Abstract:Object. The authors evaluated different surgical methods used to treat thoracic ossification of the ligamentum flavum (OLF). Methods. Data obtained in 40 patients who underwent posterior decompression for thoracic myelopathy caused by thoracic OLF were studied retrospectively. There were 32 men and eight women. All patients underwent posterior decompression in which laminoplasty was performed or laminectomy combined with lateral fusion. Every surgical specimen was stained with H & E, and scanning electron microscopy was performed in 20 cases. The mean follow-up period was 28 months. Postoperative outcomes were evaluated using a recovery scale based on the Japanese Orthopaedic Association classification. There were a total of 168 ossified segments in this series, 77.4% of which were located in the lower thoracic spine. Marginal osteophyte formation was found in 36 patients; in 32 of the 36 patients, these marginal osteophytes were located at the intervertebral space either higher or lower than the ossified segment. Scanning electron microscopy showed elastic fiber breakdown, proliferation of collagenous fibers, calcification, and OLF in the same microscopy region. Laminoplasty was performed in four patients. In three cases surgery resulted in unchanged or worse outcome (increased kyphotic deformity in two), and in one it resulted in good outcome. Laminectomy combined with lateral fusion was performed in 36 patients, in 30 cases of which it resulted in a good or fair outcome, and increased kyphotic deformity in only one. Of these 36 laminectomy-treated patients, an en bloc laminectomy-treated procedure was performed in 16 patients; in 11 of the 12 patients with lateral or diffuse-type lesions the surgery resulted in a good or fair outcome. En bloc laminectomy, however, seems ineffective in the treatment of patients with thickened, nodular-type thoracic OLF, as the procedure resulted in worse outcome in two of the four patients. The authors have thus developed a new modality of laminectomy that they have termed "separating laminectomy," which they performed in 16 patients with thickened, nodular-type OLF; in 13 cases it resulted in a good or fair outcome, and in only one case did it result in a worse outcome. Conclusions. The pathogenesis of thoracic OLF is mainly due to the localized mechanical stress on the ligament. Laminectomy combined with lateral fusion is the treatment of choice for thoracic OLF; furthermore, in terms of the configuration of the ossified lesion, en bloc laminectomy is suitable for the treatment of lateral-type and diffuse-type OLF, and the separating laminectomy is suitable for the thickened, nodular-type OLF.
[Longterm Outcome after the Decompressive Surgery for Thoracic Myelopathy Due to the Ossification of the Ligamentum Flavum].

Abstract:OBJECTIVES:To investigate the long-term surgical outcome of thoracic myelopathy caused by the ossification of the ligamentum flavum (OLF) and evaluate the related risk factors.METHODS:Forty-four patients who underwent decompressive laminectomy with thoracic OLF between January 1990 and December 2005 and got more than 5 years follow-up were retrospectively reviewed. Among these 44 cases, there were 29 male and 15 female whose ages at operation were 52 years averagely (27-68 years). The 2-year follow-up results and long-term outcomes were classified according to the modified Epstein's standard, and then the rates of excellent or good (REG) were calculated. The correlation between the long-term REG and the patients' ages, durations of symptoms, decompressed levels, and dural leak were analyzed.RESULTS:The mean follow-up period of these 44 cases was 8.5 years (5-19 years). The REG at 2 years after laminectomy was 77.3% (34/44), while the long-term REG was 65.9% (29/44). There was one case who had suffered from an acute spinal cord injury got a poor post-operative outcome. The other 43 cases had chronic durations, including 22 cases whose pre-operative durations of symptoms were less than 12 months and 21 cases whose durations were equal to or more than 12 months. And the long-term REG of these two groups were 77.3% (17/22) and 57.1% (12/21) respectively (P>0.05). The REG of those cases whose decompression levels were limited in T1-T9 was 78.9% (15/19), while that of those cases whose laminectomy was relevant to thoracolumbar segment (T10-L2) was 58.3% (14/24) (P>0.05). There were 7 cases who had excellent or good short-term results and poor long-term outcomes. The reasons of these changes included coexistence of lumbar spinal stenosis in three cases and the growth of the OLF at the adjacent levels in four cases.CONCLUSIONS:Although the short-term results of the decompressive surgery for thoracic OLF is good, the regular long-term follow-up is necessary because the symptoms may reoccur or deteriorate secondary to lumbar spinal stenosis or the growth of OLF at the adjacent levels near former decompressive levels; the duration of symptoms which is more than one year and the decompression levels that is involved to T10-L2 segments are possibly related to the poor long-term outcomes.

