Hospital (Shanghai, China) were enrolled. The periods of follow-up were 1 month, 1 year, 5 years, and 7 years after surgery. The routine eye examinations included uncorrected distance visual acuity (UDVA) and corrected distance visual acuity (CDVA), manifest refraction, and corneal tomography. Results . All surgeries were executed without any complications. At the final visit, an UDVA of 20/20 or better was achieved in 26 eyes (100%) and 11 eyes (42%) exhibited no change in CDVA. 9 eyes (35%) gained one line, 6 eyes (23%) gained two lines, and no eyes lost CDVA. 24 eyes (92%) and 26 eyes (100%) were within ±0.5 D and ±1.00 D of the target refraction, respectively. A mean refractive regression of −0.17 D was observed between 1 month and 7 years postoperatively. Mean corneal front curvature (MCFC) was significantly decreased between pre- and post-SMILE surgery ( P < 0.0001 ). Higher-order aberrations (HOAs) and vertical coma were significantly increased after SMILE compared to those measured before surgery (all P < 0.001 ). There were no significant differences in trefoil and spherical aberration between pre- and post-SMILE surgery (all P > 0.05 ). Conclusion . SMILE is an effective, safe, and stable procedure for moderate and high myopia, with relatively constant corneal stability and wavefront aberrations. Abstract:PURPOSE: To compare the visual quality, accommodation, and stereoacuity between patients with a myopia magnitude of-3.00 to-8.50 diopters (D) who underwent ICLV4c (STAAR Surgical) implantation and small incision lenticule extraction (SMILE). METHODS: The visual quality parameters, amplitude of accommodation (AMP), and stereoacuity were compared between and within the ICLV4c and SMILE groups (40 eyes/ group) with a mean spherical equivalent (SE) of-6.14 D. Differences in these parameters before and after surgery were analyzed using SPSS 22.0 software (SPSS, Inc).RESULTS: Modulation transfer function (MTF) cut-off increased significantly by 3.24 cycles/degree (t =-2.111, P =.041) and AMP decreased significantly by 1.66 D (t = 7.519, P < .001) from baseline to 3 months after surgery in the ICLV4c group. However, the MTF cut-off was significantly lower (t = 2.123, P = .037) and the AMP was significantly better (t =-3.605, P = .001) in the SMILE group than in the ICLV4c group at 3 months postoperatively. Other parameters such as Objective Scatter Index, Strehl ratio, and Optical Quality Analysis System (Visiometrics) under different contrast values did not show significant changes (P > .05). Stereoacuity did not improve significantly, except in a patient with anisometropia (2.50 D) in the ICLV4c group.CONCLUSIONS: The ICLV4c implantation yielded a better visual quality but worse AMP than did SMILE in patients with a myopia magnitude of-3.00 to-8.50 D in the short term.

Abstract:Aims To evaluate the long-term outcomes of small incision lenticule extraction (SMILE) in subjects with myopic astigmatism of ≥2.00 dioptres (D). Methods Patients who underwent SMILE 4 years prior with astigmatism ≥2.00 D and ≤1.00 D were assigned to the high astigmatic group (HA group) or the low astigmatic group (LA group), respectively. The visual and refractive results as well as corneal wavefront aberrations were measured. Results The preoperative cylinder was −2.47±0.54 D in 43 eyes in the HA group and −0.55±0.28 D in 31 eyes in the LA group. At 4 years, the residual cylinder was −0.31±0.29 D in the HA group and −0.20±0.28 D in the LA group (p=0.088). An uncorrected distance visual acuity of 20/20 was achieved in 88.4% of eyes in the HA group and 93.5% of eyes in the LA group. The efficacy index was 0.99±0.14 and 1.10±0.21 (p=0.025), and the safety index was 1.11±0.20 and 1.22±0.21 in the HA and LA groups, respectively (p=0.012). Eighty-six per cent and 90.3% of eyes were within ±0.50 D of the attempted cylindrical correction in the HA and LA groups, respectively. Vector analysis showed that the magnitude of error was −0.14±0.28 D and −0.05±0.16 D (p=0.085), the angle of error was −0.13±4.48 degrees and −2.57±29.42 degrees (p=0.592), the correction index was 0.94±0.13 and 0.94±0.35 (p=0.959), the index of success was 0.15±0.14 and 0.46±0.62 (p=0.517), and the flattening index was 0.93±0.13 and 0.71±0.59 (p=0.450) in the HA and LA groups, respectively. Conclusions This study demonstrates that SMILE is effective and safe for correcting high astigmatism. Vector analysis shows a tendency for the undercorrection of astigmatism in subjects with high astigmatism.

