Abstract:Objective: To develop a non-invasive,effective,and convenient virtual cystoscopy system so as to realize three-dimensional visualization and observation for the bladder lumen of patient with bladder cancer(BC).Method:Based on the magnetic resonance imaging(MRI)datasets and medical imaging interaction toolkit(MITK)libraries to establish a database for image information of patients.And based on the arithmetic of Fast Marching Segmentation and arithmetic of live-wire to design semi-automatic partition method of image so as to extract region of interest(ROI),and realize three-dimension(3D)reconstruction and other functions,and obtain 3D mode of bladder lumen pre reconstruction.Using virtual endoscope function to carry out multi-angle observation and realize the detection for lesion of bladder lumen.Results: The 3D visualization and observation of bladder lumen can obtain importantly diagnosis-related information such as tumor locations,amount and size,and can realize preoperative detection of BC.Conclusion: The development of the virtual cystoscopy system,the non-invasive measurement of visualization BC and 3D visualization observation can provide new assistant mean for clinically early screening of susceptible population and postoperative reexamination of patients.
Quantitative Analysis of Bladder Wall Thickness for Magnetic Resonance Cystoscopy

Abstract:Objective: To find an effective way for quantitative evaluation on wall thickness variation of human bladder with/without bladder tumor, a novel pipeline of thickness measurement and analysis for magnetic resonance (MR) cystography is proposed. Methods: After the acquisition of volumetric bladder images with a high-resolution T2-weighted 3-D sequence, the inner and outer borders of the bladder wall were segmented simultaneously by a coupled directional level-set method. Then, the bladder wall thickness (BWT) was estimated using the Laplacian method. To reducing the influence of individual variation and urine filling on wall thickness, a thickness normalization using Z-score is performed. Finally, a parametric surface mapping strategy was applied to map thickness distribution onto a unified sphere surface, for quantitative intra- and intersubject comparison between bladders of different shapes. Results: The proposed pipeline was tested with a database composed of MR bladder images acquired from 20 volunteers and 20 patients with bladder cancer. The results indicate that the thickness normalization step using Z-score makes the quantitative comparison of wall thickness quite possible and there is a significant difference on BWT between patients and volunteers. Using the proposed pipeline, we established a thickness template for a normal bladder wall based on dataset of all volunteers. Conclusion: As a first attempt to establish a general pipeline for bladder wall analysis, the presented work provides an effective way to achieve the goal of evaluating the entire bladder wall for detection and diagnosis of abnormality. In addition, it can be easily extended to quantitative analyses of other bladder features, such as, intensity-based or texture features.

Abstract:In this paper, we propose a coupled level set (LS) framework for segmentation of bladder wall using T1-weighted magnetic resonance (MR) images with clinical applications to virtual cystoscopy (i.e., MR cystography). The framework uses two collaborative LS functions and a regional adaptive clustering algorithm to delineate the bladder wall for the wall thickness measurement on a voxel-by-voxel basis. It is significantly different from most of the pre-existing bladder segmentation work in four aspects. First of all, while most previous work only segments the inner border of the wall or at most manually segments the outer border, our framework extracts both the inner and outer borders automatically except that the initial seed point is given by manual selection. Secondly, it is adaptive to T1-weighted images with decreased intensities in urine, as opposed to enhanced intensities in T2-weighted scenario and computed tomography. Thirdly, by considering the image global intensity distribution and local intensity contrast, the defined image energy function in the framework is more immune to inhomogeneity effect, motion artifacts and image noise. Finally, the bladder wall thickness is measured by the length of integral path between the two borders which mimic the electric field line between two iso-potential surfaces. The framework was tested on six datasets with comparison to the well-known Chan-Vese (C-V) LS model. Five experts blindly scored the segmented inner and outer borders of the presented framework and the C-V model. The scores demonstrated statistically the improvement in detecting the inner and outer borders.
A New Partial Volume Segmentation Approach to Extract Bladder Wall for Computer Aided Detection in Virtual Cystoscopy

Abstract:We propose a new partial volume (PV) segmentation scheme to extract bladder wall for computer aided detection (CAD) of bladder lesions using multispectral MR images. Compared with CT images, MR images provide not only a better tissue contrast between bladder wall and bladder lumen, but also the multispectral information. As multispectral images are spatially registered over three-dimensional space, information extracted from them is more valuable than that extracted from each image individually. Furthermore, the intrinsic TI and TZ contrast of the urine against the bladder wall eliminates the invasive air insufflation procedure. Because the earliest stages of bladder lesion growth tend to develop gradually and migrate slowly from the mucosa into the bladder wall, our proposed PV algorithm quantifies images as percentages of tissues inside each voxel. It preserves both morphology and texture information and provides tissue growth tendency in addition to the anatomical structure. Our CAD system utilizes a multi-scan protocol on dual (full and empty of urine) states of the bladder to extract both geometrical and texture information. Moreover, multi-scan of transverse and coronal MR images eliminates motion artifacts. Experimental results indicate that the presented scheme is feasible towards mass screening and lesion detection for virtual cystoscopy (VC).