Abstract:Objective:To investigate the surgical outcome and influence factors of thoracic myelopathy caused by ossification of the ligamentum flavum(OLF). Method:All of the 135 patients with thoracic myelopathy induced by OLF treated with en-bloc removal of the spinal canal′s posterior wall termed as the "cap uncovering" technique from January of 1986 to April of 2004 were studied retrospectively,and the post-operation results were observed.The possible influence factors including age at operation,pre-operation duration,decompression site,OLF types,JOA score and so on were analyzed.Result:82 of 135 were followed up for an average period of 66 months(24~168 months),and the percentage of follow-up was 60.7%. The functional results were excellent or good in 74.4%,with the tatal satisfactory rate of 92.7%.The recovery process of 68.4% patients ceased 2 years after operation,while 26.3% patients continued in 2~5 years after operation.Statistics showed that the pre-operation duration,the age at operation and the decompression site was factors influencing the long-term outcome remarkably(P<0.05).The outcome of cases whose decompression site limited in T10~L2 vertebrae was worse than those in T2~T9.Conclusion:Removal of the posterior wall of the spinal canal via the "cap uncovering" technique is a reliable and effective treatment for thoracic OLF.The pre-operation duration,the age at operation and the decompression site are major factors influencing the outcome.
Short-term and Mid-term Outcome of Surgical Treatment for Ossification of Ligamentum Flavum in Thoracic Spine

Abstract:Objective:Retrospective study of 23 cases of thoracic myelopathy caused by ossification of ligamentum flavum(OLF) were reviewed to study the short-term and midterm outcome of surgical treatment and the value of the score system. Methods:9 patients were females and 14 were males. The mean age was 55.1 ±7.2years. All were diagnosed by Magnetic resonance imaging(MRI)and presented lower limb dysfunction. Most patients were complicated with other spinal diseases. Patients with OLF below the T10 were treated by the surgery decompression and posterolateral fusion with the pedicle screws. The outcome was evaluated by Japanese Orthopaedics Association (JOA) score. Results:All patients were followed up over 1 year. Hitherto 8 patients were followed up over 6 years. The mean preoperative JOA score of 5.6 improved to 8.8 one year after surgery,the mean percentage of recovery was 57.8%. Conclusion:Surgery is the only effective treatment for myelopathy caused by OLF in thoracic spine. The key of the operation is to avoid spine injury. The short-term outcome is relatively good and the mid-term outcome has a trend to worsen.
Radiological Manifestations and Surgical Outcome of Combined Upper Cervical Cord Compression and Cervical Ossification of the Posterior Longitudinal Ligament with a Minimum 2-Year Follow-Up

Abstract:Combined upper cervical cord compression associated with cervical ossification of the posterior longitudinal ligament (OPLL) is a rare and under-recognized disorder. The aim of this study was to investigate the radiological manifestations and surgical outcome of this combined disease.Between May 2011 and July 2015, patients who underwent surgery for combined upper cervical cord compression and cervical OPLL in our institution were included in this study. After a minimum 2-year follow-up, radiological and clinical data were collected. The etiology of upper cervical cord compression and radiological features of cervical OPLL was determined. Surgical outcome was evaluated with Visual Analogue Scale (VAS), Japanese Orthopedic Association score (JOA), space available for the spinal cord (SAC) at the cephalad adjacent level, occupying ratio of OPLL and cervical lordosis.In total, 24 patients (11 men and 13 women) with a mean age of 57.9 years old were included. The etiology of upper cervical cord compression included craniovertebral junction deformity (n=10), atlantoaxial subluxation (n=5), and OPLL extending to C2 level (n=9). The extent, type, and thickest level of cervical OPLL varied among the patients. Significant improvement of VAS and JOA score was noted postoperatively and at a minimum 2-year follow-up. The result was satisfactory in SAC at the cephalad level and occupying ratio of OPLL. There were no significant differences in C2/C7 lordotic angle at the preoperative, postoperative and the last follow-up examination.In conclusion, the radiological manifestations of combined upper cervical cord compression and cervical OPLL varied among the patients. Satisfied results can be achieved with adequate surgical treatment a minimum 2-year follow-up.

