Abstract:The neutrophil-to-lymphocyte ratio (NLR), an inflammatory marker, plays an important role in the inflammatory mechanisms of the pathophysiology and progression of acute ischemic stroke (AIS). The aim of this study was to identify the potential factors associated with functional prognosis in AIS. A total of 303 AIS patients were enrolled in this study; baseline information of each participant, including demographic characteristics, medical history, laboratory data, and 90-day functional outcome, was collected. Multivariate logistic regression analysis revealed that NLR, systolic blood pressure (SBP) and National Institutes of Health Stroke Scale (NIHSS) score were found to be independent factors for poor functional outcomes. Receiver operating characteristic (ROC) curve analysis was performed to estimate the predictive value of the NLR for 90-day functional outcome, with the best predictive cutoff value being 3.06. In the multivariate logistic regression analysis, three models were constructed: Model 1, adjusted for age, sex, SBP, and TOAST classification (AUC = 0.694); Model 2, further adjusted for the NIHSS score at admission (AUC = 0.826); and Model 3, additionally adjusted for the NLR (AUC = 0.829). The NLR at admission was an independent predictor of 90-day prognosis in patients with AIS. The risk factors related to poor 90-day functional outcomes were higher SBP, higher NLR, and a greater NIHSS score.

What problem does this paper attempt to address?
The problem that this paper attempts to solve is: **To explore the association between neutrophil - to - lymphocyte ratio (NLR) and 90 - day functional prognosis in patients with acute ischemic stroke (AIS)**, and to identify potential factors that may affect the 90 - day functional prognosis of AIS patients.
Specifically, the main objectives of the study include:
1. **To evaluate the effectiveness of NLR as a predictor of 90 - day functional prognosis in AIS patients**. Through multivariate logistic regression analysis and receiver operating characteristic (ROC) curve analysis, verify whether NLR can be an independent prognostic factor.
2. **To explore the influence of other clinical factors on the 90 - day functional prognosis of AIS patients**. The study considered a variety of clinical factors, such as systolic blood pressure (SBP), National Institutes of Health Stroke Scale (NIHSS) score, TOAST classification, etc., and constructed multiple prediction models to evaluate the combined influence of these factors.
### Research Background
Acute ischemic stroke (AIS) is a common cerebrovascular disease with a high disability rate and a high mortality rate. Timely prediction and intervention can effectively reduce the disability rate of patients. Inflammatory response plays an important role in the pathophysiological process of AIS, especially in terms of brain injury and tissue repair. In recent years, some new inflammatory markers, such as NLR, have been proven to be closely related to the clinical prognosis of stroke.
### Research Methods
- **Research Design**: A single - center retrospective cohort study, including 303 patients diagnosed with AIS from July 2021 to October 2022.
- **Data Collection**: Collected the baseline information of patients, including demographic characteristics, medical history, laboratory data and 90 - day functional prognosis (using the modified Rankin Scale mRS score).
- **Statistical Analysis**: Statistical analysis was performed using SPSS 26.0, including univariate and multivariate logistic regression analysis, ROC curve analysis, etc.
### Main Findings
- **NLR is an independent predictor of 90 - day functional prognosis**. The optimal prediction cut - off value is 3.06, with a sensitivity of 70.3% and a specificity of 67.0%.
- **Other significant prognostic factors**: Higher SBP, higher NIHSS score and higher NLR are associated with poor functional prognosis.
- **Accuracy of the prediction model**: The model combining SBP, TOAST classification, NIHSS score and NLR (AUC = 0.829) shows the highest prediction accuracy.
### Conclusion
As a simple and reliable inflammatory marker, NLR has important value in predicting the 90 - day functional prognosis of AIS patients. The research results emphasize the importance of early identification of high - risk patients and taking effective intervention measures to improve the clinical outcomes of patients.