Abstract:This paper proposes a framework for detecting the suspected abnormal region of the bladder wall via magnetic resonance (MR) cystography. Volume-based features are used. First, the bladder wall is divided into several layers, based on which a path from each voxel on the inner border to the outer border is found. By using the path length to measure the wall thickness and a bent rate (BR) term to measure the geometry property of the voxels on the inner border, the seed voxels representing the abnormalities on the inner border are determined. Then, by tracing the path from each seed, a weighted BR term is constructed to determine the suspected voxels, which are on the path and inside the bladder wall. All the suspected voxels are grouped together for the abnormal region. This work is significantly different from most of the previous computer-aided bladder tumor detection reports on two aspects. First of all, the T(1)-weighted MR images are used which give better image contrast and texture information for the bladder wall, comparing with the computed tomography images. Second, while most previous reports detected the abnormalities and indicated them on the reconstructed 3-D bladder model by surface rendering, we further determine the possible region of the abnormality inside the bladder wall. This study aims at a noninvasive procedure for bladder tumor detection and abnormal region delineation, which has the potential for further clinical analysis such as the invasion depth of the tumor and virtual cystoscopy diagnosis. Five datasets including two patients and three volunteers were used to test the presented method, all the tumors were detected by the method, and the overlap rates of the regions delineated by the computer against the experts were measured. The results demonstrated the potential of the method for detecting bladder wall abnormal regions via MR cystography.

Abstract:Bladder cancer is the fifth cause of cancer deaths in the United States. Virtual cystoscopy (VC) can be a screening means for early detection of the cancer using non-invasive imaging and computer graphics technologies. Previous researches have mainly focused on spiral CT (computed tomography), which invasively introduces air into bladder lumen for a contrast against bladder wall via a small catheter. However, the tissue contrast around bladder wall is still limited in CT-based VC. In addition, CT-based technique carries additional radiation. We have investigated a procedure to achieve the screening task by MRI (magnetic resonance imaging). It utilizes the unique features of MRI: (1) the urine has distinct T1 and T2 relaxation times as compared to its surrounding tissues, and (2) MRI has the potential to obtain good tissue contrast around bladder wall. The procedure is fully non-invasive and easy in implementation. In this paper, we proposed a MRI-based VC system for computer aided detection (CAD) of bladder tumors. The proposed VC system is an integration of partial volume-based segmentation containing texture information and fast marching-based CAD employing geometrical features for detecting of bladder tumors. The accuracy and efficiency of the integrated VC system are evaluated by testing the diagnoses against a database of patients.

Abstract:At present,the most reliable and accurate method for detecting bladder cancer is optical cystoscopy,but cystoscopy is invasive,expensive and field of vision is limited.We want to use a virtual cystoscopy technology based on medical imaging and image processing instead of optical cystoscopy.In this paper we uses the T1-weighted MRI bladder images,using a level set method for segmentation of the bladder wall,right after the image segmentation using the marching cubes algorithm for surface rendering,using vtk platform for the display of virtual cystoscopy.Experimental results show that method can be successfully achieved the bladder wall 3d display.

Abstract:Purpose:To evaluate the techniques and clinical applications of virtual endoscopy based on spiral CT scan (CTVE) in bladder tumors. Methods:Thirty five patients suffered from bladder tumors were undergone volume scanning using spiral CT. Virtual endoscopy images were generated from volumetric data using software and the bladders were observed from multi directions by Fly Through software. All cases were compared with the diagnoses of fiberoptic cystoscopy. Results:Thirty four cases of bladder tumors were presented clearly on CTVE scans including 2 cases missed on fiberoptic cystoscopy and the diagnoses were confirmed by operation or pathology except for 1 case of bladder tumor (diameter less than 0.5 cm) missed. Conclusions:CTVE has proved to be a non-invasive and reliable methods in the diagnosis of bladder tumor, which can be served as a supplement to fiberoptic cystoscopy.