Abstract:Thoracic ossification of the ligamentum flavum (TOLF) is a common cause of progressive thoracic myelopathy. TOLF is typically treated with surgical decompression. A variety of surgical techniques, including laminoplasty, laminectomy, and lamina fenestration, are used for the effective treatment of TOLF. However, traditional methods are associated with a substantial risk of perioperative complications, including dural laceration and/or iatrogenic spinal cord injury. Therefore, it is important to develop an efficient and secure surgical technique for TOLF. Herein, we describe a method for laminectomyperformed at the thoracic spine using an ultrasonic osteotome combined with a conventional osteotome. This technique can reduce intraoperative complications. This is a relatively safe and easy -to-learn method that should be recommended for the treatment of TOLF.
Clinical Progression of Ossification of the Ligamentum Flavum in Thoracic Spine: a 10- to 11-Year Follow-Up Study

Abstract:Background Thoracic ossification of ligamentum flavum (TOLF) can be asymptomatic and progress insidiously. But, long-term follow-up results of clinical progression of TOLF are still unknown. Methods The clinical progression of 81 patients with TOLF at our center, followed for 10 to 11 (mean, 10.3) years from May 2010 to November 2021, were analyzed. Among them, 51 patients with thoracic myelopathy were caused by single- or multi-segment TOLF, and received partial TOLF resection (30 patients) or total TOLF resection (21 patients). The remaining 30 patients showed TOLF on imaging examinations, but TOLF was not the responsible compressing factor causing myelopathy and with no TOLF resection. The mJOA score (total 11 scores) and spinal operation were used to evaluate the clinical progression at follow-up. Results During the 10- to 11-year follow-up of 81 TOLF patients, 71 (87.7%) had no deterioration of neurological function, and 10 (12.3%) patients had deterioration of neurological function and had another spinal operation, including only 4 (4.9%) suffered thoracic myelopathy caused by the progression of TOLF; 6 (7.4%) for other spinal diseases: 2 (2.5%) had fall damage and acute spinal cord injury at the TOLF level; 2 (2.5%) had thoracic myelopathy caused by ossification of posterior longitudinal ligament (OPLL); 2 (2.5%) had cervical spondylosis and received cervical operation. Conclusions Most TOLF (87.7%) patients had no clinical progression and received no reoperations for TOLF in the ten-year dimension (mean, 10.3 years). Narrow spinal canal for TOLF increases the risk of traumatic paraplegia.
Evaluation of Progression of Ossification of Ligamentum Flavum in the Thoracic Spine Using Computed Tomography

Abstract:Objective: To evaluate the progression of Ossification of Ligamentum Flavum (OLF) in the thoracic spine based on cross-sectional Computed Tomography (CT) comparisons. Method: OLF in spinal segments that did not require surgery because there were no cord compression of 17 patients who underwent laminectomies for treating symptomatic OLF were retrospectively reviewed (mean follow-up 6.3 years). Initial (preoperative or postoperative but before discharge) and final follow-up canal area unoccupied ratio (CAUR) of no-operated OLF were compared to evaluate the progression of OLF. Factors influencing OLF progression were also analyzed. Two-year follow-up and final follow-up results were assessed to evaluate the influence of OLF progression on long-term outcomes. Result: Sixty OLF levels were compared between initial and final follow-up CT. The final follow up CAUR was significant lower than the initial in all OLF levels. Repeated measurement was used to reduce the effect of measurement error, and progression of OLF affected approximately 60% of patients and 33% of OLF segments, with annual growth rate in OLF area of 1.61%. CT mature/immature classification was an independent risk factor for the development of OLF. No patient exhibited neurological deterioration due to OLF progression. Conclusion: Thoracic OLF that are not operated on still progress, and OLF classified as immature ossification on CT develop more significantly. Additionally, progression of OLF is not associated with neurologic regression in 5- 9 years follow-up, thus preventive laminectomy is unnecessary for OLF segments that do not compress the spinal cord.

