Abstract:Objective Atrial Fibrillation (AF) may result in stroke, heart failure and even death. Despite maximal antiarrhythmic agents many will have persistent/permanent AF. Posterolateral right atrium (RA) is a common area of origination of AF but poses challenges during catheter ablation due to proximity of right phrenic nerve. In these cases surgical assistance may be required to protect phrenic nerve and also allows for a hybrid (surgical epicardial and endocardial) ablation with better success rate. Design and method A 59-year-old lady presented with breathlessness of NYHA 3 category and several episodes of palpitations and dizziness. Her past medical history included an old myocardial infarction, CRTD implantation for ventricular tachycardia, type 2 diabetes, treated hypothyroidism, and hypertension. She had severe left ventricular dysfunction (LVEF 20%) and was awaiting heart transplantation on a long awaiting list. Electrophysiology (EP) study revealed that she had persistent AF and needed ablation of SA node for symptomatic relief. Her regular medications included Digoxin, Apixaban, Valsartan, Spironolactone, Nitrates, Nicorandil, Ivabradine, Bisoprolol, Doxazocin, Thyroxine, and Rosuvastatin. Previous endocardial ablation had failed because of risk of potential injury to phrenic nerve due to its anatomical location at the posterolateral part of RA. Hence a combined approach (epicardial via right thoracotomy and endocardial) was planned. Routine blood tests, CXR, ECG, Transthoracic Echocardiography, coagulation profile were checked. Upon arrival in EP lab standard ASA (American Society of Anaesthesiology) monitors were attached along with cerebral oximetry and bispectral index. A wide bore peripheral line(16G) and 20G right radial arterial line were placed under 1% Lignocaine infiltration. Induction of general anaesthesia was carried out with intravenous Etomidate, Fentanyl citrate and Rocuronium bromide and size 8mm endotracheal intubation was placed using video laryngoscope. Then 8.5Fr five lumen central venous line was inserted via right internal jugular vein. Thereafter a X7-2t TOE (trans oesophageal echocardiography) probe and a diaphragmatic pacing oesophageal temperature probe were placed. Maintenance of anaesthesia was done by Sevoflurane, Fentanyl and Rocuronium. TOE was done to rule out LAA thrombus and check for heart function and any pericardial collection post ablation. A single lumen bronchial blocker was used to isolate right lung. Serial arterial blood gases were done to monitor one lung ventilation(OLV). Via open right thoracotomy surgeon retracted the phrenic nerve, performed cryo-ablation (-65 degree C by N2O) of RA. Cardiologists performed rest of the RA ablation safely by radio frequency based on EP mapping. The surgeon kept the phrenic nerve retracted with a hook to avoid injury during endocardial ablation. Diaphragmatic pacing also guided this. She was extubated on table. Post procedure AF was converted to sinus rhythm. She was shifted to intensive care with minimal Noradrenaline support and was discharged to ward next day. Anaesthetic challenges were inducing a patient with poor heart function, Lung isolation and OLV, TOE skills, arrythmia management. Results and conclusions Posterolateral part of RA being in close proximity to phrenic nerve poses threat to result diaphragmatic palsy. Here combined approach of ablation is a safe and alternative technique. Anaesthetic challenges are enormous in high risk patients and needs expert skills.