Clinical Evaluation of SMILE for Correction of Myopia

Abstract:Objective To investigate the clinical efficacy,safety,predictability and stability of femtosecond laser small incision lenticule extraction (SMILE) for the treatment of myopia.Methods In this prospective observational study,94 patients (170 eyes) with myopia underwent SMILE surgeries.The average age of the patients was 26.8±4.7 years (range 18 to 44 years).The mean preoperative spherical equivalent (SE) was-6.20±1.37 diopters (D),from-3.25 to-9.13 D.The median myopic astigmatism was-0.75 (0.75) D,up to-3.25 D.The mean corneal thickness was 554.7±27.5 μm (range 484 to 600 μm).Routine ophthalmic examinations were performed.The patients were followed from 6 months to 2 years with uncorrected visual acuity (UCVA),best corrected visual acuity (BCVA),manifest refraction,intraocular pressure,corneal topography,and slit lamp examinations.The results were analyzed with a paired t test and linear regression analysis.Results All procedures were completed successfully.At the long-term follow-up examination,97％ of eyes had a UCVA of 1.0 or better and 100％ of eyes had a UCVA of 0.8 or better.BCVA was equal to or better than the preoperative BCVA in 100％ of eyes.35％ gained one line and 10％ gained two lines of BCVA.The mean postoperative SE was-0.01±0.45 D and 87％ and 100％ were within ±0.50 and ±1.00 D of the attempted correction.The root mean square (RMS) values of total higher order aberrations (HOA),spherical aberrations and coma increased by a significant amount after surgery:0.15±0.12 μm,0.04±0.11 μm,0.14±0.14 μm (t=-6.861,-2.078,-5.672,P＜0.05).Conclusion The SMILE procedure is safe,effective,predictable and the results were stable for treating myopia.HOA,spherical and coma aberrations increased postoperatively but the change in spherical aberrations was small.

Abstract:Aims To compare long-term clinical outcomes following small incision lenticule extraction (SMILE) and femtosecond laser-assisted laser in situ keratomileusis (FS-LASIK) for myopia and myopic astigmatism correction. Methods In this retrospective study, we enrolled a total of 101 patients (101 eyes) who underwent SMILE or FS-LASIK 3 years prior. Measured parameters included uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), manifest refraction and corneal wavefront aberrations. Results No significant differences in patient characteristics were found between the two groups. At the 3-year follow-up, UDVA was better than or equal to 20/20 in 90% and 85% (p=0.540) of the eyes; the efficacy indexes were 1.05±0.19 and 1.01±0.21 in the SMILE and FS-LASIK groups, respectively (p=0.352). Safety indexes were 1.19±0.17 and 1.15±0.20 in the SMILE and FS-LASIK groups, respectively (p=0.307). Eighty per cent and 65% of eyes were within ±0.50 D of the attempted spherical equivalent correction after SMILE and FS-LASIK, respectively (p=0.164). Vector analysis revealed no significant differences in astigmatic correction between the two groups (p>0.05). Surgically induced spherical aberration was higher in the FS-LASIK group than in the SMILE group (p<0.001). Conclusion Long-term follow-up analysis suggested that both SMILE and FS-LASIK were safe and equally effective for myopic and astigmatic correction.

