OP02.02: Quantitative Comparison of Cardiac Performance Index Derived from a Novel Automatic Method and Traditional Technique in Normal Fetuses

Abstract:To compare the left ventricular (LV) myocardial performance index (MPI), obtained by a novel automatic method (auto-MPI) and traditional technique in fetuses with structurally normal hearts, and to investigate the reproducibility of measurements of automatic MPI and the intraobserver and interobserver variability. The LV MPI were measured using a novel automatic method and the traditional method (mod-MPI) in 98 normal fetuses. The MPI difference between the two methods and intraclass correlation coefficient of the agreement between the methods were tested. The reliability of measurements of auto-MPI and the intraobserver and interobserver variability were assessed. Both automatic and traditional LV MPI were measured successfully in 93 cases (94.9%) by two experienced echocardiographers. The mean auto-MPI and mod-MPI were 0.4044 ± 0.0673, 0.4073 ± 0.0723, respectively. A paired sample t-test directly compared two approaches, which revealed no systematic difference (P = 0.777). The intraclass correlation coefficient was 0.872 and the 95% confidence interval (CI) was 0.813–0.913 for the two methods. Bland-Altman analysis tested the bias was 0.003. Coefficients of variations of the automatic and traditional methods were 9.36% and 11.88%, respectively, for assessing intraobserver variability and 13.58% and 18.46% respectively, for assessing interobserver variability. Intraclass correlation coefficients of the automatic method were higher than those of traditional method, with 0.858 and 0.817, respectively, for assessing intraobserver variability and 0.664 and 0.507, for assessing interobserver variability. The reproducibility of measurements of automatic MPI was stable. Intraobserver variability and interobserver variability for the automatic method were less than those for the traditional method.