Abstract:Virtual cystoscopy (VC) is a developing noninvasive, safe, and low-cost technique for bladder cancer screening. Multispectral (T-1- and T-2-weighted) magnetic resonance (MR) images provide a better tissue contrast between bladder wall and bladder lumen comparing with computed tomography (CT) images. The intrinsic T-1 and T-2 contrast of the urine against the bladder wall eliminates the invasive air insufflation procedure which is often used in CT-based VC. We propose a new partial volume (PV) segmentation scheme with inhomogeneity correction to segment multispectral MR images for tumor screening by virtual cystoscopy. The proposed PV segmentation algorithm automatically estimates the bias field and segments tissue mixtures inside each voxel of MR images, thus preserving texture information. Experimental results indicate that the present scheme is promising towards mass screening by virtual cystoscopy means.

Abstract:Bladder cancer is reported to be the fifth leading cause of cancer deaths in the United States. Recent advances in medical imaging technologies, such as magnetic resonance (MR) imaging, make virtual cystoscopy a potential alternative with advantages as being a safe and non-invasive method for evaluation of the entire bladder and detection of abnormalities. To help reducing the interpretation time and reading fatigue of the readers or radiologists, we introduce a computer-aided detection scheme based on the thickness mapping of the bladder wall since locally-thickened bladder wall often appears around tumors. In the thickness mapping method, the path used to measure the thickness can be determined without any ambiguity by tracing the gradient direction of the potential field between the inner and outer borders of the bladder wall. The thickness mapping of the three-dimensional inner border surface of the bladder is then flattened to a two-dimensional (2D) gray image with conformal mapping method. In the 2D flattened image, a blob detector is applied to detect the abnormalities, which are actually the thickened bladder wall indicating bladder lesions. Such scheme was tested on two MR datasets, one from a healthy volunteer and the other from a patient with a tumor. The result is preliminary, but very promising with 100% detection sensitivity at 7 FPs per case.
Segmenting bladder wall and estimating wall thickness for magnetic resonance virtual cystoscopy

Abstract:A level set method for bladder wall segmentation and wall thickness estimates was developed to extract the features of bladder abnormalities in a virtual cystoscopy system,using T1-weighted magnetic resonance(MR) images.The local intensity contrast information is used to construct the image energy with two level set functions to segment the inner and outer borders of the bladder wall.A path integration distance is used to estimate the bladder wall thickness,by mimicing the distribution of the electric field line between two iso-potential surfaces.The bladder wall thickness is then mapped to a pseudo-color space for rendering the reconstructed 3-D bladder model.Ten clinical cases including volunteers and patients were used to test the system.The results show that the system is robust and effective for automatically segmenting the bladder wall,estimating the wall thickness,capturing the abnormal variations of the wall thickness in the patient dataset,and further showing the abnormal variations on the 3-D model to offer reliable information for virtual cystoscopy aided diagnosis.

Abstract:As a non-invasive bladder tumor screening approach, magnetic resonance imaging (MRI)-based virtual cystoscopy (VCys) has received increasing attention for a better soft tissue contrast compared to computer tomography (CT)-based VCys. In this paper, some preliminary work on segmenting the inner boundary of bladder wall from both T1- and T2- weighted MR bladder images were presented. Via an iterative maximum a posteriori expectation-maximization (MAPEM) approach, the tissue mixture fractions inside each voxel were estimated. Considering the partial volume effect (PVE) that MR images suffer from, the advantages of such mixture-based segmentation approach are (1) statistics-based tissue mixture model that shapes each tissue type as a normal-distributed random variable, (2) closed-form mathematical MAP-EM iterative solution, and (3) capability and efficiency of the estimated tissue mixture fractions in reflecting PVE. Given the extracted inner bladder wall, manipulations could be further taken, for each individual voxel located on the inner bladder wall, to identify the outer bladder wall prior to the measurement of wall thickness. Not limited to geometrical analysis, the consideration of PVE in the study of early stage abnormality on the mucosa in the scope of VCys is believed to provide more textural information in distinguishing from neighboring artifacts about the surface deformations that is due to bladder tumors.