Abstract:BackgroundThis study aimed to evaluate 10-year refractive outcomes, corneal stability, axial length, and wavefront aberrations in patients who underwent SMILE to correct myopia.MethodsThirty two patients (32 eyes) who underwent SMILE-based myopic correction. Corrected distance visual acuity, uncorrected distance visual acuity, corneal stability, axial length, and wavefront aberrations were evaluated preoperatively and at 1 month and 1, 5, and 10 years postoperatively.ResultsAt 10 years postoperatively, the safety and efficacy indices for the patients included in this study were 1.19 +/- 0.21 and 1.04 +/- 0.27, respectively. For 26 (81%) and 30 eyes (94%), correction to within +/- 0.50 D and +/- 1.00 D of the target was achieved, respectively. Over the 10-year follow-up duration, a mean -0.32 +/- 0.56 D regression was observed (-0.03 +/- 0.06 D/year). Relative to baseline, horizontal and vertical comas significantly increased, as did the incidence of higher-order aberrations (all P < 0.001), whereas axial length and corneal elevation remained stable during follow-up.ConclusionThese results indicate that the SMILE-based correction for myopia of up to -10 Dioptres is safe, effective, and stable, with relatively constant wavefront aberrations and corneal stability over time after treatment.
Early Visual Outcomes and Optical Quality after Femtosecond Laser Small-Incision Lenticule Extraction for Myopia and Myopic Astigmatism Correction of Over-10 Dioptres

Abstract:PurposeTo investigate early visual and refractive outcomes, corneal stability and optical quality after femtosecond laser small-incision lenticule extraction (SMILE) for treating myopia and myopic astigmatism over -10D. MethodsThirty eyes (30 patients) with myopia and myopic astigmatism of over -10D were treated with VisuMax((R)) femtosecond laser (version 3.0; Carl Zeiss Meditec AG, Jena, Germany). Six months postoperative safety, efficacy and predictability were evaluated. Corneal Scheimpflug topography was measured preoperatively, 1day, 3months and 6months postoperatively. Wavefront aberrations were measured preoperatively, 3months and 6months postoperatively. ResultsSix months postoperatively, LogMAR uncorrected and corrected distance visual acuity (CDVA) were -0.0130.086 and -0.073 +/- 0.069, respectively. 73% (97%) of eyes were within 0.5 (1) D of target refraction. No eyes lost CDVA, 43% (13 eyes) gained one line and 7% (two eyes) gained two lines. Mean corneal back curvature (KMB) and posterior central elevation (PCE) did not change significantly comparing preoperative and 6months postoperative data (p=0.91 and 0.77, respectively). Comparing 1day with 6months postoperative data, central corneal thickness (CCT), mean corneal front curvature (KMF), KMB and PCE did not change significantly (p=0.27, 0.07, 0.52, 0.71, respectively). Total higher-order aberration (HOA), spherical aberration and coma increased significantly (p<0.01) but trefoil remained stable (p=0.49). ConclusionOur results indicate that SMILE can correct myopia and myopic astigmatism of over -10D predictably. No early ectasia was observed. Long-term changes in visual quality and corneal stability require further investigation.