Abstract:Study Design. Retrospective review. Objective. To perform a single-institution analysis of incidence, treatment, and clinical outcome in patients with thoracic ossification of the ligamentum flavum (OLF) who experienced dural tears and cerebrospinal fluid (CSF) leakage. Summary of Background Data. There is a paucity of clinical reports focusing on dural tears and CSF leakage after thoracic OLF surgery. Because dural adhesion and dural ossification are common features of thoracic OLF, the incidence of CSF leakage in OLF patients is high and represents a significant clinical challenge. Methods. A total of 266 patients with thoracic OLF were admitted to our hospital from 1995 to 2011. Each patient's medical records were reviewed to identify cases of dural tears and CSF leakage. Information on therapeutic strategy used to repair the dural tears and complications related to CSF leakage was extracted. Results. The incidence of dural tears and CSF leakage in OLF patients was 32% (85/266). The incidence of dural ossification was 25.2%. The dural tears were repaired with a range of materials, including gelatin sponge, muscle/fascia, artificial dura, silk suture, and fibrin glue. The intraoperative repair procedure did not resolve CSF leakage in 65 cases, and 16 of those cases experienced complications related to the continued CSF leakage, including CSF pseudocyst, wound dehiscence, and meningitis. Fifty-eight patients with CSF leakage were eventually cured by a series of comprehensive treatments, which included prone position, continuous pressure by sandbag, ultrasound-guided puncture, and aspiration. Only 7 patients required reoperation. Conclusion. Dural ossification was the main reason for dural tears. In all, 78 of the 85 patients with CSF leakage or dural tear were successfully cured. The success rate was 91.8%, which indicated that a series of comprehensive treatments was an effective strategy to treat these patients.

Abstract:Purpose We set out to establish a magnetic resonance imaging (MRI) and computed tomography (CT)-based diagnostic method for determining the responsible segments in thoracic myelopathy due to ossification of the ligamentum flavum (OLF). Methods Forty-four patients who underwent surgery for treatment of myelopathy due to OLF between June 2005 and May 2013 were enrolled in this study as the myelopathy group. Forty-four patients who were identified through CT and MRI scans to have OLF but had no definite neurologic deficits prior to the examination were included as the control group. MRI and CT examination were reviewed, and the degree of spinal canal compromise was graded on axial T2-weighted MRI. Anteroposterior spinal canal diameter was measured at the maximally stenosed level on axial and sagittal CT. The canal grade and the cross-section area-occupying ratio were measured and calculated on the CT scans. The diagnostic coincidence rates for the indices were then compared. Results Cases of Grade IV were all in the myelopathy group while cases of Grade II were all in the control group. The canal grade (paramedian) was the most relevant continuous variable with the largest JOA score ( r = 0.685, P < 0.005). A canal grade (paramedian) of <60 % can be used as a critical value for determining OLF-induced myelopathy (sensitivity and specificity, 95.5 %). Conclusion Spinal canal compromise is relevant to spinal cord deficits in patients with OLF, and a canal grade (paramedian) can be used to quantify spinal cord deficits. Additionally, a canal grade (paramedian) of <60 % on axial CT scan can serve as a critical value for diagnosing OLF-induced myelopathy, especially for Grade III compression on T2-weighted MRI.
Surgical outcome and associated factors of "cap uncovering" en-bloc removal of the spinal canal′s posterior wall surgery for single-level thoracic ossification of ligamentum flavum