Abstract:Objective:To design automatic detecting system for bladder tumor based on magnetic resonance imaging (MRI) so as to overcome the disadvantages of invasive wound and blind area of optical cystoscope and explore early thinking for noninvasive and convenient detection.Methods: According to the established automatic detection method of bladder tumor, the T2 weighted image sequential data of 10 patients with bladder tumor were implemented primary verification. The T2-weighted MR images were acquired, and the adaptive shape prior constrained level sets (ASPCLS) algorithm was applied to cut inner and outer surfaces of the bladder wall. The three-dimensional bladder wall thickness(BWT) was calculated by using the Laplace partial differential equation. Based on above results, through used fuzzy C-Means with spatial information(sFCM) to extract suspected tumor regions. At finally, the systematic function of extracting suspected region of bladder tumor was realized.Results: The results indicated that the system could relatively accurate extract suspected region of bladder tumor, and the average Dice' similarity coefficient (DSC) of the suspected region drawn by system and the true region drawn by doctor was 86.7％. And it was better results. Conclusion: The automatic detection system can primarily realize the relatively accurate detection for bladder tumor and the extraction of suspected region. And it can provide newly noninvasive method for the early detection and preoperative diagnosis of bladder tumor.

The research objective of this paper are: (1) to incorporate a statistical maximum a posterior (MAP) framework, for tissue segmentation (local computation); (2) to determine a set of intrinsic parameters for modeling bladders (global computation); (3) to develop a hybrid-based diagnosis system based on both the local feature computations and the global parameters obtained.

Abstract:Magnetic resonance visual cystoscopy or MR cystography (MRC) is an emerging tool for bladder tumor detection, where three-dimensional (3D) endoscopic views on the inner bladder surface are being investigated by researchers. In this paper, we further investigate an innovative strategy of visualizing the inner surface by flattening the 3D surface into a 2D display, where conformal mapping, a mathematically-proved algorithm with shape preserving, is used. The original morphological, textural and even geometric information can be visualized in the flattened 2D image. Therefore, radiologists do not have to manually control the view point and angle to locate the possible abnormalities like what they do in the 3D endoscopic views. Once an abnormality is detected on the 2D flattened image, its locations in the original MR slice images and in the 3D endoscopic views can be retrieved since the conformal mapping is an invertible transformation. In such a manner, the reading time needed by a radiologist can be expected to be reduced. In addition to the surface information, the bladder wall thickness can be visualized with encoded colors on the flattened image. Both normal volunteer and patient studies were performed to test the reconstruction of 3D surface, the conformal flattening, and the visualization of the color-coded flattened image. A bladder tumor of 3 cm size is so obvious on the 2D flattened image such that it can be perceived only at the first sight. The patient dataset shows a noticeable difference on the wall thickness distribution than that of the volunteer's dataset.
A Unified EM Approach to Bladder Wall Segmentation with Coupled Level-Set Constraints

Abstract:Magnetic resonance (MR) imaging-based virtual cystoscopy (VCys), as a non-invasive, safe and cost-effective technique, has shown its promising virtue for early diagnosis and recurrence management of bladder carcinoma. One primary goal of VCys is to identify bladder lesions with abnormal bladder wall thickness, and consequently a precise segmentation of the inner and outer borders of the wall is required. In this paper, we propose a unified expectation-maximization (EM) approach to the maximum-a posteriori (MAP) solution of bladder wall segmentation, by integrating a novel adaptive Markov random field (AMRF) model and the coupled level-set (CLS) information into the prior term. The proposed approach is applied to the segmentation of T(1)-weighted MR images, where the wall is enhanced while the urine and surrounding soft tissues are suppressed. By introducing scale-adaptive neighborhoods as well as adaptive weights into the conventional MRF model, the AMRF model takes into account the local information more accurately. In order to mitigate the influence of image artifacts adjacent to the bladder wall and to preserve the continuity of the wall surface, we apply geometrical constraints on the wall using our previously developed CLS method. This paper not only evaluates the robustness of the presented approach against the known ground truth of simulated digital phantoms, but further compares its performance with our previous CLS approach via both volunteer and patient studies. Statistical analysis on experts' scores of the segmented borders from both approaches demonstrates that our new scheme is more effective in extracting the bladder wall. Based on the wall thickness calibrated from the segmented single-layer borders, a three-dimensional virtual bladder model can be constructed and the wall thickness can be mapped onto the model, where the bladder lesions will be eventually detected via experts' visualization and/or computer-aided detection.