Abstract:PURPOSE: To investigate objective optical quality and changes in light scatter after femtosecond laser small incision lenticule extraction (SMILE) for moderate to high myopia correction. METHODS: Prospective clinical study of 66 eyes of 66 patients (23 males, 43 females) with a mean age of 28.67 ± 6.62 years, mean spherical refraction of −6.06 ± 1.57 diopters, and mean cylindrical refraction of −0.68 ± 0.46 diopters. Every patient had a 3-month follow-up after a standard SMILE procedure. An optical quality analysis system was used to measure modulation transfer function cutoff frequency (MTF cutoff ), Strehl2D ratio, and objective scatter index (OSI). RESULTS: At 3 months postoperatively, mean spherical equivalent was 0.03 ± 0.24 diopters and corrected distance visual acuities of all patients were equal to or better than preoperative values, with a mean safety index of 1.12 ± 0.17 (range: 0.80 to 1.50) and mean efficacy index of 1.18 ± 0.21 (range: 0.80 to 1.50). Mean OSI increased from 0.75 preoperatively to 1.09 at 20 days postoperatively ( P < .05), and gradually declined to 0.94 at 40 days and 0.82 at 3 months postoperatively ( P > .05). MTF cutoff and Strehl2D ratio did not change significantly compared with preoperative values ( P > .05). Multiple linear regression analysis revealed that preoperative OSI was significantly correlated with optical quality at 3 months. Patients with less preoperative OSI tended to acquire higher MTF cutoff (b = −8.61) and lower OSI (b = 0.65) ( P < .05). CONCLUSIONS: SMILE had little impact on retinal image quality after myopia correction. Postoperative OSI showed a temporary increase and then returned to normal. Abstract:PURPOSE: To compare visual outcomes and aberration outcomes in small incision lenticule extraction (SMILE) versus femtosecond laser LASIK (FS-LASIK).METHODS: This prospective, comparative, nonrandomized clinical study included 68 eyes of 37 patients receiving SMILE and 55 eyes of 30 patients receiving FS-LASIK between December 2011 and January 2013 at the Fudan University Eye and ENT Hospital (Shanghai, People's Republic of China). Patients were followed up at 3, 6 months and 5 years after surgery. Main outcome measurements included uncorrected (UDVA) and corrected (CDVA) distance visual acuity, manifest refraction, central corneal thickness (CCT), total corneal refractive power (TCRP) and wavefront aberrations.RESULTS: About 98% (49/50) of treated eyes in the SMILE group and 95% (39/41) in the FS-LASIK group had a postoperative logMAR UDVA of 0 or better. Spherical equivalent (SE) after 5 years was -0.01 ± 0.35 D in the SMILE group and -0.23 ± 0.41 D in the FS-LASIK group. A regression of -0.02 ± 0.39 D in the SMILE group and -0.12 ± 0.32 D in the FS-LASIK group was observed between 6 months and 5 years postoperative time-points. TCRP increased by 0.39 ± 0.38 D in the SMILE group and 0.45 ± 0.49 D in the FS-LASIK group between 6-month and 5-year time-points. However, no statistically significant difference was found between the SMILE and FS-LASIK groups in terms of ∆SE, ∆CCT and ∆TCRP between 6 months and 5 years postoperative time-points.CONCLUSION: Myopic regression was observed in terms of TCRP but not in subjective refraction. No statistically significant difference in stability was found between SMILE and FS-LASIK.

METHODS: In this prospective study, 93 eyes of 93 patients (44 women, 49 men) were treated with SMILE-CCL. Mean age was 26.09 +/- 4.39 years and mean spherical equivalent was -6.31 +/- 2.16 diopters. The Objective Scatter Index (OSI) was examined with an objective optical quality analysis system. Bowman's layer microdistortions in the central 4-mm optical zone were measured using Fourier-domain optical coherence tomography. RESULTS: The mean OSI value increased from 0.67 preoperatively to 1.09 at 20 days, and then decreased to 0.84 at 3 months (P < .05). Microdistortions were observed in 62.37% of the eyes at 20 days and 48.39% at 3 months postoperatively (P < .05). The width of the microdistortions per meridian was 283.18 +/- 197.19 mu m at 20 days; this decreased to 156.00 +/- 159.86 mu m at 3 months (P < 0.05). Regression analysis models revealed that older age (b = 0.02, P =.03) and higher preoperative spherical equivalent (b = -0.09, P < .05) resulted in higher postoperative OSI values; no significant association was found between postoperative OSI values and microdistortion-related parameters (P > .05). Higher preoperative spherical equivalent (b = -0.28, P = .01) and thinner preoperative corneal thickness (b = -0.02, P = .01) were associated with more microdistortions. CONCLUSIONS: SMILE induced a temporary increase in OSI values and Bowman's layer microdistortions. The microdistortions did not influence intraocular scattering.