Abstract:Objectives: To assess the efficacy of "cap uncovering" en-bloc removal of the spinal canal′s posterior wall and the prognostic associated factors for thoracic myelopathy caused by single-level thoracic os-sification of ligamentum flavum(OLF). Methods: Patients with thoracic myelopathy induced by OLF underwent en-bloc removal of the spinal canal′s posterior wall termed as the "cap uncovering" technique between February 2005 and May 2013 and were retrospectively reviewed. Exclusion criteria were as follows: other tan-dem spinal diseases, OLF of more than one segment, cases caused by injury. A total of 44 cases was in-cluded. Among these 44 cases, there were 23 males and 21 females with a mean age of 56.8 years(range 24-76 years). Segmental distributions was as follows: 20 cases of T10/11, 14 cases of T11/12, 4 cases of T9/10, 2 cases of T8/9, 2 cases of T2/3, 1 case of T4/5, 1 case of T1/2. The modified JOA score and the re-covery rate were used to measure the outcomes. The mean JOA score was 6.68±1.76 preoperatively. The fol-low-up results were classified according to the recovery rate and then the rate of excellent or good was calcu-lated. The degree of spinal canal occupation was graded on axial T2 weighted MRI. The type of OLF and in-tramedullary high signal intensity on T2-weighted MRI was also evaluated. The spinal canal diameters were measured at the maximally stenosed level on axial and sagittal CT. The spinal canal diameter on axial CT was measured at three sites: the midline of the canal, the boundary of the canal, and the paramedian point. The canal diameter occupied ratio and the cross-section area occupied ratio were calculated. Sato′s classification and the growth position of OLF were observed on CT. Correlations between the surgical outcomes and various factors[age, gender, preoprative JOA scores, preoprative duration, unilateral/bilateral ossification, leakage of cere-brospinal fluid(CSF), the growth position, operative segment, high intensity signal in the spinal cord, shape on the sgittal MRI, dural ossification, Sato′s classification, degree of spinal canal occupation on axial MRI, canal diameter occupied ratio on sagittal and axial CT, the cross-section area occupied ratio] were analyzed through univariate linear correlation analysis, and multiple linear regression analysis was then used. Results: The mean follow-up period of these 44 cases was 40 months (range, 10-99 months). At final follow-up, the JOA score increased to 5-11 (mean 8.84 ±1.83), and the JOA recovery rate was -20%-100%(mean 58.17%). Surgical outcomes were as follows: 13 excellent, 20 good, 9 fair and 2 poor. The rate of excellent or good was 75.0%(33/44). The univariate linear correlation analysis showed that preoprative JOA scores, unilateral/bilateral ossification, dural ossification, the growth position, high intensity signal in the spinal cord, Sato′s classification and degree of spinal canal occupation on axial MRI, canal diameter occupied ratio on sagittal CT, canal diameter occupied ratio on axial CT (boundary), canal diameter occupied ratio on axial CT (paramedian), the cross-section area occupied ratio might be correlated to JOA recovery rate (P<0.05). Age, gender, preoprative duration, leakage of CSF, operative segment, shape on the sgittal MRI, canal diameter occupied ratio on axial CT (midline) were not associated with JOA recovery rate (P>0.05). The multiple linear regression analysis revealed that only canal diameter occupied ratio (paramedian) on axial CT and preoprative JOA scores significantly correlated with recovery rate(P<0.05). Unilateral/bilateral ossification, dural ossification, the growth position, high intensity signal in the spinal cord, degree of spinal canal occupation on axial MRI, Sato′s classification and other occupied index had no significant effect on surgical outcome (P>0.05). Conclusions:"Cap uncovering" en-bloc removal of the spinal canal′s posterior wall surgery is effective for single-level thoracic OLF. Canal diameter occupied ratio (paramedian) on axial CT and preoperative JOA scores have significant effect on surgical outcome.
Differences in the Demographics and Clinical Characteristics between the Ossification of the Posterior Longitudinal Ligament and Ossification of the Ligamentum Flavum in Patients Who Underwent Thoracic Spinal Surgery for Compressive Myelopathy

Abstract:Ossification of the posterior longitudinal ligament (OPLL) and ossification of the ligamentum flavum (OLF) are related diseases associated with the ossification of spinal ligaments that can occasionally lead to thoracic myelopathy. We retrospectively analyzed the clinical data of 34 consecutive patients who underwent thoracic spinal surgeries for OPLL and/or OLF at our hospital between July 2010 and June 2022, and statistically compared data between patients with thoracic OPLL (TOPLL; n = 12) and those with thoracic OLF (TOLF; n = 22). The mean age of the TOPLL group was significantly lower than that of the TOLF group (53.7 vs. 68.4 years). The TOPLL group exhibited a greater female predominance than the TOLF group (58.3% vs. 18.2%). The median body mass index of the TOPLL group was significantly higher than that of the TOLF group (33.0 vs. 26.0 kg/m2). Patients with TOPLL significantly required instrumented fusion and repetitive surgical intervention more than those with TOLF (83.3% vs. 9.1%; 50.0% vs. 0.0%). Although neurological deterioration just after the intervention was more common in patients with TOPLL (41.7% vs. 4.6%), no difference was observed in thoracic Japanese Orthopaedic Association score and recovery rate in the chronic phase between TOPLL and TOLF. The TOPLL group had a younger onset, female dominance, and a greater degree of obesity when compared with the TOLF group. The surgery for TOPLL is challenging, considering that it requires long-range decompression and fusion, subsequent operations, careful management, and long-term follow-up, when compared to TOLF, which necessitates only simple decompression.