Abstract:This paper proposes an adaptive window-setting scheme for noninvasive detection and segmentation of bladder tumor surface in T-1-weighted magnetic resonance (MR) images. The inner border of the bladder wall is first covered by a group of ball-shaped detecting windows with different radii. By extracting the candidate tumor windows and excluding the false positive (FP) candidates, the entire bladder tumor surface is detected and segmented by the remaining windows. Different from previous bladder tumor detection methods that are mostly focusing on the existence of a tumor, this paper emphasizes segmenting the entire tumor surface in addition to detecting the presence of the tumor. The presented scheme was validated by ten clinical T-1-weighted MR image datasets (five volunteers and five patients). The bladder tumor surfaces and the normal bladder wall inner borders in the ten datasets were covered by 223 and 10 491 windows, respectively. Such a large number of the detecting windows makes the validation statistically meaningful. In the FP reduction step, the best feature combination was obtained by using receiver operating characteristics or ROC analysis. The validation results demonstrated the potential of this presented scheme in segmenting the entire tumor surface with high sensitivity and low FP rate. This study inherits our previous results of automatic segmentation of the bladder wall and will be an important element in our MR-based virtual cystoscopy or MR cystography system.

Abstract:In this paper, we propose a coupled level-set framework for segmentation of bladder wall using T1-weighted magnetic resonance (MR) images. The segmentation results will be used for non-invasive MR-based virtual cystoscopy (VCys). The framework uses two level-set functions to segment inner and outer borders of the bladder wall respectively. Based on Chan-Vese (C-V) model, a local adaptive fitting (LAF) image energy is introduced to capture local intensity contrast. Comparing with previous work, our method has the following advantages. First of all, unlike most other work which only segments the boundary of the bladder but not inner border and outer border respectively, our method extracts the inner border as well as the outer border of bladder wall automatically. Secondly, we focus on T1-weighted MR images which decrease the image intensity of the urine and therefore minimize the partial volume effect (PVE) on the bladder wall for detection of abnormalities on the mucosa layer in contrast to others' work on CT images and T2-weighted MR images which enhance the intensity of the urine and encounter the PVE. In addition, T1-weighted MR images provide the best tissue contrast for detection of the outer border of the bladder wall. Since MR images tend to be inhomogeneous and have ghost artifacts due to motion and other causes as compared to computer tomography (CT)-based VCys, our framework is easy to control the geometric property of level-set functions to mitigate the influences of inhomogeneity and ghosts. Finally, a variety of geometric parameters, such as the thickness of bladder wall, etc, can be measured easily under the level-set framework. These parameters are clinically important for VCys. Diagnostic potential of virtual cystoscopy of the bladder: MRI vs CT. Preliminary report

Abstract:Abstract. The aim of the study was to evaluate the diagnostic potential of the half-Fourier acquired single-shot turbo spin echo (HASTE) sequence for MRI cystoscopy compared with CT cystoscopy and the gold standard conventional cystoscopy. The MRI- and CT cystoscopy was performed in 29 patients with bladder tumors documented at cystoscopy and a control group using a 1.5-T unit (6-mm slice thickness) and a helical CT (3-mm collimation, pitch 1) after filling the bladder with air. Axial MRI and CT images were transferred to a workstation for application of virtual cystoscopy and compared with routine cystoscopy. Axial images and virtual cystoscopies were read by three readers for size and location of tumors. Forty-seven tumors were diagnosed at cystoscopy (12<1 cm, 35≥1 cm). The sensitivity for detection of tumors smaller than 1 cm was 88.9% at MRI cystoscopy and 100% for tumors of 1 cm or larger. These results were not statistically different from the other modalities. Three tumors smaller than 1 cm were not detected at CT cystoscopy, four not all identical at MRI cystoscopy. One tumor with a wall thickening was detected on axial CT and MR images and CT cystoscopy by all observers, but only by two at MRI cystoscopy. The MRI cystoscopy is a diagnostic modality with results comparable to conventional and CT cystoscopy.

Abstract:To find an effective way to quantitatively analyze the thickness variation of human bladder wall under different states, in this paper, we proposed a novel pipeline for thickness measurement, analysis, and mapping of bladder wall based on T2-weighted MRI images. The pipeline includes major steps of data acquisition, automatic segmentation of bladder wall, 3D thickness calculation, thickness normalization, and standardized bladder shape mapping. Based on the proposed pipeline, 20 datasets including 10 patients and 10 volunteers were used to explore the distribution pattern of wall thickness and find the difference between cancerous tissue and normal bladder wall. The results demonstrated the potential of wall thickness as a good indicator of bladder abnormalities, indicating its possible use in lesion detection on the bladder wall.