Abstract:PURPOSE: To investigate changes in corneal backscatter after small incision lenticule extraction (SMILE).METHODS: One eye was randomly selected from 34 patients who underwent SMILE procedures. Corneal backscatter was analyzed using Scheimpflug technology before surgery and at postoperative 1 day, 1 week, 1 month, 3, 6, and 12 months, and 3 years. Three different corneal depths (the anterior 120 mu m, the center, and the posterior 60 mu m) and three concentric radial zones (0 to 2, 2 to 6, and 6 to 10 mm) were analyzed. Another 40 eyes of 40 patients who underwent femto-second laser-assisted LASIK (FS-LASIK) were examined before surgery and at postoperative 1 day and 3 years as the control group.RESULTS: At postoperative day 1, a significant increase in corneal backscatter was detected in the SMILE group compared to preoperative values in the central 0 to 6 mm zone of the anterior layer and the 0 to 2 mm zone of the central layer (all P < .01). This increase was higher than the increase in the FS-LASIK group in the central 0 to 6 mm zone of the anterior and central layers (all P < .01). No significant differences in corneal backscatter were detected between preoperative values and the 1-week, and 1-, 3-, 6-, and 12-month follow-up visits in the SMILE group (all P > .05). Moreover, a significant decrease in corneal backscatter was detected in the three zones of all three layers in the two groups at 3 years postoperatively compared to preoperative values (all P < .001) and no significant difference was found between the two groups (all P < .05).CONCLUSIONS: Corneal backscatter peaks and declines to the baseline in the early period after SMILE and corneal transparency achieves an improvement within 3 years postoperatively.

Abstract:PURPOSE: To investigate long-term changes in posterior corneal elevation after small incision lenticule extraction (SMILE).METHODS: Thirty-six eyes of 20 patients (7 male, 13 female) who underwent SMILE were recruited for this prospective consecutive study. The mean preoperative manifest spherical equivalent was -6.24 +/- 1.47 diopters (D) (range: -3.88 to -8.75 D). Posterior corneal surfaces were measured by a Scheimpflug camera (Pentacam; Oculus Optikgerate GmbH, Wetzlar, Germany) preoperatively and at 1 month, 6 months, 12 months, 2 years, and 3 years after surgery. Posterior central elevation (PCE), posterior maximal elevation (PME), and posterior elevation at the thinnest corneal point (PTE) in the central 4 mm area above the best fit sphere (BFS) were analyzed. The BFS was the same across all followup periods and determined by the preoperative data. A P value of less than.05 was considered statistically significant.RESULTS: All surgeries were executed without complications and no case of keratectasia was observed during the follow-up. An uncorrected distance visual acuity of 20/20 or better was achieved in 100% of eyes at the last visit. The mean change of PCE, PME, and PTE at 3 years after the operation was -2.39 +/- 2.85, 0.50 +/- 3.33, and -2.33 +/- 2.90 mu m, respectively. There were significant differences in the measurements of PCE and PTE before surgery and 3 years after surgery (P =.009); however, there was no significant difference in PME before surgery and 3 years after surgery (P = 1.000). No correlation was found between changes in posterior corneal elevation and residual bed thickness, ablation depth, and preoperative thinnest corneal thickness.CONCLUSIONS: The posterior corneal surface, measured using the Pentacam, was stable after SMILE in the long-term follow-up. The cause of the slight backward change of PCE and PTE needs further study.