Abstract:To answer two questions: (1) what are the common complications associated with surgery for thoracic myelopathy caused by ossification of the posterior longitudinal ligament, and (2) which surgical approach is safer with regard to the incidence of post-operative complications.
Selection of the Surgical Methods for the Thoracic Ossification of Ligamentum Flavum Combined with Cervical Spondylotic Myelopathy

Abstract:Objective To investigate the difference between the different surgical methods for thoracic ossification of ligamentum flavum(OLF)combined with cervical spondylotic myelopathy(CSM).Methods From January 1991 to January 2003,56 cases with thoracic OLF combined with CSM were reviewed retrospectively.Forty of 56 cases had been followed up for more than two years.There were 22 males and 18 females with an average of 58 years(range,27-70 years).The course of the diseases were 1 to 120 months,with an average of 16.5 months.The pathological factors for thoracic myelopathy included:OLF for 25 cases,OLF combined with ossification of posterior longitudinal ligament(OPLL)for 12 cases,OLF combined with thoracic disc herniation for 3 cases.There were 23 cases combined with cervical OPLL and 17 cases combined with cervical stenosis.Eighteen cases underwent cervical laminoplasty and thoracic laminectomy,9 cases underwent only thoracic laminectomy,and 13 cases underwent both cervical and thoracic posterior decompression in multiple stages.Results Forty cases were followed up for 24 to 227 months,with an average of 67.5 months.According to Epstein standard,the excellent and good rate was 88.9% for 18 cases who underwent cervical laminaplasty and thoracic laminectomy(16/18),66.7% for 9 cases who underwent only thoracic laminectomy(6/9),and 53.8% for 13 cases who underwent both cervical and thoracic posterior decompression(7/13).Statistics analysis showed that the outcome of one-stage surgery was better than multiple stages surgeries,and outcome of those whose operation intervals less than 1 year was better than those of more than 1 year.Conclusion Thoracic and cervical decompression should be performed at one stage for those with upper thoracic OLF combined with CSM.Thoracic decompression should be performed firstly for those with severe lower extremities syndrome and few upper extremities syndrome.Thoracic and cervical decompression in one or multiple stages should be performed for those with severe upper and lower extremities syndrome,and operation interval should be less than one year.
Predictive Risk Factors of Poor Preliminary Postoperative Outcome for Thoracic Ossification of the Ligamentum Flavum

Abstract:Objective: The aim of the present study was to ascertain the independent risk factors of poor preliminary outcome and to reveal the value of these factors in predicting the postoperative prognosis. Methods: A total of 165 patients diagnosed with thoracic myelopathy because of thoracic ossification of the ligamentum flavum (TOLF) were enrolled in this retrospective study. All of them underwent posterior decompressive laminectomy surgery in our hospital from May 2016 to June 2019. The postoperative improvement of symptoms was evaluated using the modified Japanese Orthopaedic Association (mJOA) scoring system. Clinical data, such as age, sex, body mass index (BMI), duration of symptoms, history of hypertension and diabetes, tobacco use, history of drinking, symptoms of incontinence, number of compressed segments, and preoperative mJOA score, were respectively recorded. Radiologic features data included sagittal maximum spinal cord compression (MSCC), axial spinal canal occupation ratio (SCOR), grades and extension of increased signal on sagittal T2-weighted images (ISST2I), types of increased signal on axial T2-weighted images (ISAT2I), and the classification of ossification on axial CT scan and sagittal MRI. The t-test, the χ2 -test, Fisher's exact test, binary logistic regression analyses, receiver operating characteristic (ROC) curves, and subgroup analyses were used to evaluate the effects of individual risk predictors on surgical outcomes. Results: A total of 76 men and 89 women were enrolled in this study. The mean age of all patients was 58.53 years. After comparison between two groups, we found some risk factors that may be associated with postoperative outcomes, such as age, preoperative mJOA score, BMI, history of hypertension, MSCC, SCOR, grade and extension of ISST2I, type of ISAT2I, axial type of ossification, and sagittal type of ossification (P < 0.05, respectively). Binary logistic regression analysis revealed that older age (odds ratio [OR] = 1.062, 95% confidence interval [CI] = 1.006-1.121, P = 0.030), number of compressed segments (OR = 1.916, 95% CI = 1.250-2.937, P = 0.003), bilateral and bridged types of ossification (OR = 4 314, 95% CI = 1.454-8.657, P = 0.019; OR = 6.630, 95% CI = 2.580-17.530, P = 0.004), and grade 1 and 2 ISST2I (OR = 8.986, 95% CI =3.056-20.294, P < 0.001; OR = 7.552, 95% CI = 3.529-16.004, P < 0.001) were independent risk factors for a poor preliminary postoperative outcome. ROC curve analysis showed that the grade of ISST2I had an excellent discriminative power (area under the curve [AUC] = 0.817). In addition, risk factors have different values for predicting the clinical outcome in each subgroup. Conclusion: Age, duration of symptoms, number of compressed segments, SCOR, grade, and extension of ISST2I and classification of ossification were associated with the preliminary prognosis, and the intramedullary increased signal on sagittal T2-weighted MRI was highly predictive of poor postoperative outcome.

Abstract:Objective To investigate different surgical procedures for treatment of thoracic ossification of ligamentum flavum.Methods From January 1994 to June 2008,56 cases of thoracic ossification of ligamentum flavum underwent different surgical procedures.There were 40 males and 16 females,aged from 43 to 76 years(average 58.1 years).The courses of disease were 3 months to 5 years,average 13.4 months.CT and MRI examinations were used to observe ossification involving levels,distributions,ossification nest shapes,spinal canal stenosis and spinal cord compression and so on.All patients were treated with en bloc or dissolved laminectomy combining with posterolateral fusion.Japanese Orthopaedic Association(JOA)score was used to evaluate postoperative outcomes.Results The patients were followed up for 18 to 70 months,with an average of 25 months.The mean JOA score increased from 6.25±2.47 preoperatively to 7.53±3.20 at the final follow-up.According to CT scans,the ossifications were divided into lateral type in 6 cases,diffuse type in 17 and thickened nodular type in 33 cases.Patients of lateral type was treated with en bloc laminectomy,and the excellent and good rate was 83.3%.In the patients of diffuse type,11 were treated with en bloc laminectomy and 6 with dissolved laminectomy,and the excellent and good rate was and 81.8% and 83.3% respectively.For patients of thickened nodular type,4 were treated with en bloc laminectomy and 29 with dissolved laminectomy,the excellent and good rate was 50% and 82.8% respectively,and 2 cases presented spinal cord injury aggravation.Conclusion En bloc laminectomy combining with lateral fusion is ideal surgical procedure for lateral type and diffuse type,dissolved laminectomy combining with laeral fusion is suitable for thickened nodular type.
Pathological Unit and the Octagonal En Bloc Resection of Thoracic Ossification Ligamentum Flavum

Abstract:Objective To describe the pathological unit and octagonal en bloc resection for the treatment of ossification ligamentum flavum(OLF)in thoracic spine with spondylotic myelopathy.Methods Ninety-five patients from January 2002 to January 2007 were diagnosed as thoracic OLF,61 males and 34 females with an average age of 53.9 years(range,31-78 years).There were upper thoracic spine OLF in 32 cases,middle thoracic spine OLF in 24 cases and lower thoracic spine OLF in 39 cases.Single-segment OLF was found in 53 cases,double segments OLF was found in 38 cases and three segments OLF was found in 4 cases.CT scan multiplanar co-localized reconstruction was employed to detect the structure of spine with OLF.The Japanese Orthopaedic Association(JOA)lower limb motor function score,sphincter function score and motor function improvement rate were used to evaluate the outcomes.Results CT scan was engaged to observe 141 OLF pathological unite.The OLF pathology unit was defined as all the spine structures between the extension lines of the lower margin of the OLF two adjacent pedicles.Each OLF associates with an OLF pathology unit.The mean follow up duration was 38.3 months(range,24-60 months).Among 86 patients with sensations disturbance before operation,67 totally recovered and 19 relieved after operation.Trunk restrictions in 69 cases before operation were completely recovered after operation.Postoperative JOA sphincter function score was 2.651±0.334,comparing with preoperation score(2.262±0.561),and the difference was statistically significant.Postoperative JOA motor function score was 3.694±0.429,which was significantly increased than preoperative score 1.539±0.873,and motor function recovery rate was 87.57%.There was excellent in 71 cases,good in 17 cases and fair in 5 cases.The excellent and good rate was 94.74%.Conclusion The octagonal en block resection is relative safe for treatment thoracic OLF with myelopathy.Pathological unit of OLF in thoracic spine is more accurate to summarize the pathological contents and features of the OLF and its adjacent structure.
The Influence of Ossification Morphology on Surgery Outcomes in Patients with Thoracic Ossification of Ligamentum Flavum (TOLF).