Abstract:Abstract Background: To study the 5-year changes in the posterior corneal surface after small incision lenticule extraction (SMILE) for high myopia.Methods: Eighty eyes received SMILE was included in this prospective study. They were allocated into two groups based on the spherical equivalent: high myopia (40eyes, -7.49±0.70D) and moderate myopia (40eyes, -4.43±0.87D). Certain points of posterior corneal elevation (the central point (PCE), thinnest point (PTE), maximal point (PME), and in various corneal areas) were evaluated using a Scheimpflug camera (Pentacam; Oculus GmbH, Germany) preoperatively and at 6 months and 5 years after surgery. Results: All surgeries were completed uneventfully and no ectasia was developed throng the observation. The safety index and efficacy index were 1.14 and 1.03 in the high myopia group, and 1.16 and 1.06 in the moderate myopia group, respectively. Most of the calculated values in the high myopia group showed a slight increase at 6 months but decreased at 5 years. At 5 years postoperatively, the value of the PTE was significantly lower than at baseline in both groups (P≤0.047); a statistical difference was also revealed in the PME in the moderate group with slight changes (10.15±3.01μm vs. 11.60±4.33μm, P=0.002); no statistical significance was observed in other calculated values (P≥0.067). Similarly, no significant linear correlation was noted between changes in all values and the residual bed thickness either (P≥0.057).Conclusions: SMILE causes no protrusion in posterior corneal surface for correction of high myopia at the follow-up visit of 5 years.

Abstract:Purpose: To compare the long-term refractive effects of smallincision lenticule extraction (SMILE) and femtosecond laserassisted laser in situ keratomileusis (FS-LASIK) on the correction of high myopia and astigmatism.Setting: The Eye and ENT Hospital of Fudan University, Shanghai, China.Design: Retrospective case series.Methods: Patients had SMILE or FS-LASIK for high myopia. Subgroup analyses of high myopia (-6.00 to -8.75 diopters [D]) and extremely high myopia (>=-9.00 D) were performed. The main outcome measure was refractive predictability, compared between SMILE and FS-LASIK groups. Secondary outcomes included efficacy, safety, and residual astigmatism.Results: This study included 121 patients (121 eyes: 75 in the SMILE group and 46 in the FS-LASIK group). No differences were found in terms of refractive predictability between SMILE and FS-LASIK in eyes with highmyopia: 56% vs 58.7% achieved +/- 0.50 D of attempted correction (P =.771) and 81.3% vs 76.1% achieved +/- 1.00 D of attempted correction (P =.489). Efficacy indices of the SMILE and FS-LASIK groups were 1.02 +/- 0.24 and 1.03 +/- 0.24 (P =.742), respectively; safety indices were 1.23 +/- 0.22 and 1.20 +/- 0.24 (P =.324), respectively. Logarithmof the minimumangle of resolution uncorrected distance visual acuity and spherical equivalent in the high myopia subgroup were better than in the extremely high myopia subgroup after both SMILE and FS-LASIK (P <.01).Conclusions: SMILE and FS-LASIK were both effective in correcting high myopia and myopic astigmatism. Abstract:Small incision lenticule extraction (SMILE) is a "flapless" keratorefractive surgery with excellent safety, efficacy, stability, and predictability for myopia correction. A recent global multicenter study also reported good refractive outcomes for hyperopic SMILE. SMILE has shown advantages including improved biomechanical strength, fewer dry eye symptoms, less corneal denervation, and fewer surgery-induced higher-order aberrations over laser in situ keratomileusis (LASIK). However, night vision complaints, including glare, halos, and starbursts, could still occur after SMILE. These symptoms have been proven to be closely related to the effective optical zone (EOZ), which is defined as the achieved area of corneal ablation. A larger postoperative EOZ may indicate better visual quality, making EOZ an important safety parameter for keratorefractive surgeries. As SMILE has gained wider application globally, the EOZ following SMILE has also been increasingly studied in the field of refractive surgery. This review provides an update on topics related to the EOZ after SMILE, including its measurement and influencing factors, aiming to benefit the personalization of the surgical algorithm and ultimately improve the visual quality after the SMILE procedure.