Abstract:Background To determine whether there is a correlation between the type of ossification and radiological parameters, modified thoracic JOA scores, and complications in patients with thoracic ossification of ligamentum flavum treated by posterior thoracic surgery. Methods This retrospective cohort study included 48 patients with thoracic myelopathy caused by single-level thoracic ossification of ligamentum flavum who underwent thoracic posterior approach surgery in our Hospital o between December 2013 to December 2018. Patients were divided into unilateral, bilateral, and bridged groups in axial position, and beak and round groups in sagittal position. The differences were analyzed according to the ossification morphology. Results In axial myelopathy, there was no significant difference in preop and postop JOA scores and RR among the three groups in axial position ( P = 0.884). In sagittal view, there was no significant difference in preoperative JOA score between the two groups ( P = 0.710), while the postop JOA score and the recovery rate in the beak group were significantly lower than that of the round group ( P = 0.010, P = 0.034). Two-way ANOVA showed that sagittal morphology had a significant effect on postop JOA score ( P = 0.028), but axial morphology don’t ( P = 0.431); there was no interaction between them ( P = 0.444). For the recovery rate, sagittal morphology also had a significant effect ( P = 0.043), but axial ossification don’t ( P = 0.998); there was no interaction between them ( P = 0.479). Conclusion Sagittal morphology had a significant adverse effect on postop JOA score and surgical outcome, while axial morphology had no effect on surgical outcome, and there was no interaction between sagittal morphology and axial morphology.
Thoracic full-endoscopic unilateral laminotomy with bilateral decompression for treating ossification of the ligamentum flavum with myelopathy

Abstract:BACKGROUND: The aim of the present study was to evaluate the curative effect and safety of thoracic full-endoscopic unilateral laminotomy with bilateral decompression (TE-ULBD) for treating ossification of the ligamentum flavum (OLF) with myelopathy.METHODS: Between January 2015 and December 2018, 23 consecutive patients with symptomatic thoracic OLF were treated with TE-ULBD. Of these, 21 (13 women and 8 men, aged 49-75 years) were included in the study and followed up for a minimum of 1 year. The mean blood loss was 15.48 mL (10-30 mL), operative duration was 78.86 min (55-115 min), and hospitalization was 5.05 days (3-15 days). The Japanese Orthopaedic Association (JOA) was used to evaluate spinal cord function, and the curative effect was defined by the JOA improvement rate. The area of OLF (AOLF), the maximum spinal cord compression (MSCC), and the area of spinal cord (ASC) were used to evaluate OLF clearance and spinal cord decompression status.RESULTS: At the final follow up,the JOA score was 8.33 points (5-11 points), which was a significant improvement from the preoperative 5.33 points (3-9 points, P<0.01). The excellent and good rate was 76.19% (16/21). The average preoperative AOLF and AOLF ratio were 85.27±23.66 mm2 and 57.86%±11.86%, respectively, and the postoperative AOLF and AOLF ratio were 16.27±11.75 mm2 and 8.13%±5.38%, respectively. The MSCC increased from 27.99%±13.51% preoperatively to 48.02%±6.66% postoperatively. The ASC was 42.90±10.60 mm2 preoperatively and 64.54±21.36 mm2 postoperatively. There were statistically significant differences in all parameters preoperatively and postoperatively (P<0.01). One case had postoperative hematoma, and the symptoms gradually eased after 3 weeks of conservative treatment. There were no other complications. No recurrence of OLF was detected during the follow-up period.CONCLUSIONS: TE-ULBD is safe and effective for thoracic OLF with the advantages of reduced trauma and bleeding, and faster recovery.

Abstract:Ossification of the ligamentum flavum (OLF) is an uncommon but potentially serious spinal condition which can cause progressive compression of the spinal canal with associated devastating neurologic compromise. Although debate exists regarding the exact etiology of OLF, overexpression of genes and transcription factors centered around the Notch and Wnt signaling pathways because of increased mechanical stress seems to be related. There are many clinical and radiographic presentations of OLF; however, progressive myelopathy is the most commonly encountered. Radiographic analysis may reveal isolated OLF or OLF combined with ossification of other areas of the spine, such as disk, posterior longitudinal ligament, and dura. When surgery is necessary for OLF, several surgical strategies exist including open laminectomy with excision, endoscopic decompression, Bridge Crane resection, en block resection, and combined anterior and posterior approaches. Resection may be complicated by dural adhesion or dural ossification, and postoperative neurologic deficits are not uncommon.
